Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

The choice of ventilation schemes in the Northern Mujsky railway tunnel and definition of the nec-essary quantities of air ensuring regulated radiation conditions in the tunnel system has been accomplished. The influence of natural draught on the stability of tunnel ventilation has been estimated. The design of a portal venti-lation gate allowing minimal influence from natural factors is described. The dependence of pressure loss in the sliding ventilation gates on their spanning value of the vehicle tunnel section has been determined. The selection of parameters for an atmospheric air heating system in winter has been carried out. The operation regimes of air heaters, dependent on the temperature of atmospheric air, have been established.

Anecdotal evidence suggests that re-entering a sealed heating poses a significant hazard, whereby the original heating is rejuvenated at an accelerated rate forcing withdrawal and re-sealing of the area. Bulk-scale laboratory experiments using a two-metre column confirm these mine site observations. A high volatile bituminous coal from New South Wales, Australia was allowed to develop a significant hot spot before being inerted with nitrogen and allowed to cool. Air was then allowed back into the coal after the nitrogen was turned off and a hot spot developed almost immediately at the same location as the original hot spot before it quickly migrated towards the air source within a day. Two more re-entries were simulated. Each successive re-entry developed a broader hot spot and took longer to reach thermal runaway. These results clearly show the behaviour that can be expected when trying to recover a section of an underground coal mine that has been sealed due to a spontaneous combustion event.

INTRODUCTION The Transbay-Caltrain Downtown Extension (DTX) Project involves the construction of an approximately 1.5-mile rail line that will extend Caltrain commuter service and the future California High Speed Rail system into downtown San Francisco. The underground rail extension will be constructed by mined tunnel and cut-and-cover methods. Tunneling challenges include difficult ground conditions, low rock cover, the presence of historic buildings along the alignment, and the large tunnel span, ranging from approximately 50 to 65 feet. Three mined tunnel construction methods were evaluated: the Stacked Drift Method, the New Austrian Tunneling Method/Sequential Excavation Method (NATM/SEM), and Tunnel Boring Machines (TBM). This paper describes the challenges presented within the mined tunnel section, and summarizes the evaluation of tunneling methods that led to the selection of NATM/SEM. PROJECT OVERVIEW The existing Transbay Terminal opened in downtown San Francisco in 1939. This facility is antiquated, and does not meet current building seismic safety standards or the transportation needs of the Bay Area. For these reasons, San Francisco voters approved Proposition H in 1999, which proposed to construct a new or rebuilt regional transit center at the site of the existing terminal and extend Caltrain commuter rail ser-vice to the new transit center. The new transit center is the hub of the Transbay Transit Center (TTC) Program, which will greatly enhance regional transit service by providing connectivity for six Bay Area transit providers at one facility. INTRODUCTION The Washington Metropolitan Area Transit Authority (WMATA) has let a contract for the excavation of three rock stations, spaced about 3550' (1082m) apart, beneath Connecticut Avenue in Washington, D.C. Two previous rock stations have been completed, one of which had a crown pilot tunnel excavated to explore geologic conditions that would be encountered during the station excavation. This pilot tunnel was primarily for inspection by bidders and design was based on core hole information. The other rock station had no pilot tunnel and was also designed on the basis of core hole information. The three rock station areas, which are the subject of this paper, have been explored by crown pilot tunnels and an as-built report, with geologic maps and sections, has been prepared, for use by the design engineer as the basis of design for support of the structures. The geotechnical report describes the geologic structure in detail and relates it to excavation and support of the three crown pilot tunnels and the running tunnels which will be enlarged to create the 58'W x 44'H x 700'(+)L (17.6m x 13.3m x 212m +) stations. The information was complete enough to permit the design engineer to visualize the geologic structure and compute loads for the various types of rock conditions. It was therefore possible to determine the size and spacing of the steel sets for the several different conditions of rock structure encountered in the pilot tunnels.

Accurate prediction of gold (or other metal) production from a heap leach operation is essential to the economic vitality of any such operation. However, this is also one of the most challenging issues facing managers of these facilities. Three approaches to modeling gold production from the Valley Leach Facility (VLF) at Anglogold’s Cripple Creek & Victor Gold Mining Company are presented. Part of a major sewer expansion for the City of Columbus, Ohio. The Big Walnut Outfall Augmentation Sewer (BWOAS) consists of 4,000m (13,200 ft) of 3.66m (12 ft)diameter tunnel passing mainly through glacial deposits with a water head of up to 2 bar. The project involves many new innovations, including: Slurry wall work shaft with excavation below the water table; Hybrid universal rings combining traditional steel reinforcement with fibers; New, full EPB Lovat TBM; Two component grout system pumped to the TBM from the surface; and, Linabond corrosion protection liner. The paper looks at the planning approach adopted to meet the various challenges presented, combined with an overview of progress to date.

This paper presents the results obtained by processing a gold and silver telluride mineral concentrate from a mine in Colombia. An acid wash, also known as acidification, was carried out, followed by oxidation with nitric acid as a pretreatment. Cyanidations of the pretreatment residues were carried out and the results were compared with results from cyanidations of base materials. The removal of carbonates in the acidification, a tellurium extraction close to 90% during the pretreatment, and a 30% improvement in the gold recovery compared to the conventional process were achieved.

The fact that methane ignitions continue to occur at the mining face indicates that monitoring with machine-mounted methanometers does not always indicate the presence of high methane concentrations. Methane concentrations at the face change quickly due to changes in airflow. By measuring these changes in airflow, it may be possible to predict changes in face methane levels more quickly. Currently there are no techniques or instruments to accurately measure airflow inby the mouth of the ventilation curtain or tubing. Tests conducted in the NIOSH ventilation test gallery compare the data provided by one-, two-, and three-axis ultrasonic anemometers. The effects of changes in airflow direction and turbulence on instrument readings are discussed, and guidelines are given for selecting the type of instruments to be used for monitoring airflow near the mining face.

In the United States, the U.S. Environmental Protection Agency (EPA) is set to implement its Tier 2 air quality standards governing high-horsepower, off-road diesel engines. The regulations take effect in January2006. In Europe, similar standards, called Stages, are due to take effect about the same time. And other markets, such as Japan and Australia, have their own standards on tap. Tier 2 standards will further reduce the amount of nitrogen oxide (NOX), sulfur dioxide (SO2), diesel particulate matter (DPM), carbon monoxide (CO) and hydrocarbons off-highway diesel engines are allowed to emit. And even more restrictive Tier 4 standards are due in 2011. The upshot is that operators of high-horsepower engines, particularly in mining, will begin using in their new off-road haul trucks, excavators, shovels and other mining machines diesel engines that are designed to meet these more stringent standards.

The advent of the more efficient and effective rare-earth roll (RER) separator in the early 1980?s began a new era of dry magnetic separation. The RER units quickly became the separator of choice within the materials processing industry. Though RER separators are now commonplace throughout the world, only modest advancements have been made to the original design. This paper details recent, more significant, rare-earth magnetic separation developments. These developments maintain the best features of existing technology while minimizing shortcomings. The discussion includes discerning features that aid in better metallurgical performance. Also discussed are improved mechanical aspects that allow for both easier operation and maintenance. Performance details from various test programs are included.

The Powder River Basin (PRB) of northeastern Wyoming and south-central Montana is the largest strippable reserve of compliant coal (0.6 or less lb of sulfur per million Btu) in the United States. In mining parlance, it is the Mother Lode! Wyoming’s portion of the Basin provides one fifth of the coal produced in the United States annually, and the state has been the nation’s top coal producer for the past six years. Based on 1993 production statistics and estimated strippable compliance coal reserves, Wyoming may remain the nation’s top producer for the next 76 plus years. Past articles on the PRB have focused mainly on production. After living and working in the Basin for the past five years, I have developed an appreciation for the area and its major industry that transcends size and production data. In the fall of 1993, MINING ENGINEERING’s publisher and I decided to present a series of articles that would cover many other aspects of the Basin. Ten topics were outlined, and specific individuals, selected for their knowledge and experience, were asked to contribute articles about their specialties. Although Montana has four large mines producing coal from the PRB, this series of articles will concentrate on the Wyoming portion of the Basin.

The Western Mining Resource Center (WMRC) at the Colorado School of Mines (CSM) received a grant from the National Institute for Occupational Safety and Health (NIOSH) to perform a study that will help reduce the amount of respirable dust generated from the cutting of coal. The study is based upon laboratory testing and field experience. This information is being used to develop a new cutterhead design in conjunction with Joy Mining Machinery. The new cutterhead design is being evaluated with existing computer models developed at CSM. The new continuous miner cutterhead will be tested at the Twenty Mile Coal Company’s Foidal Creek Mine. Baseline testing with the current cutterhead will also be performed. This paper presents laboratory test results, design methodology, and test results performed to date.

Good times make for happy miners. And the past few years have seen a lot of smiling mining engineers, geologists, metallurgists, corporate executives, analysts, investment bankers and shareholders. And nowhere was that more evident than at the 74th Prospectors and Developers Association of Canada’s (PDAC) annual meeting. The meeting, held in Toronto March 5-8, attracted a record 14,500 people. Of those, the PDAC said attendees came from an estimated 100 countries. And about 40 countries sent national delegations to the meeting. Argentina, for example, sent nearly 100 people. The PDAC meeting has always been a event where companies and individuals can get a look at numerous properties in all parts of the world that could potentially become major mines. And through the many receptions, hospitality suites and just sitting in the local restaurants and taverns, deals are usually made.

In underground coal mining, particularly room-and-pillar methods, the coal pillars play a significant role in roof stability. If the pillar dimensions are increased, the pillar bearing capacity also increases, but more coal remains in the pillar and therefore the recovery of mining operation decreases. Therefore, determining the optimum pillar dimension based on technical, economical and performance parameters is important. In this paper, the coal pillar is designed by using the concept of ground reaction curve (GRC). For this purpose, the GRC for coal pillars is drawn and then the elastic and plastic zone of coal pillar behavior is determined. Based on elastic and plastic zone, different equations are presented to draw a pillar reaction curve and the process of coal pillar design based on GRC is explained. As a case study, this new technique is applied to study the coal pillar state in Tabas coal mine of Iran. The results show that the presented new approach is successfully used in the design of the optimum coal pillar dimensions, and also determines the elastic and plastic behavior limit, which is important especially for the logical design of yield pillars.

City Water Tunnel No. 3 is being built in order to provide a redundant system for delivering 4,542,500 m3 (1.2 billion gallons) of water per day throughout the five boroughs of New York City. The present tunnels, City Tunnel No. 1 and 2 (Figure 1), were put in service in 1917 and 1936 respectively, and both have been in continuous service. Tunnels No. 1 and 2 can-not be shut down for inspection or repair until City Tunnel No. 3 has been completed. City Tunnel No. 3 has two stages (Figure 1).Stage One is 20.1 km (12.5 mi) long and is presently delivering drinking water to the boroughs of the Bronx and Manhattan. Stage Two consists of the Brooklyn-Queens Tunnel 17.7 km (11 mi) and the Manhattan Tunnel 14 km (9 mi). The Brooklyn-Queens leg is complete and is set to be put in service soon. The Manhattan Tunnel is being constructed and is the subject of this paper. The Manhattan Tunnel (Figure 1) is comprised of three legs: the north, south and east legs. The con-tract includes the excavation of 8,976 m (29,500 ft) of tunnel and the lining of 14,561 m (47,773 ft) of tunnel. The south tunnel, 5,705 m (18,720 ft), was excavated but not lined, under a previous contract. The tunnel ranges from 181 m (600 ft) to 152 m (500 ft) in depth in the bedrock under Manhattan. The tunnel contract also includes the construction of nine new shafts, 161 m (530 ft) average depth, and the completion of a tenth shaft (the previously built construction shaft). The Manhattan Tunnel contract was awarded to the sole bidder Schiavone/Shea/Frontier Kemper Joint Venture (SSF) for $668,532,680.00, on January 31, 2005. The Manhattan Tunnel was designed ?in house? by the New York City Department of Environmental Protection (DEP) and the construction management was provided by a combined staff of the DEP and a joint venture of Jenny Engineering and URS.

Sequential excavation of the Beacon Hill Station has been accomplished in highly complex and inter-layered sand, silt, and clay soils with multiple perched groundwater tables. Successful excavation has required the installation of a deep dewatering well system used to depressurize sand layers above and within the excavated tunnel faces. In addition, the presence of sand layers in the crown of the Platform Tunnels required jet grouting at depths of up to 49m (160-ft). This paper discusses the design intent, construction, and effectiveness of the dewatering system and the selective jet grouting of sand horizons and shaft break outs for the successful construction of the deepest SEM station excavation ever completed in the United States.

The Duval Corporation Sierrita copper and molybdenum mining complex is located approximately twenty-three miles south of Tucson, Arizona. The original open pit mine, Esperanza, was put into operation in 1958 and utilized one tailings dam until the Sierrita operation began in 1970, and a second pond was started. Both tailings operations continued to be operated separate and independently of each other until 1977, when the Esperanza Mill was temporarily shut down, and the Esperanza Tailings Dam was retired from service. Except for short periods of use, no new tailings were put on the surface. With the reactivation of the Esperanza Mill in 1979, the Esperanza tailings were combined with the Sierrita Mill tailings and put on the Sierrita Dam, leaving the Esperanza Dam in a retired state.

Equipment management embraces modern management techniques. These include reliability-centered maintenance, life-cycle costing, capital performance, the effective use of information and productive organizational models.

Ventiflex is a flexible ventilation duct system with a global market within mine and tunnel ventilation. The system is distributed in a wide range of qualities and dimensions. It is very important that ventilation systems are designed correctly in mines and tunnels. An incorrectly dimensioned system might result in bad air quality and too high costs. The objective of this study has been to assess some important design parameters for ventiflex comprising the friction loss factor for the duct wall itself, as well as the shock loss factors and the leakage factors for the different couplings systems. This has been obtained by carrying out a large scale experimental program on ducts with all the three different couplings: Velcro, zip and steel-ring. Pressure profiles and air flow rates have been measured, and based on these data the different factors have been calculated. The following results have been obtained: A friction factor l = 0,012 (K = 10) for the duct wall, a shock loss factor z = 0,0125 for velcro- and zip couplings, a shock loss factor z = 0,005 for steel-ring couplings, and a leakage factor f* = 0 mm2/m2 for all three coupling types. The latter result proves that no leakage is possible for a correctly installed duct with proper alignment. The results also confirm that ventiflex complies with class S, or better, according to the SIA-norm. However, whether ventiflex should be designed as class S, A or B according to the SIA-norm, depends on the particular application.
