The purposes of the research were to assess the applicability of new accident investigation methods to mining accidents, and to adapt those found to be applicable in the development of a new methodology for use in mining. The development included field testing, evaluation and modification of the new methodology. The report is in two volumes. This, volume one, discusses primarily the Phase I work: background research, design of a methodology for field evaluation, and preliminary field tests in Utah mines. Phase I was completed in June, 1980. Also discussed are some activities to prepare for a field evaluation during the period May 1980 -October 1981. Volume 2 discusses the subsequent field tests, modifications of the initial methodology and the results of use of the methodology in two mining companies for a period of approximately eighteen months, and in other companies for selected loss incidents during that period.

Ten proposed processes for the recovery of manganese from domestic resources as a high-grade oxide product are described. Nine of the processes employ sulfur dioxide, sulfuric acid, or a combination of both for leaching. The tenth process utilizes sulfur dioxide to sulfatize the ore at 600° to 850°C before water leaching. In addiction to these ten complete processes, six processes from the patent literature are described. Methods for receovering MnSO4?H2O are also reviewed.

ORES PROCESS OF ORE CONCENTRATION-PATENTABILITY. The patent issued to Sulman, Pickard, and Ballot, November 6, 1905, and assigned to Minerals Separation and Minerals Separation American Syndicate, for new and useful improvements in ore concen- tration, its object being to separate metalliferous matter from gangue by means of oils and fatty acids that have a preferential affinity for such metalliferous matter, the principal feature of which is "agitat- ing the mixture to cause the oil-coated mineral to form a froth," was clearly anticipated by other patents, and each step in the process described in the Sulman, Pickard, and Ballot patent is fully described in more than one of the prior patents, with the single exception of the reduced quantity of oil used; but the discovery that the small fraction of oil is sufficient to produce flotation of the metalliferous matter can not be made by itself or in combination, the subject of a patent. This Bureau of Mines report presents results from experimental studies on wood fires in a laboratory-scale tunnel network. Experimental measurements include ignition delay time, flame spread rate, ventilation air and gas flow rates, wood temperature, gas temperature, pressure and composition, thermal flux, and smoke particulates. These measurements form a data base for fire-scaling studies and provide information on various fire processes, such as flow interaction between ventilation air and fire, fire throttling and reverse flow, and duct fire energetics. Results show how hazards of duct fires relate to the development of (1) asphyxiating and toxic fumes, (2) explosive gas mixtures, (3) throttling of ventilation air, and (4) high-temperature combustion products. Such fire hazards should be carefully considered when testing flammabilities of materials used in a confined underground mine environment.

"Important stratabound lead-zinc deposits in southeastern B. C. occur in rocks that range in age from Proterozoic Helikian to Middle Cambrian. The Sullivan deposit, and similar but smaller deposits in the Sullivan area are the only important deposits that occur in clastic rocks with little carbonate content. The highly deformed and metamorphosed Shuswap deposits, of probable Hadrynian to Helikian age, are in mixed marble, calc-silicate gneiss and pelitic schist assemblages, rocks transitional between clastics and carbonates. Deposits in younger rocks, such as the Kootenay Arc deposits in the Lower Cambrian Badshot-Reeves Formation and the Monarch -Kicking Horse deposits in Middle Cambrian dolomite, are hosted by platformal carbonate rocks.An important regional control for the distribution of deposits appears to be a northeast-trending tectonic zone that crosses the Rocky Mountains and the Purcell Mountains just south of the Sullivan area, and projects southwestward toward the Salmo camp at the southern end of the Kootenay Arc. Pronounced northeast-trending gravity and magnetic anomalies, basic volcanism of Hadrynian age (the Irene volcanics west of Creston) and a preferred alignment of Mesozoic granitic rocks indicates that the zone coincides with deep crustal structures. Within the zone are dramatic and abrupt thickness and facies changes in Hadrynian Windermere rocks and lower Paleozoic rocks, and on the east side of the Rocky Mountain Trench in Helikian Purcell Supergroup rocks, indicating that block-faulting was active intermittently within the zone from Helikian through to lower Paleozoic time. Right-lateral and reverse movements occurred on northeast-trending tear faults within the zone in Mesozoic time."

The organization of the Explosives Division is indicated by the accompanying chart (fig. 1). As this has been discussed in a previous report3/, it will not be considered further in this report. The activities of the various units are outlined below. I. INFLAMMABILITY OF GASES AND VAPORS4/ Determination of the Explosion Hazards of Combustible Gases and Vapors Found in Mines and Industries The Explosives Division maintains, at the Pittsburgh Experiment Station, a laboratory specially equipped for the determination of the explosion hazards of combustible gases and vapors. Of particular importance, is the determination of the chemical composition within which explosions may occur and beyond which there is no danger of explosion, combustion or inflammation. These limits, known as the limits of inflammability, as determined by this and other laboratories, have been compiled in Bulletin 279 of this Bureau. The last revised edition (1931) of this bulletin has been exhausted. Since it was published, the Pittsburgh laboratory has determined the limits of about 40 combustibles and has ascertained more precisely ant accurately many previously published limits.

Operational excellence is generally known as a management system integrated across critical functions that is geared towards continually improving operational performance. Functional areas such as health, environment and safety, quality and human resources are the focus of many corporate-wide operational excellence programs. Operational excellence also focuses on improving areas such as customer orientation, employee empowerment as well as process, and systems optimization. It is widely viewed as critical to sustaining business performance improvement. Although it is difficult to find a universally accepted definition, or even common foundation in many respects, operational excellence remains a popular term in the business world. Many organizations have developed their own version of organization excellence, influenced by such individual factors, other similar institutions or even third-party service providers. A mining organization is no different from other organizations in that adopting and implementing effective operational excellence programs can dramatically improve business execution and performance across regional or global interests. This paper brings together various perspectives of operational excellence and reconciles them for a useful composite for the mining industry. While most agree that operational excellence should include normal functions in a manufacturing operation such as production, maintenance and engineering and processes such as quality, environmental and continuous improvement, many other factors for operational excellence seem to vary from perspective to perspective. Thus, programs that manufacturers engage in may seem the same, but may, in fact, be different in key areas. This may affect organizational performance in tangible ways. Management must buy into the company?s definition of operational excellence and be dedicated to achieving it. A review of many of the existing definitions of operational excellence, explicit or embedded as part of a program, seems to yield the following common factors: ? It should help organizations better execute their processes and services. ? It should be geared towards improving quality ? It requires that the various operations should be aligned with the overall enterprise strategy. ? It should be geared towards continuous improvement. A composite of operational excellence definitions suggest that it is: ? Geared towards helping the organization achieve sustained profitability; ? tied to strategic alignment and business objectives; ? tied to solid investment strategies; ? integrated as part of the culture, process, time plan and performance; ? a relevant performance measurement system; ? extended to include all aspects of the supply chain (including external suppliers); ? integrated with health, safety and environmental (HSE) aspects. And, while the pursuit of operational excellence intuitively suggests that businesses should improve, it is very difficult to achieve given the difficult challenges facing companies today. Mining businesses face numerous pressures to perform in a global marketplace such as demand, energy cost, variable cost and pricing fluctuations as well as infrastructure challenges to further confound the environment. Description of methods employed and cost data are given for surface-mined land reclamation in Moraine State Park, Butler County, Pa. This project was conducted under the authority of the Appalachian Regional Development Act; a total of 177 acres of land was rehabilitated for specific land use purposes. The use of large equipment expedited the project work, and similar results could be anticipated on similar reclamation programs.

Significant amounts of cobalt are present in readily accessible spent copper leach solutions. The Bureau of Mines investigated the feasibility of extracting cobalt from these solutions in a multiple-compartment ion-exchange (MCIX) column. The effects of aqueous flow rate, resin flow rate, column height, and compartment height on cobalt extraction were determined. Cobalt extractions of 92 pct or higher were achieved at solution flow rates of up to 5.5 gpm/ft of column cross-sectional area and an aqueous-to-resin flow ratio of 40:1. Overall cobalt extraction increased as the column height increased, but cobalt extraction per unit of length was more efficient in 8-and 12-ftco1umns than in an 18-ft column. Solvent extraction procedures were utilized to purify and concentrate the MCIX column eluates and produce a cobalt sulfate solution containing 70 to 80 giL Co. Metallic cathodes containing over 99 pct Co were e1ectrowon from this solution. Zinc, nickel, and copper byproducts were also recovered during processing of the ion-exchange eluates. Estimated total capital cost for a commercial-scale 10,000 gpm of a spent leach solution containing 26 million. With credits for zinc, nickel, and copper estimated net operating cost was $9.36/lb Co.

Dilation and contraction of the rock surrounding horizontal access open-ings of a deep mine were analyzed during stope advance to determine what type of measurements, made with relatively inexpensive instrumentation, would best reflect the rock behavior. A variety of instruments, mostly mechanical extensometers, were used. Detailed descriptions are given of Down-the-hole extensometers, floating rockbolt clusters, transit surveys, and other instruments and measuring techniques. Although all of the radial-deformation-rate graphs illustrated useful rock behavior data, the most informative graphs were compiled from data procured with the radial extensometers that penetrated, to various depths, the rock around the access openings. An analysis is given of measurement data on the rock around lateral drifts and across and adjacent to the nearly vertical vein of the Star mine, Burke, Idaho. Several useful relationships were found between the rock deformation rate and the stoping process. Information was produced on the elastic state of rock around an underground opening, effectiveness of stope support, rheology of rock, zone of influence around stopes, effectiveness of rockbolts, axis of maximum strain in a plane perpendicular to the long axis of the lateral, and other phenomena. This information could be of particular value in determining location, design, and support of underground openings.

"Newmont’s Mike prospect, currently undeveloped, is a blind Cu-Au(-Zn) deposit on the Carlin Trend, containing >8.5 M oz. Au, 1027 M lbs. Cu, and 809 M lbs. Zn as Carlin-type gold and polymetallic Cu-Zn minerals in Paleozoic lower-plate carbonates and shales. This project was undertaken 1) to examine the effectiveness of different lixiviants for leaching the Mike supergene Cu-Au ores via bottle roll tests, and 2) to determine what ore minerals resist dissolution in each leaching agent by comparing the mineralogy and chemistry of head and tail samples. Sulfuric and sulfurous acids were used to leach copper and cyanide was used to leach gold. Thiourea, glycine, and other lixiviants were used to leach both base metals and precious metal. QEMSCAN and SEM study of six samples of different ore types and grades indicate that the Au occurs as varieties of native gold, including auricupride and electrum. Major Cu minerals are native Cu, cuprite, malachite, chrysocolla, and conichalcite (Ca-Cu arsenate), with locally significant Cu in jarosite and goethite. Gangue mineralogy is dominated by quartz, sericite, chlorite, alunite, smectite and kaolinite, K-feldspar, and iron oxides (sensu lato). Results of bottle roll tests and leach residue mineralogy will be presented and discussed at the conference.INTRODUCTION Resource and Geologic Setting The Mike deposit is a large, blind Cu-Au(-Zn) deposit in the Maggie Creek district of the Carlin Trend of Nevada, USA, near the Gold Quarry mine (Figure 1). The mineral inventory, based on drilling, is 8.568 Moz. Au, 1027 M lbs. Cu, and 809 M lbs. Zn at average grades of 0.72 g/t Au, 0.34 wt% Cu, and 2.13 wt% Zn (inventory for a cone at $400/oz Au and $0.80/lb Cu by Norby and Orobona, 2002)."

As the problem of inclusions in steel has become increasingly important, more and more attention has been paid to the source of the oxide inclusions in steel. In killed steels the oxide inclusions are traceable to the reaction of deoxidizers with the iron oxide dissolved in the steel during the refining period of the open-hearth heat. In a large class of steel known as "rimming steel" the action of the steel during solidification depends entirely on the gas evolved, and this evolution in turn is controlled by the amount of iron oxide in the liquid steel in the mold. It is therefore of great importance to be able to determine the amount of iron oxide dissolved in steel and to devise methods for the control of iron oxide in any class of steel. The object of the present investigation was to obtain a true value of the oxygen con-tent of liquid steel. If the steel has not been deoxidized, the greater percentage of the oxygen is present as iron oxide in solution.

In 1986, at the request of the U.S. Bureau of Land Management, the U.S. Bureau of Mines studied part of the 24,600-acre Orejana Canyon Wilderness Study Area (OR-001-078), in order to evaluate its identified mineral resources. The study area, located in Harney County, 76 mi northeast of Lakeview, OR, is underlain by Miocene basalt, rhyolite flows, tuffs, and tuffaceous sedimentary rocks. No mines, prospects, or claims were found within the study area. According to the U.S. Bureau of Land Management, the tuffaceous rocks have a moderate favorability for the occurrence of diatomite and a low favorability for zeolite resources. Also, the rhyolite flows have a low favorability for the occurrence of perlite resources. However, U.S. Bureau of Mines' studies indicate that no metallic or nonmetallic resources are present in the study area.

FABRICATION of semiconductor devices.requires the formation of electrical contacts, often involving the use of lead, tin, and lead-tin solders. In several recent instances when germanium had been bonded to nickel with lead, a new metallic phase appeared, unattached to either germanium or nickel, but rather suspended as a continuous foil wholly contained within the lead matrix. Since such metallic structures do not form in nickel-lead or lead-germanium systems under identical conditions, a brief study was made of the characteristics and possible formation mechanisms of this material. Cold compressive strength (CCS) and apparent porosity (AP) of pellets are essential properties regarding the burden of blast furnaces. The optimal required values for industries are 2.5 KN CCS and 21% AP. Blaine fineness and firing temperature are the decisive parameters analyzed here. During firing, microstructure phases such as silicate, hematite, and pore are formed. The phase’s grain density during induration influences the pellet’s CCS and AP. The influence of the factors over the response is evaluated by I-optimal (response surface methodology) design. Blaine fineness, firing temperature, pore phase grain density (PPGD), silicate phase grain density (SPGD), and hematite phase grain density (HPGD) affect the CCS and AP of pellets. The RSM-I-optimal predicted responses with a coefficient of determination (R2) of 0.89 for CCS and 0.91 for AP of iron ore pellets. Optimal conditions were Blaine no. = 1668 cm2/g, 1250 °C firing temperature, PPGD value 90 no/mm2, SPGD value 250 no/ mm2, and HPGD value 490 no/mm2, with 4.5 KN CCS 27%. The induration processes were investigated through statistical design of experiments for process optimization. Statistical analysis indicated the direct and interactive influence over CCS and AP. In this study, RSM-I-optimal modeling is used for iron ore pellets processing. The I-optimal prediction could be applied to make the process economical because information is obtained through fewer experiments.

With the launch of Immersive Technologies' next generation of Advanced Equipment Simulators for the mining industry, correlating and influencing safety, productivity and maintenance with operator competency improvements is now made even more effective. In early 2008, Immersive assembled a project team to investigate the potential linkages between operator competency and performance. Advanced Equipment Simulator Users with a range of sophistication were interviewed and consulted. The result was new and enhanced 'value realisation' programs to directly assist mine sites to deliver the expected return on investment (ROI) from simulators and operator competency development. In just over one year, 29 mine sites around the world have benefited from 34 completed programs. Case studies have been compiled to demonstrate the impact of the implementation of this approach, including at the following operations: Anglo Coal - Callide (Australia), Freeport McMoRAN - Grasburg (Indonesia), Equinox Minerals - Lumwana (Zambia), Peabody Energy - Cabllo (USA), and Xstrata - Lomas Bayas (Chile). Significant performance improvements have been achieved including: reductions of 47 per cent in gear shifting and 37 per cent in over speed errors, improvements of 68 per cent in park break errors and 12 per cent in spotting time, and increase of 54 per cent in passing emergency fire handling competency tests. This presentation draws on these case studies to clearly demonstrate the linkage between improved operator competency through the implementation of training programs tailored around Advanced Equipment Simulators and production performace measured by a range of standard metrics at mines around the world.

A process was investigated for preparing Specification-grade nickel (containing less than 1 percent cobalt) by stripping it electrolytically from a nickel sulfate-boric acid solution and using the stripped electrolyte for the preferential dissolution of nickel from basic carbonates of nickel and cobalt. The basic carbonates were produced at the plant at Nicaro, Cuba, owned by the U.S. Government. Determination of the optimum conditions for deposition of Specification-grade nickel indicated that the upper limit of cobalt concentration was 0.1 gram per liter. The pH, temperature, and concentrations of nickel and boric acid in the electrolyte were not critical. An aluminum cathode and two lead anodes were used at a cathode current density of 1.2 amperes per square decimeter.
