During the response to the Moura No 4 disaster, use was made of the New South Wales Mines Rescue Service’s ‘Mineshield’ inertisation equipment. The equipment had to be transported from Newcastle, approximately 1400 kms, and the liquid nitrogen a similar distance. The time taken, and therefore availability of the liquid nitrogen, became a significant problem. Operational issues were also experienced during use. Typically the liquid nitrogen is converted to a gas (vaporised) before being injected into the mine. However, as the propane gas tanker used for the vapourising unit was delayed, an attempt was made to inject the liquid nitrogen directly down uncased boreholes. This proved unsuccessful.Following the Moura No 4 disaster and the Moura No 2 disaster, recommendations were made relating to having inertisation capacity for the Queensland coal mining industry. Today the Queensland industry has multiple options available for inertisation, including the high-volume GAG and smaller volume units such as boilers and membrane separation units. Although it ultimately aided re-entry into Moura No 4, most likely due to the issues associated with supply to Central Queensland, liquid nitrogen has not found favour within the Queensland industry as an inertisation tool.Coregas has commissioned an air separation plant in Mackay, improving the availability of liquid nitrogen to Central Queensland. Based on the availability of this nitrogen, recent sealing operations at Kestrel have trialled using liquid nitrogen and associated vapourising units. The South Nottinghamshire Area has not been greatly troubled by spontaneous combustion until the last few years, during which the Blackshale Seam has been developed and exploited. The problem was first encountered at a colliery which was heavily preoccupied with a major reconstruction scheme, and with the sudden appearance of a completely new hazard both men and management had to adapt themselves very quickly to meet the situation. The paper contributed to the Symposium by the South Nottinghamshire Area illustrates the problems by reference to actual incidents, and shows how the lessons learned from them have been applied to the shaping of future policy on the avoidance and treatment of spontaneous combustion.

Solution mining, primarily by heap leaching, has emerged as an important method for mining gold and silver in the United States. Heap leaching offers many economic and environmental advantages over conventional production techniques, in particular for lean ores and old mine waste rock. In order to more effectively pursue research on heap, dump, and in situ solution mining of gold and silver, the Federal Bureau of Mines conducted an extensive survey of leaching practices and problems. This presentation summarizes trends and statistics. Leaching is projected to provide 20 pct of primary mine produced gold and 10 pct of primary silver by the end of 1982. Current leaching practices in the United States are reviewed; operational problems and research programs to alleviate those problems are discussed.

In response to recent mining disasters and new mine rescue team legislation, NIOSH researchers conducted meetings across the United States with mine emergency response experts to investigate current needs and issues. Some of the issues include emergency response preparedness, mine rescue contests, real-life training capabilities, and training facilities. Many new teams are being formed that must be trained for mine rescue team competitions as well as be ready to respond to a variety of mine emergencies including a fire or explosion, a massive roof collapse, mine inundations, or vertical shaft rescue situations. Therefore, it is important that these teams are adequately prepared and trained. This paper presents a summary of domestic and international coal mine rescue training facilities and identifies those that provide unique, real-life, and/or state-of-the-art training. Research findings from this report will be used in further NIOSH investigations to improve coal mine rescue training. "Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

It is challenging to determine the optimal mining strategy to maximise value when a mine plan includes mineralised material which does not meet the full cost of mining but has value exceeding the variable cost of mining (‘marginal material’). Mining too much marginal material dilutes the operation’s head grade and undermines profitability. Conversely, leaving too much metal behind fails to achieve the full value of the endowment. Finding the balance between the two will maximise value delivered from the asset. This dilemma has traditionally been approached by calculating a suite of cut-off grades to define the grade of material that meets economic targets for an orebody (or zone within) and should therefore be included in the mine plan. A problem with this approach is that individual mining blocks within the zone have different costs, value and confidence levels, with each having economic co-dependencies between them. Blocks of identical grade but different mining contexts will have distinctly different economic outcomes. For example: • a block may be a low-grade increment or shell around a higher-grade stope; • a block may be available to be mined when there is spare mining capacity; or • a block may defer access to significantly higher cash flow material. Each of these circumstances would result in different value from the blocks if included as part of the mine plan. Cut-off grades calculated as averages over zones of material, or in isolation from schedule-based information, are missing the entire mining context that determines the ultimate economic outcome of making a mining decision. There are tools and techniques available to address this for open pit mining, however underground mining has different nuances. There are more intricate economic and physical dependencies between potential ore blocks, and importantly, the decision to leave material in situ can be made at any time. The case study in this paper explores a new methodology for underground mining, targeted towards strategic planning applications. It examines whether the economic logic usually applied via cut-off calculations can be implemented in the schedule so that optimal economic decisions can be made on a block-by-block basis, taking into account the mining context of the block, with the objective to maximise asset value. The results of the case study scenarios in Table 1 demonstrate the magnitude of the potential improvement in NPV that can be attained by a block by block valuation approach

To reduce the number of injuries resulting from fires on mobile, diesel-powered mine equipment, it is crucial to promptly suppress a fire once it is detected. The focus of this research was to determine the effectiveness of fire suppression agents. Large-scale tests were conducted using five different fire suppression systems based on: dry chemical, wet chemical, dual agent (dry and wet chemical), carbon dioxide, and water mist. Suppression nozzles were placed around the diesel engine where diesel fuel, engine motor oil, and hydraulic circulating oil spray fires were ignited. The results of this study can help mining companies and manufacturers by providing scientifically based data on the capabilities of the different fire suppression systems.

The treatment of iron-hydroxide-containing water from ground water and surface water is achieved by a combination of the process steps of flocculation and subsequent mechanical dewatering. The aim is to create solid pellet structures by extending the structure forming flocculation process with a build-up pelletizing process which can be transferred directly into a drying unit. A Taylor-Couette-reactor (diameter of the inner cylinder – 40 mm, gap width – 5 mm) was used for these experimental studies. To produce the necessary flow profile the inner cylinder was rotated at a speed of 230 rpm. The iron hydroxide-containing suspension (dry matter content: 2 mass %) was added simultaneously with the addition of a cationic high-molecular flocculant in various flocculant dosages and residence times of 5 to 15 min. The pellets were examined for parameters such as geometric dimensions, dry substance content and dewatering behaviour. As the results show, there is a direct relationship between the residence time and the geometric dimensions of the pellets. After a residence time of 5 min, round pellets with an average diameter of 6 mm were formed. With an increase in the residence time up to 15 min, a compaction of the pellets could be observed, which resulted in an increase in the dry substance content of 8.0 to 11.2 mass % on average. After completion of the pelletizing process, the pellets were mechanically dewatered by filtration. By applying this innovative wet pelletizing of iron-hydroxide containing suspensions in a Taylor-Couette-reactor, it is possible to carry out the process of flocculation combined with the pellet formation with simultaneous pellet compaction in a single process step. The resulting pellet structures are distinguished from conventional floc structures by an improved water discharge capacity. After classification the pellets can be marked directly as easy-to-handle bulk material, suitable as feedstock in percolation columns for the purification of contaminated fluid streams

The use of cemented rock fill (CRF) for support of open stope walls has increased in the Australian underground mines operations over the last five years due in part to its low capital cost. In open stoping the exposed CRF masses require adequate strength and stiffness, in order to resist the forces and to limit the displacement associated with movements in the rock mass surrounding the excavations. The CRF material preparation often involves the use of large particles, either from crushed rock or from development mining waste sometimes up to 1 m in diameter. However, the use of waste rock, which has a poorly graded particle size distribution may not achieve the required minimum target strength due to segregation and large air voids within the fill mass. This paper describes the optimisation of CRF strength by either crushing the waste rock or the addition of sand to the mix. Both methods increase the level of fines in the mix design. Cylindrical samples having 400 mm diameter and 800 mm heights were prepared for uniaxial compressive strength (UCS) testing. The UCS test was determined using the Western Australian School of Mines (WASM) large-scale static test frame. The laboratory results were compared with a database of large-scale CRF strength mixed without sand from a number of mine sites around Australia. The comparison shows a significant strength development in the CRF mixing with addition of sand for a given cement percentage. Due to the higher strengths achieved by the addition of sand and the crushing of the waste material these CRF types require less cement to achieve the same strength. The study aims to evaluate the pillar behavior during the depillaring operation by continuous miner (CM) technology. A panel with five headings developed as per the coal mine regulation of Indian coal mines [2] has been selected for the study. The depillaring operation has been carried out using CM technology after widening and heightening the original entries. An extensive field investigation has been carried out throughout the depillaring operation to access the mining conditions. Depillaring operation has been performed smoothly up to the middle of the panel (fourth row out of eight rows of four pillars each). Advanced blinking of the auto warning tell-tale (AWTT ) has also been observed on some of the instruments during the splitting of pillar no. 18. The panel shows extremely difficult mining conditions at this stage. The case has been numerically simulated using three-dimensional numerical techniques. The simulation result has indicated the high stress and yield condition on the working and the barrier pillar (surrounding goaf from both sides) after the extraction of pillar no. 18. The analysis has also indicated the marginal stability of the 2nd barrier pillar (namely “pillar B”). The failure of the barrier pillar could be the responsible for global instability of the panel as observed in the field. Understanding the importance of the barrier pillars, panel design has been modified by incorporating an additional row of barrier pillars. The stability of the modified panel has been evaluated. Both working and barrier pillars have been observed stable in the modified panel. This could be an alternative layout considering safety at the working face.

This report is volume 10 of an eleven-volume final report for a Grant completed by The Pennsylvania State University for the Bureau of Mines to develop a dynamic general purpose computer simulation model for underground coal mining. The simulator developed under the Grant has a wide range of possible applications in the design and evaluation of underground coal mining systems from the standpoints of health and safety, productivity, and mine environment. The Master Design Simulator consists of nine free-standing function-oriented subsystem models linked together into an integrated, comprehensive model of an entire mining system. The subsystems comprising the Master Design Simulator include geology and reserves, methane generation, roof support, subsidence, production, rail haulage, cost, water generation, and ventilation. This volume of the final report describes the Ventilation subsystem of the Master Design Simulator. The Ventilation subsystem has the capabilities to handle: (1) branches in parallel or series, (2) fans in parallel or series, (3) multiple fans, (4) methane, compressed air, or other injections into the system, (5) leakages and fixed quantities, and (6) calculations of absolute pressure at nodes. The program uses the Kirchoff's Junction and mesh laws, and the Hardy-Cross iterative technique to converge to the desired accuracy in solving fox the head quantity. A case study is included in the report illustrating the use of the model. The appendices of the report constitute a user's manual for the Ventilation subsystem, and include a program listing.

The vast coal resources of North Slope, Alaska can be expected to attract substantial interest because the nearly flat lying coal beds of the coastal plan are specially amenable to surface mining. Mining on a scale large enough to justify coal development in this area could have severe environmental effects. The mitigation would be uncertain because large scale coal mining and reclamation operations have not been carried out in the arctic The deficiency of the scientific data base is another factor that requires consideration if coal mining has to develop in accordance with the objectives of PL 95-87. The objective of this paper is to establish the interrelationship between the problems due to permafrost and factors in reclamation of post mining land in order to find ways to transform nature in a rational and purposeful manner and to suggest specific reclamation approaches that may be appropriate in this region.

Drill blasting is a widely used method for rock fragmentation in mining, quarrying and construction industries. Environmental effects such as air shocks, dust, flying rocks and ground vibration are unavoidable results and undesired effects of blasting. Among these ground vibrations can have a damaging effect and may influence on long distances. Therefore various researchers have developed mathematical models to predict ground vibration after blasting. These models take into account the amount of explosive, distance and several coefficients related to the field under investigation. The developed models to predict ground vibration may yield different results for the same case. Selection of the most suitable model requires engineering judgment and evaluation of models. At this point, a computerised system helps the engineer to evaluate and select the most suitable model in order to design and plan a blasting pattern. The safe charge of explosive and magnitude of vibration after blasting should be predicted beforehand. Therefore a computer program namely 'prediction of blast induced vibration' (P-BIV) has been developed. This paper introduces and presents a computer program (P-BIV) which includes the set of models developed to estimate ground vibration to provide blasting engineers a tool to determine the amount of explosive for safe blasting.

"Although seismic retrofit is recognized as a key micropile application, widely-used reference documents do not clearly address design and testing recommendations for micropiles that are loaded primarily under seismic conditions. The principles of micropile design and construction and their application to seismic retrofit projects are well understood and executed by leaders in the field. As the micropiling market grows, a wider audience relies on the information presented by DFI and similar industry groups. State-of-practice updates appear periodically in conference proceedings, but examination of recent bid documents makes it clear that there are opportunities to improve the specification of load, deflection, testing and acceptance criteria on many seismic retrofit projects. Updates to the FHWA Manual or DFI Sample Specification that specifically address seismic retrofit cases would be useful tools to disseminate information and enhance the state of practice.Micropile Survey UnderwayAs one step in this process, I am distributing surveys to contractors and designers prior to DFI Super Pile ’09 (June 4-6, San Francisco) seeking recommendations for design and testing procedures applicable to seismically loaded micropiles. I am also requesting any existing references that could be promoted as a standard for these cases. Anyone interested in participating in this survey, should contact me at rjameson@malcolmdrilling.com. Seismic RetrofittingMicropiles are ideally suited to seismic retrofit applications. Their high-tensile and compressive capacity can sustain the large concentrated loads induced during seismic events. They can be installed within and around the access limitations of existing structures, and can be drilled through and grouted into the wide variety of soil and rock materials encountered in seismically active zones.Seismic retrofits have included micropiles since the early 1990s, and they are now widely specified in the Western U.S. In the San Francisco Bay Area alone, thousands of micropiles have been installed for seismic upgrades of highway and transit aerial structures, and retrofits of commercial, residential and public buildings. Micropile use is growing in both public and private sectors, with large-scale retrofit projects currently underway for both the Bay Area Rapid Transit (BART) system-wide upgrades and renovations at San Francisco Airport."

Studies have linked long-term inhalation of diesel exhaust with adverse health effects such as cardiovascular disease, cardiopulmonary disease, and lung cancer. Diesel exhaust levels in underground mines can approach 100 times the environmental level and are therefore monitored closely in the mining industry. Elemental carbon (EC) (a product of diesel combustion commonly referred to as ?soot?) has proven to be a good indicator of diesel exhaust levels. The current procedure for measuring EC levels involves taking personal samples from a mine worker?s breathing zone and/or stationary samples in various parts of the mine and subsequently sending these samples to a lab for analysis by NIOSH 5040 method. Unfortunately, the whole process can take up to several weeks to complete. To address the problem of waiting weeks for results, NIOSH?s Pittsburgh Research Laboratory is developing a portable device for measuring levels of elemental carbon. This optical device takes a continuous sample of air in the surrounding environment and measures the absorbance of light passing through a filter that collects diesel exhaust particulates within the instrument. This technology can provide a near real time measurement of EC with accuracy within 10% error. The ability to take near real time measurements, combined with the instrument?s portability (currently small enough to be worn on a miner?s belt), makes this device extremely useful in the mining industry.

In his April 10, 1978 budget address, Finance Minister Chretien declared that the federal government will restrict the growth of its spending to less than the trend growth of the gross national product; over the past year, both the federal and a number of provincial governments have taken action to cut back on spending policies. This new emphasis in restraint would seem 10 be a response to a mounting public reaction to the rapid growth of government spending - and taxation - over the past 25 years. Although this relatively new government posture on public spending may be admired by many, to others it raises the question: "Can the growth in government spending really be stopped?" This article comments on just how government revenues - and spending - have grown over the past few years in Canada, and mentions some of the issues relating to a slowdown in that growth.

A review of technical literature covering Canadian mining emphasizes the advancement and changes that have been made in mining practice. However, current technical publications rarely outline basic principles or state definite rules that govern profitable mine operation. it may be argued that the broad principles of mining have long a go been outlined in various handbooks, text-books, and periodicals, and that these principles never alter. This is in great part true, but modern machinery, improved meta1-lurgical practice, and other advances, have in part modified these principles or changed the manner of their application. Many engineers hesitate to state specific rules, because it is so easy to quote an exception to a rules that any such statement appears to lose weight upon its very utterance. The exception to rules, and the reluctance to formulate rules, has forced such maxims as "the only rule in mining is the point of a pick," and "gold is where you find it". As an illustration of rules and their exceptions, one might quote the time-honoured maxim that a mill should not be built before a shaft has been sunk on a property in point of fact, Canadian mining history is replete with examples of wasted capital where a mill was built before a shaft had been bottomed. Y et this past year has seen a property enter profitable production whereon the building of the mill was started before the shaft was sunk. Instances such as this should not be accepted as evidence that a rule has been overthrown, but rather as an exception to the rule. The case cited pays tribute to the application of the diamond-drill combined with the correlation of sound geology. Incidentally, both these features emphasize a modern trend in the development of Canadian mining properties-a trend that is perhaps worthy of further comment. It is such trends that affect the principles of mining or modify their application. It is with such that this paper deals, at ]east in part. It is hoped that the statement of principles and the opinions herein outlined will be of sufficient interest to arouse discussion.

"Alumina is the main source of aluminium and it is extracted almost exclusively from bauxite ores found in the Southern hemisphere. Clays are abundant minerals that contain from 15 to 40% w/w of alumina from which it can be extracted by various processes. This paper reviews the processes used for the extraction of alumina from clays and discusses results obtained with a clay sample from the Murdochville area in the Quebec Province. The application of a conventional process to this clay did not yield an acceptable recovery although smelter grade alumina was produced. A modification to the process allows increasing alumina recovery from 85% to more than 92%. The application of the improved flow sheet to other alumina carriers failed to yield comparable recovery, showing that an optimum processing path for the recovery of alumina should be found for individual clay materials.INTRODUCTIONMost of the aluminium smelters are fed with alumina extracted from bauxite found in the Southern hemisphere. Bauxite is processed by the Bayer process that uses a caustic leach of the ore. In the middle of the century, the US Bureau of mines, motivated by the fear of an alumina delivery disruption, conducted several research projects aiming at producing alumina from non bauxitic ores. These projects led to the development of various acid leaching processes (Habashi, 1993). The sulphuric and hydrochloric acid leaching processes were brought to pilot plant evaluation (Bengtson, 1979), while nitric and other leach liquors were tested in the laboratory (Phillips and Wills, 1982; Habashi, 1983). Although clays are abundant and inexpensive minerals, the studied processes remained uncompetitive with the alumina extraction from bauxite (Hudson, 1987 page 42; Peters et al., 1962) for which reserves are estimated to support alumina production for the next 50 years (Oye et al., 1999). On the other hand, the local extraction of alumina from clays could significantly reduce the transportation costs of alumina to smelters and provide an alternative source of alumina supply from the Southern hemisphere countries. Also, the extraction of secondary products for some types of clay (Nasyrov et al., 1999) containing significant amounts of potassium and sodium could provide an additional return that may improve the economics of alumina production from clays."

"THE LOCATION of Canada's present aluminum industry is determined by availability of hydroelectric power, and ready access to ocean transport facilities. The chief raw material for the production of the metal is imported by ship over several thousand miles of water. In times of freedom of shipping and uncontested movement this provides the industry with its cheapest method of operation. However, during the early part of the last decade the general conditions of ocean and rail transportation were such that interest in the treatment of materials other than bauxite was greatly stimulated. This was the case not only in Canada, of course, but in most other countries.Canada has no deposits of bauxite and much research was carried out on other aluminous materials, chiefly silicates, in an effort to develop more or less economic and feasible methods of treatment. In the United States, this research was considerably more extensive. Some projects were early shown to be of little or no value, whereas others, including the widely publicized lime-sinter and soda-lime-sinter methods, became important. In North America, however, apart from the application of the sinter method to the hitherto waste-product (red mud) of the normal Bayer plant operation, none of these processes contributed appreciably to the production of aluminum during the war. In certain European countries, on the other hand, largely because of their situation with regard to bauxite deposits, they had a more general application, and Germany, for example, produced alumina from clay, ashes, and slag by at least four methods."
