Over the period 2000 to 2003, roof falls have accounted for 4 to 14% of the fatalities in underground mining operations. The National Institute for Occupational Safety and Health (NIOSH) is conducting research to reduce the frequency, exposure, and risk of these events through an ongoing program of field and laboratory studies. One area of research involves the evaluation of polyurethane grouting technology that is commonly used to stabilize fractured mine roof strata. Concurrently, NIOSH is conducting work to evaluate the application of ground penetrating radar for advanced delineation of problematic mining areas. In this study, NIOSH partnered with Sub-Technical, Inc. who injected a polyurethane grout (also known as glue) into a roof area of NIOSH's Safety Research Coal Mine. The mine roof area was scanned using ground penetrating radar technology before and after grout injection in an attempt to determine the extent of grout infiltration. A comparison of the pre-and post-grouting radar records showed a significant change at a mine roof depth of about four to five feet. The interpreted radar records were then compared with drill core information, borescope evaluation of roof-bolt holes in the study area, and underground observations. At this site, the interpretations of the radar records correlated with data obtained from the core holes, borescope evaluations and underground observations. This study showed that ground penetrating radar technology can be a useful tool for detecting changes in mine roof due to the injection of the grout.

Agnico-Eagle started sinking the Penna Shaft in 1995 with a hoisting plant capable of hoisting 1800 t/d from a 2240 m deep shaft. Exploration success led to the installation of a hoisting plant capable of extracting 4500 t of ore and 1800 t of waste per day from a depth of 2210 m. The introduction of a new rope selection factor, permitting a safety factor of 4.0, represented the breakthrough required for a cost-effective project. This paper describes the process taken to introduce the new rope selection factor and its impact on the original plant.

An innovative process to treat transition metal oxide waste materials from diverse sources is presented. This technology involves using reverse-polarity DC plasma driven electrolysis in molten oxides. The process results in the recovery of the transition metal and other heavy metals from waste. By-products generated throughout the processing was converted to environmentally acceptable slag. The significance of this technology is that the treatment of the waste is accomplished in one step which in turn lowers the cost of remediation. Moreover, this process uses high temperature properties of thermal plasmas, which makes it possible to use molten oxides instead of halides as electrolytes. Chromium was successfully reduced from chromium oxide through the plasma driven electro-reduction process. It was shown that the selection of the appropriate ionic conducting oxides was critical for the formation of a slag that would enhance the reduction of chromium oxide.

"A new office complex is being constructed within 25m of the Hudson River in New Jersey. The project, about 140m by 100m, will include 4 basements under the entire footprint. The perimeter was to be secured by a 0.8m thick diaphragm wall, laterally supported by high capacity tieback anchors to rock. Columns were to be supported on footings or piers to rock at a nominal bearing pressure of 3.8MPa at depths up to 17m. Although some variation in rock quality was observed in borings, a substantial vertical seam of weathered and/or decomposed rock, 1 to 2 meters wide by as much as 8 meters deep, was encountered during construction necessitating field redesign. In some locations, the seam dipped below rock that is more competent. Footings were redesigned for support on drilled caissons, or soft rock was excavated and replaced with concrete necessitating a reduction in bearing pressure of the footings by one half. The tower covering the south half of the site was topped out in 2002 and the garage levels in the north half of the site were completed in 2003.PROJECT DESCRIPTIONIn September 1999, a major investment company began the due diligence process to evaluate a property on the shore of the Hudson River in Jersey City, New Jersey for a new multi-use complex. The site offers a spectacular view of lower Manhattan directly across the river from the site of the former World Trade Center. The site, covering two city blocks, was previously occupied by manufacturing and office facilities of Colgate-Palmolive.The building footprint is approximately 140m by 100m, the southern half of which is occupied by a 260m tall office tower, shown in Fig. 1. As of this writing, design of the structure for the northern half of the site has not been finalized, but it is expected to comprise an 11-story office building. A glass-covered atrium will occupy the street that formerly separated the two blocks. As part of the due diligence process and preliminary design evaluation, alternatives were considered to accommodate various numbers of parking places, both above and below existing grade. A major consideration for below grade parking is the proximity of the Hudson River some 25m east of the east building line."

An open pit a mine can be thought of as a business unit, with capital, operating and maintenance costs that exceed those of many large business enterprises. Truck-and-shovel operations have an inherent “looseness” to them, unlike manufacturing or processing operations where production components are tightly coupled (through conveyors, material flow in pipes, assembly lines, and so on). Effective monitoring and control of truck-and-shovel operations requires a combination of both information systems and performance management. Production monitoring systems can impact operations at three levels: minute-byminute decisions during the shift, procedures, and policies. The technology of production monitoring ranges from real-time data collection to manual entry of time card data into a computer. Time card information is too coarse to effectively manage a multi-million dollar fleet of mobile equipment, so a higher level of production monitoring is required. People issues affect whether the implementation will be a success or a disappointment. Considerations that affect the people issues are discussed, including common obstacles, and key success factors. A balanced scorecard approach must be used, since uni-dimensional measures of performance like tons produced give no indication of whether the production was achieved at the expense of another factor.

At the 25th Annual B.C. Mine Reclamation Symposium in September 2001, Huckleberry Mines Limited received the Jake MacDonald Reclamation Award for its exemplary mine waste management plan, conceived to prevent the onset of acid rock drainage (ARD) and facilitate reclamation of the site. The mine is an excellent example of a total mine waste management plan, with all materials scheduled in detail for the life of the mine. The filling plan for the waste management facility takes into consideration the requirement for permanently flooding the potentially acid-generating rock. From an acid generation point of view, the tailings fall in the zone of potentially acid-generating material. To avoid long-term ARD concerns, it is planned to float pyrite out of the tailings toward the end of the mine life, thereby generating a non-potentially acid-generating tailings stream which will be used to cap the deposited mine waste. The pyrite-rich tailings will be discharged into the deeper portion of the impoundment. On closure, a permanent water cover will be maintained over the mine waste. Only beaches of non-potentially acid-generating tailings will remain exposed. Mining will be phased to also allow one of the pits to provide storage for potentially acid-generating rock and to be flooded at the end of mine life.

Slag from stainless steelmaking is known to disintegrate, which is an environmental problem and prevents the cooled slag from being utilized. This disintegration is due to the transformation from the ßto the ? polymorph of di-calcium silicate. The transformation is accompanied by an increase in volume of about 11% and a total disintegration of the slag. AOD slag from stainless steelmaking was re-melted in a laboratory induction furnace, together with olivine and calcinated dolomite, to investigate the prevention of disintegration by increased MgO content of the slag with small changes in CaO to SiO2ratio. The modified slag was examined with X-ray diffraction and scanning electron microscopy. The trials showed that an increased MgO content can change the mineral composition towards formation of more Merwinite and Bredigite, which prevents disintegration of the slag. Highwall mining is an important coal mining method. Upwards of 60 highwall miners are presently in operation, and they may account for approximately 4% of total U.S. coal production. A review of the Mine Safety and Health Administration (MSHA) accident data over the 20 year period from 1983 to 2002 identified 9 fatalities attributable to auger and highwall mining of which inadequate ground control accounted for 1/3. In the past 5 years, 1 fatality occurred in highwall mining. Estimates of the manpower requirements in highwall mining suggest that its fatality rate is essentially the same as for surface coal mining, thus highwall mining appears to be a very safe modern mining method. Highwall stability is the major ground control related safety concern, and operators are required to develop and follow an appropriate highwall mining ground control plan. The plans usually specify the following geotechnical parameters: hole width, maximum hole depth, maximum overburden depth, seam thickness, web pillar width, barrier pillar width and number of holes between barriers. Calculated web pillar stability factor exceeded 1.3 for most designs evaluated. This study examined records from 5,289 highwail miner holes with a total completed footage of about 2,560,000 feet to understand the reasons for early pull out. Average lost footage is typically about 20% of planned footage. Only 35% of the holes reached planned depth, and 20% were short due to rockfalls. Water and adverse geology accounted for 15% of the losses. Mechanical/electrical problems, guidance, and slope stability problems accounted for the remaining 30%. Web pillar stability factor for these holes also exceeded 1.3 in 95% of cases. Trapped highwall miners present a safety hazard to workers during their recovery. Best practices to avoid trapped highwall miners include: avoid mining in stream valleys, avoid mining near outside corners, careful alignment of each hole and the use of an onboard guidance system, Several issues in highwall mining ground control require further investigation including highwall mining through old auger workings, highwall mining near old underground mines, multiple-seam and multiple lift highwall mining and finally the size and frequency of barrier pillars.

A mine’s inherent capacity is dictated by factors such as equipment, design, people, processes, and environment. Actual production may approach, but never surpass, the inherent capacity limit. The inherent capacity ceiling can only be increased through re-engineering. Autonomous haulage systems have the potential to significantly increase a mine’s inherent and realized capacity. Realized capacity may be maximized through the optimization of soft factors such as the processes, behaviours, and organizational systems, controlling asset utilization, availability, and performance. Truck shovel dispatching systems revolutionized the utilization of mobile assets. High-precision global positioning systems (HP-GPS) are increasingly being adopted to optimize equipment performance. Real-time maintenance management systems have the potential to revolutionize equipment availability and significantly reduce cash costs. This paper explores some of the solutions available to mines to increase their inherent capacity and maximize their realized capacity.

The bulk of plain carbon steel is produced through conventional Basic Oxygen or Electric Arc Furnace routes where the development of the AC Ladle Furnace has made it possible to also produce alloy steels. Smaller Ladle Furnaces have limitations such as lower efficiency; high electrode consumption and diminutive yields rendering them uneconomical to operate and making the processing of low carbon steel grades difficult due to carbon pick up. The demand for special steels and alloys with strict specifications is increasing while commodity prices remain competitive. The production of special steels and alloys have specific process control requirements and the development of the induction melting furnace (IMF) has made it possible to produce smaller batches economically. Refining and alloying of the melt in the IMF further enhance the efficiency. The INDUREF furnace offers increased productivity and flexibility through increased thermal efficiency and can be utilised for holding molten metal for further processing such as granulating, atomising, nitriding, alloy additions and desulphurisation of Ferro-alloys and steels. The system utilises a specially engineered ladle which is placed in an induction-heating assembly comprised of a stationary water-cooled copper coil and lamination packs. The addition of equipment for vacuum degassing and decarburisation of steels and alloys is also feasible. Benefits are: ? No electrode consumption or carbon pick-up ? "Environmental desulphurisation is an attractive alternative for the management of acid generating tailings. Non selective froth flotation is the most commonly used method to produce desulphurized tailings, as shown from many previous works (McLaughlin and Stuparyk, 1994; Bussière et al., 1995; 1998; Benzaazoua et al., 2000a,b; 2002). This process can reduce the volume of problematic tailings to manage, by making a sulphide fraction concentrate. The desulphurized tailings can then be used as a moisture retention layer in a cover with capillary barrier effects to prevent acid mine drainage.In this paper, the authors present the results from different laboratory and pilot scale flotation tests which confirm that low sulphide, non-acid generating tailings can be produced by froth flotation. It is also demonstrated that the sulphide concentrates obtained from desulphurisation, when mixed with binders gave good long term results in terms of mechanical behaviour, thus demonstrating that the sulphide fraction can be used for paste backfill material. From previous work (Bussière et al. 1998, 1999) on the evaluation of the operating and capital costs of the desulphurisation process for different ore types, the use of desulphurized tailings to create a dry cover was then studied for several scenarios. The desulphurisation process was compared with other technologies (underwater disposal, engineered soil cover) currently used to prevent acidic drainage (AD) for a new mining operation and the advantages and disadvantages of desulphurisation are presented as well as the costs of this method of AD prevention.INTRODUCTIONThe principal environmental problem of the mining industry today is the generation of acidic drainage (AD) resulting from the natural oxidation of the sulphides in mine tailings. This phenomenon involves the generation of acidity which increase the amounts of toxic metals in the waste water released into the environment by a mine. The environmental impact of the AD has been described in detail in previous work (e.g. Down et Stocks, 1977; Ritcey, 1989; Gray, 1997; Ripley et al., 1996; Aubertin et al., 2002)."

A thermodynamic model and computer program, CuModel, has been developed to model the distribution behaviors of Cu, Fe, S, O, Ni, Co, Sn, Pb, Zn, As, Sb, Bi, Au and Ag, copper loss in the slag, and heat balance in copper smelting process, matte converting process and copper con-tinuous smelting process. It can be predicted for any set of controllable process parameters such as feed composition, smelting temperature, degree of oxygen enrichment and volume of oxy-gen-enriched air. This model accounts for physical entrainment between matte and slag phases. It can be used in Isasmelt process, copper flash smelting process, Mitsubishi process, P-S con-verting process and Noranda process. The predictions by the present computer model are com-pared with the known commercial data from Guixi Smelter in China, PASAR Corporation in Philippines, Horne Smelter in Canada, MIM Limited in Australia and Naoshima Smelter in Ja-pan. The agreements between the computer predictions and the commercial data are excellent. In this paper, the applications of CuModel in the copper metallurgical industries have been pre-sented. CuModel has been used by Daye Smelter and Guixi Smelter in China, and by PASAR in Philippines as well.

Sulphate decomposition was studied in the conditions of a typical copper flash smelting heat recovery boiler. The aim was to determine the kinetics of the copper sulphate decomposition reactions and to find out if they are likely to take place in the process conditions prevailing in an industrial HRB. The information is essential in CFD-modelling of the reactions taking place in the boiler and for optimising the process. Thermal decomposition of sulphates was studied by thermogravimetry. The experiments were conducted with a commercial copper sulphate, a partially sulphated copper oxide and an actual process flue dust. The main variables studied were temperature and gas composition of a N2-O2-SO2-mixture. The samples were characterized with SEM, optical microscope and XRD. Temperature affected the decomposition rate strongly but the gas composition had a minor effect on the kinetics. The zinc pressure leaching (ZPL) plant at Teck Cominco's Trail Operations was started up in 1981 and represented the world's first commercialization of the technology. From the outset, the process was designed to operate using iron-rich Sullivan Mine zinc concentrate. When the Sullivan Mine reached the end of its life in 2002, the ZPL plant underwent a significant process change with the treatment of 100% Red Dog zinc concentrate. Through careful improvements to existing plant processes, controlled changes to plant operating parameters and a rapid adjustment to the modified process by operations personnel, the plant has been successfully converted to processing a feed of 100% Red Dog zinc concentrate. This paper discusses the challenges faced during the conversion and describes the modifications to plant operating procedures, as well as changes to plant processes.

The intent of this manual is to provide guidelines for inspectors of drilled shaft construction otherwise referred to as drilled piers, caissons or bored piles. It should also be helpful to train beginning geotechnical engineers with limited first hand experience or for geotechnical engineers who are training inspectors. It is expected that the general guidelines and procedures described here will be supplemented by local geotechnical engineers working in a particular region based on local practice and experience. Since drilled shaft construction procedures vary widely based on local geology and size of project, an all-inclusive manual is not considered practical. This manual focuses on traditional topside inspection for routine drilled shaft construction. Other inspection and evaluation methods that rely on electronic equipment are available, but these methods require extensive training and experience in the interpretation of results. These more sophisticated methods of drilled shaft inspection and evaluation are common in construction, but they are usually provided by specialty subconsultants. The subconsultant will typically submit a report for each shaft, which will be reviewed by the geotechnical engineer. Other manuals are available to describe the electronic methods of drilled shaft inspection.

In the construction field and to a lesser degree in mining, flagman are often necessary to move traffic and or pedestrians through work zones. Everyone readily recognizes that they are essential for the safety of the general public and the employees on the job. What is not often considered is the safety of the flagmen themselves. Actually, let’s call these employees “flaggers” recognizing the fact that there are as many women as men doing this job. The first problem we need to overcome is the misperception that flagging is a low skill function that can be done by any untrained employee. The reality is that the flagger works in a very hazardous position with serious responsibility for the safety of his fellow employees and the public at large. And to do this job properly re q u i re s that the flagger receive sufficient training before being sent out to the highway. It is also necessary to convince the flagger of the value and importance of their job in order to motivate them to do a professional job.testing, documentation of both, and accurate re c o rdkeeping are key aspects of HM126F. Of course, training and documentation requirements will vary depending on the type of operation involved, and there may be some differences in the application of requirements depending on your jursdiction. Nonetheless, failure to train and properly document such training can result in heavy fines and/or penalties.

Whatever system is devised to control noise and ground vibration from blasting operations must present an effective compromise between the rights of the individual to be undisturbed by the actions of others, and the necessity for society, the whole of the population individually and collectively, to have the services and the products of commerce, industry and transportation along with the relaxation of recreational and cultural pursuits, all at a price within the means of the same society. One of the first jurisdictions in North America to attempt to quantify acceptable environmental limits for specific blasting operations was Ontario, Canada’s largest and most populous province. A process for development of guidelines applicable only to quarries and strip-mining operations was begun in 1974 and culminated with their inclusion in a “Model By-Law” published by the Ontario Ministry of the Environment in August 1978. This paper examines the process of development of the final guideline document and its application over the last 25 years. While acknowledging that the published guidelines for air blast and ground vibration control have provided a useful and generally workable framework for the purposes of land use planning and complaint or damage claim reduction, a number of flaws, weaknesses and misinterpretations connected with their application are identified. Recommendations for amendment of the controversial items are submitted with a view to making these and potentially similar guidelines in other jurisdictions, more acceptable to both government and industry.

The Meikle Mine is a mature underground gold mine utilizing primary and secondary longhole open stoping method with delayed backfill. Combined production with the newly developed Rodeo Mine is 4700 tons per day (4265 tonnes per day) of ore. The vast majority of Meikle Mine production comes from secondary and remnant stope blocks. Remnant stopes are ore blocks that were left behind in the initial mining sequence due to various reasons. Mining of these remnant blocks has created many challenging blasting situations. The application of electronic initiation to blast remnant stopes has proven extremely beneficial. Electronic initiation has provided the blasting engineer with a high degree of flexibility with respect to timing to successfully extract these difficult stopes. Post blast assessment of the remnant stopes using the mine’s cavity monitoring system (CMS) demonstrates that the mine is maximizing ore recovery and minimizing waste dilution. The largest remnant stope blast to date required 54 holes and produced 6700 tons (6075 tonnes) of ore. Another area where the application of electronic detonators has proved successful is in the extraction of slots in which ground conditions are extremely poor. Taking the slot blasts in one shot with double-decked holes as opposed to two single-decked shots has eliminated the need to clean and or re-drill holes. The ability to open the slots in one blast has helped reduce the total mining cost. This concept has recently been expanded to blast complete stopes in one shot. This reduces the stope cycle time considerably and allows the mine to bring ore forward. Current plans for 2004 are to excavate 50% of mine wide stope production using electronic initiation technology.

Good Morning. Thank you for your kind welcome, Lyle and thank you for having me here today. I spoke at the fall meeting of the Utah state RMCMI in Price and it's nice to be invited back. I can see by the program that you have two great days of interesting and informative meetings, including the mine rescue team exhibition and the safety recognition award presentations. As you all know, I am here standing in for Dave Lauriski, who is in China with Secretary of Labor Chao. As you also know, Utah is near and dear to his heart - and so is the Rocky Mountain Coal Mining Institute. I want to assure you Dave would be here if it were at all possible. But it is my good fortune that I could come in his place. We have made great progress as an Agency thanks to the hard work and dedication of MSHA's employees. But we are always looking for new ways to solve problems and continue to send more miners home safe and healthy every day. And we have been working on some great ways to do that, including thinking strategically. During my presentation today I'm going to talk about where the Agency is in terms of our transformation to the MSHA of the 21st century, a little bit about how we got to be where we are, and then describe our roadmap to the future state. And I'm going to focus on the significance of our plan and the planning process to demonstrate the implication for the mining industry. Former President Dwight Eisenhower once said, "In preparing for battle I have always found that plans are useless, but planning is indispensable." Now I do like Ike, but I would have to take exception to the first part of the sentence. I have found plans and the planning process to be indispensable - the plan to give me a roadmap to my goal, and the process to focus me on the steps I must take to get there. We all know that having a strategic plan makes the difference between success and failure. We've seen it time and time again, in business, in our lives - and in government: Having a strategic plan is crucial to success. And the process of making the strategic plan is crucial to its success. Putting together a workable strategic plan - a good roadmap - that will successfully lead you to your goals requires a thorough, careful process that takes into account each factor that you may encounter along your way to success, as well as those with whom you will travel. In order to talk about our current plan I want to take you back to three years ago, when Dave Lauriski went to Washington to lead MSHA. He knew that change was in the air.
