This paper deals with the causes and mechanisms of a massive pillar failure in a room-and-pillar mine. The mine employed a nine-entry panel system using supersection technique under a cover ranging from 91 to 168 m (300 to 550 ft). Chain pillar squeeze and large roof-to-floor convergence were first observed at the belt transfer point on the very inby side of the main entries. It propagated out by quickly, and, within 24 hours, it had covered an area approximately 1,250-m- (4,100 ft-) long by 762-m- (2,500-ft-) wide. The failure was caught and stopped by a solid barrier pillar. Using computer modeling, this study indicates that the massive failure was initiated because of a combination of undersized chain pillars in some panels and weak immediate floor. Also, the study shows that the massive pillar failure expanded to a large area because of the absence of barrier pillars between some panels and between mains and panels. Based on computer modeling, alternative mine plans were proposed to avoid such problems.

Federal Regulations require that coal producers conduct a written, in-house investigation with every lost time injury accident. Besides their obvious value in accident prevention, competently conducted investigations can provide a number of additional benefits to include positive supervisor involvement, the control of rising worker's compensation costs and a measure of protection from civil/criminal proceedings. The paper describes such topics as MSHA minimum requirements, investigator selection and training, accident site procedures, witness interview techniques and the management review process. The reader is also presented with a number of information resources available through MSHA, the National Safety Council and other sources.

The topography reconstructed following land disturbance is the foundation for all other reclamation practices. As the manufactured drainage basins, hillslopes, and stream channels approach a steady-state condition, the amount of adjustment of these landforms by geomorphic processes following reclamation decreases. As the adjustment required to establish a steady-state condition decreases, the prospect of reclamation success increases while the prospect of site maintenance decreases. Careful topographic reconstruction is a good investment.

Four methods to assess the relative floatability of fine coal have been tested: flotation with frother only, salt flotation, timed-release analysis, and flotation with frother and collector. A practical and simple criterion that combines the separation efficiency (capacity factor) and kinetic rate constant (intensity factor) has been suggested. Five coal Samples from Utah were tested. Based on test results, it was concluded that the floatability test with collector and frother is closer to practice, and according to the suggested floatability criterion, coal fines can be classified into four categories of relative floatability: very easy, easy, moderate, and difficult.

The determination of the surface area that may be impacted by underground mining is an important consideration in subsidence engineering. A number of parameters have been utilized to identify this influence area, most notably the angles of draw and break. This paper discusses the implications of selecting an appropriate "damage angle" for defining a potential damage area due to mining, within which surface structures may be impacted. Furthermore, the influence of both mining and geological factors (i.e., mining width, depth and overburden lithology) on this angle is examined using the Surface Deformation Prediction System (SDPS) software. Finally, recommendations are given for applying these concepts in subsidence control.

A tunnel boring machine (TBM) was designed and built at the Cold Regions Research and Engineering Laboratory in 1992 for use in Antarctica. The TBM system was tested at the South Pole where a 120-m long tunnel was bored to prove the concept. Modifications were made to the system resulting from this work. In 1999,boring of a 600-mutilidor began as part of the new South Pole Station. Tunneler hydraulics reliability problems prevented the completion of the tunnel before the end of the short construction season. This paper discusses the design and operation of the TBM, proposed solutions to the problems encountered, and current operations of the system.

Introduction Atlas Consolidated Mining and Development Corp. (ACMDC) started gold production at its Masbate gold mine early in 1980. This involved an open-pit mine and a 3.5-k/d (3850-stpd) carbon-in-pulp processing facility (Pizzaro and Antonio, 1981). When this operation started, the mine began stockpiling sub-grade milling material averaging 1 g/t (0.029 oz per st) gold. Furthermore, geologists estimated that there were additional low grade reserves within the ultimate pit limits. These amounted to 2.4 Mt (2.6 million st) with an average grade of 1.1 g/t (0.03 oz per st) gold. The success of low cost heap leaching prompted an investigation to determine if this tech¬nique would be applicable for the low grade Masbate ore. The findings were favorable and a series of laboratory tests were undertaken. These tests indicated the amenability of this subgrade milling ore to heap leaching. Depending on ore size, gold extractions of more than 60% were obtained. To further define operating parameters for a commercial heap leaching plant, a 400-t (440-st) batch pilot plant was built and operated. These test results were used to design the first commercial heap leaching plant in the Philippines. One design concern was the control of solutions during the rainy season. Due to the material's low grade, every effort was made to minimize capital and operating costs. To this end, surplus mining equipment from the copper operations of ACMDC and used machinery in the crushing circuit were considered. Location: The Masbate gold heap leaching operations are located on the northeastern part of Masbate Island, Philippines. This is about 480 km (300 miles) south-east of Manila. Climate: The climate is moderate with two distinct periods. The dry season runs from January through June, and the rainy season extends from July through December. During the wet season, the rainfall at the mine site averages 300 mm/m (11.8 in. per month) for a total of about 2000 mm/year (78 in. per year). However, heavy downpours in excess of 100 mm/h (4 in. per hour) can occur. Geology: Four distinct rock types have been identified in the pit area. They are tuffaceous agglomerates, andesitic agglomerates, metasedimentary rocks, and hornblende augite andesitic porphyry. The tuffaceous agglomerate occupies the northern and western flanks of the mine. Andesitic agglomerates are localized distinctly in the central portion of the volcanic plug within the mine site. The eastern boundary of the mine is occupied by metasedimentary rocks, whereas, the hornblende augite andesitic porphyry occurs as an intrusive within the andesitic agglomerate and tuffaceous agglomerate. Some areas of the pit contain high proportions of clay minerals, requiring an agglomeration step in the flowsheet. This ensures satisfactory percolation through the heaps. The 2014 George A. Fox Conference at the Graduate Center, City University of New York in New York City was presented under the theme, ?21st Century Challenges for the Tunneling Industry. Bruce Grewcock, chief executive officer and president of Peter Kiewit Sons? Inc., opened the conference with a keynote address in which he set the tone for the rest of the day by touching on the many challenges the industry is facing, which are plentiful and complex. Covering the breadth of the industry, he spoke about everything from the technical challenges that come from working beneath the streets of some of the most densely populated regions in the world to the challenges that come from working with the people who live in those areas. ?My observation is that we, as an industry, are being asked to put structures in places where they are needed, but are complex environments to work in,? said Grewcock. ?We work in an industry that relies on some of the most technically challenging engineering in the world, and when we are done with a project, we cover it up and no one ever sees it.? Despite these issues, Grewcock said that, in his opinion, the biggest challenge the industry is facing is that of people; more precisely, where to find the next batch of people. Looking out at the conference-record crowd of more than 400 registered attendees, Grewcock quipped that he saw plenty of gray beards and asked aloud, ?Where will we find, educate and train the people to tackle the issues that this industry will face in the future??

The main train station of Leipzig in Germany is currently being upgraded from a head station to a partially thru station. In order to achieve this, significant ground improvements are necessary. Soil freezing for example is used to create the excavation box retaining walls. The freezing works will be carried out from two tunnels with an inner diameter of 2.4 m (~ 8ft). The tunnels were constructed by the pipe jacking method. The cutter head of the machine was equipped with discs and scrapers. Driving and excavation was carried out using a bentonite supported face. Due to the small arrival shafts, the machine had to be lifted out in components. The essential limitation of settlements during the drives was achieved even better than expected.

Data on dust-control practices, mining conditions and occupational exposures were gathered for approximately 80 underground continuous mining units. Despite silica contents on dust samples in excess of 5%, nearly forty units successfully maintained silica concentrations at or below 100 µg/m3 on a majority of occupational dust samples, while the remainder had difficulty maintaining this level. These two sample sets were termed Group A operations and Group B operations, respectively. This data was compared to identify causes of high occupational silica dust exposures. Analyses of dust-control parameters and geologies revealed only minor differences between these two groups. However, subsequent comparisons of face ventilation design showed considerable differences in silica exposure and silica content between Group A and Group B at the continuous mining machine and roof bolter operator occupations. These differences were minimal when using exhaust curtain ventilation with a dust scrubber. This face ventilation system can benefit operations having difficulty controlling silica dust exposure and silica dust content. The collected data also showed that occupational samples from Group B operations possessed generally higher silica exposures and silica content than similar samples from Group A. The single head roof bolter (helper) possessed among the highest silica exposures and silica contents in both groups.

INTRODUCTION As the experience in Canada indicates, it is becoming more and more common to evaluate major mining projects, especially those relating to uranium, with respect to their environmental effects. As part of this evaluation public hearings are becoming the rule rather than the exception. In this area the 1970's and presumably the 1980's can be characterized as the age of "Environmental Assessments" or "Environmental Impact Statements" which form the basis of these hearings. These documents tend to require significant amounts of research, data collection and effort in their preparation, and tend to be closely scrutinized by the hearing body and by groups in opposition to the project. This opposition is becoming more organized and sophisticated. The assessment or impact document, together with the evidence supporting it during the hearings can: (a) mean the difference between a lengthy and hence costly hearing and a smoother, shorter and less expensive one, (b) result in requirements for extensive modification at the expense of the proponent to his project, (c) result in the proponent being required to carry out extensive studies, research or monitoring programs, and (d) conceivably may in certain situations result in the project actually being turned down. Therefore, while on the one hand it is important to have available technical answers to concerns relating to uranium, its associated and perceived hazards, it is getting more and more important to be able to present these answers in both a written and oral format in such a way as to meet the criticism and concerns of the opposition, be they citizens in general or special interest groups including environmental lobbyists. Shotcrete has been used for approximately 20 years to facilitate tunnel excavation through poor ground. Early examples of such usage in North America include the recuperation of the Granduc Mines access tunnel in Northern British Columbia, and similar work performed in the Carley V. Porter and Big Walker Tunnels in the United States. Mining companies such as Craigmont Mines in B.C. and Hecla Mines have used shotcrete in their workings for the past ten years. Shotcrete is now a widely utilized fool in the mining and tunnel construction industries in North America, but the accumulated experience is too limited for many to fully appreciate the capability of shotcrete to control ground movements and to understand the limitations of this tool. It is common, for instance, to use shotcrete to "bridge the gap" between rock bolt-supported and steel-rib supported ground and to avoid the use of shotcrete in water-bearing ground. Shotcrete can, and has been, used economically and practically under most severe ground conditions.

Surface mining of coal in the South developed into a major industry during the 1960's. The economic significance of the industry is much greater than is readily apparent when one considers only the value of the coal mined. The abun-dant supply of relatively inexpensive steam coal for electric power generation that has been derived from the surface mines has provided the foundation for the industrial expansion that has occurred throughout the South. The region, in gen-eral, has enjoyed the benefits of plentiful low cost electric power. However, re-cent political decisions, at all levels of government, threaten to destroy or severely curtail the surface mining of coal and eliminate the economic advantages once enjoyed by the region. These political decisions are rapidly changing the basic rules of operation and fundamental economics of surface coal mining. Recent changes, resulting from the enactment of environmental and health and safety laws and various bureaucratic rules and regulations, have had a three-fold effect: 1) Greatly increased operating cost, 2) increased lead times for mine development, 3) limited potential reserve base for future mining. Also current legislative proposals are casting a shadow of uncertainty over the future of all coal mining operations that has created serious delays and in-creased cost for the expansion of the coal industry necessary to meet our project independence goals. Technology Surface mining is the safest and most economical way to produce raw materials from the earth's crust. It also affords a more healthier working environment for the individual miner. This paper presents the ventilation design for an underground testing facility in a salt medium. The underground workings consist of two-12 ft. (3.66 m) diameter watertight shafts and approximately 6000 ft. (1830 m) plus of drifting. These facilities are designed for the purpose of providing access for an in-situ testing of a salt horizon in which a variety of hazardous and non-hazardous materials could be temporarily stored or permanently deposited. The ventilation system includes a design basis for gassy mine conditions and provides for the option of bulk air cooling and heating.

INTRODUCTION Today at LKAB, the primary mining method used is sublevel caving, independent of ore-body size, shape, dip, and geographical location. The mining of small ore bodies with relatively flat dip involves a lot of develop¬ment work, a large initial investment, big dilution, and large ore losses. In 1975 serious investigations into other possible methods, especially for those small ore bodies which do not lie in direct contact with other ore bodies, began. Different methods were studied and a number of in¬teresting ideas came from Russian mines. An analysis was made of one of the proposed methods, sublevel benching, in which mining is carried out from raises, and which uses the throw from the blast to place the broken ore. However, there were reservations about working in big open rooms at great depth, and doubts about the possibility of getting acceptable drilling results in an acceptable environment from raised platforms. Testing was therefore delayed for a time. However the investigations continued and in the fall of 1976, it was decided to try vein mining in a small ore body (Indian) (see Figs. 1 and 2). MINING RESULTS The results are now available from the mining of Indian and are as follows: Ore plan area 1200 m2 Level height 50 m Ore volume 60 000 m3 Ore tonnage 270 000 t Iron content 62% Dip 0.69 rad (40°) Ore thickness 14 m Ore compressive strength 26 MPa Ore C-factor (fracture density) 0.21 Hanging wall compressive strength 28 MPa Hanging wall C-factor 0.27 About 40 000 m of the holes were drilled by ring drills with an average productivity of 85 m/shift; loading of a round (2 rows, 750 m of hole) was usually accomplished in one shift. The total ore recovery was about 270 000 t with an average iron content of 59%, and three to four shifts were required per round (4700 t) to load. The principal of "throwing" the rock during blast¬ing worked well, and no ore remains on the footwall. ROCK CONTROL Before mining commenced at Indian, it was dis¬covered that the rock strength in its vicinity was rela¬tively poor. Since the ore is flatly dipping, and mining was to take place with open rooms, the strength of the hanging wall was of great importance. A supplementary program of exploration drilling was done during the fall of 1976 in order to assess fracture spacing and rock strength from the drill cores. Results showed that the strength of the hanging wall was low because of the many fractures. A decision was made to try cable bolting of the hanging wall by extending some of the ordinary blast¬holes. Cables (20 mm diam) were grouted in these larger holes and the rest of the hole was then utilized for ordinary loading (see Fig. 2). About 100 bolts altogether were installed and, to date, only small falls have occurred and the hanging wall is holding. Its exposed area is 2100 m2 DEVELOPMENT WORK On the 350-m level, four crosscuts (two per mining room), one longitudinal drift, and two elongated drifts (one per raise) for the raise climber were developed as shown in Fig. 1. Of the 230 m of drift, 60 m were in ore and the rest in waste. For mining room A alone, a total of 115 m of drift were needed with 30 m in ore. The drift dimensions were about 4.5 X 5.5 m. In the drifting on the 300-m level, two connecting drifts were driven from one existing transport drift forward to the respective raises. The total development was 93 m, all in waste (Fig. 1). The two raises having a length of 80 m each were driven at an angle of 0.68 rad (39°) from the hori¬zontal. They were driven along the footwall contact with a separation of 30 m and had an area of 10 m2. LONG-HOLE DRILLING The drilling of long holes with the ring drilling unit began in the fall of 1977 in raise A. The equipment consisted of an Alimak raise climber, equipped with a lower platform from which the charging was done.

Use of geostatistics to solve mining problems is gaining importance as the cost of developing new mines is increasing and there is a need to mine more marginal deposits. But, until recently, coal companies have shown little or no interest in geostatistics. However, the stringent quality controls on coal burning has led to a surge in interest in improved methods for estimating clean coal characteristics. Until now, most of the geostatistical applications have centered on the problems of estimating in-situ coal characteristics (Sabourin, 1975; Wood, 1976, 1979; Fang and Stark, 1976; Pauncz and Nixon, 1979; and Knudsen et al., 1980). Little work has been done in applying geostatistics to coal washability, except that by Armstrong (1980). Applying geostatistics to coal washability basically means the estimation of cleaned coal characteristics, with the aid of geostatistics. Cleaned coal estimation plays a very important role in overall emission control strategy, as can be seen by the general flow diagram of such a strategy shown in Figure 1. The scope of this paper is limited to one aspect of the overall emission control strategy shown in Figure 1; namely, the prediction of cleaned coal characteristics using geostatistics.

It had been more than a decade since this many booths were occupied at an SME Annual Meeting, but at the 2010 meeting in Phoenix, AZ 630 booths were sold to 470 vendors from the entire spectrum of the mining and minerals industry. Exhibitors such as Metso, FLSmidth, West-Tech, ABB, NIOSH, Eriez and Resource West took their usual prominent places on the exhibit floor and were joined by newcomers such as tunnel boring machine manufacturer Herrenknecht AG, Frontier Kemper, the International Society of Explosive Engineers and more than 100 other first-time exhibitors. On the floor, one could find everything from a new pair of boots to a new career in the recruitment pavilion where some of the biggest companies in mining (Barrick Gold, Freeport-McMoRan, Newmont Mining, etc..) set up shop. The following is a look at some of the companies and products that were on display at the 2010 meeting. Rock bolting as a method of support continues to increase with applications in many other industries in addition to mining. Nevertheless, with nearly 55,000,000 roof bolts installed in coal mines alone last year, this remains as the single greatest use. While bolts have frequently supported ground where conventional timbering could not, there are relatively few design criteria; and trial-and-error procedures prevail. Furthermore, development of suitable instrumentation that is simple to install and readout, sensitive, durable, reliable, safe and economical in evaluating the effectiveness of a bolt over long periods of time lags behind. Therefore, the pull test continues to be the most popular method of evaluating the applicability of a certain type of roof bolt under specific installation conditions. At Penn State in the Department of Mining, research has been conducted for a number of years to measure bleedoff in carefully controlled laboratory experiments as well as in underground investigations.1,2,3,4 Unfortunately, most of the instrumentation developed has been primarily suitable only for research purposes, not possessing all of the aforementioned characteristics desirable for routine underground use. Other groups also have met with restricted success. Therefore, while relatively crude, the torque wrench continues to remain as the most widely used load measuring device. While both field and laboratory tests continue to be conducted, analytical analyses are attempted to discover the more important design parameters in order that more efficient anchorage might be devised.

The Greenwich Collieries Co. is a subsidiary of the Pennsylvania Mines Corporation, and a wholly owned subsidiary of the PP&L Co. which serves the northeastern portion of Pa. PP&L annually consumes about 11 million tons of coal per year and is the largest consumer of both bituminous and anthracite coal on the eastern seaboard. Development work for Greenwich started in 1969. The coarse coal plant was constructed in 1970, with the raw fines bypassed. This worked very well for our requirements at that time; however, the Greenwich mining plans were extensively changed, longwall mining units were installed and low coal was encountered, resulting in dirtier coal, a finer coal and an ever increasing ash in the shipped product. All this demanded a more efficient ash removal process for the fine coal fraction. In 1971, the Pennsylvania Department of Environmental Resources and the Federal Environmental Protection Agency were pushing for the now stringent sulphur emission limits at our power plants. The Greenwich Mine coal reserves contained large quantities of 3.5 to 4% sulphur along with some low sulphur reserves. It was, therefore, essential that any new plant have the capability of efficient sulphur removal to meet the Pennsylvania Department of Environmental Resources and the Federal Environmental Protection Agency required limits. To meet these limits our design would have to be capable of producing a product of 2.0% sulphur or less.

Discussion by E.J. Garrison A.C. Noble presents a clear and concise summary of the factors pertinent to a competent reserve evaluation of a mineral occurrence. However, I believe that he confuses the difference between a resource and a reserve. By definition, a reserve estimate includes all known economic, legal, mining, metallurgical and environmental factors impacting the recovery of the product(s) found in the deposit. A resource estimate, however, is an estimate of what is there - what is available for exploitation and should not include economic and other considerations. Only after defining the resource, its extent, location, physical and chemical properties, can an intelligent selection process begin of the extraction and processing technologies most appropriate to the circumstances encountered. The early and almost certainly inappropriate application of these often cookbook choices has probably lead many companies to walk away from good properties. (How many mines have been found in others rejects?) An example of the process is the early selection of metallurgical samples, after encouraging values are found in a few holes or surface samples, in an attempt to determine the economics of the prospect. Unfortunately, the sampler does not know how representative of the occurrence the sample is because he does not know what the deposit looks like. As a consequence, the property becomes saddled with a bad metallurgical sample that is often inappropriately used in evaluating the deposit which it does not represent. Reply by A.C. Noble E.J. Garrison asserts that I confuse the difference between a resource and an ore reserve. In fact, this paper was motivated by the observation that resources and reserves are almost universally confused throughout the industry. While Garrison is concerned that too-early consideration of engineering, metallurgical and economic factors will result in dropping properties too soon, some issues that must be considered include: •The purpose of exploration is to discover ore reserves, not to discover resources. •Ore reserves are always smaller than resources. • Decisions on exploration expenditures must be made based on the expectation of the quantity of ore reserves that will ultimately be defined. Noble's Fig. 2 can be used to give guidance as to when it is appropriate to begin reserve calculations. When exploration enters the "flat" portion of the curve, representative samples can be selected for metallurgical studies and appropriate mining methods chosen. It is at this stage that we know enough about the deposit to make intelligent choices of representative samples and recognize constraints on mining caused by the deposits shape, environment and physical properties. Once the appropriate technologies have been chosen, then their cost can be estimated with reasonable accuracy. Early entry of economic and process factors gives the false appearance of reserve status to the resource estimation. It would be more informative and less subject to abuse and misunderstanding if resource calculations were made at a number of cutoff grades. The cutoff grades should be chosen to cover the range of grades found in practice in existing operations of similar type. This would emphasize the open nature of resource estimates and encourage creative evaluation of mining and process technologies. It does not preclude the use of economic, environmental and recovery technology factors in deciding the advisability of continuing exploration expenditures, but would highlight their speculative nature. It is thus hoped that decisions on the future of the deposit would not be made on the presumed suitability of a given technology but on the actually known reality. I agree that resource estimates should be reported as a range of cutoff grades. Since reporting at a cutoff implies application of underlying economics (even by analogy to similar properties), however, it should be clearly stated that resources do not consider economic, mining or cost factors, and that ore reserves will be substantially less. The dilemma of the explorationist is that until the continuity and limits of ore-grade mineralization have been established, it is difficult to make quantitative estimates of either resources or ore reserves. Further industry discussion is needed regarding standards for disclosing early exploration information in such a way as to fairly represent the potential of an exploration property.
