"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Breakthroughs in software technology and in computer hardware capabilities have opened new opportunities to develop more accurate process simulators and to reduce the operating time to reasonable levels. As a result, there are several new simulation packages being commercialized for modeling coal preparation plants and pulverized coal-fired power plants, and the users of these packages are reporting significant improvements in process efficiency and operating costs by implementing the simulators' results. This paper provides an overview of some of the models currently in use at CQ Inc., including the ASPEN PLUS"" Coal Cleaning Simulator, the Ultramax® process optimization software package, and EPRI's Coal Quality Impact Model?. Case histories of specific applications are also presented.

Lithium is a top-quality mineral in the energy market, with growing demand. It has many applications in medical, nuclear, and energy industries. The purpose of this article is to quantify and analyze the economic potential and projected global demand for lithium annually. It aims to determine Peru’s (specifically Puno) positioning in the global context in 2025 and 2030. The article also seeks to identify the main regulatory, environmental, and social challenges to sustainably exploit lithium in Peru. The study employs applied research (mixed approach), descriptive-correlational scope, and observational, crosssectional design. The methodology demonstrates the use of statistical techniques to model and make predictions about this energy-transition metal. Academic sources were reviewed to enable a comparative approach and projection into the future of lithium for development in the mining industry. In the last 10 years, lithium demand has grown, noting a 600% increase in the economy and projected annual growth of +26%. Exponential growth in resources is projected, where econometric, R-multivariable and R-linear models estimate that Peru could supply 1.5% in 2025 and 3% in 2030 of global lithium. Estimated growth of 5–15% in socio-environmental conflicts is projected with the start of lithium exploitation (Peru – Puno). The global potential of lithium demonstrates exponential growth. Lithium reserves in Puno - Peru would rank 13th between 2025 and 2030. It is necessary to establish a clear regulatory framework to avoid excessive increase in socio-environmental conflicts.

Wollastonite is recognized as one of the most unique, non-metallic minerals in the world. Its history for use in industrial applications is recent compared to other minerals, however, its distinctive physical properties, such as acicular particle shape, and alkaline pH creates incorporation possibilities ranging from bowling balls to automotive fascia, from ceramic tiles to automotive brakes, and from thermal insulation board to industrial and trade sale coatings. Its future use will be impacted by the exploitation of deposits which will ensure maintenance of the acicularity and purity of this naturally occurring mineral. Additionally, closely held surface treatment technology will open up additional niches requiring highly engineered performance.

"There is certainly no argument that LED lighting systems have taken a strong foothold in the lighting marketplace over the last several years. This has even proven true in the world of transportation and transit lighting, for which new LED cobra head style luminaires are replacing traditional HID cobra head luminaires on North America’s roadwaysand linear LED light fixtures are replacing conventional linear fluorescent light fixtures in North American rail transit facilities. However, LED technology has taken a while longer to penetrate the very specific market of vehicular tunnel lighting in this continent. Though both the New York/New Jersey Holland and Lincoln Tunnels have recently been retrofitted with LED lighting, and an LED Lighting RFP is currently out for Boston’s Central Artery Tunnels, very few vehicular tunnels in North America to date have been illuminated with LED light fixtures. This is largely because cost effective LED lighting, that met our typically stringent tunnel lighting standards, was until recently simply unattainable. However, with the rapid advances in LED technology over recent years, owners and specifiers of North American tunnel lighting projects have finally begun to adopt LED lighting systems as the norm. In fact, a recent Lux Fit/Yole Development “LED in Road and Street Lighting Report” stated that “LED luminaire growth will be driven firstly by tunnel lighting, and then relayed into highway, road, residential and amenity lighting applications starting in 2014.” 1 Our highways are continually becoming more congested. As a result, Provincial and State Departments of Transportation (DOTs) are constantly looking for innovative ways to improve traffic flow. One way DOTs are now helping to ease congestion is by expanding the use of tunnels and long underpasses in large interchange reconstruction projects. With less available right-of-way to work with, and the high costs of maintaining elevated bridge structures over the years, it often makes more sense to “go under” (rather than over) the traffic congestion points. With the addition of these new tunnels and long underpasses comes the issue of proper illumination within these structures, for a safe and efficient driving task. DOTs use recommended lighting practices and guidelines for highway lighting, which include specific criteria for tunnels, set by either the Illuminating Engineering Society of North America (IESNA) or the American Association of State Highway and Transportation Officials (AASHTO). However, both IESNA and AASHTO point out that these recommended practices and guidelines are not a mandate, and DOTs are free to make their own decisions about lighting criteria. Accordingly, effectively illuminating tunnels is often times a challenge. This paper will examine the key considerations of the tunnel lighting application in generalas well as what LEDs now bring to the table and how, accordingly, traditional lighting technologies will be phased out in the future illumination of tunnels."

Back in 1970 Congress passed the National Mining and Minerals policy Act, which had as one of its basic tenets to rely on a strong and stable private minerals industry for the development and production of our mineral resources. Since then, a substantial amount of legislation has been introduced that impacts on the minerals industry. Although most of the bills proposed are not passed into law, each bill affecting minerals and mining is given careful review by the Bureau of Mines. Comment is forwarded with the object of being helpful to legislators by providing our assessment of the probable impact of each bill proposed along with suggestions to improve them. Actually few bills introduced become law. For example, in the 93rd Congress 4,260 bills were introduced in the Senate and 17,690 in the House. In the final accounting there were passed 123 private laws and 649 public, 3 percent of the bills introduced. There would be little merit here in attempting to analyze each mineral-related bill that was proposed. Many are similar and aim at making a certain aspect of our minerals activity fit needs. So, to simplify the task the many bills have been grouped by subjects. Slide 1 shows the principal topics and the number of related bills introduced. Putting it altogether, there are 26 principal subjects and almost 200 bills. And these numbers are probably off somewhat, as this accounting was made early in December.

This paper describes development of a cost report that is descriptive, logically oriented, and concise. Since the costs involved relate to mining, this reporting technique could help bridge any possible communications gap between "mining men" and computer personnel, at the same time giving the computer-oriented mining individual information to assist him in his job performance. As computers become entrenched in the mining world, meaningful report assimilation becomes a major task. No one should be more capable of defining required results than the man requesting a report. If he is able to clearly state his needs and has researched his problem, it will be reflected in the computer output. During 1970, The Anaconda Company's Berkeley Pit Operation, at Butte, Montana, received many different computer-generated equipment cost reports. Up to thirty-six different components for each type (class) of pit equipment contributed cost data to the Berkeley Pit cost system files for both operating and repairs during current month, year-to-date and accumulation-to-date time periods. This reporting approach was taken to facilitate analyzing this voluminous data. SUMMARY A Major Component Cost Analysis (Exhibits 6-A thru 6-C) developed by arranging the top ten operating and repair percentages (component cost divided by total cost) in descending order, and then accumulating these percentages to a possible one-hundred percent. While tabulating the operating and repair cost percentages by equipment unit number in the current month, year-to-date and accumulation-to-date time periods, a frequency chart is constructed for each equipment

Central to Newmont Gold's Mine planning system is the ability to interactively perform cut and fill operations on topographic contours and to accurately evaluate grades and tonnages within contour outlines using polygon set manipulation routines. This paper develops the applied geometry concepts that underlie these routines. Included are representation of volumes with contours, point in polygon test, block proportion in polygon calculation, polygon area and centroid calculation, and intersection, union, and complement between polygon sets.

"INTRODUCTION Shotcrete lining forms an integral part of conventional tunneling and is widely applied for underground excavations. Early strength gain of the shotcrete is a crucial aspect for ground support and safety of operatives. Strength requirements are dependent on various factors such as lining thickness, ground type, excavation size and tunnel depth. The early strength gain is typically monitored using destructive tests, such as needle penetration, stud driving or coring samples for uniaxial compressive strength testing in the laboratory. Being destructive, these tests cannot be directly performed onto the lining without causing damage that must be repaired, which is a particular problem for permanent linings. For this reason and to avoid the need for operatives to work near to exposed ground and/or fresh shotcrete, these destructive tests are often performed on panels, which are sprayed at the same time as the tunnel lining. All current testing methods are also very local, testing only a small part of the lining or a panel, which may not be representative because the temperature history could be significantly different. Therefore, these tests do not provide an accurate or complete picture of the lining strength gain. New testing methods that are non-destructive and can scan the whole lining remotely would be extremely desirable. This paper describes a new method, using thermal imaging techniques, that achieves these aims. It also discusses the real-time on-site application of the method, providing insight into the experience gained and conclusions derived. Shotcrete Shotcrete used for tunnel linings requires immediate strength development. The strength development is a direct result of the hydration reaction of cementitious materials present in it. A progressive sequence of the hydration reaction changes it from a solid suspension (typically referred to as fresh concrete) to a solid skeleton with a porous network and thereafter, into a solid with predominantly discontinuous pores (Byfors 1980). In the case of shotcrete, early strength is needed to support the self-weight and then continuing early age strength gain is required to begin to support ground loads. These strength requirements, along with other workability needs, are met by careful concrete mix design, the use of admixtures, such as accelerator and superplasticizer (BS EN 934-5 2007), and supplementary cementitious materials, such as silica fume."

Numerous underground coal mine ventilation seals have been installed in the United States of America prior to 2006 and after implementation of the Mine Improvement and New Emergency Response (MINER) Act. Seal engineering, design, placement, and construction is regulated in industry, but post-construction monitoring of seals is limited to visual inspection of the outby face and seal perimeter, convergence measurement, and atmospheric monitoring. To monitor seal integrity and to provide safe working conditions, a form of non-destructive testing (NDT) is necessary to accurately reflect the health of the seal by detecting post-construction changes, deterioration, or defects within in-situ seals in underground coal mines. Previous research from the National Institute of Safety and Health (NIOSH) has indicated the potential for utilizing ground penetrating radar (GPR) for assessing in-situ mine seals. This paper provides an update to the assessment of proposed NDT methods for seal evaluation using commercially available GPR technologies and systems. Full-scale free-standing seals and large-scale seal specimens were analyzed with an array of GPR units from various manufactures using multiple frequencies.

Restricted fields of vision are a common problem with underground mining equipment. Also, when lighting systems are provided, they may be positioned where they cause glare. This restricts the visibility around the machines even further. To help address these problems, the U.S. Bureau of Mines has developed a PC based software package to aid in the analysis of certain human engineering aspects of mining equipment design. The software is available for use by equipment manufacturers and mining companies for design work on new machines, as well as for evaluating proposed modifications to existing machines.

"Can the ever-increasing tonnages of ore and waste being mined today be accommodated simply by using bigger and better crushers? This paper reviews the latest primary crushing and large cone crusher designs. It highlights how—in addition to using the larger crushers now available—maximizing the utilization percentages in each of the four stages of the rock size reduction circuit will assist in meeting the larger production requirements of existing and new mining operations. Its goal is to help consultants and operators select the optimal equipment and custom-designed items in the material handling flowsheet from the mine to the mill, maximizing operating hours per year. INTRODUCTION Are larger crushers the answer to meeting the demands of existing or new mining projects for increased tonnage? Boyd (2002) predicted that larger crushers for primary and fine crushing would be beneficial and contribute to the further development of mineral processing. Larger crushers for primary and fine crushing applications are now available. The rock size reduction process from the mine to the selected milling process facility has not changed since man first broke a rock to get at the interesting mineralization within. The rock size reduction process circuit from the mine to the mill can be summarized in four stages: • Stage 1: Mining (rock breakage and fragmentation) • Stage 2: Pre-screening (scalping) • Stage 3: Primary crushing • Stage 4: Fine crushing (secondary, tertiary and quaternary) Plant throughput and ore delivery schedule will form the basis for flowsheet design and equipment selection. Once the project design criteria and the mine plan have been established, the circuit’s utilization percentage (percentage of hours when the circuit is being utilized) can be optimized for each of the four stages. The mining and process departments are the key stakeholders in the selection of the final comminution circuit, and both departments should be familiar with and contribute to the design of the circuit. This paper focuses on the four stages of a rock size reduction circuit from the mine to the mill, equipment selection, various ore and waste handling flowsheets, and improvements in crusher station design, all with the purpose of maximizing availability and plant utilization percentages. What is a large tonnage? From 1907 to 1914, the American Panama Canal excavated 300 million tonnes of material in 7 years (History of the Panama Canal, Wikipedia). Today’s large mining operations can move the same material in 10 months, using large crushers or multiple crushers for ore processing and large crushers and a mixture of truck haulage and conveyors for handling the large waste tonnages. Rio Tinto Coal Australia (RTCA) developed Meandu Mine in Queensland, Australia in 1983 to supply thermal coal to the adjacent newly constructed Tarong Power Station (TPS). In designing the mine, RTCA selected Meandu?s best economic reserves for a 25-year life based on station owner Tarong Energy Corporation?s (TEC?s) estimated life for TPS. That 25-year plan changed dramatically in the early 2000s when TEC approved life extensions to TPS?s four existing units plus construction of a fifth unit. After reviewing Meandu?s remaining reserves, RTCA decided the nearby Kunioon reserve offered the more economic option for maintaining deliveries beyond 2010. Those plans changed yet again in 2007 when TEC chose to purchase the Meandu Mine and nearby reserves rather than extend RTCA?s contract. TEC then selected Thiess Pty Ltd (Thiess) to continue operating Meandu until the new mine at Kunioon could be developed.

The mining industry is constantly in a state of flux -boom or bust; mine openings and closings. Part of the reason is the nature of ore deposits -they are there or they are not. Once they are mined, they are reclaimed and the move is on to the next deposit. Risks are usually high, and payoffs can be great or not so great. Many factors go into making a property payoff, and mining companies are continually searching for ways to remain profitable as long as possible. Computers and the people who can make them work are one of the ways available to improve productivity and ensure this happens. Computers have been used in the mining industry for the last 30 years (and more in a few cases). During the past 10 years, the explosion of micro-computing has changed how many mining companies do their work.

Extracting lunar ice, to be processed into pure water or hydrolyzed into oxygen and hydrogen, faces many challenges. On the Moon icy-regolith deposits that contain up to 10% water are found within Permanently Shadowed Regions (PSR) at both lunar poles, where temperatures approach -220 ⁰C. NASA has identified several technology gaps that exist in developing these resources including: • Excavating large quantities of icy-regolith • Delivering large quantities of acquired resources • Hardware that is lightweight and energy efficient • Hardware that is reliable and durable. NASA uses public challenges to incentivize innovative approaches to closing these gaps. The Break the Ice Lunar Challenge provides over three million dollars in prize money to teams that compete in developing solution to mining the Moon. This paper describes the rules for this challenge and presents some of the proposed solution.

Highwall mining, which is a relatively new mining method, is a preferred and often the only feasible method for extracting coal reserve left by surface mining operations in areas with steep terrain and in closely spaced thin coal seams. Though its safety record is comparable to that of surface mining, failures of mine structures in highwall mines, especially the highwall, have occurred. Methods for the systematic designing of highwall mines to meet the safety and operational challenges are still evolving. This paper presents the results of research into improving the existing design methods and developing new ones for highwall mining operations. The design methodology is aimed at ensuring structural stability of the highwall, roof, web and barrier pillars and preventing subsidence. It takes into consideration the major ground control challenges of soft and layered mine roofs, and interactions from mining closely spaced coal seams. An optimization design process applying a pressure arch concept can be incorporated to improve the recovery rate of coal reserve. A computer program based on the design methodology was developed.

From a start in 1957, milling 3,000 TPD of copper ore, the Cyprus Pima Mining Company, which is located 20 miles south of Tucson, has expanded its operations four times, and is currently treating over 55,000 TPD of ore.1 The most recent expansion added a 14,000 TPD semi-autogenous concentrator to the operation. Shortly after startup in January, 1972, the semi -autogenous plant exceeded the designed tonnage rate.2 Ore from the mine is delivered to a 54 inch gyratory crusher and is reduced to a minus 8 inch product. From the crusher the ore is stacked on a surge pile and then reclaimed and fed to the two parallel grinding circuits. A 28 ft. diameter by 12 ft. long semi-autogenous mill and a 16.5 ft. diameter by 19 ft. long ball mill make up each grinding circuit. The semi-autogenous mills are in closed circuit with screens and the ball mill circuits are closed with hydro- cyclones. Rougher flotation follows the grinding operation. Concentrate regrinding and cleaner flotation is performed in Mill I, the older mill. The generalized flowsheet is shown in Figure 1.

The perennial subject of comminution is deserving of a fresh approach if one can be found. This is attempted here by directing attention primarily to principles rather than to theories. While theories attempt to explain how a certain process acts, principles delineate the laws which the action-must follow. Comminution may be defined as the progressive separation of particulate matter. At the present time this is accomplished commercially by applying energy in the form of compressive and, shearing forces. Such forces strain and distort the particles they contact. When-the applied stress locally exceeds the breaking strength at the weakest flaw in the particle, a crack tip forms. The surrounding strain energy flows to the crack tip, which rapidly extends through the particle as a break. The minute changes in shape and the resulting energy flow cause the failure of other highly stressed flaws, with the consequent formation of a progeny (1) of new smaller particles out of its destroyed progenitor. The applied mechanical energy is transformed into strain energy as the particle is distorted, and then into heat when it fails. Only a tiny fraction of the order of 1/1000 of the applied energy necessary for breakage appears as new surface energy in the product particles. Comminution is the process by which rock or ore is reduced to a size where the liberation of the minerals is maximized without changing the chemical or physical properties of the rock. Comminution accounts for about 50% of all energy that mines use (Klein et al., 2018). Growing demands from stakeholders and shareholders for sustainable development and CO2 reduction will increase the need for energy-efficient comminution. Ores with different mineralogical characteristics, textures, strengths, apparent stiffness, and mass-specific fracture energies need to be processed with a specific, new, and efficient comminution technology to increase the production and profitability of mining companies. The adoption of crusher wear parts built with new materials, such as non-Hadfield manganese steel or synthetic materials or traditional manganese with wear-resistant components, could improve the efficiency of the crushers, reduce downtime, increase production, improve the desired P80 (the aperture through which 80% of the product will pass) product size distribution, and enhance mine safety.

The behavior of non-sulphide gangue in chaloopyrite flotation circuits has been investigated. Plant and laboratory tests were carried out on five different ores, and linear relationships were found to exist between the rates of recovery of water and free nonsulphide gangue in the concentrates although there was evidence from pilot plant tests that these were linear approximations to parabolic curves over the limited regions which were observed. There was evidence that the free non-sulphide gangue enters the concentrate by hydraulic classification and a classification function has been proposed to define this behavior. This function is suitable for inclusion in a mathematical model of the process.
