"Introduction In studying the mine disaster files of the Bureau of Mires for preparation of a bulletin on coal-mine explosions, a table was compiled covering various features of fan installations and the handling of air currents at the time of and after the disasters. As a sidelight on this discussion, there was tabulated the loss of life and number of men injured.Discussion of collected dataOf the 164 disasters tabulated, 148 were explosions, 14 were fires, and 2 involved both fire and explosion. The total loss of life was 4,158 and 412 were injured. There were 145 disasters in bituminous coal mines, 13 in anthratice mines, 2 in lignite mines, 1 in a cannel coal nine, and 3 in metal mines. By States, 35 took place in Pennsylvania; 17 in West Virginia; 16 in Oklahoma and Alabama; 15 in. Kansas and Illinois; 12 in Indiana; 7 in Colorado; 6 in Kentucky; 3 each in Ohio Tennessee and Wyoming; 2 each in Arkansas, Montana, Washington, Virginia, New Mexico and Utah and 1 each in Iowa, Missouri, Maryland, and California."

In the late 1970's and early 1980's a considerable number of large scale open pit mines were developed in N.S.W. and Queensland. At the time these mines were planned, the industry was characterised by rising coal prices, rising fuel prices, and a host of other labour and government constraints different to the situation today. Yet some mines have clearly been able to cope with this change much better than others. While one mine may now be expanding and profitable, its neighbour may still be labouring under painful adjustment processes. This paper examines the major reasons for this difference. It suggests more robust criteria for assessment of future projects to reduce their sensitivity to changes outside management control.

Labour protection, that is prevention of occupational disease and reduction in the frequency of accident, has always been a matter of major concern of mining industry. Management and the government should promote and maintain high safety standards through some measures to reduce accident frequency and occupational disease. This can be achieved by using the science and technology for improving the working conditions, educating workers and managers about accidents and prevention techniques. The detailed analysis of an accident requires knowledge of many factors such as location, time, type, cost of accident, victim, nature of the injury, personal and environmental factors etc. that can be obtained from the standard coded accident report form. There is a need to prepare safety data in a common format to produce a common safety reporting system and ‘Occupational Health and Safety Management Tool’ is being designed to manage this by using database and Internet technology. Precautions should be taken according to results from database queries by providing comprehensive and accurate national data. To have an accident is unfortunate, however, to have an accident and learn nothing from it is unforgivable. The El Boleo Copper Project is currently in the final design stages for development of a 3-MM-tonne/yr underground copper mine located in Baja California, Mexico. The nearly horizontal mineralized zones are amenable to exploitation by conventional mechanized coal mining technology using continuous miners, shuttle cars, and belt haulage. The mine design resembles a typical full-extraction coal mining operation utilizing a sequence of mains, sub-mains and production panels. Due to numerous faults, mining will be conducted as several small adjacent mines accessed through adits from large arroyos cutting through the mine area. Up to four seams may be mined in some areas with a maximum overburden depth of about 250 m. The ore zone is relatively weak and plastic, subject to extensive creep over time. The mine workings will pass through areas of historical shortwall mining panels where the ground has flowed into old entries and gob areas, and then reconsolidated. A small test mine was developed to evaluate mechanical cutting performance, roof-bolt anchorage capacities, roof and pillar stability and caving behavior. In-mine measurements were combined with two- and three-dimensional FLAC models to evaluate alternative development and production pillar designs. The paper will discuss the various factors influencing ground stability at the El Boleo project and the strategies employed in developing a suitable ground control design.

COLUMBIUM (niobium) and tantalum are similar refractory metals with closely related properties and uses and close association in ore deposits. Until recently both metals, particularly columbium, were essentially laboratory curiosities. However, since the beginning of World War II, applied technology, under the impetus of national security requirements, has realized many chemical, electronic, nucleonic, and missile and rocketry applications, where high temperature or corrosive environments require the use of columbium or tantalum. At the same time, advances in these fields have resulted in intensive research on the geological distribution, resources, metallurgy, and properties of these two metals; their alloys and compounds; and the development of a full-fledged columbium-tantalum industry. The future appears to promise continued acceleration in such studies and ever-increasing demand for these elements. The raw material supplies of columbium are abundant, and the extractive metallurgy of columbium is well enough advanced to assure adequate supplies of the element at reasonable prices to meet any projected demands. Although the extractive metallurgy of tantalum is similarly well developed, raw material supplies are so limited that, lacking new ore discoveries, an increase in demand to five times the 1959 level would severely tax ore-production facilities. Approximately 120 tons of tantalum and about one-half as much columbium was produced in 1959. Additional quantities are consumed as compounds or additive alloys such as ferrocolumbium and ferrotantalum columbium (55).3

This paper investigates the use of backfill zones and yielding pillars for subsidence control. The investigation begins by choosing an appropriate subsidence predictive technique to simulate the proposed subsidence-control schemes. Two case studies are performed in order to delineate the accuracy and utlility of the chosen predictive technique and then the results of these case studies are used to establish appropriate sets of regional site parameters. Next, a detailed analysis of the major variables affecting the outcome of the subsidence-control simulations is conducted and graphical displays of the exact sensitivity of the surface subsidence to these variables are given. Finally, alterations to the surface subsidence that are expected from using two major subsidence-control techniques are presented. The first technique to be assessed is a backfilled pillar centered longitudinally in a panel. With this arrangement, a minimal amount of backfill material can produce substantial decreases in vertical displacements. The second technique to be assessed is the use of yielding gate pillars to reduce the sinuosity of the transverse subsidence over adjacent panels. With proper design, this technique can greatly curtail surface strains and help minimize remedial drainage control.

"The On-Stream Analyzer has proven and is still proving to be an extremely valuable tool for the control of flotation circuits. This paper examines the possibility of going further in the utilization of the provided assays using a real-time material balance program to calculate the distribution of solids within the streams of a circuit. Mass balance results can be used for process control, process analysis and detection of sensors malfunction. Examples from 3 Canadian and one American concentrators are presented to illustrate the potential of the method.IntroductionThe On-Stream Analyzer (OSA) has gained, over the last two decades, the status of key instrument for the control of flotation plants. The OSA is currently used in almost every sulphide flotation plant and its application in oxide mineral flotation plants is increasing. On-Stream Analyzers provide X-rays pulse counts that are converted into assays using multilinear regression programs. The estimated assays are passed to the flotation operators, via an operator console, or to control loops of various degrees of complexity (McKee, 1991) implemented in a Distributed Control System (DCS). Operators and/or controllers take control actions based on these assays to ensure an adequate operation of the plant. Although this application pays off very rapidly for the purchase of the OSA, one should ask: Could we get more from the OSA?Some plants reported the use of OSA measurements to process optimization (McKee, 1991). The optimization strategy is based upon economic indices calculated from metal recovery using the 2 product formulae (Wills, 1992, page 124). The 2 product formulae is however sensitive to measurement errors in the assays and can lead to considerable scatter in the calculated indices (Hodouin et al., 1993). Least squares algorithms provide an interesting alternative to filter out measurement errors (Spring, 1992). This paper presents a recursive least squares algorithm that estimates in a real time fashion the mass flow rates in a flotation plant using OSA and mass flow measurements. The algorithm is robust and its recursive structure is not demanding in terms of computation time and memory. The method was tested with data from four concentrators. Results of the material balance algorithm prove to be useful to detect sensors malfunction and may potentially be used for the control and optimization of flotation plants.The paper is divided into two sections. The first section gives a description of the proposed method. The mathematics of the method are kept to a minimum, since the detail is presented elsewhere (Bazin and Franklin, 1994). The second section shows the application of the method to different flotation plants."

Microtunneling provides a necessary solution for multiple utility crossings associated with the New York Metropolitan Transportation Authority Capital Construction Company?s (MTACC) Eastside Access Project for the Long Island Rail Road. Construction of four TBM tunnels and their approach structures at the eastern end of the project in Sunnyside, Queens required significant modification and expansion of the railroad infrastructure at this location. Maintenance of railroad traffic during project construction is a principal requirement. As a consequence fourteen (14) utility crossing segments are being installed under active traffic by microtunneling methods. Microtunnels were installed in a variety of conditions including depths, types of soils and construction restraints.

The perforating of multiple tubingless completions, in which two or more strings of 27/8-in. OD casing are installed in the same borehole, presents two basic problems. First, good completion practices require an eficient perforator that leaves no debris to interfere with subsequent completion operations. Second, and of more complicated nature, a system is needed for controlling the direction of fire so that adjacent strings are not damaged. Development of of a 2-in. OD, steel retrievable, shnped-charge gun has solved the first problem. The second was resolved through development of three different devices to provide directional perforating for the two types of completion methods being employed today. These include a mechanical orienting device and two self-orienting radiation devices. Conceivably, the latter methods could be adapted to the directional perforating (of .upper zones) of conventional dual and triple completions without removing production tubing and packers. INTRODUCTION The recent trend to multiple tubingless completions,' wherein two or more strings of 27/8-in. OD casing are installed in the same borehole, has presented two basic perforating problems. The first problem to be overcome was the design of a gun that would result in efficient perforations without causing debris to bridge in these small-diameter casings. The second problem was, of course, to devise a system for controlling the direction of fire such that adjacent strings would not be damaged. The problem of gun debris in slim-casing completions has been well defined in the numerous 27/8-in. single tubingless completions which have been effected during recent years with the expendable-type perforators. Failure of expendable-gun debris to settle out properly, even with good gun break-up, has resulted in bridging inside these small-diameter casings. This, in turn, has interfered with completion and re-conditioning operations. The past two years have clearly shown, in fact, that debris problems are not confined to this small range of casing with most designs of expendable-type guns. A high incidence of bridging has even been reported in 4%-in. OD casing. The perforating of multiple tubingless completions has entailed, therefore, the development of a completely debris-free gun of adequate performance, as well as the necessary devices to provide positive directional perforating in the two completion methods presently being employed. THE GUN —DESIGN CONSIDERATIONS The design of a shaped-charge gun for slim-casing perforating has presented a maze of interrelated prob-lems. Since the primary specification has been leveled at efficient charge performance with complete elimination of gun debris, it has been necessary to develop a retrievable steel-carrier gun of tubular construction to house the charges. The carrier must be retrievable through 2.29-in. API pump-seating nipples which are usually installed in 2 7/8-in. strings. To provide for recovery of the gun from the well, swelling of the carrier on firing must be carefully controlled. Control of swelling automatically means control of explosive weight in the shaped charge which, in turn, influences charge performance. Carrier swelling also depends upon the confinement pressure at the level being perforated, which imposes a hydrostatic pressure-depth limitation. Burr interference on the wall of the carrier, resulting from passage of the jet through the carrier wall, must be eliminated by suitable design. Clearly then, a satisfactory shaped-charge gun design for slim-casing operations represents a fine balance between carrier swelling, hydrostatic pressure, explosive weight and charge performance. Additionally, to lend itself to directional perforating, the gun design must provide a positive locking mechanism for individual charges within the gun carrier. Changes in the configuration of late Cenozoic volcanic arcs related to Pacific-Australian plate convergence in northern New Zealand may have localised large-scale epithermal gold-silver mineralisation to a particular part of the volcanic arc system, the Coromandel Volcanic Zone (CVZ). The history of arc-type volcanism in northern New Zealand began with the development of the continental margin Northland Volcanic Arc from early-mid Miocene (26-15 Ma). From 18-15 Ma the Northland Volcanic Arc extended south into the northern and western part of the CVZ where it is dominantly composed of andesite with lesser dacite and very minor rhyodacite. In the south, centre and east of the CVZ, younger andesite-dacite volcanism overlapped in space and time (14-5 Ma) with voluminous rhyolitic volcanism associated with rift-hosted calderas and minor basaltic eruptions. This change to bimodal rhyolitic-basaltic volcanism and extensional tectonics was a result of southwest (SW)-directed propagation of the Colville-Lau oceanic island+ arc into the north-northwest (NNW) trending Northland Volcanic Arc and its underlying continental crust. The extensional tectonic regime created in the collision zone between the two arcs is also the site of the 14-6 Ma Coromandel epithermal gold-silver province, which contains some 50 separate deposits and has a production (1862-1996) of about 300 t Au and 1150 t Ag. Extensional tectonics facilitated the emplacement of silicic magma at shallow crustal level and led to reactivation of deep basement faults. The resulting enhanced heat flow and fault-focused fluid flow localised large hydrothermal systems and associated extensive low-sulphidation epithermal gold-silver deposits in the southern part of the province, as at Waihi, Golden Cross and Karangahake.

Twin bore running tunnels driven approximately 4,500 route feet by a Mitsubishi Heavy Industries (MHI) EPB Tunnel Boring Machine link the West Portal and East Portal on this complex Light Rail Project. One third along the way, the TBM arrives at the Beacon Hill Station which is the deepest underground “binocular” station of its kind in North America and then “walks-across” the station to be re-launched on the other side to complete the drive. This paper discusses briefly the layout and design considerations but then focuses more deeply on the approach by the Contractor to drive and supply the tunnel. The paper addresses the approach used to launch, re-launch and recover the TBM and touches briefly on muck volume control and monitoring plus settlement monitoring and the performance achieved. Also of interest is a unique solution to a difficult launch situation involving a secant pile tieback wall designed with several rows of double I-beam steel walers spanning the eye at the West Portal under the I-5 freeway.

"In regular CIL circuits, activated carbon is used for two purposes: avoiding solid/liquid separation after cyanide leaching, and minimizing gold losses in the tails. The carbon distribution and gold profiles in the successive CIL stages are operating variables, not objectives. When the CIL feed contains a preg robbing constituent however, such as native carbon, there will be competition for gold cyanide and this conflicts with minimizing of losses of gold to CIL tails. This paper examines some of the limitations of activated carbon in CIL circuits handling preg robbing Carlin Trend ores.INTRODUCTION Carbon-In-Leach (CIL) has been a great success treating regular gold ores, but the technology is still challenged by preg robbing feedstocks. Little information is available in the gold milling literature on CIL gold extraction from “free” milling gold ores which are preg robbing, or on autoclave discharge (POX’ed) slurries of double refractory ores. Neither feedstock is readily available for experimentation. No literature reference could be found describing deliberate modifications to CIL operations or CIL circuits due to the presence of two types of carbon: “native” and activated. There is some literature on the gold cyanide adsorption activity of native carbons (Schmitz, Duyvesteyn, Johnson, Enloe & McMullen, 2001). Various tests have been devised to measure this activity, commensurate with CIP and CIL practices (Tretbar, Arehart & Graves). In this paper, the term “native” carbon indicates (severely) preg robbing carbonaceous materials found in gold ores. There is considerable literature on the gold cyanide adsorption activity of man-made “activated carbons”. Various tests have been devised to measure this activity, commensurate with CIP and CIL practices. These tests are comparative, i.e. results relate to a standard carbon and depend on the test methodology and carbon history. The test results are equilibrium oriented, hence of limited value to CIL circuit designers or plant operators handling preg robbing solids; in CIL circuits most carbons are not loaded to capacity. Information on gold cyanide adsorption and desorption kinetics might be more relevant and that is one subject explored in this paper."

The Otago Schist is crossed by numerous deformation zones that formed in the latter stages of metamorphism and uplift of the metamorphic belt. These deformation zones are long (tens of km) linear features at the regional scale, and are generally narrow (km scale or less). Many of these structural zones separate schist domains with different rock types and structural/metamorphic histories. The Hyde-Macraes Shear Zone is one such deformation zone, and this zone hosts the active Macraes gold mine. The Rise & Shine Shear Zone, 80 km NW of Macraes, is also enriched in gold and has been mined historically. Mineralisation in both these shear zones occurred during the transition from ductile to brittle deformation as the schist belt was uplifted from greenschist facies metamorphic conditions. Recrystallisation of quartz and micas in the early stages was overprinted by cataclasis, with sulfide and gold precipitation throughout. Many of the other late metamorphic deformation zones formed under similar greenschist to sub-greenschist facies conditions, with similar overprinting of recrystallisation textures and folds by more brittle textures such as breccias. Under these conditions, originally regionally pervasive deformation at the highest metamorphic grades was evolving towards more focussed high strain zones at lower temperatures and pressures. Shear zones initially focussed fluid flow along microshears and mineral grain boundaries, resulting in 1 to 100 m scale alteration and replacement in the host schist, with only local development of quartz veins. Fluid flow was not controlled by open fractures, and the rate of fluid movement was probably slow and pervasive. After uplift into the brittle regime, the schist belt underwent tectonic extension. Extension was accompanied by development of swarms of steeply dipping veins, with localised mineralised normal faults. Hydrothermal fluid flow throughout this uplift history was a normal consequence of late stage devolatilisation of the metamorphic belt. Hence, all late metamorphic deformation zones should be considered prospective for gold accumulation.

This paper will describe the research approach being initiated by the Spokane Research Center to assess hazards and design criteria to prevent ground and muck falls in and around ore passes in hard-rock mines. The major goal of this research is to compile detailed information about ore pass processes and hazards, identification and testing of prevention strategies, and development of computer-based measurement and design methods. Recent ore pass failures have underlined the lack of OE pass design capabilities and standards available to both mine engineers and MSHA personnel. Design criteria and ore pass hang-up prevention and remediation strategies will include the effects of internal ore pass loads, blasting to relieve hang-ups, and rock mass stress redistribution and structural interaction from nearby mining. New visualization and three- dimensional analysis methods will be presented as an extension of existing ore pass design and analysis techniques. A sophisticated particle-flow analysis program will be used to simulate the response of various ore pass designs to a wide range of loading conditions driven by rock mass deformation, muck flow, ore pass blockage, and blasting in both nearby stopes and the ore pass. Analysis techniques will be calibrated and design criteria validated at the Spokane ore pass test facility. These tools will serve to reduce research costs by limiting the number of experimental field tests required.

Subsidence phenomenon as an unwanted consequence of underground mining can cause problems for environment and surface structures in mine area. Surface subsidence prediction for inclined and steep seams has been given less attention than horizontal seams due to the difficulties involved in the extraction of such coal-seams. This paper introduces a new profile function method for prediction of surface subsidence due to inclined coal-seam mining. The results of calculation with the new function indicate that the predicted value has good agreement with the measured data. Over the past few years developments in heavy minerals processing have concentrated on the mineral separation portion of the business. The best example of this would include new developments in rare-earth magnetic separators and associated flowsheet modifications. Except for some minor modifications in spirals, the æwetÆ side of the separation business has lagged behind the ædryÆ side. Basic concentrator design has not changed in 20 years. Recent pilot trials by Outokumpu Technology have shown the benefits of a new gravity flowsheet that combines density separation and spiral concentration on a variety of heavy mineral deposits. This concept is already well utilised within the iron ore industry. The synergistic combination of these two technologies allows smaller, simpler, and controllable gravity concentrators to be built while providing equivalent or better grade and recoveries of valuable heavy mineral. This is contrasted to the all spiral flowsheet characterised by the diminishing returns of multiple stages of spirals and associated recirculating loads. Examples of the flowsheets and associated benefits will be presented from a variety of cases.

The development of information concerning mesothelioma in South Africa started with the 1959 finding of Dr. J. Christopher Wagner of cases of mesothelioma in persons living in the vicinity or working in crocidolite mines in Cape Province. After more than 40 years, the data from South Africa now provide important insights into the differences in mesothelioma risk associated with different asbestos fiber-types. Three case series reports and one analytical report were reviewed in this session. In total, information on 504 proven cases was presented. There were 28% of cases arising from occupational exposure in mining, 35% from non-mining occupations, 23% from environmental exposures; 14% where no exposure was established and 9% where the exposure was unknown. Among the 129 cases for which occupational exposure and asbestos fiber-type were established, 115 cases were associated with occupational exposure to Cape crocidolite; 1 case associated with occupational exposure to Transvaal crocidolite and 9 cases with occupational exposure to amosite (grunerite asbestos) and 4 cases with occupational exposure to mixed fiber-types. There were no cases associated with chrysotile asbestos. There were 33 cases of environmental mesothelioma linked to Cape crocidolite exposure and 2 to exposure to amosite and crocidolite but none to Transvaal amosite alone or to chrysotile. The evidence shows that amosite is capable of causing mesothelioma but that the major risk in South Africa is associated with Cape crocidolite.

Hatworth Colliery, operated by RJB Mining (UK) Ltd, is successfully mining the Deep Soft Seam at a depth of 1000m using single face retreat longwalling. The seam is characterised by a weak roof generally comprising 4 to 6m of well bedded carbonaceous mudstones overlain by weak siltstones. The paper describes the layout and support design strategies which have enabled the colliery to adopt sole support by rock reinforcement techniques for its longwall gateroads. To achieve this success the colliery layout has had to be radically modified to take account of the in-situ horizontal stress field. The layout is also closely controlled by environmental considerations, the seam being one of the gassiest in the UK Economic support by strata reinforcement has been achieved using a combination of rockbolting of the roof and sides installed at the face of the heading, and systematic double birdcage cable bolting installed on drivage and supplemented ahead of the retreating panel. The layout and support strategies have been developed through a combination of detailed underground measurement of support and stress parameters backed up by numerical modelling using FLAC. The FLAC modelling has paid particular attention to careful representation of bedding properties and validation through underground stress measurement.
