Historically the insurance industry's involvement in any project has been relatively late in the process. Often it was a case of the last item to be ticked off the checklist has been to ensure that the insurance is in place. However, in more recent times insurers have imposed more restrictions on the cover they are prepared to provide, which has in some cases necessitated a change in the contracting structure to accommodate the insurance policy(ies). In addition, changing contractual practices have changed the allocation of risk between the various parties involved. This calls for a more proactive approach to the design and implementation of an insurance program for both the construction and operational phases of any resource project. This paper explores how a risk managed approach to the design and implementation of an insurance program for a resource project can align itself with current management approaches to project development. From the authors' experience, this approach is best adopted early on in the project (often in conjunction with the bankable feasibility study) and follows AZ/NZS4360 to objectively examine and compare a wide range of risk exposures of the project in a time-effective manner across the following areas: ? personnel; ? environment; ? general management; ? compliance - legislation, systems, protocols; ? asset management; ? change management; ? financial impacts to the organisation; ? products/services; ? service delivery; and ? technology. Having identified and then analysed the risk exposures according to common consequence and likelihood ratings the next step is to develop a risk map of all considered exposures. The true value of this approach lies in the development and implementation of a risk treatment plan for identified exposures where the exposure is unacceptable. The plans will lead to the optimal solution to the appropriate risk prevention, risk mitigation and risk transfer options available. This paper also examines other benefits to the project that may arise from adopting a risk managed approach in addition to the ability to design an insurance program which recognises the exposures and the ability of the organisation (and other project parties) to retain risk.

Namakwa Sands is a heavy minerals mining and beneficiation business in the Anglo Base Metals Division, and operates along the West Coast of South Africa. The business encompasses mining, mineral concentration, separation, and smelting operations. The smelting process comprises the carbonaceous reduction of ilmenite to produce titania slag with a TiO2 content of 86%, and iron with a carbon content of 2.5%. The Namakwa Sands smelter, situated near the Saldanha Bay harbour, commenced smelting operations in 1994, when a 25 MW DC-arc furnace was commissioned. The smelting operations were expanded in 1999, with the commissioning of a second 35 MW DC-arc furnace. Despite the fact that ilmenite smelting poses a technical challenge in terms of the high temperatures required, the physical characteristics of the slag, the constraints placed on the feedstock, and the tight product specifications, Namakwa Sands has continually increased slag production since 1995. High furnace feed-on utilization and furnace stability, complemented by continuous improvement drives, will be thevehicle to drive performance in the short-term, with longer-term performance enhancements due to strategic process development. A bewildering variety of continuous thickeners and clarifiers are used in mineral industries and other heavy industries. General topes arc: ? Conventional thickeners ? Thickeners with flocculating feedwells ? Solids contactors ? Tray thickeners ? Lamellas ? Conventional thickeners with Lamellas ? High flow, fluid bed thickeners These units must be adapted to varied applications. They must provide operating flexibility to meet process needs. Such factors multiply the decisions which must be made prior to the selection of a thickener. The seven types of thickeners listed above, with their variations, share design concepts which limit their application and which can be used to evaluate their performance. Some of these arc: ? Feed velocities must be limited and/or controlled. ? Natural or reagent flocculation is necessary. ? Horizontal settling area is the basis for capacity. ? Overflow velocities must be limited and. or controlled. ? A controlled underflow is necessary. Thickener selection is the application of these needs to the basic process concepts. The basic function of a thickener, in the minerals industry, is the separation of a thickener sludge containing fine solids from water. In addition to this primary separation function, others include: ? Separation of solids from water ? Separation of flocculated solids from slimes ? Storage of solids ? Control of percent solids ? Storage of water ? Process control by absorbing fluctuations in feed ? Process control by increase in head clue to the depth and percent solids of the thickened sludge. Most thickener designs, suitable to minerals applications, are based on the separation of minus 65-mesh, 2.7 specific gravity solids containing slimes. These designs are routinely adapted to coarse and or denser materials. Materials of lower specific gravity such as coal, wood, char, some chemical precipitates and organics act Hike slimes if a thickener is used. Perhaps the most conservative approach to sizing and application is the use of comparative data from similar plants or similar industries. Thickeners are-routinely used in a great number of industries from which there is good historical information available. In the minerals industry, thickeners are used in: clay sizing, coal refuse, heavy media, copper concentrates, copper tailings, gold flotation concentrates, and countercurrent decantation for cyanidization in gold and CCD in uranium. Concentrate and tailings thickeners are used in iron ore plants, and concentrate and tailings units in lead-zinc plants. In addition, there arc thickeners designed to handle lime mud; limestone concentrates and tailings; phosphoric acid; potash; and rutile concentrates. Thickeners are, in addition, widely used in gravel plants for water reclamation. This gives excellent data on clay settling in some cases. Thickeners are used extensively for slurries generated by flue dust from wet scrubbers. Thickeners in flue dust applications are operating with considerable variations, treating blast furnace dust. BOF flue dust, and wood char. Similar applications are met in the power industry with fly ash and SO2 scrubber systems. Even though field data is preferred, such data has limitations as listed: ? Flowsheets are seldom identical. ? Data is often incomplete or limited. ? Ores vary significantly. ? Operating needs are changing. For these reasons, mathematical and theoretical methods are needed for application of field data to a specific project. In addition, laboratory data for similar projects and, if possible, laboratory data on a representative sample should be used to specify the thickener. There is a great volume of thickener literature. The references for this paper, along with their references, and past AIME papers provide excellent background. Older literature can be reviewed in light of current methods. It is interesting to note that the requirements for higher flow rate designs, more extreme flocculation systems, etc., can be logically introduced by reading Gaudin's Sections on Flocculation and Thickening, which was written before field installations could illustrate all of his ideas. The problem, then, is to select hardware which will take into consideration the functions of flocculation, separation of the solids, water clarity, control of the underflow, and control of the overflow. While the neurological and vascular aspects of Hand-Arm Vibration Syndrome (HAVS) has been generally accepted as a medical condition, the medical criteria and the clinical findings used to establish the diagnosis has been more difficult to bring to consensus. The criteria was first quantified by the Taylor-Palmear scale.1 This criteria was subsequently modified in 1986 at the 1st Stockholm Workshop2,3 to included more acceptance for the neurological effects that characterized the predominate findings in some workers. The relationship between hand-arm vibration and Carpal Tunnel Syndrome was defined in NIOSH 97-1414. While the aforementioned documents have defined the clinical entities associated with hand-arm vibration exposure, agreement on the clinical findings and test to confirm the diagnosis has be been more difficult to bring to consensus. Clinicians assessing HAVS has relied on a number of varied neurological and vascular tests. The neurological testing has focused on assessing damage to the sensory capability of the fingers for the neurological component including tests to measuring ability to sense vibration, cold or other end point finger sensor functions. However, the vascular testing has been traditionally focused on the ability to either measure vascular function or to reproduce the vascular blanching that occurs in HAVS with cold water provocation. Recent assessment of this testing in the United Kingdom Coal Miner’s study has questioned the value of this testing especially in reviews by McGeoch.5 In an attempt to provide some type of definitive testing to substantiate vascular damage from hand-arm vibration exposure, angiography is an alternative or adjunct to cold water provocation testing. The standards that have been established to predict the level, type and incidence of HAVS have been based on clinical studies and reports that have essentially been all male populations. However, the recent entry of women into more vibration intensive jobs has brought about the exposure of some women to high levels of vibration previously only previously experienced by men. However, there have been only few studies that look at HAVS in women6. Although exposed the same vibration levels, it has not been clear that the latency and type of pathology of HAVS in women will be the same as for men. The purpose of this study is to look at recent case studies of men and women exposed to jobs with high levels of hand-arm vibration with extensive clinical testing for both the neurological and vascular components of HAVS as well as other associated upper extremity conditions such as Carpal Tunnel Syndrome.

It is generally accepted that the optimal flow velocity for gravity classification is the final particle settling velocity or, which is the same, the particle spin velocity. However, we have found differences between the settling velocities and the spin velocities of the very same particles. Here we present the results of experiments in a cascading gravitational classifier comprising eleven stages. The feed material was introduced at each stage successively, from the first to the eleventh. The optimal flow velocity was determined for each case. From these experiments it follows that the value of the optimal flow velocity depends to a greater extent on the point of introduction of the feed (i.e., on the design of the device) than it does on the interaction between the solid phase and the flow.

An environmentally clean and virtually waste-less technology for treatment of domestic solid waste and other toxic chloride-containing materials has been developed and proven on a full scale by the Gintsvetmet Institute in cooperation with some other Russian organizations. The proposed process resolves the acute and globally significant problem of dioxin formation. The purpose of the proposed technology is to ensure such destruction of the structural lattice of toxic compounds that would guarantee irreversible transformation of the resultant substances to prevent formation of dioxins. The preconditions that are required to achieve this purpose are: temperature of >125O0C; an oxidant excess coefficient of >I .05; a certain retention time of the products under these conditions (ADZ s); and thermal instantaneousness of the process involving a transition with no intermediate phase. These conditions were achieved with the experimental development of the Vanyukov furnace design commonly used for sparged smelting processes at non-ferrous metallurgical plants in Russia and Kazakhstan.

The in situ deformability of a closely jointed basalt rock mass was determined by two different methods: the NX-borehole jack method where displacements of opposing curved platens were related to applied hydraulic pressures, and the modified Goodman jack method where tangential strains on the borehole walls were related to induced tangential stresses. The modulus obtained by the modified Goodman jack method were much higher than those obtained by the NX-borehole jack method. To explain the discrepancy, the influence of fractures and test variables, such as depth, orientation, hole location, and applied pressure on the calculated modulus of the rock mass, were analyzed by factorial analysis. It was found that the orientations and measurement depths have statistically significant effects. The in situ deformability values obtained by nonlinear regression analysis were also found comparable with other measurements and empirically predicted values for the basalt rock mass.

In this article, a case is made for the importance of the transfer and management of tacit knowledge to ensure continued sustainable competitive advantage within the pyrometallurgical industry in South Africa. The state of knowledge management (KM) is assessed through a theoretical and empirical study. The empirical part of the study was conducted by means of a survey questionnaire. An important prerequisite within the research entailed determining the current state of KM within the pyrometallurgical industry, and the findings are presented throughout this article. The research argues for initiatives to address the high turnover of pyrometallurgical knowledge workers. This industry needs to entrench tacit knowledge and therefore address the problems associated with the continuous loss of inherent specialized knowledge. Appropriately, emphasis is placed on identifying initiatives required to embed and retain not only human capital lost through mobility, but rather retaining tacit knowledge through a practical KM framework.

A breaker line support (BLS) monitoring system, BLSmon, has been designed and constructed by the CSIRO and installed and commissioned at Laleham No.1 Colliery, Queensland. This system was designed to record hydraulic leg pressure and canopy position measurements and relay these, in real time, to the surface. The system has shown itself to be capable of operating on the BLSs without any adverse affect on production, Analysis of the results of the monitoring exercise has shown that rate of change of leg pressures on the BLSs could be used as an aid in the prediction of adverse mining conditions. Under favourable mining conditions the supports were observed to initially unload from the set pressure, this unloading reduced with time and by the end of a typical lift cycle had either ceased or reversed slightly. This behaviour was considered to be due to one of four possible causes, these being; the compaction of floor debris, and/or compaction of the floor, and/or crushing of the roof and /or support creep. The actual cause can only be determined through detailed monitoring of both roof and support convergence. However observational data would suggest that compaction of debris and softened floor material beneath the supports was the most likely cause. The few positive loading rates recorded under these conditions were of short duration and occurred towards the end of individual lifts. Where mining conditions were unfavourable the negative rates of change of pressure, unloading of the support, were reduced throughout a lift, in both duration and magnitude, when compared with favourable mining conditions. Over the same period the positive rates of change were observed to be greater. The rate of change of loading and convergence can be utilised to identify the onset of instability in the lift area. However, before this facility can be utilised by the mine for the reliable prediction of instability more monitored data and sophisticated processing techniques are required. A mechanism has been postulated for the deformation behaviour of the roof and pillars in a mechanised pillar extraction operation from the continuously monitored data, observational data and results of previous studies. The 7 m wide fenders were observed to yield under abutment stresses before lifting commenced. The stability of these fenders throughout the lifting sequence was considered to be dependant upon the magnitude of their post peak load carrying capability. This is influenced by the post peak stiffness and the extent of roof lowering in the vicinity of the fender. Difficulties with fender extraction occurred where remnant coal left standing in the goaf caused uncaved goat to bridge to the mined fender. Difficulties were also experienced where sub-vertical jointing occurred in the roof of the split reducing it's ability to bridge effectively. Despite the relatively short monitoring period, the potential role and application of continuous monitoring in advancing understanding of the rock mass response to mechanised pillar extraction operations has been demonstrated. This understanding now requires enhancement and shows great potential for aiding the optimisation of mechanised pillar extraction operations.

(written discussion?).-Dr. Brokaw has evidently given a great deal of time and study to this phenomenon and his conclusions seem sound and logical. .Mr. Lee Hager was probably the first geologist of repute to attach any great importance to the occurrence of paraffin dirt in connection with the Gulf Coast oil-producing salt domes. The writer has been reliably informed that through his discussion, the idea was promulgated that the presence of this so-called paraffin dirt was one of the most important and reliable indications of the presence of an oil-producing salt dome. Despite very strong evidence to the contrary, Hager evidently still has considerable confidence in this theory, as brought forth in his discussion1 of Dr. Brokaw's paper. Halter here gives all the paraffin dirt occurrences known to him, and adds, "Fifteen of these localities, including practically all of the greatest fields, have produced oil in some quantity." . The acceptance of the list of occurrences as submitted by Hager depends upon what material we are justified in including under the term "paraffin dirt." If we agree to a broad classification, whereby paraffin dirt is. to include all spongy, earthy, decomposed, vegetable material, or spongy soil impregnated with the same, then Haggler's list will have to be not only accepted but perhaps enlarged. On the other hand, if we confine ourselves to a strict interpretation of the substance known as paraffin dirt, based .upon typical occurrences, general understanding, and application, several of Hager's occurrences will be seriously questioned by many geologists. The typical material is excellently described by Hager as a dirt or soil-like substance "resilient to the step, like rubber, which falls apart in small squares when dug. "The major ilmenite reserves are located in India at Chavara (CH), Manavalakurichi ( MK) and Chatrapur, Orissa (OR). India stands first in terms of ilmenite reserves, which are estimated to be 278 million tonnes out of which, 142 million tonnes are processable. Mineralo-chemical analysis showed that Chavara ilmenite is the most weathered one with about 60% TiO2 while Chatrapur is the least weathered with about 50% TiO2. XRD and SEM studies of different magnetic fractions of these three mineral deposits established the co-existence of ilmenite with pseudorutile, rutile and haematite phases in substantially varying amounts. It is observed that the tropical weathering rather than the source rock difference has apparently played an important role in imparting major compositional and structural dissimilarities among the ilmenites in different places. Further, the chemical analysis, XRD and SEM characterisation of differently weathered samples of MK ilmenite separated by the magnetic fractionation showed that the degree of alteration was invariably associated with the decrease in magnetism, increase in the rutile phase and the change in Fe2+/Fe3+ ratio of ilmenite grains. Magnetically separated fractions when subjected to metallisation with carbonaceous reductant such as coke showed that the degree of reduction increases with the degree of alteration and with the decrease in the Fe2+/Fe3+ ratio.In the processing of ilmenite through the Becher process, aeration rusting of pre - reduced ilmenite is found to be the rate-limiting step thus, affecting the productivity and economy of the process. Investigations have proved that the addition of some organic compounds such as glyoxal, sucrose etc., to the system accelerates the reaction to such an extent that the rusting time has been reduced to less than half. The formation of acids by the these organic compounds during the rusting reaction was proved responsible for maintaining the pH of the solution in the range 3-4 at which iron hydroxide do not precipitate and block the pores in the reacting surface of the grain."

Synopsis U.S. Mine Safety and Health Administration (MSHA) accidents statistics have identified ore-pass hazards as a significant safety problem in U.S. underground metal mines. The statistics show that nearly 75% of injuries are directly or indirectly related to pulling or freeing of ore-pass chutes, the use of hand tools in ore passes, fails of broken rock in ore passes and structural failures of chutes or gates and ore-pass walls. Researchers at the Spokane Research Laboratory of the National Institute for Occupational Safety and Health (NIOSH) are investigating hazards in and around ore passes in hard rock mines. Risk assessment methods, such as fault-tree analysis, have been employed to identify the most probable causes of orepass failures, define research priorities and analyse the factors that result in malfunctioning and unsafe ore passes. Static and dynamic loads are being measured in a laboratory setting with the use of a reduced-scale orepass mock-up. Field tests are being initiated in mine ore passes to determine static and dynamic loads. Data from the test ore passes are being compared with data from the mine to characterize true system behaviour. Computer modelling with the use of closed-form solutions, finite-element analyses and a newly developed particle flow code predicts static and dynamic leads from the flow and the impact of ore and waste in the ore pass. Preliminary results indicate that dynamic impacts from ore and waste rock might be considerably greater than expected. The total static load is substantially less than is typically used fur the structural design of chute and gate support members. It is suggested that damping factors, normal and shear stiffness and mass frictional characteristics have a significant effect on particle flow and resulting impact loads. This is being verified with the use of results from laboratory tests, field tests and particle flow analyses. Ore passes are vertical or steeply inclined openings in a rock mass through which ore and waste are transported and where they mar also be stored. A typical underground mining system is shown in Fig. 1. The arrows indicate the locations of ore passes and loading pockets and their relative configurations. The control of material flow is potentially one of the most hazardous operations in underground mining. According to MSHA data. most of the ore pass-related accidents in the U.S.A. are associated with the loading-unloading cycle and removal of blockages from ore passes. In particular, attempts to free ore hang-ups have resulted in multiple fatalities. The hazards are obvious; there is uncertainty as to when the ore will break loose and miners are unable to escape falling rock to a safe location. Although ore passes and chutes and gate systems for underground metal and non-metal mines roust meet the requirements specified in the U.S. Code of Federal Regulations (CFR). Parts 57 and 75, recent structural failures of ore-pass linings and gates have underlined the lack of adequate ore-pass design standards available to both MSHA enforcement staff and mine engineers. Because of the importance of an ore pass to the safe operation of a mine design criteria need to be assessed and new guidelines developed. Researchers at the Spokane Research Laboratory of the Nationals Institute for Occupational Safety and Health (NIOSH) are investigating hazards in and around ore passes in hard rock mines and studying design criteria for ore passes. In order to develop more accurate procedures for scale-up and design of semi-autogenous-grinding mills through detailed mathematical models, a better understanding of the charge motion and rock/ball segregation is needed. To achieve this a 0.61 m (2.0 ft) diameter x 0.305 m (1.0 ft) length plexiglas mill was constructed as a scale model of a commercial SAG mill. Rock motion in the rotating mill was simulated using various sizes of ceramic balls. Quantitative measurements of dynamic angle of repose, water and slurry motion, holdup and size segregation of the charge were taken. In addition, a video-presentaiton (available to the public) was prepared to elucidate charge and water motion at various mill speeds, charge volumes, rock size distribution and flow rates.
