This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. Keywords: Pyrometallurgy, circular furnace, PGM, platinum, Peirce-Smith converter, Zimbabwe The Zimplats Smelter, located on the northern part of the Great Dyke, at the Selous Metallurgical Complex, is the only PGM smelter in Zimbabwe. Zimplats? operations currently consist of the Ngezi Platinum Mine, which is a 1.5 Mt/a open-cast and 0.7 Mt/a underground mine, and a Concentrator and Smelter to which ore is transported 80 km by road. The final converter matte product is transported to Impala Refining Services in South Africa for refining and marketing. The Zimplats Smelter operations consist of a concentrate filtration plant, a Drytech flash dryer, a modified Elkem / Hatch 13.5 MVA circular furnace, two 10? x 15? Peirce-Smith converters, and a matte granulation plant. The Zimplats smelter uses a three-field electrostatic precipitator, supplied by ELB-Brandt, for removal of particulates in the gas streams from both the converter and furnace. Construction of the smelter commenced in 1995, under the BHP / Delta Gold of Australia joint venture. The furnace was lined mid-1996, and smelting operations commenced in 1997. Due to design challenges, the Elkem furnace was modified by Hatch in 1998, and the smelter had not reached full capacity when operations were suspended in 1999. Re-commissioning took place in January 2002.

A mining system consists of many sub-systems such as drilling, blasting, loading, hauling, ventilation, hoisting and supporting. During mining operation, these sub-systems may experience various problems that stop the operation because of possible environmental equipment and safety issues. In order to ensure delivery contracts in the required quality and safe mining medium, the operation should be, at least, performed in the specified reliability level of the system. If the system reliability decreases below the specified level, there will be safety and financial losses for the mining company. Therefore, the mine should be maintained by a reconstruction procedure to guarantee the operation continuity. Given that each sub-system has a different reliability function and maintenance cost, the determination of reconstruction time will be a complicated decision making problem. In this paper, the determination of reconstruction time is formulated as a nonlinear optimization problem and solved by genetic algorithms (GA). A case study was conducted to demonstrate the performance of the approach for an underground operation. The results showed that the approach could be used to determine the best action time. Uranium exploration over a wide area of the Southern Frame Embayment, South Australia, has defined a number of Lower Tertiary fluvial palaeochannels incised in older rocks. The buried channels contain similar stratigraphicsequences of interbedded sand, silt, and clay, probably derived from the adjacent uranium-rich Olarv Provinee. Uranium mineralization is pervasive within two major palaeochannels, and four small uranium deposits have been found in the basal sands of these channel sequences, at the margins of extensive tongues of limonitic sand.One of the channels is examined in detail with respect to channel morphology, stratigraphy, and rock alteration and the geology and ore controls of the contained uranium deposits are described.A geochemical zonation reflected by variation in limonite, pyrite, carbon, and humic malleI' is defined across one of the deposits. A genetic model is proposed suggesting formation by a uraniferous geochemical cell which migrated down the stream gradient and concentrated uranium on its lateral margins adjacent to the channel bank.

In 1998, the Concentrator Modeling Center was established within the Coleraine Minerals Research Lab to develop models for simulation of iron ore processing plants, and to assist plant operators in improving performance of their aging plants. The short-term objective was to develop necessary models for reliable simulation using conventional techniques, while the long-term objective was set as liberation-based modeling and simulation of these plants. The short-term objective was achieved in 2002, after the development of models for magnetic separators, hydroseparators and fine screens. A project funded the US Department of Energy was carried out to demonstrate the capability in improving iron ore processing at the Mittal plant. As a result of this study, a 10% improvement in throughput at the plant was obtained, and simulation became a desired tool for plant operators. A significant step in the long-term objective was reached in 2005 with the development of a liberation model. This was followed by incorporation of models capable of processing liberation data into the software. By early 2007, the Center had the capability of performing liberation-based simulation of entire plants.

"This paper describes the modifications made to the Fox Mine copper concentrate bin in order to obtain a continuous flow condition. The design of these modifications is based on test results obtained on Fox Mine concentrate using the flow factor testing machine in the research facilities of Cominco at Trail, B.C.The original design consisted of a cylindrical steel bin on a concrete slab with two slot-discharge openings. Based on predictions of the maximum mass flow hopper angle for conical and wedge shaped hoppers for the concentrate on various potential bin lining materials, Ultra-High Molecular Weight (UHMW) Polymer was selected as a liner.Using the recommended angles for conical and wedge shaped hoppers, the profile of the bin base was changed to suit the existing slot openings.The initial method of fastening the liner resulted in failure of the connections. Subsequent modification to the zinc concentrate bin fastener system proved satisfactory and it has now been in operation for over one year without any separation of the lining material.Utilization of flow testing procedures on concentrate samples has demonstrated that proper design of storage bins or hoppers to achieve continuous flow can be incorporated into the design of new plants."

Evidence of hydrothermal activity on two Mid-Atlantic Ridge oceanic core complexes (OCCs), at 13º30’N (the Semyenov 1 through 5 fields) and 13º20’N (the Irinovskoe field) (Fig. 1) were studied using CTD casts, dredges, TV-grabs and SP surveys during previous research cruises to these sites (Cherkashev et al., 2013, McLeod et al., 2009). In late 2013, the Semyenov and Irinovskoe fields were revisited, this time with an autonomous underwater vehicle (AUV) and a remotely-operated vehicle (ROV) during the ODEMAR (Oceanic Detachments at the Mid-Atlantic Ridge) cruise on the French research vessel "Pourqoui Pas ?". Here we present the first visual surveys and direct sampling of these vent sites. High-resolution (~2 m) bathymetry of the 13º30’N and 13º20’N OCCs and their associated vent fields was collected during 4 and 5 AUV dives, respectively, using the GEOMAR AUV "ABYSS" (Fig. 1). These surveys were flown between 70 m and 100 m above the seafloor, typically at a constant height to reduce vehicle movement. The AUV was equipped with Eh, turbidity, and temperature sensors in order to detect active hydrothermal venting. Over twenty ROV dives with the ROV "VICTOR 6000" allowed for the visual exploration of the OCCs and collection of 59 hydrothermal rock samples from the vent fields. As a result of these exploration activities, the first robust estimates of the size of the deposits, and the geologic controls on sulfide accumulation at these sites can be determined.

Computer applications are ideally suited to the day to day operations of grade control and short term planning on a mine site. By refining this part of the operation there is scope to increase profits by more accurately predicting ore grade and tonnage, and allowing the mining engineer more flexibility in his approach to scheduling. Large numbers of blasthole or ditchwitch samples need to be processed in a fast and efficient manner, and stored in a database for future reference. The mining engineer has to calculate tonnages and subdivide the blast or digging area into mineable areas of waste, low and high grade ore. New grade and tonnage information must be used by the mining engineer to update his short term plan. Answers must be obtained quickly and efficiently, but a high level of individual input is necessary in order that maximum use is made of the information. Geostatistical analysis of samples can be used to refine the interpolation techniques. Reconciliation between the original grade model, the newly sampled area, and the final product should be monitored continually.

Analysis for pillar extraction potential (A-PEP) is an intelligent tool, which could be used as a preliminary output indicator when considering the secondary extraction of regional support pillars in the Witbank and Highveld coalfields of South Africa. The safe and economic extraction of underground coal pillars is of paramount significance and importance to the South African coalmining industry to ensure continuity of supply of this fossil fuel. Pillar extraction practices in South Africa have been responsible a significant proportion of safety and fatality statistics in relation to its relative output (less than 5 per cent of total South African coal production) and, as a result, research was undertaken in local pillar extraction operations and extended to similar operations in New South Wales, Australia to assess a way forward for pillar extraction in South Africa. It was found that, although little in the way of new technologies has emerged in this field in the recent past, significant contributions could be made in the field of risk analysis to obtain a generic design methodology for pillar extraction in South Africa. The resultant A-PEP tool to assess pillar extraction potential is based on certain physical, risk and economic factors, which combine to indicate operational success in terms of economic, health and safety attributes. At the time of A-PEP?s development there was alack of operating underground coal pillar extraction operations, which made it difficult to validate its applicability. Since then, however, A-PEP has been successfully tested and validated against two unique underground coal pillar operations, which is the discussion of this paper. One operation is situated in the Witbank coalfield and is extracting pillars, which are 20 years old and at a depth of 80 metres below surface, while the other operation is situated in the Highveld coalfield-which is extracting 6 month-old-pillars underneath a massive dolerite sill at a depth of 220 metres. A-PEP has shown that its predictive nature is consistent the workings at both operations, irrespective of their unique conditions, and as such it can be successfully used as a mine planning tool. The A-PEP mine planning tool thus represents a positive step toward an integrated approach to underground coal pillar extraction when considering legal and operational aspects, which could form the basis for legislative guidelines when considering the future of pillar extraction in South Africa.

The coalmining industry in most countries has, at various times, suddenly been confronted with a decrease in demand for coal. Readjustments made to reduce productive capacity to match the reduced demand for coal have often resulted in hardship to mineworkers, loss of capital investment to mineowners and loss of coal reserves. The consequences of the methods of readjustment adopted in the past have contributed to the low esteem in which coalmining has come to be regarded by a large section of the gel).eral public in many countries.Four sections of the coalmining industry, representing different periods of time, different countries, and different methods of industrial organisation have been examined:Great Britain and the U.S.A. during the inter war years, the post war European Coal and Steel Community, and post war Great Britain under the National Coal Board. An assessment has been made of the methods adopted in each case to reduce productive capacity to match a reduced demand for coal. Finally the fundamentals of the problem-as it affects the coalmining industry of any country-are considered, and possible solutions put forward.During the 1950's, surplus productive capacity was a problem in many coalmining countries, including Australia and New Zealand. As a rule no techniques are developed beforehand to solve problems introduced by a sudden decrease in demand for coal. The situation is usually tackled by a series of short term decisions based on expediency, which often in the long run only aggravate the problem.

Reconnaissance studies of beach deposits along approximately 220 miles of the shoreline of Bristol Bay, Alaska, were made to determine if more detailed investigations were warranted. In addition to preliminary mapping of the deposits, samples were obtained from auger borings spaced at 1-mile intervals in key areas; shovel samples were collected to supplement the borings. Most samples were roughly concentrated in the field; magnetic separation tests, petrographic analyses, and determinations for radiometric equivalent uranium, gold, platinum, iron, and titanium were made in the Bureau of Mines laboratory at Juneau, Alaska. Spot samples from some beaches along the shores of the Alaska Peninsula indicate small deposits containing up to 10 percent recoverable titaniferous magnetite; some larger deposits were indicated to contain from 1 to 2 percent total heavy metal, principally as titaniferous magnetite. Significant amounts of other commercial minerals were not detected.

The Coffee Creek lode gold discoveries are located in the White Gold district (Dawson Range), west-central Yukon, 130 km south of Dawson City and 30 km south of the Golden Saddle gold deposit (Figure 1). Extensive structurally-controlled gold mineralization has been discovered during the 2010 and 2011 exploration drilling campaigns, highlighting this part of the Dawson Range as a significant emerging gold district. [ ] In this contribution we review the geological framework and describe the mineralization styles and metal signatures observed to date on the Coffee project. The main Coffee area is underlain by a 15 km by 4 km geochemical footprint, characterized by discrete linear gold-in-soil anomalies that correspond to various mineralized corridors. Of several mineralized areas tested in the past two drill seasons by diamond core and reverse-circulation (RC) drilling, we focus on the Supremo, Double Double, Latte and Kona zones; areas with the most information obtained from drilling thus far (Table 1; Figure 2).

Since 2001, the Hybrid Stress Blast Model (HSBM) project members have developed a software suite to model the complete blasting process from non-ideal detonation to muck pile formation. To preserve the physics and improve solution time, the breakage engine uses a combination of analytical models and 2D axisym-metric finite differences to model near-field crushing, coupled to 3D discrete lattice fracturing and distinct element numerical methods to model throw and muck pile development. The model has been validated by comparison with laboratory and field tests in kimberlite. Multiple blasthole simulations are used to demonstrate how changes to blasting parameters can influence downstream efficiencies. Case studies of wall control blasting show that the presplit design must balance the two opposing effects of increased damage with increased charge and decreasing attenuation of the seismic waves with decreasing charge. Modelling of decoupled explosives needs further development. Reducing the charge towards the back of the trim blast results in a much more significant decrease in back damage than altering the timing. The model demonstrates how separation along the weaker planes in a jointed rock can coarsen the fragmentation, leading to inefficient beneficiation, and extends the damage to a distance of at least twice the burden behind from the blasthole, severely compromising the wall stability.

Hydrometallurgical processes can be exceptionally corrosive. This is particularly true in acid leaching processes for copper, cobalt, nickel and rare earth elements. In the last five years, several large hydromet plants have been built in North America (Vale - Long Harbour, Baja Mining ? El Boleo and MolyCorp - Mountain Pass). Hundreds of storage tanks, extraction vessels and electrowinning cells along with miles of piping were designed and fabricated to withstand the aggressive environments inherent in these processes. One key element all three projects had in common was the extensive use of FRP in equipment design. As a material of construction, FRP provided equal if not improved durability relative to alternative corrosion resistant alloys at a considerably lower cost. This paper will highlight the extensive use of FRP in these three world scale projects and why it was chosen for each process.

"Abstract In recent years, there have been major problems with deep excavations in urban areas supported by diaphragm walls. In some cases, like the fatal Nicoll Highway collapse in Singapore the quality of the D-walls was not the cause. However in other projects, like the problems with the major leak in the walls of the Boston Big Dig and with leakage in a few projects in Belgium and The Netherlands was related to poor D-wall quality. Leakage through the D-walls at the joints in deep excavations led in these cases to very serious settlement behind the walls, in the case of the North- South line in Amsterdam, there was severe damage to historical buildings. This paper describes ongoing research that focusses i on acquiring more knowledge on and improvement of D-wall installation, in particular the flow process prior and during casting ii on the detection of anomalies prior to excavation and iii on the composition of the concrete mixture in view of a better and more sustainable concrete quality of D-walls.INTRODUCTIONIn the case of deep excavations in urban areas close to neighbouring buildings, diaphragm walls are one of the best feasible solutions for the retaining structure. Although the costs of D-walls generally exceed the costs of alternative wall solutions, the advantages are decisive in many situations. The main advantages of D-walls are the limited impact of the installation on the built environment, the high stiffness and bearing capacity and the retention of water, even at considerable depths. This makes Dwalls often the preferred solution for deep excavations Sometimes the D-walls are the temporary support while for the final stage a complete concrete box is constructed in the excavation. In many other cases the D-wall has the permanent function of retaining soil as well as groundwater.In the last ten years, there have been major problems with deep excavations in urban areas supported by diaphragm walls. In some cases, like the fatal Nicoll Highway collapse, 2004 , the quality of the Dwalls was not the cause. In other important failures like the collapse of the Archives in Cologne, 2008 and Dubai Infinity Tower Flooding, 2007 a combination of design and quality issues played a role. However, the major leak in the walls of the Boston Big Dig was related to poor D-wall quality (McNichol, 2000). Van Tol et al, (2010) describe a number of case studies in Belgium and the Netherlands, with leakage through the D-walls at stop end joints. In several cases with deep excavations these leakage caused serious surface settlements behind the walls. In the case of the North-South line in Amsterdam, there was severe damage to historical buildings. All these cases have generated considerable additional costs and delays, and subsequently public and political concern. Van Tol et al. (2010) present several possibilities to improve the quality of D-walls.Other disputes related to D-walls deal with the quality of the concrete and the sustainability of D-walls applied as a permanent wall. In particular the porosity of the concrete cover is sometimes problematic."

Electricity will play a major part in providing energy services in the 21st century. Ecologically Sustainable Development (ESD), that is accommodating the needs of the present without compromising the needs of future generations, will provide a focus for optimising the use of electrical energy. Environmental externalities may redefine the cost structure of electricity. Strong corporate environmental management will ensure that current environmental objectives are met and that future ones, identified by the ESD process and environmental externalities accounting, can be developed and implemented by receptive business managers. This paper addresses the current environmental issues of electricity production. It outlines a management strategy for the SECV becoming a community leader in protecting the environment. ESD is discussed in terms of using non-renewable resources wisely, extending their availability and putting in place mechanisms which ensure their ultimate substitution by renewable resources and conservation. The paper explores options for dealing with costing environmental externalities, particularly in relation to greenhouse emissions.

To reduce fire hazards from spontaneous combustion of coal in longwall gob areas, a series of computational fluid dynamics (CFD) simulations were conducted by National Institute for Occupational Safety and Health (NIOSH) to model the spontaneous heating of coal in longwall gob areas. The previous modeling results demonstrate that spontaneous heating of coal usually occurred behind the longwall shields and along the face with a bleeder ventilation system. Assuming a stationary longwall face, the spontaneous heating could turn to a spontaneous fire in several days for the most reactive coal under favorable conditions. When the longwall face advances, the spontaneous heating process will be significantly affected. In this study, the effect of longwall face advance on the spontaneous heating in the gob area is investigated using the CFD model developed in previous studies. One longwall panel with a bleeder ventilation system is simulated. The width of the panel is 300 m, while the length of the panel is changed between 1,000 to 2,000 m. The same permeability and porosity profiles are used for gobs with different lengths. The spontaneous heating first develops in the gob when the longwall face is stationary. Then, the face advances at a certain rate. The face advance is simulated as a series of discrete movements, and the effect of the face advance on the maximum temperature developed during the face stoppage is examined. Soft to medium dense silty and clayey soils are found in many parts of India and conventional friction piles have normally been used for foundations of high rise buildings and other structures on such soils. The author investigated the use of Bored Compaction Piles and these have been used on these soils achieving substantial economies in cost over conventional friction piles. Similarly highly expansive clays, commonly known as Indian Black Cotton Soils occupy about 20 percent of the Indian land area. Short bored underreamed piles have been developed for such soils and a very large construction of various types of buildings and other structures have come up on such piles over the last thirty years in various parts of the country. The paper describes investigations leading to the design and development of both these type of foundations.

The authors are to be congratulated for presenting the first comprehensive paper on the theory and practice of threshold pressure determination in connection with underground gas storage. It is of particular value since most previous experimental work on this subject has been proprietary to the commercial laboratories engaged in such testing and to their clients. This dlscussion is presented in amplification of the subject paper, not in criticism, and is based upon our experience with approximately 5,000 threshold pressure determinations. Our comments are directed primarily to the experimental procedures as described by Thomas et al. It should be noted that, while clay mineral-rich lithologies are the most common caprock materials, the samples used (Table 1) include low permeability sandstones and carbonates, but do not include shales or other formations in which clav minerals exert a dominant effect. While our experience on samples analogous to those investigated by the authors generally confirms their findings, we have found that even more rigorous experimental conditions must be imposed where clay minerals are the controlling factor. These additional conditions result from the well-known molecular character of the clay minerals themselves in which water is interlaminated with, and bonded to, silicate sheets, thus becoming an essential component of the molecule. In many clay minerals the character of the intersheet bonding, and thus the dimension of the lattice, is strongly affected by variations in the identity and concentration of cations present in the intersilicate water. On a supermolecular scale, adsorbed water films on particle or "flakes" of shale provide much of the cohesive force necessary to maintain mechanical strength. The thickness of these films is variable and inversely proportional to intergranular pressure within the dimensional limits involved in caprock investigation. The thickness of the intersilicate water layers may also be subject to pressure under some conditions. These considerations are reflected in several necessary modifications of the experimental procedures described by the authors. Two of these are, in our judgment and experience, of such importance that they cast real doubt on the validity of data obtained on argillaceous materials without their application. The very careful resaturation procedures proposed by the authors should give good results in connection with the relatively "inert" lithologies used in their experiments. Our experience suggests, however, that they would create more problems than they would eliminate if applied to argillaceous materials. 1. Drying of the sample at the temperature and for the duration indicated by the authors not only will dessicate the sample with consequent shrinkage cracking, parting along bedding planes, and even total disassociation in the case of fissile shales, but it may also alter the character of the clay minerals themselves — in some cases irreversibly. 2. Prolonged evacuation to 2 microns Hg further dessicates the sample and is even more effective in reducing the intersilicate water content. 3. Resaturation with water differing in pH or chemical content from that existing originally in the sample also serves to alter the nature of the sample, particularly its structure and strength. 4. Our experience has been that, even with the thorough precautions proposed by the authors, complete resaturation of very fine grained materials is rarely obtained. Yet our experience confirms the
