Although researchers have struggled to replicate Heinrich’s safety triangle findings (Heinrich 1931) for various reasons (e.g., no access to his original database and it is unclear if the database was of a single establishment/operation or for an entire industry), the occupational safety community has continued to adopt and expand the applications of this theory (e.g., McSween 2003). Within the mining industry, the potential exists for operations and companies to use concepts from the safety triangle as a way to inform or challenge the practices employed within their health and safety management systems (HSMS) to prevent incidents (Backlund 2016). This paper considers data obtained from the Mine Safety and Health Administration’s databases and demonstrates the validity of applying the safety triangle theory at the mine level. The results support the use of this theory and demonstrate that lower severity incidents can predict fatalities in a subsequent year where significant nuances and caveats to applying the theory are identified and discussed.

"The National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Mining Research Division (PMRD) conducted underground evaluations in an attempt to quantify respirable rock dust generation when using an untreated rock dust and a rock dust treated with an anti-caking additive. Using PDM 3600s and 3700s, these evaluations measured respirable rock dust levels arising from a flinger-type application of rock dust on rib and roof surfaces. Rock dust with a majority of the respirable component removed was also applied in NIOSH’s Bruceton Experimental Mine (BEM) using a bantam duster. The respirable dust measurements obtained downwind from both of these tests are presented and discussed. This testing did not measure miners’ exposure to respirable coal mine dust under acceptable mining practices, but indicates the need for effective continuous administrative controls to be exercised when rock dusting to minimize the measured amount of rock dust in the sampling device. INTRODUCTION Two factors have combined to renew concern regarding respirable dust measurements and compliance. This renewed recognition is due to two regulations that have modified operators’ rock dust schedules. First, the 80% incombustible requirement in intake entries has increased both on-section and outby-the-section rock dusting. Secondly, the recent respirable dust regulation change that has increased the number of respirable dust samples taken per sampling cycle. Previously, when only 5 samples were needed bi-monthly, rock dusting could be scheduled to not interfere with the respirable dust sampling schedule. The significant number of samples now required under new regulations combined with the need to maintain 80% total incombustible content in active mining areas require the schedules for respirable dust sampling and rock dusting to intermix. This raises concerns about the amount of rock dust that potentially is included in the sampler weight gain. Additionally, the respirable dust maximum was lowered from 2.0 mg/m3 to 1.5 mg/m3 (effective August 1, 2016) increasing concern that rock dust contamination may be an issue. Currently, the acceptable practice is to apply administrative controls such as keeping the miners upwind during rock dust application or to schedule rock dusting during a maintenance shift when the number of miners present is lower. NIOSH’s research examining the quality of rock dust has led to the development of hydrophobic rock dusts."

The Office of Mine Safety and Health Research, Spokane Mining Research Division, is continuing its high-energy, high-displacement testing of field-scale shotcrete panels. A test program was developed to determine the relationship between applied force, displacement and energy for both unreinforced and reinforced shotcrete panels. Reinforcement options consisted of synthetic macro-fibers, sprayed polyurea liners, chain-link mesh, welded-wire mesh and combinations of them. During testing, photogrammetry was used to measure the geometric changes of the panels, including volume changes and panel cracking. These measurements were correlated with the load and displacement data, allowing visual observation to be related to the applied force and displacement. The test results provide a comparison of the mechanical performance of the various panel types and can assist in interpreting the condition of installed surface support. Visual assessment of the loading cycle and strength capacity of shotcrete in underground excavations will improve mine safety by providing a means to better quantify the stability of installed shotcrete support.

As directed by the MINER act of 2006, the NIOSH Office of Mine Health & Safety Research (OMSHR) has annually awarded contracts to enhance mine safety and health. OMSHR has a history of research that focuses on both respirable coal mine dust and respirable crystalline silica (or quartz) dust, and contracts have been awarded since 2010 in support of our intramural research efforts. In 2018, a National Academies of Science consensus study report recommended expansion of respirable dust research. As a result, beginning in 2018 through 2021, eighteen contracts were awarded focusing on this area. These contracts can be grouped in three topic areas: characterization of coal/dust, development of real time monitors, and improvement of respirable dust controls. This paper summarizes the outcomes of completed contracts and the progress of current contracts.

INTRODUCTION Today at LKAB, the primary mining method used is sublevel caving, independent of ore-body size, shape, dip, and geographical location. The mining of small ore bodies with relatively flat dip involves a lot of develop¬ment work, a large initial investment, big dilution, and large ore losses. In 1975 serious investigations into other possible methods, especially for those small ore bodies which do not lie in direct contact with other ore bodies, began. Different methods were studied and a number of in¬teresting ideas came from Russian mines. An analysis was made of one of the proposed methods, sublevel benching, in which mining is carried out from raises, and which uses the throw from the blast to place the broken ore. However, there were reservations about working in big open rooms at great depth, and doubts about the possibility of getting acceptable drilling results in an acceptable environment from raised platforms. Testing was therefore delayed for a time. However the investigations continued and in the fall of 1976, it was decided to try vein mining in a small ore body (Indian) (see Figs. 1 and 2). MINING RESULTS The results are now available from the mining of Indian and are as follows: Ore plan area 1200 m2 Level height 50 m Ore volume 60 000 m3 Ore tonnage 270 000 t Iron content 62% Dip 0.69 rad (40°) Ore thickness 14 m Ore compressive strength 26 MPa Ore C-factor (fracture density) 0.21 Hanging wall compressive strength 28 MPa Hanging wall C-factor 0.27 About 40 000 m of the holes were drilled by ring drills with an average productivity of 85 m/shift; loading of a round (2 rows, 750 m of hole) was usually accomplished in one shift. The total ore recovery was about 270 000 t with an average iron content of 59%, and three to four shifts were required per round (4700 t) to load. The principal of "throwing" the rock during blast¬ing worked well, and no ore remains on the footwall. ROCK CONTROL Before mining commenced at Indian, it was dis¬covered that the rock strength in its vicinity was rela¬tively poor. Since the ore is flatly dipping, and mining was to take place with open rooms, the strength of the hanging wall was of great importance. A supplementary program of exploration drilling was done during the fall of 1976 in order to assess fracture spacing and rock strength from the drill cores. Results showed that the strength of the hanging wall was low because of the many fractures. A decision was made to try cable bolting of the hanging wall by extending some of the ordinary blast¬holes. Cables (20 mm diam) were grouted in these larger holes and the rest of the hole was then utilized for ordinary loading (see Fig. 2). About 100 bolts altogether were installed and, to date, only small falls have occurred and the hanging wall is holding. Its exposed area is 2100 m2 DEVELOPMENT WORK On the 350-m level, four crosscuts (two per mining room), one longitudinal drift, and two elongated drifts (one per raise) for the raise climber were developed as shown in Fig. 1. Of the 230 m of drift, 60 m were in ore and the rest in waste. For mining room A alone, a total of 115 m of drift were needed with 30 m in ore. The drift dimensions were about 4.5 X 5.5 m. In the drifting on the 300-m level, two connecting drifts were driven from one existing transport drift forward to the respective raises. The total development was 93 m, all in waste (Fig. 1). The two raises having a length of 80 m each were driven at an angle of 0.68 rad (39°) from the hori¬zontal. They were driven along the footwall contact with a separation of 30 m and had an area of 10 m2. LONG-HOLE DRILLING The drilling of long holes with the ring drilling unit began in the fall of 1977 in raise A. The equipment consisted of an Alimak raise climber, equipped with a lower platform from which the charging was done.

"The kinetics of leaching of a saprolitic ore from Indonesia by citric acid solution under atmospheric pressure was investigated. An examination of the effects of leaching temperature, citric acid concentration, pulp density and ore particle size on the dissolution rate of nickel found that they all had significant influence on the rate. The highest nickel recovery (95.6 percent) was achieved under the leaching conditions of ore particle size of 212-355 microns, citric acid concentration of 1 M, leaching time of 15 days, pulp density of 20 weight/volume percent, leaching temperature of 40°C and shaker speed of 200 rpm. The shrinking core model was found to be appropriate for describing the leaching kinetics of this ore in citric acid solutions at atmospheric pressure. The experimental data were well interpreted by this model with rate of reaction controlled by diffusion through the solid product layer. Using the Arrhenius expression, the apparent activation energy for nickel dissolution was evaluated as 12.38 kJ/mol. Finally, on the basis of the shrinking core model, a proposed empirical kinetic model for the leaching of nickel from this Indonesian saprolitic ore was expressed as a mathematical model, which was verified as consistent with the observed experimental results.IntroductionIndonesia has some of the world’s largest nickel laterite reserves and was the largest producer of nickel – in the form of nickel lateritic ores – in the world in 2013 (Merchant Research and Consulting Ltd, 2014). However, only high-grade saprolites are processed in Indonesia using pyrometallurgical methods to produce ferronickel and nickel matte. Lower-grade saprolites and limonites are removed, becoming waste or overburden in mining site areas. Hydrometallurgical methods, involving either chemical leaching or bioleaching, are now seen as the most important technologies for treating nickel laterites in the future because of their advantage of not only extracting nickel and iron but also other valuable elements. In addition, hydrometallurgical methods consume less energy and have lower capital and operational costs. Although they are recognized to be more suited to limonites, some researchers have studied their use in the extraction of nickel and other metals from saprolites (Guo et al., 2009; Lu et al., 2013). Hydrometallurgical processes for the extraction of metals, especially nickel, promise to be meaningful alternatives to pyrometallurgical processes to primarily treat low-grade saprolites for which pyrometallurgical processing is not favorable"

Unit operations currently ignored because of their costs, will become mandatory to coal plant designers and operators to ensure maximum energy recovery and sulfur reduction in the entire coal stream. Test work conducted in the 6.4 and 10.52 m Aerofall Mills at the Iron Ore Company of Canada Carol Project showed that the conventionally used ?deflector?-type liners which form the basic principle of Aerofall Mill grinding did not necessarily provide the best grinding performance. A new design, using low profile liners incorporating radial ribs of a proper height, actually improved the mill throughput in the order of 9 to 20% without sacrificing grinding efficiency. In addition, the new design reduced liner weight and scrap losses, which resulted in a reduction of liner consumption by 25%. It provided better protection to the mill shell and reduced the overall maintenance cost for the mill

The Hampton Roads Bridge-Tunnel Expansion Project (HRBT), initiated by the Virginia Department of Transportation (VDOT), will add two new 1.5-mile TBM bored tunnels to the two existing immersed-tube tunnels between artificial islands, part of the connection between Norfolk and Hampton under the Hampton Roads Harbor in Virginia, USA. The new bored tunnels will be built by Hampton Roads Connector Partners (HRCP), a joint venture of Dragados USA, Vinci Construction, Flatiron Constructors and Dodin Campenon Bernard. This paper covers technical discussions that led HRCP to select a Variable Density TBM to address the challenges of this tunnel project, including but not limited to (i) the diameter of the bore: a 46 feet (14.04 m) diameter machine that constitutes the second-largest TBM ever used in the United States; (ii) shallow ground-cover under the water table; (iii) challenging and varying geology along the alignment, with mixed-face conditions and different types of ground modification zones; and (iv) the high TBM operating pressures required—all these and other challenges in a sensitive marine environment.

The title of my paper, "A Company's Response to Federal Coal Leasing," brings to mind a picture of a dog chasing its tail. This is what the coal industry, has been doing since the Federal coal leasing moratorium went into effect in 1971. We've gone around in many circles, but we are only very slowly, if at all, getting closer to catching our tails, i.e., efficient, economical, and rational Federal coal leasing. My companies, The North American Coal Corporation and its four Subsidiaries, The Falkirk and Western Plains Mining Companies and The Coteau and Missouri Valley Properties Companies, together hold in excess of 4.5 billion tons of lignite reserves in North Dakota. Some of our North Dakota properties are substantially interspersed with Federal coal, with the government holding title to the minerals in every other section. This checkerboard pattern of Federal ownership has made it difficult to develop and market some of our reserves, and, as a result, it has become imperative that we be familiar with and active in the Federal Coal Leasing Program.

The incidence of heat stroke in the South African gold mining industry is today small (less than 1 in 10 6 shifts worked). Acclimatization and selection procedures have been developed and applied throughout the industry, underground working conditions have, and are being improved, and personnel are to-day made more aware of heat stress and its consequences than ever before. Although these actions have, over the years, brought about a steady reduction in the incidence of heat stroke (see Figure 1) the small incidence of heat stroke which persists indicates that heat stress is still a problem. This realization provides the stimulus for the work presented in this paper, which has as its primary objective the development of reliable heat stress limits for men working in South African gold mines. [ ] In keeping with recent trends3I4 the approach followed in this paper is based on the physical principles of heat transfer. Slurry densities and holdup were measured for open circuit grinding tests in a laboratory overflow ball mill of 0.3 m diam by 0.6 m long, by emptying and drying after stoppage of the mill. Contrary to some other results in the literature, there was negligible variation of slurry level in the mill as a function of flow rate over a range of increase of flow rate of about 5 to I. However there was a gradual increase of solid holdup at higher flow rates because the slurry density in the mill increased. Equations are given for holdup and slurry density in the mill as a function of over flow diam, ball load, and feed slurry density. The average level of slurry necessary to give over flow was found to be much higher than that necessary to fill the mill to the overflow level at rest. The total level of balls and slurry was only increased slightly by ball loads higher than the overflow level. The overflow diam appeared to be the major controlling factor in determining the mill holdup.

The performance characteristics of a multigravity separator for the recovery of heavy minerals of lead, zinc, copper and silver from a lead rougher-flotation concentrate were studied. Factorially designed experiments were carried out on three operating variables, i.e., the amount of wash water, the drum inclination and the drum rpm, of a multigravity separator. The experiments were carried out at two levels of each variable. With the experimental data, regression equations were developed for different metal grades and recoveries. The analysis revealed that drum speed has a major influence in determining the heavy-metal grade and recovery. Drum inclination is the next most important variable, with the amount of wash water being the least important. A concentrate containing 40% lead, 9.4% zinc, 1.5% copper and 1,400ppm silver was obtained with recoveries of 74.8%, 65.9%, 64.1% and 53.2%, respectively. Finally, the predicted values of grades and recoveries at intermediate levels are compared to actual experimental data to test the validity of the equations developed.

The Homestake Gold Mine in Lead, South Dakota, commenced operations in 1876. As is the case with old, deep, continuously developing mines, a ventilation improvement is periodically required to maintain acceptable underground environmental conditions. Recent historical improvements at Homestake include a 5.2m diameter air shaft driven in the 1960's and a 2.1 MWR (580 ton) chilled water plant installed in 1978. By 1983, it became apparent that yet another ventilation upgrade would be needed to keep pace with accelerated development in the deepest mining sections. A study was therefore undertaken to project ventilation requirements for the next ten years and to select the method(s) most suitable to meet those requirements. The study called for a two-phase approach in which air flow and air conditioning were carefully balanced. Phase I, the installation of a new 2240 kW surface fan and several underground circuit changes, was completed in 1986. This paper describes Phase 11: the planning, design, installation and initial performance of the air-conditioning portion of the upgrade.

The Massachusetts Water Resources Authority is currently improving its primary water supply system for the Boston Metropolitan area. The improvements planned by the Authority, which provides water for 46 communities and over 2.5 million people in eastern Massachusetts, include the MetroWest Water Supply Tunnel (MWWST) to provide redundancy in the transmission system, a large capacity water treatment plant and extensive covered storage facilities. The 28.3 km (17.6 miles) long, mostly 4.3 m (14 ft.) I.D. MetroWest Water Supply Tunnel will allow repair of the aging Hultman Aqueduct, which for 56 years has served as the transmission main from the water supply reservoirs to Boston. On completion of the tunnel, the old pipeline aqueduct will be repaired and brought back into service as a parallel, redundant transmission main. This paper describes design considerations and the design process for pressure tunnels in general and the MWWST in particular. It provides an overview of how the design of pressure tunnel and shaft components progresses.

This paper deals with some aspects of concrete construction in remote locations and/or cold climates. The writer's experience covers 25 years in construction in western and Central Canada. I will bring forth some pertinent observations and cost facts learned from this experience. Let us start by saying that there is nothing more important than preplanning the entire project. By this I mean the production of the engineering design, working drawings, soliciting of prices and ordering of all equipment and contractors work, the delivery of all equipment and all onsite work. All of this should be placed on a master schedule, preferably a C.P.M. We have been using C.P M. Scheduling for all of our work for 5 years and have found it in-valuable. This preliminary schedule will show initial items of work and target dates. After main equipment orders are placed and deliveries confirmed, the construction schedule can be revised to include manpower and construction equipment requirements. From this information the size of camp is determined. This, of course, must be ordered several months ahead. The manpower schedule can be broken into trades to tell us the number of men of each trade required at any time. This is important in helping to determine where the men must come from and whether or not any incentive is needed to get them. The schedule will also point out if it is impossible to get the project closed in before winter becomes too severe unless overtime is worked or the project is started in early spring. In most parts of Canada cold weather starts in October and lasts through April. A common term of reference has been to wait for the frost to come out of the ground before starting work in the spring. The Chaucha porphyry copper/molybdenum deposit was discovered by a United Nations team of geologists working with a ?counter- part" team of staff of the Ecuadorian government's General Directorate of Geology and Mines (DDGM) in July 1968. The exploration program formed the latter part of phase I work plan of a two-phased exploration project financed by the United Nations Development Programme and initiated in late 1964. The author was a team geologist during the period 1965-1967 and subsequently project manager for the period 1968-1972. The Chaucha property lies some 10 km (6 miles) east of the north- south-running western boundary of what was known as Operation Area No. 5. This area of some 10 500 km2(4050 sq miles) is a square truncated to the southeast and centered on the city of Cuenca, the major provincial capital (Azuay Province) of southern Ecuador. The eastern half of the exploration area is dominated by the Cretaceous- Tertiary sedimentary valley (basin) of Cuenca (Caar-Azuay) and its related drainage area. The western half, in which Chaucha lies, is dominated by the high Cordillera Occidental volcanic plateau and the western erosional valleys of the Andes draining to the Pacific. The valley that dominates the eastern part of the area [I500 m (4920 ft) above sea level] is transected by the Pan-American Highway. Because of increased agricultural activity associated with the Cuenca Valley, the area is reasonably well covered by a road network of varying quality. The high western area up to 4000 m (13,120 ft) in altitude is largely barren and completely inaccessible by vehicle. The geological reconnaissance map of the region is shown in Fig. 1.

The paper under discussion provides an excellent and concise study of compartment ergonomics for thin seam equipment. However, in accomplishing this, some very important operational aspects were ignored. Since these are likely to be determinant, the authors' response is solicited. The novel shuttle car with its cross-car, end-mounted compartment is operationally very different from the current one. The latter can be considered a highly mobile extension of the continuous miner or loader, with a pass-through conveyor, dump boom, and extra storage. As such, the vehicle's kinematics are the simple "I," or shuttle pattern, bent for going around pillars, with an intermediate stop at the "change-out point" (Adler, 1986 ). The action of the cable reel adjusts well to this pattern. The proposed shuttle car's motion either superimposes a "Y," time is lost (5 seconds) for each cycle, space is needed, line of sight is compromised, and a cable anchor is probably required at the dump. A chute is almost prohibitive in construction time and cost. The lack of a dump boom further curtails flexibility in the proposal. The value of a standard hand and opposite standard hand configuration also needs consideration. Many front-end loaders have a rocker (overshot) capability that is designed to achieve an "I” pattern. And crawler loaders sometimes use a side dumping bucket variant for the same purpose. These examples indicate the pattern's importance. For underground, thin seam application, the ram car has become popular. This is an articulated truck, battery-powered, and with an ejector dump. Its operational patterns would be similar to the novel shuttle car, except without the loading flexibility provided by a conveyor. Since the cab location of the proposed shuttle car largely obviates some of the major advantages of the present shuttle car, the question arises whether this configuration could be adapted to the current ram car and scoop
