The most frequent causes of accidental injury and fatality are carelessness and ignorance; and the most common accidents are haulage accidents, fall, drowning, and fire. However, in this modern day and age, severe injury has become the dire consequence of an ever-increasing number of industrial and mining accidents as well as sport-, military- , etc. Mine accidents have declined dramatically in number and severity through decades of research, technology, and preventive programs. However, preventing recurrence of disasters like those of the past remains a top priority requiring constant vigilance by management, labor, and government. The goal of the present research is to investigate and identify root causes associated with types of mining accidents (victim being caught by an equipment, bodily motion, over exertion, etc) and nature of injuries such as (sprain and strains, fracture, dislocation, amputation, etc). Further, we hope to discover the impetus of the root causes occurring more and more often in this modern day of technology and quality awareness. With the different dimensions of accidents and injuries brought into the fold, there is the need for researchers and mining operators to comprehend and better still visualize significant patterns of associations among these variables of interest. We assume that, there is a high correlation between many dimensions of nature of injuries and types of accidents. This suggested a principal component analysis to reduce the dimensions for easy and amenable interpretation.

For improvement of urban traffic infrastructure, the City of Amsterdam is cur­rently constructing the 9.5 km long North-South Metro line passing through the central districts of Amsterdam. Focal section of the project is the Central Station linking the NS Line with the Central Railway Station, the East Metro line, new bus and tramway terminals, ferries and individual traffic. Construction works required for the project are of high complexity and technical standards. They are executed under severe restrictions concerning programming, space, logistics, and impact on the integrity of existing buildings and structures as all existing installations and facilities have to maintain fully operational during the construction period. The paper deals with the main geotechnical features of the project and in particular with the diaphragm wall and bored pile construction at the Voor-plein and the De Ruijterkade sections of the Central Station.

Introduction Newmont Mining Corp.'s Carlin Gold Mining Co. mines gold ore and waste material from four open-pit mines in Northeastern Nevada. Combined production rate is 24.5 Mt/a (27 million stpy). The company is producing gold bullion from the 2.4 kt/d (2600 stpd) Carlin Mill No. 1. and the 2.3 kt/d (2600 stpd) Maggie Creek heap leaching plant. In August 1985, the 6.3 kt/d (7000 stpd) Carlin Mill No. 2 at Gold Quarry came on-line. The total combined throughput of 11.2 kt/d (12,000 stpd) will make Carlin Gold Mining one of the largest US gold producers. Feasibility studies are underway to increase the total gold out-put from Gold Quarry. This will be done by adding a continuous dump-leaching process that will recover gold from lower grade ore. History In 1961, Newmont exploration geologists located claims in the Lynn Creek mining district near Carlin, NV. An exploration drilling program revealed significant gold mineralization. A stepped-up drilling program revealed an ore body with an estimated 10 Mt (11 million st) of ore containing 109 t (3.5 million oz) of gold. Test results in 1963 allowed metallurgists to develop the flowsheet and design for a 1.8-kt/d (2000-stpd) countercurrent decantation (CCD) cyanide plant. Carlin Gold Mining Co. was formed to operate the property. Carlin purchased its first mining equipment in 1965. This consisted of two Bucyrus-Erie 2.7 m3 (3.5 cu yd) shovels and nine 27.2 t (30 st) Wabco Haulpak trucks, two Inger¬soll-Rand track-mounted drills, one Chicago Pneumatic T-650 rotary drill, one rubber tired Michigan 280 dozer, one Caterpillar D-8 36A track dozer, one Caterpillar 16 motor grader, and the necessary ancillary equipment. The fleet was capable of mining 3.2 Mt/a (3.5 mil¬lion stpy) working two eight-hour shifts, five days a week. In 1967, one P&H 1500 3.8 m3(5 cu yd) diesel electrical shovel and three 59 t (65 st) Dart trucks were added to the fleet, along with a Chicago Pneumatic rubber tire mounted rotary drill. This equipment addition increased the mining capabilities to 5.7 Mt/a (6.3 million stpy). Three separate pits, the Main, East, and West, within a 1.6-km (1-mile) radius of the Carlin mill were being mined. The crew consisted of 46 hourly men and three supervisors. This equals 497 t/ total hourly manshift (548 st) or 407 t/total manshift (515 st). Rising gold prices in the early 1970s allowed lower grade deposits to be mined at greater distances from the mill. The Boot-strap pit, 19.3 km (12 miles) north of Carlin, was started in 1973. The Turin metro alluvium soil is mainly made of large boulders, cobbles and gravel , few sand and almost no fines. This grain size curve is out of the range of application for EPB and Slurry TBMs. In order to bore 5 of the total 7,5 kms of Line 1 tunnel, two specially designed Lovat EPB TBMs were utilised in combination with a systematic non structural pre-grouting of a small volume rectangular slab just above the tunnel section. The same pre-grouting was not however applicable for lack of access on surface in some sections of the remaining 2,5 km of tunnel that had to be bored below important buildings/structures, with under ground water pressure up to 1,5 bar, with curves radius of 180 m only. For these more critical sections it was necessary to study and implement a new technical solution and improve the capacity of the TBM to work efficiently in EPB mode even in this unsuitable ground and overall conditions. After months of studies the selected and adopted solution has been to inject inside the cutterhead chamber, in addition to the almost standard foams & polymers additive mixes, a suspension of limestone powders. This with the scope of compensating the lack of fines of the natural ground with rock powders.

Ball mills are commonly used for regrinding operations in the mineral processing industry. But, in recent times, efforts are being made at the introduction of stirred mills particularly for those instances where very fine grinding is required. Media type (steel shot, glass beads, alumina balls and crushed quartz), media size, mill speed and the number of passes required to obtained the target product size are studied. The product size and energy consumption are used in the evaluation of the test results. The grinding media size, shape, density, filling ratio, mill circumferential speed and grinding time play a distinct role in product size fineness.

The objective of this paper is to examine some modern forms of auxiliary ventilation fabric ducting with streamline design fittings and the efficiency of construction practices. The most dynamic form of auxiliary ventilation is a moveable fan and ducting, which is easy to install and uses light weight fabric. The porous nature of some ducting construction results in leakage and loss of efficiency. Flexible fabric ducting is primarily associated with metalliferous mining and industrial material quarrying. Some applications are found in coal in stone/rock development and with use of dust scrubbers.Forcing auxiliary ventilation systems employing circular fabric ducting have been examined. Leakage of air from ducting is affected by ducting material and construction methodology (such as welded or sewn), quality of installation, number of joints, total length, pressure differences between the inside and outside and diameter. The best ducting system with welded construction can have up to four times less leakage as compared with some systems with sewn construction. It is also evident that the sewn type ducting will deliver less air to working faces and require more fan power, hence higher operating costs to run when connected to the same fan.A case study has also been undertaken to compare predicted operating costs of auxiliary ventilation systems connecting various types of ventilation duct fittings. The study also assesses the technical importance of streamline design with low aerodynamic resistance to reduce the shock losses of the duct fittings. It was found that the system connected with streamline designed fittings is able to provide some power cost savings and deliver more air compared with the system fitted with conventional fitting pieces.Using a ventilation ducting system with a lower leakage and frictional characteristic represents a sound solution to new challenges faced by the industry for increasing demand for better quantities and quality of air delivered to working faces; this is due to higher production schedules and stricter mine health and safety regulations within the Australian mining industry.CITATION:Wu, H W and Gillies, A D S, 2017. A rotating disc of precipitant zinc in a single stage flow reactor was used to study the effects of deposit mass on the reaction kinetics of copper cementing on to zinc from dilute aqueous solutions.The reaction rate is controlled by external mass transport and the mass transfer coefficient (relative to the original precipitant area) is dependent on the quantity of deposit present. For deposit masses up to about 0·3 mg/cm2 the mass transfer coefficient (first order rate constant) is that expected for the counter-diffusion of reactant and product cations through a laminar boundary layer to a smooth rotating disc. For depositmasses from 0·3 mg/cm2 to about 5 mg/cm2, the mass transfer coefficient increases significantly with deposit mass. For deposit masses greater than 5 mg/cm2 and at least up to 12 mg/cm2, the mass transfer coefficient has again a constant value which is some four to seven times the initial ("deposit free") value. The rate constant for the entire range of deposit masses studied (0 to 12 mg/cm2 ) can be expressed empirically as a cumulative normal distribution of the logarithm of the deposit mass.The enhancement in the mass transfer coefficient for deposit masses greater than 5 mg/cm2 relative to the initial mass transfer coefficient depends on the local operating conditions and increases with increasing disc rotational speed. This enhancement is, however, independent of the previous history of reaction rates.

Metal dusting is a corrosion phenomenon that occurs in strongly carburizing gas atmospheres at elevated temperatures. Reaction kinetics and product characterizations of Alloys 602CA and 800 were examined by scanning electron microscopy with energy dispersive X-ray analysis (SEM-EDX) and X-ray diffraction (XRD).The results showed that Alloy 602CA is more resistant to metal dusting than Alloy 800. Visual examination and SEM surface analysis showed that Alloy 800 suffered metal dusting attack after a relatively short exposure period. The amount of coke deposits increased on Alloy 800 with increased exposure from 96 to 336 hours. X-ray diffraction on the reacted surfaces identified graphite and austenite in both alloys and some iron oxides/spinel for Alloy 800.

"The Underground Research Laboratory (URL) is located in southeastern Manitoba, in the Lac du Bonnet granite batholith of the Canadian Shield. It was constructed by Atomic Energy of Canada Limited (AECL) to conduct geotechnical research for the Canadian Nuclear Fuel Waste Management Program, assessing the concept of permanent disposal of nuclear fuel waste deep in plutonic rock.One of the primary objectives of the research program at the URL is to develop a nondestructive rock mass characterization method that can be used during siting, design and construction of a used fu el waste disposal vault. An experimental program was established to assess radar reflection profiling as a technique for fracture characterization in granite. The highly resistive, homogeneous nature of the Lac du Bonnet granite is ideal for the propagation of electromagnetic energy at the radar range of frequencies and water-filled fracture zones act as excellent reflectors because of their contrasting electrical properties.Radar reflection profiling was undertaken to characterize a major fracture zone located between 5 m and 10 m below Room 211 on the 240 m level of the URL. The profiling was done at a frequency of 120 MHz, on a 1m grid laid out on the bare rock floor of the room. Reflectors identified in the analog data were correlated between the profile lines, digitized, and a three-dimensional surf ace for each recognizable reflector was constructed. These surf aces were then compared with data obtained from the examination of numerous cores from adjacent boreholes intersecting the fracture zone. A very good correlation was found between the major radar reflectors and the most intensely fractured regions of the zone. Modelling studies also showed that a series of prominent shallow radar reflectors were caused by a borehole drilled close to Room 211."

The Huntly West Mine has undergone significant changes in the last two years, which will affect the remainder of the mine's life. The paper describes the evolvement of plans and an overview of the technical considerations made in planning for and implementing the first New Zealand longwall operation. The technical investigations involve the longwall equipment, exploration, ventilation and strata control. The focus on these areas was to ensure that unacceptable project risks were addressed. Implementation of longwall mining at West Mine is planned in three phases, Development, Panel One extraction trial, and Full Longwall operation.   At present the development phase is complete, longwall installation is in progress and production start-up is planned for the end of August, 1991.

Dioctahedral clay minerals in the volcanic-hosted Golden Cross epithermal Au deposit, New Zealand, have been investigated by petrography, X-ray diffraction (XRD), scanning electron microscopy (SEM) and transmission electron microscopy/analytical electron microscopy (TEM/AEM) to determine the nature of these minerals in a hydrothermal ore deposit. Heterogeneity in the distribution of dioctahedral clays is observed from the hand-specimen scale to the TEM scale, indicating that primary rock textures and fluid pathways (related to rock type, porosity/permeability and fluid/rock ratios), in addition to temperature, are important factors controlling the distribution. As the proportion of illite in mixed-layer illite/smectite (I/S) decreases, TEM observations show decreasing layer stacking order, parallelism of packets, and packet size, and increasing apparent porosity. AEM structural data show a corresponding decrease in AlIV, AlVI, and K+, and an increase in Si, Ca2+, and Fetotal. The observations of this study indicate that local dioctahedral clay mineral reaction progress is controlled mainly by permeability and structure, but that this is superimposed on a predictable, system-wide clay mineral distribution that is controlled by temperature and fluid/rock ratio.

The objective of this study was two-fold: 1. To perform a world literature search pertinent to the analysis, selection and design of hoisting systems and their components and to codify the search into a literature compendium. The search resulted in a technical data file of over 1500 hard copy items. The compendium index was also suitable for establishing a comprehensive computer-loaded data base on the subject. 2. To develop a comprehensive system of guidelines to cover salient aspects of analysis, selection and design of electrical, mechanical and structural components of a hoisting system with a view to establish parameters for maximum safety and efficiency of operation, installation and revision of the system. 3. Considerations in the mine shaft layout, design, sinking, outfitting, testing and commissioning are set forth against the background of requirements of applicable parts of Code of Federal Regulations, Title 30.

The purposes of the research were to assess the applicability of new accident investigation methods to mining accidents, and to adapt those found to be applicable in the development of a new methodology for use in mining. The development included field testing, evaluation and modification of the new methodology. The report is in two volumes. Volume one discusses the background research, the design of a methodology for field evaluation, preliminary field tests in Utah mines, and work done to prepare for a formal, complete evaluation by federal government mine safety officials. This, volume 2, discusses the field evaluation done by the contractor and cooperating mines after the program was re-directed by the Bureau of Mines. It relates to research done during a period of approximately eighteen months, ending in April, 1983.

In the presence of oxygen, galena is oxidized in an aqueous medium containing ammonium acetate in accordance with the following reaction: PbS + 1/2 0, + 2 NH~Ac -» PbAc, + So + 2 NH: + H2O. This reaction was studied in an autoclave under oxygen pressure by polarographic measurement of the concentration of lead ion in solution. Oxidation rate is parabolic. The reaction products are lead acetote and elemental sulfur, the latter forming a film on the galena crystal. Reaction kinetics appear to be determined by the diffusion of lead from the galena phase to the solution interface through the film of sulfur. RECENT applications of pressure leaching on a commercial scale have emphasized the metallurgical importance of the oxidation of sulfide minerals in aqueous media by oxygen under pressure. Research experience has shown that sulfates, thio-nates, and metal oxides are the products of humid oxidation of sulfide minerals in basic solutions. The metal oxides may be solubilized in the presence of a complexing ion.' Certain oxide minerals are also amenable to treatment by this type of oxidation.' Several kinetic studies have been made of reactions of this type. Stenhouse' found that pyrite (FeS?) oxidized in a sodium hydroxide solution to produce a soluble sulfate and an insoluble layer of iron oxide and that the kinetics of the reaction were controlled by oxygen diffusion through the oxide layer. Andersen' studied the oxidation of galena (PbS) in a sodium hydroxide solution under oxygen pressure. He found the lead and sulfur to be simultaneously oxidized to produce plum bite ions and sulfate ions, and that the reaction rate was proportional to the surface area of the galena. The kinetics appeared to be determined by the reaction of oxygen and water with the galena surface. In each of the above reactions the metal and sulfur components of the mineral were oxidized simultaneously and at equal rates. In the present study the alkaline medium has been replaced by an approximately neutral solution of ammonium acetate in which lead sulfate is soluble. Under the conditions no sulfate was produced and no sulfur could be detected in the solution. The metal component of the mineral was solubilized and the liberated sulfur remained as a film of elemental sulfur on the surface of the galena crystal. A kinetic study of this reaction was undertaken to establish the kinetic mechanism and to determine the rate controlling step. Specific reaction rates were determined by oxidizing crystals of galena of measured surface areas in an autoclave maintained at the desired temperature and oxygen pressure. The rate of reaction was followed by measuring the concentration of lead in the ammonium acetate solution with a cathode ray polarograph, employing stationary platinum electrodes incorporated in the autoclave. This circular is part 5 of the third paper (entitled "Excavation") of a series summarizing the technical problems involved in the production and preparation of sand and gravel. Part 1 discussed the use of power shovels, draglines, and excavator cranes; part 2, power scrapers, slackline cable-way excavators, and hydraulic monitors; part 3, hydraulic, clamshell, ladder, and dipper dredges; and part 4, haulage systems in general, with particular reference to locomotive and car, hoist and car, and remote-control systems. This circular discusses the use of motor trucks, conveyor belts, pumps end pipe lines, barges and towboats, and aerial trams. MOTOR-TRUCK HAULAGE During recent years motor trucks have gained considerable ground as a successful competitor of locomotive haulage for quarries and gravel pits. Often where trucks have displaced locomotives and cars the operators readily admit an increase in both labor and total haulage operating costs but point to some other advantage as offsetting the extra expense. In other instances haulage costs have beep reduced by the use of trucks. In still others operators, having replaced locomotive haul with trucks, have again returned to the earlier method. Obviously gasoline-driven motor trucks cannot successfully compete with other types of haulage under all conditions. The apparent' growth in the number of advocates of truck haulage, however, indicates that it has passed the experimental stage and become a permanent competitor to other tyres.

An account is given of experimental apparatus for generating a short duration high velocity water jet and the effect of its impingement on various rock samples under air or water cover. The penetration was found to be highly dependent upon the rock structure; in general material with closely interlocking grains was resistant.The damage sustained by a stationary sample held close to the jet did not vary greatly whether the sample was in air or confined under a water layer; but when the sample was translated under the jet there was a marked increase in the damage sustained.Some consideration was given to the possible practical application of a water jet for deep hole drilling and an estimate made of the power requirements and transmission losses for such a system.

The purpose of this effort was to investigate the feasibility of employing a piezoelectric transducer for seismic communications applications in mine environments. The transducer, a government furnished device incorporated a high Q, spring-mass resonator to load the air side of the piezoelectric crystal. The resonant frequency was seismic load dependent and fell in the range of 300 to 340 Hz. The effort included a mathematical analysis of the transducer, with corroborative laboratory tests, together with actual communications experiments performed first in an open-formation limestone quarry and finally in an operating iron mine. This work showed that the transducer could be coupled efficiently to hard, competent rock media with power conversion efficiencies of 70 to 80 percent (electrical to mechanical) in typical installations. In the iron mine, when radiating peak power was about 30 W (acoustical) in a gated pulse mode, signals were received at a distance of 760 ft through a massive rhyolite porphyry formation. This represents a 15% radiation efficiency with an input of 200 watts. Despite the high ambient noise level due to operating mine machinery, a signal-to-noise ratio of about 20 dB was achieved using a very narrow band filter to process the received signal, and a data rate of about two pulses/sec was demonstrated.

The Buchans mining district is located roughly at the center of the island of Newfoundland and contains some 18 separate mineral deposits. The first of these deposits was discovered in 1905, the latest discovery having been made in 1979. To the present (1983), some 16 Mt (17.7 million st) grading 1.33% Cu, 7.5% Pb, 14.5% Zn, 115 g/ t (3.7 oz per ton) Ag, and 1.4 g/t (0.04 oz per ton) Au have been mined. Mining operations are now largely terminated, but a few hundred thousand tons of economically marginal reserves may be mined in a salvage operation. The Geological Association of Canada Special Paper No. 22, "The Buchans Orebodies, 50 years of Mining and Exploration," contains papers which describe the history, geology, and the application of evolving geological concepts in exploration. Papers by Neary, Swanson, and Thurlow and Swanson in that volume are topical to the present discussion. Successful exploration techniques at Buchans included physical prospecting, utilization of the state-of-the-art geophysical techniques, saturation diamond drilling which was made possible by the high gross value of the existing ore, and, since 1955, diamond drilling directed by fairly well understood geological parameters. The record of discovery started in 1905 when the Buchans River deposit was found by Matty Mitchell, a prospector employed by the Anglo- Newfoundland Development Co., a British company which owned mineral rights over a very large area in the Red Indian Lake-Exploits River drainage system.

The Bureau of Mines investigated the applicability of the freeze-drying technique for preparing dispersion-strengthened tungsten powders suitable for conversion by powder metallurgy into materials which might have improved high-temperature strength. Extruded rods from compacts of tungsten-5 vol pct thoria had at 1,650° C a tensile strength of 30,300 psi and a 10-hour stress-rupture strength of 12,700 psi, These are slightly below those measured for swaged rods of commercial tungsten-3.7 vol pct thoria. Only those tungsten-thoria dispersions containing a stoichiometric deficiency of oxygen yielded a significant strengthening effect. A low-melting eutectic formed in extruded tungsten dispersions containing either zirconia or yttria, which caused a lowering of the tensile strengths. These tensile strengths were only slightly higher than strengths of elemental tungsten made from commercial powder having a particle size of 2.4 microns (Fisher subsieve). Sheet made from this commercial tungsten had a slightly higher tensile strength than tungsten made by freeze drying, but the freeze-dried tungsten sheet had a lower recrystallization temperature, a lower ductile-to-brittle transition temperature, and a higher purity.
