Para solucionar dicho problema, se realiza una gestión del error que incluye una serie de medidas preventivas como descubrir, evaluar y eliminar los factores que producen el error dentro de la fuerza laboral y así desarrollar la realidad intrínseca de la organización contra los errores humanos.

The United States Bureau of Mines (USBM) awarded a contract to Foster-Miller Associates, Inc. (FMA) to develop guidelines for designing, constructing, stocking and maintaining rescue chambers in underground mines. This report develops information and recommendations which could be used by a mine operator who desired to install a rescue chamber. Volume I covers the following major areas: atmospheric life support, configuration and construction, chamber location, power and lighting, equipment and supplies, communications, and psychological aspects and training. Volume II contains the following appendices: History of Barricading, Chamber Pressurization Considerations, Diffusional Infiltration of Gas, Compressed Air Storage, Location Examples, and Psychological Aspects.

This paper provides estimates of large-scale temperature gradients that can be used to support modeling of natural convection and cold-trap processes in thermally perturbed drifts. Temperature influences estimates of the quantity and chemistry of water contacting engineered components (e.g., drip shields and waste packages) in a potential high-level waste repository at Yucca Mountain, Nevada. Temperature gradients drive natural convection and cold-trap processes, which could affect the distribution, quantity, and chemistry of liquid-phase water contacting drip shields (waste packages).The confluence of high temperatures, liquid-phase water, and high chloride may lead to corrosion of drip shields and waste packages. An overview of the evolution of environmental conditions in thermally perturbed drifts is presented to provide a context for the estimates of temperature gradients. A mountain-scale conduction-only model is used to estimate temperature gradients and temperature differences for a drift located at the center of a potential repository. Thermohydrologic models and heat transfer algorithms are used to evaluate the reasonableness of the conduction-only results. Estimates of temperature using an in-drift heat transfer algorithm account for the possibility of drift degradation and formation of a rubble pile on the drip shield. Temperature differences between the center and ends of the drift are estimated to rapidly increase to 80 °C soon after repository closure. That temperature difference decreases over time to 15 °C after 10,000 years. Local temperature gradients are generally estimated to be less than 0.2°C/m, though the gradients can be much larger near changes in lithology and near the end of the drift. The estimates of temperature gradients do not account for intermediate- and small-scale variations, such as those influenced by differences in heat loads between waste packages, intra-layer variations in thermal properties of the surrounding fractured rock, or localized drift degradation.

The paper explores the substance, circumstances and significance of the Whelpley and Storer Copper Process. The 1866 Whelpley and Storer patent specified a process combining mechanical and chemical engineering. The process called for a down-drafting shaft furnace with a horizontal flue connected at the bottom of the shaft. Powdered sulphide ore was roasted in the shaft and then sent forward fully oxidized to a bath of sulphuric acid and/or calcium chloride. The “water furnace” was developed in Boston, at a time when the city was home to most Michigan copper mining companies. The process was field-tested at Quebec’s Harvey Hill copper mine. In 1865 James Douglas, Jr. took over management of the Harvey Hill mine with the challenge of saving the family investment. He needed a way to make money from low-grade sulphides. Through his relationship with chemist T. Sterry Hunt, Douglas learned of a possible way ahead using the process developed by Whelpley and Storer. The paper confirms the centrality of publications, meetings, and networking in the advancement of metallurgy. The process is of interest historically, as much as anything, because it is one of the pillars of James Douglas’ distinguished career in copper. INTRODUCTION At the 1866 meeting of the American Association for the Advancement of Science (AAAS) in Buffalo, New York, T. Sterry Hunt (1826–1892) read a paper “On the Metallurgical System of Messrs. Whelpley and Storer.” The paper is part of a chain of events that constitute the foundation of James Douglas’ (1837–1918) success in the copper mining industry. To understand the career of Douglas over the early years of modern copper mining, we must necessarily start with Whelpley and Storer. Going back to Hunt’s paper, he concluded his remarks by asserting that the Whelpley and Storer Process was “capable of such wide and varied application, that it is probably destined to have a very important influence on the industry of the world” (Hunt, 1867a). He reached this conclusion reasoning that the process had overcome a critical metallurgical hurdle (Hunt, 1867a). Hunt, a geo-chemist in modern terms, recalled that the chemist of those years easily separated copper from “sulphuretted” ores by converting it to a soluble salt from which pure metal was precipitated by “an equivalent of iron.” The problem was to “…apply this principle in the working of copper ores on a great scale...” (Hunt, 1867a). Hunt noted that this very problem had “…engaged the attention of many metallurgists, and has given rise to various processes for obtaining copper by the moist way, many of which are nearly or quite perfect in their operation, but are lacking in practicability and cheapness” (Hunt, 1867a). Hunt thought Whelpley and Storer had overcome the low-grade sulphide hurdle with a commercially viable engineering breakthrough. Yet in the history of copper metallurgy, the process is missing. What happened?

Paper written on project work carried out at the University of Pretoria in partial fulfilment of the requirements of the B Tech degree at the Tshwane University of Technology This work tested whether ore particle size affects the carbon-based pre-reduction of titaniferous magnetite. Pre-reduction is performed at a temperature of some 1100°C in a rotary kiln, in the Highveld Steel process. Laboratory isothermal reduction measurements were performed with ore-coal mixtures at this temperature, to test the effect of ore particle size. The results show that size does limit the degree of reduction for particles larger than 15 mm.

The environmental compliance process affects all aspects of hardrock-mining operations, from exploration through post-closure. The primary components of the environmental compliance process are requirements under the National Environmental Protection Act of 1969 (NEPA) and the permitting requirements associated with various other federal, state and local laws. This study considers the Greens Creek Mine and the development of two other projects (Alaska-Juneau & Kensington). These mines were used as the foundation for a case study of the impacts of the environmental compliance process on hardrock-mining activity in Southeast Alaska. The case study presents the institutional and legal requirements, followed by a discussion of the impact of this process on the production and timing of the three mines. The results of this preliminary study reveal some interesting findings, some expected and others unexpected, regarding the impact of the permitting process on the exploration, development and (proposed) operation of the three mining projects in the Juneau area. Determining the cost of compliance with these requirements is an important step in evaluating the appropriate course of action to protecting the environment while providing the necessary raw materials needed for society. This preliminary study highlights the initial starting point for such an analysis.

The extraction of lignite in open-cast mines has a severe impact on the landscape. Proper landscaping after the shut-down of the mines has always been the statutory responsibility of the mine operator. The large area of the land used for open-cast lignite mining requires special regional planning procedures well in advance of the permitting procedure stipulated in the mining law. The lignite plan determines, among other things, the objectives of land reclamation. These objectives will then be outlined in more detail in the mine operation plans and implemented accordingly. Apart from the actual mining laws and regulations, a large number of environmental laws also determine the process of land reclamation. The success achieved in the field of land reclamation in all mining regions during the recent decades has shown that the lignite mining industry creates new landscapes that do not leave any scars in the countryside and merge very well with it.

In order to increase productivity and decrease reducing agent rate (RAR) of blast furnaces, slag content of sinter ore must be decreased. However, gangue content in iron ore tends to increase because of resource depletion. Therefore, amount of high-grade concentrate in sinter ore feeds should be increased, but this situation causes a decrease in permeability of the sintering bed so that the productivity of sinter process decreases. In order to increase the amount of concentrate, NSSMS has developed the P-type separate granulation method, RF-MEBIOS and SPEx II technologies. These technologies have been introduced to actual sintering machines. In this report, a new technology using microparticles of iron ore is proposed to increase the amount of concentrate in sinter blends. Basic experiments simulating the sinter making process using microparticles of iron ore were undertaken. Crushed pisolite, containing >60 wt per cent -10 µm was prepared for binder. The pisolite microparticle binder was mixed with another Australian sinter feed ore in a high-speed agitating mixer. They were mixed for about one minute, and then granulated using a pan pelletiser. After that, they were added to another raw material mixed by a drum mixer, and then charged in a pot for a sinter pot test. As a result, when the amount of concentrate increased from 13 wt per cent to 20 wt per cent, productivity was almost kept constant using pisolite microparticle binder.CITATION:Kamijo, C, Yamaguchi, Y, Matsumura, M, Kawaguchi, T and Nomura, S, 2017. This report summarizes the incidents that preceded the December 9, 1970, propane-air explosion in Port Hudson, Mo., and then attempts to establish the nature of the explosion from the ensuing events. Special emphasis is given to possible ignition sources, the distribution of flammable vapors, and the analysis of blast damage. Both near-and far-field damage indicated that this explosion may be attributed to the detonation of propane in air with an energy release equivalent to that from about 50 tons of detonating TNT.

Mines and plants for sale, downsizing, bankruptcies, consolidation, slab imports, minimill growth displacing traditional integrated steel-making. These are some issues for North American iron ore and integrated steel producers. But the blast furnace still remains the workhorse of the ironmaking industry. And direct reduction and other alternate ironmaking technologies have yet to make any serious inroads in North America. How come? This article addresses the current state of primary ironmaking in the changing North American steel industry, the direct reduction market and new technologies under development that may hold some promise.

In 1969, new coal mine health and safety legislation was enacted in the United States. As a result of this legislation, the Department of Interior, Bureau of Mines has carried out communications research aimed at developing whole mine communications for increased safety and efficient operation. The research has advanced and many promising results are now being demonstrated to the mining industry. However, there remains a challenge to further improve wireless communications within the mine workings and through the overburden above the workings. The objectives for future research and development will be discussed, together with the physical and operational constraints that new mine communication systems must face.

Tholeiitic intrusion-hosted nickel (PGE-Cu) sulfide deposits are under-represented in Australia’s inventory of known resources of this deposit type, relative to regions globally with comparable geology. To support discovery of major new deposits, a continental scale analysis of Australia’s potential for tholeiitic intrusion-hosted nickel (PGE-Cu) sulfide deposits is being undertaken. The analysis applies a mineral systems approach to map the four essential components of the ore-forming systems:1. sources of ore constituents2. crustal and mantle lithospheric architecture3. energy sources or drivers of the ore-forming systems4. gradients in ore depositional physico-chemical parameters.The results reveal several regions of Australia with potential for discovery of large tholeiitic intrusion-hosted nickel (PGE-Cu) sulfide deposits, mostly around the margins of Archean and Proterozoic lithospheric blocks. This paper by Mr Mclvor is yet another contribution to the eternal controversy surrounding the benefits, or otherwise, of high circulating loads in closed circuit grinding. Throughout the paper it is difficult to establish whether the author is referring to effects relating to the mill, classifier, or to the overall circuit. Table 1 is put forward as indicating the advantage of operating at high circulating loads. From the two (it would be interesting to see more) product sizing points provided in each case, the conclusions to be drawn are that cases one and two have very similar size analyses. The third case, however, indicates a much flatter curve, that is to say, higher fines production and a very wide spread of size ranges. If the objective of this circuit is to provide a closely sized product with minimum overground slimes to the downstream process, then it would appear to be the less "efficient" by far. No data is given for product pulp densities either. The term "efficiency" should never be used without a careful and clear definition. In this case, "efficiency" is used to express an apparent percentage reduction in operating work index (Bond) from the measured "standard" at 250% circulat¬ing load operation. The oxidation of structural materials at high temperatures in gas-cooled systems can be greatly retarded by the addition of small amounts of hydrogen to the inert coolant. In experimental equipment for gasifying coal with heat transferred by helium, the oxidation of graphite and metals (molybdenum, tungsten, and certain stainless steels) by trace impurities in the coolant helium was virtually eliminated by adding 0.1 to 1.0 percent hydrogen. The reaction of hydrogen with praphite does not preclude its use to retard oxidation; at 2,400° F. only 0.2 pound of graphite per square foot per year is lost for each percent of hydrogen in the helium. The addition of hydrogen appeared to prevent the mass transfer of carbon often associated with oxidation of graphite, without causing noticeable embrittlement of stainless steels and molybdenum or other damaging effects. This method has been tested in precision gas-recycle systems and applied successfully for more than a year in a pilot-scale unit operating at 250 pounds per square inch gage and 2,400° F. It is now used routinely and can be applied to many gas-cooled systems operating at high temperatures.

Looking back over the last 150 years or so, it is not hard to see that electricity has been a primary force in shaping human development. It has been a basic ingredient in our industrial revolutions that simultaneously improved the productivity, working and living conditions within our farms, factories, hospitals, offices and homes. Today, it has become an essential part of our daily life and our future. Amazingly, we keep finding new ways to use electricity to make our lives more safe, comfortable and productive. It is a great success story of human ingenuity, achievement and development. Demand for electricity The convenience and value of electricity has not gone unnoticed by consumers. According to the U.S. Department of Energy (DOE) (Energy Information Administration, 2009), electricity is the fastest growing form of energy in the world and during the next 20 years the world will likely consume twice as much of it as it does today. In 2008, almost 20,000 terawatt hours of electricity globally were consumed (Energy Information Administration, 2009). That is enough electricity to light up more than two million baseball stadiums or power more than 11 billion computers continuously for an entire year. The United States has the dubious distinction of be-ing the largest consumer of electricity in the world with enough power to provide each American with more than 3 kW of electricity ? that is about twice the power currently available to Europeans, eight times the power available to the Chinese and more than 25 times the power available to the citizens of India (American Coal Foundation, 2009).

This paper documents the use of bench testing equipment developed to enhance the understanding of, and improve upon, common roof bolt support systems. Most roof bolt systems consist of components that are covered within the scope of ASTM (American Society for Testing and Materials) standards. However, ASTM does not specify testing methods or performance standards for complete roof bolt ?systems?. It only designates testing methods and minimum performance standards for roof bolt ?components?. Improved understanding and continuous improvement of these systems is essential given the enormous impact that they have on the safety and profitability of underground mining operations. Bench testing equipment has been developed that effectively measures and records multiple points of deformation in a rock bolt support system while load is being applied. With this testing capability, an improved understanding of component and system stress distribution can be obtained and devices and methods for visual load indication can be developed. This paper is formatted as case studies where testing was performed to demonstrate load versus deformation profiles and to develop system improvement products.

Tracor MBA under a contract to U.S. DOI Bureau of Mines designed, developed, fabricated and demonstrated a conveyor belt service machine. It is self-contained, battery powered, rubber tired and capable of handling, storing and transporting conveyor belting, wire rope and associated structures for sectional conveyor belts. While moving the tail section to its next position the machine discharges the belting, wire rope and belt structures. The belting is elevated and separated to provide for the insertion of the structures with minimum expended effort. A hydraulic system provides the power for steering, braking, winding or unwinding under controlled tension the belting and wire rope, operating a load-bed slat conveyor, raising, lowering and locking connection hitches and powering external hydraulic tools. Also, a pneumatic system is available to operate air impact tools. The service machine, with its present components can function in coal seams as thin as 48 inches, double wind conveyor belting 42 inches wide, 400 feet long, load or unload 3/4" wire rope on two separate reels and is controlled by two operators. A complete belt move using this machine requires only four workers. Successful underground demonstrations were conducted in coal mines located in West Virginia and Kentucky.
