The 26th Peruvian Mining Convention (Convención de Ingeneieros de Minas del Perú) was held Sept. 15 through 19, 2003, in Arequipa. The convention was sponsored by the Peruvian Institute of Mining Engineers (Instituto de Ingeneiros de Minas del Peru). The Institute has been organizing these events since 1954. They are now held every other year. The conventions spotlight Peru’s mineral resources and have contributed to the tremendous growth of the Peruvian mining industry. This year’s meeting consisted of technical sessions, forums, field trips and social activities. It was held at the Tecsup Institute of Technology of Arequipa. More than 3,000 mining professionals attended. As always, the meeting was well organized, and participants expressed gratitude to the organizing committee for another successful meeting.

The three thousand foot long 66-inch diameter Folsom East Interceptor Section2B was completed accident free, on time and under budget with no measurable surface settlement. The tunnel passed through nested cobbles, highly abrasive sands and gravels, and abandoned mine workings with less than five-feet of cover at times. Preventing settlement was critical since the tunnel passed under a heavily traveled state highway. A pilot project to test microtunneling and pipejacking methods in the same ground conditions abandoned both drives before completing the planned drive lengths. This paper discusses performance of the conventional tunneling method selected to excavate the ground and the effectiveness of the ground modification techniques employed.

The development of information concerning mesothelioma in South Africa started with the 1959 finding of Dr. J. Christopher Wagner of cases of mesothelioma in persons living in the vicinity or working in crocidolite mines in Cape Province. After more than 40 years, the data from South Africa now provide important insights into the differences in mesothelioma risk associated with different asbestos fiber-types. Three case series reports and one analytical report were reviewed in this session. In total, information on 504 proven cases was presented. There were 28% of cases arising from occupational exposure in mining, 35% from non-mining occupations, 23% from environmental exposures; 14% where no exposure was established and 9% where the exposure was unknown. Among the 129 cases for which occupational exposure and asbestos fiber-type were established, 115 cases were associated with occupational exposure to Cape crocidolite; 1 case associated with occupational exposure to Transvaal crocidolite and 9 cases with occupational exposure to amosite (grunerite asbestos) and 4 cases with occupational exposure to mixed fiber-types. There were no cases associated with chrysotile asbestos. There were 33 cases of environmental mesothelioma linked to Cape crocidolite exposure and 2 to exposure to amosite and crocidolite but none to Transvaal amosite alone or to chrysotile. The evidence shows that amosite is capable of causing mesothelioma but that the major risk in South Africa is associated with Cape crocidolite.

With the dawning of a new century came the beginning of a new era in the coal industry. Instead of the traditional practice of only buying and selling produced coal in the United States, a coal futures market was established. The New York Mercantile Exchange began trading coal futures in July 2001. But, even in this new era, many of the old is-sues that have beset the coal industry still remain, as was evident in2001. Coal production increased to are cord level during 2001, according to preliminary data from the US Energy Information Administration(EIA). Production in 2001 rose by4% to 1.01 Gt (1.12 billion st), compared with 2000 (Table 1). This barely surpassed the previous record level of 1 Gt (1.11 billion st)set in 1998. Coal consumption remained unchanged. The additional production was used to replenish the depleted stockpiles that had resulted after two years of declining coal production and increasing coal consumption.

The Pittsburgh Research Laboratory (PRL)1 of the National Institute for Occupational Safety and Health (NIOSH) in collaboration with the Mine Safety and Health Administration (MSHA) Technical Support Center has devised a prototype hand-held instrument which can provide a direct assessment of the potential explosibility of a coal and rock dust mixture. Current Federal regulations require that rock dust be applied in all underground areas of a coal mine to mitigate the propagation of a coal dust explosion. To determine compliance with the Federal regulation, mine inspectors collect dust samples from sections of underground coal mines for an MSHA laboratory analysis of incombustible content. However, the laboratory analysis may not be completed for more than a week or two, during which time the mine area from which samples are taken may be inadequately protected. The operation of the Coal Dust Explosibility Meter (CDEM) is based on the difference in optical reflectance between light rock dust and dark coal dust. For a given coal volatility, the optical reflectance of such mixtures is relatively constant at the limit of explosibility, independent of the coal dust particle size. The CDEM is not intended to replace the current MSHA laboratory analysis of coal mine dust samples for incombustible content, but to serve as a supplemental device for enhancing mine safety through improved rock dusting practices.

INTRODUCTION Perhaps no other area reflects the art as well as the science of mining-engineering design quite as obviously as strata control. This is due to the fact that the design of mining structures is difficult because rock rarely behaves as a homogeneous medium. Further, the original state of stress and the mechanical proper- ties of the in situ rock are somewhat difficult to deter- mine. However, those aspects that are hard to treat mathematically can and have been successfully approached by model studies and in situ measurements. This chapter will review the fundamentals of strata- control design and will provide examples of applications to typical problems. STRESSES AROUND MINE OPENINGS A close examination of Fig. 1 reveals that, prior to mining, stresses exerted in the overburden can be in both the horizontal and vertical directions. In the example problems of Chapter 1, it was shown that the maximum compressive stress (uv) at any depth (D) below the surface can be approximated as follows: [ ] Further, the lateral stress (oh) is related to the maxi- mum compressive stress by utilizing Poisson's ratio (µ): [ ] As soon as a heading is driven, such as the one in the coal seam shown in Fig. 2, the stresses shift to the sides, or abutments, of the opening. Clearly, these compressive stresses around the opening exceed the pre-mining conditions. The problem of designing an underground opening, therefore, is to determine the maximum stresses and to note if they exceed the ultimate strength of the rock. Table 1 lists several rocks with their corresponding strengths. Figs. 3 to 5 depict the boundary stress concentrations around circular, ovaloidal, and rectangular openings. M is the ratio of the horizontal stress to the vertical stress, or as defined above, p/(l-p). This text will concentrate on the state of stress represented by M = 0.33, which corresponds to the condition of no lateral deformation in a rock having a Poisson's ratio of 0.25. For an ovaloid with an opening width-to-height ratio of 2.0 (Fig. 4), the boundary stress concentrations are at their maximum values at the comers of the opening, with values in excess of 3.0. On Nov. 13, 1985, Colombia's Ruiz volcano erupted, killing an estimated 25,000 people and burying several villages. Damages were estimated to be about $US300 million. The following is an account by SME member Antonio Rivera of the volcanic activity that led to the eruption. His account also includes a description of the geography and some geology of the affected area. Mining operations in the area were mostly unaffected, although some operations were forced to close until the roads leading to them could be repaired. Introduction The Ruiz volcano is located on the Central Cordillera of Colombia about 28 km (17 miles) southeast of Manizales, the capital of the state of Caldas. Ruiz is one of nine volcanoes that form a complex volcanic chain within the middle portion of the Central Cordillera. The Cordillera has 30 other volcanoes in the southern part of Colombia. Ruiz volcano Ruiz's elevation is 5200 m (17,060 ft). It is covered by an ice cap estimated to be 20 m (66 ft) thick at its summit, an area that covers 17 km2 (6.5 sq miles). Drainage is composed of several rivers that flow into the Magdalena River on the east and Cauca River on the west. The rivers flowing to the east are Guali, Azufrado, Lagunillas, and Recio. The rivers that flow to the west are Claro, Molinos, and Chinchina. There is a road from Manizales to El Refugio, a ski area located on the northeast flank of the volcano at 4800 m (15,750 ft) in elevation. The Ruiz volcanic complex was built in two stages. The first is a shield type of volcano (mainly lavas) formed about a million years ago. The second is a younger stratovolcano overlaying the ancient one. It consists of lavas and pyroclastics and was built over the past 500,000 years. Twenty-four eruptions of Ruiz are known to have occurred during the last 14,000 years. Its products mainly consist of lava flows, pyroclastic flows, mudflows (lahars), and directed lateral explosions. Historic eruptive record 1595 - Debris avalanche, blast, pyroclastic flows, mudflows, ash flows, and ash fall. 1828-1829 - Ashes (phreatic). 1831-1833 - Gas emissions and ashes. 1845 - Mudflows and pyroclastic flows. 1985 - Sept. 11- Fumarolic activity, phreatic eruption, ash fall, and small mudflows. 1985 - Nov. 13 - Ash and pyroclastic fall, major destructive mudflows (magmato-phreatic eruption). From 1845 through 1984, Ruiz volcano always had a slight fumarolic activity. It was restricted to the crater area. In November and December 1984, the fumarolic activity increased and the first ash emission and vapor-sulfur formation started. Small local earthquakes accompanied these volcanic activities. The volcano had a small erup¬tion on Sept. 11, 1985. It was manifested by phreatic explosions that ejected fragments of the vent and formed ash deposits 25 mm (1 in.) thick around an area 2.5 km (1.5 miles) from the crater. Fine material was blown by the wind and reached Manizales and neighboring areas, producing a small layer of ash up to 10 mm (0.4 in.) thick.

In the early 1970s, it became a concern that exposure to the mineral fibers associated taconite ore processed in Silver Bay, Minnesota would cause asbestos-related disease including gastrointestinal cancer. At that time data gaps existed which have now been significantly reduced by further research. To further our understanding of the types of airborne fibers in Silver Bay we undertook a geological survey of their source the Peter Mitchell Pit, and found that there are no primary asbestos minerals at a detectable level. However we identified two non-asbestos types of fibrous minerals in very limited geological locales. Air sampling useful for risk assessment was done to determine the type, concentrations and size distribution of the population of airborne fibers around Silver Bay. Approximately 80% of the airborne fibers have elemental compositions consistent with cummingtonite-grunerite and the remaining 20% have elemental compositions in the tremolite-actinolite series. The mean airborne concentration of both fiber types is less than 0.00014 fibers per milliliter that is within the background level reported by the World Health Organization. We calculate the risk of asbestos-related mesothelioma and lung cancer using a variety of different pessimistic assumptions. (i) that all the non-asbestos fibers are as potent as asbestos fibers used in the EPA-IRIS listing for asbestos; with a calculated risk of asbestos-related cancer for environmental exposure at Silver Bay of 1 excess cancer in 28,500 lifetimes (or 35 excess cancers per 1,000,000 lifetimes) and secondly that taconite associated fibers are as potent as chrysotile the least potent form of asbestos. The calculated risk is less than 0.77 excess cancer case in 1,000,000 lifetimes. Finally, we briefly review the epidemiology studies of grunerite asbestos (amosite) focusing on the exposure conditions associated with increased risk of human mesothelioma. © 2008 Elsevier Inc. The Pipeline gold ore body was discovered in Lander County, NV in March 1991. Construction of the new mine and mill did not start until early March 1996. The $289-million project was placed on a fast-track development schedule upon completion of permitting and corporate approval. The mine and mill were commissioned 11 months after the start of construction and the project ended up $40 million under budget. Project design and construction management were provided by Placer Dome Project Development Division (PDD). History The Cortez Joint Venture has operated in northeast Lander County since the late 1960s. The business structure is comprised of 60% Placer Dome and 40%Kennecott Exploration. Throughout the years, production has been from several small open-pit deposits— Cortez, Horse Canyon, Gold Acres and now Pipeline.

Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

Construction for the new 2.5 MGPD raw water intake project commenced December 2002. The project, initially designed as a 12' Ø concrete lined intake shaft,145' deep, with two offset 24" lake taps, 520' and 470' long was subsequently “value engineered” running the two lake taps through a single conduit. The offset for summer and winter pools was accomplished via a second shaft located at the lakes edge. Construction involved the use of blind drilling techniques for the shaft development and drill and blast techniques for the tunneling. All works were completed through extremely hard crystallized sandstone. The project came online August 12, 2004.

The Coronado Traffic Tunnel is a proposed two-lane, reversible, road tunnel, which will relieve severe peak hour congestion problems on surface streets. The tunnel will take traffic from the Coronado Bay Bridge to Naval Air Station North Island. The project will include 12.5 m (41 ft) inside diameter by 1,112 m (3,650 ft) of tunnel, 552 m(1,815 ft) of cut and cover box structure, and 400 m (1,315 ft) of open cut structure. The geology consists of predominately clean and silty sand, saturated sand, with some clay layers, requiring Earth Pressure Balance or Slurry Shield tunneling technology.

Hydro-Jex© releaching was undertaken during the summer of 2010 at the Valley Leach Facility (VLF) at AngloGold Ashanti?s Cripple Creek & Victor Gold Mine. Several advancements to the technique were made in response to the technical design and operational challenges posed by the unique site conditions. New geophysical resistivity monitoring techniques were developed to monitor solution migration during the injections. These developments are presented along with discussion of gold production resulting from the project.

The Reserve Mining Co., in the processing of taconite iron ore quarried from the Peter Mitchell Pit, deposited the finely crushed waste rock into Lake Superior near Silver Bay Minnesota. In the early 1970s The U.S. Environmental Protection agency filed a lawsuit against Reserve Mining Co., asking the court to force the company to stop dumping into the lake. On April 20, 1974, after a lengthy court battle, the U.S. District Court concluded that the Reserve Mining discharge into Lake Superior violated federal and state pollution laws and thus ordered Reserve Mining Co., to halt the lake disposal of taconite tailings. At the time many feared that some of the mineral particles contained in the tailings (alleged to be asbestos or asbestos-like) would cause human health problems. This Conference was thus convened to readdress the problem of the mineral content of the Biwabik Iron Formation and to whether any minerals contained in the ore could cause human disease. The four papers given in this session address the nature of the mineral particles in the taconite rock, the definition of the term ??asbestos?? and the history of the legal controversy over the possible health effects of certain mineral particles contained in the Biwabik iron range ores. Morey The Biwabik Iron Formation was a principal source of ore for the American steel industry during the 20th Century. The unaltered iron deposits, called taconite, contain approximately 30% iron and 50% SiO2. About 75% of the iron in taconite resides in the mineral magnetite, the remainder is largely in iron carbonate and several iron-silicate minerals.

A variety of grades of very white muscovite mica are now being produced from sedimentary sands and clays in Central Georgia. The mica is naturally concentrated near the base of the lens-shaped kaolin deposits and also disseminated in associated quartz sands. The same weathering and bacterial alteration processes which have transformed the kaolins from dark organic muds to white pigment-grade industrial clays have also removed discoloring iron from the micas and developed unusually white muscovite. Further, chemical alteration has removed some potassium from the muscovite atomic structure, resulting in an easily delaminated mica which can be cleaved to unusually fine, high bulking, high aspect ratio products. The peculiar alteration history of these sedimentary micas enables production of some of the whitest, finest particle size, and thinnest aspect ratio muscovite mica products on the world market today. Raw mica feed is obtained from kaolin and sand processing by-products, and also from beneath the useful base of abandoned kaolin mines. The processed ultrafine and filler grade muscovite mica products are used for paint, plastics, cosmetics, mold release, thermal and electrical insulation, epoxy, and dry wall joint cement markets.

The Eskay Creek Mine, a high-grade gold and silver underground mine in northwestern British Columbia started operations in late 1994. For the first three years of operation, ore was crushed to minus 2 inch; blended to satisfy smelter terms and direct shipped to smelters in Japan and Canada. Before commencing the above operations, preliminary evaluations had indicated that an extremely complex metallurgical facility, requiring a large capital investment, would be required to recover the precious and other economic metals. Subsequently markets for raw ore were identified and underground mining and ore transportation operations, at a nominal production rate of 300 tonnes per day, were commenced. Exploration diamond drilling in 1995 and 1996 discovered additional lower grade mineralised zones, which are less economic or uneconomic as direct shipping ore. Test work demonstrated that these lower grade zones could be upgraded to marketable concentrates in a simple flotation mill. The mill was built in 1997 and has been in operation since late 1997. The mining rate now has increased to 400 tonnes per day including 250 tonnes per day of the direct shipping ore and 150tonnes per day of the mill ore. Prime Resources Group Inc. wholly owns the Eskay Creek Mine. Homestake Canada Inc. owns 50.6% of Prime’s shares and manages and operates the mine on Prime’s behalf. This paper discusses the mining and milling operations as well as ore shipment and smelter contracts' administration.

South Africa (SA), a country in which all three commercially important asbestos minerals have been mined and milled, has retained proven cases of mesothelioma linked with environmental exposure to asbestos. This study illustrates the importance of fiber type in the occurrence of environmental mesothelioma. Four studies have reviewed the source of occupational or environmental asbestos exposure in 504 histologically proven cases of mesothelioma in South Africa. One hundred and eighteen cases (23%) were thought to be related to environmental exposure to asbestos. In the vast majority of these cases, exposure was linked to crocidolite mining activities in the Northern Cape Province. Two cases were thought to have occurred in relation to amosite and Transvaal crocidolite exposure in the Limpopo Province. In the balance of cases there was some uncertainty. No cases were reported with exposure to South African chrysotile. Consequently, in the vast majority of cases of mesothelioma, environmental exposure to asbestos occurred in the Northern Cape Province, in proximity to mines, mills and dumps where crocidolite was processed. Crocidolite appears to be far more mesotheliomagenic than amosite, and chrysotile has not been implicated in the disease. Over the past few decades a great deal of effort has gone into characterizing the effects of frother type and concentration in flotation using a variety of two-phase techniques. Whilst useful and informative, this research still does not assist in the selection of suitable frothers for three-phase industrial-scale froths. This paper investigates the effects of frother type and concentration in a three-phase industrial froth. The objective was to determine if progress could be made in terms of understanding the behavior of frothers in flotation and understanding how they affect the sub-processes occurring in the pulp and froth phases and, ultimately, the metallurgical performance of a flotation cell.

The Research and Development Services of South Africa has begun underground trials with a Robbins 52RE blindhole borer using a cutterhead fitted with high pressure water jets to assist mechanical cutting. The objectives of the work are to reduce cutter costs by increasing cutter life and to increase overall penetration rates. The paper reviews blindhole boring in South Africa and laboratory experiments in rock cutting assisted with high pressure water jets. The borer and its ancillary equipment are described. Initial results of underground boring experiments are presented. The paper concludes with the test programme for the future.

Introduction Examining the short- and long-term job prospects for engineers in the coal industry might include looking at coal tonnage projections, future export markets, federal regulations, and the decrease in overall graduates. But coal is the US most competitive basic industry. As such, why not go back to basics? That is, there are three basic challenges that need to be faced by students, faculty, and members of the coal industry. These challenges are counseling, career placement, and professionalism. Counseling Counseling can include elementary, high school, and college students, as well as individuals already employed. But what happens to students after they have selected a course of study at a particular university? As shown in Fig. 1, only 87,000, or 7.25% of our graduates have technical degrees. Generally, there is a strong market projected for these engineering graduates. However, many students who have majored in other nontechnical curriculums, have to scramble for jobs. This says something about our ability to make students aware of where the real job opportunities are. As a student spends four or five years pursuing a degree, he or she is looking for professional guidance. The student has the right to expect "real world" counseling - counseling that is not "the way it should be" but "the way it is." The influence of faculty members is significant. They should take this long opportunity to tell students not what they want to hear or what the problem's of academia or the industry are, but what the real job market is all about. If faculty do not use this opportunity, they in-advertently-through omission - mislead their students. Once a student graduates and is a working alumnus, faculty should take advantage of their relationship with the student and continue to stimulate conversation leading to a two-way exchange of ideas and information. Faculty should also encourage and solicit financial support for their mining departments from their alumni. These monies, in many cases, will be matched dollar for dollar or more, depending on the company the alumnus works for. In the private sector, the responsibility is to communicate with faculty and students. Alumni can be effective by participating in this two-way dialogue with their professors at meetings, technical seminars, and conventions. In many cases, alumni will employ summer or co-op students in their organizations. While getting to know these students, industry members should level with them on where the true opportunities are and what it takes to be successful. It is important to communicate with both faculty and students candidly on both the posi-
