Major Hazard Risk Assessment (MHRA)1 is used to help prevent major hazards, e.g., fire, explosion, wind-blast, outbursts, spontaneous combustion, roof instability and chemical and hazardous substances, etc., from injuring miners. The structured process associated with MHRA helps to characterize the major hazards and evaluate engineering, management and work process factors that impact how a mine mitigates its highest risk. The National Institute for Occupational Safety and Health (NIOSH) studied the application of this technique to US mining conditions through a field-oriented pilot project. Risk assessment teams used in the pilot project were primarily composed of mining company personnel. Ten case studies were performed over a wide cross-section of mines. These mines were representative of the important mining commodities in the US minerals industry, i.e. coal, metal, non-metal, and aggregate. Also, the sizes of the mines ranged from small to large and were located across the country. The ten case studies demonstrate that most US mines have the capability to successfully implement an MHRA and that the MHRA methodology produced additional prevention controls and recovery measures to lessen the risk associated with a select population of major mining hazards. The basic ingredient for a successful MHRA is the desire to become more proactive in dealing with the risks associated with events that can cause multiple fatalities. A successful outcome is marked by a thorough examination of existing prevention controls and recovery measures. When pressed to consider more controls to further mitigate the risk, a well-staffed risk assessment team was able to identify additional controls. For these mining operations, it was important to add additional controls, even if they were not required by existing mining regulations, to lower the risks associated with the major hazards under consideration. If a mining operation is not willing to commit its best people to an MHRA or will not provide them with sufficient time to see the process through to its conclusion, the MHRA output may prove to be useless. Additionally, if a mining operation is not prepared to discuss its major hazards in an open and honest fashion and to present the findings of the risk assessment in a written report, the MHRA output will be unclear, and attempts to monitor or audit important controls may not be possible. A MHRA is most effective when the mining operation possesses 1) a proper understanding of its hazards, 2) experience with informal and basic-formal risk assessment techniques, 3) proper facilities, machinery and equipment, 4) suitable systems and procedures that represent industry Best Practice, 5) appropriate organizational support with adequate staff, communications and training, 6) a formal and thorough plan for emergency response, and 7) a safety risk management approach that is promoted and supported at all levels of the organization.

In 1984 and 1985, at the request of the U.S. Forest Service, the U.S. Bureau of Mines examined the 87,720-acre South Boise Yuba study area (formerly known as RARE II Wilderness Recommended area I-4553) to evaluate mineral resources. The study area and additions are in Blaine, Camas, and Elmore Counties, 10, about 60 mi east of Boise. Granitic rocks of the Cretaceous age Idaho Batholith occupy the western two-thirds of the study area. A Tertiary age granitic complex has intruded, and broken into small blocks, Paleozoic age sedimentary rocks in the eastern third of the study area. Tertiary Challis Volcanics overlie both of these rock types. Principal mineralized structures are gold-and silver-bearing quartz veins and shear zones trending northwest to easterly in the Cretaceous granitic rocks. Copper, lead, and zinc are accessory metals in many mineralized structures.

Surveys to establish the quantity and quality of coals available for metallurgical, industrial, and energy use, investigations to develop better mining methods, and studies aimed at improving the health and safety of those who mine the coal highlighted Bureau of Mines research and technologic work on coal in 1968. Also being developed are more efficient methods of coal preparation, transportation, storage, and utilization. Supplementing this effort are projects designed to assure minimum disturbance of land, water, and air resources. Research centers and laboratories of the Bureau of Mines are listed in appendix A. Publications released during 1968 are cited with the various projects and in appendix C. Organizations that cooperated in the work in various ways are cited in appendix B.

The average gravity of the total crude petroleum produced in the United States during; the year 1927 was 32.2° A. P. I., or 0.864 in terms of specific gravity. A similar study, made by the U. S. Geological Survey in 1921, gave the average gravity at that time as 32.0.° To one who is accustomed to thinking of crude petroleum in terms of Oklahoma-Kansas and North Texas trades it may be surprising that the average gravity for the entire country is no higher than 32.20. The chief reason for this apparent discrepancy lies mainly in California production, which in 1927 amounted to over 25 per cent of the total, but which had an average gravity of only 24.10. A factor of less importance was Arkansas, which, though on the decline as a producing State, exerted a material effect by reason of its average gravity of 21.6°, the lowest for any State. In view of the heavy production of light crude in the Seminole area of Oklahoma during 1927, it would appear that the average gravity should have shown more than a 0.2 per cent gain over 1921. That this did not occur was due to the fact that in the interim between 1921 and 1927 important heavy oil discoveries were redo, notably the Smackover field of Arkansas.

Historically, coal miners have known that roof shales can deteriorate in contact with humid mine air, causing massive roof falls and injuries from falling rock. It is critical to recognize rocks prone to weathering and to adequately support these rocks in order to ensure the long-term stability of the openings. In a recent study, NIOSH has used a wet/dry cycling test to determine the moisture sensitivity of over 800 specimens of roof rock from 25 U.S. coal mines. Fireclays and some gray shales are the most moisture-sensitive. Rocks with disturbed bedding, in contrast to flat-bedded rocks, are also more sensitive to water. Black shales are relatively un-reactive to moisture and serve to protect more reactive gray shales above. Mines that have roof rocks with moisture-sensitivity indexes above 40% can experience slaking roof conditions, and many require high coverage surface controls. Three case studies are presented in which the moisture-sensitivity index is correlated to roof conditions underground, and can be used to indicate long term deterioration. Engineering measures are described to control moisture-sensitive roof. In one case, roof screen not only reduces injuries from rock fall but also is shown to reduce roof falls. Clean energy production from coal, oil, and gas, and the development of these and other energy sources continued to be a major objective of the energy research program of the Bureau of Mines.3 In 1971 increased emphasis was placed on the production of fluid fuels and chemicals from coal. Advances were achieved in the development of the Synthane and Hydrane gasification processes for producing pipeline gas from coal (page 31), and progress was made in research on solid wastes utilization (page 2), and on liquid fuels production from coal (page 37); Coal mine safety, solid waste disposal and utilization, coal preparation and transport, and fundamental research on coal and related products also continued to be the subject of extensive research. Improved methods for extracting petroleum and natural gas without surface and subsurface pollution featured research on these vital fuel and energy sources. This research was highlighted by studies of the fracturing systems of reservoir rocks, including subsurface fracture mapping and new fracturing techniques to achieve optimum production of oil and gas (page 52). Of equal importance were projects on oil recovery by water or gas flooding (page 52), by steam injection (page 57), on the identification of oil spills (page 63), and on means to reduce vehicular exhaust emissions (page 66).

This volume summarizes mining and petroleum legislation in 22 jurisdictions of the Near East and South Asia. Within these jurisdictions, primary attention is given to requirements that must be met in each country for acquisition of rights to permit development of mineral resources. Coverage includes : First, identification of the controlling laws or statutes governing minerals acquisition and reference to the administrative authority; second, analysis of the laws governing mines and quarries and their products; and third, analysis of laws applicable to natural gas, petroleum, and related materials. Where pertinent, sections dealing with historical background or administration have been added. In countries where minerals are subject to control of states or provinces, those areas are considered separately from the central government.

A laboratory study assessed the impacts of water spray pressure, face ventilation quantity, and line brattice setback distance on respirable dust and SF6 tracer gas concentrations around a continuous mining machine using a sprayfan or directional spray system. Dust levels were measured at locations representing the mining machine operator and the standard and off-standard shuttle car operators, and in the return airway. The results showed that changes in all three independent variables significantly affected log-transformed dust levels at the three operator sampling locations. Changes in setback distance impacted return airway dust levels. Laboratory testing also identified numerous variable interactions affecting dust levels. Tracer gas levels were measured on the left and right sides of the cutting drum and in the return. Untransformed gas levels around the cutting drum were significantly affected by changes in water pressure, face ventilation quantity, and setback distance. Only a few interactions were identified that significantly affected these concentrations. Gas levels in the return airway were grouped by face ventilation quantity. Return gas levels measured at the low curtain quantity were generally unaffected by changes in water pressure or curtain setback distance. At the high curtain quantity, return airway gas levels were affected by curtain setback distance. A field study was conducted to assess the impact of these parameters in an actual mining operation. These data showed that respirable dust levels may have been impacted by a change in water pressure and, to a lesser extent, by an increase in curtain setback distance. A series of tracer gas pulse tests were also conducted during this study. The results showed that effectiveness of the face ventilation was impacted by changes in curtain flow quantity and setback distance. Laboratory testing supported similar conclusions.

INJURY and employment figures for nonmetal mines and mills in 1959 are presented in this publication. Data for nonmetal mines are presented separately from those for nonmetal mills. The Bureau of Mines is authorized to send questionnaires to the nonmetal mining industry each year requesting their voluntary submission of injury experience and employment data. The voluntary response to these requests has contributed greatly toward the promotion of safety in the industry, and every effort has been made to make the information as complete as possible.

Under the first phase of this contract as reported in the Phase I report it was determined that certain types of commercially available molded case breakers are capable of interrupting mine dc power supplies of nominally 300 volts or 600 volts rating. These circuit breakers incorporate conventional overcurrent trip units but also can be operated by means of low power signals fed to a shunt trip coil. Further development in the Phase I program produced a means whereby a discriminating circuit breaker detector unit could sense the presence of illegitimate loads on the dc electrical system and cause the circuit breaker to trip. The detector unit operated by detecting the presence of 3000 Hz currents which were generated in the illegitimate loads by means of a 12 volt, 3000 Hz signal applied to the dc system. Further development of the program has continued in Phase II with the aim of verifying the practicability of the complete discriminating circuit breaker concept and developing other types of equipments required to implement a system. These other equipments include electronic means for raising the apparent impedance of high power legitimate loads and static filters for smaller loads. The results of laboratory work and limited underground tests have been successful. This report describes the system and the tests that have been conducted on the various equipments that comprise the complete Discriminating Circuit Breaker Concept.

This report presents a more extensive perspective of the calculated profitability of developing and producing crude oil and associated natural gas by primary methods from selected onshore areas of the United States. Estimates were made for cost of drilling, completing, equipping, and producing oil wells at four depths in seven onshore producing areas. A computer program was developed for calculating the discounted cash flow rate of return for the 64 different models in each area resulting from varying the oil-producing rate and production decline rate. The results of these calculations provide data that vividly illustrate the change in rate of return to changes in well depth, initial producing rate, and production decline rate, as well as to variations in development cost and operating expense. Cost per barrel of oil produced in terms of annual cost and total cost is also discussed. Included in the report are explanations of the method of determining costs and the procedure used for profitability analysis. Summaries of all cost estimates and results of profitability analyses are presented in the appendix in tabular form for each of the seven producing areas.

"INTRODUCTION As part of a general investigation of the zinc-producing area of southwestern Wisconsin, the Bureau of Mines undertook a limited amount of exploratory drilling in the immediate vicinity of the New Dale Rundell mine, in the Mifflin District of Iowa County, Wisc.The work was carried out during the early part of 1945, at a time when all effort was being directed toward increasing production of metals important to the Nation's war program. Bureau of Mines engineers decided to conduct exploration at the New Dale Rundell mine after a preliminary study of available data showed a reasonable possibility of indicating an important deposit. Before the Bureau of Mines investigation, 50 holes had been drilled in the vicinity. Of this number, 18 encountered ore of minable grade. Subsequently the mine was developed and operated for a brief period; however, work was stopped before all of the ore disclosed by drilling had been mined. There appeared to be a correlation between the ore shown by the drilling in the New Dale Rundell mine and another disclosure some 1,500 feet to the northwest. The Bureau of Mines exploration was pinned to explore this possible correlation. Of the 7 holes drilled by the Bureau of Mines, 3 disclosed minable ore beyond the limits of previous drilling."

In a certain application2/ it was necessary to compute the beta function B(p, q) for a certain set of values p and q. Since t he set of p and q values was much mo re finely spaced than those in any published tables, the authors decided to make their results available in this report. In connection with this application the functionB(p, q) was needed at various points in the p, q plane at intervals of 0.05, 0.05<p = q< 10.00. Fletcher, Miller, and Rosenhead2/ state that the only tables available in the desired range are those of Rearson.4/ However, the interval used by Pearson was not small enough for the present application. For this reason it was decided to calculate B(p, q) for p = q =0.05 (0.05) 10. 00.2/ This table is published because of the fundamental importance in mathematical analysis of the complete beta function.

This report describes an experiment which was conducted to compare the relative bumping hazard of a low and high profile hard hat (helmet) in a low seam mining environment. The study was specifically designed to assess the effect of lowering the helmet profile on frequency of head impacts in an underground low seam coal mine simulator. This involved subjects performing four common low coal mining tasks wearing the low and high profile helmets in the low seam simulator (36" and 48? heights). The results of the study indicated that a low profile helmet could reduce bumping frequency in underground low coal approximately 25%, and as much as 80% on selected tasks. It was beyond the scope of the study to measure the impact force, thus it was not possible to estimate the effect of the low profile helmet on severity of impacts underground. The research team recommended that a more extensive investigation be made of head injuries experienced in low seam mining (both underground and surface) with the objective being to assess the impact of a low profile helmet on total head accident exposure.

An underground mine disaster is an accident of major proportions that takes a significant toll in human lives. A disaster usually disrupts the normal functioning of a mine and may result in entrapment of miners whose normal egress from the mine is cut off. A disaster often necessitates a rescue operation and a means of keeping the trapped miners alive while they await rescue. Explosion, fire, inundation of water or toxic gases, and collapse of a major portion of a mine are among the causes of mine disasters. Miners threatened by such an occurence must either bring the situation under control (e.g., by extinguishing a fire) or remove themselves from the danger. If control is unsuccessful, they must retreat to a place of safety. Four different activities may be involved: evacuation, escape, survival, and rescue. Evacuation is the orderly exit of people from the mine using previously identified escapeways and following a previously determined evacuation plan. Miners whose normal evacuation routes are blocked may be able to find alternative exit routes and escape by their own efforts. For those who cannot, the issue becomes survival while waiting to be rescued. The survival period may last for many days while rescue efforts are mounted from outside the mine.

Accelerometers and pressure transducers, mounted in suspension components, can be used to monitor the ride of haulage trucks. Presently, it is difficult to tell what caused a jolt to the truck using either pressure or acceleration data alone. When information from a global positioning system (GPS) is recorded at the same time as shock pressures or acceleration data, the exact location of an event can be determined. Mine management can then determine what caused the jolt to the truck. This research is part of a National Institute for Occupational Safety and Health (NIOSH) project called "Engineering controls for reduction of jolting/jarring injuries in surface mines." NIOSH is investigating how the work environment of haulage truck drivers can be improved. The US Mine Safety and Health Administration (MSHA) notes that, between 1986 and 1995, 60% of the back injuries were to haulage truck drivers in metal/nonmetal surface mining. Personnel from Phelps Dodge suggested that combining acceleration data with information obtained from a GPS could generate results with a variety of uses. Therefore, researchers at the Spokane Research Laboratory (SRL) began investigating how to tie acceleration and GPS data together. It was originally thought that this tool would be used primarily for road and truck maintenance. However, as research progressed, it became apparent that it would also be useful in providing feedback about equipment operations and identifying unusual causes of jolting. Experimental approach Available components that function well by themselves were linked for the purpose of proof of concept. Using a Modular Mining Systems (MMS) product called Vital Signs, SRL researchers took an Aero-Marine Products (AMP) acceleration recorder and hardwired it to MMS Dispatch equipment already installed on a truck. AMP is one of several manufacturers of acceleration recorders frequently used to monitor freight during shipping. MMS Dispatch is used for production accounting and truck dispatching and uses GPS tracking. Through Vital Signs, an analog or digital alarm signal can be recorded. Steve Rhoades, of MMS, wrote unique code that links the Vitals Signs data and GPS location and stores this data in an ASCII log file. SRL researchers installed the equipment in a Caterpillar 793 haul truck. The result was a working prototype. Figures 1 and 2 show the instrument installation in the truck. Two different techniques were used to make measurements of the absolute value of electromagnetic noise in an operating coal mine, Itmann No. 3 Mine, located near Mullens, West Virginia. The electromagnetic environment created by 250-volt-dc and 550-and 950-volt-ac machinery in two longwall panels was measured and is reported. One technique measures noise over the entire electromagnetic spectrum of interest for brief time periods. It is recorded using broadband analog magnetic tape, and the noise data is later transformed to give spectral plots. The other technique records noise amplitudes at several discrete frequencies for a sufficient amount of time to provide amplitude probability distributions. The specific, measured results are given in a number of spectral plots and amplitude probability distribution plots.
