Benchmark practices in the mining industry include the establishment of Quality Programs — Quality Assurance and Quality Control (QAQC) — to monitor the correct execution of sampling protocols and control each stage of the ‘Sample Cycle’: Sample Collection, Preparation (comminution) and Analysis. QAQC reports commonly include statistical-numerical results that quantify performance of QAQC controls (field duplicates, preparation duplicates, blanks, standards etc). Graphics such as scatter plots, QQ plots, histograms and cumulative frequencies are used to represent the results. Statistical values including relative difference, absolute difference, relative variance, averages, T-test, and Zscores are used to quantitatively express the relationship between duplicate pairs ... but is an effective quality program just a statistical exercise? This paper aims to highlight the concept of ‘Quality Management’ (QM) as the precursor of continuous monitoring (ideally in real time) of QAQC results on Exploration, Production, Processing and Port samples, determined by trend analysis performed on a time and/or grades basis. Currently, there is a paradigm in the process of quality assurance that if individual data points fall within a predetermined acceptance limit then they are ‘acceptable’ and therefore suitable for informing operational and investment decisions. However, what is stated with respect to QM is that sometimes results found within the acceptance limits can indeed be internally biased, or show material deviations over a period of time, thereby impacting reconciliation results and/or potentially creating discrepancies with the final buyers. QM refers to the proactive detection of these ‘anomalous tendencies’, the approach and communication with the source of the information, the understanding of the reasons that cause the deficiencies, and the generation of agreed action plans to remediate the situation and ensure the consistency of results, in other words: continuous improvement on proactive manners.

Leaders have invested heavily in traditional safety management to improve safety performance over the years. This primarily involved: • fixing the environment (eliminating the hazard) • improving the system (more/less rules, procedures etc) • making safety more conscious (more/better interactions about the importance of safety). These approaches have helped improve safety performance significantly during the last 50 years and will continue to play a role in the future. Although these approaches may be part of an overall solution, they are not enough to prevent many incidents. Today, these three approaches are used more than before, and are used better than ever before. Yet, workplace fatalities in Australia have stagnated from 2013 to 2019, as have the number of incidents, even serious ones. In the USA, workplace fatalities have increased by 15 per cent in the same period.

Sedimentary rock is a lithology that is commonly encountered in mining practices. Their properties, especially the resistance against weathering, are often of critical importance to long-term engineering stability due to their ubiquitous existence on the earth’s crust. The weathering of rocks is a complex process that involves physical degradation, chemical and biological erosions. In this study, the impact of physical weathering on four common sedimentary rocks, namely sandstone, siltstone, limestone and shale, are simulated in the laboratoryoratory environment taking into consideration the impact of preservation condition, temperature and time. In order to optimise the design of the experiment, Taguchi’s design of experiment method is implemented based on the L16 orthogonal array and selected simulation conditions. The weight and dimension variation, fracture intensity, slake durability, and uniaxial strengths of specimens are then tested to quantify the impact of weathering under various conditions. The results indicate that weathering significantly influences the strength of shale, siltstone and limestone but not sandstone. The impact of weathering on dimension variation is insignificant. For soft rocks, most physical weathering took place after second cycle of simulation, resulting in strength reduction up to 100 per cent (disintegration) but generally in a range from 28 per cent to 32 per cent. Additionally, time is the most important factor contributing to the physical weathering of weak rocks, followed by residual, preservation conditions and temperature. Last but not least, some shales, depending on the content of clay minerals, tend to develop significant fracturing or even disintegrate under the influence of weathering.

Safety is a key value in most leading mining companies. Despite years of effort, and significant expense in ‘programs’, operating sites seem to have been left with no real improvement in either the elimination of fatalities or sustained reduction in injuries.Safety systems and processes have grown and evolved, often to the point where they have become overly complicated and marginally useful.MMG Limited, following the consolidation of its Australian operations under a single management structure, undertook a program to effect real and sustained change in its approach to managing safety. The program aimed at improving safety by better engaging its workforce and by making safety processes simple, useful and effective. It defined the role of the supervisor in creating safe work, the role of the people doing work and the role of safe work methods.A safety diagnostic tool was developed to assess site against safety-critical mindsets, behaviours, cultures and systems. The outcome of the diagnostic is considered by the organisation to be the true lead indicator of safety performance.While still a work-in-progress, the program at the Australian Operations of MMG Limited shows that challenging common paradigms in safety management can create real change in safety performance.CITATION:Webb, M E, 2016. Thank you for the opportunity to speak at this important conference attended by aspiring mining professionals, some of whom will become the leaders of the mineral resources industry that forms the solid foundation of AustraliaÆs past and that will fuel the exciting growth of AustraliaÆs future.I want to briefly touch on the history of our business, then look at the opportunities that exist in the contract mining industry and finally comment on what the educators need to consider in their curriculum to meet the needs of our business. Byrnecut started out as an idea in early-1987 developed by a group of four mining people. At the time the industry was on the high end of a mining boom, the era of the Bonds, Connels, Skases and so on. Riding on this wave of euphoria we borrowed $600 000 from the National Australia Bank and, without having enough knowledge to lose sleep over the potential risks, started business in mid-1987. The euphoria quickly evaporated with the 1987 October stock market crash. The bank pulled our overdraft, equipment finance rates increased to over 20 per cent per annum and my house was mortgagedseveral times over. The business learning curve was extreme for a young mining engineer who had developed his trade in the relatively sheltered environment of a large public mining house, namely WMC. We were somewhat bitter that the banks had done a runner. We were a private company, backing the business with personal assets with our shoulders to the wheel, whilst the Bonds of the world had theirnoses in the trough ripping off mum and dad shareholders. All of this had a sobering effect, namely keep your feet on the ground, stick to the knitting, work hard and worship cash flow. Since that stressful but adrenaline pumping beginning the company now employs over 600 people and is now working for major mining houses at over a dozen locations in Australia, east Africa and WesternEurope and the National bank is again our banker. On this basis I consider myself qualified for the subject of this speech.

Flinders Mines is an ASX300 iron ore explorer and developer with a tenement package located in the Hamersley Ranges region of Western Australia, approximately 70 km north of Tom Price. Exploration over the past three years has identified a Global Resource in excess of 740 Mt at a grade of 55.4 per cent Fe (+50 per cent Fe cut-off). This resource is spread across multiple deposits and has been divided into a number of mineralisation type domains based on geometallurgical and geochemical boundaries. These domains are comprised of transported haematite rich detrital iron deposits (DID) and goethite dominant Channel iron deposits (CID) as well as in situ bedded Brockman iron deposits (BID). On the basis of differences in physical and chemical characteristics, the DID have been divided into four discrete units. The four DID units form a stratigraphic sequence with the iron grade increasing and the contaminant concentrations decreasing with depth. The upper DID unit is lower grade and currently unlikely to provide a marketable product, without signifi cant beneficiation. The lower three DID units, together with the CID and BID, are of a higher quality and have the potential to yield marketable ore. The ability to define and recognise these different units has resulted in a more reliable and robust geological interpretation and hence greater confidence in the resulting resource model. This has been evident in the excellent conversion of inferred to indicated resources.

Victorian brown coals occur in massive deposits, the quality of which is determined in the first instance by means of samples taken from exploratory bores. As a number of important utilisation characteristics are connected, not with the relatively homogeneous organic mass of the coal, but with minor components such as the ash content and particular constituents of the ash, it is necessary to know how bore samples relate to coal finally used in the process concerned. Tests are described in which the statistical parameters associated with bore sampling, operational sampling and sample reduction and laboratory procedures including a range of coal analytical methods were determined. These indicated properties where analytical errors were relatively high and where sample reduction procedures contributed substantially to imprecision in results. They also showed the effects of the non-homogeneity of brown coals with higher than normal ash contents. A further experiment explored the relationship between samples taken at various stages in the course of test boring, digging, and burning a defined 'block' of coal, and demonstrated an apparent increase in the proportions of adventitious mineral constituents as the 'intensity' of sampling Increased.

At the beginning of this year it was my pleasure and responsibility to take over the Presidency of our Institute from Mr A. A. Parish who, as did his predecessors, filled the office with distinction. It is a great privilege to address you as President and it is an occasion for my wife and family, and I am most grateful for your inviting them to the buffet dinner. A formalised approach to risk management has been regulated in the United Kingdom (UK) since the Construction (Design and Management) Regulations 1994 (CDM) came into force in 1995. These regulations have recently been revised in 2007. In response to a series of significant claims around the world the UK insurance industry published a code of practice for risk management of tunnels in 2003, and that was followed by an international version in 2006. Probably one of the main contributions of this new approach is the recognition that health and safety risks can best be addressed early in the process, initially by the owner and then by the designers. One of the aims is to achieve elimination of risks by good design. The advantage of a risk-based approach to tunnel design is that all risks are considered explicitly, and any residual risks in the design are communicated clearly to the construction team. This paper looks at the implementation of this formalised approach to risk management with reference to the A3 Hindhead Project. The design has been undertaken in accordance with the requirements of the CDM regulations, and also in compliance with the Joint Code of Practice for Risk Management of Tunnel Works in the UK. An outline of the risk-based design process used is provided, including examples.

Incidents that trap miners in underground mines have serious consequences for the miners and the mine operators involved. It is of utmost importance that mine operators utilise all available tools when trying to locate and rescue trapped miners. From testing conducted at underground hard rock mines, Cumming-Potvin et al (2013) found that pre-existing microseismic monitoring systems, which are routinely used for seismic risk management, have the potential to be used for detecting and locating trapped miners.This paper reflects on the Anzac Day seismic event that trapped two miners at Tasmania Mine, Beaconsfield, on 25 April 2006. The Anzac Day event occurred at 2123 on a Tuesday evening. Rescuers were unable to confirm that the trapped miners had survived until 1930 on Sunday evening. It is not possible to speculate on how the rescue operation may have been different had the technology to use the microseismic monitoring system for the purpose of detecting the trapped miners been available. However, it almost certainly would have been different. The recent research project has demonstrated that knowing the fate of the Beaconsfield miners might have been achievable up to 100 hours earlier than was the case in 2006. That knowledge would have been invaluable to the rescue effort.By examining the seismic array at the Tasmania Mine, the reliability of detection over the working levels of the mine could be quantified. Had the methodology been available, adding six additional sensors (at a cost of $40 000), would have been sufficient to increase the likelihood of detection to greater than 90 per cent in the working levels of the mine. Geometallurgy is often a misunderstood and poorly defined concept. The first thing the paper will attempt is to provide a definition of geometallurgy. Ultimately this paper has been written to provide guidance to geologists, mining engineers and processing professionals as to what geometallurgy means in the crucial early phases of a potential mining project between the time of discovery and the time of commencing construction, should this occur. The paper covers the options available for conducting a geometallurgical study of a resource or orebody, appropriate to the level of definition required in the projectÆs development cycle. The paper will then describe how the geometallurgical information generated is subsequently used in calculations, planning and preparation of reports designed to inform and reassure stakeholders, such as the market. Ultimately in a successful project, appropriate options adopted in the development phase will form the basis of an ongoing geometallurgical program that links the major site disciplines for the good of the operation.CITATION:David, D, 2013. Brownfields geoscience measurement such as drilling and surface geophysics can be costly and it is often difficult to weigh the cost of data collection and processing against the value it unlocks at the time of extraction. This makes it difficult to prioritise geoscience expenditure and find the optimum intensity of geoscience activity across competing assets. There are multiple options for managing resource uncertainty and spatial variability through mine planning and resource engineering such as varying marketing strategy, adjusting stockpile or other infrastructure design and product blending. Consideration of these alongside increased data collection further complicates finding a comparable value of geoscience programs to determine the right blend of geoscience measurement and engineering controls. Concepts similar to Value of Information (VOI) analysis which is applied to strategic planning can be used to estimate the value of geoscience measurement and pit sequencing at tactical planning and operational timescales. This involves the quantification of resource uncertainty and prediction of how it will affect resource extraction. This paper outlines this process and demonstrates its use to prioritise geoscience at BHP Coal Assets. The value of infill drill programs in enabling operational stability is quantified and compared the value of engineering controls like increased stockpile capacity. This study supports that idea that the application of VOI methods to bulk mining at tactical time horizons has potential to inform geoscience planning and resource engineering not just separately, but as part of a unified strategy for managing resource uncertainty.

Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

This paper describes a hybrid ventilation system, which uses secondary fans that also act as de-facto primary fans to extract fresh airflow from the mine intake raise system. Also discussed are the associated problems such as uncontrolled recirculation, inadequate decline airflow, fire safety issues, prolonged blast fume clearance times, high air temperatures in main access routes, neutralisation of mine air cooling by heat from secondary distribution fans, increased ventilation system complexity, reduced flexibility and high pressure differentials (which cause safety issues with personnel access and reduce secondary ventilation system performance).Practical suggestions are provided on options to avoid these problems listed.CITATION:Derrington, A S, 2013. Quay Magnesium Limited (QMG) listed on the Australian Stock Exchange in late September 2004 having raised approximately A$30 million with the object of designing, constructing, commissioning and operating a 15 000 tpa magnesium alloy production facility in Nanjing, China. Site construction commenced in February 2005, with installation of all major components completed by the end of October 2005. Hot commissioning was simultaneously commenced with the final stages of installation, leading to the first stage of continuous ingot casting by mid November 2005. Since that time, ramp-up to the name plate capacity has been undertaken. This presentation highlights some of the more significant technical and logistical hurdles that have had to be overcome in order to meet the high quality product specifications required by a range of QMG customers.

The Pasminco mine at Rosebery in north western Tasmania is a medium sized underground base and precious metals mine. In April 1992 a major wedge failure of approximately 5000 t occurred in the backs of a main access drive to the 18 level H Lens orebody. This collapse prevented access to 190 000 t of ore which had been scheduled for production in July - August 1992. At the time of the collapse, the mine was in a transition period between mining small pillar remnants of several other orebodies and developing to the new deeper J Lens orebody.The development at this orebody was behind schedule and it was necessary to find a cost-effective and timely alternative to a 75 m bypass drive around the fall area.A Lysaght- Fletcher multiplate steel arch was available on-site from a deferred surface road project and it was decided to use this arch to regain access through the fall area. Due to the danger of continued minor falls, the entire 18 m long, 3.95 m high x 4.5 m wide tunnel had to be assembled in the 4.7 m wide x 5 m high accesss drive and pushed into the fall area as a single unit.

he Melbourne City Link project will complete a missing link between two existing major freeways in the Melbourne road network, providing a much needed bypass of the Central Business District for through traffic. The project, which is being financed under a BOOT arrangement with the Victorian State Government, involves the upgrading of significant stretches of existing freeway and the construction of new sections involving viaducts and tunnels. The tunnel sections, which are the focus of this paper, are beneath residential and business areas, the Yarra River, the Royal Botanic Gardens and significant sporting facilities where surface construction would be either impossible or unacceptable (reference Figure 1). The shorter, 1.62 km west bound Domain tunnel comprises two cut and cover sections through Olympic Park then the Yarra River, the driven section of 0.68 km length and exits through a common cut and cover section with the east bound tunnel under St Kilda Road and Sturt Street. The longer 3.76 km east bound tunnel commences at the common portal, through cut and cover under St Kilda Road then 2.86 km of driven section before exiting at a cut and cover section at Burnley. The longer Burnley tunnel has an access and ventilation shaft at approximately its mid point.

Drainage of gas has been practiced overseas for almost half a century. Peal advances in drainage of gas to combat high emissions became practicable in the late fifties and early sixties when the technology was standardised to use it under a variety of mining and geological conditions. Today methane drainage is practiced in the United Kingdom, Germany, France, Belgium, Poland, Czechoslovakia, Hungary, Ibmania, U.S.S.R., China, Japan, Canada, U.S.A. and Mexico. Australia entered the list of countries using gas drainage on 3rd March, 1980 when the first full scale gas drainage system was set up and commissioned at West Cliff Colliery to drain gas from the Bulli Seam. Methods used in Europe are different from those used in North America, Japan and Australia mainly because of the different mining methods and also because of the limited number of seams under extraction, relatively new coal fields, and comparatively larger distance between the seams. In spite of the difference in the technique, the basic aim is the same and that is to improve ventilation and reduce ventilation costs, alleviate dangerous gas accummulation, overcome outburst hazards and ultimately make mining safer and more efficient. This paper describes the new legislative regime that will apply to the regulation of the minerals industry in New Zealand.The reform of New Zealand's minerals legislation was initiated in January 1988 when the Minister for the Environment, Rt Hon Geoffrey Palmer, announced the Government's intention to review all the major laws governing New Zealand's resources. The Resource Management Bill is scheduled for introduction to the House in December 1989 and its date of enactment is scheduled for June 1990.
