When two pieces of heavy equipment interact, jolting and jarring can occur. During haulage truck loading for example, there is a chance that the operator of one or both pieces of the equipment will experience jolting and jarring. Additionally, a jolt can occur when an off highway equipment operator drives over a road defect or inadvertently strikes a berm. Aside from the operators, there is seldom anyone else that witnesses the interaction and can accurately describe the extent of the jolting. This makes it difficult for health and safety managers to address jolting and jarring. The devices and software described in this paper constitute a method for installing “black boxes” called Shox Boxes onboard equipment that already have a GPS system onboard. The resulting configuration provides an objective assessment of jolting and a chance to determine the root causes of it. The Shox Box system reviews data in real time, onboard the vehicles, 24 hrs a day, and sends pertinent information via radio to a central database. The data generated shows whether jolts are above or below a supervisor defined target value for the equipment and the tasks being performed. This allows health and safety personnel to demonstrate safe operation of heavy equipment and recommend proactive actions to maintain jolting within a reasonable range.

The authors hypothesize that excavation-induced extension fractures are caused by elastic shearing displacement that in turn causes tension oblique to a propagating shear zone. Such extension is manifested as tension on scales larger than that of microcracks. En echelon fracture patterns are a common result. FLAC modeling confirmed that localized tension is created near the corners of mine openings. In the model, tension was concentrated at the leading edge of a simulated fracture in a pattern that could promote the formation of an en echelon fracture. Particle Flow Code models confirm progressive formation of en echelon tensile failures with distance from an opening. The elastic displacements and the resulting fractures facilitate reduction in applied load at the expense of shear deformation and lateral dilation. Understanding the mechanics of fracture and deformation is vital for designing more effective ground support practices, a major objective of ground control research being conducted at the Spokane Research Laboratory of the National Institute for Occupational Safety and Health.

Heat contents were measured for Na2CaSi04 and Na2Ca2S02 between 298° and 1,300° K, for Na2Ca2Si309 between 298° and 1,500° K, and for Na4CaSi3O9 between 298° and 1,350° K. Copper-block drop calorimetry was employed. Reversible, first-order transitions were found for Na2CaSi04 and Na2Ca2Si309. The transition for Na2CaSi04 is at 830° K with a heat absorption of 160 cal/mole; that for Na2CagSi309 occurs at 750° K with a heat absorption of 1,550 cal/mole. Results for the other two silicates showed no irregularities. A table of heat content and entropy increments above 298.15° K and heat con-tent equations are included for use in thermodynamic calculations.

Introduction This Instructor’s Copy contains most of the materials trainers will need in order to use Drill Rig Incident (DRI). It includes suggestions for using the exercise, performance objectives for the training, master answer sheets, a scoring key, and a set of discussion notes that provides additional subject information. Four appendices are also included. Appendix A is the complete exercise problem booklet. It may be reproduced so that every person in the training class has one. Appendix B contains answer sheet blanks. The blanks are furnished if you choose to have the invisible ink answers that appear in appendix C printed on them at your location.3 Answer sheets are consumable. One is needed for each group of three to five persons who work the exercise together. An alternative is for each individual trainee to have his/her own answer sheet. An ideal fire warning alarm system for underground mines would be low cost, convenient, fast, reliable, and able to warn all underground workers. Present warning systems, such as phones, messengers, and stench, fail one or more of these criteria. The U.S. Bureau of Mines may have devised the ideal system. The one way communication system employs a large loop antenna and transmitter to create a 630 hertz (Hz) electromagnetic field to send information through-the-earth to cap lamp battery mounted micro- receivers worn by underground miners. Field tests of prototype equipment in 1986 resulted in through-the- rock signal transmission of over 762 m (2500 ft). Subsequent hardware upgrades for more recent tests resulted in transmissions of over one mile. Recently developed receiver circuitry employs phase- locked-loop detection circuitry responsive only to a specific series of pulses of given time duration, that eliminates false alarms from electromagnetic noise emitted by mine equipment, power lines, or lightning. Mine ventilation can be broadly defined as the branch of mining engineering primarily concerned with the controlled flow of air through mine airways to maintain an acceptable atmospheric environment wherever men work or travel underground. Martinson (1976) has defined three elements of mine ventilation, 1) an engineering element concerned with establishing quality control (removal of dust and gases), quantity control (regula¬tion of air direction and magnitude) and climatic conditioning (temperature-humidity control), 2) a biological element in which an understanding of biological response is used in assessing the mine environment, and 3) a philosophical element requiring judgements as to the degree of risk or worker discomfort that is socially acceptable. Most texts on ventilation, such as Hartman (1961), limit themselves to a discussion of the engineering element. Although also limiting his text to engineering control, Weeks (1928) thoughtfully addresses the other elements in the preface of his book where he states: "Life is brief and happiness elusive under the best of conditions; so beside the commercial aspect must be placed the humanitarian. Is it not reasonable that the operator should give his men a chance to spend their working hours in a comfortable environment, with the danger of sudden death or disability reduced as much as possible?" The statutory requirements of ventilation are, in most countries, established on the basis of biological response as determined by basic research or are dictated by social awareness, often touched off by major disasters, resulting in demands on regulatory bodies for more stringent requirements. Rock mass classification systems are extremely useful for site characterization and have been employed by the rock mechanics community for several decades. While empirical in nature, the classification systems provide a viable means to quantify the nature of rock mass, which is necessary for stability analyses. In U.S. coal mines, the Coal Mine Roof Rating (CMRR) is the most widely used classification system for several purposes, including sup-port selection, chain pillar design, assessing the stability of extended face cuts, etc. The Analysis of Roof Bolt Systems (ARBS) is an empirical method developed from the CMRR to guide selection of roof bolts as the primary support system in U.S. coal mines. In this paper, the experience of Peabody Energy in applying ARBS to sup-port design is discussed. In general, data from the Peabody mines show that ARBS predictions match well with field conditions. Peabody, however, does not use ARBS as the stand-alone methodology for support selection. Peabody uses a two-pronged approach in which the support requirement is initially estimated from the classification method, and then numerical modeling is used to select the proper reinforcement system. Such an integrated approach is necessary, as ARBS suggests only the “amount of steel” that may be used to support the roof and does not specify which type of roof bolt to use. A case study is used to demonstrate the usefulness of ARBS and Peabody’s integrated approach to support design. Also, the application of ARBS at several Peabody mines showed a very good correlation with support cost. The correlation indicated a direct relation between bolting cost and the ARBS value.

An aircraft auxiliary power unit (APU) was used as an exhaust source for evaluation and refinement of sampling and analytical procedures for gas turbine engines. The unit was also used to determine the emission characteristics of APU's in service and to study the effect of fuel composition and combustor design on exhaust emissions. Carbon monoxide, carbon dioxide, oxides of nitrogen, hydrocarbons, aldehydes, particulate loading, and odor were measured at engine operating modes representing the duty cycle encountered in airline operation. Analyses showed good repeatability, and adequate sensitivity was attained in these experiments. Smoke opacity measurements were not taken owing to the lack of a high-sensitivity smokemeter. Results showed that the minimum emission levels occurred with the standard combustor and Jet A fuel. Emission rates were highly dependent on test mode, but emission patterns as a function of test mode were similar for all fuels and combustors tested. A comparison of three identical design production combustors showed a variation in average emission rates of less than 5 pct.

Since 1988, there have been 17 documented incidents in the United States and Canada in which carbon monoxide (CO) is suspected to have migrated through ground strata into occupied enclosed spaces as a result of proximate trench blasting or surface mine blasting. These incidents resulted in 39 suspected or medically verified carbon monoxide poisonings and one fatality. To better understand the factors contributing to this hazard, the National Institute for Occupational Safety and Health (NIOSH) carried out studies in a 12-foot diameter sphere to identify key factors that may enhance the levels of CO associated with the detonation of several commercial trenching explosives. The gaseous detonation products from emulsions, a watergel, and ANFO blasting agents as well as gelatin dynamite, TNT, and Pentolite boosters were measured in an argon atmosphere and compared with those for the same explosives detonated in air. Test variables included explosive formulation, wrapper, aluminum addition, oxygen balance, and density. Major contributing factors to CO production, under these laboratory test conditions, are presented. The main finding is the high CO production associated with the lack of afterburning in an oxygen poor atmosphere. Fumes measurements are compared with the manufacturer’s reported IME fume class and with the Federal Relative Toxicity Standard 30 CFR Part 15 in order to gain an understanding of the relative toxicity of some explosives used in trench blasting.

Mine operators often rely on mine rescue teams to save lives during an underground emergency such as an underground fire, explosion or roof fall. It is extremely important that team members are provided with adequate exploration equipment and that training simulations are conducted in a realistic manner. A series of mine rescue training exercises was developed, conducted and evaluated by the National Institute for Occupational Safety and Health (NIOSH) in cooperation with the Pennsylvania Department of Environmental Protection, Bureau of Deep Mine Safety. The exercises were conducted at NIOSH’s Lake Lynn Laboratory during 1995 to 1998 and resulted in improved technology and training for mine rescue teams. For example, existing technologies were identified to help rescue teams during exploration. These included chemical light sticks to identify team members and mark crosscuts and mine materials, strobe lights for mapping out escape ways, and a helmet mounted thermal imaging system to see through smoke. A hands-free communication system showed potential for enhanced communications between team members and the fresh air base. A new high-visibility team lifeline that allows for flexibility of movement between team members was developed. A positive-pressure inflatable escape device (IED) was used to isolate the “hazardous” environment from fresh air and allow rescue team members to traverse through. An inflatable feed-tube partition that can rapidly block large openings such as underground passageways and simultaneously provide a feed-tube for high-expansion foam generators was also deployed for several simulations. During the simulations, mine rescue team members donned self-contained breathing apparatuses (SCBA) and traversed more than 305-m of mine passageways filled with nontoxic smoke. Visibility ranged from 0.3 to 0.9-m. In one area of the mine, a kerosene heater was used to simulate a fire and reduce the oxygen concentration. In another area, a 1.5 pct methane zone was established. The team members searched for "victims,” mapped the passageways, built temporary stoppings and ventilated the smoke-filled entries. The teams indicated that the simulations were challenging and very informative.

This report (System Safety 2.1) is the second in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics, and includes the benefits of implementing a system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard 61508 [IEC 1998a,b,c,d,e,f,g] and other recognized standards. The scope is “surface and underground safety mining systems employing embedded, networked, and nonnetworked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety. • 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meet the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full lifecycle of the system, and provides administrative support for the safety program of the full system.

"A knowledge of the carbon dioxide (002) content of the products of combustion, particularly from the boiler furnace, is very valuable. An instantaneous indication of the CO2 can be used as a useful guide to efficient burning. A permanent record over a. period of time gives a story to the management which tends to keep the operators more alert. Many instruments that indicate and record CO2 have appeared within the last fifteen years. More are about to appear. Although simple principles involving chemical, physical, or electrical qualities, are being used, there is at present a general feeling of uncertainty about such instruments in the boiler room any instruments are bought, run awhile, then lie idle indefinitely.Inquiries as to CO2 machines continually come to the Bureau of Mines from the general public, as well as from other branches of the Government. Some ten years ago a series of tests was made on certain instruments at that time on the market. The following test was made quite recently on four of the newer types, including one using an electrical principle. The four instruments were installed on one boiler each, according to the instructions of the makers, and the recommended routine attention was given them. The test was of a service nature duplicating the conditions under which the instruments ordinarily serve. The boiler that was available for this purpose was practically banked for some hours of the night. This caused the average CO2 for the entire twenty-four hours to be rather low, although it reached the higher ranges of the instruments during the day.Each day the charts were averaged and the results plotted to form the accompanying curve. The record of each instrument is shown by its particular mark, the dot, circle, cross and square being used in addition there is plotted for each day a rectangular area representing 1 % CO2 in height. The proper positions of this area was determined by means of the Crsat. Where the plotted point for an instrument falls within the rectangel its accuracy is within 1/2% CO2, which is considered satisfactory. Where it falls outside the rectangle, inaccuracy is indicated."

The petroleum sector continued to dominate Ecuador's economic activity in 1978, accounting for nearly 10% of the gross domestic product (GDP) and 42% of total export earnings. In 1978, a more flexible petroleum export price was adopted which permitted an expansion of foreign market opportunities. During 1979, Ecuador took advantage of disruptions in the world petroleum supply and renegotiated existing export contracts to accomodate the rapidly escalating world market price. Petroleum sales on the spot market were maximized, and prices reached unprecedented levels. By taking royalty payments in-kind, Ecuador increased its volume of petroleum available for term and spot sales. Although the volume remained about the same as in 1978, the total value of 1979 petroleum exports increased to about $920 million, reflecting a 76% increase over the $523 million received for 1978 crude oil exports. Concerned about its steadily decreasing petroleum reserves (recently estimated at about 1.1 billion barrels), rapidly increasing and heavily subsidized domestic consumption, and the prospect of having to import substantial amounts of petroleum by 1983, Ecuador announced plane which it anticipates will double reserves within the next 5 years. The Government plans to spend $1 billion over the 5-year period. Plans include drilling 325 exploration and development wells and establishment of a secondary recovery program for the Sachs and shushufindi fields. Some exploration wells are expected to reach a depth of 17,000 feet. It is thought that substantial reserves exist in the southeast Amazon region. Late in 1979, it was announced that some 10 million hectares will be available for exploration tenders-probably in blocks of about 200,000 hectares each-in the Amazon jungle of the Orients and in the coastal and offshore regions. The Government planned to commission a private, independent analysis of Ecuador's petroleum reserves that would also include recommendations on efficient production levels.

Coal is a combustible, carbonaceous, sedimentary rock that is formed by the accumulation, compaction, and physical and chemical alteration of vegetation [Simon and Hopkins 1981; Bates and Jackson 1987]. Coal is classified according to its type, grade, and rank. The type of coal relates to the plant materials from which the coal originated. The grade of coal refers to the purity of the coal-or the amount of inorganic material (including ash and sulfur) left after the coal is burned [Bates and Jackson 1987; Stefanko 1983]. The rank of coal indicates its degree of metamorphosis and roughly correlates to the geological age of the coal or the geological environment from which it has been mined [Bates and Jackson 1987]. Rank also indicates the percentage of carbon in dry, mineral-free coal [Whitten and Brooks 1973] and the degree to which the coalification process has progressed [Larsen 1981]. The geological process of coalification begins with organic materials (e.g., celluloses, lignins, and other plant compounds that are deoxygenated and then dehydrogenated) and ends with coal of various geological ages, from lignite to anthracite [Larsen 1981]. Table 3- 1 presents the American Society for Testing and Materials (ASTM) classification of coals by rank. According to Parkes [1982], high-rank coal includes anthracite and semianthracite coal ("hard coal," with 91% to 95% carbon); intermediate-rank coal includes low-, medium-, and high-volatile bituminous and sub-bituminous coal ("soft coal," with 76% to 90% carbon); and low-rank coal includes lignite (with 65% to 75% carbon or less). The rank of coal tends to increase from the western to the eastern United States, with anthracite occurring primarily in eastern Pennsylvania [Schlick and Fannick 1971]. Most of the coal currently mined in the United States is bituminous [Given 1984]. A classification of the coal in the United States is provided in Table 3-2.

Various aspects of judgment and decision-making are key themes in this book. The model presented here serves as a loose structure for the chapters that follow this one. The notion of a model is introduced because growing research interest in the subjective aspects of group and individual behavior has led to a debate over whether judgment is a skill that can be understood scientifically. A related point of contention is whether such an understanding could lead to the development of methods for estimating people's ability to make good decisions during an emergency. There is some literature that supports the potential usefulness of this approach. However, little agreement seems to have been reached on how to define and operationalize even those basic concepts necessary to assess the soundness of decisions from within their environmental and group contexts [Jensen and Benel 1977; Godden and Baddeley 1979; Baumann and Bourbonnais 1982; Brecke 1982; Stone et al. 1985]. A look at the real-world process is clearly needed. The need to attempt a better understanding of judgment and decision-making properties stems from those occasions in the existence of an organization when there is a lot at stake. The process of decision-making (which is part of the exercise of judgment) has been analyzed in situations such as corporate takeovers [Janis and Mann1977], military combat [Begland1979], clinical emergencies [Baumann and Bourbonnais 1982], and aviation events [Billings and Reynard1984]. The fundamental assumption of these analyses is that, while there are untold successes, there are also notable numbers of failures resulting from decisions that can be ascribed to one or more errors in judgment. From a cognitive perspective, any person engaged in decision-making (either alone or in a group) is actively involved in a process characterized by certain elements. These were mentioned in chapter 1, but are reiterated briefly at this point: (1) detection of a problem, (2) definition or diagnosis, (3) consideration of available options, (4) choice of what is perceived as the best option given recognized needs, and (5) execution of the choice based on what has transpired [Flathers et al. 1982; Baumann and Bourbonnais 1982]. At any moment in this process, there are factors at play that have a large impact on one's ability to solve complex problems in a limited time: (1) an internal state [Hedge and Lawson 1979] is the sum of a person's psychomotor skills, knowledge, attitudes, etc.; (2) uncertainty [Brecke 1982] is caused by faulty or incomplete information received from the external environment; (3) stress [Biggs 1968; Jensen and Benel 1977] is generated both by the problem at hand and any background problem that may exist; and (4) complexity, as it is used here, refers to the number of elements involved that must be attended to. These variables are depicted in figure 5.1, and their relationship to each other and to an outcome is indicated. This schema is designed to suggest interaction, because while the judgment and decision-making process may be conceptualized as discrete stages, experience tells us that this is not the way people function in real-world situations.

Rivers in industrial and raining areas frequently are contaminated by material dumped into them causing the water to carry in suspension large amounts of silt, which subsequently is deposited alone; the banks and, ultimately on the agricultural lands if the water is used for irrigation purposes. Two common examples of this practice are the dumping of breaker refuse in the anthracite region of Pennsylvania and the dumping of tailings from the metallurgical mills in various metal-mining districts. Studies in stream pollution and its resultant problems are very important, and surveys are constantly being made by Federal and State agencies, as well as by private companies, to determine the quantity of solids actually being carried by these watercoarses. These studies require accurate sampling of the silt-carrying water to determine the amount of solids within reasonable limits. A device that will obtain reasonably accurate samples of material from silt-bearing streams is described in this circular.

In 1947 the Bureau of Mines, as an accident-prevention measure, decided to advocate the adoption of roof bolting (suspension roof supports) in coal mines where it appeared to be applicable. Subsequent experience has disclosed that the applicability of this support is more general than was first suspected; moreover, comparison of the accident records of mines before and after adoption of the roof-bolting method shows results that more than justify the effort and funds expended. The industry has responded enthusiastically, because roof bolting proved to be not only an accident-prevention measure but also one to increase productive efficiency. In some instances this increase in the productivity of men and machines has been almost incredible and has led many mining men to conclude that through some hocus-pocus they can now cast aside many or all of the time-proven rule of good mining practice. If this tendency is not counteracted, it could easily result in an increase of roof-fall accidents, which may outweigh the benefits of roof bolting and discredit an innovation in roof control that shows great promise of materially reducing our mining accidents. From an economic standpoint it may represent the margin between profit and loss in many mining operations. The purpose of this manuscript is not to propose an inflexible code or set of rules but to summarize the experience to date as a guide for those who have the responsibility for the installation and inspection of roof bolts. This experience consists of approximately 250 investigations of roof failure in which roof bolting was involved either directly or indirectly. It should be pointed out that many of these failures were controlled, occurring during experimental periods, and comparatively few of the accidental failures actually resulted in personal injury, yet a significant number represented potential hazards.

"It has long been recognized that coke is a more desirable fuel than bituminous coal in water-gas apparatus of present design, but still considerable headway has been made in the use of the latter fuel as a substiaute for coke. This is particularly true in the Central States where the supply of high-grade coke has been very limited and uncertain during the past few years, and where coking coals of low sulphur content were available in quan¬tities. In fact, with, the present difference in price of coal and coke, it is possible to realize a marked saving when using a coking bituminous coal as generator fuel.Difficulties have been encountered in the use of such fuel, but they have, to a large extent, been overcome. One of the chief obstacles in the use of the more general utilization of bituminous coal for water-gas manufacture, is the fact that, with normal operation, the capacity per unit of time is reduced. This of course is a serious problem for plants operat¬ing at capacity with coke fuel. However, this difficulty can, to a large extent, be remedied by a changed method of operating, and can be entirely remedied by a combination of the latter with a change in the design (or proportions) of a water-gas set. The preceding statement, the reader will understand, refers more particularly to a carburetted water-gas set.In the manufacture of water gas from coke, present practice requires that for continuous 24-hour service the heat of combustion of all the carbon monoxide (CO) generated during the air blow be utilized in addition to the sensible heat of the gas coming from the generator. This is doubly true when making carburetted water gas from bituminous coking coal. The reasons why this is so are apparent when the following factors are considered."

A list of permissible mine equipment, tested and approved previous to January 1, 1942, was published In Bureau of Mines Information Circular 7207.3/ The present list covers additional equipment approved during the calendar year 1942. The items have been numbered to indicate the order in which each type of machine or device was approved in its particular classification. Thus, Information Circular 7207 gives four air compressors on the Bureau's active list, and the present circular gives the fifth to be approved. Items 90 and 94 under Loading; Machines land Conveyors also appear in Information Circular 7207. These numbers are repeated hero to show that approval was granted during 1942 for additional voltages on machines previously listed one voltage only. Equipment listed in these circulars is approved by the Bureau of Mines only when found upon inspection and test to comply with minimum standards of safety as outlined in schedules prepared on the basis of many years of experience in dealing with safety problems of the coal-mining industry. The use of such equipment in mines known to be gassy, as well as in those which may become so, is earnestly recommended in the interests of safety. This circular is published so that the industry may be kept informed as to the many types of machines, devices, and lamps that arc available for reducing explosion and other hazards incidental to coal mining. The present paper and Information Circular 7207 together give the complete list of all available permissible .Mine equipment.

The Track Rock corundum mine was among the numerous corundum deposits investigated during the war by the Bureau of Mines in an effort to develop domestic reserves of that critical mineral. In the investigation of this property 562 feet of trenches were excavated and a l25-foot adit was reopened for the purpose of locating veins to of corundum which had been previously reported to occur in that locality. Only very minor amounts of corundum were disclosed in both the surface workings and the adit. The project was begun in May 1943 and completed in November 1943.
