Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

Segment and gasket lining in tunnel boring machine (TBM) tunnels; and sheet membrane lining systems in driven tunnels are robust waterproofing systems each with a proven history of performance in Australia and further afield. Where an interface between these two systems occurs, the potential for costly failures is vastly increased and additional attention to detail is required in design, planning and construction. TBM cross-passages represent the most common occurrence of this interface, with steel compression flanges and neoprene gaskets being the typical means of connecting the waterproofing systems. There is a large variance in both detailing these flanges and associated outcomes, which makes this an area where there is room for improvement within the industry.The paper will look to tease out the various issues associated with this interface and how they can be dealt with efficiently with appropriate design measures, robust planning and carefully controlled construction methods.The paper will also look to review the new technologies and systems currently being put forward as potential alternatives, including an epoxy tape system, which has recently been successfully trialled on a project in Europe under high-pressure head.CITATION:O’Dwyer, E P, 2014. Quay Magnesium Limited (QMG) listed on the Australian Stock Exchange in late September 2004 having raised approximately A$30 million with the object of designing, constructing, commissioning and operating a 15 000 tpa magnesium alloy production facility in Nanjing, China. Site construction commenced in February 2005, with installation of all major components completed by the end of October 2005. Hot commissioning was simultaneously commenced with the final stages of installation, leading to the first stage of continuous ingot casting by mid November 2005. Since that time, ramp-up to the name plate capacity has been undertaken. This presentation highlights some of the more significant technical and logistical hurdles that have had to be overcome in order to meet the high quality product specifications required by a range of QMG customers.

Banded iron-formation (BIF) and iron ores were measured using the CSIRO developed TIRHyLogger ™ to assess its ability to map quartz, carbonates and riebeckite. The TIR-HyLogger™ measures continuously spectral refl ectance data from diamond drill core or drill chips with a high spatial and spectral resolution. Samples from twelve BIF, bedded iron deposits (BID) and channel iron deposits (CID) have been analysed. The spectral data were processed with two different approaches. The first approach, focused on the identifi cation of the major minerals, was performed using principal component analysis (PCA). It identified four minerals (quartz, ankerite, riebeckite and goethite) and provided pure spectra called end-members. The second approach included a mineral diagnostic spectral feature extraction involving wavelength position, relative peak heights and relative depths of absorptions. Quartz has a diagnostic absorption at 8.625 µm and ankerite shows a refl ectance peak at 11.275 µm. In addition to the identified end-members, two carbonate populations (calcite and siderite) and two silica species (quartz and amorphous silica) were identified.

The European Union (EU) have recently issued new Threshold Limit Value-Time Weighted Average (TLV-TWA) for diesel exhaust contaminants, which are CO, NO2 and DPM. The new limits for CO and NO2 were issued in 2017 whilst the one for DPM was issued in 2019. However, in order to allow EU mines to formulate strategies to comply with these new limits, the EU granted a transition period in which mines could use the old limits until the new limits are enforced. The new limit for CO and NO2 will be enforced from 21 August 2023 whilst the one for DPM will be enforced from 21 February 2026. The new limit for NO2 and DPM is very low, 0.5 ppm for NO2 and 0.05 mg/m3 of Elemental Carbon (EC) for DPM. Combination of these two limits would be the lowest among all mining jurisdictions worldwide. It poses a huge compliance challenge to EU mines if they keep using diesel machines. Therefore, a study and a field trial were carried out by the author in 2019–2021 to investigate whether replacing diesel machines with Battery Electric Vehicles (BEVs) would be the best answer to this challenge. This paper outlines results of this study and field trial.

Time Usage Models, used as a basis for calculating performance indicators for mobile mining equipment, have developed from very simple to very sophisticated models. This development has paralleled the increased collection and analysis of production data that itself has been enabled by the evolution of underground voice and data communications along with the relevant information systems. A Time Usage Model divides the hours that equipment is (or is not) used into standard components, by a hierarchical breakdown, so that measures of productivity can be used to compare performance of both people and equipment. The calculation of equipment availability, and thus the underlying Time Usage Model, has always been an extremely emotive topic as different groups use time usage information for different purposes. The direct link between measuring performance Key Performance Indicators (KPI) calculated from these models and employee rewards impacts upon the workforce. This paper discusses the conflict in use by various stakeholders and proposes ways to address some of the more contentious issues each supported by case studies. The simple status code functionality used by early systems has proven inadequate, leading to the evolution of “work-arounds”. These workarounds have become the norm and unfortunately have become embedded in the next generation of models and the information systems that incorporate them. This paper contends that the industry should stop building Time Usage Models to accommodate shortcomings in proprietary systems, but rather collaborate with suppliers to develop innovative methods to deal with the fundamental requirements. There are many points of differentiation that are not clear-cut. Pitfalls are discussed as well as threats to success. A framework of rules is developed as guidelines for how a Time Usage Model should be developed and applied. This will hopefully stimulate the on-going discussion as to how Equipment Availability and other components of the model should be calculated.

OZ Minerals' Prominent Hill copper-gold concentrator is located 130 kilometres south east of the town of Coober Pedy in the Gawler Craton of South Australia. The concentrator was built in 2008 and commenced commercial production in early 2009. The Prominent Hill concentrator is comprised of a conventional grinding and flotation processing plant with a 9.6Mtpa ore throughput capacity. The flotation circuit includes six rougher cells, an IsaMill for regrinding the rougher concentrate and a Jameson cell heading up the three stage conventional cell cleaner circuit. In total there are four level controllers in the rougher train and ten level controllers in the cleaning circuit for eighteen cells.Generic proportional – integral and derivative (PID) control used on the level controllers alone propagated any disturbances downstream in the circuit that were generated from the grinding circuit, hoppers, between cells and interconnected banks of cells, having a negative impact on plant performance. To better control such disturbances, FloatStar level stabiliser was selected for installation on the flotation circuit to account for the interaction between the cells. Multivariable control was also installed on the five concentrate hoppers to maintain consistent feed to the cells and to the IsaMill. An additional area identified for optimisation in the flotation circuit was the mass pull rate from the rougher cells. FloatStar flow optimiser was selected to be installed subsequent to the FloatStar level stabiliser. This allowed for a unified, consistent and optimal approach to running the rougher circuit. This paper describes the improvement in the stabilisation of the circuit achieved by the FloatStar level stabiliser by using the interaction matrix between cell level controllers and the results and benefits of implementing the FloatStar flow optimiser on the rougher train.

Expanding rock bolts anchor within a rock mass more efficiently than non-expanding varieties. This is of great importance in weak ground conditions, where grouted friction bolts perform less efficiently than in good ground conditions. Filling a Split Set with demolition concrete has been shown to double the load-carrying capacity when compared to conventional use of Split Sets. This report details verification of these results by installing and pull-testing 14 Split Sets of varying grout conditions within in a fault zone. These Expanding Split Sets achieved loads double and triple that of ungrouted and normally grouted conventional Split Sets. A literature review and chemical analysis were conducted to understand the expansion process better. It was found that 20 per cent more calcium is included in expanding cement, which is responsible for its growth. Factors most affecting the expansion process were identified. Furthermore, the expansion rate of a bolt outside a borehole was measured and 6 mm of expansion across its diameter was found. The compressive properties of the cement were compared with other types, due to expanding cement becoming uncohesive after the expansion process is complete. When confined within a Split Set, compression testing demonstrated that this grout is just as strong and rigid as normal grout. Finally, the bolts were tested by shear and it was found that the Expanding Split Set (ESS) has a similar shear capacity to grouted Split Sets, which is 70 per cent higher than that of ungrouted Split Sets. These results enabled the mechanism for improved performance in weak ground conditions to be verified. Expanding grout will expand the Split Set into its borehole of varying diameter and lock it into place, creating increased friction and resistance to pull-out. Two major types of economic secondary iron ores occur in the banded iron formation (BIF)-rich Hamersley Province of the Pilbara, Western Australia:dominant BIF-hosted bedded iron deposits (BID)detrital ore deposits (DID sensu lato), which are mainly hosted in the three province-wide Cenozoic sequences that include coeval non-ore sediments and comprise:a. the major Miocene, fluvial channel iron deposits (CID)b. the lesser Eocene and Pliocene detrital iron deposits (DID sensu stricto).Striking differences exist between the massive CID resources and the much smaller underlying and overlying DID. CID are essentially riverine alluvial ooidal rocks with abundant small fossil wood fragments and variable iron peloids, but with only extremely rare recognisable lithic remnants. The original matrix is typically ramifying layered goethite. Eocene DID are mainly alluvial with minor pisoids, whereas Pliocene–Quaternary DID are dominantly coarse colluvial gravels, both derived from and largely retaining the original textures of BID, hard cap or variably ferruginised surface BIF. The colluvial DID matrix is typically ferroan-aluminous soil, resulting in canga where replaced by goethite, which may be dehydrated to hematite by exposure. The Cenozoic deposits occur in two dominant geomorphological environments: the southern Marra Mamba to Brockman Iron Formation strike valleys containing all three Cenozoic sequences and the much later northern Brockman Iron Formation plateau valleys that include only the Miocene and Pliocene sequences. The Cenozoic detritals formed in different climatic regimes, with an extended dry period forming a prominent province-wide dehydrated carapace on the Eocene DID. The Miocene ‘optimum’ followed with its thick scrub-covered deep regolith that produced the fossil wood-rich CID and was succeeded by the arid cool period of the Oakover limestone/calcrete. A major renewal of exposure and erosion in the Pliocene resulted in the extensive iron gravels of the Pliocene–Quaternary.CITATION:Morris, R C, 2015. This paper addresses the problem of prediction of grade variables over volumes (block estimation) for an autonomous mine. Resource estimation is usually concerned with the prediction of the quality of material over volumes (blocks in resource models). This problem is tackled by developing Bayesian block estimation with the Gaussian processes (GPs) framework. GPs are commonly used for nonparametric regression and classification and offer an elegant solution to deal with incomplete knowledge and information. A GP is defined by its mean and covariance function. The parameters of the covariance function can be learnt by maximising the marginal likelihood of the data. This offers a way of automating the covariance fitting process, while at the same time allowing for an assessment of how well the covariance function fits the data. Within the GP framework the calculation of the average grade over a volume can be performed using Bayesian quadrature, which treats an integral as a random variable. First, it is demonstrated that the block kriging equations in geostatistics are the same as Bayesian quadrature. Then, it is determined when closed form analytical solutions for the integrals are possible and quadrature approaches for approximations are briefly discussed.

Much has been written about the fact that most transformation efforts disappoint. This paper addresses factors that should be taken into consideration in planning and implementing transformation programs to ensure success.In 2009, St Barbara commenced a transformation program to address issues of chronic poor quarterly operational performance; financial pressure caused by the unreliable operational performance; and an inability to develop business plans with integrity and to reliably deliver on the plans. As a result of a number of years of disappointing operational performance, St BarbaraÆs culture was not conducive to business success.This paper uses lessons learnt during St BarbaraÆs transformation program to highlight key characteristics of change programs that can generate significant sustainable benefits. While there were many benefits from the transformation program, the most significant impacts of St BarbaraÆs program have been transformation of the companyÆs culture and capability of people as business leaders; the establishment of a business systems framework to guide the design and development of systems; and developing support functions, for example centralised procurement and supply chain, business improvement and technical services.CITATION:Campbell-Cowan, G, 2012. In most mining operations the ore undergoes several processes such as drilling, blasting, loading, hauling, crushing, grinding and liberation to become the final salable product. Drilling and blasting is an important step in this process chain and itÆs results such as fragmentation, muckpile shape and looseness, dilution, damage and rock softening effect the efficiency of downstream processes. The value created per ton of broken ore is the difference between the price it commands when sold as the final product and the cost to produce it. Traditionally, the total process in the mining industry is classified into two groups as mining and milling. These are managed as separate cost centres inspite of the interdependency. Each process has a budget and production target and emphasis is usually on maximising production (tons) and minimising cost rather than the overall profitability of the whole business unit. The efficiency of each process is considered to be satisfactory as long as they are within budget and meet the production targets. The mine and mill managers usually try to optimise each process independently rather than the entire process. This paper discusses the potential pitfalls of decreasing the drilling and blasting cost per ton of broken rock without considering its impact on downstream processes. It introduces a holistic approach to blast optimisation by identifying and measuring the leverage that blast results have on different downstream processes and then optimising the blast design to achieve the results that maximise the overall profitability rather than just minimising the drilling and blasting costs. This paper demonstrates the benefits of such a holistic approach to blasting based on computer model simulations and field studies from metal and open cut coal mining.

Single wavelength fluorescence sensing has been used in the preconcentration of ores for many years in situations where it is typically specific to the mineral of interest. Examples of fluorescence sensing include the sorting of diamonds in diamond mines and the sorting of scheelite in tungsten ores. However, the number of minerals that show useful single wavelength fluorescence properties in the visible spectrum is relatively small, so the application of fluorescence sensing is currently limited to niche application in the minerals industry. Recent developments at the University of Adelaide have focussed upon extending fluorescence signal measurements into the infrared spectrum, as well as investigating multi-wavelength fluorescence known as up-conversion fluorescence. These techniques may not only significantly increase the number of minerals that exhibit fluorescence properties suitable for both identification and sensor-based sorting, but also provide mineral-specific signatures of higher quality than previously attainable. This paper details the research work carried out by the University of Adelaide and funded by CRC ORE to develop a mineral-specific fluorescence sensor. The sensor targets two fluorine bearing minerals, fluorite and fluorapatite, that are common in many base metal ores such as porphyry copper ores and silver/lead/zinc ores. Fluorine is a known penalty element in mineral products derived from these ores, so mining companies need to monitor and control the amount of fluorine in their final product to keep it below commercially agreed specifications. Currently there is no real time sensor capable of monitoring fluorine in the ore feed so monitoring and control is carried out using time-consuming standard sampling and off-line laboratory measurements. The development and application of a real time fluorine sensor will enable the mining companies to monitor and control fluorine levels in the plant, greatly reducing the time and cost associated with producing their mineral concentrates within the prescribed specifications.

Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

"Mining has still today high potential for improvement in the exploitation efficiency. Especially low-grade ores have very low recoveries. In Cu bioleaching operations from porphyry copper deposits extraction of only five and 15 per cent with maximum of 40 per cent are reached. This means that 60–95 per cent of the resource is not exploited. Consequently, any metal not extracted will end up in the waste material, therefore increasing the risk of environmental pollution. This inefficiency is mainly due to the use of element concentrations for decision-making on the daily basis of a mining operation (eg cut-off ore grade, acid-base accounting – ABA). If, instead, quantitative mineralogical and geochemical data together with information on the bioleaching behaviour of the ore assemblage (the biogeometallurgical approach), is used for decision-making right from the beginning of the exploitation, higher recoveries and less environmental impact can be achieved, ie an increase in sustainability can be reached.CITATION:Dold, D, 2016. This paper is concerned with the influence of engineering geology on design of Wall Control Blasts. In the Introduction, the Blast Management Framework and the Blast Design Requirements classification system (previously developed by the author) is drawn upon to set the context. Blasting objective for all wall control blasts involves both profile and damage control. Four classes of approaches for blast design are outlined and briefly discussed. Five distinct wall control blasting techniques are summarised. The general relationship between geology and blast design is explored in the paper. Influence of geology on pre-split and post-split design and blasting direction is highlighted. Wall damage modelling from the closest row of blastholes is also introduced using the Analytical Damage Model (ADM). The relationship of rock properties and explosive selection and in particular the Mach wave generation is introduced to assist blast designer in this regard. Finally a Conceptual Model that relates vibration, damage and rock mass condition has been presented along with some applications that have been undertaken by the developer. Eight conclusions have been formulated based on extensive work in this area and the current study.

The fire started in the development section from methane released into the mine through a roof-bolt hole. The date was 8 May 1998. The flames spread quickly as the coal was ignited. After eight hours the Safety Department decided to seal the vertical ventilation shafts and the slopes. The quality of coal in the Esmeralda Mine is very high quality, and Minerales Monclova (MIMOSA) decided to recover the facilities, however, the Esmeralda Mine coals have a very high gas content of 12 m3/t. Immediately after the mine was sealed, MIMOSA staff implemented a plan to monitor the gas levels underground and contacted specialists for assistance in bringing the mine back on-line. During the next 2.5 months, MIMOSA staff and the specialists observed and analysed the gas behaviour supported by a chromatograph. With the results of the observations and analyses, MIMOSA in consultation with the specialists developed a recovery plan based on flooding the area in which fire might have propagated and in which rekindling was highly probable. Those results were analysed by the command centre (Mexican engineers responsible for mine recovery). At the same time MIMOSA trained rescue teams which included miners who had no previous experience in mine recovery operations. By 20 August 1998, the mine command centre had re-opened the slopes seal. Using a æStep-by-StepÆ system, the rescue team began the recovery process by employing cross-cuts and using an auxiliary fan to establish the ventilation circuit. The MIMOSA team advanced into the mine as far as allowed by the water level and was able to recover the main fan. The official mine recovery date was 30 November 1998.

Geometallurgical modelling is increasingly being incorporated into mineral resource modelling and estimation as a means of increasing efficiency, decreasing operating costs and reducing risk in mining operations. Traditional geostatistical workflows for geometallurgical modelling face several difficulties, in particular, related to the non-additivity of process response variables, to highly heterotopic data sets and/or to preferential sampling designs, all of which may explain why most attempts to estimate geometallurgical response variables from assay data and mineralogy are still based on statistics and machine learning. The aim of the work presented here is to demonstrate the applicability of geostatistics to estimate copper recovery from copper sulfide ores at the Prominent Hill Iron Oxide Copper-Gold (IOCG) deposit in Australia. As recovery is non-additive, the preferred workflow is the joint estimation of the masses of metal in the feed and in the concentrate, both of which are additive variables. While the mass of metal in the feed can be estimated from abundant online assay analyses, the mass of metal in the concentrate can only be estimated from a very limited number of laboratory-scale batch flotation tests. Traditional approaches to estimating the masses of metal in the feed and in the concentrate, such as simple and ordinary co-kriging, are described and discussed. A modified version of co-kriging is introduced, which incorporates linear relationships between the mean values of the input variables, in addition to a spatial correlation model. Co-kriging with related means is shown to outperform traditional simple and ordinary co-kriging in terms of precision, accuracy and consistency of the estimates, and offers a practical and efficient tool to deal with highly heterotopic and preferential sampling designs.

Reaction products between high-temperature hydrothermal solution/uppermost mantle peridotite or lowermost crustal gabbro have been reported along Wadi Fizh of the northern Oman ophiolite; they are named as ‘mantle diopsidite’ and ‘crustal diopsidite’ respectively (Figure 1).The mantle diopsidite contains Cr-rich minerals like chromite, Cr-rich diopside (<2.5 wt per cent Cr2O3) and Cr-bearing grossular (<7.0 wt per cent Cr2O3). The chromites are varied in shape in the mantle diopsidite, euhedral to anhedral chromite grains contain globular inclusions of grossular and chlorite, and film-like chromite veinlets branch off from partially dissolved coarse chromite grains. In the crustal diopsidite, fine chromite grains are included in oscillatory-zoned skeletal uvarovite, especially in its high-Cr zones. The uvarovites are rich in rare earth elements (REEs). Direct laser ablation – inductively coupled plasma – mass spectrometer (LA-ICP-MS) analysis of the primary fluid inclusions detected Na, but not K.Our detailed studies of Wadi Fizh diopsidites clarify that the high-temperature hydrothermal solution dissolves chromite grains at the uppermost mantle section and precipitates hydrothermal chromite at the lowermost crust section. In addition, REEs were mobilised in the hydrothermal solution together with Cr. The hydrothermal solution involved in the diopsidite formation is assumed to have been rich in NaCl, and could mobilise appreciable amounts of Cr and REE.CITATION:Akizawa, N, Arai, S and Tamura, A, 2015.
