Lithium-ion (Li-ion) battery applications are increasing for battery-powered vehicles because of their high energy density and expected long cycle life. With the development of battery-powered vehicles, fire and explosion hazards associated with lithium-ion batteries are a safety issue that needs to be addressed. Li-ion batteries can go through a thermal runaway under different abuse conditions, including thermal, mechanical and electrical abuse, leading to a fire or explosion. The National Institute for Occupational Safety and Health (NIOSH) mining program is conducting research to prevent and respond to lithium-ion battery fires in battery-electric vehicles in the mining industry. In this study, experiments were conducted to investigate the effectiveness of different suppression systems including dry chemical, Class D extinguishing powder and water mist for lithium-iron-phosphate (LFP) battery pack fires. The effects of activation time and release time of the water-mist system on the suppression of the LFP battery (a type of Li-ion battery) fires were studied. The results of this study may be helpful for developing strategic firefighting and response plans for battery-powered vehicles used in mining.

Calcium carbonate is one of the highest volume industrial mineral products in North America. It is used in filler and extender applications, roofing products, floor tile, glass manufacture, etc. Calcium carbonate has over a dozen major filler and extender applications plus a host of minor uses. North American consumption of calcium carbonate fillers and extenders in 1991 is estimated at over 5,000,000 tons valued in excess of $400,000,000. There were five major suppliers of calcium carbonate fillers in the Middle Atlantic region in 1988 :?Bestone, Genstar, National Gypsum, Thomasville Stone and Lime, and White Pigment. With the exception of Genstar's Maryland operation, all of these operations are within a few miles of York, Pennsylvania. The Millington Group (TMG) purchased the assets of Bestone Corporation in February of 1989. TMG then seized the opportunity to strengthen its business position by acquiring National Gypsum's and White Pigment's Pennsylvania filler businesses in December, 1989 and July, 1990, respectively. By July of 1990 TMG's filler business acquisitions included six quarries and one underground mine, totaling over 1,200 acres and three processing facilities. The assets were restructured into two new companies - Delta Carbonate, Inc. and Penroc, Inc. (hereafter collectively referred to as Delta). Delta identified the assets of the three predecessor companies as undervalued from the outset. The initial phase of Delta's turnaround strategy was to reassess the combined mineral. mining, processing, transportation and marketing assets. The second phase was to effectively integrate these resources in order to exploit their inherent synergy.

"While there has been continual improvement in the models for predicting deposition velocity and pipeline friction losses, the fundamental design philosophy for tailings pipelines has remained the same over the previous three or four decades: design the tailings pipeline to operate in turbulent flow without a deposit on the pipe invert for all expected process conditions. Recent improvements in the understanding of slurry pipeline flows allows for the design of pipelines operating with a deposit on the pipe invert, allowing the system to operate at moderate velocities for high flow rates, optimizing system energy requirements and maximizing pipeline wear life.Tailings pipelines are increasingly operated at higher solids concentrations when there is a greater likelihood the pipeline will operate in laminar flow due to the higher tailings slurry rheology. Historically, slurry pipelines have been operated in turbulent flow. We now know that laminar-flow pipeline operation is possible as there are several operational laminar-flow tailings pipelines. While our understanding of laminar-flow slurry pipelines is not complete, recent research provides guidance on how to approach the design of these pipelines.New sensor technology has led to the development of an instrument that provides reliable indication of bed-deposit height within a pipeline. This technology is particularly helpful in operating pipelines with a deposit and has application in better understanding the operation of laminar-flow pipelines. Failure of tailings pipelines can result in consequent damage to surrounding structures and unacceptable spillage into the environment. Modern pipeline leak detection technology allows operators to better manage this risk by ensuring safe and reliable operation of tailings pipelines.This article provides an overview of the each of the above advances in the design and operation of tailings pipelines.Operating tailings pipelines with a depositTailings pipelines, and slurry pipelines in general, are typically designed to operate without a deposit on the pipeline invert. Meaning, the minimum operating velocity exceeds the turbulent deposition velocity. This approach has been adopted due to concerns regarding potential blockage of a pipeline operating with a deposit."

The Subtropolis Mine is a room-and-pillar mine extracting the Vanport Limestone near Petersburg, OH. In February 2018, mine management began implementing a new mine layout to better control the negative effects of excessive levels of horizontal stress. Almost immediately, the conditions in the headings improved. Conversely, and as expected, stress-related damage was concentrated within crosscuts. Over the last 18 months, the mine operator has diligently experimented with different techniques/methods to lessen the impact of the instabilities in the outby crosscuts. The range of controls used by the mine operator include angled crosscuts, crosscut offsets, increased distance between crosscuts, arched crosscuts, cable-bolted crosscuts, altered blasting pattern, and windows. A window is used to resist roof deformation by leaving a strong brow of roof rock within the crosscuts.

"R.E. Janes Gravel Co. is an aggregates producer is located along a paleo-channel of the Ogallala formation (locally known as the Slaton Channel) approximately 48.3 km (30 miles) southeast of Lubbock, TX. In an attempt to beneficiate the 25.4-mm (1-in.) concrete aggregate product, a log washer was installed in the processing line to liberate and remove fines (clay) from the finished product. The introduction of this extra cleaning step increased fines in the finished product, exacerbating the original problem. Several other processing options were investigated before a geological analysis was undertaken to try and define the lithology of the formation and formation characteristics in an attempt to alleviate this problem.Initial assessmentsThe mined resources are mid-Miocene to early Pliocene (16-4 mya) in age and is the eastern exposure of the Ogallala formation of the Southern High Plains escarpment. The Ogallala formation consists of coarse-grained alluvial sediments in paleovalley channel ways from the Rocky Mountains with fine Aeolian and paleosol deposition on the uplands across the High Plains region of the United States. Locally, these Rocky Mountain sediments from New Mexico were transported and deposited in a paleochannel that is approximately 0.805 km (0.5 miles) wide and 15.25 to 18.3 m (50 to 60 ft) in depth. As this channel cut across the Southern High Plains it eroded local rock sources of Late Cretaceous limestones and Triassic sandstones and mudstones incorporating them into the younger Ogallala depositional sequence.Depositional historyThe channel currently being mined is a nontypical Ogallala deposit. The Ogallala in the panhandle of Texas is divided into two units, the Couch and the Bridwell. The Couch is the older of the two and exhibits alluvial sedimentation of thicker and larger gravel (up to 76.2 mm or 3 in.) deposits fining upward to Aeolian sedimentation and thick friable caliche deposits. The Bridwell overlies the Couch and is characterized by alluvial sedimentation, a distinctive red tint and smaller gravel components (typically no larger than 25.4 mm or 1 in.) fining upward to Aeolian sedimentation and thick, well-indurated caliche cap. The Slaton Channel divides these two units and shares lithologic components of both, yet is lithologically distinct due to its Cretaceous Age limestone gravels that can be < 3.66 m (12 ft) in size. Caliche/calcrete is a calcareous (CaCO3) deposit that forms in the B horizon of soils as a rain water leachate of CaCO3 from the upper A horizon."

For decades, tunnels of all types have been a significant part of our infrastructure. In particular, water tunnels carry millions of gallons of fresh water every day to major metropolitan areas and surrounding communities. As a critical piece of public health infrastructure, it is important to inspect the condition of these tunnels regularly to ensure reliable and continuous operation. A specific challenge with water infrastructure is that it’s often difficult or impossible to shut down the system for long periods of time to perform the needed inspections. The latest method of tunnel condition assessment is the use of hybrid reality 3-D scan data. Photogrammetric and LiDAR based 3-D data ensure a reliable and objective view of tunnel conditions. The data can be used to support a detailed digital inspection including the compilation of databases that consist of a wide variety of deficiencies such as cracks and spalls within the structure. Mott MacDonald, a design engineer company worked closely with Dibit, used a 360-degree scanning system to collect high-resolution photogrammetric and LiDAR data on a recent project involving a water tunnel. The allows for full 360-degree coverage of the tunnel surface including a water covered invert. It operates at walking speed and ensures a fast and reliable data acquisition which can reduce the time of tunnel shutdowns. Several allocated inspection weeks can reduce to few days, with the final work product being a higher quality and more complete deliverable than traditional inspection methods would have provided. The true-color 3D-model has a photo resolution of 1 × 1 mm (0.039 × 0.039 inches) which makes cracks visible with a width of down to 0.3 mm (0.011 inches). The assessment of water tunnels, its technical challenges and the results of the 3D-tunnel inspection case study are illustrated and described in this publication.

The U.S. National Institute for Occupational Safety and Health (NIOSH) and the former U.S. Bureau of Mines (USBM) have put significant effort over the past four decades into researching and developing engineering controls and interventions to reduce mine workers’ dust exposures during the bagging and palletizing of industrial minerals. Workers who perform the manual bagging and palletizing of 50- to 100-lb bags typically have some of the highest dust exposures of all workers at mining and mineral processing operations. This paper provides an overview of available dust control technologies for bagging operations and presents the findings of a recent case study conducted at four industrial sand operations to identify the current types of bagging and palletizing technologies being used and to evaluate their effectiveness in reducing workers’ exposures to respirable crystalline silica dust. An evaluation is also performed to determine if further reductions in respirable dust levels can be achieved by incorporating additional modifications or improvements into the existing technology.

Progressive Design-Build (PDB) is an emerging variation of alternative delivery programs in the underground construction industry. PDB refers to the way a construction project design is developed by the Owner and the Design-Builder in a step-by-step process. According to the Design-Build Institute of America (DBIA), Progressive Design-Build allows the design and construction team to collaborate during the earliest stages of project development. This enables engagement between the three key players in a construction contract: the owner, the designer, and the contractor. Is the PDB the best alternative program delivery for large contracts? What is the perspective of the private sector? This paper will provide an overview of the different alternative delivery programs used in the underground industry, pros and cons of each of programs, and the perspective of the designers and contractors. The paper will focus on how the PDB works, reasons why owners should consider PDB for their projects, best collaboration strategies among owners, designers, and contractors to deliver a successful project, and how to manage the project risks using a PDB method.

Clays for building bricks and roofing tiles in Chile are classified as common clays and ball clays, with deposits of sedimentary and residual origin, both Cenozoic in age. Deposits are known from north to south, through 11 of the 13 Chilean Administrative Regions, 18° to 39° South latitude, and from west to east, through three of Chile&apos;s main geomorphic features, namely the Coastal Plains, Coastal Range, and Intermediate Depression. Alluvial and lacustrine sedimentary common clay deposits are located in the northern and southern Intermediate Depression. The average chemical composition of clay deposits in I and I1 Regions, is 58 percent Si02, 16 percent A1203, and 4 percent Fe203 by weight; the primary minerals are sericite, quartz, feldspar, and albite. The average chemical composition of clays in Metropolitan, VI, and VIII Regions, is 52 percent Si02, 16 percent A1203, and 6 percent Fe203 by weight; the primary minerals are montmorillonite, quartz, and plagioclase. Residual common clay deposits occur in the Intermediate Depression and in the Coastal Range internal basins, VII, VLII, and IX Regions, and they represent surficial alteration of pumice beds and crystalline rocks. The average chemical composition of clay deposits derived from crystalline rocks is 58 percent SO2, 17 percent A1203, and 9 percent Fe203 by weight; the primary minerals are sericite, quartz, and clinochlore. Clay deposits formed from pumice beds contain 47 percent Si02, 24 percent A1203, and 10 percent Fe203 by weight; the primary minerals are kaolinite, feldspar, and cristobalite. Sedimentary ball clay deposits are located in the Coastal Plains and Coastal Range, VI, VII, VIII, and IX Regions, between 34" and 39" South latitude. The average chemical composition of the ball clay deposits is 51 percent ,902, 28 percent A1203, 5 percent Fe203 by weight; the primary minerals are kaolinite, quartz, and sericite (illite?). These clay deposits supply the Chilean building sector with over one million metric tons (mt) (1.1 million short tons [st]) of material per year. The average growth rate of clay consumption during the period 1993 through 1997 was 10 percent annually. The clays are used by both mechanized plants and handcraft facilities.

Almost all of the world?s zirconium is mined in the form of zircon (ZrSiO4), the majority as a coproduct of titanium mining from heavy mineral sands deposits. These are placer type deposits, in which the dominant mineral is silica but which also contain a suite of ?valuable minerals,? often consisting of a mixture of titanium minerals, various alumino-silicates and zircon plus a range of other mineral species. The composition of the valuable minerals in such deposits is known as the mineral assemblage. There is no reasonable consistent geological relationship between zircon and titanium minerals. They can derive from separate igneous rock deposits that rarely occur together. The connection emanates once these separate rocks have been eroded by weather and water, often across a very long geological period. Once this has happened, zircon and titanium minerals often find their way to the same locations because they share two major characteristics:

As the mining industry faces increased challenges with declining ore grades and harder to access deposits, operating mine sites in an environmentally responsible and economically viable manner will require innovations that improve efficiency in energy logistics, and automation. Technological advancements in mining have been successful primarily because of the willingness and capability of these solutions to share information with one another, for example OEM payload systems sending information to Fleet Management Systems, and in turn Fleet Management Systems interfacing to ERP systems. To promote and facilitate the sharing of data, there has been a push towards open standards with initiatives led by organizations like ISO, ISA, EMESRT, and GMG. These efforts are aimed at creating a more collaborative and interoperable environment within the sector. However, when it comes to implementing autonomous solutions, a different trend dominates— closed-stack solutions. These closed stack solutions not only prohibit a mine site from selecting best-of-breed solutions, by blocking the competition from entering the market, they also risk slowing innovation that could help overcome the challenges the mining industry faces. Open Autonomy, an internationally published standard that allows any FMS to work with any Autonomous solution, breaks this closed-stack approach and invites competition from other industries. This competition comprises of autonomous suppliers from other industries and manufacturers of smaller, civil-sized haul trucks. The availability and capabilities of these smaller-sized autonomous haul trucks will help tackle the challenges of responsibly and economically mining lower quality ore grades and mining harder to access deposits.

"In 1995 and 2003, the U.S. Mine Safety and Health Administration (MSHA) conducted surveys to determine the number of atmospheric monitoring systems (AMS) that were being used in underground coal mines in the United States. The survey reports gave data for the different AMS manufacturers, the different types of equipment monitored, and the different types of gas sensors and their locations. Since the last survey in 2003, MSHA has changed the regulation requirements for early fire detection along belt haulage entries. As of Dec. 31, 2009, point-type heat sensors are prohibited for use for an early fire detection system. Instead, carbon monoxide (CO) sensors are now required. This report presents results from a new survey and examines how the regulation changes have had an impact on the use of CO sensors in underground coal mines in the United States. The locations and parameters monitored by AMS and CO systems are also discussed.IntroductionThe last survey conducted by the U.S. Mine Safety and Health Administration (MSHA) on atmospheric monitoring systems used in underground coal mines in the United States was in 2003 (Francart, 2005). A total of 146 active mines were surveyed. The survey report states that since 1995 there had been a steady increase in the use of atmospheric monitoring systems in underground coal mines as the technology of carbon monoxide (CO) sensors was improved by manufacturers. Atmospheric monitoring systems had become more reliable, and some CO sensors could be purchased with the capability to discriminate between the CO produced by a fire versus CO produced by diesel equipment. This increase in reliability had reduced the number of false alarms and the response time. The faster response time to a developing fire allowed miners to take the necessary action to put out the fire or exit the mine.At the time of the last MSHA survey, some mines still used point-type heat sensors to detect the presence of a fire on conveyor belts. This detection method works by measuring the temperature of the air around the sensor. As a fire starts to develop, smoke and hot gases are produced from the combustion process, and the air is heated up. Once the air and gases reach a designated temperature detected by the point-type heat sensor, a fire alarm signal is generated. Studies have shown that the response times for detecting the presence of a fire using CO sensors are much shorter than when using point-type heat sensors (Conti and Litton, 1995)."

Many mining engineering graduates enter the mining industry lacking training in mine emergency management. As a result, mining universities in the United Sates and other countries are beginning to form volunteer student mine rescue teams to train mining engineering students in handling mine emergency situations and give them hands-on experience managing potential emergency scenarios underground. In the early 1980s, the Missouri University of Science and Technology developed the first collegiate mine rescue team in the United States. For approximately 30 years, it was the only successful collegiate team until the Colorado School of Mines (CSM) started its program in 2009. Currently, there are at least five collegiate teams in the United States and one in Canada. At the Freiberg University of Mining and Technology in Germany, the goal is to adapt the Colorado School of Mines Mine Rescue Program to develop a collegiate team of its own. Currently, mining engineering students cover the theoretical basics of mine rescue in an occupational health and safety lecture and take a basic, one-day mine rescue training course at the Central Mine Rescue Station in Leipzig. The course covers the basic mine rescue topics and gives students the opportunity to experience working in elevated heat and smoke conditions while wearing breathing apparatus. This training gives students a small insight into the importance of mine rescue and mine emergency management. Collegiate mine rescue teaches students the importance of mine rescue and mine safety by giving them hands-on learning experiences before entering the mining industry.

"More than 6,300 people attended SME’s 2016 Annual Conference & Expo in Phoenix, AZ and many of them made the trip to the conference to take part in the excellent technical sessions that are presented on a yearly basis by the Society.Holding true to form, this year’s conference was full of options. In fact, this year there were 121 technical sessions and 751 presentations to chose from. The following is a sample of some of the technical sessions as seen through the eyes of the editors of Mining Engineering and SME technical papers.SME Sections Sharing Success: Changing thePublic Perception of MiningThis session was a workshop on how several local sections were effective in getting the message out to their communities and schools about the value of the extractive minerals industry. Several sections shared their outreach successes, including the Mining in Society merit badge activities by the Minnesota Section by Julie Lucas and SME’s participation in the World Scout Jamboree in Japan by Frank McAllister.Dan Alexander highlighted activities by the Pittsburgh Section, such as preparing minerals kits and pamphlets for elementary school classes and regional science fairs. The West Virginia University Student Chapter used the kits as handouts at their local gem and mineral show. School speakers in full mining gear were especially interesting to the young children.Other speakers were Michael Linn, Paradise Valley Unified School District — Business and School Partnerships; and Greg Kruse, Central Wyoming Section of SME — What makes a successful section?Engineering Competency ModelAmerican Association of Engineering Societies and the U.S. Department of Labor, with a grant from the United Engineering Foundation, developed a guide to the set of skills necessary for success in the engineering community -- Knowledge – Skills – Abilities — needed for all disciplines.Various engineering societies are now writing the Tier 5 occupation-specific requirements. The Competency Committee is charged to write concise and discipline-specific mining engineering, mineral processing engineering, geological engineering, and geophysical engineering by Sept. 30, 2017."

"Thousands of years ago large glaciers carved much of the landscape in Puget Sound, depositing abrasive and granular soils including cobbles and boulders along with layers of clay and sand. Tunneling in this geology – common throughout much of the northern and Midwestern U.S. – is challenging and can be unpredictable. The use of pressurized face tunneling, most commonly with EPBs and slurry type machines, has made excavation in this geology more efficient, but difficulties still remain. Through analysis of recent projects in Seattle, Washington, USA, the authors draw conclusions as to the best TBM designs and operational parameters for this variable, abrasive and often difficult ground. Performance data is analyzed with an eye towards advancements that could further improve TBM tunneling in glacially deposited soils. INTRODUCTION Over the past several ice ages, extensive glaciers reaching across much of North America transported large amounts of soil and rock as they moved across the land. The substantial weight of the ice sheets mixed together a wide range of soil types along with a variety of rocks and boulders of all sizes. Many rock formations were reduced to pebbles or particles containing highly abrasive characteristics. Any boulders able to survive were generally comprised of harder materials which were scattered throughout the soil. This highly variable mixture of abrasive soils and high strength boulders can be found in many areas across the Puget Sound and other regions of the country. Wherever they are encountered, they often create a difficult and unpredictable mining environment for both equipment design and operation. Modern TBMs are optimized early in the design process for specific types of geology, and unexpected conditions can require extensive modifications. In addition, several operational parameters need to be adjusted to balance ground characteristics, and advance rates can be significantly affected in mixed and rapidly changing soil conditions. Both EPB and slurry machines can be used successfully in these conditions, provided their respective operating criteria are met. Ideally, an EPB is operated with a relatively thick muck consistency, which accommodates moderate changes in soil permeability and maintains a reliable pressure differential through the screw conveyor. However, this also causes higher wear on rotating components and requires more energy at the cutterhead. Alternatively, slurry machines typically operate best with a more fluid muck consistency that can be easily pumped through the extensive piping system. This tends to reduce the wear on the cutterhead and the power requirements of the cutterhead drive system, but also makes the machine more sensitive to changing soil conditions. Generally, the slurry piping system is also more capable of controlling high face pressures, although the screw conveyor of an EPB can also be designed to accommodate most high-pressure conditions."

"Underground refuge alternatives require an air source to supply breathable air to the occupants. This requires pressure relief valves to prevent unsafe pressures from building up within the refuge alternative. The U.S. Mine Safety and Health Administration (MSHA) mandates that pressure relief valves prevent pressure from exceeding 1.25 kPa (0.18 psi), or as specified by the manufacturer, above mine atmospheric pressure when a fan or compressor is used for the air supply. The U.S. National Institute for Occupational Safety and Health (NIOSH) tested a variety of pressure relief valves using an instrumented test fixture consisting of data acquisition equipment, a centrifugal blower, ductwork and various sensors to determine if the subject pressure relief valves meet the MSHA requirement. Relief pressures and flow characteristics, including opening pressure and flow rate, were measured for five different pressure relief valves under a variety of conditions. The subject pressure relief valves included two off-theshelf modified check valves, two check valves used in MSHA-approved built-in-place refuge alternatives, and a commercially available valve that was designed for a steel refuge alternative and is currently being used in some built-in-place refuge alternatives. The test results showed relief pressures ranging from 0.20 to 1.53 kPa (0.03 to 0.22 psi) and flow rates up to 19.3 m3/ min (683 scfm). As tested, some of the pressure relief valves did not meet the 1.25 kPa (0.18 psi) relief specification.IntroductionThe Mine Improvement and New Emergency Response Act of 2006 (MINER Act) was enacted in the United States in the wake of three mine explosions/fires that claimed 19 lives that year. Intended to help improve underground coal mine accident preparedness, the MINER Act includes provisions that target mine safety issues in areas such as emergency response planning, adoption of new technology, training and education, and mine safety standards enforcement (U.S. Mine Safety and Health Administration, MSHA, 2006). Section 13 of the MINER Act specifically directed the U.S. National Institute for Occupational Safety and Health (NIOSH) to provide for research into the effectiveness and viability of refuge alternatives for underground coal mines. This mandate culminated in the 2009 adoption of changes to Title 30 of the Code of Federal Regulations (30 CFR) mining health and safety regulations, requiring underground coal mines to provide mine emergency refuge alternatives and associated components, to provide a life-sustaining environment for persons trapped underground. Such refuge alternatives can be either self-contained mobile units or built-inplace facilities. The regulatory changes also include provisions establishing requirements for MSHA approval of refuge alternatives and their components, and among these provisions are numerous criteria for providing a safe breathable atmosphere under positive pressure within the refuge alternatives. One specific criterion for maintaining a safe refuge alternative atmosphere requires the inclusion of an air pressure relief valve that will activate at a maximum of 1.25 kPa (0.18 psi), or at a pressure above mine atmospheric pressure, in the refuge alternative, as specified by the manufacturer when the breathable air is supplied by a fan or compressor (MSHA, 2008)."

"As engineers, we are trained to use logical, rational problem solving to insure our mines operate at maximum efficiency. We tend to use the same technical approach to design safety into all mining systems. This works well for machines, but not so much for the human component. Recent insights in the field of behavioral economics provide useful ideas for addressing the fact that we are driven by emotions more often than by rational thought. Understanding the nonrational aspect of human behavior is an important piece of any safety system design. INTRODUCTION The most common economic model is based on humans making rational decisions; that given the proper information, we will make a logical, rational decision based on what is best for ourselves (Ariely, 2009, p. xviii). In many cases this is an accurate description of how we make decisions. As an engineer, I was trained to apply the precision of mathematics and logic to solve problems. As an engineering professor, I now teach this approach to bright young minds preparing them for a life of professional effectiveness as practicing engineers. That said, we are not only rational. Daniel Kahneman, winner of the Nobel Prize in Economics, describes a model for how we think (Kahneman, 2011). System 1 is the part of our mind that is fast, intuitive, and emotional; while our logical, slower, and deliberative mind is System 2. According to Kahneman’s model, System 1 runs automatically, most of what you think and do originates in System 1. System 2 takes over when things get difficult, including the application of math and logic, and the resolution (or avoidance) of conflict. As engineers, we would typically assume that we operate in System 2 mode most of the time. This is where the model becomes useful, for Kahneman points out two important aspects of the dynamic between the two systems in our mind. First, System 1 has biases, systematic errors that lead to answering questions with an easier answer (rather than a right answer) before System 2 is engaged. Second, we can be blind to the obvious, and also be blind to our blindness. Complicating things further, System 1 cannot be turned off (Kahneman, pp. 24-25)."

"The U.S. National Institute for Occupational Safety and Health (NIOSH) has been researching refuge alternatives (RAs) since 2007. RAs typically have built-in pressure relief valves (PRVs) to prevent the unit from reaching unsafe pressures. The U.S. Mine Safety and Health Administration requires that these valves vent the chamber at a maximum pressure of 1.25 kPa (0.18 psi, 5.0 in. H2O), or as specified by the manufacturer, above mine atmospheric pressure in the RA. To facilitate PRV testing, an instrumented benchtop test fixture was developed using an off-the-shelf centrifugal blower and ductwork. Relief pressures and flow characteristics were measured for three units: (1) a modified polyvinyl chloride check valve, (2) an off-the-shelf brass/cast-iron butterfly check valve and (3) a commercially available valve that was designed specifically for one manufacturer’s steel prefabricated RAs and had been adapted for use in one mine operator’s built-in-place RA. PRVs used in tent-style RAs were not investigated. The units were tested with different modifications and configurations in order to check compliance with Title 30 Code of Federal Regulations, or 30 CFR, regulations. The commercially available relief valve did not meet the 30 CFR relief pressure specification but may meet the manufacturer’s specification. Alternative valve designs were modified to meet the 30 CFR relief pressure specification, but all valve designs will need further design research to examine survivability in the event of a 103 kPa (15.0 psi) impulse overpressure during a disaster. BackgroundThe Mine Improvement and New Emergency Response Act of 2006, known as the Miner Act (U.S. Department of Labor, 2006), was enacted in the wake of three mine accidents involving explosions or fire that claimed 19 lives that year. Intended to help improve underground coal mine accident preparedness, the Miner Act includes provisions that target mine safety issues in areas such as emergency response planning; adoption of new technology; training and education; and mine safety standards enforcement. Section 13 of the Miner Act specifically directed the U.S. National Institute for Occupational Safety and Health (NIOSH) to provide for research into the effectiveness and viability of refuge alternatives (RAs) for underground coal mines, and the U.S. Department of Labor to act on the results of such research, as appropriate. These mandates culminated in the 2009 adoption of changes to the Title 30 Code of Federal Regulations, or 30 CFR, mining health and safety regulations (U.S. Department of Labor, 2008), requiring underground coal mines to supply mine emergency RAs and associated components so as to provide a life-sustaining environment for persons trapped underground. Such RAs can be either selfcontained mobile units or built-in-place facilities. The regulatory changes also include provisions establishing requirements for the approval of RAs and their components by the U.S. Mine Safety and Health Administration (MSHA), among which are numerous criteria for providing a safe breathable atmosphere under positive pressure within the RAs. One criterion for maintaining a safe RA atmosphere is the inclusion of an air pressure relief valve that will activate at a maximum of 1.25 kPa (specified as 0.18 psi, or approximately 5.0 in. H2O), or as specified by the RA manufacturer, above mine atmospheric pressure in the RA (U.S. Department of Labor, 2008)."

In mechanized tunnelling operations performance, consumption and safety are main factors for jobsites. One of the most important components effecting these factors, especially in slurry TBM operations, are Slurry Treatment Plants (STPs). Inadequate/ suboptimally dimensioned and designed STPs create bottlenecks for tunnelling operation and jeopardize time schedules and performance. Inefficiently planned STPs could drastically increase consumption figures such as electrical power, bentonite, water and polymers, etc. Inaccurately selected and treated suspension properties can cause a high-risk potential for the whole job. This paper debates the relevant key parameters for STPs from start to finish of a project and its results in influencing factors from the manufacturer´s point of view.

For a number of years in office bull sessions, I have not so jokingly discussed the engineering or management fiasco of the year or decade. Usually, governmental projects are in the vanguard, such as tanks that won&apos;t tank, or heavy water plants that don&apos;t produce heavy water, etc., etc., all to the flutter of the taxpayers&apos; hundreds of millions, or billions of dollars. The mining industry has also brought forth a share of this type of fiasco. Ordinarily, due to lack of funds, not opportunity, the stockholders only stand losses of a few tens of millions. Little knowledge of these major errors gets to the outside world, for the ordinary stockholder is usually not very alert, and managements are extraordinarily adept at discreet midnight internments in the fine print of the footnotes of annual reports.
