Throughout all segments of the United States mining industry, 163 independent contractor employees were fatally injured between 1990 and 1997. Factors that contributed to these injuries were identified after reviewing descriptive information contained in Mine Safety and Health Administration fatality reports for each of the incidents. Thirty-three unique contributing factors were identified and multiple factors were noted for each fatality. Those most frequently discerned were "failure to follow `standard operating procedures"', (35% of the fatalities), "poor positioning of employee" (26% of the fatalities) and "failure to maintain equipment" (23% of the fatalities). Contributing factors were then associated with the accident and job classifications of the victims. Factors having the highest frequencies in the top five accident classifications where most fatalities occurred were as follows: powered haulage - "failure to maintain equipment"; machinery - "poor positioning of employee"; slip/fall of person - "inadequate fall protection" and "failure to wear safety line"; electrical - "equipment contact with power line"; and falling/rolling/sliding rock - "poor positioning of employee. " Among the job classifications in which most fatalities occurred, leading contributing factors were as follows: truck driver - "failure to follow standard operating procedures" and "poor positioning of employee"; laborer/utility man/pumper - "poor positioning of employee"; mechanic/ repairman and welder - "failure to follow standard operating procedures." Recommendations for utilizing contributing factor information as a basis for safety training are proposed. Training programs may be developed that inform independent contractor employees of factors which, in the past, have contributed to fatal injuries in their job classifications. Then, by combining this information with contributing factors identified in "related" accident classifications, a truly focused training intervention that addresses very specific workplace hazards may he developed.

"Concentrate drying at the Sullivan Concentrator goes back to 1937 when the original coal-fired dryer was put into operation. This was a second hand 8' dia. x 56' long Struthers-Wells rotary dryer that came from the phosphate plant in Warfield, B.C.In 1962, replacement of the old dryer was seriously considered. The shell and the auxiliary drive mechanism were badly worn and needed replacing. In April 1963, the new 8' dia. x 65' long fuel oil-fired Struthers-Wells dryer went in operation at an installed cost of $174,000.In 1965, it was apparent that a major rebuilding of the dust collecting system would have to be done. A study was carried out and a Ducon Dynamic Gas Scrubber was installed. The system went in operation in November 1966 and has fully met expectations.In the same year, the zinc dryer was converted from fuel oil to natural gas. It is rather gratifying that in the field of the manufacture of refractories, in which, as a whole, foreign practice has surpassed us, the United States stands pre-eminent in the making of silica brick. In some instances European manufacturers have refused point-blank to credit the possibility of making to such specifications as are Common with us, until the actual completed shapes were shipped for inspection. But both the manufacturer and consumer in foreign lands are now frankly learning from us, and benefiting by our advanced practice in this field, as is evidenced by the orders placed here and the efforts put forth abroad to come up to the standard of American-made material. In no metallurgical operation has the employment of silica brick spread with such leaps and bounds as in the building of plants for the carbonization of coal, particularly in the byproduct coke oven. Today, in practically all types of oven, the structure above the floor level (except the facings) is of silica, while in the recuperative and in some regenerative types, its use is extended to that portion of the oven below the floor as well. By reason of the fact that there are a number of classes of refractories of higher silica content than the usual so-called fire-clay brick, there is sometimes a misunderstanding as to the class of material referred to as "silica" brick. We here refer only to a brick having a silica content of 94 per cent. or more and made usually from quartzite rock with a small percentage of lime as a binder. The term "silica" brick is sometimes rather loosely applied to brick from highly siliceous clays, or to a quartzite refractory of mingled fire-clay and quartz rock; but such products are not considered in this discussion. : Materials The rock from which practically all high-grade silica brick are made is a true quartzite or metamorphosed sandstone, variously known as

THE determination of the orientation of metal single crystals and the studies of plastic deformation are greatly facilitated by the use of the stereographic projection. To draw a standard projection of the crystal it is necessary to have a tabulation of the angles between the various crystal-lographic planes of the particular crystal studied. Bozorth' has published such a table for the cubic system which can be found in many reference books; such convenient tabulation for the hexagonal metals, however, does not appear to be in the literature, although scores of workers have calculated these angles. Therefore, Table I of angles for magnesium, zinc, and cadmium has been compiled. Since the c/a ratio is different for each of these metals, three separate calculations are necessary. The angles 4 between (HKIL) and (hkil) were calculated by means of the formula: Hh+Kk+½(Hk + hK)1+~3aLl cos The c/a ratios were calculated from data given in the 1948 edition of Metals Handbook. Fig. 1 is a typical (0001) standard projection of zinc. For all practical purposes, it serves for cadmium as well, as can be seen from the table. In a paper, entitled A Uniform Method for the Assay of 'Copper Material for Gold and Silver,' A. R. Ledoux invited the assayers of this country to contribute to a symposium, in which the results of their assays of given samples of copper-matte and metallic copper would be made comparable. This symposiun 2 appeared about one year thereafter; and through it and its discussion, first became generally known the fact that the gold-yield by the all-fire method of assaying is usually higher than by the so-called combination (wet and dry) method, in which nitric acid is employed as a solvent for the copper. In the publication alluded to, the supposed cause of gold-losses in the latter method was declared to be the solvent action of copper nitrate, or of the reduction-products of the nitric acid, upon the gold. Up to a very recent date, gold has been held to be insoluble in pure nitric acid. (The solvent action of any chlorine present in the nitric acid must here properly be disregarded as belonging in the realm of carelessness.) Among later allusions to this topic, W. R. Van Liew3 ascribes the solution of the gold in the combination-assay entirely to the action of nitrous acid at the high reaction-temperature of copper and nitric acid; F. B. Flinn4 believes the low result in gold to be due exclusively to mechanical losses—finely-divided gold penetrating through the filter-paper—but presents no experimental data confirmative of this belief; and 0. A study has been undertaken to establish the relationships between the O&K wedge test cutting resistances and the geotechnical parameters of rock materials in the context of bucket-wheel excavator operation. The total iron ore production in Sweden in 1969 was 33.3 million metric tons. 31.7 million tons were exported mainly to Western European countries. 27.8 million tons (87 per cent) originated from the mines at Kiruna, Svappavaara and Malmberget, located just north of the Arctic Circle in Sweden. Planned production for 1970 is 25 million tons during 10 months, due to a strike at the beginning of the year. Present production plans for the next five years expect an annual export capacity around 33 million tons in 1975, including 5.7 million tons of pellets. The mining and shipping at the loading terminals of Narvik on the Atlantic coast and Luleå at the Gulf of Bothnia are managed by LKAB - a 96 per cent state-owned company. 4 per cent of the share capital is owned by the private Grängesberg Company. The railway - 106 miles from Kiruna to Narvik and 130 miles from Malmberget to Luleå - is operated by the State Railways in Sweden and Norway. Comprehensive expansion of the Narvik ore port will be carried through during the next five years to increase the annual shipping capacity to at least 35 million tons and meet the development of rapidly growing ship sizes.

The Institute has lost a valued member by the death of Eckley B. Coxe, Jr. He bore the name of one who was largely instrumental in founding the Institute, and to whose support and guidance as active member, Vice-President, and President, the Institute owes. much. Eckley B. Coxe, Jr., was the nephew of the late Eckley B. Coxe. He held a large interest in the coal estates owned and developed by his family, and he was thus naturally led to take a continuing interest in mining matters, and for years and up to the time of his death was a full member of the Institute. He died at his residence, Drifton, Pa., on Sept. 20 last, aged 44. He was a member of the class of '93 of the University of Pennsylvania,. of which great institution he was a devoted alumnus. His educational and charitable activities were numerous, but his chief life-work and interest was in archeological investigation and research in Egypt, carried on through the Museum of the University of Pennsylvania of which Museum he was President. The discoveries made by the various expeditions financed and directed by him, and the notable publications issued by his direction and at his expense, chronicling the results, are among the most important researches ever made of buried Egypt. Mr. Coxe was a member of the International Historical Society and of many educational and charitable organizations. By his will, among other bequests, he left $500,000 to the Museum of the University of Pennsylvania and $100,000 to the University, the income of the latter fund to be applied to increasing the salaries of the professors of the Institution. He also left $100,000 to the Children's Hospital of Philadelphia, of which he was a manager, and in addition thereto $10,000 of which the income is to be applied in providing Christmas presents for .the child patients, and in providing an annual dinner for the patients and attendants of the hospital. He bequeathed $50,000 to Leonard Hall, an associate mission of the Episcopal Church, at South Bethlehem, Pa., $25,000 to the Epileptic Hospital and Colony Farm (Philadelphia) ; $50,000 to the Philadelphia Orthopaedic Hospital and Infirmary for Nervous Diseases; $15,000 to the United Charities of Hazleton, Pa., the income to be applied to the salaries of nurses; and $25,000 to the Mining and Mechanical Institute of the Anthracite Coal Region of Pennsylvania, founded by his uncle, Eckley B. Coxe, and of which Eckley B. Coxe, Jr., was an active trustee up to the time of his death. This Institute is known as one of the best preparatory schools for technical education in Pennsylvania, and is mainly devoted to extending an opportunity for education by day and night schools to the boys and young men of the Anthracite Region. Its principal is an alumnus of Lehigh University; it has an able local Board of Trustees, and an Advisory Board consisting of the Provost of the University of Pennsylvania, the President of Lafayette College, and the President of Lehigh University, all three of which institutions have established prize scholarships in the institute. Mr. Coxe carne of a distinguished ancestry, the record of which is fully given in the biographical notice of his uncle, Eckley B. Coxe, by Dr. R. W. Raymond, in Vol. XXV of the Transactions.`

Large-scale tests in the U.S. Bureau of Mines Experimental Coal Mine show that the explosion hazard of coal dust on shuttle-car roadways can be con-trolled by proper application of water, dry rock dust, a saturated solution of ammonium phosphate, or mixtures of water or the water-phosphate solution with rock dust. The experiments were made with loadings of 1.0 and 5.0 pounds of mine size coal dust per linear foot of entry, and with a 3-or 4-inch-thick layer of coal rubble on the floor, The data from the tests with mine-size coal and rubble are comparable. The water contents of the dust loading were determined from samples collected just prior to initiation of a gas-air mixture in an ignition zone at the face Dust samples were collected 1 inch deep from the coal-rubble deposit. One experiment was made with the dust in the coal rubble bound with latex applied as a water solution by spray. The binding action was inadequate and explosion propagation was obtained.

Commercial processes of lead softening directly involve the behavior on fusion of mixtures of the oxides of antimony and lead, and the vapor pressures of these materials. Practically no quantitative data have been available for the discussion of lead softening from the point of view of the fundamental metallurgy of the process. The Pacific Experiment Station of the United States Bureau of Mines, in cooperation with the University of California, in the course of its program of study of the fundamental properties of oxides and sulfides, recently made vapor pressure measurements upon the various forms of pure antimony trioxide.1 When these measurements were considered in connection with the softening and "fuming" furnaces which are often used for lead purification, an unexpectedly anomalous condition appeared. In brief, this anomaly consisted in the fact that in certain softening furnaces, where lead bullion of approximately 1 per cent antimony content was being treated at a temperature of approximately 1300" F., no appreciable volatilization of antimony occurred, there being produced only a "skim" containing from 15 to 20 per cent Sb, 65 to 60 per cent Pb, as mixed oxides. In the "fuming furnaces," where the temperature was only about 1400" F. and where an accumulation of "skims" from the primary softener was being treated with certain other antimony-containing materials, a considerable volatilization of antimony as oxide occurred; an "impure" skim of some 60 to 65 per cent Sb and 10 per cent Pb was also produced as mixed oxides. The latter material cannot be suitably treated in hearth-type or softener furnaces. The essential chemical difference between the first and second step is the customary use of some reducing material (such as petroleum coke) in the later operation. An obvious interpretation of the effect of coke in facilitating the fuming of the antimony is the supposition that the antimony may be reduced to metal, and volatilized as such. Harmony Gold operates the Mount Marion underground gold mine near Kalgoorlie, Western Australia. The mine consists of two ore zones, East and West, and mining of these zones is currently conducted using the sublevel caving method. When mining commenced in the Western zone, the span was insufficient to induce caving of the country rock above. Dyno Nobel, together with Harmony and 360 Drilling, developed an innovative drill and blast solution in order to promote cave propagation and improve the safety of personnel at the operation. The solution involved a series of shallow angle drill holes up to 90 m into the hanging wall of the West zone. These were charged with Titan« 7000 underground bulk emulsion and the HotShotÖ electronic initiation system. The blast used over seven tonnes of emulsion and 46 detonators and achieved the desired result without damaging nearby mine infrastructure.

How do you portray a large structurally and stratigraphically complex deposit to return a meaningful resource estimation, and produce a geological model that is useable for planning on a daily basis? The George Fisher Mine is located 22 km north of Mount Isa and forms part of XstrataÆs Zinc Group operations. It is currently a major producer of zinc, lead and silver in the region with an estimated total resource for all categories of 127 Mt at 100g/t Ag, 5.3 per cent Pb and 9.2 per cent Zn (June 2005). Production in 2005 totalled 2 647 492 wet tonnes for 317 699 kg Ag, 140 317 tonnes Pb and 230 331 tonnes Zn from underground operations. George Fisher (GF) consists of two deposits that are structurally complex on a mine scale, George Fisher North (GFN) and George Fisher South (GFS). Regional faulting has resulted in a 2 km long package of variably pyritic shale and siltstone that separates the deposits. This paper looks specifically at GFN. Distinct correlations exist between the two deposits based on stratigraphic matches; however, orebody characteristics, geometry and fault architectures differ significantly between the two. The deposits consist of multiple, sedimentary-hosted, stratiform orebodies from 2 to 25 m wide, separated by waste intervals of variable width. Each ore-waste interval is modelled as an individual geological domain. Structurally, George Fisher North (GFN) contains a series of major late-stage NE faults with dextral offset(s) of up to 130 m. Further complexity is added by NS, NW and NNE striking fault orientations that can produce either a loss or repeat of stratigraphy, local drag folding, orebody attenuation and block rotation. Due to this structural complexity and orebody offsets, domains at GFN are further divided into fault blocks (FB). The GFN model comprises eight FBs over a strike length of 1 km, all of which are modelled independently. As drilling progresses and additional resources are converted, further FBs will be added at major fault offsets to accommodate orebody and model expansion. This will be in addition to what is already a large and time-consuming data set to manage and update. For example, each FB has 18 domains each with two constraining wireframes (hanging wall and footwall). Each wireframe has several sets of point data depending on the type of data available, ie diamond drilling, mapping or interpretation points. This equates to over 1100 separate files to manage. Additional wireframes and files are also required for major structures, daily planning requirements and block model runs. In total some 1500 files are managed, updated and maintained for GFN alone. The ongoing need to expand as a business and optimise production in the current economic climate where there is a global demand for metal has led to an increase in production at GF. This has resulted in a rapidly growing data set and highlights the importance of practical and efficient modelling to achieve a meaningful resource estimate. This paper focuses on the geological modelling, modelling approaches and techniques employed for the GFN deposit.

Introduction The Cannon mine at Wenatchee, WA has recently been brought into operation to recover gold from the sandstones of the Swauk formation. The mine is a joint venture between Asamera Minerals (US) Inc. and Breakwater Resources. A tailings impoundment was constructed to impound 2.7 to 3.6 Mt (3 to 4 million st) of tailings at 1.8 kt/a (2000 stpd) over about five years. This paper describes the design and construction of the impoundment. In particular, it describes the 100 m (300 ft) high earthfill embankment that is the main feature of the facility. Project environment The project is located on the outskirts of Wenatchee, a town of about 40,000 people. The town is situated on the flat alluvial terraces beside the Columbia River. The Cascade Mountains rise from the terraces in a series of ridges and valleys. The mine is at the foot of the Cascades. The tailings impoundment is in one of the steep valleys. The climate is mild and dry (Table 1). Site selection Twelve alternative sites within a 16-km (10-mile) radius of the mine were examined. Decision making theory was used to select the most suitable site. Advantages of the chosen site include its proximity to the mine and its low visibility from points of common public access. Also, the area was already heavily impacted by an existing silica quarry. The major disadvantage of the site is the need to pump tailings over a distance of 1.5 km (1 mile) and against a static head of 200 m (655 ft). Also, it is located in a gulch just upstream from the town of Wenatchee. Site geology The impoundment is generally underlain by interbedded sandstones and siltstones. The siltstones are often shattered. The sandstones are often friable. Both rock types are extensively jointed. Some isolated faulting of the sediments occurred before the area was covered with basalt flows. About 10,000 years ago, at the end of the ice age, the basalts were covered by 3 to 10 m (10 to 33 ft) of wind blown loess - a fine sandy clay and silt. River deposits filled the base of the valley to depths between 5 and 25 m (16 and 82 ft). Under the main part of the embankment, these materials were removed to expose bedrock. Ground water flow is generally through the more permeable layers. The permanent water table is about 5 m (16 ft) beneath the ground surface. The Gotthard Base Tunnel (GBT) is 57 km (35 miles) long and consists of two par-allel single-track tubes with an ex-cavation diameter ranging from 8.8 to 9.5 m (28.8 to 31.2 ft). These are interlinked by cross-passages every 312 m (1,000 ft). Two multifunction stations (MFS) are located in the sections Sedrun and Faido, at one-third and two-thirds of the entire tunnel length (Fig. 1). These will be used for the diversion of trains to the other main tube via tunnel crossovers, for the housing of tech-nical infrastructure and equipment and for potential emergency stops. To reduce construction time, the GBT was divided into five sections and was excavated from several sites simultaneously. The tunnel was excavated from the two portals, Erstfeld in the north and Bodio in the south, and also from three intermediate attacks, with their corresponding access tunnels, in Amsteg and Faido and two vertical 800-m (2,624-ft) deep shafts in Sedrun.

A NEW preparation plant has been put in operation to treat coal from the recently opened Concord mine, located about 15 miles west of Birmingham, Ala., by the Tennessee Coal, Iron & Railroad Co., a U. S. Steel subsidiary. The mine produces metallurgical grade coking coal from the American seam for use as coke in the company's blast furnaces at the Fairfield and Enseley works. The capacity of the operation is 1000 tons of run-of-mine per hr, approximately half of which is delivered to the by-product coke plant at Fairfield after beneficiation to metallurgical grade. The remaining portion of the mine run is rock and refuse removed during preparation. No boiler coal or other fuels are produced by this mine.
