Domestic Production and Use: In 1976, 12 companies operated 31 primary aluminum reduction plants, with 3 firms accounting for 63% of production capacity. Washington, Oregon, and Montana accounted for 36% of the total production; Gulf Coast States 19%, Northern States 29%, and other Southern States 16%. Based on published producer prices output of primary metal in 1976 was valued at $3.8 billion. Aluminum consumption by 25,000 firms was centered in the East Central United States. Of aluminum consuming industries in 1976 building accounted for about 25%; packaging 22%; transportation 19%; electrical 13%; , consumer durables 8%; and other uses, 13%.

The annulus thickness, for bolts anchored with resin, effects the systems final performance. Recent developments in the manufacturing process of these support systems, coupled with improved resin characteristics, have resulted in design changes to minimize hole drilling requirements and reduce resin usage. The amount of resin to properly anchor a 7/8-SRD (Special Rolled Dimensions) bolt in a 25.4 mm (1-in) diameter hole has been reduced to about one-third of the resin required to anchor a No. 7 rebar bolt (22 mm (7/8-in) diameter) traditionally installed in a 35 mm (1-3/8-in) diameter borehole. More importantly, laboratory and field tests, indicate that higher bond strengths and anchorage capacities can be realized with reduced annulus systems. These factors have a direct bearing on reinforcement efficiency and subsequent roof support performance.

An underground wet shotcrete trial was completed last year to determine the effectiveness of wet shotcrete as an alternative support mechanism to dry shotcrete in the kimberlite environment. The wet shotcrete was manufactured at the underground batch plants, transported to site using an agiecar and transferred to the shotcrete pump. The wet shotcrete is pumped to the nozzle where compressed air at 7 bar and an accelerator, Meyco SA 160, is added and enables the shotcrete to be placed successfully onto the tunnel wall. No tunnel guard was previously placed on the sidewall and hangingwall. Geotechnical testing indicates that the Grace fibre performed extremely well with the energy absorption tests at 7 kg/m3and 8kg/m3where both achieved over the mine requirement of 750joules. The Grace fibre and wet shotcrete mix also achieved a compressive strength in excess of 55 MPa, well above the 45 MPa requirement for Premier Mine. In South Africa there are two suitable wet shotcrete machines available for the mining industry, the Spreymec, manufactured by Tamrock and the Fermel/Meyco wet shotcrete machine. The Spreymec is a fully imported machine. The Fermel component is manufactured in South Africa and the Meyco shotcrete components are imported and supported by MBT (Master Builder Technologists) in South Africa. The Fermel/Meyco machine is assembled in South Africa. Premier Mine has allocated some R2, 100,000 towards the implementation of the wet shotcrete system. If implementation is successful there should be an internal rate of return gain of 76% with regard to this project.

In this paper the link between process metallurgy, classical minerals processing, product centric recycling and urban/landfill mining is discussed. The depth that has to be achieved in urban mining and recycling must glean from the wealth of theoretical knowledge and insight that have been developed in the past in minerals and metallurgical processing. This background learns that recycling demands a product centric approach, which considers simultaneously the multi-material interactions in man-made complex ‘minerals’. Fast innovation in recycling and urban mining can be achieved by further evolving from this well developed basis, evolving the techniques and tools that have been developed over the years. This basis has already been used for many years to design, operate and control industrial plants for metal production. This has been the basis for Design for Recycling rules for End-of-Life products. Using, among others, the UNEP Metal Recycling report as a basis (authors are respectively Lead and Main authors of report), it is demonstrated that a common theoretical basis as developed in metallurgy and minerals processing can help much to level the playing field between primary processing, secondary processing, recycling, and urban/landfill mining and product design hence enhancing resource efficiency. The corrosion of aluminosilicate refractories by soda from three different sources has been studied by microscopic methods. Sodium sulphate from oil ash, sodium chloride used in glazing structural clay products, and sodium borate from borax-impregnated wood ash have fundamentally the same effect on the refractories. The soda is the chief medium of reaction and fluxing, the sulphur, chlorine, and boron being volatized as sulphuric, hydrochloric, and boric acids.INTRODUCTIONBecause of their strong fluxing action, alkalis are one of the main causes of corrosion and slagging of refractories.The action of potash and soda on refractories follows somewhat similar lines, although it is common for one alkali to be greatly in excess of the other. The course of the reaction in a number of cases of alkali attack has been studied by microscopic and chemical methods. For example, the action of potash on blast furnace refractories was studied by Kraner (1942) and Rigby and Richardson (1947), who described the occurrence of various potassium aluminium silicates. Its action on cement kiln refractories was described by Brisbane, Segnit, and Weymouth (1957). The action of soda on glass tank refractories has been studied by many people, for example Bisby (1956) and Resch (1954). "Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

This report summarizes all work performed under Phases I, II and III of U.S. Bureau of Mines Contract H0387006, "Development of a Prototype Retrofit Noise Control Treatment for Jumbo Drills." The noise and vibration levels of four jumbo drills were characterized and a survey of percussive rock drill usage is reported. A Gardner-Denver DH 123 drill was chosen as a demonstration drill, and extensive noise diagnostic studies were performed. Noise control treatments were designed, developed and evaluated in a reverberant drill test facility. The treatments consisted of an integral enclosure/muffler for the drifter, vibration isolation of the drifter from the enclosure and feed, and a drill steel sheath. A 19 dBA reduction in overall sound power level was achieved with no loss in drilling rate. The prototype retrofit treatments were field tested in an aboveground mine and in a production section of an underground zinc mine. Long-term durability and operator acceptance of the enclosure/muffler treatment were excellent. Preliminary evaluation of the drill steel sheath was encouraging, although additional testing is required.

Best practice gold (Au) recovery is not an accident. It is the result of proper operation of the adsorption circuit using good quality activated carbon. A conventional carbon ‘regeneration’ kiln cannot process carbon fouled with floatation chemicals and achieve best practice gold recovery. A high temperature, controlled atmosphere carbon ‘reactivation’ kiln is required. Carbon circuit design allows for low quality carbon by increasing the carbon advance rate, carbon inventory and setting an easily achieved and commercially prudent solution tails target of typically 0.02 ppm Au. These design criteria are important to ensure the commercial success of the gold operation in the first instance. Most carbon ‘regeneration’ kilns will produce carbon of sufficient quality to achieve a 0.02 ppm Au solution tail in the absence of flotation reagents.Best practice recovery is considered less than 0.005 ppm Au in solution reporting to tails. If the carbon is heavily fouled, as it will be if used for flotation leach circuits, then a conventional carbon ‘regeneration’ kiln cannot achieve world’s best practice recovery no matter how much carbon is advanced. World’s best practice gold recovery requires a carbon ‘reactivation’ kiln and the whole of the carbon circuit operated optimally.The competing reactions occurring in a carbon ‘reactivation’ kiln are presented and discussed in terms of kiln design and operation. New industrial and laboratory findings are presented to fill gaps in the body of knowledge and identify that a large reduction in residence time at temperature will not significantly impair reactivation, but can significantly reduce the size and cost of the kiln or permit significantly increased carbon throughput.The carbon circuit performance is modelled in terms of gold adsorption, as a function of carbon reactivation using ‘simulation of carbon-in-leach/carbon-in-pulp circuits’ (SIMCIL). An optimum carbon advance rate is identified as a function of optimum carbon ‘reactivation’ kiln operation to achieve 0.005 ppm Au in the tails solution. The steps for the beneficiation of iron ores differ from ore to ore, depending on the mineralogical phases of the particular iron ore and the nature of the association of the gangue minerals with iron oxide in that ore. The extent to which the beneficiation can be undertaken economically depends both on the amount and the nature of the gangue. The analysis of the phases present in an orebody optimises the mining and process operations (separation, beneficiation, sintering, etc).X-ray diffraction (XRD) provides qualitative and quantitative information about the main ore phases as well as the gangue mineralogy and the amorphous content present in a sample. In addition, modern quantification methods such as Rietveld refinement allow the calculation of the distribution of elements in solid solutions such the substitution of Fe with Al in goethite or hematite.The use of high speed detectors has made XRD an important tool for grade and process control. Finger printing of XRD data can facilitate multi-dimensional mapping of ore deposits and drill cores, identifying regions of favourable mineral compositions. Grade control using XRD can be completely automated and controlled remotely.The two case studies in this paper demonstrate how XRD and finger printing can be used for grade control, process optimisation and quality control of a goethitic and a magnetitic iron ore. "Historically, tailings management in a northern climate has involved the deposition of tailings into the Tailings Management Area (TMA) as conventional slurry. This type of system is obviously viable but presents challenges that are difficult to manage as circumstances change over the life of mine. These issues can include:? Increases in mill throughput, and? Life of the TMA for the life of mine.This case study will present how thickened, high-density, non-segregating tailings are used as an alternative to conventional slurry disposal to achieve greater storage capacity and enhanced environmental performance for a gold tailings facility in a northern climate. This paper will focus on how this change in deposition strategy accommodated the increased throughput of the mill and the increased life of mine. Specifically, the paper will discuss the addition and integration of new processes to the exiting mill system to accommodate the tailings management issues. It will highlight the challenges from laboratory testing to pre-feasibility to the engineered design of the thickened tailings system implemented at Musselwhite Mine.BACKGROUNDGoldcorp Canada’s Musselwhite Mine, in Northwestern Ontario (120 km north of Pickle Lake), is an underground gold mining operation that began production in 1997. The current mill throughput is about 4000 tpd (1.4 Mtpa). Gold extraction is based on a CIP process with cyanide destruction.The existing tailings management area (TMA) is located about 2 km southwest of the mill complex. The TMA is contained by high ground to the north and to the west and flanged by a moraine ridge along the south side. A number of dams have been constructed along this ridge. The dams were first built in 1996 and subsequently raised in 2000 and again in 2005. Excess tailings pond water is discharged to a polishing pond then released to the environment via a wetland.The TMA was originally designed to contain 13.7 Mt of tailings under a ponded surface for closure. The water cover was to inhibit adverse long-term environmental effects because the tailings contain pyrrhotite and other sulphide-bearing minerals that are potentially acid generating. The design was modified during Stage 3 Construction in 2005 to expand the storage capacity to 17.3 Mt (Golder, 2005). As of end of 2009, over 16 Mt of tailings had been deposited in the TMA. The slurried tailings were historically deposited at 50% solids."

"The New Orleans area has been repeatedly flooded by major storms and hurricanes since its French colonial era. After severe flooding in May 1995, the U.S. Congress authorized a massive flood protection project called the Southeast Louisiana Urban Flood Control Program (SELA) in 1996 to improve interior drainage in Orleans, Jefferson, and St. Tammany parishes. However, the project was not fully funded before the landing of Hurricane Katrina in 2005 and Hurricane Rita, which breached many of the area’s levees and flooded 80 percent of the city. Finally, the project was fully funded in 2008 and has been worked on towards its 2017 completion. Another large project simultaneously worked on in the area is the Hurricane and Storm Damage Risk Reduction System (HSDRRS). This system was designed for a 100-year level of flood risk reduction. One particular project within the HSDRRS rendered successful due to an unconventional method of pile driving that was utilized to install sheet piles to reinforce an existing concrete I-wall. This paper will discuss how and why this non-vibratory method of sheet pile installation allowed the project to be successfully completed ahead of schedule.INTRODUCTIONSteel sheet piles were utilized on many of the SELA and HSDRRS projects for permanent cutoff walls under concrete I-walls or T-walls or for temporary earth retaining walls of drainage structures. Although the SELA Project was established in 1996 after severe flooding the year before, hardly any work was done on it until the two catastrophic storms Hurricanes known as Katrina and Rita triggered its revival. These two hurricanes also triggered the HSDRRS soon after the storms’ devastation. The SELA project authorized to improve interior drainage systems while the HSDRRS project was authorized to raise or restore levees and floodwalls mainly along Lake Pontchartrain, the Mississippi River, and Lake Borgne in addition to other areas. Figure 1 shows the SELA Projects that have been completed, are still under construction, or approved for construction. The projects fully highlighted in yellow within the map specified the press-in piling method. These improvements included drainage systems, pump stations, surge walls, and outfall canals as enhanced protection for future storms similar to Hurricane Katrina that caused so much damage by inundating the New Orleans area in August and September 2005 respectively (Facts and Figures, 2012). The completion of these projects were the responsibility of the U.S. Army Corps of Engineers, New Orleans District.Due to the fact that many of the projects were in densely populated areas, the Army Corps of Engineers specified a non-impact, non-vibratory press-in piling method for driving and extracting sheet piles for many of the projects to mitigate noise and vibration impacts. The soft soil conditions in the New Orleans area was also a contributing factor in the Army Corps of Engineers’ decision to specify this piling method due to the risk of settlement for existing historical as well as modern structures. In addition, there were other projects that elected to use the press-in method without any contractual requirements. One of these projects was the Inner Harbor Navigational Canal Reach II Emergency Interim Repair which was part of the HSDRRS project that has been under construction since its authorization in 2005 (Facts and Figures, 2012)."

The ordinary Portland cement (OPC) is the key pozzalonic component of the concrete mix, widely used as the construction material world-wide. Billions of tons of OPC are used in the construction industry each year. However, calcinations of limestone and silica that produce OPC also produce almost an equal amount of CO2 during the cement manufacturing process. Thus, OPC production accounts for nearly 7% of all anthropogenic CO2 emission each year. The main objective of this study was to develop a geopolymer-based composite to be used as a substitute for OPC in concrete application to significantly lower the carbon foot print of the construction industry. A waste material produced during the coal combustion process was used as the main raw material for the geopolymer-based composite. It was mostly the non-metallic content of the F-type fly ash produced from the combustion of bituminous coal. The authors envisioned to extract some of the valuable metal oxides from fly ash prior to using the major non-metallic portion of the fly ash for geopolymer concrete application. The results obtained from this study exhibited that the compressive strength of the geopolymer concrete compares favorably with that of the conventional concrete prepared using the OPC.

"INTRODUCTION The mines in the eastern part of the now active region of the Tri-State zinc district of Missouri, Kansas, and Oklahoma have unusually acid water, which during periods of moderate or no rainfall is used in the mills to replace losses from evaporation, seepage, and tailings disposal. The East Bay Municipal Utility District is undertaking seismic upgrades of the Claremont Tunnel in the San Francisco Bay area. The tunnel carries treated water across the active Hayward Fault to supply a major metropolitan area. Studies indicate the tunnel could be severed by fault offset during a major seismic event. A design approach to achieve limited operation of the tunnel following fault displacement has been established and design details are currently being evaluated. This paper describes the project, the design approach, and summarizes significant construction constraints.

"Abstract: The paper describes a large innovative project completed in 2012-20123 by Trevi Group, to develop equipment capable to install ultra-deep diaphragm wall. The project highlights the capacity of SOILMEC to design and build a new hydromill able to reach an unprecedented depth of 250 m, including new devices for monitoring and correct the verticality. The entire project can take advantage of the long and solid experience matured by Trevi and Rodio (now part of Trevi Group), developing in house or, else, select the elements necessary for a high quality product in the ground.The paper describes the results of different systems used to monitor and control the verticality and the twist of panel during excavation, together with the method used to pour concrete. Finally, the results of verification coring and lab tests on concrete samples recovered at various depths are discussed. The test field has been supervised by representatives of three of the major Italian Universities, and lasts almost one year from August 2012 to June 2013.The verification holes made use of an innovative combination of directional drilling and coring, to check the quality of the plastic concrete for the full length of the panel. The data confirmed that the test panel has high quality concrete, meeting or exceeding to the prefixed requirements.With this test, super-deep cut-off walls reaching depths of 250 m have become a reality. This would change the perspectives of dam engineering both for existing dams rehabilitation and for new dams design and construction. With the execution of this test panel, the technology of milled concrete cutoffs has made a ”quantum leap”, pushing the limit of practicable dams."

The effectiveness of an industry?s safety activities can be gaged best by the accident records of that industry. In an effort to gage conditions in the petroleum industry, and with the objective of helping to reduce the number of accidents in it, the Bureau of Mines from time to time has tabulated, analyzed, and published pertinent statistical information3/. These reports have dealt with distribution, causes, and costs and related phases of accidents for various and collective divisions of the petroleum industry in several sections of the country. This type of study aids the personnel of the Bureau in planning the course of its activities in accident-prevention work and gives useful data to persons engaged or interested in the promotion of safety in the petroleum industry, which will aid them in their efforts to reduce the number and severity of accidents. A majority of petroleum operators and ?service" contractors have acknowledged their responsibility to provide their employees with safe working conditions and to their stockholders for greater economic value of invested capital by establishing safety divisions within their respective organizations. Such divisions have a primary duty, namely, to see that properly designed and guarded machinery and working equipment are furnished the employees and that they are trained in the safest practical methods of performing their work. Open fire in coal mines is one of the most serious threats to miners, as well as to the mine. Open fire can often be effectively dealt with by prompt local action, otherwise it very quickly becomes uncontrollable. In one incident, none of the available open fire control technologies, viz., water deluge and sprinkler systems, high-expansion foam, high-pressure high-stability nitrogen foam, water misting, and ventilation and pressure control techniques, were effective for saving the mine without sealing from surface, since the fuel-rich environment prohibited underground access due to the methane explosion hazard. The authors have developed a methodology for dealing with advancedstage open fires underground by the application of a modified ventilation control technique. It is based primarily on a better understanding of the behaviour of open fires, proper diagnosis of the problem, application of judicious ventilation control techniques, and selection of suitable fire indices for assessing the status of an open fire. This methodology was used to successfully control an open fire in Surakachhar 3 and 4 incline mine Surakachhar, central India. The fire area was sealed underground and production subsequently resumed in record time. The paper discusses the behaviour of open fires, particulars of the mine, diagnosis of the problem, experimentation methods, and the results obtained.
