The 1889 Johnstown Flood, which killed 2,209 people, was a preventable disaster stemming from systemic decisionmaking failures. This paper applies modern Risk-Informed Decision Making (RIDM) frameworks to analyze the historical context and explore alternative outcomes through hypothetical multiverse scenarios. By reconstructing the risk landscape of the South Fork Dam, we demonstrate how structured hazard identification, risk assessment, and stakeholder communication could have mitigated or prevented the catastrophe. Three counterfactual scenarios—proactive maintenance, community engagement, and regulatory oversight—are evaluated to quantify potential lives saved and economic benefits. The study acknowledges historical barriers, including technological limitations, lack of governance, and socioeconomic disparities, which constrained risk-aware decisions in the 19th century. The analysis draws critical parallels to contemporary engineering challenges, particularly in tailings storage facility (TSF) management, where similar risk blind spots persist. The paper concludes that RIDM is not merely a technical tool but a governance imperative, essential for ethical and sustainable engineering practice. By learning from historical near-misses and failures, the modern engineering community can strengthen resilience and accountability. This work contributes to the growing body of retrospective risk analysis, offering a methodological framework for applying RIDM to historical disasters to inform future safety standards.

This paper describes the test work, process design, commissioning and operation of a processing system at the Ergo gold operation that improves the extraction efficiency from gold tailings material. The improvement in recovery is derived from the separation of the gold contained within the sulfides of the tailings material and a dedicated process stream to recover that gold.The sulfides are separated by means of a flotation circuit and that resulting concentrate is subjected to a regrind of below 20 ¦m to enable a greater liberation of the gold. The ultra-fine material is then subjected to a dedicated leach circuit to recover the now exposed gold particles.The case study is an example of the challenges of introducing new technology and methodologies to a well-established operation and thereby attaining improved efficiencies and tangible business benefits.CITATION:Ebell, M B and Capstick, D, 2013. Tower of Lot L, L1 & M, Kuala Lumpur City Centre when completed would stand 700 m tall. This Mega tall Skyscraper would merely be less than 500 m away from the world tallest Twin Tower, the Petronas Twin Tower. From June 2018 to February 2020 BAUER (MALAYSIA) SDN. BHD. constructed 225 nos bored piles with diameters of 2.0 m and 2.5 m for this tower. To comply with the depth requirement of the initial design and the need to have the pile toe 2.0 m above competent bedrock, the drilling equipment was required to be able to achieve a maximum drilling depth of 150 m. Actual piles constructed on site were deeper than 120 m. These dimensions caused superlative challenges which included but were not limited to controlling the open borehole stability over time with polymer, installing 70 t heavy steel cages and pouring up to 550 m³ concrete continuously in conjunction with installation of 88 t heavy kingposts into workable fresh concrete. The piling works were executed with one BAUER BG 48 and one BG 72, the biggest drilling rig globally available.

The U.S. Bureau of Mines investigated the feasibility of using ammonium carbamate (NH2CO2NH4) leaching to recover manganese from steel plant slag. This investigation showed that treatment of the slag in hydrogen prior to leaching enhanced manganese extraction. Up to 80 pct of the manganese and 50 pct of the iron could be extracted from a silicon steel slag that had been pretreated in hydrogen at 700° C. Two 4-h leaching stages conducted at ambient temperature with a solution containing ammonia and carbon dioxide dissolved manganese and iron as their carbamate complexes. Heating the pregnant solution to 65° to 85° C decomposed the complexes and precipitated the metals as insoluble carbonate salts. Ammonium carbamate leaching of other slags from several steelmaking plants produced lower manganese extractions, which suggests that the method cannot be applied satisfactorily to all steelmaking slags

In industry, the strength of a blasting cap is often equated to the type of explosive it can detonate. Comparable cap strength is becoming less important as nearly all caps manufactured today can initiate high explosives, such as primasheet, with ease. Previous studies were conducted to assess the quantity of energy released by various detonators into the directional propulsion of the detonator’s casing. However, the question of how much of this propulsion energy is needed to initiate an explosive remains unanswered. This study will assess the fragmentation size and velocity needed to initiate a high explosive when they are not in direct contact. For this study, primasheet was used as the main charge as it is relatively insensitive and can be situated in different positions relative to the detonator. Electric, non-electric and electronic detonators were used to evaluate initiation potential. The primasheet is observed in response to being in contact with a detonator, and not in contact with the detonator at various distances and angles. This allowed the quantity of energy transferred into the primasheet to be observed and for the initiation of the sheet to be recorded using high-speed video. This study will report the energy necessary to initiate the primasheet at various distances and angles from the charge from a number of widely used, commercial, hot wire blasting caps.

Many persons hold the opinion that the metal indus- ries are fortuitous and that the occupation is one of sordid toil, and altogether a kind of business requir- ing not so much skill as labour. But as for myself, when I reflect carefully upon its special points one by one, it appears to be far otherwise. So wrote Georgius Agricola in 1556. He could have been espousing the sentiments of an indus- trial archaeologist investigating mining sites in 1986. South Australia has a very rich legacy from its industrial past-especially in the area of its mining history. Industrial archaeology, which is part of the broad field of archaeology, is concerned with the material and documentary remains of this industrial past. This includes the products and processes of manufacturing, industrial, technolog- ical and engineering activity and their relation- ships to people and the environment. `Archaeology' immediately conjures up the image of excavations in Greece or Eygpt. Excavation however is only a part of archaeological methodology. Industrial archaeology utilizes documentary historical rec- ords combined with field survey techniques (both above and below ground) to create a composite picture of the past. Interest in our recent past has burgeoned in Australia in the last few years. This interest manifests itself in many ways-the unprecedented popularity of Australiana and col- lecting, the plethora of Australian historic films, the growth of the National Trust, local historical societies and genealogical research. Fortunately these trends have been reflected in academic and legislative circles. Various univers- ities, institutes of technology and colleges of advanced education run courses in aspects of cul- tural resource management and conservation of the historic environment. These courses are com- prised of archaeology, architecture, geography, his- tory, materials conservation, museum studies, and national parks management and planning.

During the past year considerable work dealing with the mechanical properties and microstructure following the anneal under uniform condi-tions of certain types of commercial rolled brass has been done in this laboratory. Since these experiments were conducted with the active cooperation of the Bridgeport Brass Co., involving free use of its product, and have thus furnished information which is characteristic of some of its special mixtures, full publication of the numerical relationships and comparative results encountered does not seem justifiable. Some of the more general features of this work are, however, available and it is our present purpose to use the material in question, along with some results of special tests, in discussing the characteristics of recrystallization as related to the degree of hardening by strain and the ordinary annealing variables. We desire to make acknowledgment of the many courtesies extended by Mr. Charles Ferry, Metallurgist of the Bridgeport Brass Co., and the value of his cooperation, which has greatly stimulated the study of brass in this laboratory. A clear and systematic presentation of the principal relations between temperature and time of anneal, the changing physical properties and microstructure and the degree of previous reduction by cold-rolling, was first made, in the case of rolled brass and copper, by the French engineer, C. Grard,' in 1909, although much fragmentary information of this general character had appeared earlier and the main facts involved were undoubtedly known to many brass specialists through their own private research. To develop alloys suitable for use at elevated temperatures, columbium and tantalum were combined with tungsten, hafnium, vanadium, zirconium, and titanium, and the resulting alloys were evaluated with respect to fabricability, hardness, Electrical resistivity, strength at elevated temperatures, oxidation resistance, and bend transition. Of the 23 compositions tested, Cb-2OHf-5W-10Ti exhibited superior high-temperature properties. A recrystallized sample of this alloy was fabricable at room temperature, had a strength of 44,000 psi at 1,200° C, and had an oxidation rate of only 7.7 mg/cm2/hr at 1,000° C.

The subject of this session is "Equipment Installa­tion, Recent Developments". This is a subject which is not usually addressed at conferences of Foundation Engineering. It is rare to find sessions dealing with the technological side of deep foundations, a subject which only recently seems to have aroused sufficient interest at national and international conferences. It is therefore natural for the subject of "Equipment", or the tools used to implement the various "technologies", to be over­looked or mentioned only occasionally during the discussion of other issues. At conferences, most of the attention is attached to research and to the theoretical interpretation of the performance and behaviour of deep foundations, as well as to establishing the most suitable calculation algorithms and approaches for the study, design and control of foundation structures. It is almost as if the subject of equipment is felt to be less important or, perhaps, that it is considered to belong more strictly to mechanical engineering than to civil engineering.

This report presents results of the research and development work and operations performed during stage II of the gas combustion retorting research program conducted by the Colorado School of Mines Research Foundation and six major oil companies at the Bureau of Mines Anvil Points Research Facilities. The experimental program, including retort modification and air-gas distributor development, effects of shale quality and particle-size distribution, and other variables on the process, is discussed. Although operating problems are not fully resolved, it appears that the gas combustion retorting process can be successfully applied to achieve oil yields in excess of 85 percent of Fischer assay at mass shale feed rates of 500 pounds per hour per square foot of cross-sectional bed area, approximately twice the rate previously demonstrated to be reasonably practicable.

Lime has become a general and loosely used term to denote almost any kind of calcareous material or finely divided form of limestone or dolomite, as well as burned forms of lime. However, according to Webster, lime is only calcined limestone (known as quicklime, unslaked lime, or calcium oxide). It also embraces the secondary product of quicklime, i.e., hydrated lime or slaked lime (calcium hydroxide). There are two basic types of limestone used for lime manufacture-high calcium and high magnesium (dolomitic). The dolomitic quicklimes and hydrated limes correspond to their "high calcium" counterparts. The only difference is that the dolomitic types are a combination of the elements calcium and magnesium, in varying percents, whereas high calicum limes contain less than 5% MgO down to 1/2%. In either case a pure form of limestone of at least 97% combined carbonate content is needed to make salable lime, barring a few exceptions. The calcination of both types is diagrammed chemically as follows: [ ] In the foregoing reversible reactions the limestone is burned (calcined) in lime kilns, with the carbon dioxide content of the stone expelled as a gas. The weight loss during calcination of pure high calcium limestone is 44%; with equimolecular dolomitic limestone, the weight loss is 48%. Since lime has a strong affinity for CO_, particularly if moisture is present, it will readily revert to its original carbonate form. This occurs when quicklime "air-slakes" and adsorbs CO.. from the air. Thus, quicklime is perishable and should be stored in dry, water-tight areas. Depending upon the type of kiln used and the physical structure of the stone, the size of the quicklime may range from sandlike granules to 8 in. lumps. However, pebble sizes ranging from 1/4 in. to 2 in. are the most common. Screened quicklime fines are also com¬pressed (pelletized) into 1 in. pellets. For some purposes ground or pulverized forms of quicklime are used, ranging from No. 10 mesh to dust. Generally a pebble of quicklime is about the same size as the pebble of limestone before calcination (perhaps shrinking slightly). A more stable form of lime is hydrated lime. This is obtained by adding water to quicklime, slaking it into a dry, fine, subsieve size white powder. Because its affinity for moisture has been satisfied, it is not vulnerable to further moisture like quicklime. However, it has the same strong affinity for CO_. The following reversible chemical reactions illustrate how hydrated lime when heated (or dehydrated) can revert to the original oxide form:

5.1-PURPOSE, PROCEDURE, METHODS, MANAGEMENT Exploration can be technologically defined, with regard to the discovery of new mineral deposits, as all the activities and evaluations necessary before an intelligent decision can be made establishing size, initial flowsheet and annual output of new extractive operations. The purpose of mineral exploration is the discovery and acquisition of new mineral deposits amenable to economic extractive operations now or in the future. A prime objective of mineral exploration, however, is to find and acquire a maximum number of such mineral deposits at a minimum cost and within a minimum time. Immediately upon the heels of a successful ''exploration" discovery follow activities called, first, development, then operation. Under U.S. Internal Revenue Code regulations (Secs. 1.615.1 and 1.616.1) in effect at the time of this writing, which state that the "development stage" has been reached when "deposits of ore or other minerals are shown to exist in sufficient quantity and quality to reasonably justify commercial exploitation by the taxpayer," exploration in the technological sense may well continue after a mineral property has entered the "development stage" as defined for tax purposes. In many instances continued exploration, after reaching the tax defined "development stage," has disclosed additional reserves which justify a much larger, more efficient and more profitable operation than that which would constitute the minimum feasible. Even after a mine goes into production, continued ore search generally will increase the reserves, either justifying expansions in plant capacity or prolonging the life of the operation, but this work (actually "exploration") is normally considered, for bookkeeping and tax purposes, a part of mine development. Each exploration entity defines its own objectives in terms of acceptable mini- mum rate of return on investment for any deposit found and retained, and in terms of which commodities, and markets, are to be involved. For these reasons, exploration programs, and aims, will vary over a wide range, although all have the common objective of endeavouring to create new profit centers for the future. 5.1.2-EXPLORATION METHODS Exploration has evolved in the past several decades into a highly sophisticated and expensive endeavor in which large corporate entities, or subsidaries, normally are directed and staffed by highly trained exploration specialists. In some instances consultants, with expertise in a given field, are utilized to the benefit of such corporate groups. The "grasshopper" approach to exploration-searching only for outcropping minerals and/or investigating only mineral prospects offered by the discoverers of such outcrops has given us most of the past and present mines in the US., but this method is rapidly approaching the point of diminishing returns in areas that have been thoroughly prospected for currently marketable commodities. Electrolytic techniques have historically been used to remove arsenic, antimony and bismuth from copper refining electrolytes. However, both solvent extraction and ion exchange techniques have been heavily researched and in a number of cases commercially introduced. In this study a large number of solvent extraction reagents and ion exchange resins were screened to determine the selectivity of the reagents for impurity removal. Solvating extractants were found to efficiently remove arsenic from electrolyte. The extent of arsenic extraction increased in the following order: TBP < DBBP < DPPP < Cyanex 925 < Cyanex 923. Antimony and bismuth extraction from electrolyte were demonstrated with the reagent 2EHAP04. The ion exchange resins UR-3300 and Duolite C-467 were found to strongly extract antimony and bismuth from electrolyte. C-467 appeared to be superior to UR-3300.

This U.S. Bureau of Mines report describes methods to enhance the capacity, stiffness, and timber utilization of wood crib supports. Since stiffer supports minimize strata deflections that contribute to roof instability, efforts to maximize crib stiffness while maintaining stability through a wide displacement range should be a design priority. The stiffness and capacity of conventional crib designs, employing alternating layers of wood timbers that are loaded perpendicular to the grain, can be increased by increasing the interlayer contact area or by using higher strength wood. Contact area can be increased by using wider timbers or employing more timbers per layer. Concepts that provide parallel-to-the-grain timber loading will provide substantial increases in crib stiffness and capacity. Two concepts that utilize this principle are examined: (1) an original South African design that constructs wood cribs from preformed layers (mats) of timber where short blocks are oriented vertically between horizontal slabs; and (2) a design that employs full lengths of timber placed vertically in the interior of a conventional crib structure. These designs also provide more efficient timber utilization by providing greater support capacity per volume of wood used in the crib construction.

Numerical simulation of the fluid flow and heat transfer has been carried out for a waste-heat boiler in an Outokumpu type copper flash smelter. In this boiler, SO2-containing off-gas flows into the waste-heat boiler radiation section at about 1350°C. A fraction of cooler off-gas from the electrostatic precipitator at about 200°C was recirculated into the radiation section, and a small amount of ambient air was leaked into the boiler from the bottom. The computational results show that the flow is strongly recirculating and distinctly three dimensional in most of the boiler, particularly in the radiation section. The hot off-gas cooled down rapidly after entering the radiation section due to radiation. The predicted flow pattern and temperature distribution were in a good agreement with laboratory models and industrial measurements.

The principal procedures for the statistical analysis of gold assay and other trace-element data depend on applying either the normal frequency distribution or the lognormal frequency distribution. The consequences of applying both methods are examined through reference to example data from United States and South African gold mines. It is concluded that for estimation of average grades, except for certain data with a very high coefficient of variation (ratio of standard deviation to mean), use of the lognormal distribution is ordinarily undesirable because bias may be introduced and little efficiency can be gained. However, if biased average grades can be tolerated, as in the examination of data from surveys in exploration geochemistry, use of the log-normal distribution may be desirable. Various statistical analyses are detailed, and the tables presented to implement the analyses are more complete than those previously published.

Preventing the spontaneous combustion of remnant coal, which is indicated by carbon monoxide (CO), is a significant issue for coal mining safety. The N00 longwall mining method with Z-type ventilation is advantageous for the prevention and control of CO; however, research regarding this topic is limited. To investigate the distribution of CO in a goaf and working face, a sealing material and technique for blocking the goaf were introduced after proposing a forming method for a Z-type ventilation system; furthermore, a monitoring method was designed and applied in the field. The results demonstrated that the CO concentration during mining in the upper corner and return airway ranged between 4–6 ppm and 3–7 ppm, respectively. More importantly, no CO was generated in the goaf. The results indicate that the advantages and mechanisms regarding the prevention of spontaneous ignition using the N00 longwall mining method with Z-type ventilation are as follows: (1) The adoption of the downward ventilation technique is not only beneficial for reducing the air pressure difference between both ends of the working face, but also prevents CO accumulation in the upper corner. (2) The basis of a spontaneous combustion disaster is eliminated owing to the absence of coal pillars reserved in the goaf. This study proposes a superior method for mitigating spontaneous combustion disasters related to remnant coal in the goaf in a coal mining field and contributes to the better understanding of the N00 longwall mining method for coal mining safety.
