THE ROLE OF MANAGEMENT IN PLANT SAFETY Safe Production In plant management, safe production is an ideal. It becomes reality when: 1. Plant employees work in healthful and safe conditions. 2. Plant safety is in harmony with productivity. 3. Worker absenteeism and turnover are low-the work force is stable. Plant management must apply its talents, skills, and resources to build and maintain the foundations of safe production on a system of policies, procedures, and attitudes. It must do so by marshaling all managerial functions - staffing, direction, control, innovation, and representation-toward that goal. Ideally, a plant safety program is proactive. Management should plan and implement safety activities that are aimed at prevention. Past accidents-accomplished facts-should not be the sole guide of future actions. In other words, positive preventive measures, not the fewness of accidents are the criteria for safety production. An ideal safety program is one which is cost effective. Management should view plant safety as part of a broader program-loss control. Loss control includes prevention of fires and other accidents, unhealthful conditions, and occurrences that cause bodily injury, illness, property loss, or damage. Management evaluates the employees' productivity and safety. An effective training program aimed at loss control is the hallmark of safe production. Ideally, both labor and management should be well versed in safety. They believe in the premise that work-related injury and illness, as well as property loss and damage from fire and other accidents, are manageable and preventable. Such belief prompts them to learn more about the work process and potential hazards associated with each job. They also learn how to expect and prevent fues and other accidents, and respond to emergencies at the plant. JOHN HAMRIC: Our next topic leader is David Huth, superintendent of sulfuric power generation, DAP, and shipping for IMC-Agrico, Nichols Plant, Mulberry, Florida. With a title like that it's no wonder he believes in empowerment. His topic today is "The Triumphs and Pitfalls of Using Empowerment Techniques." DAVID HUTH: Empowerment and its benefits are a desirable alternative to the traditional management structure. The difference between the two is the amount of decision- making authority and responsibility given to individuals by a supervisor. Just what is empowerment and how does it work? Empowerment is relinquishing to individuals the authority and responsibility to make daily decisions. Can everyone be empowered? What if they make a wrong decision? Will I empower myself out of a job? These are questions that all managers have when they hear the word empowerment. In today's fast-paced and high-pressure business environment, the old way of managing people must be replaced with a new approach. No longer can we, as managers, make all the decisions. The increased work loads due to downsizing, consolidation, and synergies have stretched us too thin. What about our employees? How can we motivate them into the high-performance teams that our companies are demanding? What possibly could satisfy both these issues? The solution to these problems can be solved with the power of employee empowerment. Employee empowerment will not only lessen the load on managers but will build teamwork, motivation, and pride into the employees who work for the managers who use it. American business was rudely awakened in the early 1980s when the Japanese companies not only caught up to American companies but literally flew by them. America was fat, dumb, and happy after World War II. Why wouldn't they be? They had all the factories. Everyone else had to rebuild. So for 40 years American business lounged in the limelight of very little true competition. But times change. Unfortunately, American business did not. Now the cry to perform in the new global economy has come down from the corporate boardrooms. Down through the ranks it has been felt. Guess who is supposed to make it work? But where are all those people who are going to make this rejuvenation and corporate renaissance possible? Many of them are gone-lost in downsizing, synergies, and rightsizings

As of the end of October, 1990, there have been eight spontaneous combustion heatings reported to the Mine Safety and Health Administration (MSHA) at the No. 5 Mine. A reportable heating is defined as one with elevated surface temperatures of the strata greater than ll°C (20°F) above the ambient. The first heatings occurred in November, 1986. The last three heatings were located near the start line of "K" Longwall Panel in the southwest where mining had begun on May 21, 1990. All of the heated areas had been at the time of detection, or were sometime areas of high ventilation pressure differentials or high air velocities. All of the heated areas are in an expanded seam with associated, extensive floor heave which is characteristic of all of the expanded seam thus far exposed by development mining in the No. 5 Mine. Jim Walter Resources, Inc. submitted a plan to MSHA which would reduce the potential for spontaneous combustion at the No. 5 Mine by reducing high air velocities and high pressure differentials in the gob. These two parameters are the dynamics which feed oxygen to the pyrite, thus initiating the oxidation process which can lead to spontaneous combustion. No. 5 Nine operated under this plan for 29 shifts before mining on the longwall was stopped by MSHA and the Spontaneous Combustion Control Plan revoked. This paper briefly details the geology of the expanded seam and history of the heatings. It also gives a retrospective view of the operation under the Spontaneous Combustion Control Plan and reasons why it was revoked by MSHA. Currently, knowledge of the reaction process of the footwall coal seams at No. 5 Mine and other potential spontaneous combustion seams in the United States is limited. There are techniques used in the United States and other countries to minimize the potential for heatings in spontaneous combustion prone areas. The No. 5 Mine has attempted to utilize this current technology. However, in deep, gassy mines the problem is complex.

Basement rocks in southern Oklahoma are divided into two groups. One group is the Lower Cambrian Wichita Igneous Province, a diverse igneous suite that crops out in the western Arbuckle Mountains and in the Wichita Mountains. These largely bimodal igneous rocks were emplaced in an intracratonic-rift environment. The other group, the older, is a set of Precambrian rocks of the Eastern Arbuckle Province, yielding ages between 1,350 and 1,400 Ma, exposed in the core of the eastern Arbuckle Mountains. These are deeply eroded mesozonal granitic rocks and associated granitic gneisses cut by numerous dikes ranging in composition from diabase to rhyolite. The southern Oklahoma igneous rocks have long been utilized mostly as dimension and crushed stone. Most of the early quarries were in granite of various red hues for monuments and facing stones. Large crushed-stone quarries are now exploiting both granite and rhyolite, which is used chiefly as aggregate and railroad ballast. The exposed basement rocks in Oklahoma represent an underdeveloped and accessible resource.

The pressure system of ventilation with large diameter bore holes has been very effective in the ventilation of two gaseous mines, the Olga No. 1 and No. 2 Mines in McDowell County, West Virginia. The No. 4 Pocahontas Coal Seam on the Olga property lies 580 feet below the valleys. The seam is very gaseous as well as dry and dusty, which makes good ventilation all the more essential. Irregular and often extensive areas of the coal seam have been replaced with impervious shales and sandstones in "Fault" or "Want" areas. The replacement materials have sealed the methane in the coal seam and thus increased the quantity of gas that must be handled as mining is done. Often the No. 5 Pocahontas Seam comes down to meet the No. 4 Seam at the Faults and considerable methane is sometimes released from the upper seam. The factors mentioned, together with the irregular and unpredictable lateral shape' of the faults, result in long narrow stringers of coal that are very gaseous and difficult to mine. So far as the Olga Coal Company is concerned, the most effective ventilation under the conditions outlined has been the use of large diameter bore holes with a pressure ventilation system. The ventilation of a mine can be accomplished in two fundamental ways. The mine atmosphere maybe reduced below atmospheric pressure by an exhaust fan; or it may be raised above atmospheric pressure by a pressure fan. In either case, the coursing of the air is so planned as to have the haulage and traveling ways on intake air. While the majority of the coal mines in the United States, no doubt, use the exhaust system of ventilation, there are several advantages of the pressure system which are extremely beneficial.

Dealing with a trend in inverse distance estimation, IWD, is presented. A measurement is proposed to be consisted of a portion due to a mean and a portion due to a residual. The procedure consists of de-trending the measurement via a linear or nonlinear regression. Residuals are computed, and IWD parameter estimation is conducted on the residuals. Estimation at un-sampled points is then computed by estimating the mean and adding on the estimate of the IWD on the residual. A possible BOXCOX transform is also proposed.

World food production is not matching the exploding increase in world-population. Since arable land is limited, ways are being sought to increase yields by turning_ to fertilizer. Phosphate, an essential fertilizer ingredient, is undergoing a period of growth unparalleled in history. The United States is at once the world's largest producer of food and of phosphate rock. Both food and fertilizer output in the United States are being enlarged to meet the demands posed by the threatening world food crisis. New phosphate deposits are being developed in Florida and North Carolina to assure the United States' continued preeminence in this vital resource. Large fertilizer complexes are also being constructed with an eye to both domestic and foreign markets. The growth of the U. S. phosphate industry has been characterized by developing technology in the processing and distribution of phosphate raw materials. The trend from low analysis to high analysis materials accompanied by a change from a large number of small fertilizer plants oriented toward the marketplace to a fewer number of large plants located at the raw material source is illustrative. These changes in technology and distribution have also been important factors in the development of new mining areas in Florida, the Western States and North Carolina.

This paper will introduce and discuss a range of techniques that utilise new developments in Computer Graphics (CG) and Virtual Reality (VR) for the investigation of incidents and accidents in mining environments. The future of mining safety may not be totally led by the demands of the mine operators, but sometimes by the academics, commercial software developers and legal pioneers who generate new ideas and products and set admissibility precedents which may change the way certain traditional investigative operations are performed. In any accident investigation the outcome from the investigating team is often a complex affair and the results of the investigation are often reported in large, convoluted documents. Computer-generated forensic reconstructions allow investigators to present accident scenarios in an accessible and easily understood, visual manner. Surveys in the US have also shown that people have increased recall of computer-generated images over many other forms of evidence. The AIMS Research Unit in the UK have been pioneering the use of Computer Graphics (CG) and Virtual Reality (VR) technology for both industrial and road traffic accident reconstruction. This paper and accompanying presentation will contain examples of forensic reconstructions developed by the authors that explain some of the methodologies used to ensure admissibility of this type of digital evidence. The paper will contain a number of case studies based on UK and International industrial accidents. The paper will focus on how AIMS Research is extending the capabilities of the accident reconstructions created to take advantage of the latest developments in VR technology. As digital technology makes its impact within professional and commercial fields, computer generated forensic reconstructions will form important admissible evidence in the courtrooms of the future. The technology will also make a great impact in the way we train our workforces, with highly visual three-dimensional (3D) interactive training tools becoming the norm. Acceptance by the public, courts and workforce is an important issue, and implementation strategies must be considered carefully.

Barrick Goldstrike Mines is improving the reliability of mining and processing equipment through the use of failure analysis and condition monitoring activities. Program results include an increase in equipment capacity and an overall reduction in operating costs. Identifying equipment failure modes and developing associated capital asset management practices can minimize failure consequences. Successful programs have included alterations to management practices, equipment modifications, and/or the implementation of condition monitoring programs. By controlling or minimizing the effects of incipient equipment failures, corrective actions can be taken earlier in the failure cycle. This results in an associated reduction to repair costs and equipment down time. Mining companies invest in their capital assets to provide the capacity necessary to meet operational needs. Over time, functional failures occur that reduce this productive capacity. Diagnostic technologies now exist that allow the condition of equipment to be monitored. Early response to this information minimizes the effects of functional failures and restores equipment re- liability. Reliability is a performance indicator of overall equipment condition. It is most often expressed in terms of mean time between failures. When the established objectives of a capital asset management program include the maximization of this failure interval, trailing indicators, such as availability and operating cost improve. The probability of equipment failure is influenced by engineering design and operating conditions. The performance level resulting from this operating environment is expressed as "inherent reliability." Without maintenance intervention, equipment will ultimately fail to perform its intended function and become less reliable. Therefore, maintenance must be managed as the supplier of reliable operational capacity. Maintenance can only supply equipment performance that approaches the level of inherent reliability. Actually achieving inherent reliability requires that all maintenance decisions are made at the economically optimal time. Enhancing performance levels beyond inherent reliability can only be achieved by altering operating conditions or practices or through physical equipment modifications. An evolution in maintenance management philosophy has been under way since the beginning of the industrial revolution. As equipment has become more sophisticated, so have maintenance practices. At the beginning of the industrial age, equipment tended to be over designed and slow moving. The first maintenance philosophy was one of reactive maintenance. Maintenance intervals were dictated by the operational status of the equipment. As a result, production interruptions were common and capacity needs were met through an investment in redundant capital assets.

Climax Molybdenum Company, a Division of American Metals Climax, Inc., operated a hydrometallurgical plant* at Climax, Colorado from August 1966 through August 1968 to recover molybdenum from a mixed oxide and sulfide ore. The ore treated for this addition to the existing plant contained sufficient Molybdenite values to be mined for Molybdenite recovery alone, so that the oxidized molybdenum recovery could be regarded as a byproduct operation. Climax is located at an elevation of 11,300 feet. All process equipment, except thickeners, was enclosed in buildings. Annual snowfall is approximately 250 inches per year. Paved highways and rail service to the plant are maintained and kept open all year. Most of the employees live in the town of Leadville, which is approximately twelve miles from the plant site. The sulfide flotation plant handles 40,000 to 45,000 tons per day of ore. The mine is one of the world's largest underground mines.

An actor-critic reinforcement learning approach is presented to improve the production of an industrial mining complex by defining shovel allocation and adapting the short-term plan given grade-control decisions. Blasthole data updates the simulated orebody models, which are input into a stochastic grade-control method based on a spatially constrained clustering approach that minimizes the profit-loss function. These aspects are embedded in a discrete-event simulator that defines the material flow from faces to processors. A case study at a copper mining complex shows the methods’ ability to adapt to new information, improving the quality of the material feed and increasing cash flow by 23% compared to a baseline case.

Perspective Man is a model maker. Throughout recorded history man has-used models both to provide insights into the workings of the processes which surround him and as a step towards the eventual control of those processes. These models have covered the field from extremely large systems (solar and celestial. models) to infinitesimally small systems (sub-atomic .and atomic models). Models have been developed for social systems, ecological systems, biological systems and economic systems: all fields of human endeavour contain the fruits of this restless model-making. The Role of Models Since the evidence of mans' industry in formulating models is so abundant, it is worthwhile stating some of the reasons. Based on the simple definition that "a model is a convenient and useful representation of some facet of the 'real' world", then a model provides a means for: 1. Understanding the thing being studied. 2. Communicating that understanding to others. 3. Manipulating the thing being modelled. The common denominator implicit in these reasons is that- improved control is the. objective off all modelling or simulation work. Improved control shows itself either directly by the development of devices which perform the control functions or indirectly by improvement in the decision making process itself. For example, in a process plant, a suitable model of part of the process could lead either to the use of automatic control devices such as set-point controllers or to the implementation of instructions (pre-defined decisions) which permit the operator to better control the process. Typical process control systems use both of these since fully automatic control is restricted to processes where the required information can be effectively measured.

Although most bauxite mined is used to make metallic alu¬minum, more than a million tons of bauxite is used annually in the United States in the manufacture of refractories, chemicals, abrasives, and cement. Raw material requirements are becoming increasingly difficult to meet because high-grade bauxite, required for some uses, is limited. The supply of raw material for high-alumina refractories is most critical because a low-iron bauxite is needed. Although there are substantial bauxite reserves in the world, bauxite suitable for use in refractories is limited to a few districts.

The heat of immersion of coals in different immersional liquids is measured as a function of particle size and moisture content. Decrease in particle size shows a concomitant increase in the heat of immersion. The heat of immersion is dependent upon the nature of immersional fluids. In addition the nature of moisture present in coal influences the heat of immersion.

A quiet revolution has taken place in the conveyor belt industry, Man-made materials, improved production facilities, better splicing techniques and emphasis on repair and maintenance have all contributed substantially to lowering material handling costs for the mining industry, It wasn't too many years ago that the rubber industry had only cotton for a belt carcass and tree grown rubber for the protective cover. Then came the development of monofilament rayon. This material had desirable elongation and elastic properties and was the first synthetic fiber to be used in place of cotton. But wet strength and other limitations restricted its use, and the conveyor belt industry turned to nylon. Initially, nylon was considered suitable only for the cross threads or filler threads of a belt carcass. Then technical advancements in heat setting and fabric weave created a major change in belt construction. As a result of these advances the industry was able to take advantage of the full tension strength of nylon cord. Another breakthrough in the use of synthetics was the tremendous increase in adhesion resulting from a chemical bond, rather than the limited physical bond available with cotton. But the advantages of nylon had a more Car-reaching effect, We were able to design a two-ply nylon belt that did the work of a four- or five-ply cotton or rayon belt. A six-ply nylon belt now replaces a twelve-ply cotton elevator belt. Arid nylon resists moisture, acid and mildew, It also has excellent resistance to impact and can be produced with controlled elongation and elastic properties that work to great advantage in reduced-ply belt constructions.

Blending inevitably occurs to some degree during mining and mineral processing operations. Traditional mine scheduling assumes that all variables average linearly, which allows the calculation of the blended material properties as a linear weighted average. However, metallurgical properties such as metal recovery and Bond Work Index (BWI) are known to average nonlinearly. The nonlinear properties of the blended material can be calculated using nonlinear blending models. A simulated annealing algorithm is developed and applied to a short‐term mining face optimization problem to maximize the total metal recovered considering nonlinear blending. Compared to a standard schedule plan, the optimized one shows relevant improvements in synergistic and antagonistic blending behaviors.

Gold was discovered at the Haile mine in Lancaster County, South Carolina in 1827 or 1828, and since that time the mine has been worked intermittently by both open pit and underground methods until its forced closure in 1942 by World War II. Production figures are incomplete, especially for the early years, but the total gold produced is estimated to have been greater than 200,000 ounces. Thus, the Haile mine has been the most productive gold mine in the eastern United States. The upper, residually enriched ores were relatively rich, but the bulk of the production has come from the mining of lower grade ores.

The main problem encountered in separating primary and secondary metals is to selectively control the solubility of the individually important metals with¬out affecting their ultimate recoverability As a class of hydrometallurgical chemicals, peroxygens have a versatility which permits the selective recovery of individual metal species. "The proliferation of unconventional gas well development in the Northern Appalachian coalfields has raised a number of mine safety concerns. Unconventional wells, which extract gas from deep shale formations, are characterized by gas volumes and pressures that are significantly higher than those observed at many conventional wells. The gas is composed largely of methane as well as other hydrocarbons. Hundreds of planned and actively producing wells penetrate protective coal pillars or barriers within active mine boundaries, including chain pillars located between longwall panels. Gas released from a well damaged by mining-induced ground movements could pose a risk to miners by flowing into the mine atmosphere. The mining-induced ground movements that may cause well damage include conventional subsidence, non-conventional subsidence (e.g., bedding plane slip), pillar failure, and floor instability. This paper describes the known risk factors for each of the four failure mechanisms. It includes a framework that can guide the risk assessment process when mining takes place near gas or oil wells.INTRODUCTIONSouthwestern Pennsylvania has served for centuries as a producer of coal, oil, and natural gas. Coal mines were first developed in the Pittsburgh Coal Seam in the 1760s, with the first recorded mine in 1761 being the Fort Pitt Mine (Edmunds, 2002). Natural gas was introduced to the region when the Haymaker Well in Murrysville, Pennsylvania experienced a blowout on November 3, 1878. Although it was originally drilled prospecting for oil, it produced an estimated 30 million cubic feet per day of natural gas. Without well controls or pipeline infrastructure in place, it burst into flames and lit the night sky for over a year. Soon after, the Penn Fuel Company continued development of the Murrysville gas field and is credited with making Pittsburgh, Pennsylvania the first major city in the U.S. to have natural gas supplied for home and industrial uses. “Natural gas is king in Pittsburgh,” proclaimed an October 18, 1885, article in the New York Times headlined “Value of Natural Gas, Its Use Clarifies the Atmosphere at Pittsburgh” (AOGHS, 2018). By the early 1900s, coal, oil, and natural gas production had reached the southern extents of Pennsylvania in Greene County. The county had first produced oil in 1886, and shortly after began producing natural gas in 1890. By 1906, over 1,300 wells had been drilled (Stone and Clapp, 1906)."
