The National Institute for Occupational Safety and Health (NIOSH), Pittsburgh Research Laboratory, has undertaken a study to determine how well self-contained self-rescuers (SCSRs), deployed in accordance with Federal regulations (30 CFR 75.1714), hold up in the underground environment with regard to both physical damage and aging. This report presents findings regarding laboratory-tested SCSRs in the sixth phase of testing from mid-1996 to early 1998. The SCSRs were tested on human subjects and on a breathing and metabolic simulator. These results indicate that most of the apparatus, if they pass their inspection criteria, perform satisfactorily. However, the deployed CSE SR-100s exhibited significantly higher inhaled CO2 levels than new units, as in the previous phase. This will cause higher ventilation rates in most users, which will, in turn, result in higher breathing pressures, possibly causing users to prematurely remove the apparatus. CSE Corp. has developed a noise test that can identify apparatus suffering from chemical-bed degradation causing the early CO2 breakthrough. This test was added to the inspection criteria for the SR-100. In addition, several of the MSA Portal-Packs that passed their inspection criteria were found to have KO2 dust in their mouthpieces. Further investigations by NIOSH and the Mine Safety and Health Administration confirmed these findings, which resulted in the decertification of the apparatus and their removal from service.

Preparedness is an important element of any underground mine's strategic plan in dealing with an unexpected event, such as a fire. A fully implemented fire preparedness and response plan is essential in reducing the probability and seriousness of a mine fire. This report describes the development of an underground coal mine fire preparedness and response checklist (MFPRC). The checklist is a data collection instrument for profiling both the fire prevention and response capabilities of a mine site and usually requires 3 to 4 days to complete. The checklist encompasses conditions, procedures, and equipment that have frequently been identified as the primary or contributing causes of underground coal mine fires. At least 1 day is needed underground to evaluate the water system. This entails measurements of water flows and pressures at fire hydrants, and water throw distances of fire hose and nozzles at several locations (mains and branch lines). A few of the other topics that are discussed with mine personnel include detection and suppression systems, combustible materials, mine rescue and fire brigades, and firefighting equipment. The MFPRC was developed by the National Institute for Occupational Safety and Health (NIOSH),Pittsburgh Research Laboratory. Under a Cooperative Research and Development Agreement (CRADA) with Cyprus Amax, Twenty mile Coal Co. (Oak Creek, CO), the checklist was field tested and further refined. Additional field tests were conducted at several other operating coal mines.

Dump site injuries occur in all major mineral industries. Working in elevated areas near an edge is a common hazard for operators of off-highway mobile mining equipment. In this Information Circular (IC), serious injuries involving haulage trucks working at dump sites are examined for the period 1988 to 1997. Data were acquired from injury reports gathered by the Mine Safety and Health Administration (MSHA). The IC is organized into three sections. The first presents an overview of the frequency and severity of injuries. The second analyzes injury characteristics using MSHA-defined data fields and author-defined injury classifications. Key findings are discussed in the third section. In summary, 370 serious injuries were identified that involved trucks at dump sites, of which 26 were fatalities. Although haulage truck activities at dump sites resulted in a small proportion of the total number of injuries at surface mines, these injuries were much more likely to result in death or significant amounts of lost time than most other surface mine injuries. These findings support the belief that haulage truck activities at dump sites are worthy of continued study by those interested in improving the health and safety of workers at surface mines.

A training software package for new mine employees, called Miner Training Simulator (MTS), has been developed by researchers at the National Institute for Occupational Safety and Health. MTS is a computer-based tool that allows a trainee to enter a simulated mine and interact with his/her surroundings in order to learn basic mining concepts, safety procedures, mine layouts, and escape routes. The training simulator software and instructions for its use are described in this report. Also, each mine using the software will have different requirements with regard to safety training. To customize the simulator for these differences, an interpreted scripting language is used to define interactions between the trainee and the virtual mine and objects in it. The scripting language, called Tool Command Language, uses simple commands to control various actions in the simulation, such as sounds, safety messages, hazards, and movement of objects. The basics of the scripting language are described here, along with many examples and instructions for building a script for MTS.

This report (An Introduction to Safety) is the first in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment's life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics, and includes the benefits of implementing a system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard 61508 and other recognized standards. The scope is "surface and underground safety mining systems employing embedded, networked, and non-networked programmable electronics." System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system's safety. • 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a "proof of safety" that the system and its operation meets the appropriate level of safety for the intended application. This study reports the results of a survey of organizations that used problem-solving simulation exercises to support mine health and safety training. The exercises are designed to teach judgment and decision-making skills within two broad domains: (1) how miners respond to mine emergencies (e.g., first aid, self-rescue, and escape) and (2) how miners integrate safety concepts within the context and performance of routine production work. The simulation exercises were distributed by the Mine Safety and Health Administration's National Mine Health and Safety Academy near Beckley, WV. The present study focused on those organizations that ordered the exercises from the Academy over a 2-year period (1995-96). Researchers assessed how the materials were used and trainer perceptions of the impact of these materials on annual mine health and safety training . A total of 147 organizations ordered the exercises over the 2-year period. Survey responses were obtained from 52 of these organizations. Sixty percent of the respondents rated the exercises as more useful than traditional instructional materials, 40% as equally useful, and 0% as less useful. Most trainers (79%) felt that the exercises help them to make better use of workers’ knowledge and experience during training. Many (71%)also reported that the simulations helped to improve the effectiveness of their instruction and promoted a problem-solving approach to their training. Fifty of the fifty-two respondents reported that they would like to have new simulations developed. A large majority showed a proactive interest in helping to develop and field test new exercises. Ninety-four percent of the trainers judged the exercises as a good value; 92% planned to order more simulations in the future. Based on this study, recommendations are offered for the continuation and expansion of simulation exercises as one method for occupational health and safety training.

This report reviews the performance and applicability of technology for the control of emissions from diesel-powered equipment used in underground coal and metal/nonmetal mines. The methods discussed include Mine Safety and Health Administration-approved low-emission engines, engine derating, fuels, fuel additives, diesel oxidation catalysts, and diesel particulate filters. The potential of each of these technologies is examined individually and in combination. The performance estimates are derived from the published literature and presented in narrative and Tabular form. The purpose of this report is to help the mining industry select the most appropriate method to reduce underground exposures of miners to diesel exhaust in the context of the recently developed diesel regulations. It is important to note that the control technologies discussed in this report have received limited evaluation in underground mines. Additional research is ongoing, and some engineering design changes may need to be implemented before all of these diesel emission control technologies can be safely and successfully used in underground mines.

This Information Circular from the National Institute for Occupational Safety and Health (NIOSH) documents and supplements the information presented in a series of workshops held during 2002 and 2003. The primary intended audience consists of all who are involved in developing and conducting miners? training. According to the Mine Safety and Health Administration (MSHA), mine operators reported 240,000 full-time equivalent workers and independent contractors reported 42,000 full-time equivalent workers working on mine property during the year 2000. Unfortunately, these workers have a relatively high risk of suffering serious work-related injuries and illnesses. The mining industry has the highest rate of occupational fatalities among all U.S. industries. The fatality rate is 30 deaths per 100,000 workers compared to 4.6 for all private industry(Morbidity and Mortality Week Report, 2001; NIOSH, 2002).Compared to workers in other industries, miners also have a relatively high rate of nonfatal lost-time injuries, and their injuries tend to be more severe (Bureau of Labor Statistics, 1999). Many miners are also exposed to significant health hazards, including coal and silica dust, diesel exhaust, and noise. More than 1000 U.S. miners die of lung disease each year(NIOSH, 1999).

This handbook describes effective methods for the control of mineral dusts in mines and tunnels. It assumes the reader is familiar with mining. The first chapter deals solely with dust control methods, regardless of the application. It is a brief tutorial on mining dust control and will be of help to the reader whose dust control problem does not conveniently fit any of the mining equipment niches described in later chapters. The subsequent chapters describe dust control methods for different kinds of mines and mining equipment. This includes underground coal and hard-rock mines, as well as surface mines, stone mines, and hard-rock tunnels. Because dust sampling has so many pitfalls, a chapter on methods used to sample dust is included. For those occasions when there is no practical engineering control, a chapter on respirators is also included. Except for those listed as ?future possibilities? in the longwall chapter, the dust control methods described are practical and cost-effective for most mine operators. If controlling dust were a simple matter, dust problems in tunnels and mines would have been eradicated years ago. Unfortunately, most underground dust control methods yield only 25% to 50% reductions in respirable-sized dust. Often, 25% to 50% reductions are not enough to achieve compliance with dust standards. Thus, mine operators must use several methods simultaneously, usually without knowing for sure how well any individual method is working. In fact, given a 25% error in dust sampling and day-to-day variations in dust generation of 50% or more, certainty about which control methods are most effective can be wanting. Nevertheless, over the years, some consensus has emerged on the best dust control practices. This handbook summarizes those practices.

A computer model was created that generates contact data by means of simulation while altering several variables associated with the machine and its operator. These variables include work environment, the operator's anthropometry, work posture, choice of risky work behavior, and the machine's appendage velocity. In the model, a contact means two or more objects intersecting or touching each other, e.g., the appendage colliding with the operator's hand, arm, head, or leg. This report documents the code development of special features of the computer model, random virtual human motions and behaviors, which made it possible for researchers to study hazardous interactions, such as contacts between the operator and machine. The original idea for random virtual human motions began with the need for a model that, when simulated, would experimentally mimic machine and human actions that could cause actual injuries or fatalities in the workplace. Injury incident investigation reports do not usually contain enough information to aid in studying this problem, and lab experiments with human subjects are also not feasible because of safety issues and ethical issues. Early investigation that evaluated the feasibility of random motions resulted in a primary programming structure and building blocks for code expansion. Basic motions and standard derivation data provided random parameters for the operator's movement in the random motion code. Experimenting with basic motions in preliminary models helped researchers devise a random motion technique that worked with xyz motion parameter values from basic motion parameters. Both values were the building blocks used in motion commands that comprised the core of the random motion code. When the code is executed in a three-dimensional computer model, it generates realistic starting positions for the operator's (virtual human) body and body parts and realistic motions mimicking a work sequence with a valid range of variation in movements. Researchers developed several rules for the random motion technique that, when applied, will maintain directional integrity of a virtual human's basic motions. From previous research, researchers formulated the algorithm that determines the behavior during machine operation. The decision algorithm was needed by the random motion code to randomly select what behavior to use for a simulation execution for a realistic representation of the operator's motions and risky behaviors during work tasks. The operator's chance of avoiding the moving appendage was also a concern to ensure that data reflecting a near-miss would not be considered a contact. Therefore, to filter a database obtained from simulations for actual contacts, it was necessary to track the operator's field of view and determine from this information when the appendage is in and out of the operator's view. NIOSH researchers successfully applied random motions and risky behaviors to examine the speed range of a roof bolter machine appendage for different workplace scenarios and compare statistically which is most likely to cause contacts (and possible injuries) to miners. Researchers in this study believe that the use of such simulations, treated with advanced statistical procedures, are extremely useful tools to evaluate the hazards of tasks where it is not possible to perform experiments with human subjects.

The Spokane Research Laboratory (SRL) of the National Institute for Occupational Safety and Health (NIOSH) and Thiessen Team USA, Spokane, WA, have developed a pull claw that can be attached to any accessible friction bolt ring to test the performance (i.e., load-carrying capacity) of bolts without the use of a pull collar. This would allow true random testing of friction bolts, which need to be evaluated to determine if they are still providing an adequate level of support months or years after installation. Such support is critical to maintaining safe working conditions for underground miners.

Beginning in 1991, scientists at the Pittsburgh Research Laboratory recorded interviews with individuals who are recognized as experts in mine emergency response. These 30 veterans related stories and observations from events experienced during as many as 47 years of response activities. Overall, the response veterans averaged 29 years of mine emergency response experience and 35 years of mining experience. Interviewees included representatives from mining companies, the United Mine Workers of America, and State and Federal agencies. Most of their comments dealt not so much with technical aspects of particular mine emergency responses, but rather the human side of more general topics, including preparedness, experience, people on-site, mine rescue teams, and decision-making. An analysis of their interviews provides an overview of lessons learned on-site at some of the largest mine disasters since the mid-1940s. This knowledge was gathered so that it could be provided to today's miners and to tomorrow's emergency response personnel. It is expected that the collective wisdom obtained can be used to help train new responders and guide those decisions that will have to be made on the scenes of future events.

Introduction This Instructor’s Copy contains most of the information needed to use the Wearing Hearing Protection Properly 3-D training reel. It offers practical suggestions on how to use the reel as a training aid, performance objectives for deciding if the training is appropriate for your needs, and discussion notes that provide additional information to the user. The 3-D reel was designed as a self-teaching aid that drillers and others who work at a drill site can use wherever the opportunity for such training presents itself. The latter could be a company preshift requirement or a tailgate session where workers would have an opportunity to view and discuss the reel. It could also be an individual looking at the reel in a pickup truck prior to beginning work at a drill site. The 3-D training reel is versatile in that it can be used for a small group of persons or for a single worker. For any of these situations, information is transferred to the individual by viewing each of the seven colorful, in-depth 3-D scenes and relating the words that appear in each scene to what is depicted in the visual. Introduction This Instructor’s Copy contains most of the materials trainers will need in order to use Drill Rig Incident (DRI). It includes suggestions for using the exercise, performance objectives for the training, master answer sheets, a scoring key, and a set of discussion notes that provides additional subject information. Four appendices are also included. Appendix A is the complete exercise problem booklet. It may be reproduced so that every person in the training class has one. Appendix B contains answer sheet blanks. The blanks are furnished if you choose to have the invisible ink answers that appear in appendix C printed on them at your location.3 Answer sheets are consumable. One is needed for each group of three to five persons who work the exercise together. An alternative is for each individual trainee to have his/her own answer sheet. This study examined musculoskeletal injury risk at four mining sites: underground coal, underground limestone, surface copper, and surface phosphate. Each site offered opportunities to investigate musculoskeletal disorder (MSD) injury risks and how those risks might be identified and categorized. The National Institute for Occupational Safety and Health (NIOSH) worked with these sites to (1) identify work activities that showed evidence of MSD injury risk, (2) examine physical risk factors that can lead to MSDs for a handful of work tasks at each site, and (3) develop a set of ideas for problem-solving to help reduce risk factors for examined work tasks. For each site, NIOSH implemented a plan that was refined over the time period of this study. The plan consisted of four steps. The first step was to use mine injury records, a musculoskeletal discomfort questionnaire, front-line supervisor interviews, and a list of management concerns to identify work groups and work activities that have significant evidence of MSD risk factors. The second step was to select work tasks for evaluation. The third step was to interview those who do the work and make observations to characterize the MSD risk factors and musculoskeletal symptoms that exist for target tasks. The final step was to conduct brainstorming sessions with workers who perform the work or have a stake in the production task. The brainstorming sessions served to identify general strategies (ideas) for reducing MSD risk factor exposures. A final report of findings was presented to mine management and workforce representatives at each site. The risk factors and ideas for improvement identified for each site were specific to the target tasks examined. These target tasks were diverse, but there were some key similarities. For instance, jobs were found at each site that required a significant amount of manual work involving the upper extremities and low back. Handling heavy and awkward objects, forceful arm and shoulder exertions, and working in awkward postures were common for a variety of jobs across the four sites.

The analysis will provide the National Institute for Occupational Safety and Health, MSHA, and the mining industry with a better understanding of the causes and hazards associated with mine fires and an increased awareness aimed at preventing and reducing fire hazards. It will also form a basis for future fire research programs.
