Fatality prevention and zero harm are key focus areas for resource companies. However, they remain an ongoing challenge for high hazard industries, including the mining sector. As one example the latest International Council on Mining and Metals (ICMM) Safety Performance Report shows that, while there is generally a declining fatality trend since the organisation commenced publishing data in 2012, there were still 33 fatalities across member companies in 2022 (ICMM, 2023b). Every fatal incident brings significant pain and suffering to the colleagues, friends and family of those involved, however the failures and causal factors leading to each incident are frequently very familiar to people working in the industry, particularly those tasked with managing risk and safety. In the words of Dame Judith Hackitt, Grenfell Tower Fire Investigator, ‘there are no new accidents – just different people making the same mistakes because they haven’t learned’ (Hackitt, 2022). The recurring themes in terms of the nature of fatal hazards and their associated causes across the mining sector show that, despite improvements evidenced by the favourable declining fatality trends over the long-term (in general and outside of catastrophic multiple fatality events), changes are required to achieve fatality elimination and the aspiration of zero harm. This extended abstract and the accompanying presentation apply a mining frame of reference to the idea that there are no new accidents, examining how this applies within the industry and why the lessons from past failures are not always well understood or applied to prevent the reoccurrence of material unwanted events. Many contemporary mining and resources organisations have extensive internal incident history data and a wide variety of information on fatality incidents is publicly available, including the annual ICMM Safety Performance Report. This extended abstract and the accompanying presentation explore how this existing data can be used effectively to learn from the past and proposes practical ideas regarding strategies for successful knowledge transfer to the workforce as a key component of preventing history repeating and improving safety performance across the industry. As noted by ICMM, ‘sharing lessons from failure is vital to improving health and safety, but it’s not enough’ (ICMM, 2023a).

Springvale Mine is an underground coal mine in the western coalfield of New South Wales which utilises retreat longwall mining technique. The mine encountered significant gateroad floor heave issues during retreat of LW415. Floor heave has subsequently impeded longwall advance as well as longwall bolt up and recovery to some extent during the extraction of the next five longwalls. Floor heave at Springvale Mine develops throughout the lifecycle of the mine roadways, and is most prevalent in longwall gateroads. Floor heave has been measured as early as 2 weeks after roadway development and continues to develop through the post-development phase, with a rapid increase in close proximity to the longwall face. This paper presents a case study of the evolution of understanding of floor heave behaviour at Springvale Mine based on geotechnical characterisation, an intensive convergence monitoring program (roof and floor movement) and spatial correlation of monitored locations with longwall face position. The analysis considers previously known factors contributing to floor heave including mining depth, geological and physical properties of the rock mass and mine design geometry. Investigation results indicate a strong correlation between floor heave magnitude and roof dilation magnitude, geophysically inferred faulting in underlying strata, areas of localised stress concentration and mine roadway dimensions. This paper also discusses the predictive model which was developed as a result of the study, which is now the basis of the floor heave removal program used to manage roadway convergence at the mine. Glencore operates zinc concentrators at both Mount Isa and McArthur River. Both orebodies have a history of complexity and required the development of new technologies to maximise the value from these long-life assets. The Mount Isa orebodies were plagued since operations started in 1931 with complex mineral textures making separation challenging and many areas uneconomic. Examples are given in Figure 1 with different grain sizes from the same orebody in different areas. The McArthur River deposit, located in the Northern Territory, was discovered by Mount Isa Mines geologists in 1955 (Logan, Murray and Williams 1990). The extremely fine-grained nature of the ore was quickly recognised and the difficult separation of the minerals was soon obvious. The geographical, metallurgical and economic factors resulted in the orebody not being developed until the technologies for the mineral separation had been developed. The development of the IsaMill allowed for an economic mineral separation flowsheet to be developed and ore treatment commenced in May 1995 (Nihill, Stewart and Bowen 1998). Given the ongoing global crises of climate change and nature loss, with the World Economic Forum (WEF) identifying them as the top risks to global economies in terms of both likelihood and impact in the coming ten years (WEF, 2022), there is increasing, pressing demand for companies to demonstrate they understand their climate and nature related risks and look toward contributing genuine beneficial environmental outcomes. Within the mining industry, mine closure is the obvious field in which climate and nature positive strategy can be linked with operations. To do so, there is a need to challenge long-held assumptions around closure in the mining and regulatory context; particularly, to shift the focus from closing individual mines to considering regional opportunity generation and value creation.

Batch centrifugal gravity concentrators have become standard equipment in gold grinding circuits for recovery of gravity recoverable gold (GRG). The downstream benefits of installing a gravity circuit are well understood, and in general, if there is a reasonable amount of GRG in the ore, a gravity circuit is usually installed. The benefits of gold gravity recovery have also been well documented, with a large database of successful case studies. As well, predicting production scale gravity recovery results ahead of installing gravity has become a common practice. The missing link between lab and full scale has always been the ability to predict the recovery benefit of installing gravity ahead of actually doing so. Being able to predict the recovery benefit in advance would help an operation in assessing the economic benefit and help justify the capital costs of the gravity circuit with much less uncertainty.

The routine use of gravity gradiometry for the detection of orebodies with no outcrop is now being undertaken. The commodities and orebody styles range from banded iron ore formations to volcanogenic massive sulfide (VMS) style polymetallic deposits. This recent innovation of flying gravity gradiometry was pioneered by BHP Billiton and Lockheed Martin with the Falcon technology (Lee, 2001), and now two additional systems, also using Lockheed Martin instruments, are deployed. In South Africa, magnetic gradiometry is being developed and deployed with the same objectives in mind. In this case, a system from Institute of Photonic Technology (IPHT) is being used. The detectable wavelengths from such systems have decreased to less than 200 m in the last year, and it is expected in 2011 that wavelengths to less than 100 m, with anomaly signal strengths of less than 2 Eotvos for gravity gradiometry will be viable. The key to these advances lies in the improvements of processing software and geological modelling. In particular, methods using all the tensor components are starting to be routinely deployed to filter, level and grid the observed signal. These methods do, however, require full insight and understanding of the reference coordinate systems used by the various instrument designers and survey operators. The digital terrain models need also to be observed to an accuracy of better than 20 m cell sizes, using such techniques as Lidar. Then the terrain effects can be modelled. The topographic surface represents the largest and most proximal density contrast encountered in an airborne survey. Hence terrain effects can have significant impact on airborne gravity gradiometry data. A decorrelation of the terrain effect with the observed signal requires a method of best fitting with a regional surface density. A novel technique has been developed for this purpose. This technique uses a tensor algebra to estimate a density between 1 - 4 g/cc that explains the average observed signal. Armed with this result, the anomalous buried bodies can be revealed by subtraction.

The Mesozoides of North-Eastern Asia represent a collage of exotic terranes which were accreted to the margins of the Siberian continent. In the Late Precambrian and Phanerozoic, the passive margin of the Siberian continent (the current north-east) broke up into a number of discrete fragments as a result of rifting events in the Riphean, Early and Middle Paleozoic. Rift related mineralisation includes stratiform and stratabound copper and base metal mineralisation in the structures of the Omulevka, Okhotsk and Prikolyma basement terranes, as well as along the edge of the Siberian continent in the Paleozoic sediments of the Verkhoyansk passive margin. During the Late Mesozoic the terranes were reassembled in a series of accretionary events. At the end of the Jurassic, the velocity of the Kula plate increased, and the opening of the Canadian Basin in the Arctic commenced, changing the vectors along which the Siberian and North American plates moved. Island arcs terranes were pushed back onto the Omolon super-terrane which was in turn pushed onto the Siberian Craton in a series of massive collisional events that resulted in the restructuring of the whole region. Numerous granitoids characterised by collisional and island arc chemistries with Au and accompanying Sn, and W mineralisation were emplaced. Mesothermal gold-quartz mineralisation was emplaced in a series of narrow belts that adjoin major crustal sutures and that spatially overlap belts of granitoids and zones of subgreenschist metamorphism. Each of these gold-quartz vein belts corresponds with a specific collisional phase. In the middle Cretaceous, the Okhotsk-Chukotka volcanic belt developed along the paleo-Pacific margin of the Siberian continent. Epithermal Au-Ag developed in the volcanics and shallow mesothermal Au-Ag-quartz and Au-Sb mineralisation developed in the Mesozoides. Porphyry and skarn style mineralisation developed in both the island arcs and in the continental Okhotsk-Chukotka Volcanic Belt.

Oaky North is AustraliaÆs highest producing underground coal mine. In 2011, the mine produced 8.7 Mt of high-quality coking coal. The high quantities are a result of the constant strive for optimising the operating time of major production units, which involves investing numerous resources, including diesel equipment, in supporting and maintaining their operation.In July 2011 it was identified that the diesel particulate matter (DPM) levels in the underground workings were above acceptable levels, due to the high level of diesel activity. Over the next 12 months, several controls were implemented to reduce exposure to personnel: the main ventilation system was upgraded; the production forecast was adjusted to better manage the diesel fleet, removing surges in diesel usage; and an electronic diesel tag board was introduced to centrally manage the distribution of diesel usage around the mine.From the start of the campaign in emission reduction, an intense DPM monitoring regime was established. Within a few months of the introduction of the centralised tag board, there was a major reduction in emission levels. After the installation and commissioning of the new main fans, the emission levels dropped further, such that over-exposures are now a rarity.Oaky North continues to explore new systems to better utilise their diesel fleet while still maintaining control over DPM emissions.CITATION:Melloy, A, Holt, D and Jurak, M, 2013. The Mt Tom Price deposit is a world-class high-grade hematite deposit in the Hamersley Province of Western Australia with an original resource of 900 Mt of almost pure hematite, averaging 63.9 wt per cent Fe. Petrological and geochemical studies at both the Southern Ridge and the North deposit at Mt Tom Price have identified three hypogene alteration zones between unmineralised BIF and high-grade iron ore: distal magnetite-siderite-stilpnomelane; intermediate hematite-magnetite-ankerite-talc-chlorite; and proximal martite-microplaty hematite-magnetite-apatite alteration zones.Fluid inclusions trapped in siderite within the distal magnetite-siderite- stilpnomelane alteration zone at the North deposit revealed primary high salinity (25.5 eq wt per cent NaCl-CaCl2) inclusions that homogenised between 107¦ and 142¦C into liquid. Fluid inclusions trapped in ankerite within ankerite-microplaty hematite veins in the intermediate hematite-ankerite-magnetite-talc-chlorite alteration zone at the North deposit revealed mostly H2O-CaCl2 pseudosecondary and secondary inclusions with salinities of 22.4 - 25.4 eq wt per cent and 22.9 - 25.9 eq wt per cent CaCl2, respectively. Pseudosecondary inclusions homogenised between 153¦ and 449¦C and secondary inclusions homogenised between 103¦ and 157¦C. Fluid inclusions trapped in apatite within talc-microplaty hematite veins in the intermediate hematite-magnetite-talc-chlorite-apatite alteration zone at Southern Ridge revealed that primary, medium salinity (7.0 - 9.0 eq wt per cent NaCl) inclusions homogenised between 181¦ and 257¦C and primary high salinity (22.8 - 25.9 eq wt per cent CaCl2) fluid inclusions homogenised between 118¦ and 257¦C into liquid. Microthermometric analysis of quartz from quartz-hematite veins from the Southern Batter Fault, Southern Ridge show a complex fluid inclusion history. Primary fluid inclusions consist of: low- and high-salinity H2O-NaCI inclusions trapped at temperatures of approximately 140¦ to 230¦C; vapour-rich inclusions of unknown compositions; and æcomplexÆ salt-rich (Ca, Mg, K and Na inclusions).Secondary inclusions consist of medium-salinity fluid inclusions trapped at temperatures of about 140¦ to 280¦C.A two-stage hydrothermal model is proposed for the formation of both the Southern Ridge and North deposits. Early 1a hypogene alteration involved the release of hydrothermal NaCl-CaCl2-rich (25.5 eq wt per cent) basinal brines (110¦ - 150¦C) from the underlying Wittenoom Formation and directed upward along normal faults and focused within the silica-rich rocks of the Dales Gorge Member, by the shales of the underlying Mt McRae Shale Member and overlying Whaleback Formation. Within the Dales Gorge Member hydrothermal basinal brines migrated laterally within large-scale folds with permeability controlled by shale bands and the NW trending dolerite dyke sets. Fluid rock reactions transformed unmineralised BIF to magnetite-siderite-iron silicate BIF, with subsequent desilification of the chert bands. Stage 1b hypogene involved an increase in temperature of the hydrothermal, CaCl2-rich saline (24 eq wt per cent) basinal brines (250¦ - 300¦C) resulting in formation of hematite-ankerite-magnetite-talc-chlorite alteration and the crystallisation of microplaty hematite. Late stage 1c hypogene alteration involved the interaction of low-temperature (~120¦C) basinal brines with the hematite-ankerite-magnetite mineral assemblage. At the Southern Ridge deposit this process was very intense (ie high fluid flux), therefore resulting in the almost total removal of ankerite and resulting in the increased porosity of the ore. Stage 2 supergene enrichment in the Tertiary resulted in the removal of residual ankerite and apatite and the weathering of the shale bands to clay.

A study was conducted to evaluate the effectiveness of the three types of sample grinders routinely used in the mining industry for the sample preparation of gold ores with particular emphasis placed on testing their capacity to produce a representative assay sample of coarse gold ores. The grinders used in this study are the Labtechnics LM5, the Mixermill 4010 ringmill and the Keegor vertical spindle pulveriser, commonly called a Keegormill. The study was based on the preparation of samples from two sources of known coarse gold ore, the Union Reefs Gold Mine and the Brocks Creek Gold Mine in the Pine Creek region of the Northern Territory of Australia, and a fine gold ore which was from the Macraes Gold Mine in New Zealand. The Union Reefs Mine contains visible coarse gold with 30 per cent to 40 per cent of the gold in the plus 75 μm fraction of milled ore. Discreet particles up to 5 mm have been recorded, Hellsten (1994). Union Reefs Gold Mine recovers 40 per cent of its bullion production through a gravity circuit, Riches (1997). Brocks Creek ore contains visible coarse gold with gravity bullion production fluctuating between 50 per cent and 65 per cent, depending on the mill feed. Average gravity bullion production from the Brocks Creek Mine is 55 per cent, Wilke (1997). The average gold grain size of Macraes Mine ore is 20 μm with 40 per cent of the gold in sub 10 μm particles. No gravity bullion is produced from the Macraes Mine, O'Keefe (1997). The testwork demonstrated conclusively that it is possible to prepare a representative sample for assay, of a coarse gold ore, when a Keegormill is used to pulverise the sample to a nominal 100 μm particle size (90 per cent passing a 100 μm screen by wet sieving). Both the Mixermill and the Labtechnics LM5 failed to prepare a representative sample, even when ground to a nominal 75 μm particle size.

The mining industry, and in fact the world, has undergone significant changes in the past five to ten years, ranging from the metals boom, the global financial crisis and the rise and fall of national economies, to name just a few. There has been a focus on sustainable development for projects and operations in the mining industry, mainly pushed onto the industry by communities, governments and other agencies. While sustainable development and operation is a worthy priority for mining operations, discussions with mining industry personnel (with a strong slant on the mineral processing perspective) have shown other issues have surpassed the typical issues of water resources, energy availability and the environment. These include staff turnover and skills shortages, extracting metal from more complex orebodies and the uptake of new step-change technology.A number of key operating mineral processing personnel were interviewed to determine their perspective on the current issues facing the mining industry. Local accumulation of methane in workings of a longwall is one of the most common causes of mining accidents. This gas has many sources: it may leak from goaf through permeable sealings, or through gaps between roof support sections. This stream is then mixed with ventilation air. In the case of auxiliary ventilation ducts, or free jet fans, we have to consider the propagation of a clean air stream in areas with an air-methane mixture. Therefore, modelling the mixing process of methane with ventilation air is important for determining the hazardous areas and thus to ensure safety. Computational fluid dynamics (CFD), specifically the finite volume method, provides tools for such modelling. However, this method requires validation. For this purpose, we have compared measurements with simulations for selected test cases having sufficient similarity to the present problems. In typical methane explosion hazard areas, such as a longwall, it is difficult to make measurements sufficiently to confirm credibility of 3D CFD simulations. As a relevant alternative, an injector methane drainage station placed in an underground gallery has been selected. The stream of drained goaf methane is entrained by compressed air powered injectors and then mixed with ventilation air. Research in this area does not interfere with mining operations, it can be performed safely in a repeatable way. Flow within the site has many similarities to conditions in mine drifts of the longwall area. Comparative research of measuring the methane concentration distribution in the mine drift downstream injector station, with simulations of propagation of the methane stream, was carried out. For measuring the methane concentration distribution in the mine drift a multipoint measurement velocity system developed at the Strata Mechanics Research Institute was used. For the simulation a finite volume method for turbulent flow of gas mixtures was applied. Preliminary results of comparison are shown as graphs and tables.CITATION:Janus, J and Krawczyk, J, 2013. At Newmont’s Jundee Operation, exploration and resource geologists collaborated to produce an open pit resource model. Using a variety of modelling and estimation techniques but no new data, an uneconomic underground target was converted into a potentially economic open pit evaluation. During an industry downturn, it is more important than ever to efficiently share expertise, ideas and resources to problem solve and produce meaningful project outcomes. The two departments brought together different methods and ideas, and the process showed that certain methods worked better than others in different situations, thus the appropriate method was applied to each part of the evaluation. The results were used to create new procedures and improved lithology modelling and resource model estimations. From ancient times to the modern world, the mining industry has played a pivotal role in shaping the society that we live in today. It remains an integral part of everyday life and is a catalyst for both technological and economic growth. With finite resources on Earth, and growing pressures surrounding climate change and the environment, space mining opportunities have become of increasing interest. Recent discoveries have shown celestial bodies including the Moon and surrounding asteroids, contain an abundance of critical minerals, as well as resources necessary to sustain human life. These discoveries coupled with a transition into the digital and autonomous age, offer an innovative solution to solving some of the mining challenges currently being faced on Earth. Similarly, they also offer a pathway to enabling deep space exploration and off Earth exploratory missions. However, like with mining operations on Earth, space remains and immensely challenging and complex operational environment. The inhospitable nature of space poses new challenges, including communication barriers, lunar dust, cosmic radiation, and gravity. The challenges and risks are not just technological, as Environmental, Social, and Governance (ESG) factors must also be considered to prevent unintended consequences and hazards. Furthermore, future workforce, law, financial, and security systems must evolve to enable space mining to take place. These are all known aspects to begin with, but as the complexity of the space grows it becomes apparent that a holistic approach to assess and manage these factors will be required.

Sampling equipment manufacturers endeavour to ensure that the mechanical equipment installed in sampling systems are designed and manufactured to adhere to the requirements of the International Organization for Standardization (ISO) or other standards relevant to the application. The mechanical parameters necessary to achieve these standards can be well controlled and requirements such as constant speed can be met through good mechanical design and engineering. However, when the equipment is installed and put into operation, the ISO compliance of the system is out of the hands of the equipment manufacturer and handed to the operator, who may be a generalist control system integrator.Making a sampling system perform according to standards, and being able to monitor and report on compliance require that the sampling system knowledge and the control integration is combined and applied in an integrated approach. Using a dedicated library of control routines developed for sampling can change a system from having compliant equipment to being a compliant system. The optimum mining of an orebody in an underground mining operation is fundamental to the mine’s ability to operate both sustainably and efficiently. The effective mining of an open stope or a cave is largely reliant on the recovery of the initial void created by conventional drilling and blasting. These initial voids are known by various names such as a Longhole Rise, Slot or Winze and can be mined either as a downhole or as an uphole. Regardless of the underground mining method, the process of the design and implementation of this initial void is largely uniform across many operations with various patterns of slot or winze designs used across these different operations. Diligent design, implementation and review strategies in drilling and blasting of these will facilitate cost-effective mining with minimal disruption to mine schedules and production targets. When an operation struggles in opening the initial void in underground mine blasting, it has a negative impact on productivity, mineral recovery, and the overall economics of mining. Near field blast vibration monitoring is a vital tool to use in analysing slot blasts and will provide additional feedback into the reconciliation part of the drill and blast process. It is one that is often overlooked in the mining industry in many operations due to many factors such as lack of skilled personnel, or a lack of required resources. The data obtained from near field blast vibration monitoring can be used to provide additional feedback into blast reviews and help in any drill and blast optimisation projects. This paper highlights key aspects of underground drill and blast reviews and the use of near field blast vibration monitoring for post blast analysis and shares learnings from various projects that the authors have been involved in within Australia and abroad.

Grade control is a process that requires the interaction between many components of the mining operation and successful grade control is only realised through good data, robust systems, and a well-trained, motivated and disciplined workforce.The Mt Magnet Gold Project Pty Ltd (MMGP), a wholly owned subsidiary of Ramelius Resources Limited, started mining at Mt Magnet in February 2012 with high expectations for success. Positive cash flow would fund an aggressive exploration and mine development program in the area to increase mine life. Disappointment followed when after start-up the operation had issues achieving budget grade and realising the predicted grade of the mined ore. In January 2013, things started to improve. During the next six months, gold production steadily increased with successive months achieving record production. What had changed?Improvements started following the realisation that controlling grade was not the sole responsibility of the mining geologist and that everyone on-site has a role to play. Over the 40 years that the author has been engaged in the Australian underground coal mining industry, there has been a continuous improvement in safety performance as the sector has grown in size and productivity. The underground environment hosts many hazards with significant risk profiles that are kept in check by effective health and safety management systems that have evolved largely following major underground disasters. Observations are made on the changes observed for specific aspects of the underground coal sector, with particular reference to ventilation issues, and predications are made as to the needs of the future.CITATION:Robertson, B, 2013. The zinc-lead-silver mineral resource at Lady Loretta ranks amongst the worldÆs richest when measured in terms of in situ metal grades. However, the project remains undeveloped 28 years after its discovery in 1969. The primary obstacle to development has been the fine and complex grain structure of its ores, which make flotation concentration inefficient even with fine grinding. The size and remote location of the deposit have also hindered exploitation. BUKA planned to directly fume the volatile target metals from the ore when it acquired the project. Intensive, post-acquisition modeling showed the innovative processing route to be uneconomic, notwithstanding high metals recovery and low waste content in fume product. Progressive modifications and supplements to the underlying fuming process enhanced economics and resulted in a new mine-to-metal processing route for zinc-lead-silver ores. The proprietary, three-stage BUKA Zinc Process offers technical, economic and environmental advantages. It is expected to find widespread application in the treatment of ores and residues. However, neither continued R&D nor application to Lady LorettaÆs currently delineated resource can be justified under prevailing market conditions. The objectives and results of each step in the process development sequence are outlined with reference to their impact on the commercial viability of the Lady Loretta project.

The Yinachang deposit in south-west China contains large amounts of Fe, Cu and rare earth elements (REEs). Apatite in this deposit records complex REE mineralisation and remobilisation processes.Fe-Cu-(REE) mineralisation at Yinachang occurred at ~1700 Ma, and three stages of alteration and mineralisation are identified: Na-(Fe) alteration, Fe-(REE) mineralisation and Cu-(REE) mineralisation. In the Fe-(REE) mineralisation stage, REE-rich apatite formed together with magnetite. In the Cu-(REE) mineralisation stage, fluids undersaturated with REEs interacted with early-formed REE-rich apatite, during which large proportions of REEs, especially light REEs (LREEs), were leached from apatite.At ~840 Ma, apatite experienced a second interaction with fluid. In that process, REEs were removed from the apatite structure but immediately reprecipitated into new phases within the apatite. Thus, numerous REE mineral inclusions including bastnaesite, monazite and minor Y-phosphate occur in the apatite. There are also a very small portion of LREEs that were transported out of the apatite, resulting in the growth of bastnaesite and monazite grains outside the apatite.This study demonstrates that REEs can be mobilised in multistage hydrothermal activities and highlights the significance of fluid compositions for REE transportation and deposition behaviours in hydrothermal systems.CITATION:Li, X and Zhou, M-F, 2015.
