Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a ?plateau? and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have ?exemplary? safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA?s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization?s mindset? What characterizes these organizations? decision making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensive research since 1997 of organizations that suffered unexpected disasters. Their huge focus on safety, the achievement of lofty goals and celebration of safety successes led to ?mindset? that they are leaders in safety, protected by an extraordinary safety system and that their safety incident figures indicated a truly world-class safety performance. It led them into the trap of the seven deadly delusions?

This report describes a process for recovering chromium and other metals from superalloy scrap. Part I contains the results of laboratory scale experiments which were conducted to test a complex flowsheet utilizing a wide range of extractive metallurgical operations. The novel basis for the process is the formation of a sulfide matte in which chromium is concentrated in a discrete chromium sulfide phase. Mineral processing and hydrometallugical procedures are used to separate chromium sulfide from the other matte constituents. The products of the process are a chromium-nickel alloy suitable for reuse in the superalloy industry, electrolytic nickel, electrolytic cobalt, and an iron-molybdenum residue. Recovery of the principal elements contained in the scrap are chromium(Cr)-93 percent, nickel(Ni)-99 percent, cobalt(Co)-96 percent, molybdenum(Mo)-92 percent. Part II encompasses a preliminary engineering design of a pilot plant capable of treating 100 pounds(lb) of superalloy scrap per hour. Also included is an economic analysis of the operation of the pilot plant.

The Lick Fork prospect shows appreciable nickel sulphide mineralization in a narrow, steeply dipping, layered sequence of metamorphosed hornblende peridotite, gahhro and hornblende gahhro, intrusive into the Pedlar Formation (charnockitic pyroxene granulite) of the Virginia Blue Ridge Complex (Precambrian). Rocks of the intrusion have equilibrated to granulite- facies metamorphism, as indicated by pervasive polygonal texture of silicate aggregates, development of biotite- and hornblende-preferred orientation, compositional equilibration of the silicate minerals, the Mg-Fe2+ (orthopyroxene-dino- pvroxene) distribution coefficients (average 0.57), (MgO + FeO + Fe20 3)/Al20 3ratios in orthopyroxenes, and Ti contents and AI(i\)/A I(\i) ratios of hornblendes. The sulphides, consisting of pyrrhotite-pentlandite (with up to 4 wt% Co)-chalcopyrite-pyrite-marcasite-violarite- bornite—mackinawite, are virtually restricted to the ultramafic unit of the intrusion and constitute 20-30% of the samples of hornblende peridotite studied. Although the basal accumulation of the sulphides may suggest magmatic segregation, sulphide- silicate textures typical of sulphide liquid immiscibilitv are not present, even in the sparsely mineralized gahbro units. On the other hand, the transgressive and relict textures are strongly suggestive of replacement of the silicate minerals by sulphides. These include chaleopyrite ± pyrrhotite ± pentlandite (violarite) ± pyrite ±magnetite veins crosscutting silicates, sulphide penetration into silicates at silicate-sulphide contact zones, rounding of silicate grains against sulphide matrix, and islands of silicate minerals (olivine, hornblende, pyroxene, biotite) in sulphide masses. Metamorphic mobilization of pre-existing sul­ phides is a possibility, but involvement of post-metamorphic, hydrothermal fluids is suggested by the following features: paucity of annealing and deformation textures in the sulphides, replacement of metamorphic and deformed silicates, lack of differential sulphide mobilization and the common occurrence of sulphides along grain boundaries and triple junctions of poly gonal silicate aggregates. The consistent association between sulphides and hydrothermal alteration products of silicates suggests that they are cogenetic. The hydrothermal fluids may have merely mobilized and concentrated the pre-existing, disseminated, magmatic sulphides, or there may have been significant addition of sulphides during this process. This question cannot be resolved unequivocally with the existing data.

"THIS IS THE THIRD of the consecutive Annual Forums on Grinding that were instituted by Mr. L. E. Djingheuzian in 1957. The first was on the subject of Ball Milling (Transactions, Volume LX, 1957) and the second on The N on-Cataracting Ball Mill (Trans-actions, Volume LXI, 1958). From the discussions at these forums it was evident that one of the most important factors affecting the efficiency of grinding is the shape of the liners. In order to make a study of liners in use, it was •decided to seek the co-operation of operators across the country by sending out questionnaires requesting basic information. Additionally, these operators were urged to set down briefly any pertinent information on the subject acquired through experience and experiments at their own particular plants. A compilation and analysis of these data were to form a basis for discussion at the Forum on Liners at the Annual Meeting in Montreal, April, 1959. The response of the milling fraternity to this solicitation was most gratifying, and clearly indicated the importance attached by the operators to the effect of liner design upon overall grinding performance."

Indian limestone mines which are highly mechanized in operation and supply limestone to the cement plants have been quite successful in devising ways and means to optimize blasting parameters, so as to have best fragmentation, powder factor, and cost of excavation. In primary blasting mechanized or manually mixed ammonium nitrate Fuel Oil is quite popular in dry holes, and in case of watery holes conventional packaged slurry/emulsions are being used. In order to avail the low density characteristics of the ANFO, some limestone mines have also used Saw Dust and Rice Husk along with ANFO. A number of field trials have been undertaken to explore the suitability of mixing rice-husk and saw dust with ANFO. Presently, about 10% of rice husk/saw dust is being used only to expand the explosive column. The main technical consideration for the use of saw dust/rice husk, has been to reduce the final density and increase the occupancy of explosive in the blast-hole. This not only lowers the detonation pressure in the blast holes, but also reduces cost of explosive without compromising on the fragmentation quality. Besides, with sleeping of the charge there is no substantial fall in the blast quality.

Since 1922, when a brief report3 on the production and utilization of garnet was issued by the Bureau of Mines, there have been a number of interest—in developments in the technology of that mineral. Greater activity has been displayed in mining, new mills have been erected, and more attention has been paid to improvements in milling so that a more efficient and economical recovery of garnet has been possible. These activities have resulted in larger production; therefore, attempts are being made to broaden the market for garnet abrasives, and to find new uses for the unusual combination of physical properties than this mineral possesses. The writers have recently completed a field study of the occurrence, methods of mining and milling of garnet, and the manufacture and utilization of abrasive garnet products. This preliminary report indicates the most important sources of garnet, the methods by which it is recovered, and the manner in which it is recovered, and the manner in which it is utilized.Chemical and Physical Properties.The term ""garnet"" is applied to a closely related group of minerals which crystallize in the caste forms and possess similar physical properties, although their chemical composition may exhibit greater variation. The chemical composition of the garnet group may be represented by the general formula 3R"" O.R""' 203.3S102, in which R represents the bivalent elements calcium, magnesium, in the manganese state and ferrous iron, R"" represents the trivalent elements, ferric iron, aluminum and chromium in the chromie state. An account of the amorphous substitution of the different bivalent and trivalent elements, specimens whose composition may be represented by a definite formula are rare, the composition of any given specimen generally exhibits treat complexity.

Productivity and operating expenses are everything when it comes to judging how well your mine and company are doing. Stakeholders are always looking at these numbers and trying to understand if productivity is improving, if operating costs are trending downward and what you are basing your decisions on. o make improvements you first need to understand how you are performing. If you can?t get this basic information, you will never improve. You will not be able to make simple decisions related to maintenance, operations or production. Forget about planning, as you will only be guessing based on the numbers some stakeholder or your executive wants to see. The problem miners run into is not that the data doesn?t exist; it does, but typically it?s raw, it?s everywhere and its volume is huge. Today there is more on-board computing capability in shovels than we had on our desktops three years ago. Add other mobile equipment, plant systems and multiple OEM vendors to the equation and the challenge is daunting.

The fiscal year which ended June 30, 1932, although one of acute financial de ression was, nevertheless, one of distinct encouragement to those who have long struggled against the much too prevalent occurrence of accidents in coal mines, especially of coal mine fire and explosion dis- asters, both of which are now known to be readily preventable. Table 1 summarizes some important information about explosions in the United States coalies for the fiscal year which ended June 30, 1932, insofar as such information is available to the safety division of the United States Bureau of Mines, the personnel of which are under instruction to get in touch with and report on all mine explosions in their respective districts, including not only those occurrences with loss of life but also those in which for- tunately no lives, are affected. Table 1 gives data on 33 ignitions in 10 States with a total of 87 deaths for the year; the number of ignitions (33) is considerably groater than the number (26) for the previous fiscal year, and is exactly the aver- age number for the past four fiscal years; the mumber killed in the 33 ignitions (57) is materially lower than that (217) for the preceding fiscal year or the average (150) for the past four fiscal years. The States with the greatest number of deaths from explosions in the fiscal year ending June 30, 1932, were Virginia; 54; West Virginia, 11; Pennsylvania, 7 (6 being in anthracite mines); Alabama and Colorado, 4 each; Illinois and Kentucky, 3 each; and Maryland, 1. The greatest number of ignitions listod wes that for West Virginia, 7; then came Pennsylvania with 6 (2 being in anthracite mines); Alabama with 5; Colorado, Illinois, Tennessee and Virginia, 3 each; and California, Kentucky, and Maryland, 1 each. "Albian Sands Energy (ASE), an oil sands surface mining and extraction operation, is currently undergoing start-up and is scheduled to ramp-up to full production by mid-2003. Startup includes the selection and implementation of over 75 IT systems or computer applications which include: engineering applications; systems required to control mining activities, maintenance, the processing plant and utilities; and also systems required to run the company, such as human resources, finances and inventory/ warehousing.This paper outlines the processes used for the selection, implementation and integration of these systems in order to optimize the use of information and to meet the technical, business and IT requirements of the company. The first step in ASE’s approach to identifying system requirements is the development of a business activity model, outlining all tasks involved in running the operations. The computer applications required to accomplish each task are then identified by having the IT team work closely with each technical and business unit. Historically, at some companies, the selection of systems has been led by the IT team or by consultants, with little input from the technical or business units. Typically, this results in products that do not meet the company’s expectations. To ensure a more positive outcome, ASE believes that it is imperative that the company’s technical and business people be closely involved in the selection process.Integration of the IT systems is also key, including linking all applications through a data warehouse and EDMS. Over 120 required integrations have been identified. This ensures the storage of all data in only one location, data security, and easy data and document retrieval for all requirements throughout the company"

The processing of heavy mineral concentrates can be enhanced by the use of power ultrasound to clean their surfaces of oxidation products and fine coatings, mainly through the large, but very localized forces, produced by cavitation. Results are presented of the application of power ultrasound to remove coatings from the surfaces of mineral sand concentrates in order to improve the sharpness of their high-tension separation and, in some cases, the quality of the low iron components such as zircon. Improvements of 10–15% in recovery and up to 20% in grade compared to unsonicated material were achieved in a single stage laboratory high-tension separation. Cavitation also ensures that a fresh, clean surface is presented to leachants during the leaching of suspended solids. At the same time, the effect of cavitation and the shearing effect of the sound wave passing over the solids ensure that the saturated boundary layer is kept to a minimum if not completely removed. This application of power ultrasound helps to speed up leaching processes as applied to the removal of excess alumino-silicate contaminants in some ilmenite. The greater part of these publications was available during the preparation of Geophysical Abstracts and Ms been abstracted in them. The rest was not received and remained unaccounted for. The Ludwigshafen and Oppau plants are largely independent I. G. factories situated some 23 miles apart on the west bank of the Rhine facing lannheim. The former factory is a general organic chemical works specializing in dyes, dye intermediates, plastics and synthetic rubber,. while the latter is a heavy organic chemical factory producing ammonia, methanol, higher alcohols and their derivatives. Before the war much of the I. G. research and development work was carried out at Ludwigs- hafen; in particular the high pressure research section directed by Dr. Mathias Pier played a very active part in all hydrogenation developments. Apart from the possibility that a full scale hydrogenation or Fischer Tropsch plant had been erected at Ludwigshafen/Oppau during the war, the main objectives of an oil team investigation were: - (a) To obtain from Dr. Pier and his staff an advance general picture of German fuel production processes. (b) To study the fuel developments still in the research and pilot plant stage. (c) To examine any new developments in hydrogen or synthesis gas manufacture. (d) To look for new processes for production of high anti-knock fuel constituents and for new fuel additives. (e) To investigate new chemical syntheses based on raw materials obtainable from oil. The Ludwigshafen and Oppau factories were first visited by the large team listed in Appendix A during the period 25-31st March. The investigation revealed that no large plant for synthetic fuels or lubricants had been operated at this target. All the important members of the Fuel Research Section staff had been evacuated to Heidelberg and information on the work of this department was therefore limited to that contained in seized documents, the greater part of which have still to be examined in detail. Valuable information on synthesis gas production from hydrocarbon gases was obtained as well as interesting data on a num- ber of chemical syntheses. Two members of the above team Holroyd and Faragher accompan- ied by Major Tilley (interrogator) went on to Heidelberg, contacted Dr. Pier and the chief members of his staff on April 1st and obtained from them the hydrogenation information given later in the body of the report. Later, after visiting Leuna, Holroyd, Faragher and Ellis paid a second visit to Heidelberg and to Ludwigshafen/Oppau on 27th-30th May to check a number of details, particularly with respect to hydrogenation costs, and to obtain information on the pressure.conversion of water gas to hydrogen and on the manufacture of Wickel pressure vessels.

The mining industry is changing rapidly. Competition among OEM and aftermarket vendors increases dramatically each year as a result of mergers and acquisitions, and especially technology advancements in the mining industry. Adding value to mine operations by delivering technical value and differentiation is becoming more subtle and more difficult to accomplish. Because of this, identifying and protecting intellectual property has become more important to the product development, engineering and marketing/sales continuum than it had been in the past. How companies protect intellectual property and determine the value of those assets is critical to the sustainability and differentiation of technology providers in the mining industry.

Fine powders normally exhibit, to a more or lesser extent, some ability to participate in dust explosions. Zinc dust as used in the extractive metallurgy of zinc, is not known to be a reactive powder in the industry. No evidence could be found in the literature where zinc dust was involved in an explosion or explosive type event. On 3 May 2002 an explosive type incident occurred at Zincor in Springs, South Africa, which led to a fatal accident. Given the history of relative benign behaviour of zinc powders under conditions conducive to dust explosions, it was decided to share the information of this incident with industry in an effort to prevent similar incidents. Therefore, the purpose of this paper is to describe the incident that occurred at Zincor, discuss the major findings, report on corrective actions, as well as publish the results of subsequent tests that were performed to determine the conditions under which zinc powder exhibits explosive characteristics.

The Bureau of Mines investigated the resources, costs, capacities, market relationships, and short- and long-run supply of phosphate rock and phosphoric acid. The 206 mines and deposits evaluated in 30 market economy countries contain an estimated 35.1 billion mt of recoverable phosphate rock (demonstrated resource level). U.S. resources are sufficient to satisfy the domestic and export markets for phosphate products well beyond the year 2000. Because of resource depletion at current producers, however, new properties (with higher cost levels) need to be developed if U.S. production levels are to be maintained. Existing worldwide capacity can satisfy expected demand through the early 1990's. Expansion at existing mines or low demand growth could mean that no new property development will be needed before the late 1990's. Worldwide, almost $8 billion could be required for development of new phosphate rock properties between now and the year 2000, given 3-pct annual growth in demand. Most properties that could develop in the 1990's would require price increases of 20 to 50 pct to break even. To earn a 15-pct rate of return on investment, prices must rise to nearly double the present level of $24 to $29 per metric ton.

This publication contains analyses and related source data for 256 natural gas samples from wells and pipelines in 17 States. All of the samples were obtained and analyzed during calendar year 1977 as a part of Bureau of Mines investigations of the occurrences of helium in natural gases of countries with free market economies. This survey has been conducted in the United States by the Bureau of Mines since 1917. In late 1965, the survey was expanded to include foreign gasfields. The analyses published herein were made by mass spectrometer and a special helium analysis apparatus, which are described in Bureau of Mines Bulletins 4862 and 576.3 The results of the investigation of the occurrences of helium are published annually to make the information available as soon as possible to the gas industry and the public in general. Nineteen previous publications have presented the results of 11,317 gas analyses prior to 1977. The first three, Bulletins 486,4 576,5 and 617,6 contain analyses and related source data on 5,218 gas samples collected from 1917 through 1960. The other 16

Coal research and related activities in 1964 by the Bureau of Mines resulted in many advances in the production, preparation, and utilization of this valuable natural resource. Mining research during the year was marked by work on hydraulic mining, coalbed degasification, mine drainage control, mine sealing, strip-mine rehabilitation, and quenching of mine-refuse fires. Bureau research continued to produce information leading to increased safety in underground mining. Such research included roof and rib control via bolting and bonding, pressure monitoring of coal and mine rock during mining operations, and development of safety devices for detecting conditions leading to roof falls. Other safety work was continued on methane detectors, coal-dust suppression, and mine venti lation equipment and techniques. Mine explosion and explosives research, mine equipment and materials, safety rule modernization, and safety education and training of mine workmen were continued. Coal storage and preparation work in 1964 ranged from freezeproofing of lignite for rail shipment, through washability studies of coals and washing equipment, to research on new ways to remove ash and sulfur from coal. Electric power production, which ,accounts for a major portion of the market for coal, received further study. Unique power generation systems, inc luding coal-burning gas turbine and megnetohydrodynamic generators, were intensively investigated. Conventional power systems were improved through continuation of work on furnace performance, powerplant stack gas purification, and engineering and boiler water service.
