Fatality prevention and zero harm are key focus areas for resource companies. However, they remain an ongoing challenge for high hazard industries, including the mining sector. As one example the latest International Council on Mining and Metals (ICMM) Safety Performance Report shows that, while there is generally a declining fatality trend since the organisation commenced publishing data in 2012, there were still 33 fatalities across member companies in 2022 (ICMM, 2023b). Every fatal incident brings significant pain and suffering to the colleagues, friends and family of those involved, however the failures and causal factors leading to each incident are frequently very familiar to people working in the industry, particularly those tasked with managing risk and safety. In the words of Dame Judith Hackitt, Grenfell Tower Fire Investigator, ‘there are no new accidents – just different people making the same mistakes because they haven’t learned’ (Hackitt, 2022). The recurring themes in terms of the nature of fatal hazards and their associated causes across the mining sector show that, despite improvements evidenced by the favourable declining fatality trends over the long-term (in general and outside of catastrophic multiple fatality events), changes are required to achieve fatality elimination and the aspiration of zero harm. This extended abstract and the accompanying presentation apply a mining frame of reference to the idea that there are no new accidents, examining how this applies within the industry and why the lessons from past failures are not always well understood or applied to prevent the reoccurrence of material unwanted events. Many contemporary mining and resources organisations have extensive internal incident history data and a wide variety of information on fatality incidents is publicly available, including the annual ICMM Safety Performance Report. This extended abstract and the accompanying presentation explore how this existing data can be used effectively to learn from the past and proposes practical ideas regarding strategies for successful knowledge transfer to the workforce as a key component of preventing history repeating and improving safety performance across the industry. As noted by ICMM, ‘sharing lessons from failure is vital to improving health and safety, but it’s not enough’ (ICMM, 2023a).

Al2O3 in natural iron ores generally occurs in goethite, kaolinite, gibbsite, hematite and iron carbonate. When the content of alumina in sinter increases from 1.11 per cent to 3.50 per cent, the reducibility of sinter decreases with the addition of four different types of alumina. When gibbsite and kaolinite are added, the low temperature reduction disintegration (RDI+3.15 mm) of sinter increases with the increase of Al2O3 content in the range of 1.5 per cent to 3.5 per cent. Furthermore, when adding pure reagent alumina, the increase of Al2O3 content in the range of 1.5 per cent to 3.0 per cent will increase the RDI+3.15 mm of sinter. The addition of ferric oxide–alumina spinel with the Al2O3 content in the range of 1.5 per cent to 2.5 per cent, also increases the RDI+ 3.15 mm of sinter. The influence difference is results from the change of the mineral composition and microstructure of sinter because of the occurrence state of Al2O3. Under the condition that Al2O3 content of sinter is 2 per cent, the porosity of sinter with kaolinite is 37.26 per cent, and the shape is irregular. When gibbsite and ferric oxide–alumina spinel are added, the porosity of sinter is about 16 per cent, and the pores are round. When pure reagent alumina is added, the porosity of sinter is 17.49 per cent, and the pores are regular and small. When kaolinite, gibbsite and ferric oxide–alumina spinel are added, platy calcium ferrite is mainly formed in the sinter, and some of it is columnar calcium ferrite. On the other hand, when pure reagent alumina is added, in addition to platy columnar calcium ferrite, acicular calcium ferrite is also formed in the sinter.

The steelmaking desulfurisation plays an important role in coal-based steelmaking route of blast furnace and BOF converter within the flow sheet of BF-deS-BOF. With the advent of H2-based ironmaking and steelmaking process, the transition flow sheet of DRI-Smelter-BOF has been assessed and developed by the steelmaking industry. What is the role of deS in H2-based route of DRI-smelter-BOF? This paper will present the influence of sulfur-containing iron ore on steelmaking desulfurisation and the leading role of EAF-smelter played in H2-based steelmaking route. The sulfur contents of iron ore vary considerably around the world and lower-grade iron ores with high sulfur are likely to be used for H2-based route, a considerable amount of sulfur that comes from DRI will report to the melt of EAF-smelter and the operating conditions of EAF-smelter also impact the sulfur content of the hot melt for steelmaking deS. The EAF-smelter of H2-based route is the focus of the steelmaking industry at moment, unfortunately, an industrial-scale EAF-smelter has been the missing link of the flow sheet of DRI-Smelter-deS-BOF, despite similar EAF-smelters being used for various applications of Fe-Ti and Fe-Ni processes. The latest progress is that several pilot-scale and industrial-scale EAF-smelters with reducing capability are planned for construction within next several years and low-grade Australia iron ores are certainly to be employed as basic feedstocks for H2-based route of DRI-Smelter-deS-BOF.

Efficient project planning during the detailed feasibility to final engineering stages of a project requires not only the accurate prediction of the grade, tonnes and recovery, but their degree of variability as well. An understanding of the major parameters involved in the grade and tonnage estimates is essential, and the communication to those receiving the results is most important. The personnel involved in the project require more than just a global reserves classification. Quality parameters other than grade should be evaluated as carefully as the estimate itself. This paper does not present any novel ideas to overcome or enhance any of this. Rather, it takes a step back and tries to look at the global picture of requirements for proper project planning.

An ilmenite ore from the Murray Basin was initially characterised using combined X-ray Fluorescence Spectroscopy (XRF), X-Ray Diffraction (XRD), Scanning Electron Microscope (SEM) and Electron Probe Microanalysis (EPMA) techniques. XRF analysis showed the ilmenite sample contained 58.7 wt per cent TiO2 and 26.2 wt per cent Fe2O3. Major impurities included: Cr2O3 (4.5 wt per cent), Al2O3 (2.7 wt per cent), SiO2 (1.9 wt per cent), Mn3O4 (1.5 wt per cent) and MgO (1.1 wt per cent). Manganese and magnesium were present as solid solution components within the ilmenite lattice while the elevated Al2O3 and SiO2 levels were due to separate aluminosilicate gangue minerals. Chromium was associated with discrete chrome spinel gangue phases of variable composition, (Fe,Mg)(Cr,Al)2O4. Quantitative XRD analysis of the sample identified rutile (12 per cent), ilmenite (11 per cent), and pseudorutile (67 per cent) as the main titanium-containing phases and chrome-rich spinels (seven per cent) as the major impurity phase. Minor quartz, aluminosilicates, zircon and monazite were also present. The ilmenite was mixed with synthetic graphite, pressed into pellets and reduced carbothermally in nitrogen at 1200 and 1400°C with C/O molar ratio of 1.2. Titanium oxide in the ilmenite was converted to titanium oxycarbonitride. Iron oxides were reduced to metallic iron. The Changes of impurities as a result of reduction was examined by SEM and EPMA analyses. Iron and chromium oxides within Fe-rich chromite were reduced to form a Fe-Cr-C alloy. Mg-rich chrome spinels were partly reduced at 1200°C. Zircon particles were not reduced at 1200°C, but were significantly reduced at 1400°C, with the particles becoming porous and the silicon being removed.

The Mary Kathleen tailings storage facility (TSF) contains 7.1 Mt of uranium mill tailings. This waste repository was rehabilitated in the mid-1980s using a multibarrier dry cover. At the time of mine closure, it was predicted that acid mine drainage (AMD) generation would not eventuate and that uranium, heavy metals and radionuclides would be adsorbed within the tailings and not seep from the TSF into the environment. This study reports on the environmental status of the TSF. Gamma-ray measurements indicate an intact dry cover of the waste repository. However, seepage (~0.5 L s-1) of acid (pH 5.71), saline (0.31 per cent), high conductivity (8.8 mS cm-1), slightly oxygenated (DO 2.6 mg L-1), radioactive waters occurs from the toe of the tailings dam into the former evaporation ponds and local drainage system. Seepage waters are calcium-magnesium-sulfate rich with elevated Fe, Mn, Ni, U and Zn values. Oxygenation of tailings seepages causes the precipitation of Fe and coprecipitation and adsorption of other metals (Pb, U, Y), metalloids (As), rare earth elements (Ce, La) and radionuclides (U-235, U-238, Th-227). In contrast, Ba, Ni, Sr, Mn, P, S, Ca, Mg, Na and K as well as Ra nuclides (Ra-226 and Ra-228) remain in solution until pH neutralisation and evaporation lead to their precipitation in efflorescences and sulfate-rich evaporative sediments further downstream. Dry capping of Mary Kathleen uranium mill tailings has resulted in physical containment of the wastes. The dry cover shows no signs of erosion and radiation levels are within acceptable limits. However, the flow rate of surface seepage from the TSF occurs at a much higher rate than predicted. Also, the neutralising potential and adsorption capacity of the tailings and underlying aquifer were overestimated at the time of mine closure. AMD has developed mobilising metals, metalloids, rare earth elements, alkalis, radionuclides and sulfate from the waste repository into surface water seepages.

The æMountain Camp reefÆ at Alford's mine, Wakamarina,Marlborough, had no recorded gold production although assays of 5 - 26 g/t were reported, but it did produce c. l000 kg of scheelite. The lode consists of a lower massive quartz layer, and upper, cm-banded, ribboned quartz layers, and has a very low sulphide content. Within the massive layer, relict early fine-grained mosaic quartz with idioblastic scheelite is surrounded by recrystallised granoblastic coarse-grained quartz with calcite and plagioclase, in which an original ribbon banding texture is mimiced by strings of tiny inclusions. In both the banded and massive layers, rounded quartz grains, created by cataclastic milling of grain margins and along syn-vein fractures, have been partially annealed within later quartz overgrowths. Late fibrous quartz and calcite fill brittle cross fractures.   Deformation events D1 and D2 are associated with mid-late Jurassic metamorphism of the Pelorus Group, D2 producing the main regional schistosity (S2, and the earliest scheelite. D3 is an extensional mylonitic shear event that formed the dominant schistosity in Alford's mine (S3). The quartz lode was emplaced within the mylonite by a series of individual dilation pulses (D4). The D5 event caused crenulation of the S3 mylonite, kink banding and chevron folding of the S2 schist and S3 mylonite, and reverse faulting, rodding and cross-fracturing with late fibrous quartz-calcite veining of the D4 quartz lode.   Fluid influxes during successive dilations of D4 fractures progressively caused recrystallisation of earlier quartz bands on the footwall, annealing and recrystallisation of quartz along milled crystal margins and fractures, and deposition of minor sulphides. There was thus a dynamic interplay between fault dilation, lode fracture, fluid influx, lode band deposition, and lode recrystallisation. The purpose of this paper is to provide the rationale for decision making when considering the use of three heading gate roads in Australian underground coal mines.There are currently 30 underground longwall mines in Australia with all but two employing two heading gate road longwall layouts. The volumetric capacity of these two heading ventilation circuits is evidently viable in typical block lengths and in low to moderate gas regimes. However, in gassier regimes, potential longwall production rates of 10 Mt/a, and or where block lengths are being extended to well beyond historical norms, a conventional two heading U circuit may not be adequate. Other than seam gas emission, block length and production dependent heat loads together sealed area management requirements also have to be considered.The two mines currently employing three heading gate roads have the opportunity to revert to two headings at any time but, based on operational experience to date, have not done so due to the consequential reduction in gas management capacity and lower production rates associated with gas delays and limitations on tailgate access.Although there are a number of alternative two heading longwall circuits available and employed worldwide, the custom and practice of promoting inert goaf atmospheres in most Australian coal mines to mitigate the risk of catastrophic gas explosions and spontaneous combustion often prevents the use of gas loaded bleeder systems. In this respect, Australian underground coal mines may be considered the safest in the world.The current situation is that three heading gate roads have been proven successful in Australian conditions, albeit at additional cost, where two heading ventilation circuits cannot manage net seam gas emission to the ventilation system at the production rates required. Large GDP increases in the Pacific Rim countries have been accompanied by similarly large increases in demand for oil products. In the case of residual fuels, this demand has been tempered by the substitution of other energy products such as coal or gas.In the region's developing and newly industrialised nations, it has not been possible to make the very large investements in infrastructure which allow other energy sources to be used for transportation. As a result, GDP growth has been mirrored in the demand growth for automotive fuels. Other non-sustitutable products such as lubricants have also benefitted by the region's GDP growth.The combination of a number of years of sustained growth, coupled with increasing quality requirements is now stretching the upgrading capacity of the region's refineries. If, as seems likely, the region continues to enjoy a high GDP growth rate, then this will be accompanied by increasing reliance on finished products coming out of the Middle East. The owners of the most simple form of refinery in which crude is simply distilled and the naphtha reformed into petrol will face major capital investments. If they decide not to bear this cost, they will face increasing problems in selling their surplus residual fuel oil at an economic price. Quality will become an increasingly important issue. Not only in the well publicised areas such as unleaded petrol, but also for products such as lubricants which play a major role in the efficiency of modem equipment. Quality of service such as hours of opening, ease of access, and service station facilities will also become increasingly important as the oil companies use these to attract customers and gain brand differentiation.

"Mining companies routinely invest in improvements to their processing operations, but often these enhancements do not work to full effect.Improvements can be ineffectual because underlying process problems are not identified or prioritised for correction or repair.This is frequently because ‘black box’ solutions deployed to implement process control improvements typically rely on instrumentation to provide data for algorithms to work effectively, and these solutions are often ineffective or disabled at mineral processing sites.There are three key reasons they can fail to deliver: vital instrumentation is not calibrated or functioning as required basic process control and loop tuning was not implemented correctly or is in need of tuning there is an inability to support the ‘black box’ solutions on-site.Many of these problems can be overcome by implementing operational analytics that identify where problems are occurring and prioritise where improvements need to be made.Incorporating operational analytics into process improvement is not a new methodology. It is more a back-to-basics approach to make more efficient use of capital and systems already deployed.Data collection systems, such as the OSIsoft PI System, provide real-time and historical process information for process control experts to analyse and identify areas for improvement. However, most systems are not used effectively. With some simple enabling, data collection systems can provide evidence for prioritising investments in improvements and determining an investment’s effectiveness.MIPAC advocates that the initial step, with analytical evidence, is that clients review the control strategies and implement small influential instrument repairs/calibrations and operator training. Poor standards of electrical installation and maintenance continue to contribute to electrical related fatalities and injury risk in the mining industry. Electrical isolation has the potential to initiate major unwanted events (MUEs) and as such requires thorough analysis to demonstrate adequate management. There have been 719 incidents related to accidental contact with electricity over the past five years in the Queensland mining industry (Queensland Government DME, 2012a, 2012b) with the last fatality being recorded in 1997 (MESA, 2012).A project funded by the Australian Coal Association Research Program (ACARP) has developed a form of a safety case using the four-layered æCoal Mine Safety Regime (CMSR)Æ approach. The CMSR is transparent and robust as it includes full site risk assessment and special issues risk assessment to be demonstrated as part of the process.The practical application of the CMSR has been demonstrated using a longwall mining case study site, where a unique 11 kV armoured face conveyor (AFC) is in operation. The case study has involved progressive application of the requirements of each layer of the CMSR (see Figure 1) to high voltage (HV) electrical isolation related issues. The project has specifically concentrated on understanding and analysing two high priority MUEs, electric shock and unplanned movement of equipment, through Layer 1 analysis. These two MUEs are an important part of the project as they have demonstrated the steps that may be taken to satisfy the risk management processes at Layer 1 of the CMSR and have contributed to resolving issues at the other three layers of the CMSR.The analysis of HV electrical isolation has been used as an exemplar to demonstrate the CMSR process and its practical application. The CMSR is applicable to major/principal hazards management as identified by the New South Wales and Queensland jurisdictions.This paper provides an overview of the development of a form of safety case using the CMSR and introduces the case study application for HV electrical isolation risk management at underground coal mines focusing on the major unwanted event of electric shock.This project provides the mining industry with a documented, publicly accessible, form of safety case using the CMSR approach for the first time.CITATION:Kizil, G V, Griffiths, D, Bye, A and Joy, J, 2012. This paper presents an application of the newly developed risk-cost-benefit (RCB) framework to support risk management decisions for the selection of wall monitoring technologies at an open cut mine site. The RCB framework quantitatively assessed the risk of unstable slope-induced wall failure and its impact on both loss of life and coal recovery. The RCB framework was utilised in the selection of an optimum group of risk-controls to effectively manage the risk of wall failure to ensure safety and optimal production occurred. The RCB framework incorporates mathematical modules that integrated RCB calculations and risk-control effectiveness assessments to determine the risk reduction and financial impact of the examined safety initiatives.This paper provides an overview of the five-staged RCB framework, incorporating the RCB decision support tools, RCBGEN (Risk-Cost-Benefit Generator) and RCEMETHOD (Risk-Control Effectiveness Method), and demonstrates its application to slope stability risk management at the case study site. The RCB charts are designed to display a balance between risk, cost and benefit for easy interpretation by decision makers to support the objective selection of an optimum suite of risk-controls. The RCB analysis revealed that a combination of ævisual inspections, automatic extensometers and radarÆ for fast- and slow-wall failures with structure were sufficient to reduce the probability of loss of life (PLoL) to the æModerateÆ zone as defined by the Australian Geomechanics Society (AGS, 2007) for the case study site. The Engel Model search simulator is described with particular emphasis on preparation of input parameters. The computer has a "black box" role in the same way a telephone or radio is a "black box" in communications. A search technique (sensor) is selected and applied in two stages of'search, namely, reconnaissance and detailed. The model relies on knowing and quantifying the parameters of the target sought and the search technique; this is usually done by analogy studies from available literature and experience. The parameters are then balanced by feasibility studies on the search area. Case histories show that the study makes the user aware of what he is likely to find rather than what he wants or needs to find to be viable. Two case histories are presented: one for Mississippi Valley type lead- zinc deposits and another for stratiform copper. The first study showed particular benefits in the input stage and highlighted the fact that one could expect a lot of encouragement by way of interesting results but with little chance of finding the high-grade,

Before presentIng my address I wish to acknowledge the honour which has been conferred on me by my appointment to the position of President of the Australasian Institute of Mining and Metallurgy.It is a matter of great concern to us all to find our Empire at war. I am confident that, as was the case in the war of 1914-1918, members of this Institute and of kindred Institutes and Associations will contribute in full to the Empire's needs and that in the end our cause shall triumph.I think it appropriate that I should take as the subject of my address that of the Broken Hill Lead-Silver-Zinc Industry. The subject matter of this address has been confined to the operations of Mining, Ore Dressing, Lead Smelting and Refining and production of Electrolytic Zinc in Australasia. The review of the Broken Hill Lead-SilverZinc Industry would not, of course, be complete without reference to those Australian industries which trace their origin to the Broken Hill field, but· it is thought that an outline of these industries would make thIs review too extensive.HISTORICALMany efforts have been made to translate the early history of Broken Hill and its subsequent growth throughout the years into one of the wor~d's most famous mining fields in the form of a simple story.

This paper explores the use of a Process Safety Management (PSM) system to manage risk in the mineral processing and metallurgical refining industries. Important elements of a PSM system are described. International regulations and non-regulatory guidelines covering PSM for high hazard industries are reviewed. Case studies of incidents in the mineral processing and metallurgical refining industries are presented to understand the lessons learned from failure to manage risks using a PSM system. Conclusions are drawn to adopt PSM in the minerals and metals processing industry as a tool for loss prevention and risk management.

The pilot scale testing of Australian coals for their NO, emissions was carried out at the ACTC to investigate the influence of coal properties, furnace conditions and different NO,, control strategies on NO,, emissions. The NOx emissions for the Australian coals tested were found to vary over a wide range and primarily depended on combustion temperature and excess oxygen conditions. These coals did not exhibit any fundamental difference in NOx levels to other world coals. Under similar combustion conditions, the conversion of coal nitrogen to NO. was found to be best correlated with coal rank parameter, R, max per centage. The use of staged combustion as a control strategy was shown to reduce their NOx levels by 56 - 77 per cent.

High Voltage Pulse (HVP) technology is a novel process that could potentially be the step-change needed in ore processability in the mining industry. The propensity of high voltage pulses towards mineral-enriched ore particles during treatment makes this technology selective and has the potential to enable ore pre-concentration by size separation. In this study, the potential for pre-concentration of a low grade copper-gold ore sample subjected to HVP pretreatment is investigated and compared to that achievable after mechanical breakage in JKMRC’s rotary breakage tester (JKRBT). The data show that a better pre-concentration result can be achieved after HVP processing than conventional breakage. The gold pre-concentration potential is much better than that observed for copper. 4D HVP pre-concentration characteristic curves were produced to assess the metallurgical performance possible as a function of energy input. The case study shows that when targeting a nominal metal recovery of 90 per cent for gold, the HVP pre-concentration technique can split the low grade ore (0.9 ppm gold and 0.28 per cent copper in the feed size 26.5-37.5 mm) into two components – a low grade oversize material (0.3 ppm gold and 0.15 per cent copper) and a high grade undersize material (1.0 ppm gold and 0.3 per cent copper). The mass split to the low grade is 23 per cent. Under this mass split scenario, gold recovery to screen undersize is 90 per cent and copper recovery is 85 per cent. The HVP treatment of this feed size requires a specific energy (spark) of 2.8 kWh/t.There is further research that would be required before industrial uptake of the HVP pre-concentration technology would be possible.
