Shotcrete usage in declines and developments has become pervasive as operators strive to achieve production improvements. Issues of mix control, fibreload, thickness control, temperature and curing conditions all play a part in the quality control. Often to overcome some of the quality control issues designers over specify the strength requirements in order to meet their operational constraints. Adherence to the standard civil testing requirements of 28 days moist cured at 21ºC without properly calibrating these results to the changing environment underground leads to contractual disputes. There are a number of standard tests specified including cylinder tests, cored tests, beam tests and round determinate panel (RDP) panel tests. The logistics of storing these samples and then transporting them to concrete testing laboratories without due regard to temperature changes with time can influence the results. All the samples can be readily handled by personnel except the RDP panel. The standard RDP panel is 800 mm diameter by 75 mm thick and requires an ITH machine at the mine and a fork lift at the laboratory to move it around. This paper will examine the possibility of a panel which provides scalable results to the standard RDP panel but can be easily handled by personnel and/or tested on site with suitable equipment. This allows for faster turn around and better quality control for the sample prior to testing. This enhances the identification of quality control issues at the mine and improves safety.

The Tom Price orebody, in the Hamersley Ranges of North West of Western Australia, is mined by Hamersley Iron Pty. Limited. The orebody is a folded sequence of hematite and shale bands capped with goethite, with a con- sequential variable iron content. The overall grade is below that required to yield spec- ification product, necessitating selective mining, 'Zone Plans' based on geological structure have been developed to yield forward information on material types and grades. Short and medium term mine plans - the initial stage of quality control - are based upon these 'zone plans'. Blasts are laid out to break to the grade boundaries, thus minimising dilution, and the samples taken from the blasthole cuttings provide blast grades. Blending of shovel out- put from different blasts forms the operational basis of grade control. Shales, being more friable than hematite, produce fines that are lower in iron grade than lump. Quality control in the mine centres around maintaining the target fines grade, compensation being applied in the mine if the fines grade lies outside the target range.

Mt. Whaleback, the property of the Mt. Newman Joint Venturers, contains more than one thousand million long tons of high grade hema- tite averaging 64% Fe, ranging from 70%, the grade of absolutely pure hematite, to 54%. The ore occurs in the Dales Gorge and Joffre Members of the Brockman Iron Formation. The interbedded shale bands common in other orebodies occurring in these horizons are seldom more than a few inches thick and are commonly partially enriched to about-35% Fe. In the generally hard, massive hematite orebody, the gangue minerals tend to break preferentially to the fines, or -6 mm product, and for this reason it is of a somewhat lower grade than the lumpy product. The control of the chemical qualities is concentrated on the fines product for it has been found that maintaining the iron and alumina levels in this product ensures that all chemical specifications are met in both the lump and fines products. Physical qualities are monitored to ensure that the physical quali- ties of decrepitation and degradation of the lumpy product are maintained at levels accept able to the buyers.

For many years practicing geotechnical and structural engineers in Texas and Louisiana were reluctant to include augered cast-in-place (ACIP) piles as a viable deep foundation alternate. Three primary concerns were the lack of a final driving resistance to confirm adequate capacity, the inability to inspect ACIP piles prior to installation, and the fact that pile integrity is highly dependent on the skill of the ACIP pile contractor's field personnel. The Deep Foundations Institute (DFI) Committee on Augered Cast-In-Place Piles has responded to these quality control concerns by establishing standards of practice for ACIP pile installation and ACIP pile inspection. Regional areas of the country have different subsurface soil and rock conditions and many times this will influence local installation and inspection requirements. The best way structural and geotechnical engineers can address quality control concerns is to be familiar with the DFI standards of practice, to know the special requirements for a particular project site, and to incorporate them into a well-written ACIP pile specification. The purposes of this paper are to: 1) review the main fundamentals of a successful quality control program, 2) describe how these fundamentals can be applied to the installation of ACIP piles, and 3) add personal experiences from ACIP pile projects in Texas and Louisiana. Nondestructive test methods are frequently used as a quality control tool to evaluate questionable piles. An additional brief discussion of two nondestructive test methods is presented to complete the paper.

The installation of augered cast-in-place (ACIP) piles requires careful quality control to assure a successful pile installation. Unlike steel, pre-stressed concrete or other types of driven piles, which are shop fabricated, delivered to the site and typically driven to a minimum tip resistance, augered cast-in-place piles are installed to a pre-determined tip elevation or to a refusal criteria. The structural section of the ACIP pile is constructed during pile installation and cannot be inspected prior to installation. Therefore, a carefully administered quality control program is essential to a pile foundation that will meet or exceed design requirements. This requires an experienced team consisting of the Geotechnical Engineer, Contractor and Inspector. The focus of this paper is to discuss the responsibilities and duties of the ACIP piling Inspector. Qualifications of the Inspector The qualifications of the inspector are related to the complexity and difficulty of the proposed project. Ideally, the Inspector should be an individual with a degree in civil engineering with special emphasis in geotechnical engineering. A properly trained technician with geotechnical engineering experience working under the direct supervision of a geotechnical engineer may be able to provide competent inspection on some projects. It is important that the Inspector have a good understanding of subsurface and ground water conditions and how they will affect pile installation. The Inspector must understand ACIP pile installation procedures and be able to communicate with the Contractor, Structural and Geotechnical Engineers; and maintain a detailed pile record for each installed pile.

Concentrate feed changes and the loss of technical and operational expertise during the previous market downturn contributed to the challenge of making the required improvements. Significant progress has been made in the converter aisle by focusing on employee engagement and standardizing best operating practices for a new generation of skimmers. This paper documents these initiatives and seeks to balance the art and science of converting.

In Novemeber of last year I visited a mica mine in Hunan Province, China. For nine months , we had been trying to sell Chinese mica to a customer in Belgium. Talking to the customer was like negotitating with a stone wall. The buyer refused to communicate any -thing other than his steadfast refusal to consider Chinese mica. He would not advise us what he wanted, what specifications or grades. He would give no indication if our price at market, or reasonable, or totally out to lunch. He would not even accept samples. It took no genius to guess that the mica buyer had at some time in the past bought a shipment of Chinese mica and had received a disaster. I invited the mica mine in Hunan in order to see for myself what could be done - without massive investment in equipment and instruments - to upgrade the quality of the mica for export. We had seen this type of customer reaction before. One disgruntled customer who had pur¬chased pigment grade barites direct from China ranted and raved about unreliable and inconsistent quality. It did no good to explain to him that China is a big country, with hundreds of mines scattered throughout many areas, each producing a different product. Our further explanation that Chinese export organizations also varied in degree and efficiency of customer service fell on deaf ears. Samples had no effect on his perception. Eventually we showed him photoraphs of two mountains of pigment grade barites. One lot was our sales of pigment barites to a major European minerals processor. Our Chinese barites stood proudly white and beautiful next to a hill of other material which now appeared at best mediocre and dull by comparison, and which the buyer had decided to reject after he received our Chinese barites. Finally the customer was sufficiently impressed to look seriously at our guaranteed specification and the outturn analysis performed by our other customer.

SINCE the deposits were found in 1927 in the Paradise Range of western Nevada, more than 1 mil- lion tons of magnesite and half a million tons of brucite have been mined and processed into a variety of granular refractory materials used as furnace linings by the steel industry. An additional million tons of magnesite were used during World War II to produce magnesium metal, and some magnesium oxide from the area is going into magnesia cements. Located near Gabbs, Nev., on the west flank of the Paradise Range, the principal bodies occupy an area a mile long by half a mile wide, and lesser occurrences have been found in a belt more than four miles long and in places almost two miles wide. Magnesite reserves containing less than 5 pct lime are estimated at more than 25 million tons. Reserves of better grade material are considerably less.

Quality assurance and quality control (or QAQC) plays a key role in safeguarding the level of confidence of the data generated during mineral exploration and mine development, and is also part of the reporting code requirements for resource estimation and reporting, as well as mining investment. Implementation of good policies and working protocols in quality management will result in the generation of reliable information, which is critical for decision-making on the future of the mining project. The use of certified reference materials, i.e., standard, blank, and duplicate samples, proves effective and efficient for ensuring the level of data confidence in terms of precision, accuracy, and sample contamination. This is well demonstrated by the practice of QC in the exploration programme for the chrysotile tailings storage facilities of Havelock Mine, using 1254 samples from 53 drill-holes. Different statistical tools were applied to analyse the assay results from these samples. The outcome substantiates that the QC practice established during the Havelock exploration project is aligned to best practice guidelines in the industry. The resource delineated indicates that the tailings material at Havelock could potentially become a significant source of supply to the global magnesium market.

The reliable quality assessment of grouting operations is one of the key issues in establishing impermeable grout curtains around shafts, drifts and tunnels. Reliable opera¬tional information on the decrease in the permeability of rock during the grouting process facilitates the assessment of the effectiveness of the completed curtain and assures production quality. Quality control and assurance also fa¬cilitates the early detection and elimination of possible de¬fects in the grout curtain. Thorough control of the grouting process provides the capability to evaluate decreases in the permeability of water-producing fracture zones prior to the beginning or the continuation of excavation of the shaft, tunnel or drift. In order to achieve this objective, post¬grouting hydrodynamic investigations are conducted prior to the initiation or re-initiation of work on the underground workings to measure the decrease in the permeability coef¬ficient of aquifers during grouting operations. 8.1 QUALITY ASSURANCE AND CONTROL OF THE ISOLATION OF ROCK BY GROUTING THROUGH HOLES DRILLED FROM THE SURFACE Quality assurance and control are based on measuring the decrease in the permeability coefficient of water-pro¬ducing horizons as a result of injecting grout into the drill¬holes. For this purpose, the value of the acceptable residual permeability coefficient is calculated beforehand based on the acceptable value of ground water inflow into the shaft, drift or tunnel as specified by the "construction norm and integrated practice (SNIP)" (Anon., 1976). Having ob¬tained the values of aquifer thickness, permeability, and head distribution for each aquifer from flowmetric data (dis¬cussed in Chapter 3), the average value of the permeability coefficient of all aquifers intersected by the shaft is calcu¬lated as [ ] where Ki is the permeability coefficient of the "ith" aqui¬fer; Pi is the pressure of the "ith" aquifer; and M; is the thickness of the "ith" aquifer. The total expected inflow of ground water into the shaft is [ ] where RK is the radius of the contour of influence; RCFB is the radius of the shaft; and p is the coefficient of dynamic viscosity of the ground water. The necessary reduction of the coefficient of permeabil¬ity of the aquifer [ ] where Q is the expected inflow of ground water into the shaft from eq. 8.2; Q.-fin is the acceptable inflow of ground water; and a is the safety factor. By knowing the average permeability value of the rock and the permeability reduction coefficient, it is possible to determine the value of the acceptable residual permeability coefficient for all the aquifers [ ] The values of the permeability coefficient, pressure, and thickness for each aquifer are determined from the data gathered during the hydrodynamic investigations conducted in grout holes prior to beginning the grouting operations as discussed in Chapter 3. After injecting the grout into the first of the holes, the injection into each of the following grout holes is preceded by repeated hydrodynamic investi¬gations. Using the data obtained for each water-producing zone, a graph is constructed where the numbers representing the grout holes in their order of grouting are placed on the abscissa, and the values of the aquifer permeability coeffi¬cient are placed on the ordinate (Fig. 103). This curve char¬acterizes the change in permeability of the given hydro¬stratigraphic unit as grouting operations are completed, one hole at a time. The results of grouting operations are considered to be positive when each sequential observation records a steady decrease in the permeability of the aquifer. The permeabil¬ity measured prior to the injection of the grout into the last hole must be lower than or equal to the acceptable perme¬ability as determined by the application of eqs. 8.1 through 8.4. Table 8.1 presents illustrative data on the hydrogeo¬logic properties of aquifers at a mine shaft prior to the initiation of grouting operations. Data also are presented on the reduction in the permeability coefficient during the in¬jection of the grout. The shaft in question intersects eleven aquifers. Based on data concerning aquifer pressure, thick¬ness, permeability, and the expected inflow rate of water for each of the strata, the average permeability coefficient is calculated according to eq. 8.1 as K = 0.2 x 10-12 m2. As the supply chain for magnesium and magnesium alloys continues to change, due to the consolidation of producers, it is essential that quality control methods be closely examined with each new supplier added. An examination of the various process steps that are needed to assure quality in the final product will be discussed thus giving a basis for the auditing of new magnesium suppliers.2. IntroductionMagnesium recycling in the past has been somewhat limited due to its use more as an alloying agent for aluminum based alloys and as a desulfurization agent in iron and steel than in more readily recycled structural products. Recycling efforts continue to improve as more production is finding its way into automotive structural components, hand tools, and a variety of computer and cellular telephone applications.With the closure of several large North American primary magnesium production facilities over the past decade we have only recently witnessed a tightness in supply that is driving magnesium prices significantly higher, nearly doubling the price in the last year alone. The rapid escalation in pricing will provide additional economic incentives to fully utilize recycling for the recovery of an even greater percentage of both old and new sources of magnesium scrap. Political pressure continues to intensify towards the secondary recovery of any and all recoverable resources of which magnesium recycling readily applies.Economics will also likely drive the development of new sources of magnesium production, both from primary and secondary production. As new sources of magnesium are offered to the marketplace the potential for product quality issues may increase. When considering any new raw material supplier a decision must be made as to the impact of defective material on the intermediate or finished product. Quality defects in magnesium ingot can result in a variety of appearance, corrosion, and even adverse structural effects in the finished product. The potential severity of the impact of defective raw materials on the finished product will determine and often justify a close look at any new supplier. An on-site audit of a new supplier of a critical raw material is often justified to ensure that an adequate quality assurance system is in place. We will now look at a number of key production points that should be examined during an on-site audit that have can have an impact on the overall quality of the finished product."

"Control of analytical data quality is usually referred to in the mining industry as Quality Assurance and Quality Control (QAQC), and involves the monitoring of sample quality and quantification of analytical accuracy and precision. QAQC procedures normally involve using sample duplicates and specially prepared standards whose grade is known. Numerous case studies indicate that reliable control of sample precision is achieved by using approximately 5% to 10% of field duplicates and 3% to 5% of pulp duplicates. These duplicate samples should be prepared and analyzed in the primary laboratory.Bias in the analytical results can be identified by inclusion of 3% to 5% of the standard in each sample batch. Several different standards are used, with values spanning the practical range of grades in the actual samples. A blank (a sample in which the concentration of metal of interest is below detection limit) should also be included. Standard samples alone cannot identify biases introduced during sample preparation, and therefore approximately 5% of the duplicate samples (coarse rejects and pulp) should be processed and assayed at another, external, reputable laboratory.This paper discusses techniques used for estimation of errors in precision and accuracy, and overviews diagnostic tools. It is shown that one of the most commonly used methods, the Thompson-Howarth technique, produces consistently lower results than other methods. These results reflect the nature of this method, which relies on the assumption of a normally distributed error, and thus produces biased results when errors have a skewed distribution. This study concurs with the suggestion of Stanley and Lawie (2007: Exploration and Mining Geology, v. 16, p. 265-274) to use the average coefficient of variation (CVAVR(%)) as the universal measure of relative precision error in mine geology applications: Le contrôle de la qualité des données analytiques est habituellement désigné sous l’appellation d’Assurance et Contrôle de la Qualité (ACQ), elle implique le suivi de la qualité des échan¬tillons et la quantification de la reproductibilité et la précision analytiques. Quality control is essential for augered cast-in-place (ACIP) piles (also known as continuous flight auger (CFA) piles outside the United States) and drilled displacement (DD) piles since the pile is constructed below the ground surface out of the direct sight of the personnel constructing and inspecting the pile. ACIP piles are being successfully constructed to support high-rise structures, hospitals, arenas and stadiums, mass transit systems, and power and industrial facilities. The use of DD pile technology is also gaining ground in the United States, typically utilizing proprietary systems such as Omega (currently utilized by Richard Goettle Inc., and LG Barcus and Sons, Inc.), Berkel, and DeWal (currently utilized by Morris-Shea). Different types of ACIP/DD pile installation equipment are shown in Figures 1 and 2. ACIP piles are installed into granular and cohesive soils, and soft rock to lengths currently approaching 130 ft (39.5 m), with diameters up to 24 to 36 inches (610 to 915 mm), and sustaining design loads approaching 900 kips (4000 kN). Current DD pile installation equipment in the United States is capable of installing up to 79-ft-long (24 m) piles with diameters ranging from 12 to 18 inches (305 mm to 460 mm).

Components of bulk explosive products include ammonium nitrate, water, fuel, stabilizers, and calcium nitrate. The physical attributes include prill/emulsion ratio, density, and viscosity and in the case of prilled ammonium nitrate, hardness, size distribution and fuel oil absorption. These components and attributes in conjunction with ground conditions determine all aspects of explosive performance including the risk of fume. This paper presents a fume risk assessment matrix of explosives components and physical attributes as a function of a simple description of ground conditions. Instrumentation to provide a measure of the quality of the explosive product are also described and these include both “on bench” physical measurements and laboratory analysis. “On bench” methods include “in hole “density, velocity of detonation (VOD), detonation pressure and detonation temperature. Part of the quality control process involves a mobile laboratory unit which at this stage can provide measurements of water content, ammonium nitrate content, calcium nitrate content, fuel content, prill characteristics and prill/emulsion ratio. This unit can be taken to sites to enable the testing of bulk and sensitized products at site rather than after transportation.

"In this paper, the accuracy of the data measured by a newly developed displacement measuring system, DPRMs (Digital Pile Penetration and Rebound Monitoring system) and its applicability as a quality control tool were verified by field measurements of pile penetration and rebound for pile driving performed at 3 construction sites and 1 test site. The DPRMs data were found to be much better than handwork data. A new dynamic formula for pile capacities is proposed uses the results of a pilot dynamic load test and the final pile penetration. The ratios of pile capacities obtained by the new formula to those by the dynamic load test were 94%±7.4% in the contrast with 123±28.3% by the 5S formula and 68±24.0% by Hiley's formula. This result shows the superiority of the new formula. The ratios of the maximum compressive stresses calculated by using DPRMs to those calculated by using PDA (Pile Driving Analyzer) were about 90-110±10%. This result confirms the applicability of DPRMs as an overdriving monitoring tool. Two sets of DPRMs were used to measure the 3-D displacement behavior of a pile head at the test site. The more the pile head moves laterally, the less the final set values were expected because of the loss of driving energy, but this trend was not obvious within the limited data obtained at the test site.INTRODUCTIONPile construction monitoring consists of two parts. In the first part, enough capacity of the pile is secured to stand the design load. In the other part, pile damage during driving is prevented. Although several methods can be applied to achieve the first part, PDA is nearly the only tool for measuring pile stress.Several methods such as a static load test, a dynamic load test, an Osterberg cell, and numerous driving equations can determine the pile capacity. Among them, the static load test and the dynamic load test are most widely used. The static load test is advantageous because it has the highest reliability. The disadvantages are as follows: it is costly and time-consuming. Also, the capacity of the pile under construction cannot be evaluated. The dynamic load test has lower reliability than the static load test, but it is cheaper than the static load test, is reliable enough to determine the pile capacity, can evaluate the capacity of a pile during construction, and can offer information about construction conditions such as the pile integrity and the pi le stress during driving. The dynamic load test takes a relatively short time and is low cost compared with the static load test. However, it is still costly, time-consuming, and unnecessary to apply to all piles in the field as a construction measurement tool. Therefore, driving resistance (blowcounts) is commonly measured to confirm the pile capacity at construction sites."

Quality control is a system which should guarantee that quality is achieved. It is the execution and verification of a project in an organised way, in order to obtain the maximum of certainty that the results meet the requirements. This concerns all stages of the project, from the soil investigation, over the design, to the execution. Quality control can and has been successfully realised for important projects by means of a quality plan. A quality plan is based on a number of general considerations, but should certainly be focussed on the particularities of a specific project. Practical examples of quality plans, which have been realised on sites in Belgium, are developed and the advantage of the system is shown by concrete case studies.

With the destruction of the US-90 Bridge spanning St. Louis Bay by Hurricane Katrina, the State of Mississippi awarded its first major design-build contract in order to have a completed four-lane bridge in a period of less than two years. The structure consists of 76 pre-cast concrete beam spans with a total bridge length from Bent 1 to Bent 77 of 11,124 ft (2.1 miles). All 77 bents are founded on pre-cast, pre-stressed concrete piles and consist of two foundation types: pile bents and water line footings. A total of 53 pile bents were utilized where the deck is less than 52 ft above the water. Further divided, 10 pile bents, supported by 24 in. square, solid piles, are located on land. The remainder of the pile bents are located within St. Louis Bay and supported by 36 in. square, voided piles. All the waterline footings are supported by 30 in. square, voided piles. In total, 138 -24 in., 437 -30 in., and 463 -36 in. piles support the structure. Production pile design was based upon information gathered from the subsurface investigation, design soil profile and 23 out-of-position test piles. Verification of the design was established by monitoring the first production pile driven at each bent using the Pile Driving Analyzer. Particular challenges included highly variable coastal deposits, loads in excess of 1600 kips per pile and a compressed construction schedule. Initial drive capacity of a portion of the production test piles was less than expected resulting in a minimum 14 day setup. Following the setup period, restrikes were necessary to verify capacity. Project production piles ranging from 50 ft in length to in excess of 160 ft in length were used to achieve the required resistance. This paper includes a brief overview of the foundation design and indicator test pile program. The main objective is to provide a summary of the quality control procedures including establishing the driving criteria for each bent and to present issues that arose during production pile driving and the solutions that followed.
