A survey of roof and pillar conditions in underground limestone mines in the United States has revealed that bench mining of the floor between pillars can cause instability in the pillars at the perimeter of the benched area. Increased loading of these pillars was observed at several mines. Large inclined geological structures that are exposed in pillar ribs were observed to contribute to pillar instability. The paper describes a study that was carried out using numerical models to assess the effects of bench mining on pillar load and stability. It was found that the benched pillars shed load onto the surrounding pillars owing to their reduced stiffness. The pillars at the perimeter of the benching area will start shedding load as soon as benching increases the height of one side of the pillar. A case study is described which shows the effects of bench mining on limestone mine pillars.

Methane in coal constitutes a serious hazard in coal mining. Draining the methane in advance of mining can mitigate this hazard, conserve energy, and offer economic benefits. This Bureau of Mines report evaluates the Pittsburgh coalbed in Greene County, Pa., to determine if advance drainage is feasible. The gas content of virgin reserves in Greene County is estimated at 100 to 200 ft3/ton, a total of over 370 billion ft3 of gas in an area approximately 310 mi2. Because of the similarity to natural gas, drained coalbed gas could be recovered and added to pipeline supplies of natural gas. The value of conserved gas would offset the cost of drainage and extend energy resources.

Fiji has been an independent country within the British Commonwealth of Nations since 1970. It consists of an archipelago of more than 300 islands ranging from tiny coral atolls, cays, and rugged coral limestone islets, to the larger mountainous islands of Kandavu, Taveuni, Vanua Levu, and Viti Levu of volcanic origin. The islands of Fiji straddle the 1800 meridian about 1,800 kilometers south of the Equator in the part of the southern Pacific Ocean known as Melanesia. The production of gold and silver from the Emperor Mine at Vatukoula on the island of Viti Levu, the main island of Fiji, remained Fiji's only metallic mineral producer. Mining began at the Vatukoula site in 1933, producing 124 million troy ounces of gold and 147 thousand troy ounces of silver by yearend 1985.

Cost Systems Cost systems are organized methods of collecting, allocating, and accumulating resource utility expressed in terms of money. There are two major spheres of cost accumulation: financial accounting and cost accounting. Financial accounting deals primarily with historical costs and provides a means of measuring and recording cost events which have al¬ready taken place. While there are no legal requirements to control the methods of recording and reporting cost data in the United States, the procedures in use are guided by professional organizations such as the American Institute of Certified Public Accountants. The final pro¬ducts of financial accounting practices are the income statement and balance sheet which describe the financial status of the organization under study. Cost or managerial accounting is an expansion of financial ac¬counting procedures, providing more detailed cost breakdowns on present operations with one objective being the projection of present costs into the future. The projection process tends to limit the accuracy of the information; however, to management and project justification per¬sonnel, data on future operations tends to be of greater value. The basic principle behind cost accounting is the cost cycle. As products are developed from raw materials, the value of the work-in¬process increases due to the cost input of the various fabricating ele¬ments. Thus, from the basic input elements of facilities, labor, and raw materials, costs are added to each unit of production as a function of the work performed on that unit.

The origin of gaseous hydrocarbons in coal, predominantly methane, can be traced to the natural processes of coalification. The higher rank coals or the more deeply buried seams retain more gas than low rank coals or shallow seams. It is estimated that up to 460,000 cubic feet of all types of gases are produced during the formation of one ton of coal (Cervik, 1969). The chief components of these gaseous products are methane, carbon dioxide, nitrogen and water vapor. Most of these gases escape during the various stages of decomposition but with eventual presence of overlying impermeable beds, there may be as much as 2,000 cubic feet of gas retained per ton of coal. Over geologic time, diffusion and other processes such as geologic disturbances in the earth's crust remove all but 500 to 2000 cubic feet of gas per ton of coal (Cervik, 1969). It is this residual methane which is of primary concern when mine openings are made in coal measures. Extensive data compiled by the United States Bureau of Mines reveal that some coal mines in this country emit up to 20 million cubic feet of methane per day (Spencer, 1970; Irani et al, 1972). Toxic, poisonous and respirable gaseous pollutants and explosive gases, predominantly methane, are injected into the ventilation stream from active sections and gobs. The liberation of methane in gob areas may, under certain circumstances, become extremely large. In some instances it may constitute up to 70 percent of the total yield of gas in a mine. Laboratory mineral-dressing investigations were conducted on talc-bearing samples from Arkansas and Texas under the Bureau of Mines program of examinations and surveys, which is concerned, in part, with the improvement of conservation and utilization of domestic mineral resources. Two of the soapstones in question were successfully upgraded by flotation, utilizing stage additions of a frother and an amine to promote their talc content. The process could enable the producers to meet more rigid specifications of higher priced talc commodities. INTRODUCTION Talc is an industrial mineral with distinctive properties, such as softness, smoothness, large surface area in relation to mass, chemical inertness, and good retention as a filler, that make it particularly valuable to a number of industries. Sales to 6 industries -ceramics, paint, insecticide, rubber, roofing, and paper -consumed 84 percent of the domestic production of talc, pyrophyllite, and ground soapstone in 1952. To improve electrical safety and lengthen light bulb life on trolley haulageways. Approach A resistor inserted in series with each pair of light bulbs, decreases the voltage applied to the bulbs and eliminates heavy arcing when voltages rise. How It Works Under certain conditions, incandescent light bulbs connected directly to a 300-volt d-c trolley wire can be a hazard, particularly during weekends when electrical equipment is nut in use. When little electrical power is being used, the voltage (on the trolley lines and at the substation) can rise to a value which will destroy the bulb filament, cause arcing, and damage the bulb holder. In one coal mine where light bulbs in certain locations fre¬quently failed, the problem was overcome by inserting a 100-ohm, 50-watt resistor in series with each pair of light bulbs. The light bulbs are rated at 300-volts and 100-watts. Figure 1 shows the light bulbs and resistor circuit. This resistance was added in order to reduce the current through a short circuited bulb holder, from several hundred amperes to a much safer value of 3 amperes. However, the resistor in series with each pair of light bulbs, also reduces the voltage across the lighted bulbs to approximately 240 volts during periods when the trolley line voltage is up to 300 volts. With this arrangement, no light bulb failures occurred over a 6-month period. The resulting light reduction has not been objectionable.

This report (Safety File 3.0) is the fourth in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics (PE), and includes the benefits of implementing a system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard IEC 61508 [IEC 1998a,b,c,d,e,f,g] and other standards. The scope is “surface and underground safety-related mining systems employing embedded, networked, and nonnetworked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety. • 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meet the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full lifecycle of the system, and provides administrative support for the safety program of the full system. • 4.0 Safety Assessment.—The independent assessment of the safety file is addressed. It establishes consistent methods to determine the completeness and suitability of safety evidence and justifications. This assessment could be conducted by an independent third party. • 5.0 Safety Framework Guidance.—It is intended to supplement the safety framework reports with guidance providing users with additional information. The purpose is to assist users in applying the concepts presented. In other words, the safety framework is what needs to be done and the guidance is the how it can be done. The guidance information reinforces the concepts, describes various methodologies that can be used, and gives examples and references. It also gives information on the benefits and drawbacks of various methodologies. The guidance reports are not intended to promote a single methodology or to be an exhaustive treatment of the subject material. They provide information and references so that the user can more intelligently choose and implement the appropriate methodologies given the user’s application and capabilities. [ ]

Introduction This Instructor’s Copy contains most of the materials trainers will need in order to use Drill Rig Incident (DRI). It includes suggestions for using the exercise, performance objectives for the training, master answer sheets, a scoring key, and a set of discussion notes that provides additional subject information. Four appendices are also included. Appendix A is the complete exercise problem booklet. It may be reproduced so that every person in the training class has one. Appendix B contains answer sheet blanks. The blanks are furnished if you choose to have the invisible ink answers that appear in appendix C printed on them at your location.3 Answer sheets are consumable. One is needed for each group of three to five persons who work the exercise together. An alternative is for each individual trainee to have his/her own answer sheet. The Maslach Burnout Inventory (MBI) was administered and scored for 154 Mine Safety and Health Administration (MSHA) employees kom the US Department of Labor. These employees serve as accident investigators for serious accidents and fatalities as part of their job duties. During a workshop on stress designed for them, subjects volunteered anonymous written anecdotes, confirming and illustrating the findings. The subjects represented locations across the USA and all mining commodities: coal, metal, non-metal, stone, and sand and gravel. The Maslach Burnout Inventory assesses three aspects of experienced burnout: emotional exhaustion, depersonalisation, and reduced personal accomplishments. The average results of the scores for each of the three subscales for the study group of MSHA accident investigators fell in the moderate range. These scores show that the subject population was at some risk for emotional exhaustion and depersonalisation, tending toward burnout. The group evidenced a lower sense of personal accomplishment in their jobs, indicating a higher risk for burnout. From this outcome accident investigators may be at moderate (note: this does not mean average) risk for burnout. The authors recommend follow-up with this population - specifically in training to work with grieving families; and greater organisational support for the accident investigators in this area would be appropriate. Emerging laser technologies are quickly gaining acceptance in the industrial workplace. Besides the risk of human exposure, one safety concern with the more intense lasers is the potential for ignition of flammable gases, vapors, dusts, fibers, or flyings often found in hazardous (classified) industrial settings. In underground coal mines, lasers are being considered for remote measurement of explosive methane gas. Current federal safety regulations for underground mines contain no specific guidance for evaluating the safety of optoelectronic components such as diode lasers. The National Institute for Occupational Safety and Health is conducting research to help provide a scientific basis for developing appropriate safety guidelines for optical equipment in the presence of flammable methane gas and/or coal dust. One phase of the study is an experimental evaluation of methane-air ignition by laser heated small particles. Minimum observed igniting powers for laser energy delivered by 200-, 400-, and 800-um core diameter fiber optic cables and directed onto selected targets in methane-air atmospheres were 0.6, L l, and 2.2 W, respectively. Comparisons are made with other ignition phenomena, including heated wires.

THIS CHAPTER of the Minerals Yearbook contains injury experience and related employment information for the coal-mining, coking, oil and gas, peat, and native asphalt industries in the United States for 1962. Injury experience is measured by the number of injuries sustained per million man-hours of exposure in each industry. No attempt has been made to combine these data and present rates reflecting the mineral-fuel industries because the hazards of the separate industries are not comparable. Tabulations showing the trend of injuries and employment for all mineral industries are presented in volume III of the Minerals Yearbook. COAL The injury-frequency rate for the coal-mining industry of the United States increased less than 2 percent, according to reports received by the Bureau of Mines. The number of fatal and nonfatal injuries per million man-hours in 1962 was 45.86, based on final data for anthracite mines and preliminary data for bituminous coal and lignite mines.

An experimental program was conducted to evaluate the accuracy of some current methods for predicting the flammability of gas mixtures containing hydrogen and flammable or nonflammable volatile organic compounds (VOCs) in air The specific VOCs tested were toluene, 1,2-dichloroethane, 2-butanone, and carbon tetrachloride. The lower flammability limits (LFLs) of gas mixtures containing equal molar quantities of the components were determined in a 19.4-1 laboratory flammability chamber using a strong spark ignition source and a pressure criterion for flammability. All but one of the LFL values for the individual components were in agreement with earlier literature values. However, the LFL of l,2-dichlorocthane was found to be significantly lower than the range of values reported for previous determinations in smaller chambers. Two methods for calculating the LFL of mixtures were considered. The Group Factor (atomic) Contribution Method was determined to he generally more accurate than the LeChatelier Method for estimating the LFL of the gas mixtures reported here, although the LeChatelier Method was usually more conservative. The Group Factor Method predicted higher values (nonconservative) for the LFLs of several mixtures than were experimentally measured. For the case of a mixture of hydrogen and carbon tetrachloride, the Group Method estimation of the LFL was seriously in error.

"The fire boss in coal mining and the shift boss in metal mining, though held responsible to a large extent for safety of underground employees and of the mine, are, it seems to me, not always vested with sufficient authority nor given compensation commensurate with their duties. The metal-mine shift boss, although his position as regards his relations between employer and employee, is frequently an unenviable one, has as a rule much more authority than the fire boss and is not expected to perform the menial tasks required of the latter by many coal-mining companies.In most coal-mining States the coal-mine foreman and the fire boss are re¬quired to possess a certificate of competency obtained by passing an examination. The foreman is legally responsible for the safety of the miners, as in most instances the superintendent is not required to pass an examination. Although the foreman is held legally responsible for the safety of the miners, the law does not, in general, compel the superintendent or other higher officials to delegate sufficient authority to the foreman to enable him to put his wishes or judgment into effect; and this condition has tended in many instances towards perpetuation of unsafe conditions.The fire boss is held legally responsible for conditions in his ""beat"", but he is generally given only such authority as the mine foreman sees fit to delegate, frequently very little, Moreover, the fire boss is generally required to cover his ""beat"" within two hours before the entrance of the underground workers. Only too often his territory is so great that in order to reach all the working faces and chalk the required date at the face, he must travel at a rapid walk or even run. When such haste is necessary to cover the required ground, it is manifestly im¬possible for the fire boss to make a careful inspection or examination of the roof, timbering, wiring, gas, etc., at each place, and doubtless in his hurry many unsafe conditions escape his notice."

Fatal accidents at coal mines throughout the United States numbered 170 during May , 1923 , according to reports received by the Bureau of Mines of the Department of the Interior from State mine inspectors . The production of coal during the month was 54,649,000 tons ; hence , the fatality rate was 3.11 per million tons . In May last year the fatality rate was 4.22 , based upon an output of 20,636,000 tons , including 35,000 tons of anthracite . Because of the miners ' suspension of work last year, the production of fresh-mined anthracite in May 1922 was completely stopped ; in May 1923 the anthracite output was 9,573,000 tons . For bituminous mines alone , the fatality rate in May 1923 was 2.67 per million tons as compared with 4.17 in May a year ago . During the first five months of the present year , the fatality rate for bituminous mines was 3.55 per million tons , based on 809 deaths , as against a rate of 3.70 for the corresponding months last year, based on 611 deaths . The rate for 1923 represents a reduction of 4 per cent . For anthracite mines alone the five-month fatality rate for 1923 was 5.36 per million tons , based on 228 fatalities , as against a rate of 6.72 for the first five months of 1922 based on 147 fatalities . The rate for 1923 represents a reduction of 20 per cent . The average fatality rate for May during the past ten years ( 1913-1922 ) is 3.37 per million tons for bituminous coal mines and 6.02 per million tons for anthracite mines . A gas explosion , due to open light , occurred on May 6 at Aguila , Colorado , and resulted in the death of ten men . This explosion brought the number of "major disasters " in 1923 to 4 , with a loss of 145 lives . During the first five months of last year, there were 7 similar disasters with a loss of 82 lives . The year's record to date for all fatalities from all causes shows a reduction in the fatality rates per million tons from haulage accidents and from falls of roof and coal , an increase in explosions of gas and dust and in accidents due to electricity , and no change in the fatality rate from explosives . The reductions in haulage accidents and in falls of roof and coal are particularly gratifying, because these two classes of accidents usually cause about 65 per cent of all fatalities in the coal-mining industry . This report, which updates and supersedes the Office of Surface Mining&apos;s 10-year progress report published in 1987, deals with the accomplishments of the regulatory and abandoned mine land reclamation programs during the first 15 years of the Surface Mining Control and Reclamation Act of 1977 (SMCRA). Part 1 is an illustrated description of on-the-ground accomplishments; Part 2 presents statistical information for the 15-year period. The data in this statistical report are organized in two sections: national statistics and state statistics. The national statistics provide an overview of coal mining and the implementation of SMCRA since 1977. The information reported for the states presents a brief description of the coal reserves in each area, accompanied by detailed statistics and graphs that relate each state to the nation as a whole. The statistics compiled in this report are from the Office of Surface Mining&apos;s files, computer data systems, and annual reports. During the early years of the program, many of the data categories were evolving and changing; therefore, data reported over the 15-year period are not complete for all states for all years, and where data was unavailable or zero, a dash (-) is shown for that item. The data sources used to compile the tables and graphs are listed and described in detail on page 76. Due to the number of requests for this information in the past, statistical information about coal mining and its regulation will be available each year as an appendix to the Office of Surface Mining&apos;s Annual Report to Congress.
