Falls of ground continue to be one of the most serious causes of injury to U.S. miners. Of the 256 fatal injuries that occurred in mining between 1996 and 1998, 59 (23%) were caused by falls of ground (Table 23.1). Falls of ground affect some sectors of the mining industry more severely than others. For instance, nearly 40% of the 98 coal mine fatalities between 1996 and 1998 were caused by falls of ground. Underground miners are at much greater risk than surface miners. Nearly half (45 out of 101) of underground mine fatalities were attributed to roof, rib, and face falls, while only 6% of the 155 surface fatalities were caused by falls of highwalls or slopes. The goal of this chapter is to provide guidance to safety professionals tasked with preventing ground fall injuries. This chapter combines an analysis of the Mine Safety and Health Administration's (MS HA) accident and injury data with a survey of industry "best practices" to safeguard miners from ground falls. Ultimately, this approach can help to form the basis of a sound, proactive ground control program for the mining industry. SOURCES OF DATA All the injury data examined in this study were derived from MSHA's fatal investigation reports and the MSHA accident database. Because falls of ground often result in serious injury, MSHA "Fatalgrams" and fatal investigation reports provide a useful snapshot of ground control issues in the mining industry. These reports are available to the public (on the M FHA Web site at www.msha.gov). Fatalgrams, are one-page summaries that are usually published within a month of an accident. They contain very basic information about the accident with a graphic and a short section on relevant best practices. Fatal investigation reports are the official accident investigation reports filed by MSHA personnel. These reports contain general information about the mine, a, description of the accident, physical factors involved in the accident, a conclusion, and enforcement actions, Enforcement actions typically identify citations and discuss violations to the Federal Mining Law. MSHA also maintains comprehensive statistical data on the mining industry's accident and injury record. The law requires that mines file a report on every reportable accident that occurs, containing information on the accident's location, severity, classification, activity, and nature of injury A short narrative is generally included as well. Accident reports can be searched by many of the above fields.

"One phase of the Bureau of Mines' work in the field of health and safety is the promotion of the use of safe, satisfactory, and reliable respiratory protective devices. To promote the development of such devices the Bureau has established schedules of minimum requirements for oxygen breathing apparatus3/ gas masks/, supplied-air respirators5/ (hose masks, airline respirators, and abrasive blasting helmets, hoods, or masks), and mechanical-filter respirators6/.Manufacturers of respiratory protective devices may submit their equipment to the Bureau for approval under the requirements of these schedules.As a prerequisite to such submittal, the Bureau requires a statement that the device is completely developed and of the design and material the manufacturer believes suitable for a finished marketable device; that the device has been subjected to inspections and tests such as described in the schedules, and that it met those requirements when tested by the manufacturer or his testing agency; and a. statement regarding the nature, adequacy, and continuity of control of the quality of the respirator. If the manufacturer can satisfy these requirements, and if the device, when tested by the Bureau, meets its requirements, a certificate of approval is granted by the Bureau of Mines. The approved devices bear distinguishing marks that indicate that they have been approved, the purpose for which they are approved, and the limitations of their use. If it is found at any time that approved devices purchased in the oven market and tested do not meet the requirements of the schedule, the Bureau may rescind that approval. Lists7/ of approved devices are published periodically for the convenient information of the public.Respiratory equipment may be divided into two main groups: (1). Those that afford protection by removing atmospheric contaminants and (2) those by which a respirable atmosphere from an uncontaminated source is supplied to the wearer.The devices that protect by the removal of contaminants may be divided into two subgroups: (1) Those that protect against gaseous contaminants and (2) those that protect against particulate matter. There may, of course, be combinations of these two.The devices by which a respirable atmosphere is furnished to the wearer also may be divided into two subgroups: (1) The self-contained type, by which the supply of oxygen, or material from which the oxygen can be generated is carried by the wearer, and (2) the hose type, by which air is supplied to the wearer through a hose or tube."

This chapter explores leadership behavior in a life-threatening situation-fire in a coal mine. Previous chapters have discussed the database of interviews with miners who escaped from underground fires. Researchers raised questions such as: (1) Who led the miners out of the mine? (2) Did leadership make a difference in the escapes? (3) Was the escape leader the hierarchal leader? (4) What, if any, characteristics did the escape leaders possess? Subsequently, researchers analyzed the mine fire database from a group interaction perspective to address the leadership questions posed. Leadership has been one of the most researched topics of human behavior in the twentieth century. Studies have ranged from individual characteristics of leaders, to situational leadership, to interaction of leader and follower, suggesting different leadership techniques for different followers. The question that emerges here is: Are there different types of leadership that "fit" different kinds of situations? In a crisis situation like that examined in the present study, such information about leadership may significantly improve the chances of escape. To address these issues, the study team looked at the formal authority structure before each fire, considered leadership behavior or lack of leadership during the escapes, and examined those conditions associated with the emergence of leadership. According to Bardo [ 1978], "Emergent behaviors are those forms of action, and the norms, values and beliefs governing those actions, that rise out of the disaster situation." This chapter discusses previous studies in the area of crisis leadership and examines the emergent behaviors of leaders under duress during the mine fire escapes. Previous Studies The research on leadership during emergency situations has consisted mainly of simulation and field studies, with the principal concern being escape from building fires. During the 1980s, Hayashi [1988] created a computer simulation model to evaluate leader behavior in a fire. Although his purpose was to aid in planning for disaster prevention, his findings are relevant because they address the issue of situational leadership in crisis-where a leader changes his or her behavior to fit the situation. Essentially, his simulation model was designed to judge the actions and thinking of leaders. The simulation was tried by 101 subject/leaders 4 times each. The simulation consisted of amaze containing the leader, an informal leader, and 50 evacuees. Interestingly, the results indicated that the leader's actions were not dictated by circumstances. Any differences in behavior were attributed to the individual characteristics of each leader. The study also showed that the worse the situation became, the less individual

"Haulage accidents usually are responsible for about 17 or 18 percent of all coal-mine fatalities and about 21 percent of all non-fatal coal-mine accidents. When the relatively small number of employees continuously exposed to haulage hazards is considered, it would appear that haulage is more hazardous than any other phase of coal-mine operation.Haulage accidents have increased from year to year, doubtless due to the increase in electrical haulage, and particularly to the use of larger cars, heavier locomotives, longer hauls, and greater speed. Unless very definite steps are taken to eliminate the causes of these accidents, they will continue to increase rather than be reduced, as they can and should be.Underground haulage in many of the large modern mines of today is similar in many respects to railroad practice. In recent years the railroads have made wonderful strides in reducing accidents, and instead. of being one of the most hazardous occupations, as it once was, railroading is now one of the safest. Doubtless, if thought and action, similar to that given to reducing accidents among railroad passengers and employees, were given to safety in mine haulage, under¬ground haulage accidents could be reduced in about the same ratio.Haulage accidents may be classified, according to responsibility, into two groups - those for which management is largely responsible and those for which the employee is largely to blame; there is, however, an overlapping of responsibility from one group to the other. If, therefore, haulage accidents are to be reduced, it is absolutely essential that both management and employees cooperate to this end.In the first group, for which management is largely responsible, may be included accidents due to faulty equipment, moor track, a poorly arranged system of haulage, lack of clearance or of overhead protection, and dirty roadways; even in these features, however, the employee is not without a measure of responsibility, as failure of haulage equipment possibly may be due to lack of care on his part, failure to report when equipment is out of order, or failure to do a good workmanlike job of installing or repairing or operating.To the second group of accidents for which the employee may be largely responsible, belong those caused by such careless practices as jumping on and off fast-moving trips, coupling on the ""fly"", running at excessive speeds, failure to properly block cars, and similar hazardous and unnecessary acts of omission or commission. If, however, management condones such practices, or, as in some instances, insists upon them, it must accept its share of responsibility for the accidents that inevitably result.Some of the most prolific sources of haulage accidents will be discussed briefly, together with the steps that may be taken to minimize their occurrence."

This chapter focuses on smoke as it relates to escape from underground mine fires. Among the topics discussed are the measurement of visibility in smoke; smoke-related hazards such as the production of CO, hydrochloric acid (HCI), or other byproducts of combustion; and miners' personal experiences while escaping through smoke. Smoke Measurement and Visibility In general, smoke consists of hundreds of thousands of very small particles. These particles have some "size," usually expressed in terms of their diameters, and they have some concentrations, usually expressed either in the number of these particles per unit volume or the total mass of the particles per unit volume. Humans cannot see individual smoke particles because they are too small. Similarly, "umber concentrations" and "mass concentrations" of smoke particles do not have much meaning to people unless they are trained technically. Still, individuals know that they can see smoke, independent of all the technical jargon used to describe it. Also, they know that when the smoke level gets too high, it is no longer visible. In fact, nothing is visible because the smoke absorbs all of the light in its surroundings. The eye is only sensitive to light in the wavelength region from about 400 nm to about 700 nm. The maximum sensitivity of the human eye is to light that has a wavelength of about 555 nm. It is important to know how the eye responds to light because if its response is known, it is possible to use a light detector that has almost the same response as the eye. Such a detector can then be used to quantify the visible characteristics of smoke because it responds in the same manner as the human eye. Smoke is visible because it either scatters or attenuates (diminishes) light. In some instances, smoke is visible because the smoke particles reflect light which is then detected by the eye. The eye actually "sees" an intensity of light that has been reflected from a cloud of smoke particles. Imagine shining a flashlight into a cloud of smoke. Someone off to the side can actually "see" the beam of light as it traverses the smoke cloud. This is called scattering. Smoke is also visible be¬cause it attenuates light. Imagine having someone shine a flashlight into your eyes. As smoke begins to build up along the beam of the flashlight, the light begins to dim. The smoke is visible because it is now reducing the intensity of light that falls upon the eye. As the smoke level increases, it is said to obscure our visibility. When the beam is no longer visible, the smoke obscuration is said to be 100%. In other words, none of the light energy from the flashlight makes its way through the cloud of smoke. Another way of saying that the obscuration is 100% is to say that the transmission of light through the cloud is zero.

Over the past two decades, there have been significant reductions in industrial exposures to hand-arm vibration, especially when specific tools and work processes have been redesigned to incorporate anti-vibration and ergonomic principles. Nevertheless, Hand-Arm Vibration Syndrome (HAVS) remains a significant occupational health problem as disease symptoms continue to occur even when vibration exposure levels believed to incur low risks have been reached2. This inconsistency may be related to the methodology that is typically used to estimate workday vibration exposure levels, involving laboratory and/or very short duration field measurements coupled with estimates of overall eight-hour tool operation times determined from brief observations of tool tasks and/or self-reported surveys. One solution is to use small, commercially-available, personal vibration dosimeters to calculate, record, and display long-duration vector sums and energy equivalents of vibration. However, since these devices are attached to the worker and require tool-mounted accelerometers, they are incompatible with the worker performing normal duties involving putting down or changing tools. In addition, these commercial systems do not allow for the characterization of the transmission of vibration to the hand such as monitoring the mechanical coupling between the hand and the tool handle (e.g., grip forces). O’Boyle and Griffin showed that variations in applied force can alter vibration transmission characteristics by 50% or more indicating that the measurement of grip force is essential for modeling vibration transmissibility and vibration exposures5. In summary, a need exists for the development of a method and device that will more accurately characterize workday-long vibration exposures. Methods A portable, light-weight, Vibration Exposure Monitor (VEM) system was developed to record user-specific tool-operating times, vibrations, and grip forces throughout all, or a representative part, of a workday. It monitors frequency weighted and unweighted accelerations from a palm-mounted uni-axial accelerometer (Model 352C22, PCB Piezotronics, Depew, NY) and calculates exposure levels using the root-mean-square (RMS) and higher power mean values, such as the root-mean-quad (RMQ) and the root-mean-oct (RMO). Grip forces are also monitored using a palm-mounted force sensor (Model 400, Interlink Electronics, Camarillo, CA) from which average grip force levels and exerted grip extrema are calculated. At the core of the VEM system is a commercially available, battery-powered microcomputer (Tattletale, Model 8v2, Onset Computer, Onset, MA) with one megabyte of memory and eight analog channels using 12-bit sampling at a single-channel maximum of 100 kHz. Analog signal processing (i.e., anti-aliasing, with cutoffs at 4 and 1250 Hz, and ISO 5349-13 frequency weighting) is accomplished using custom circuitry that is directly interfaced with the microcomputer. "Information gathered during recent months by field men of the U. S. Bureau of Mines shows that in one State alone there have been 71 fatalities from electrical causes during the last 51/2 years, of which 55 were from contact with mine trolley wires operated at 250 volts or less, direct current. The 1928 law relating to the guarding of trolley wires in the-mines of this particular State reads: ""All trolley wires carrying a voltage of class 2 must be properly shielded except where the same are at least 61/2 feet above top of rail."" Class 2 voltage is defined as being ""between 300 volts and 600 volts direct current, or 240 volts and 480 volts alternating current."" From this it may be inferred that no guards are required, for 250-volt trolley wires, and the natural conclusion to be drawn is that the lack of guards or shields must be held responsible for these fatalities. To prevent fatalities from this source, it has been advocated that every foot of trolley wire in mines should be guarded. While some such drastic step may appear to be amply justified, a closer examination of the records shows that lack of guards is not entirely responsible but rather that carelessness and neglect in the observance of certain well-established simple rules concerning installation and location of trolley wires must also bear a share in the responsibility. In -other words, if trolley wire is installed carelessly and its maintenance neglected, failure may be expected of any guards that have been put up.There are a-number of factors affecting safety as related to trolley wires in mines, among which the following may be enumerated.1. Installation relative to track and passageways.2. Guarding.3. Maintenance.4. Discipline.Acknowledgment is made to D. Harrington and K. L. Marshall of the safety division of the bureau for assistance given in securing data on cost of installing guards for trolley wires.The statement that fatalities should not be charged in every case to lack of guards is supported by the following example: A man operating a locomotive was electrocuted when the trolley wheel jumped the 250-volt trolley wire and allowed it to sag against his shoulder. He was sitting in the regular place provided in the locomotive, but the wire was 11 inches inside of the rail and only 39 inches above it where the wire touched him, or 11 inches from the roof at a distance of 12 feet from the nearest hanger. The accident occurred between two hangers 44 feet apart, and the trolley wire was inside the rail over the entire distance, being 2 inches inside at one hanger and 10 inches inside at the other hanger. A bolt in the roof indicated that there had been a hanger at one time between the two on the 44-foot span.This example is a violation of good practice in the installation and maintenance of trolley wire. In the first place the wire should have been outside instead of inside the rail. In the second place there should have been at least one intermediate hanger so that the span would not be so great and the sag would be reduced. Furthermore, it is possible that the wire was not taut enough even when the intermediate hanger was in place.To provide a degree of safety for men in the mines certain coal-mining States, recognizing the need, have enacted laws to regulate the installation of trolley wires. These limit the distance that the wire must be kept outside and above the rail. Thus, the 1927 mining laws of Ohio specify that:"

It was recognized early that a list of petroleum and natural gas publications was needed for the Bureau's work, and unofficial publication of a ?straight forward? listing was begun. It was at first a chronological list; however, suggestions submitted by its users prompted a format change to show publications under broad, general topics. Author and subject indexes were developed, becoming more detailed with each new edition. This present list supersedes the ones compiled in 1949, 1952, and 1957, and several supplements, which were aside available to the public by the Library Of the Bureau?s Bartlesville Petroleum Research Center. The unofficial field publication came to be widely known nail in considerable demand by those who are primarily interested in the Bureau's publications on petroleum and natural gas so that it became, desirable to have it published as an Information Circular. It is hoped drat, this official publications will result in a wider and more effective use of the Bureau's research results which are reported in its publications.

The Bureau of Mines, in its investigations of respiratory apparatus for use in mines and allied industries; tests equipment that is voluntarily submitted by manufacturers in accordance with published schedules of tests. These tests are designed to insure that the equipment has the minimum requirements for safety in use. Approval certificates are issued for apperatus that meets the bureau's specifications, and the manufacturer is permitted to mark his equiment with a seal showing that it has been approved as "Permissible." A complete list of per- missible self-contained oxygen breathing apparatus, gas masks, and hose masks tested prior to January 1, 1930, was published in Bureau of Mines Information Circular 62309. The present list includes all equipment tested and approved prior to July 1, 1930.

Experimental modifications have been made by the Bureau of Mines on standard pneumatic rock drills to reduce the noise of the air exhaust, drill steel resonance noise, and noise radiated by the drill body. A close-fitting case and muffler around the drill body, consisting of a metallic honeycomb skeleton filled with viscoelastic absorber on the inside and a durable outer shell, provide both exhaust and drill body noise muff ling and absorption. Drill steel resonance noise is reduced by a constrained-layer treatment consisting of a tubular metal cover bonded to the ouside of the sod by a viscoelastic filler. Damping alloy components have been developed to reduce metallic resonance noise. Combined, these modifications have reduced the drilling noise level in granite from 115 dbA to 97 dbA. Data are presented on the individual and combined effects of these modifications on drilling noise and performance.

"The town of Ajo is built in the desert and consisted of but a few houses when the New Cornelia Copper Co. started operations. Since there were no stores at Ajo or vicinity the New Cornelia Cooperative Mercantile Co. was established to serve the needs of the employees of the New Cornelia and allied companies.The New Cornelia Copper Co. advanced the money to erect the store building and to buy the stock, and assumes all risks. In all, $85,000 has been advanced, representing the capital stock of the enterprise.The store is run by a store manager appointed by the Copper Company and who reports only to the general superintendent. A committee of seven workmen, representing the various depart¬ments, meet with the store manager monthly and make suggestions for the betterment of the service and, present complaints which have been brought to their attention.The store is well managed, carries a large and well assorted stock and is modern in every respect. Prices are at the same level as in the other Southwestern mining camps. The Copper Company does not particpate in the profits of the store but does receive six per cent interest on the capital advanced. At the end of the year, after making proper deductions for interest on capital account, depreciation of store equipment and fixtures, and, for a proper sum to the surplus account, the rest of the profits is distributed among the employees of the company in proportion to the amount of their purchases."

Recently 30 men wore viewing the demonstration of a now portable electric arc-welding outfit. A few hours later 17 of the 30 men reported to the doctor for treatment. They were suffering from traumatic conjunctivitis. In two cases the gain was very severe and the symptoms were similar to those of iritis. orphine had to be administered to afford relief from pain. Only two men of the 30 wera not affected in some way from this exposure. These two men were thick-lensed orange-colored glasses. Several of the men wore orange-colored glasses with thin lenses, but the latter were not heavy enough to afford protection.against an exposure as long as took place. The distance of the eye from the are also in- fluences the possibility of injury. Conjunctivitis is an inflammation of the con- junctiva; the conjunctiva is the mucous membrane covering the inside of the eye- lids and part of the eyeball. Traumatic conjunctivitis is caused by foreign bodies in the eye, exposure of the eyes to high winds, dust, smoke, intense light from electric arc lamps, and from electric welding apparatus. In the instance mentioned above, the inflammation was due to the ultra-violet rays. The ultra-violet rays lie beyond the violet rays in the spectrum and are invisible to the eye. In some cases the effect is so severe that, in addition to conjunctivitis, an inflammation of the skin similar to sunburn is produced. The symptons of conjunctivitis caused by intense light or by the ultra- violet rays are marked photophobia (abnormal intolerance to light), excessive secretion of tears, intense starting of the lid, contraction of the pupil, some- times swelling of the lid, and small ulcers developing on the eyeball or cornea. Unless properly treated by a physician immediately, chronic inflammation of the conjunctiva, cornea, iris, or retina, and possibly blindness, may result. Under proper treatment most cases get wall in a few days. All treatments should be under the direction of a physician. That usually advised is to place ice packs on the patient's eyes three or four times daily. The pack should be left on from 15 minutes to an hour. The eyes should be irrigated with normal salt solution (a teaspoonful to a quart of sterile vater) or a saturated solution of boric acid several times daily. If there is a discharge of pus, a few drops of a 25 per cent solution of argyrol or a 5 per cent solution of protargol should bo placed in the eyes three to six times daily. The patient should be confined to a darkened room until his condition improves in order to avoid complications. These treatments will reduce the swelling, give the patient confort, and prevent the development of chronic conjunctivitis. In severe cases it may be necessary to administer morphine to relieve the pain.

"ObjectiveDevelop a portable emissions measurement apparatus to sample and measure tailpipe concentrations of diesel particulate matter (DPM), carbon monoxide (CO), carbon dioxide (COJ, nitric oxide (NO), and nitrogen dioxide (NOJ under steady-state engine load and speed conditions during a 1-minute test.BackgroundThe U.S. Bureau of Mines (USBM) has a comprehensive research program to develop ways to reduce exposure of miners to diesel exhaust pollutants. Engine maintenance has been shown to have important effects on such exposure.Accurate measurements of tailpipe emissions are becoming an essential part of a good maintenance program for underground mines that use diesel-powered vehicles. Past studies of particulates in U.S. underground coal mines have shown that DPM alone may be present at concentrations greater than 1 milligram per cubic meter (mg/m3), half the 2-mg/m3 limit for respirable coal mine dust. A poorly tuned (smoking) diesel engine may be the sole cause for respirable dust concentrations exceeding the limit. Although there is currently no standard specifically for DPM in U.S. mines, regulatory steps are underway, and significant reductions of DPM levels in both coal and metal mines will most likely be required.Tailpipe emission measurements can provide diagnostic information to identify engine malfunctions such as clogged air cleaners, faulty injectors, or incorrect injection timing. In some cases, mine vehicle tailpipe DPM concentrations have been reduced up to 50 percent by engine maintenance and adjustments.An accurate measuring instrument would also be useful for determining the effects of exhaust control devices on tailpipe emissions. Little information is currently available about the overall effectiveness of exhaust controls after long-term use. Raw exhaust concentrations with and without the control device can be measured in underground mines, and from these data a control efficiency can be calculated. Knowledge of the effectiveness of exhaust control devices over time is needed to make intelligent decisions on bow to use and maintain these devices, and when to replace them."

"The Generic Mineral Technology Center on Respirable Dust was established in August 1983, at the request of the Bureau of Mines, to conduct research that affords ""...each miner the opportunity to work underground during the period of (his/her) adult working life without incurring any disability from pneumoconiosis or any other occupational disease"". The Center consists of the Mining and Mineral Resources Research Institutes (MRI) of The Pennsylvania State University (PSU) and West Virginia University (WVU) in association with the MRIs of the Massachusetts Institute of Technology (MIT) and the University of Minnesota (UMN). (FIGURE 34) Michigan Technological University (MTU) is also affiliated with the Center in the area of diesel particulates.The Center brings together experts concerned with particles causing potentially disabling or fatal diseases including pneumoconiosis (""black-lung""), silicosis, and asbestosis. The latter is of deep concern not just to workers in the mineral sector of the economy but also to the general populace. The primary goal of the Center is to reduce the incidence and severity of respirable dust disease through advancing fundamental understanding of all aspects of respirable dust associated with mining and milling, and the interaction of dust and lungs. The Bureau of Mines has improved a mine dust incombustibles analyzer originally developed at the Bartlesville Energy Research Center (BERC). This analyzer provides quick, accurate analysis of the percentage of incombustible material present in dust samples taken from mine ribs, roof, and floor so that costly overdusting may be eliminated while still maintaining a proper level of incombustibles. The changes being effected on the present design of the BERC model IV rock dust analyzer should simplify its operation and calibration procedures, resulting in increased accuracy.

State governments are increasing their efforts each year toward promoting the safety of miners .. Accidents underground are being avoided and mines made safer by means of revising and adding to their mining laws , by enforcing them through mine inspection departments , and by hearty co - operation with other agencies working miners ' safety . The U. S. S. Bureau of Mines as " permissible " for use in gaseous mines ; it is accordingly reproduced in part : "hen during my first inspections of several coal mines in the district I found gas , or fire- darp being given off more or less constantly from the freshly cut coal at numerous points throughout the mines , I became uneasy relative to safety conditions , and am still considering your safety from many angles , but I am more concerned about the gas given off and the coal dust produced daily, than any other factor at the present time .

As here tofore lime manufacturers are invited to cooperate to the fullest extent by supplementing the Bureau's information with the results of their own experiences and observations . Information is presented on remote sealing of a mine passageway through a borehole by five methods : PPnneeuummaattiicc injection of mineral wool and sand, ureaformaldehyde foam , balloons , urethane foam, and caving by blasting . An adaptation of the method using pneumatically conveyed mineral wool was tried in an operating mine . Further research is needed to simplify and to improve the techniques involved , as well as to explore the possibilities of other methods and combinations .
