This final report delineates the findings of the test programs, simulations, and analyses performed to determine the requirements for the effective application of berms and guardrails as truck restraint systems for elevated mine roadways. Included arts criterion for effective vehicle restraint for guardrail applications and recommendations for berm applications based on scale model tests and computer simulations. Full scale haulage vehicle-berm interaction field tests were performed to correlate the results of these model tests and computer simulations. Recommendations for the construction of berms were made on the basis of tests of berm bearing strength, response of the vehicle to different berms, and predictions based on the obtained correlation

The geology of the roof of the Pittsburgh coal bed was studied and mapped in nine contiguous counties containing the bulk of the remaining reserves of this coal bed in the northern Appalachian Coal Basin. Drill core records from Belmont and Monroe counties, Ohio, Washington and Greene counties Pennsylvania, and Ohio, Marshall, Wetzel, Monongalia, and Marion counties, West Virginia provided the data for twelve computer generated regional geologic variable maps. Four of these geologic variables (overburden thickness, vertical distance to the rider coal, lithology at the five foot horizon above the roof of the mineable Pittsburgh seam, and the number of lithologic changes within the 30 foot interval above the roof of the mineable Pittsburgh seam) were used to estimate the risk of potential roof falls by a methological scheme developed during this study. A hazard zone risk map was developed showing a generalized regional trend of increasing risk from west to east across the basin. Mapping of individual geologic variables and mapping of hazard risk can be done at any scale. The accuracy of the mapping is primarily dependent on the density of the drill core records. Geologic feature maps and the hazard risk map contained in this report are reduced (1 :500,000 scale) versions of the larger scale (1 :62,500) maps prepared for the Bureau of Mines as map folios.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. This is the final report of a study done by IIT Research Institute for the Bureau of Mines to identify the problem areas and technological inadequacies in noncoal (metal, nonmetal, stone, and sand and gravel) mining and processing systems that are the primary causes of death, injury and occupational disease, and to make recommendations to improve safety and health in these operations. The findings and recommendations presented in the report are based on: (1) analyses of MESA injury and employment data for the period 1961-70, MESA fatality reports for underground noncoal mines (1968-72) and lost time accident data for 1970-72 obtained from mining and processing operation: (2) visits to a variety of noncoa1 mines and mills throughout the country to observe the operations and to study the safety organization and practices used in the industry and (3) interviews with industry and government representatives knowledgeable in the field of noncoa1 mining and processing safety and health. The report provides a comprehensive overview of safety and health problems.

To aid in winning the recent world war, the Bureau of Mines conducted a National Fuel Efficiency Program, directed to commercial plants throughout the Nation, to save greatly needed fuel, a basic resource vital to industrial production. In planning this program it was realized that to accomplish a satisfactory reduction in fuel consumption in any plant, it would be necessary to reach, enthuse, and usually educate to some extent the actual operators of the various types of plant equipment. A major part of the programs consisted, therefore, of visits to plants and work with the operators, including a review with them of their operating conditions and of educational and guiding literature pertinent to the operating problems involved. Through a Nation-wide organization, local volunteer engineers served in making visits to local plants. Each cooperating plant appointed an employee as the "waste chaser," who headed the work of saving fuel in that plant. The educational literature was designed to interest the operator, as well as serve as a guide in eliminating waste of fuel. Volumes have been written on many subjects relating to the saving of fuel. Writings of the treatise type contain considerable good information but do not have wide appeal and probably will be read only by a few studious operators. The problem is not one of getting clown on paper everything that the experts know and would like to have the operator know, but rather to write something that will actually interest the operator and stimulate him to action. Past experience led to the adoption, therefore, of the "quiz-sheet" type of writing, using a quiz sheet of not more than two pages for any principal item. The results obtained during the National Fuel Efficiency Program in many types of plants further demonstrated that the short quiz sheet is satisfactorily appropriate and does accomplish results. Each sheet was prepared and checked by many engineers expert in the particular subject matter of the sheet. This report presents a study of the Cooper sand of the Fork Run pool of Warren and McKean Counties, Pa., and a prediction of the secondary oil recovery by gasflooding the reservoir. A computer program employing a modification of the Craig, Geffen, and Morse secondary recovery calculation was used to make the gas flood performance forecast and is presented in the appendix. The Cooper sand reservoir in the Fork Run pool lies at a depth of about 1,900 feet and covers an area 1/2 mile wide by 2-1/4 miles long. Original oil in place is estimated to have been 8,266,000 STB. The primary recovery mechanism is solution gas drive, and ultimate primary recovery may reach 20 percent over a well life of 90 years. Two computer predictions were made for gas flooding the Cooper reservoir. The first was for a 25-acre pilot five-spot utilizing existing wells, but project economics were poor since only 25 percent of injection gas is utilized for pilot oil recovery. Recovery was 71,000 STB, or 22 percent of oil in place at flood initiation, but required 25,700 scf gas injected per STB of oil recovered. In the second prediction, pilot area performance was projected for the entire Fork Run pool, and economics became good. Recovery was 1,753,000 STB from 25 five-spots after the injection of only 6,400 scf per STB of oil recovered.

A series of experiments were undertaken by the Bureau of Mines to determine the emission products of several types of conveyor belting and other combustible materials found in mines. These experiments were conducted under intermediate-scale, simulated mine conditions to determine smoke characteristics and gas concentrations. From these determinations, heat-release rates, particle sizes, obscuration rates, combustion yields, and production constants were calculated. Three types of belts were investigated: chloroprene, also known as neoprene (NP); polyvinyl chloride (PVC); and styrene-butadiene rubber (SBR). The belts were designated as ignitable or self-extinguishing depending on the length of the burning time and the subsequent combustion products. Under these experimental conditions, the SBR belts were the easiest to ignite. The PVC and NP belts tended to self- extinguish within a few minutes after ignition, but were still capable of maintaining a brief flaming period. These conveyor belt combustion results are compared with previous analyses of wood, transformer fluid, and coal fires. Together they form a data base by which findings from future experiments with other mine combustibles can be compared.

This report examines the feasibility of Employee Assistance Plans for the coal industry. Three issues are examined in detail. First, the prevalence of off-the-job factors such as alcoholism, drug abuse, and marital problems are examined in a sample of miners from the eastern, midwestern, and western underground coal fields. Then data were presented that examined the link between these off-the-job factors and behaviors critical to the mines such as absenteeism and turnover. Lastly, we examined the effectiveness of the major Employee Assistance Program in underground coal mining--the HELP Program. The report contains recommendations on whether coal mining companies should implement these programs and how these programs should be designed.

MORE THAN 30 MILLION EMPLOYEES each year are exposed to hazardous occupational noise, and approximately one-third of them develop permanent hearing loss (NSC, 2000). Possible explanations for the latter statistic include worker regard for noise as a mere nuisance rather than as an occupational safety and health hazard, and consequent neglect of established protective measures (e.g., hearing protection); lack of knowledge concerning noise; improper use of hearing protection; worker indifference; and inadequate training (Bauer & Babich, 2004). Many drill rigs generate high sound levels, which vary according to the type of machine, its location with respect to the physical surroundings, and the location of the sound measured around the rig. A NIOSH study measured noise output from several types of drill rigs during field investigations (Ingram & Matetic, 2003). Two types of data were recorded the actual noise generated by the drill rig at various positions around the rig and the noise to which the drill operator was exposed during the drilling cycle. For the latter, sound level measurements up to 120 dBA were recorded while hammering casing (metal on metal) in drilling a water well (Photo 1). Several types of administrative controls exist to address worker exposure to loud drill rig noise. Rotating duties to decrease exposure time and posting warning signs in high-noise work areas are two such controls. Another is the use of hearing protection. Worker use of hearing protection has been disappointing, however. One study by the University of Michigan found that construction workers used earplugs and earmuffs only between 36% and 61% of the time that they were necessary (Lusk & Kerr, 1998). In a 2001 study, OSHA reported a relatively low rate of 30.4% for use of earplugs among construction workers (Walker, 2001). Why are rates for hearing protection use so low? One explanation may be workers' lack of concern for noise in the workplace. They expect workplace noise to be a loud and in¬ evitable nuisance and, therefore, accept it as part of the job. As a result, they disregard proven measures that can protect their hearing. Other workers may use hearing protection, but do so improperly. They may misguidedly remain in high-noise areas for extended periods thinking they are protected when in fact they are not. Other reasons for low use and/or misuse of hearing protection include poor education, lack of awareness and inadequate training (Nash, 2003). To address the training issue, NIOSH developed two exercises from which drillers can learn about noise, hearing loss and hearing protection. The first is an invisible ink exercise, titled "Drill Rig Incident" (DRI) (Photo 2); the second is a 3-D slide reel training aid, "Wearing Hearing Protection Properly" (WHPP) (Photo 3). Deterministic and stochastic simulation were combined to develop a computer model of an open pit mining system using bucket wheel excavators and related materials handling equipment. The model is closed and dynamic in the sense that information is fed back to control activities at various levels and reservoirs. The results of interaction between unit operations in one time interval determines overall system behavior for the next interval. Equipment performance curves and system profile characteristics rather than time studies are applied to determine the operation of each activity at any time period. For that part of system where performance is dictated by a random variable~ subjective probabilities are employed. Two stages of research were considered to formulate this model. Stage one involved the formulation of equipment sub-assemblies for the sequence of equipment moves along with related assignments and routines for information retrieval. The second stage of research combined all sub-assemblies into an integrated open pit mining simulator. The model is not tailored for any specific open pit operation but is readily adaptable to any system of surface mining including operations having a high degree of complexity. Accident rates continue to show that underground mining jobs areamong the most hazardous occupations in the United States and roof bolter operators continue to have one of the highest accident incident rates (MSHA, 1991) despite efforts by labor, government, and industry to reduce and eliminate work-related hazards. In the last two decades we have seen technical innovations such as the ATRS, and there has been an increased emphasis on hazard awareness through programs such as MSHA's REAP, but bolter operators continue to be injured and killed. Researchers at West Virginia University examined risk in the roof bolter's job and developed new approaches to accident and injury prevention. The study concentrated on the workers' job performance, machinery and the work environment to identify miner's risk and hazard exposure while bolting.

As part of a continuing effort to develop technology that maximizes mineral and metal recovery from domestic resources, the Bureau of Mines investigated a method to totally utilize copper converter slags without recycling them back through the matte smelting furnace. The research was directed toward (1) recovering the contained copper in converting slags by producing a recyclable copper-iron matte, (2) carbothermically reducing the slags to recover the iron, and (3) producing slag wool from the depleted slags. This report deals with the reduction of three industrial converting slags, containing 2.0 to 7.3 wt-pct Cu, in an 800-kVA electric arc furnace to produce a recyclable copper-iron matte. The addition of sufficient iron pyrite to provide 29 to 113 pct of the stoichiometric sulfur (based on producing 40-pct-Cu matte) produced a recyclable matte (36 to 61 pct Cu) at an energy consumption of 520 to 700 kWh per ton of solid material charged. The expected energy saving for treating molten slag in an electric furnace (5-to 9-MVA range) is about 400 kWh per ton of molten slag treated.

Systematic practices for protecting crops from frost damage began in the United Metes more then 40 years ago when the Weather Bureau Fruit-Frost Warning Service was organized; however, various practices employed in frost protection are centuries old. Although there are areas where little doubt exists as to whether protection is needed, there are also circumstances where there is some doubt as to the need for frost protection. Many factors influence this, including not only critical temperatures but also the maturity dates of the crops relative to marketing. Critical temperatures for frost damage to trees, plants, blossoms, and fruits very with weather, plant variety and nutrition, and stage of growth. In the case of truck crops, a single night of protection from frost injury might yield big dividends with a variety of sweet corn or tomato that matures early while price is high yet be worth very little with a midseason or late-maturing variety. Consequently, it is the individual grower who must decide whether to use frost-protective measures on the basis of his returns, the quantity and quality of his crop, the marketing situation, the sensitivity of his crops to frost damage, and the frequency of damaging frost in his area.

In 1911 the Director of the United States Bureau of Mines instructed the chief mining engineer of the bureau , during an investigation of coal-mine safety conditions in Europe, to ask the directors of the various mine-experiment stations whether they would favor a European conference . The majority of the officials in immediate charge of experiment stations favored the plan , and Victor Watteyne, inspector general of Belgium, suggested that the United States call such a conference to be held immediately after the Eighth International Congress of Applied Chemistry, which was to assemble in New York on September 3, 1912 . Accordingly, invitations were issued by the United States Government to those countries having mine-experiment stations , namely, Austria-Hungary, Belgium, France, Germany , and Great Britain, and to those that contemplated the establishment of such stations , Canada and Mexico . All but Great Britain and Canada accepted ; Mexico designated a representative , who later found it impossible to come. The Government of Great Britain in declining, expressed regret at its inability to accept ; an informal statement was made that the opening of the experimental gallery at Eskmeals , England , occupied the attention of the officials in charge . Delegates were appointed by their respective Governments as follows: Austria-Hungary, Julian Czaplinski , royal mining engineer. Belgium, Victor Watteyne , inspector general of mines . France, J. Taffanel, director of the Lievin experiment station . Germany, Carl Beyling, bergassessor, director of Dortmund experiment station. United States , Charles E. Munroe, consulting explosives expert and George S. Rice, chief mining engineer of the Bureau of Mines . Subsequently Mr. William O'Connor , a mining engineer and mine operator of Wales , who opportunely appeared at the Pittsburgh station with letters of introduction from the South Wales Institute of Engineers and from prominent mining officials of Great Britain , was unanimously invited to take part in the meeting as an unofficial representative of Great Britain . Messrs . Beyling, Taffanel , and Watteyne were met in New York on September 3, by George S. Rice, who accompanied them to Washington, where they were welcomed by Director J. A. Holmes , of the Bureau of Mines , and where , with the delegates to the Eighth International Congress of Applied Chemistry , they were presented to President Taft . Mr. Taffanel was unfortunately called back to France just before the scheduled time of the conference on account of a mine disaster ; the other delegates , including Mr. Czaplinski , convened at Pittsburgh on the morning of September 14. Mr. Rice, representing the Director of the Bureau of Mines , acted as temporary chairman , and called the meeting to order.

Need for lightweight building materials was created in the latter part of the nineteenth century by a radical change in building design in which dead load or structural weight was transferred from thick, heavy, load- bearing walls of former construction practice to a load-carrying framework of beams and columns with thin walls. This new design was brought about by the introduction of structural steel followed by reinforced concrete and structural concrete as materials for the erection of the load-supporting frame. The new materials and method of construction made possible the erection of skyscrapers and the building of bridges with long spans, and made feasible additions of one or more floors to existing buildings. As size and design of the load-bearing framework are controlled chiefly by dead load, the natural result was a. search for and use of light- weight materials in partitions, floors, and exterior walls. However, these thin walls created problems of heat and sound. insulation, so that building materials which possessed good heat- and sound-insulating properties in addition to light weight were objects of extensive research. One group of materials developed to fill this need comprised lightweight concretes, of which some were obtained by the use of lightweight aggregates. Although the advantages of lightweight.materials were evident from the first skyscrapers, there was a transition.stage.between the old bearing- wall and modern skeleton-frame types of building, with the heavy and massive construction of the former superimposed upon the latter. Hence, it was not until the period between 1910 and 1930 that the new lightweight concretes were recognized and used more extensively in the erection of large, nonresidential buildings. From this type of construction, light- weight-aggregate concretes entered-fields of residential and small non- residential building; in which they were found desirable for production of prefabricated, large-size; masonry units. Although the weight of each unit was reduced sufficiently to enable one man to handle it easily, properties of the new concretes other than light weight were the principal factors in the ready acceptance of these new materials in residential building. Manufacture of precast units includes complete lines of masonry and back-up blocks, bricks; floor beams, columns and partition, roof, and floor tiles and slabs. Recent improvements in casting machinery, development of the vibration-pressure machine, and a more thorough study and knowledge of the requirements and properties of the concrete mix have all contributed materially to the rapid expansion in use of lightweight aggregates in this industry.

The object of this paper . is to make known the superior classification accomplished in a hydraulic classifier designed to make use of principles determined by recent research ... The details of the design , the results of preliminary research , and the final testing are given . The classifier of the new design shown in figure 1 and another one like it are in operation as concentration classifiers in a Birmingham iron- ore mill recovering 4 tons per hour of concentrates from table middling that ` otherwise would require regrinding and retabling . The four factors that contribute to the successful operation of these classifiers are ( 1 ) a teeter column of small diameter , ( 2 ) use of an open - bottom tooter column classifier with a periodic pincer instead of the usual type , which terminates in a vortex, ( 3 ) a sorting chamber under separate hydraulic control , and ( 4 ) a teeter column of more than the usual depth . · At the outset it must be stated positively what the authors mean by hydraulic classification : They mean the reconstruction of a multimineral ground product into fractions graded with respect to size , so that cach fraction has the heavy mineral segregated in the fine sizes and the light mineral segregated in the coarse sizes . This definition presupposes that in multiplespigot classification the low specific gravity minerals are carried beyond the first cells to the later ones . The reader must take into account that , from the standpoint of classification , " ores " may be divided into two types , represented - by the following : ( 1 ) Joplin flint - blende ores , which , because of the hardness of the flint and the softness of the blonde , yield upon comminution a product that already has the essence of classification ; and ( 2 ) Birmingham Big Seam iron oros , which contain " easily shelled - out " sca sand and , upon reduction to about 10 mesh , yield a heterogeneous mixture in which the size analyses of the respective " floats " and " sinks " arc almost identical .

This report summarizes the work performed under USBM Contract No. J0100038, "Demonstration of Noise Control Techniques for the Crushing and Screening of Nonmetallic Minerals." The objectives of the program were to: a. Identify the prevalent sources of crushing and screening noise in portable and stationary plants b. Design retrofit noise control treatments for those sources c. Field test control treatments under actual operating conditions. A census of the nonmetallic industry was performed to determine the number and types of crushing and screening equipment. Based on the census, eight plants were selected and surveyed to assess the variety of noise sources, determine the worker noise exposure to those sources, and to identify possible constraints that had to be considered in designing and installing the retrofit controls. Three plants were selected from those surveyed and retrofit noise control treatments were designed, including: a. A primary crusher operator control booth b. Screen decks made of resilient cloth c. Screen feedbox impact pads d. Rubber feed plate for secondary crusher e. Rubber feed chute linings for secondary crusher f. Noise attenuating curtain for secondary crusher. A new primary crusher control booth was installed at one plant, and the operator noise levels were reduced from 97 dBA to 78 dBA. Screen noise at the two secondary crushing plants was reduced by 3 dBA to 7 dBA; crusher noise was reduced by 5 dBA at one of these two plants. Noise levels at normal outside operator and cleanup locations were reduced by 4 dBA to 7 dBA at one secondary plant, but operator/cleanup noise levels were not significantly reduced in the second plant due to rapid wear of installed materials. At one of the two secondary plants the retrofit noise control materials displayed excellent wear characteristics. The rapid wear of retrofit materials at the second plant was due to improper materials being supplied and changing circuit conditions.

Bench-scale research was undertaken to determine if adding ammonia to cyanide leaching solutions would enhance the extraction of copper from an argillaceous Michigan copper ore. Two leaching techniques, (1) rolling using a tumbling bottle and (2) aerating using a modified Buchner funnel, and variations in pulp temperature; were employed in evaluating the effect of ammonia concentration on copper extraction. Under these experimental conditions, the extraction of copper was shown to be independent of ammonia concentration and entirely dependent on cyanide concentration.
