As technology evolves, accidents may occur because human-system interactions were not considered adequately in the process. A systematic methodology can be used to evaluate the causes of mishaps and to develop recommendations that will enhance safety. A recent trend observed in underground coal mining is used to illustrate this principle because an underground mine is a dynamic work environment. Mining is characterized not only by frequent geologic changes but by technological evolution that can seriously degrade human performance and compromise worker health and safety The case exemplified in this paper involves implementation of remotely controlled equipment for extended cut mining. Extended cut mining technology evolved with minimal ergonomic consideration even though the change from on¬board to remote machine operation dramatically modified the role of the human component. In response to concern that system interactions should be examined more closely. a methodology was developed to identify hazards.

It is the purpose of this paper to review theoretical model concepts in through- the -earth transmission of electromagnetic waves. While the subject is very broad in its defined scope, we will be conscious of the specific applications to mine rescue and emergency telecommunications in coal mine environments. The plan of the paper is to outline previous theoretical efforts without attempting to attach names or organizations to the specific developments. However, we include a guide to the literature as an appendix. Without pretending to be entirely impartial we include a critique of the limitations of the available analytical models. As I see it the central problem is to calculate the steady state mutual impedance of a four-terminal network that is the black box characterizing the through-the-earth path. Problems of secondary importance include the determination of the input impedances of the two pairs of terminals and the associated transient responses. [ ]

Cost Systems Cost systems are organized methods of collecting, allocating, and accumulating resource utility expressed in terms of money. There are two major spheres of cost accumulation: financial accounting and cost accounting. Financial accounting deals primarily with historical costs and provides a means of measuring and recording cost events which have al¬ready taken place. While there are no legal requirements to control the methods of recording and reporting cost data in the United States, the procedures in use are guided by professional organizations such as the American Institute of Certified Public Accountants. The final pro¬ducts of financial accounting practices are the income statement and balance sheet which describe the financial status of the organization under study. Cost or managerial accounting is an expansion of financial ac¬counting procedures, providing more detailed cost breakdowns on present operations with one objective being the projection of present costs into the future. The projection process tends to limit the accuracy of the information; however, to management and project justification per¬sonnel, data on future operations tends to be of greater value. The basic principle behind cost accounting is the cost cycle. As products are developed from raw materials, the value of the work-in¬process increases due to the cost input of the various fabricating ele¬ments. Thus, from the basic input elements of facilities, labor, and raw materials, costs are added to each unit of production as a function of the work performed on that unit.

This report describes three cases of carbon monoxide (CO) poisoning in a manhole, including one fatality, from CO migrating through soil after nearby use of explosives. A municipal sewer project involved installation of new pipes and manholes. Work of the general construction contractor was interrupted when a subcontractor detonated 265 pounds of nitroglycerin-based explosive 40-60 feet south of the manhole to break up underlying rock. A construction worker who descended into the manhole 45 minutes after the explosion collapsed within minutes, and two co-workers descended into the manhole to rescue him. One rescuer retrieved the unconscious worker, and the other rescuer died in the manhole. All workers had elevated carboxyhemoglobin levels. In the subsequent NIOSH investigation, air monitoring was conducted with real-time instruments, and air samples were collected in Tedlar bags. Laboratory analyses of the bag samples collected near the bottom showed 1905 parts per million (ppm) CO, 19.5% oxygen, and 3% carbon dioxide. Direct reading instruments showed progressively higher concentrations as the sensor was lowered into the manhole. Subsequent chamber tests on sample explosive yielded 27 liters CO per kilogram detonated. Based on this value, the surface blast at the construction site may have produced about 3,250 liters (114.8 cubic feet) CO. The CO in this incident most likely was released from the nearby explosion and migrated through soil and fractured rock into the manhole. The blasting and construction industries should be made aware of this previously unrecognized CO exposure hazard associated with surface blasting. The extent of CO exposure from explosives used in construction is not known, and additional information on the extent of CO exposure must be collected. In addition, confined space entry procedures (including monitoring confined space atmospheres before entry) should be observed; CO monitoring of confined spaces in the presence of blasting can prevent future incidents such as this one.

The notion of wayfinding, as conceptualized by planners, geographers, and psychologists, denotes the ability of an individual to move from one point to another through physical space. In order to achieve this movement, a person relies on a cognitive map of spatial representations [Passini 1984]. Which features of this cognitive mapping will be stressed depend, in part, on the researcher's perspective. A planner, for instance, would tend to emphasize the effect of physical structures on mobility. A psychologist, on the other hand, might focus on individual differences in how human minds encompass and represent physical space [Evans et al. 1984]. There is yet another dimension to wayfinding that needs consideration, and it rests upon the idea that reality, as experienced by human beings, is mediated: "[People] have preselected and preinterpreted this world by a series of commonsense constructs ... which help them find their bearings in their natural and socio-cultural environment and to come to terms with it" [Schutz 1967]. These "common sense constructs" are arrived at socially and constitute the agreed-upon schemas that guide people's everyday activities. According to this principle, cognition is governed by some nonlogical factors that reflect not only individual procedures but collective ones as well. These group strategies, which are shaped by shared rules and values, influence "the information gathered, the ways it is processed, the inferences that are drawn, the options that are being considered, and those that are finally chosen" [Etzioni 1992]. From this perspective, cognitive maps, rather than being individual-centered templates of environmental images [Rovine and Weisman 1989] or representations of spatial relationships [Evans 1980], are partially group-centered schematic processes. As such, they are subject to reinterpretation, revision, and outside intervention [Kaplan and Kaplan 1982]. As intermediaries between the environment and behavior, cognitive maps serve as bases for decision-making. Traditionally, it has been assumed that good maps facilitate correct decisions, which in turn leads to optimal performance during wayfinding [Hunt 1984]. Given the argument that there is a social (non¬cognitive) facet to cognitive mapping, however, this image of a cognitive map as some sort of static reference construct that motivates individual action is too narrow and mechanistic. If cognition involves less a knowledge of the environ¬ment than it does the process of "giving it meaning through imposing an order on it" [Rapoport 1976], then wayfinding behavior is not j ust a function of setting and individual differences, but is also a function of one's "normative-affective" structure [Etzioni 1992]. Rapoport [1976] used such an assumption as the base for a set of hypotheses about the connection between "external demands" and "organismic factors." Heavy equipment manufacturers have made a long-term commitment to minimize operator vibration exposure for comfort, performance, and health reasons. Domestic and international guidelines/standards and EC laws dictate exposure limits based on measurement of vibration at the interface between the seat and the operator’s buttocks using seat-pad accelerometry.1-4 This is historically based on the assumption that the only major source of vibration is transmitted through the seat pan. However, vibration may also be imparted to the head and neck via the steering wheel and/or arm-rest controls and a relatively rigid upper body.5 Unfortunately, little is known regarding the influence of arm position on head and neck motion. The purpose of this study was to investigate relative head and trunk motions during riding simulations of large construction equipment, using three different arm control options. Methods Five typical heavy equipment ride files were “played back” through a man-rated Servo Test 6-degree-of-freedom vibration system. An 8-camera Vicon motion capture system operating at 200 frames per second, recorded the motion of reflective surface markers on 5th, 50th, and 95th percentile right-handed male subjects, using 3 seat and control configurations (steering wheel (SW), floor mounted armrest controls (FM), seat-mounted armrest controls (SM)). Two trials were performed for each ride and seat control combination (each trial: 60 sec of 6-dof and 60 sec of vertical vibration). The relative motions (change in distances) from the marker over the xiphoid process (caudal end of sternum) to markers over each shoulder, each mid-clavicle, the presternal notch, and to each of four markers on a tight band around the head were calculated (12,001 frames, 6-dof motion only). As a rigid body control, distances between markers on the head band were also monitored. The standard deviation (SD) of the 12,001 distances between pairs of markers was normalized by the mean (L) of the associated distances producing: SD/L which was used as a measure of motion. Error assessments were also performed by analyzing the motion between relatively fixed markers (on the headband). A repeated measures analysis of variance was used to evaluate the results. While five ride files were used, only one ride file containing significant lateral acceleration components was analyzed for comparing the effects of two armrest controls versus use of a steering wheel for this part of our study. Results Values of SD/L between the points on the relatively rigid head band were consistently small and similar to each other for all conditions with one exception due to treatment (SM v SW, p=0.0145). SD/L between the markers over the xiphoid process and the presternal notch, another region that should be relatively rigid, were also similar to each other for all conditions. Use of floor-mounted, arm rest controls versus a steering wheel produced a significant increase in the value of SD/L between the xiphoid process and: the right shoulder marker (92%, p=0.0316), the right mid-clavicle marker (47%, p=0.0478), and the right-front marker on the head band (28%, p=0.0182). The Bureau of lines as repeatedly called attention to dangerous practices in the transportation and opening of kogs of black blasting powder. In 1914 Diwin Higgins (Prevention of Accidents from Explosives in Metal inos, Miners' Circular 19, Bureau of Mines, p. 15) stated "canisters should always be opened by removing the bung. To use a pick or other sharp tool for cutting a hole in the canister is dangerous. After the necessary amount of powder has boon taken out, the canister should be kept closed. The powdor should always be handled at a safe distance from an open light." In 1919, Geo. S. Rice (onthly Statement of Coal line Fatalities for June, 1919, Bureau of Mines, pp. 16 to 18) calls attention to the dangers that arise in underground transportation and handling of explosives, and calls special attmtion to the danger of hauling explosives in motor trips; the unnecessary risk involved in hauling men on the trip at the same time, and the necessity of keeping open lights away from black blasting powder. In 1919 the writer (Sensitiveness of Explosives to Frictional Impact, Tech. Paper 234, Bureau of Mines, pp. 13, 15) called attention to the fact that black blasting powder is not as sensitive to frictional impact as the nitro-glycerin dynamites and the gelatin dynamites. Black powder may be ignited by direct impact and the writer believes that this may occur without presupposing a preliminary spark. It is well known, however, that black blasting powder is very sensitive to ignition by spark flame, or heated materials, and these may be produced by an electric current, by an open light, by incandescent particles from a pipe, cigar, or cigarette, by the tearing of metal sheets or scraping of metal against metal, and possibly, by other means. -- The above discussion will serve to introduce a newly recognized hazard the ignition of black blasting powder by the forcible opening of kegs with wodden tools. The details of three accidents in which kegs of black blasting powder ex- ploded instantly when a wooden tool was driven forcibly through the top of the kegs are as follows: 1. In 1915, a powder man employed by a coal mining company stated that he was opening a 25-pound log of black blasting powder by driving a hole in the top with a wooden sprag when the powder in the leg ho was opening ignitod. This caused the ignition of fifty-two additional 25-pound logs of the powder, and the fatal burning of this and another powder man. The ono making this statement lived five hours after the explosion.

Pennsylvania anthracite coal is almost universally recognized to be the solid fuel best suited for domestic use when cleanliness , convenience and cost are considered . At various times and in various localities , however, conditions so affect the supply that it becomes insufficient or is too high in price and recourse must be had to other fuels as substitutes . In practically every district in the United States where anthracite has been the common domestic fuel , some other fuel is available as a substitute . In even the most severe climates such fuels are used either as emergency substitutes for anthracite , or as the ordinary domestic fuel . There may be some drawback to the use of such fuel , such as the need of more frequent attention to the furnace , smoke , soot and dust , in the case of solid fuels lower in price than anthracite , but these inconveniences must be accepted as necessary accompaniments to a decreased fuel bill . The cleanliness and ease of handling anthracite in the house must be considered in the light of a luxury and a higher cost of heating with this fuel must be expected . That a growing number of people are willing to pay for such added convenience is evident from the increasing mumber of heating installations using oil , gas and electricity , all of which are more convenient and usually more expensive even than anthracite .

This report deals with the processing and marketing of iron and steal scrap in the intermountain States of Arizona, Colorado, New Mexico, Utah, and Wyoming and the northwestern Plains States of Nebraska, North Dakota, and South Dakota. The quantity and quality of iron and steel scrap consumed in both the steel mills and iron foundries in these States in 1964 were decided by (1) the type of facilities used to produce an end product; (2) the availability of a dependable source of iron and steel scrap at competitive prices; and (3) the, ratio of the cost of iron in iron ore to iron in scrap. In recent years the quantity of scrap used per ton of steel ingot produced has decreased. The major individual. consumers of iron and steel scrap were the steel mills in Arizona, Colorado, and Utah, foundries in Utah and Arizona, and copper leaching, operations in Arizona, New Mexico, and Utah. About 2 million tons of iron and steel scrap was used by the iron and steel industry in 1964. Although there has been a decline in the amount of scrap used per ton of steel ingot produced because of changes in steelmaking methods, there has been a slight increase in the overall quantity of scrap used because of the increase in ingot production. It is estimated that the scrap consumption in the steel industry will increase by 19 percent by 1980. In addition to the scrap consumed by the steel industry, 273,000 tons of ferrous scrap were used by copper-leaching operations. Thirty-one percent of the ferrous scrap used by the steel industry in 1964 was purchased from dealers and other sources. The CF&I Steel Corp. (formerly The Colorado Fuel and Iron Corp.) at Pueblo, Colo., complemented its blast- and open-hearth-furnace operations with basic-oxygen converters in 1962. The basic-oxygen and open-hearth furnaces were charged with 30 and 90 percent scrap, respectively. Future use of the open-hearths would probably be used to supplement ingot production requirements. Because it was more economical to use less scrap and a high molten-metal charge, the United States Steel Corp. at Geneva, Utah, which uses the open-hearth process to make steel, charges the furnaces with 25 to 35 percent scrap. The Allison Steel Manufacturing Co. at Tempe, Ariz., uses a 100 percent scrap charge in its electric furnace. Although it was estimated that there were 327,000 tons of various grades of motor vehicle scrap generated in 1963, no estimates were made concerning the total quantity and grade of scrap available in the area. Most of the scrap used in the steelmaking industry originated within the area. It was advantageous to sell scrap generated in eastern Nebraska, North Dakota, and South Dakota to markets in the Midwest because of freight costs.

We are here today to witness the graduation of 63 young men as Engineers in the mineral industries . It is an important occasion and one in which the public takes a keen interest , since this school has long been famous for the educational facilities it affords young men, and for the high professional standing of its graduates . : : : It is fitting that once each year we should pause to review the path the student has traveled , and to reflect upon the perspective opening before him as an engineer . I was invited to come from Washington to take part in this ceremony and to say something to these young graduates . In searching for a theme ," it occurred to me. that perhaps my best contribution would be to duscuss certain beliefs as to college training , deal with the situation in which the young engineer finds himself on graduation , touch on the new ideas of the social and political responsibilities of the engineer , and point out what he may expect to find in practice , and , above all , what he must do to orient himself properly in the mining profession and maintain the best traditions of our brotherhood . Let us first review your college days and see what they have done for you . I am told there are still many temptations in college . I know there used to be . Rest assured the same holds true in the professional life ahead of you. The main difference is , perhaps , that your greatest temptations in college were those offered by your teachers , who tried to entice you into realms of harder work, closer application , increased concentration , clearer thinking and better understanding , by animating your curiosity and increasing your powers of reasoning and of observation . In other words , here you have carefully and systematically been decoyed toward finer and loftier things . Do not expect that similar helpful influences will greet you on every side in your professional life . Often quite the contrary is the case .

A simple , quick, and relatively accurate device is needed for determining the presence of very small concentrations of organic halide gases in various atmospheres . Such a device would enable safety engineers , health officials , and others to discover quickly the presence of these gases in a suspected atmosphere and thus be able to apply protective measures promptly. This detector should prove useful in detecting leakage of halide gases from refrigerating and air - conditioning systems and in determining the presence of halide solvents in the atmosphere of dry - cleaning and degreasing establishments where organic halide solvents are used . Virtually all poison war gases are halide compounds . Because of the high toxicity of these gases , even in very low concentrations , a detector having the proper sensitivity should be useful in civilian defense for determining when atmospheres are relatively free of poison gases .

Materials handled at metal and non-metal mines in the United States increased 10 percent compared with those of 1965 as a result of expansions in output at existing mines and development of others to meet increasing demands for mineral products. The tonnage of commercial materials mined or quarried increased as did the quantity of waste handled. Crude material from which metal or nonmetal products were derived comprised 71 percent (75 percent in 1964) of the total materials handled. Great strides were made in mining technology and there were indications that, in the future, earth moving by the mining industry will involve materials-handling systems of increasing size and complexity. Materials Handled.-Output of ore and waste at metal and nonmetal mines and quarries in the United States in 1966 totaled 3,518 million tons (3.213 million tons in 1965). The quantity of materials handled has increased at an average annual rate of 5 percent since 1962. Ore production increased 4 percent over that of 1965, also comparable to the average annual rate of increase in the last 5 years. Waste removal in 1966 increased 25 percent over that of the previous year.

At the microscopic laboratory of the Bureau of Mines Intermountain Experiment Station, Salt Lake City, Utah, efforts are being made to devise methods of differentially coloring finely crashed minerals as a means for their identification. Staining would facilitate sight recognition of minerals under the microscope and would be of great value in the study of crushed ores, concentrates, and tailings. Since the treatment of ores for their beneficiation is primarily based on the recovery of the minerals containing the clements of value, it is extremely important that these minerals should be known and that a method for quickly determining them when in minute fragments should be available. Herein lies one of the greatest uses for color methods. In many cases it is a difficult matter for even the trained petrographer and mineralogist to identify finely crushed minerals quickly. Although ordinary petrographic methods are accurate, they are often time--consuming and are not suitable to the needs of millmen who require, especially in the examination of tailings a method for determining minerals by inspection.

The Bureau of Mines has conducted research to determine whether the total elapsed delay time for blasting bituminous coal in underground mines could be safely expanded beyond the present 500-ms limitation without igniting a methane or methane-coal dust atmosphere. The results indicated that the increase of total delay from 500 to 1,000 ms had no detectable effect on safety relative to incendivity as long as permissible practices were observed in all other aspects. Research was also conducted to evaluate the safety of 18-in hole spacing for delay blasting in coal relative to misfires. For a variety of permissible explosives, misfires were observed for about 50 pct of the holes at 18-in spacing. Based on this observation, the Bureau has recommended that the new regulations prohibit hole spacings of less than 24 inches in underground coal mines. Misfires were also observed for 24-in spacing, but the number of misfires and test shots was too small to sup- port any firm conclusions.

The DuBois fluorite property in the southwestern part of Hardin County, Ill., has been active intermittently since 1904 and has produced an estimated total of 1,000 to 2,000 tons. The persistence of the vein and the stratigraphy suggested extensions of the ore body formerly mined. The Bureau of Mines investigated the property by core drilling early in 1944. Four holes were bored which totaled 1,106 feet and extended along a strike length of 1,150 feet. The data obtained in the course of the investigation are presented in this report.

Produce an annotated bibliography to serve as a comprehensive source of published information available on the use of soil amendments in coal mine reclamation and the characterization of coal mine soils. The Problem There is a substantial amount of information available tin coal mine reclamation and revegetation, particularly on the use of soil amendments to ameliorate site conditions and aid vegetation establishment, growth development, and productivity. Additionally, site characterizations based on chemical, physical, and mineralogical properties have been thoroughly researched. This information, however, is widely disseminated throughout various professional and scientific journals, State and Federal documents, proceedings of conferences, theses, and other technical writings. A critical need exists for a comprehensive and up-to-date summary of published information concerning the uses, methodologies, and types of soil amendments that arc necessary for reclamation success.
