The model originally proposed by Austin, et al., has been modified in the light of further investigation of the breakage laws in SAG milling. The model uses a fully-mixed reactor for short L/D mills and calculates the breakage of small particles by the nipping action of balls and pebbles. The autogenous self-breakage of larger lumps is treated as an effective means of fast breaking rock and much slower breaking rock. Experiments show that fast autogenous breakage can be treated as a first-order breakage with an overall specific breakage rate which incorporates fracture and chipping. Similarly, slow breakage incorporates fracture and abrasion. Autogenous breakage is cushioned by the accumulation of fine material. Simulation results are compared to results from tests in a 2 m diameter continuous pilot-scale SAG mill. Future work is discussed.

A number of recent studies 1-4 suggest that steelmaking expansion might be achieved more economically by a combination of the prereduction furnace and the electric steelmaking furnace rather than through new blast furnace construction. Supergrade concentrates would be the first step in an evolving technology which bypasses the blast furnace and in which the concentrates are first made into pre¬reduced pellets and then used directly in electric steelmaking, Prereduced pellets made from superconcentrates should not only be competitive with scrap, but should be a preferred feed for the electric furnace because of the absence of such residual elements as copper and nickel. Furthermore, geographic areas remote from present steelmaking centers may find the electric furnace to be an economical method of providing local steel requirements. The development of successful methods of producing superconcentrates may also be expected to find application to conventional blast furnace burdens. The need for methods of increasing grade of domestic concentrates will become increasingly urgent in the face of competition from high grade imports. Only a small percentage of domestic iron ore production can qualify as superconcentrates, (Superconcentrates for the purpose of this investigation are limi¬ted to those concentrates which contain a maximum of 2 percent S'02). During the last several years the Twin Cities Metallurgy Research Center of the Bureau of Mines has directed research efforts to develop methods of manufacturing superconcentrates from present standard grade products by cationic flotation, screening, and magnetic separation.

"The reactivity of an enargite mineral electrode was investigated spectroelectrochemically under conditions typically found in alkaline sulfide leach solutions, with pH 11-13 and Na2S additions of 50 and 250 gL-1. A bulk enargite electrode was cycled between 0 and -1,000 mV for each set of solution conditions. Raman spectroscopy was used to determine and compare surface formations against species in mass-balanced EH-pH diagrams generated using STABCAL. Previously, it was determined that arsenic could be selectively leached under alkaline conditions at an average solution potential of -300mV, across the pH range 11-13. Operating under such conditions allowed for the selective leaching of arsenic with elemental sulfur formation. From several of the voltammograms obtained during this current testing, the formation of elemental sulfur appeared at increasingly reducing conditions with increasing sulfide concentration. At high concentrations, copper started to dissolve, possibly as a bisulfide complex, reducing the selectivity of the leach, unlike previous studies. However, the presence of the higher sulfide concentration is also anticipated to prevent passivation by consuming any surface formation of elemental sulfur. This study provides insight into the operating parameters of alkaline sulfide leaching in regards to the selectivity of arsenic leaching, as well as guidance for control of reagents and solution potential and, of course, the need for water treatment afterwards.IntroductionEnvironmental limitations on the pyrometallurgical processing of arsenic-bearing concentrates have forced operations to consider other solutions. Attempts to hydrometallurgically process enargite (Cu3AsS4) fall into two categories: the co-dissolution of arsenic and copper under acidic conditions, and the selective leaching of arsenic under basic conditions. The authors believe that the selective leaching of arsenic is more amenable because it produces a solid copper phase, preferably chalcocite (Cu2S) or covellite (CuS) that could then be processed using conventional methods already in place in the copper and gold industries. Potential leaching methods include alkaline sulfide leaching (Achimovicovica et al., 1999; Anderson and Twidwell, 2008a,b; Tongamp et al., 2009 and 2010), hypochlorite leaching (Vinals et al., 2003; Curreli et al., 2005; Mihaljovic et al., 2007), ammoniacal leaching (Gajam and Raghavan, 1983), and acid leaching (Gow et al., 2014b) with or without fine grinding to mechanically activate the enargite surface and increase normally slow leach reaction kinetics (Achimovicova et al., 1999; Balaz et al., 2000; Balaz and Achimovicova, 2006). Stability in Exxon's Colony Pilot Mine in the Piceance Creek Basin, Colorado has been monitored by periodic pillar stress-determinations End roof-to-floor convergence measurements since 1971. Pillar failure has caused marginal stability in some areas of the mine. Measurements taken during the process of failure provided valuable information on the in-situ pillar strength and long-term behavior of the mine structure. Four pillars were reinforced by 32 mm (1-1/4 in) diameter tensioned grouted bolts to increase long-term stability in the northern area of the mine. Stability of this area is necessary because future plans envision its use as an exit/access and ventilation exhaust. Computer analyses were performed to help evaluate the effect of pillar reinforcement on long-term stability. Results indicate that bolting is particularly useful in stabilizing failed pillars, but its effects are significant only in the immediate area of the pillar. The projected significant decrease in deformation of two extensively failed pillars near a main entry indicates that bolting reinforcement will assist the long-term stability of the opening.

An experimental procedure was developed for studying magnetic roasting on a accurately-controlled laboratory scale. The loss-in-weight method was used and the course of reduction could be followed under a reducing gas atmosphere containing as many as five different components. Some important variables of the operation were checked on a marginal Mesabi iron ore containing porous and dense varieties of hematite as well as pure goethite. The conditions simulated were those for the grate roasting of a coarsely-sized ore burden. Temperature was maintained at 600°C; gas composition at 4.25% CO, 4.25% H2, 17.0% CO2, 15.0% H20, and 59.5% N2,, and gas flow rate at about 1,5 lineal feet per minute. The ore charge was composed of quarter-inch cubes. Under these conditions, the ore porosity was found to be one of the governing variables of the operation. Results showed that an ore size limitation existed as a function of this ore porosity. For efficient magnetic conversion, the size limit was 3/8 inch for very porous ores and about 65 mesh for very dense ores. An anomaly was noted in the weight losses incurred during actual magnetic conversion. For goethitic ores, these were generally about 13 per cent higher than could be accounted for theoretically. The effect was attributed to possible non-stoichiometric structures forming during the reduction process.

During coal mine operations, roadways may be located close to faults. When designing these roadways, it is important to determine the width of the rock mass between a roadway and a fault area to prevent water inrush. In this paper, the Xintian Coal Mine in the northern Guizhou Province of China is selected as a typical case. Through numerical simulation based on orthogonal tests, range analysis, and multiple linear regression (MLR) analysis, the process of water inrush from faults in excavation roadways is studied. The results show that the influencing factors of the safe width of a rock mass preventing water inrush (RMPWI) can be ranked according to the degree of influence from large to small as follows: the hydraulic pressure of the fault, the surrounding rock grade, the overburden depth of the roadway, the cross-section height, and the dip angle of the fault. Among them, the dip angle of the fault, the cross-section height, and the surrounding rock grade are negatively correlated with the safe width, while hydraulic pressure of fault and overburden depth of roadway are positively correlated with safety width. The predicted value for the safe width based on MLR is greater than the measured value of the width of the RMPWI when water inrush occurs, but less than the calculated value based on the empirical formula in the standard, which proves the accuracy and practicability of the prediction method. This research provides a certain basis and reference for the prevention and control of water inrushes in mines.

The understanding of public debates over mineral exploration and mining largely originates from exceptional situations such as mining accidents of conflicts. Less is known about how mining is portrayed and understood under more conventional settings. What storylines dominate the local day-to-day public debate? This article presents results from a comparative case study focusing on newspaper coverage of mineral exploration and mining in three European countries representing different geological and socio-economic contexts. Newspaper articles from the Geyer-Erzgebirge region in Germany, the Andalusia region in Spain, and Northern Finland are studied. The sample looks into the period between September 2018 and February 2020 and shows that regional newspapers report about mining issues relatively intensively even in the absence of major accidents or other media events causing peaks of attention. The tone of the articles is generally neutral to positive towards mining activities, reflecting the specific local settings, historical experiences, and future expectations. Despite the different contexts of the three countries, there were considerable similarities to the topics highlighted, including common themes of mining revival, mining events and social interaction, history of mining, and damages related to mining. Past, present, and future employment opportunities related directly or indirectly to the mining sector are key storylines. Another recurrent underlying theme is the need to balance environment and safety risks and socio-economic prosperity, typically covered through ordinary disputes among the mining sector, public authorities, regional non-governmental organizations, and local initiatives.

The Cutoff grade optimization is very critical to the open pit production scheduling. In case of single mineral deposits, algorithms have been developed for determining cutoff grade policy that maximizes the net present value (NPV) of yearly cash flows. For deter-mining the cutoff grade policy for projects with two minerals including stockpiles the problem becomes much more complex. The purpose of this paper is to present fundamentals of a generalized cutoff grade optimization algorithm that will maximize the NPV of a project subject to mining, milling, and marketing constraints. Benefits of the methodology developed are demonstrated on three different case studies.

US coal production increased 3% to a record 832 Mt (917 million st) in 1987, according to preliminary Energy Information Administration (EIA) data. Most of the increase was due to the unusually warm summer and the increased coal-fired electricity demand for air conditioning. Drought conditions in the Northwest lowered hydroelectric generation, contributing to the higher use of coal by electric utilities. However, excess production capacity and increased competition from natural gas added further downward pressure on delivered coal prices, continuing the declining price trend of the last several years. Coal production by region Appalachian coal production reached 402 Mt (443 million st) in 1987. This is about 3% more than in 1986 and just short of its 1984 peak. Virginia produced 7% more than its 1986 level, due to increased coal-based electricity demand. Smaller percentage increases were attained in eastern Kentucky, Pennsylvania, and West Virginia. Ohio and Alabama were the only major coal-producing states in Appalachia that produced less coal in 1987 than in the previous year. In the Interior region, 1987 coal production reached a record 182 Mt (201 million st), about 2% above 1986. Illinois is the largest coal-producing state in that region. Production there fell about 3.7%, primarily because of lowered demand at electric utility plants in Alabama and Tennessee. However, this was more than offset by production increases in western Kentucky, Texas, and Indiana. After falling slightly in 1986, coal production in the West resumed its upward trend to a record 248 Mt (273 million st). Wyoming is that region's dominant coal producer. It mined 7% more coal than in 1986. Wyoming's increase offset the 11% decline in New Mexico production and smaller declines in other Western states. Coal consumption and prices Domestic coal consumption reached a record 759 Mt (837 million st) in 1987. Most of the increase resulted from the unusually warm summer that led to higher than normal peak demands for electricity in the third quarter. Electric utility coal consumption was also boosted by the below-normal hydroelectric generation that resulted from low water levels in the Northwest. These factors more than offset the effects of increased nuclear power generation and competition from natural gas. As a result, coal provided 56.9% of electric utility generation in 1987, compared with 55.7% in 1986. Electric utilities consumed a record 651 Mt (718 million st) of coal. While electric utility coal consumption increased, the average cost of coal to utilities was down, from $1.58 per million Btu in 1986 to $1.52 in 1987. The average price of coal delivered to utilities in 1987 was $35.09/t ($31.84 per st), the lowest since 1981. Coal prices were held down by a glut of coal supplies and by a decline in the average price of natural gas to electric utilities. Coal consumption at coke plants in 1987 was 2.6% above the 1986's 32.5 Mt (36 million st). This increase reflects steel's recent resurgence. Coking coal demand, though, was reduced by the increased use of continuous casting and electric furnace technologies. The average price of coal delivered to coke plants fell from $56.02 to $51.35/t ($50.83 to $46.59 per st). Coal consumption in the industries other than coke plants amounted to 68.2 Mt (75.2 million st) in 1987, slightly less than in 1986. Coal lost part of its market share to natural gas, which was priced favorably relative to coal. The average price of coal delivered to manufacturing plants was at its lowest level of the current decade. It fell from $39.50/t in 1986 to $37.27/t ($35.84 to $33.81 per st). Residential and commercial coal consumption fell to 6.2 Mt (6.9 million st) that is 9.8% below 1986 and the lowest since 1980. This decline is also attributed to the relatively low price of natural gas in 1987. Coal stocks Coal consumers held 169 Mt (186 million st) of coal stocks at the end of 1987, 5.9% above the 1986 year-end level. Most of this increase was due to stockpiling by electric utilities in anticipation of a possible strike by the United Mine Workers of America early in 1988. Stocks at electric utilities increased to an estimated 154 Mt (171 million st) at the end of 1987, 5.6% higher than the previous year. This was much lower than 1984's stock buildup. Coke plants increased their coal stocks by 30% in 1987 to 3.5 Mt (3.9 million st), rebounding from the unusually low inventories of 1985 and 1986. There are three parties to the oil imports controversy: the domestic oil producers, whose side has just been ably summarized by Mr. Jameson; the oil importers; and the public. Domestic producers and importers are both motivated by economic self-interests, that is by their natural and legitimate desire for profit maximization, in their respective positions. Thus, domestic producers, rightly, regard every barrel of imported crude oil (with the exception of some specialty crudes) as a displacement of a barrel of their own crude. Hence, the lower the import Level the better they like it. Conversely, U .S. refiners whose plants are located at the seaboard or at certain inland waterways generally welcome a higher level of imports because refiner profits are based primarily on the spread between the crude oil price and the refined products price, since crude oil represents by far the refiner?s largest cost factor. The relatively low cost of foreign oil means therefore higher profits to those refiners able to process such oil, This applies particularly to integrated companies whose own domestic oil production is insufficient, to meet their refinery requirements but who have developed ample low-cost foreign oil supplies.

The design of most underground support systems has largely been based upon empirical rules. This design method has proven to be satisfactory in most cases as witnessed by the number of successfully completed works. However, the adequacy of this approach to the developing technology of ground support systems associated with deep excavation has not been substantiated. In cases where the material has definite time-dependent behavior parameters, rules of past experience become extremely vague and conservative. The Spokane Mining Research Center is currently engaged in extensive research to develop rational design methods for underground structures which will account for the many interacting factors such as in-situ properties of the rock mass, excavation methods, proposed types of support system, construction sequence, and environmental variables. The energy problem of the U. S; was emphasized by the oil embargo of late 1973 and early 1974 and it seemed for a time that the Federal Government, under pressure from the general public, might take some corrective action. However, this was not the case. With the gasoline lines gone and an apparent plentiful supply of petroleum products, public concern quickly dissipated., This country is more vulnerable to an oil embargo today than we were in 1973-1974 and this fact should be of concern to all our people. Since 1968, we have used oil and natural gas at a much more rapid rate than we have been able to find new domestic sources; thus, there is an increasing gap between domestic production and consumption and we have filled the gap with ever increasing oil' imports. These imports have increased at an alarming rate in the recent past, some 58 percent in 1976 compared with 1975. Imports are nearing 8 million barrels per day. Then, by applying some simple arithmetic, at $12 per barrel, the resulting trade deficit is $96 million per day or $35 billion per year. Obviously, this cannot be tolerated by our economy over a long period of time. Historically, we have shifted from one fuel to another as is shown in this slide. In 1850 about 90 percent of our fuel requirements came from direct combustion of wood, by 1900 we shifted to coal for 70 percent of our fuel, and now about 78 percent of fuel requirements comes from oil and natural gas. The difference is that the previous shifts were voluntary and the one facing us now is of necessity.

Increased coal production on longwalls has brought with it higher levels of dust generation. Most U.S. longwall mining operators have difficulty complying with Federal dust standards, which are stricter than European standards. Longwall operators faced with a longwall that is "out of compliance" have implemented a variety of practices designed to reduce dust levels. Mining conditions and other important variables, however, can cause fluctuations in dust levels that may overshadow the performance of a new dust control technique. This can result in the continued use of an ineffective technique or lead to the abandonment of an effective dust control technique. No single technique can keep a longwall in compliance. Maintaining compliance requires a fundamental approach to dust control -- proper application and continued use of effective dust control techniques.

The use of cyanides in the extraction of gold and silver is well known.1 Such extractions employ con¬centrations of cyanide in the range of 0.02-0.25 % sodium cyanide equivalent in leaching cycles of 24-48 hours and frequently longer. Copper minerals, even though minor components of precious metal ores, dissolve in these cyanide leaching solutions, consume cyanide, cause fouling of mill solutions, and thus interfere with the precipitation and recovery of gold and silver. 2,3,4 This paper presents a summary of laboratory investigations on the use of cyanide solutions as extractants for copper from various copper-bearing ore fractions. In view of the metal contents involved, it will be apparent that larger quantities of cyanide are required to extract copper than for the extraction of precious metals. In cyanide solutions of concentrations generally favorable to copper extraction, sulfide and oxide copper minerals were found to dissolve rapidly at room temperature and atmospheric pressure. For example, on products in the flotation size range, the major portions (e.g. 65-85%) of the extractable copper frequently dissolved in 2-10 minutes. At coarser grinds, e.g. 100% minus 28 mesh and 100% minus 10 mesh, 75-85% of the total copper was extracted in 30 minutes.

"The development of mining safety and health management as a recognized profession has been long in coming. Over the last 150 years, the mining industry has evolved from having individuals randomly involved in safety related work with no specific responsibilities or qualifications, on to the designation of individuals responsible for safety, regardless of knowledge, qualifications and experience and finally to today’s expectation that virtually every mine and mining company has designated individuals operating as safety and health professionals. Further, with the complexity, risk potential and moral expectations surrounding safety, people who address mine safety and health management are justifiably expected to function in a professional capacity including:1. Rendering a specialized service based on advanced knowledge and skill.2. Maintaining a high degree of obligation to those most affected by mine safety outcomes.3. Bound by an ethical code in working with the public, employers and miners.4. Maintaining confidentiality with an employer or client. This historical change parallels thedevelopment of other professions, e.g., attorneys, engineers, physicians, etc., with the exception that those professions require a license granted by a governmental agency, which is justified on the basis of public protection, in order to practice. Likewise, decisions made by today’s mine safety and health professionals, both directly and in association with other managers, clearly have a significant impact on the life and welfare of miners throughout the world.CertificationIn light of this, there is growing need to improve standards of competency and professionalism for those who work in mine safety and health management. An effective mechanism to accomplish this, other than licensure, is the use of accredited professional certification programs. There are many professional certification programs for safety and health throughout the world (Hale & Harvey, 2012). However, there is only one certification specific to mining —the Certified Mine Safety Professional (CMSP). The International Academy of Mine Safety and Health (ISMSH-SME) was formed with the purpose of developing and managing mining safety and health certification programs for the Society for Mining, Metallurgy and Exploration (SME). In 2016, following the discontinuation of the International Society of Mine Safety Professionals (ISMSP), which developed the Certified Mine Safety Professional (CMSP) credential in the 1990s, the IAMSH-SME began the process of updating and relaunching the CMSP program. Among other work required to gain third-party accreditation both domestically and internationally, the IAMSH-SME has developed a new Body of Knowledge associated with mine safety and health management (Table 1)."

"Over the years, the decline of high grade ores have become an increasing problem in the global mining industry. As such, techniques and chemistries must change in order to continue to meet the market demand and quality. Reverse cationic flotation of quartz and silicates is one of the most important techniques for generating the industry standard product from minerals such as iron and phosphate ores. Amine collectors are the global standard in such processes. However, as the ore quality continues to decline so must the chemistry change to meet the change in ore composition. This paper will focus on the introduction and full scale application of a novel amine collector towards a cleaner flotation process in phosphate beneficiation with Crago process. The results suggests the collector is capable of improving the phosphate recovery, in this cleaner circuit, to approximately 94% with minimal sensitivity to the everyday changes in feed grade.INTRODUCTION The days of mining high grade minerals are long gone, but the demand continues to increase. Phosphate ore, for example, is most dominantly demanded from the agriculture industry as a fertilizer component (i.e. diammonium phosphate, DAP) (FAO, 2014). Some literature suggests that global food production must increase by approximately 70% by 2050 (Cordell, 2009). It is also suggested that the peak global phosphorus production will occur around the year 2033, which is the period at which the easily accessible reserves have been depleted. Currently, phosphorus is commonly mined with a BPL (Bone Phosphate of Lime) concentration of 12-20% and a remainder typically being gangue minerals such as silica, clays, metal oxides, calcareous minerals, and carbonaceous minerals. These gangue components can lead to various negative processing and utilization consequences. Calcium, from carbonaceous minerals, is problematic due to the elevated levels of sulfuric acid required to convert the phosphate rock to the acid form (SRI Consulting, 2009). Calcium can also lead to elevated dosages of flotation reagents due to the desire of the fatty acid to coordinate to calcium minerals. Carbonate leads to increased levels of foam during the wet acid process. Silica is associated with the level of insols (acid insolubles), which must be less than 10% in the final concentrate that will be transferred to the chemical plant for acidification. Phosphate ore can generally be classified as carbonaceous or siliceous (El-Shall, 2003). Siliceous ores are commonly processed through various sizing steps followed by multi-staged flotation (Figure 1) (FIPR, 2014 & Güven, 2010). Though the higher grade phosphate pebble is declining, many mines are still able to size good enough to produce some amount of such pebble. Unlike most metal flotation processes where grinding is a necessity for an optimal process, Florida phosphate flotation does not generally require aggressive grinding and can typically operate at 0.1-1.0 mm. The rougher flotation step applies pH modification to achieve the optimal pH of 8.8-9.3 followed by conditioning with a fatty acid type collector, which builds a hydrophobic waxy layer on the surface of the particle to assist in the flotation. This generally achieves higher than 85% recovery of a product with 50-60% BPL and 15-30% insols. Unfortunately, DAP production generally requires 3.5 tons of phosphate rock containing greater than 63% BPL to produce 1 ton of phosphoric acid (ArgusMedia, 2014). Thus, further purification is required to achieve the grade by the chemical plant for fertilizer production. After the fatty acid is scrubbed off the phosphate particles using sulfuric acid (pH 2.5-3.5), the pH is generally operationally readjusted to 6.5-7.2 by washing with “fresh” water. A second collector is applied prior to the cleaner, which is a reverse cationic flotation process. This collector is typically a cationic amine based reagent, which should have an affinity for silica containing mineral"

In the USA, miners are at higher risk for work-related disorders that could precipitate an opioid prescription, injury-related opioid prescriptions, and opioid overdose. We aimed to support prevention of these outcomes with an “opioid hazard awareness” miner safety training module. In January and February 2020, the training module was embedded within required mining safety and health training for stone, sand, and gravel miners in Massachusetts (USA). Training impact and reception were assessed with a pre- and post-training survey of trainee characteristics, training content knowledge, and attitudes. Over 800 miners were trained and over 600 responded to surveys. We found statistically significant improvements in opioid risk factor knowledge and knowledge of resources to help a coworker. Nonstigmatizing attitudes were high both at baseline and following the training. The training was perceived as relevant and useful by trainees. Improving opioid hazard awareness in high-risk worker populations has potential to reduce opioid dependence and downstream impacts including overdose death. Employer injury-prevention efforts should be reinforced while worker health and safety training enhances awareness of opioid-related harms and prevention strategies for miners and other high-risk worker populations.

Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.
