Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

Recent advances in GPS, machine guidance technology and telemetry allow an unprecedented level of control of surface mining operations. GPS technology can now locate the bucket of a shovel or the blade of a dozer within 50mm (2 inches).Some of the outcomes possible are: precise selective mining – even when the ore is visually indistinguishable from waste: tracking grade and tonnage of every ore parcel through the mine: real time productivity monitoring: detailed post accident safety analysis: tracking the movements of every piece of equipment. This paper will consider the contribution that GPS based machine control systems can make to ensuring that surface mining projects deliver the outcomes promised in the feasibility studies. Case studies of installations are presented.

Grouting is a frequently utilized remedy in the planning and implementation of ground improvement and tunnel rehabilitation practice. The state-of-art grouting application used for Chicago’s McCook Reservoir project offers a unique alternative to the tunneling industry for pre-excavation grouting as well as tunnel rehabilitation, rescue and post-excavation ground improvements. The McCook Reservoir project included a computerized grouting control and data collection system for a precise and highly effective groundwater cutoff in fractured dolomites. By using balanced and stabilized grout mixes, the effective permeability of the rock mass was reduced over two orders of magnitude to less than 0.1 Lugeon by installing a double line grout curtain. The stable grout mix designs allowed high pressure and effective grouting at depths up to 125 m (410 ft).

What will this deal do to the company’s stock price? This question arises when a publicly traded mining company is presented with a new mine project, or business combination. The standard tool for addressing this question is to use discounted cash flow analysis, or net asset value(NAV), which is discounted cash flow net of liabilities. Cash flow-based analysis is invaluable for assessing project costs and other impacts on debt service capacity and internal rates of return. But cash flow analysis and NAV do not work very well for stock price analysis.

In underground mines, coal dust explosions are prevented by the addition of rock dust sufficient to render the coal dust inert. The National Institute for Occupational Safety and Health (NIOSH) developed a hand-held instrument that uses optical reflectance to measure the explosibility of a rock dust and coal dust mixture. This instrument is called the Coal Dust Explosibility Meter (CDEM). In this study, NIOSH personnel accompanied Mine Safety and Health Administration (MSHA) inspectors on their routine band surveys in five underground coal mines in MSHA District 2 (Pennsylvania) and three underground coal mines in MSHA District 11 (Alabama). While underground, NIOSH personnel and MSHA inspectors used the CDEM to assess the explosibilities of the dust samples. The values of percent incombustible content (percent IC) determined by the CDEM agreed well with those obtained later by low-temperature ashing (LTA) in both MSHA and NIOSH laboratories. Further, the meter identified some samples as potentially explosible that LTA analysis had found to possess sufficient rock dust for inerting. The CDEM provides more information on the hazards in the mine. Rapid identification of areas with explosible dust mixtures using the CDEM allows for immediate intervention rather than a wait of several weeks for laboratory analysis.

In this study, recovery of copper from solid material (obtained by leaching of chalcopyrite with oxalic acid in the presence of hydrogen peroxide) with a copper oxalate structure was carried out selectively using HCl. The chemical composition of the solid was as 29.3%, 9.7%, and 13.3% for Cu, Fe, and S, respectively. Selective recovery of copper by degrading the copper oxalate structure with a low-solubility product (Ksp = 4.43 × 10−10) was performed by conventional leaching in the presence of HCl. The optimum leaching conditions were determined as 5 M of HCl concentration, 318 K of leaching temperature, 120 min of leaching time, 25 ml/g of liquid–solid ratio and 400 rpm of stirring speed. Under these conditions, approximately 77% of copper and 3% of iron were taken into the solution. According to the characterization of the leach residue that was obtained, the oxalate structure was degraded. Moreover, the results of XRD analysis showed that the sulfur (S) and cubanite (CuFe2S3) phases were mostly observed in leaching residues, and this was supported by the SEM-EDX data. Based on the results, it may be stated that selective copper extraction can be obtained from a chalcopyrite concentrate by a two-step leaching method using a conventional hydrometallurgical technique.

The 30th annual short course ?Grouting Fundamentals and Current Practice? was held at the Colorado School of Mines, June 22-26, 2009. The field demonstration portion of the course was conducted June 25, at Hayward Baker, Inc.?s yard in Broomfield, CO. The full-scale field demonstration annually presents many different types of drilling equipment being operated as well as various grouting methods being performed under different field conditions. As part of the field demonstration, the class is shown the proportioning, mixing, testing and injection of various cement grout mixes into sand columns under controlled and measured conditions. These sand column demonstrations have been conducted under controlled and measured conditions each year since 1999 as part of the short course. The sand column demonstrations were conducted prior to 1999, but with less quality control and minimal record keeping of the proportioning, mixing, testing, injection pressures and the final permeation results. The goal of the sand column demonstration is to show the students the effect of the water cement ratio and the use of admixture as well as the fineness of the cement, portland versus ultrafine, used has on the engineering properties of the grout and the grout?s vertical permeation through the sand. Test results for the demonstrations conducted in 1999 and for two separate demonstrations, one at the grout course and one at Geo Denver, conducted in 2000 were published in the 2001 proceedings of the Rapid Excavation and Tunneling Conference (RETC) (Henn et al. 2001). The test results for the 2002 and the 2003 demonstrations were published in the 2005 proceeding of the RETC (Henn et al. 2005).

The exploitation of the underground is currently undergoing a major transformation process. Besides mining, alternative ways of utilization are playing an increasingly important role. These alternative utilizations include the storage of gases, the disposal of radioactive wastes and the implementation of geothermal storage facilities. In addition, more and more mining operations are being shut down, like the German hard coal mining industry in 2018. But the integrity of closed mines must be ensured in the post-mining phase.

This paper investigates the use of backfill zones and yielding pillars for subsidence control. The investigation begins by choosing an appropriate subsidence predictive technique to simulate the proposed subsidence-control schemes. Two case studies are performed in order to delineate the accuracy and utlility of the chosen predictive technique and then the results of these case studies are used to establish appropriate sets of regional site parameters. Next, a detailed analysis of the major variables affecting the outcome of the subsidence-control simulations is conducted and graphical displays of the exact sensitivity of the surface subsidence to these variables are given. Finally, alterations to the surface subsidence that are expected from using two major subsidence-control techniques are presented. The first technique to be assessed is a backfilled pillar centered longitudinally in a panel. With this arrangement, a minimal amount of backfill material can produce substantial decreases in vertical displacements. The second technique to be assessed is the use of yielding gate pillars to reduce the sinuosity of the transverse subsidence over adjacent panels. With proper design, this technique can greatly curtail surface strains and help minimize remedial drainage control.

A series of gas inrush events occurred during development at Grosvenor mine resulting in exposure to elevated levels of methane at the production face. A total of 22 gas inrush events occurred releasing between 15 and 130 m3 of methane during each event. The presence of an undrained seam in the immediate floor, geotechnical characteristics of the floor, and the stress environment all contributed to these dynamic floor events, while the geological characteristics of the seam below, such as the seam thickness and ash content of 75%, prevented effective pre-drainage. However, events only occurred in headings mined parallel to the principal horizontal stress direction. In cut-throughs (C/Ts) perpendicular to the principal stress direction, no events occurred, and higher methane levels were observed at the production face.

Construction of the Boddington Mine 130 km (81 miles) southeast of Perth, is about 89 percent complete. When the $2.9-billion development is finished, it will be the largest gold mine in Australia, producing 31 t/a (1 million oz/year), according to project majority owner Newmont Mining. Newmont?s acting regional group executive for Asia Pacific, Philip Stephenson, told a gold conference in Australia that the project would be ?the biggest thing for Newmont this year.? The project has been in the works for more than 10 years, but because of a three-sided ownership and fluctuating gold prices, it struggled to get started. However, Newmont is set to become the full owner of the project and is moving ahead on the low-grade project. ?We are targeting annual production of 31 t/a (1 million oz/year) for each of Boddington?s first five years. When it is in full production, Boddington will be the largest gold mine in Australia,? Stephenson said. ?The mine?s costs applicable to sales will be around US$9.64/g (US$300/oz) and that puts Boddington into the bottom quartile of the global cost curve. ?Strategically for Newmont, Boddington coming on-line will improve Newmont?s overall global geopolitical risk profile as it delivers greater exposure to an AAA-rated country.? Boddington has a projected mine life of 20 plus years. It has current total reserves of 622 t (20 million oz) and rising. Stephenson said Boddington has the largest reserve base of all gold projects in production or those that are about to go into production in A-rated countries or better. And it will be an important asset for many years to come. Newmont will acquire the remaining 33 percent share held by AngloGold Ashanti in a deal that should be concluded in the next two months. In this manuscript, the facts about cyanide are presented beginning with a brief history and definition and ending with a discussion on cyanide alternatives. Other topics include speciation, physical characteristics, toxicity, manufacturing, cyanogenics, cyanide cycle, separation, destruction, and markets including those outside the mining industry. Emphasis, of course, is placed on gold production by leaching and how it is influenced by ore type, mineral characteristics and economics as well as its advantages and disadvantages.

This study focuses on the amosite mining region in South Africa and associated health effects, compared to other mined asbestos fiber types. Historically, dust and fiber levels were high in the amosite mills and mines, and many miners and members of the surrounding communities were exposed to the fibers. Research has shown that amosite produces both benign and malignant disease. Nevertheless, the mesotheliomagenic potential of amosite is several fold lower than crocidolite. The risk of disease associated with amosite exposure is difficult to quantify. Reasons for this include the scarcity of available information, including fiber measurements, and case ascertainment, as well as the juxtaposition of the amosite and crocidolite asbestos seams in South Africa. © 2007 Elsevier Inc. "‘Gas hydrate’ is the common term for a solid material formed from the combination of various gases as ‘guest’ and water as ‘host’. A series of large-scale scientific drilling and coring programmes conducted since 2000 revealed that natural gas hydrate exists in a wide variety of accumulations in the sediments. The scaled-up onshore and offshore field production tests during the last decade show that gas production technology will be realized to make gas production from gas hydrates viable in the future, recognizing its potentials as one of the future sustainable energy resources.The total produced gas was approximately 470 m3 by the hot water circulation method over 6 days of operation in the permafrost-associated Mallik 2002 project in the Mackenzie Delta of Canada. In the Mallik 2007-2008 project, the gas was successfully produced stably by the depressurization method for 6 days, up to 13,000 m3 cumulatively. The depressurization method applied in the Mallik test was shown to be an effective way to produce gas from gas hydrates. The Alaska field trial to inject mixed gas of CO2 and N2 to exchange CH4 in 2012 was successfully done for the first time to produce maximum 1,270 m3 per day. A remarkable achievement was made by Japan when it completed its first offshore production test in the Eastern Nankai Trough, producing approximately 120,000 m3 of methane by the depressurization method over 6 days in March 2013. The technical challenges and scientific uncertainties obtained from Nankai Trough production test provide Korea with more considerations as to the aspects of well completion, reservoir formation and seafloor stability, sand control, flow assurance, environmental monitoring, and etc., due to the different geological setting and geomechanical properties of the Ulleung Basin in Korea."

The government of India convened a precedent-setting meeting in New Delhi on Nov. 12, 2001.The meeting brought together regulators and the mining industry in an attempt to fix the industry’s mine development problems in that country. More than 150people attended the all-day session. Ram Vilas Paswan, Minister of Coal and Mines, chaired the meeting. Ravi Shankar Prasad, Minister of State for Coal and Mines, was also present. The secretary of the Department of Mines and the Joint Secretaries and Additional Secretary also participated. The Department of Mines’ mandate is for economic growth: to get new mine development going and to make investment in mining attractive for both domestic and foreign-owned companies.

Many countries, provinces, territories and states in the European Union, Australia, Canada, the United States and elsewhere have begun implementing sustainability programs. Most of those programs, however, stop short of sustainable management of aggregate resources. Sustainable practices do not always have to be conducted under the title of sustainability. This case study describes how Lafarge implemented the principles of sustainability even though there was an absence of existing local government policies or procedures addressing sustainable resource management. Jefferson County is one of three counties in the six-county Denver, CO, region that has potentially available sources of crushed stone. Crushed stone comprises 30percent of the aggregate produced in the area. And it plays a major role in regional aggregate resource needs. Jefferson County is home to four of the five crushed stone operations in the Denver region. Lafarge operates one of those four quarries.

Slurry diaphragm walls are commonly used as both temporary excavation support walls, and permanent structural walls, for circular shafts and long rectangular structures (such as underground subway stations); however, they are not commonly used for more compact rectangular or square shafts. In these instances, a second wall is typically cast inside the slurry walls to form the permanent structure. This paper discusses the approach used on the Silicon Valley Rapid Transit Project in California to analyze these compact rectangular types of structures for static and seismic loading, and to determine whether or not slurry walls can be confidently used as both the temporary and permanent walls for two tunnel ventilation shafts.

They are gone now. Gone like that long ago generation that gave them birth. The Homestake Mine, nestled in the northern Black Hills, closed in 2001 after 125 years, the longest run of any major American gold mine. The Homestake Mining Co., which guided its destiny for those many years, is gone too. It merged with Barrick Gold. The Hearst family, which guided the early destiny of both, has long since gone beyond its mining heritage and into the national and even international spotlight. The mine and the company passed from the scene with as little fanfare as heralded their birth, amid the Black Hills gold rush of the 1870s. They both grew into American mining legends, as did the town they sired and nurtured, Lead. In a real sense, the mine and the company symbolized one of the last links to a vanished world and a mining era.

There is a growing interest among transportation planners to make more use of the subsurface to preserve the landscape and make transportation more efficient and economical. At the present time, utilization of the underground for transportation facilities is severely restrained by the adverse environmental impact and high construction costs. However, recent studies performed by the United States Department of Transportation indicate that, in spite of such problems, the volume of underground construction has significantly increased during recent years and projections point to continuation of these trends(1). It is estimated that the total volume of transportation tunnel construction will exceed $4 billion in this decade. This level of Investment justifies a significant research program. Due to the common interest in transportation tunnels and to the similarities of problems encountered during their construction, the tunneling research activities of the Federal Highway Administration (FHWA) are closely coordinated with the programs other agencies of the U. S. Department of Transportation DOT); specifically, the Federal Railroad Administration (PRA) and the Urban mass Transportation Administration (UMTA). The FHWA is responsive to the particular needs of these agencies but will benefit from the research results obtained also. In 1971, the Office of Secretary, DOT, initiated plans for an overall coordination of the DOT, FICA, UMTA, and FHWA tunneling research activities. The objectives of this effort have been to reduce tunneling costs by 30% and to increase the rate of excavation by 200% within 10 years.
