This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. To achieve an efficient separation in the flotation process, valuable particles need to be selectively segregated from the gangue while utilising a minimum number of flotation stages. TodayÆs existing flotation technologies are effective for fine particle size classes; however, limitations due to particle buoyancy and bubble-particle detachment restrict the effectiveness of traditional flotation approaches on coarser particles. As a result, flotation circuits are generally configured to process only fine particle size classes in order to maximise recovery in the primary stages. Eriez has invented a novel flotation technology, the HydroFloat, which overcomes these limitations by carrying-out flotation in a dense, fluidised-bed medium allowing for the recovery of coarse particles (>0.250 mm). Over the last 15 years, this technology has been successfully applied to industrial minerals with several full-scale units installed to recover particles up to and exceeding 3 mm diameter in primary flotation circuits. This paper includes discussions on the history of this technology, process theory, existing applications, and a review of recent sulphide-based laboratory test work conducted for Newcrest mining and others. A new high intensity grinding technology has been developed and is now being commercialized by Hicom International of Australia. This has led to the development and manufacture of Hicom 120 grinding mills which are now being sold internationally for a range of mining industry applications. The first commercial process application has been for the diamond sector, where the ability of the mill to remove host kimberlite rock by attritioning, with no damage to the thereby liberated diamonds, is seen to be a most beneficial attribute. There are, however, many other mineral industry applications for this mill. The design of the Hicom mill is unique. Based on a high speed centrifugal motion of the grinding chamber, very high power intensity is delivered to the grinding process. Most significantly, a high acceleration field, 50 times stronger than gravity is produced, creating very high power input per unit of mill volume, and exceptionally fast particle breakage rates, typically 50 - 100 times greater than those in conventional tumbling mills. A mill developing such forces has significant implications to comminution technology and practice.

A rapid, accurate method for predicting the dew points of gas condensate systems and their subsequent normal and retrograde phase behavior with pressure decline has been developed. The method predicts dewpoint pressure, volume percent liquid, condensate content of produced gas, and the liquid and gas compositions at pressures below the dewpoint pressure. Use of a modified form of the BWR equation of state, which was established by regression calculations on experimental data from several gas condensates, provides depletion behavior predictions that are significantly improved over predictions using K-values based on convergence pressures and Standing's correlation for density. INTRODUCTION Prediction of complex hydrocarbon-mixture thermodynamics is fundamental to reservoir and gas process engineering. In particular, accurate descriptions of phase behavior are needed to plan production operations for gas-condensate reservoirs. Although analysis of reservoir behavior during producing and cycling operations can be made for simple reservoir models, extensions of these studies to more complex reservoir systems are still difficult, both with regard to numerical techniques and the phase behavior of the fluids. This report presents a mathematical method for handling the complex phase relationships encountered in retrograde condensates; it represents an advance in reservoir behavior technology in that it will aid the development of closed-form mathematical representations of total production systems. A typical phase diagram for a complex hydrocarbon mixture is shown in Fig. 1. When the reservoir temperature T, is higher than the critical temperature Tc, and lower than the critical-condensation temperature Tct' the mixture is a gas condensate. Decreasing the pressure on a condensate while maintaining nearly constant temperature will result in two phases forming at the dew point. Further decrease in pressure will cause additional liquids to be formed. This phenomenon is called retrograde condensation. As pressure continues to decrease, normal phase behavior regions are encountered (liquid vaporizes as the pressure decreases) until the low-pressure dew point is encountered. When a gas-condensate reservoir is produced, gas is withdrawn and the volume of fluids in the reservoir remains nearly constant as the pressure declines. Heavier components tend to accumulate in the liquid phase as a result of retrograde condensation, while lighter components tend to concentrate in the vapor phase. Although lifetime estimates for sulfate reduction based on total substrate carbon typically range from 20 to 30 years, field and laboratory experiments showed that rates of sulfate reduction decreased over time as readily available organic material was consumed. When the rate of acid input exceeded the rate of sulfate reduction, alkalinity was removed from the substrate, pH decreased and metals increased. Output pH increased from 4 to over 6 after the system was shut down for several weeks and the rate of acid input was reduced. Successful long term treatment requires a design based on the input pH and the long-term organic decomposition rate.

In 1963, the full-scale engineering study for the development of an iron, steel, and vanadium plant was initiated. This was the birth of Evraz Highveld and its process flow as it is known today. Highveld started out as a steelworks that produced steel and vanadium-bearing slag from a titaniferous magnetite iron ore mined from its own mine site near Steelpoort. The process utilized co-current rotary kilns for prereduction, and submerged-arc furnaces for the production of pig iron with approximately 1.15% vanadium. Because of various technical challenges that the submerged-arc furnaces experienced over the years, a decision was taken in 2004 to convert the furnaces to open slag bath (OSB) operation, in order to enable the maximum production of iron and vanadium while improving the control over operational parameters. The conversion proved to be highly successful and, since 2005, three furnaces have been converted to OSB operation. The fourth furnace will be converted in the first half of 2011. The conversion to OSB furnaces not only yields higher vanadium recovery to the metal at the iron plant, but also improves the overall throughput of the steel plant. This is the result of the higher carbon content of the liquid iron from the OSB furnaces, eliminating the need for anthracite addition to the shaking ladle process. This reduces the blowing time by approximately 60%. The conversion of the furnaces to OSB configuration is seen as an enabling technology change and a prerequisite to the implementation of other plant improvements that are envisaged. The long-term vision of EHSV is to convert five furnaces to OSB mode, and permanently decommission the two remaining submerged-arc furnaces. This will allow EHSV to maintain current steel and vanadium production but at a significantly improved overall efficiency.

Isotopes and hydrochemistry were used to define groundwater flow systems and better understand the hydrogeological setting of the Karijini National Park, which is in the Central Pilbara region adjacent to the Marandoo iron ore mine. Based on the stable isotope composition of the water samples, the data can be divided into two main groups. Groundwater is characterised by depleted dD and d18O, suggesting no significant evaporation effect. Surface water on the other hand is more enriched in dD and d18O due to evaporation. The relatively high concentration of Cl- compared to rainfall and depleted dD and d18O values of groundwater indicate that recharge of the aquifers is occurring during intense rainfall events when rapid infiltration occurs. Evapotranspiration then acts to concentrate ionic species prior to recharge. Relationships between the major ion concentration and catchment area, surficial Tertiary cover and distance between recharge and discharge were identified. The results show that these catchment variables combine to determine unique water chemistry for each sub catchment in Karijini National Park. The TDS concentration of the groundwater in the Marra Mamba Iron Formation that hosts the Marandoo ore body is higher than most of the water bodies surrounding the mining area suggesting significant modification or different recharge mechanisms to that of the Karijini groundwater.

"An EXTENDED ABSTRACT is available for download. A full-length paper was not prepared for this presentation. The formation of orogenic gold deposits is commonly a two-stage process. The first stage involves preconcentration of gold in a suitable source rock, such as carbonaceous black shales, felsic volcaniclastics or komatiitic volcanics. The second stage involves the release of gold (commonly with arsenic and/or tellurium) from the source rocks during metamorphism or magmatic intrusive events, accompanied by the replacement of source rock pyrite by pyrrhotite. Recent research suggests that synsedimentary gold trapped in the structure of diagenetic arsenian pyrite in reduced carbonaceous facies of continental margin sedimentary basins is an ideal source of gold for many orogenic and Carlin-style gold deposits. The conversion of diagenetic arsenian pyrite to pyrrhotite during basin closure and greenschist facies metamorphism is the key process releasing gold and arsenic to the metamorphic fluid, with metal deposition in structural trap sites to form the deposits.CITATION:Large, R R, 2015. Magnesia ramming materials have been widely used in China instead of magnesia bricks as furnace lining materials of ferroalloy refining furnace. The paper introduces the development of the ramming material, recounts its chemical composition, physical property and application characteristics. Because of its high MgO contents and fine sintering property, the ramming furnace bottom has the properties of good refractory, monolithic construction and thermal shock resistance. The changes in mineral composition and furnace tearout characterization show the reasons for the bottom?s long service life. The obvious benefits are widely achieved, the annual saving cost in refractory is about 2 millions US dollars.

Construction of foundations for the Skyway Segment of the San Francisco Oakland Bay Bridge required innovative techniques due to the mass of the structures being inserted and the unique subsurface of the Bay. Twenty of the pier structures required large sheet pile cofferdams. Eight other foundations were installed in open water without cofferdams at the deeper part of the Bay. This narrative will present the initial planning, as well as the stages of installation, for each type of foundation. The first section will discuss foundation constructability modifications made during the initial planning phases of the project. Considerable effort was expended by Caltrans, the owner, T.Y. Lin the designer, and KFM (Kiewit/FCI/Manson) the contractor in modifying the design to assure that required tolerance would be maintained during the installation of the foundations. The final section will describe the steps required for installation including: initial access dredging, installation of cofferdams, preparation required to install the footing boxes, footing box installation, foundation support pile installation, and pile to box connections.

The efficiency of an ore-processing unit depends on the consistency of the characteristics of raw material entering the plant. When the mined ore is highly variable in quality, the only way to ensure consistency is to homogenize the ore prior to feeding to the processing plant. The homogenization can generally be achieved by the bed blending operation. Given that the stockpiling and reclamation processes are very expensive, it is necessary to design the process in such a way as to minimize variabilities of specified properties of raw material. In this paper, for alternative stacking types, optimal stockpile geometry is found in three stages: First, stockpile input is simulated by sequential Gaussian simulation, and then the variance reduction ratios (VRR) as a criterion of stockpile efficiency are calculated for various stockpile geometry scenarios by a stockpile simulator written in FORTRAN. Second, multiple regression analysis is performed to model the VRR by the use of stockpile length, the number of layers and stacker speed as the independent variables. Finally, the model is an optimization problem. Decision variables are the stockpile length, the number of layers, stacker speed and stacking type. The genetic algorithms (GA) are used to minimize the VRR. The approach was demonstrated on data from an iron orebody. The problem was to reduce fluctuations of iron, silica, alumina and lime contents in the stockpile output. The results showed that the approach could be used for the bed blending design efficiently. In a previous paper, Oelsen, Schuermann, and Hey-nertl pointed out the possibility of obtaining complete thermodynamic functions for alloy systems from calorimetric measurements alone. Specifically, if the heat content of a heterogeneous, multicomponent system is known as a function of temperature and composition, then it is possible to derive the heats, entropies, and free energies of formation of liquid phases and of solid intermediate phases of fixed composition. The authors present equations and graphical techniques applicable to eutectic systems and systems with intermediate phases without solid solubility at lower temperatures. eijering has considered possible extension of the treatment to systems with complete solid-state miscibility at lower temperatures; however, experimental difficulties practically restrict the method to systems with very limited miscibility in the solid state. The purpose of this note is to present an alternative derivation of the heats and entropies of formation of liquid phases and of solid intermediate phases from heat-content measurements alone. Fig. 1 shows three types of phase diagram to which the analysis may be applied. The Bureau of Mines has developed a passive encoder for range knobs, a device that automates the task of obtaining the range settings from devices or instruments to be input to microcomputers. It is easily constructed and installed and will not disrupt the device to which it is being attached. It consists of a custom knob and a small printed circuit board that has light-emitting diodes and magnetic reed switches attached. A ribbon cable connects it to a microcomputer. This encoder eliminates one source of human error in an automated process.

I NT RODUCTION Theory indicates that linear water-floods should exhibit scaling and stabilization properties in both oil-wet and water-wet porous media'. Experimental verification of these proper-ties in oil-wet media was obtained some time ago', and more recently, Perkins', employing a high permeability unconsoli dated sand and a low oil-to-water viscosity ratio, has obtained flooding data in a water-wet medium which follow the pattern of scaling and stabilization. Also, Root and Calhoun3 have found similar trends in the displacement of gas by liquids from unconsolidated sands. In general, however, the experimen-tal evidence appears still somewhat conflicting since scaling and stabilization of water floods in water-wet media has not been observed in most of the studies to date'.".'. The purpose of this paper is to provide a more comprehensive picture of waterflood behavior in water-wet media and to corroborate the validity of the scaling and stabiliza-tion concepts. The need for differentiating between the intrinsic nature of water-oil displacements inside a porous medium and perturbating secondary end effects is discussed, pertinent experimental procedures are outlined and the results of flooding tests conducted at low and at high oil-to-water viscosity ratios on con- solidated, water-wet Alundum cores are analyzed. END EFFECTS IN WATER-WET MEDIA Outlet End Effects The peculiar feature of the outlet end effect in water-wet cores is that it not only results in an excessive wetting phase saturation at the outflow face,'.' but that it also retards the moment of water breakthrough: i.e., causes an undue delay in the appearance of water in the produced effluent. Conditions existing in a water-wet core when water first arrives at the outflow face are schematically shown in Fig. 1. At this flooding stage the saturation distribution, Fig. la, has not yet been distorted by the outlet end effect. A water-invaded pore at the outlet core face, opening into the oil-filled void space between the end of the core and the end plate of the flooding cell, is illustrated in Fig. lb. The curvature of the water-oil interface is concave toward the outlet, and the pressure on the water side of the interface is lower than on the oil side by an amount equal to a certain capillary pressure (Fig. lc). Before the water (of this particular pore) can be produced from the core, it is apparent that the pressure in the water phase must exceed that in the oil outside the core. Accordingly. when water first reaches the outlet face, it will not leave the core but will accumulate inside, and only oil will be produced. Eventually, as the water injection is continued. sufficient pressure is built up in the water to reverse the curvature of the interfaces in the outlet pores, and only then will water be produced from the core. This stage, illustrated in Fig. 2. corresponds to the conventional "water breakthrough" observed in flooding experiments. For purposes of discussion, it is convenient to distinguish between the flooding stage at which water first arrives at the outlet face of a core (Fig. 1) and the stage at which water is first produced from the core ( Fig. 2). Hatworth Colliery, operated by RJB Mining (UK) Ltd, is successfully mining the Deep Soft Seam at a depth of 1000m using single face retreat longwalling. The seam is characterised by a weak roof generally comprising 4 to 6m of well bedded carbonaceous mudstones overlain by weak siltstones. The paper describes the layout and support design strategies which have enabled the colliery to adopt sole support by rock reinforcement techniques for its longwall gateroads. To achieve this success the colliery layout has had to be radically modified to take account of the in-situ horizontal stress field. The layout is also closely controlled by environmental considerations, the seam being one of the gassiest in the UK Economic support by strata reinforcement has been achieved using a combination of rockbolting of the roof and sides installed at the face of the heading, and systematic double birdcage cable bolting installed on drivage and supplemented ahead of the retreating panel. The layout and support strategies have been developed through a combination of detailed underground measurement of support and stress parameters backed up by numerical modelling using FLAC. The FLAC modelling has paid particular attention to careful representation of bedding properties and validation through underground stress measurement.

Technically based guidelines for managing rock related risks associated with strip mines in South African collieries are currently vague and are to a large extent based on experience and trial and error. To date, no mine specific or universal design charts exist within the South African industry. Quantification is done on a reactive basis i.e. following reported instabilities. The conditions under which, proactive tools to manage instabilities would be practically applicable are: The design of simple easy to use charts. Designing the charts for a target audience that is primarily production management. Well organized, logical set-up of the charts that will allow for easy future refinements. Allowing for variations in rock and soil properties within the charts that would account for colliery specific and region specific variability. The factors selected for in depth investigation that were deemed to have either individually or in combination, a primary influence on the stability of strip mines are (in no order of priority): Appropriate batter angles for ?soft? materials. Surcharge loading (spoils, dumps, topographical highs) on the highwall side. Influence of the thickness of stratification on stability. Influence of moisture content on stability. Influence of the changes to the coal seam dip angle on stability. Influence of persistent ?strip long axis? parallel faults/joints on stability. Spoil stability for variations in material type. This paper provides details of the work undertaken, using principally numerical modelling techniques, to produce design charts that will allow for proactive management of rock-related instabilities on strip mines.

The 12th and 14th Street Viaducts, which carry Route 139, are located west of the Holland Tunnel in Jersey City, New Jersey. The two viaducts, provide access to and from the Holland Tunnel (which connects New York City and New Jersey) and carry upwards of 50,0000 vehicles per day from Jersey City and New York City to points west. The New Jersey Department of Transportation (NJDOT) selected AECOM to perform design services including rehabilitation and seismic retrofit for the 12th Street and 14th Street Viaducts while maintaining traffic at all times. Building a new parallel bridge just north of the 14th Street Viaduct was essential in avoiding extremely costly traffic delays. Installation of the new foundation, in close proximity to the existing viaduct, required drilling through some of the existing timber piles supporting the bridge. As a result, the existing bridge was required to be reanalyzed for this new foundation configuration. This proposed structure consisted of 8 piers each supported on a single six (6) foot diameter drilled shaft. Currently this portion of the bridge structure has been constructed. The shafts were constructed with rock sockets that were five feet six inches (5?- 6??) in diameter and were extended 11.2 feet (3.4 meters) into rock. A 2300 ton Osterberg Load Test was performed on one production shaft to confirm the capacity of the shaft. The rock socket for the test shaft was in sandstone. This paper will present a description of the design methodology and construction methods adopted for the foundations. In addition, the paper will illustrate the Osterberg Load Test including: equipment used, number and locations of strain gages, and interpretation of results. Additionally, regarding the close proximity of the new foundation, the paper will present the type of equipment used to drill through the existing timber pile foundation while minimizing the impact on the integrity of the existing foundation. We will also present the method of analysis for analyzing the redistribution of load on the remaining piles. The results presented in this paper will assist the design engineer by providing a comparison between the geotechnical design assumptions and the actual measurements obtained by field measurements for this rock formation.

With the passage of the Surface Mining Control and Reclamation Act (SMCRA), and the implementation of state permanent programs, underground coal mining operations are for the first time subject to government regulations regarding subsidence. These regulations now and in the future will directly affect the manner in which mining companies mine their coal. Since the promulgation of the original final regulations by OSM, numerous changes in those regulations have been made and many additional changes are pending in the near future. This paper will discuss state and federal approaches to regulating underground coal mines and examine issues surrounding future subsidence regulations.

This research report was produced in response to the release of the Mineral and Petroleum Royalty Bill. It involved are view of the mineral investment environments of selected countries in order to form an opinion on the appropriateness and competitiveness of the proposed royalty structure for South Africa. Many emerging countries have recently attracted a significant share of foreign investment, as a direct result of changes in policy. These are, by implication, the least risky and most favourable destinations for the ?investment dollar?. The five most competitive countries, namely Chile, Argentina, Peru, Mexico and Brazil have been identified as succeeding with their policy visions of creating favourable domestic investment environments. The investment rules of these countries served as a basis for the compilation of a Competitive Investment Framework (CIF). This is a template of best practice against which other investment environments, such as that of South Africa, can be measured. The framework indicates that mineral royalties should ideally be no more than three per cent, demonstrating that the regime proposed in the Royalty Bill is out of step with international best practice. Mineral royalties can never be viewed in isolation and in the assessment of the rates proposed in the Royalty Bill, one must consider the total taxation package. Such assessment appears in Section 5 of this report, demonstrating that there is reason for concern if National Treasury were to impose the rates stipulated in the Royalty Bill. The alternatives, conclusion and recommendations of the review appear in Sections 9 and 10. These are the result of independent analysis and assessment of the fundamental principles. It is done through the application of a Competitive Investment Framework that avoids the highly subjective and emotional views that often accompany a process like this.

This circular is one of a series published by the Bureau of Mines describing mining and beneficiation methods and costs at various mining operations throughout the United States. Inclusion of the Abbott mine in this series is of particular interest, as the complex geology and the character of the deposits make their exploitation a unique operation. The mine operates on a high-cost, comparatively small-scale basis, and only the persistence of an aggressive management has enabled it to maintain its rank among the major mercury producers of California. Rising costs and decreasing metal prices, which continuously threaten the low profit margin, are reflected in increased efficiency of operation. This report summarizes past operations and describes, in detail, the geology and ore deposits; exploration and mining methods; and furnacing and condensing operations. It covers the entire operation from initial exploration to the actual production of mercury. Comparative production costs for 1944 and 1958 are of special interest.
