Top priorities for all blasting companies and individual blasters are risk management, safety of company personnel and the general public, regulatory compliance, and liability protection. On a daily basis, hundreds of shots are detonated by well-trained, skilled, and experienced blasters without incident. Unfortunately, accidents inevitably occur, which is why blasting is considered an “ultra hazardous” or “inherently dangerous” activity from a legal perspective. Despite countless regulations, recent advances in technology, improved training, ongoing research, and improved communication in the industry, mistakes and the resulting incidents continue to occur during blasting operations. These result in dangerous or deadly flyrock, site security lapses, increased complaints, blast damage claims, and lawsuits, which lead to increased insurance costs and other financial tolls for blasting companies. In life, lessons learned the hard way are rarely forgotten. The same is true in blasting operations. Blasting missteps and the resulting events are often the best catalysts for improving risk management and thereby reducing accidents, liability, and financial tolls. This paper provides a summary of two incidents, one safety and the other liability related, and the errors that resulted in a fatality in one case and a multi property lawsuit in the other. The two incidents provide examples that serve as a case study for lessons learned the hard way in blasting operations and offer guidance for the considerations that blasters and blasting companies should incorporate into daily operating procedures.

The Red Dog zinc-lead-silver deposit is located in the DeLong Mountains of the western Brooks Range in northwest Alaska. The region is located 145 km(90 miles) north of the village of Kotzebue (Fig. 1).Red Dog’s current resource base is 143 Mt (157 million st) averaging 16.1% zinc, 4.3% lead and 83g/t (2.4 oz/st) silver. It is contained in four structurally separate deposits— the Hilltop, Main Deposit, Aqqaluk and the newly discovered Paalaaq deposit. Recent exploration for the Aqqaluk and the Paalaaq deposits has contributed to Cominco American’s understanding of the detailed structural geology of the district and its role in the placement of individual ore bodies.

Probably no other country or coutinent has so many rich deposits of various different minerals as Australia. Long ago in the days of my boyhood in the old land the marvellous discoveries of high percentage copper at Burra Burra, Peak Downs, Yudanamutana, and other mines, was one of the principal themes of conversation in mining circles whilst the specimens of malachite, azurite, etc, brought home by returning miners were the admiration of my boyish eyes.Later on the WalIaroo and Moonta mines still further helped to heighten the reputation of Australia as a copper-producing country. During the 21 years that the writer has been resident in the Australian colonies, other mineral deposits have sprung into prominence, and with a Mount Bischoff for tin, a Broken Hill for silver and lead, and a Mount Morgan for gold, Australia can now boast that it has had within its borders the leading mines of the world, in four different minerals, and still at least maintains that pre-eminence in gold, silver and tin. To the miner from the old land, Australian mining is at first a revelation and a wonder. In very many instances he looks in vain for the regular strike and dip of the veins he has been accustomed to'Smooth hanging and foot alls are often wanting. 'Huge irregular deposits, like those mentioned above, viz., Burra Burra, Mount Bischoff, Broken Hill, and Mount Morgan, are mysteries that will take time to fathom-if, indeed, they are ever thoroughly understood and in many a smaller deposit there is much to interest alike the professor in the college and the miner in the mine.

The authors studied small seismic exploration blasts for vibration characteristics and effects on structures in Mexico. Obtained were 267 three-component recordings of ground vibrations and structure responses from 62 test blasts at six single-story masonry structures. Charges were 4.55 kg (10 lb) of Vibrogel expolsive at depths of 20 and 30 meters. Initial blasts were 240 meters distant and proceeded towards the structures to as close as 5 meters. All blasts were in soil media in two basic types of terrain, low saturated areas and upland dry areas. Structural inspections were before, during, and after the test blasts.

Phosphogypsum is a byproduct of the manufacture phosphoric acid. There are approximately one billion tons of phosphogypsum stored in stacks in northern and central Florida. The Florida Institute of Phosphate Research (FIPR) is investigating potential uses of phosphogypsum. These include using phosphogypsum as a road base material, as a soil amendment in agriculture, as a daily cover in landfills, and as a construction material. The results of this research will be presented and will be discussed in light of regulatory barriers to the use of phosphogypsum.

The use of augered cast-in-place piles has seen tremendous growth under commerical structures in the past twenty years. Problems with vibration, soil densification and/or heave associated with driven piles do not occur with properly installed augered piles. Augered cast-in-place piles were originally constructed (1950s) with diameters of .31 m (12 in.) and lengths up to 6.1 m (20 ft); however with the advent of better drilling equipment, diameters varying from .41m(16 in.)to .61 m (24 in.) and typical depths ranging from 18.3 m (60 ft) to 24.4 m (80 ft) are achievable. Reinforcement may vary from a single high strength rebar (Grade 60) at the top of the pile to a continuous steel cage depending on loading (compression, tension-uplift, and lateral). Typical axial design loads (compression) for a single augered cast-in-place pile are in the 445 kN (50 tons) to 890 kN (100 tons) range.

The electrodes of a Submerged Arc Furnaces carry a high current and are subject to enormous stresses caused by both the electric arc itself, and the current distribution within the electrode. The external effect of the arc on the electrode is treated in a different paper at this conference, The purpose of this paper is to analyze the thermal effects of ohmic heating within the electrode at the arc attachment on the carbon based electrode, using data for Søderberg electrodes. The thermal conditions are then used to examine stress conditions in the vicinity of an arc. Two types of models of an electrode are presented in the paper. The first type is an accurate finite element model, based on both two and three dimensional elements and the second type is an one dimensional model, providing better insight into the problem. For the second type, a spherical model of thermal conditions in a semi-infinite solid has been developed. Results show that the electrode surface is heated quickly up to a maximum temperature and for a specific test case, the surface can only withstand the load for about 10 seconds. Also, the thermal stresses on the surface are very high which will increase erosion on the surface in addition to normal evaporation.

Ilmenite production in Western Australia commenced in 1956. There are now five producers, all in the Bunbury-Capel district, and current total production capacity is about 450,000 t.p.a.The deposits occur near the surface and average between 10 and 50 per cent heavy mineral. About 70-85 per cent of the heavy mineral is ilmenite. Other heavy minerals present include leucoxene, zircon, rutile, monazite, and garnet. The ilmenite contains 52-60 per cent Ti02. and is very low in chromium (0·03-0·04 per cent CrS03)Mining is done by sluicing, drag line excavation, or dredging. Primary concentration of the heavy minerals is achieved by spirals, revolving cones, or Reichert cones. After drying and screening, ilmenite is separated from the heavy mineral concentrate by various types of magnetic separator.Separate leucoxene, zircon, rutile, and monazite concentrates are also produced. Each plant uses a different technique and the processes used include chemical attritioning, screen plate electrostatic separation, high tension rolls, air tables, and dry launders.INTRODUCTIONThe treatment of beach sands in the Bunbury/Capel area for the extraction of ilmenite is a very recent development in the mining industry in Western Australia. Nevertheless, since the commencement of production in 1956, ilmenite has become the sixth most important mineral produced (Table 1).The increasing world recognition of Western Australia as a major producer of ilmenite (Table 2) is due primarily to the low chromic bxide content of the mineral (0' 03-0' 04 per cent Cr203), combined with a medium to high titanium dioxide content (54-60 per cent Ti02), supported by large reserves, a stable government, and close access to good shipping facilities. Ilmenite is. produced from these deposits at low cost which enables the industry to compete on world markets despite high rates for sea freights because of the distance from major markets.

"Zinc-iron alloy coatings can be produced by either thermally alloying hot-dipped galvanized steel or by electro deposition. Stelco Inc. produces a thermally alloyed Zn-Fe alloy-coated steel, which is mechanically wiped to remove free zinc.A more extensive use of Zn-Fe alloy-coated steel has been predicted for unexposed autobody and structural part applications, since this material is characterized by good spot weldability and formability, and provides superior corrosion resistance under automotive primers.This investigation has examined the coating composition, corrosion resistance, spot weldability and formability of Stelco's Zn-Fe alloy-coated steel and found its properties to be similar to those of Zn-Fe alloy-coated steel produced by post coating annealing.IntroductionRecently, there has been increased automotive interest in galvanneal, which is characterized by good spot weldability, good paintabilityt and good corrosion resistance when primed. Stelco produces a thermally alloyed zinc-iron coated sheet product similar to galvanneal. However, this product is produced by mechanically wiping the free zinc from the surface of hotdip galvanize to expose the underlying zinc-iron alloy.The surface of Stelco zinc-iron alloy-coated sheet is matte gray and has longitudinal streaks which result from contact with Kevlar wipers during production (Fig. I). This paper describes the coating composition, corrosion resistance, weldability, formability and surface appearance of Stelco zinc iron alloy coatings and, wherever possible, compares these properties to those of galvanneal."

The sulphide ores of the principal lodes of the Boulder Mines are mainly composed of a schistose guess with a varying proportion of silica and lime, containing tellurides of gold, silver and mercury, associated with iron pyrites. The treatment of a hydrobiotite containing 0·64 per cent copper with various soluHons showed that a high percentage (up to 90 per cent) of the copper can be extracted under mild conditions (1 M BCI, 85°C) or withsaline solutions (20 per cent by weight NaCI or NH4F, 400°C). The latter results suggest that the copper is incorporated in the interstratified vermiculite of a vermicuHte-biotite mica and the solutions (NaCl and NH4F) which reacted with the vermiculite without attacking the biotite gave the highest extraction yields.In general, extraction yields with saline solutions increased with the temperature (between 100 and 400°C) and duration (3 or 6 days) of the treatment but were not appreciably affected by the initial pH of solutions.The results of RCI treatment especially may have a bearing on the hydrometallurgical treatment of refractory copper ores where copper is tied to gangue silicates.

East Side Access (ESA) will extend Long Island Rail Road into midtown Manhattan via 11 km of tunnels, a new terminal station and associated rail support facilities. The hard-rock tunneling in Manhattan will traverse under Park Avenue and some of the most historically and commercially significant real estate in the world. The soft-ground tunneling in Queens will traverse beneath Amtrak’s Sunnyside Yard and the Harold Inter locking, the busiest rail interlocking in North America. This paper details ESA’s program-wide geo-instrumentation monitoring system for obtaining, processing and presenting all geotechnical monitoring data. Data streams will be real-time, such as from automated total stations, and manually read, such as from optical survey instruments and handheld data loggers. All data will be input into a web-based Geo-Instrumentation Data Management System, which will be accessible to project stake holders via ESA’s intranet or via the Internet using remote access controls.

This Bureau of Mines study was made to obtain and present facts concerning the production and marketing of rare-earth metals and thorium in the Central and Southern Rocky Mountains (Arizona, Colorado, New Mexico, Utah, and Wyoming, referred to in this report as the region) and to review the current demand for these elements in the United States. Little or no published information was available on the marketing practices for these commodities. As a result of many inquiries from industry, potential prospectors, and the general public, the various factors of resources, production (present and potential), marketing, and consumption have been investigated, and the results are set forth in this report. Material concerning occurrences and deposits is from published references detailed in the bibliography.

The World Trade Center (WTC) is located on the west side of Manhattan in New York City, approximately 0.5 miles north of Wall Street, and is owned and operated by the Port Authority of New York and New Jersey (PA). As part of the WTC site redevelopment the New York City Transit Authority?s No. 1 subway line which runs through the site below grade, was underpinned so that the soil beneath it could be removed along with the rest of the new east basement excavation. Consequently a scheme comprising micropile installation through and around the existing subway box was designed. The subway had to remain in use throughout save for planned weekend outages to allow a portion of the underpinning to be fitted through or on to the existing structure. At times this involved accessing and drilling from within the subway tunnel under short General Order (GO) Subway shutdown periods. At either end of the subway passage through the WTC site jet grout underpinning was designed to provide support and to complete the excavation and construction of the east basement wall in the areas beneath the subway where it was impossible to install a slurry wall. The jet grout blocks thus formed had to be largely installed around the clock during 53 hour long GO weekend outages and had to overcome severe logistical constraints in and over the tunnel. The jet grout blocks had to provide strengths of at least 500 psi at 3 days and 700 psi at 28 days. The jet grouted soil mass also had to have a level of permeability less than 0.03 feet/day (1 x 10-7 m/s). After an extensive trial program and then production testing it was concluded that the glacial till above the rock proved to be a particularly difficult soil strata to collect test data from and hence the level of observational monitoring was increased to progress the excavation works. Throughout the installation of micropiles and jet grout columns extensive monitoring was in place and prompted a variety of working method changes. Hematite floatability from two low grade banded iron ores of India, namely banded hematite quartzite (BHQ) and banded hematite jasper (BHJ) using fatty acids and amine as the collectors is investigated under identical physico-chemical conditions. The results indicated that hematite present in BHQ ore is easy to float using both the collectors, compared to BHJ ore. The difference of flotability between the two banded iron ores is that quartz grains are coarser and well liberated below 100 micron size in case of BHQ compared to BHJ ore. In case of BHJ ore, fine hematite and quartz grains are intimately inter-grown and are dispersed in thick jasper bands leading to liberation problems. Besides that, dusty jasper particles coat over hematite surface in case of BHJ ore during the process of grinding, which hinders specific adsorption of collector on hematite or quartz surfaces. This is substantiated by scanning electron microscopy (SEM). The flotation studies have indicated that it is possible to achieve ~64% Fe with ~67% recovery from a BHQ ore using both anionic and cationic collectors from a feed containing around 38% Fe. In contrast, the hematite recovery in BHJ ore was poor. Differences in the ore characteristics that affect the flotability of hematite present in both the ores are compared. AGAIN in 1963, as in nearly every year since 1946, the Canadian mineral industry has established a new national record in value of mineral production. The 1963 estimated total value of $2,976 million is 5 percent higher than the 1962 value of $2,845 million.2 As in previous years, metals made up more than half the value of total production. For selected years since 1949 the category percentages and total value are as follows: [ ] Crude petroleum, Canada's most valuable mineral product, accounted for 21 percent of the total value of minerals in 1963. In descending order came nickel 12 percent, iron ore 10 percent, copper 10 percent, gold 5 percent, and uranium 5 percent. Asbestos accounted for 5 percent, sand and gravel 4 percent, and cement 4 percent. The complete list of Canada's mineral products includes more than 50 basic commodities.

OD March 20, 1989, a meeting was held at the U.S. Bureau of Mines Pittsburgh Research Center at the request of Senator Pete V. Domenici of New Mexico. The purpose of the meeting was to engender a technical relationship between the National Laboratories and the minerals industry. During the meeting, the Director of the Bureau of Mines, T S Ary, suggested that since the Bureau is the focal point for mining research in the United States, it would be appropriate for the agency to act as the liaison between the National Laboratories and the minerals industry. As the first step in this process, the author was selected to perform a 6-month investigation to better define the proper role of the Bureau in this area. As part of the investigation, the mining and mineral-related technology available at 21 major U.S. Department of Energy (DOE) Research Laboratories was reviewed. It was assumed that the broad range of scientific and technical areas covered DOE research would provide fertile ground for mining-related technologies. It was hoped that the applicability of DOE research to this liaison effort would be indicative of Federal laboratories in general.

For the past several years, copper producers have faced a tough business climate. Nagging low prices forced many them to reduce their production and make other drastic cutbacks just to breakeven. And sometimes even that did not work. But, like other downturns in the industry, this latest dismal copper market allowed technology to make further inroads into mining and processing operations. Now that the copper market appears to be making a turn for the better, those years spent investing in technology by equipment manufacturers and forward-thinking mining companies should begin paying off.

An abandened breakwater, located at the rear of No. 2 berth in Dongdu Port area of Xiamen City, was not cleaned thoroughly when the backfilling of sand started. The uncleaned stone layer was about 2 to 3 meters deep, with the average piece of stone weighing about 1 to 3 kN. A large grain storing house, 220X 60m2 in area, was designed and built at the above mentioned complicated site. The foundation of the grain house are made up of the cast-in-situ piles. It is well known that it is difficult for the submarine engineering drill to go down through such large-sized stones. Having com-pared the various plans, a final decision has been made which involves explosions with small amounts of charge plus cone bit in the enlarging of the size of the pores. Such a method has successfully prevented the pores from slumping and made it possible to fulfill the project without any difficulties. This paper also introduces the ways in effectively dealing with each individual stone.
