Many of us who have interacted with senior executives from leading oil and gas companies over the last few years on the subject of HSE performance have observed a marked shift in their engagement on this aspect of their business performance. There seems to be a shared impression that HSE performance will have markedly increased impact on their businesses going forward. On the back of this very real concern, many are taking material actions to secure a step change in their safety and environmental performance: hiring respected mainstream operational personnel to lead their HSE function; investing in capacity/skills development for HSE professionals to increase their effectiveness in addressing the needs of the business; taking a hard look at the efficacy of established management and audit programs (too much paper, not enough results?); investing in information management systems so they can have access to real time reliable performance indicators; and, critically, initiatives to reduce accident statistics (especially focused on fatalities and contractors? activities) and deliver meaningful reductions in their environmental footprints. Underpinning all of these actions is a clear sense that substantial, very well-intentioned activity and investment in HSE aspects of the business has not been hitting the mark and yielding too little return in terms of performance improvement. The very serious recent events on the US Gulf Coast and at a refinery in the US Pacific Northwest have added fuel to this fire. The need for approaches which will secure a step-change in performance on the front line in the global oil and gas industry has never been greater.

In the last decade increased and prolonged interest has been displayed in the use: of tunneling or rapid excavation to solve many of the urban and industrial problems confronting our society. There has been talk and action on the interurban rapid transit systems for major cities, and even preliminary designs for a Boston to Washington interurban transit system. Much of this interest stems from the search to the solution of our increasingly complex urban problems, where the surface area must be given over more and more to the inhabitants, and the subsurface for transit, utility and other services. While tunneling is not a panacea to urban ills it holds a real solution to many existing problems. There is also, among the tunneling experts, a growing confidence that technical problems of rapid tunneling can be solved. To solve these problems, R & D money must be invested. If it is to be money invested either by private industry or governments, there must be significant returns on the investment to justify the research and development need to bring about the breakthrough that will make tunneling a major contribution to the solution of urban problems. To make these economic studies more extensive and more accurate tunneling demand figures were needed. To this end, the Organization of Economic Cooperation & Development - OCED - included in its Conference on Tunneling a section on tunneling demand. As a result of the conference the organization for Economic Cooperation and Development issued a report entitled "The Advisory Conference on Tunneling - Report on Tunneling Demand 1960-1980. " This report was presented at the OCED Conference on Rapid Excavation, held at Washington, D.C. To obtain the data in the Report, questionnaires were sent to the OCED countries and 18 replies were received from Australia, Austria, Belgium, Canada, Denmark, France, Germany, Ireland, Italy, Japan, Netherlands, Norway, Portugal, Spain, Sweden, Switzerland, United Kingdom, and the United States. While it is true, that data were missing, that sometimes the definitions were confused, and that several countries provided no cost estimates at all, in general, this report contains the most comprehensive gathering of data that ever existed in the field. All data in the OECD report is expressed in terms of meters or cubic meters, and costs are in U.S. dollars.

Dust formation and reaction mechanisms of copper matte particles were investigated in simulated flash oxidation conditions. The granulated and ground matte consisted of 72 wt% Cu, 4 wt% Fe and 21 wt% S. Experiments were conducted by using a laminar-flow furnace with a reaction gas composition ranging from 20 vol% to 75 vol% O2 in nitrogen. The particle feeding was carried out by the single-particle method and samples were quenched after a specified reaction time (approximately 0.05-004 s) in a cold water film. Experimental temperatures were between 800 and 1300°C. Chemical analysis was used to determine oxidation degree, and an optical and scanning electron microscope combined with energy dispersive analysis was used for characterization of the reacted particles.

After more than 100 years, alkaline cyanide leaching of gold and silver ores, either directly in the case of oxidized ores, or after pretreatment of refractory ores by roasting, pres­sure leaching or bacterial leaching, remains the dominant r leaching method to extract the precious metals f om ores. Leaching with halides (chlorine, bromine and iodine) and with thiourea and thiosulfates is less common but prac­ticed in some cases to dissolve the precious metals both from ores and jewelry and electronic scrap. Cyanide solutions can contain from 1 ppm Au to 30,000 ppm Au. According to the source, i.e., agitated tank leach­ing, heap leaching, eluates from stripping activated carbon or waste electroplating solutions. Similar variations occur with silver which can have a profound influence on the recovery method. The presence of other elements in the leach solu­tions, particularly mercury and copper, likewise play an important role in the procedures required to recover the gold r and silver values f om solution. The same circumstances are found when lixivants other than cyanide are used to dissolve the precious metals. For decades, the preferred method of recovering gold and silver values from alkaline cyanide leach solutions was by zinc dust precipitation according to the Merrill Crowe process developed in the late 19th century. Finely ground gold and silver ores or concentrates were either leached directly or after roasting, in agitated tanks or Pachucas and subjected to solid/liquid separation by filtering or by treat­ment in counter-current-decantation washing thickeners (CCD). This solid/liquid separation step was often very dif­ficult especially when ores containing high quantities of clay were treated which caused filtration or thickening problems. Following testing and development by the U.S. Bureau of Mines (USBM) in the late 60s, the introduction of carbon­in-leach (CIL) or carbon-in-pulp (CIP) plants to recover gold and silver from leach pulps was one of the greatest opera­tional and cost saving improvements made in the industry in South Dakota and quickly spread around the world to be the most common process to recover gold and silver from cyanide solutions.

"When boring a tunnel, homogeneous hard rock is the exception rather than the rule. While contractors can set goals such as meters per month and performance incentives, it is also prudent to plan for difficult ground. A properly designed tunnel boring machine (TBM) can tackle fault zones, water inflows, squeezing ground, or whatever the risk may be. The alternative is not a good one: modifying the machine in the tunnel after bad ground is encountered. Not only does this result in increased downtime, but it also needlessly exposes the crew to the tunnel conditions. In the worst case scenario, the entire operation grinds to a halt.However, an accurate GBR cannot always be obtained, particularly for mountainous, high cover tunnels in remote locations. And even with a thorough GBR, unforeseen geological events can occur. What can be done in these situations? Difficult Ground Solutions (DGS) is ground investigation system with a suite of options that can be added to a shielded hard rock machine or dual-mode, Crossover machine to better enable advance when conditions are unknown or difficult conditions are anticipated.DGS consists of a set of integrated features tailored to a specific project’s geology. The main components of the system allow for ground investigation ahead of the TBM, increased monitoring and methods to keep a machine shield from becoming stuck.In fault zones and squeezing groundIf fault zones or squeezing ground are known or suspected, or if there is even a possibility of encountering them, this can greatly affect the TBM operation. Shielded machines protect the crew from the surrounding rock, and they can bore and line a tunnel efficiently, but in fault zones and converging material their shields can be their downfall. Avoiding a stuck machine is paramount to project success. DGS employs several features to avoid the problem of a machine becoming stuck. The first of these is multi-speed cutterhead drives. These drives effectively give the machine multiple modes of operation—high speed, low torque for hard rock, and low speed, high torque for difficult ground. Designing a machine with high-torque, continuous boring capabilities allows that machine’s cutterhead to restart with break-out torque in difficult ground. The net effect is that the machine can keep boring in the event of a face collapse and can bore through fault zones and running ground where the potential for cutterhead jamming exists. Going one step further, multi-speed gearboxes give the machine the ideal earth pressure balance- (EPB-) type torque if larger sections of soft ground are anticipated.Secondly, TBMs can get through squeezing ground and faults using Continuous Shield Advance. This design utilizes a stepped shield configuration — where each successive shield is slightly smaller in diameter — to avoid becoming stuck in converging ground. External shield lubrication is an added insurance against becoming stuck, using a series of radial ports that can pump Bentonite into the annular space to act as a lubricant in squeezing material.As a last effort, if a machine has already become stuck the TBM can utilize augmented, or “super” thrust. Additional thrust jacks can be added to supply an extra boost in a short stroke, generating enough force to break loose a trapped shield."

Minerals Engineering Company (MECO) is a relatively small, promotionally oriented mineral exploration company founded in Colorado in 1948 during the height of the uranium boom. The company has been: 1. in uranium exploration and production 2. a drilling company 3. a manufacturer of Geiger counters 4. a major tungsten producer 5. then dormat for several years and 6. now we are back into exploration on a broad scale. We have been in underground lead and zinc production in the 1970's but we are now involved primarily in "wheeling and dealing" in mineral exploration. The company has had diverse interests in the past and we are presently diversifying to some extent into oil and gas ventures.

1.1 INTRODUCTION This chapter seeks to provide an overview of important factors involved in the implementation of heap leach technology as a vehicle for gold extraction from its ores. Toward this end, brief discussions of the various elements important to this process are presented here. Topics covered in this chapter include a brief history of the process; basic chemical reactions responsible for the process ; characteristics of ores amenable to heap leaching; location of the greatest concentration of heap leach projects in the U.S.; heap leach components, methods, regulatory/permitting considerations; and economic considerations. Most of these subjects are treated at much greater length in subsequent chapters. Thus, this chapter serves as a summary for those which follow, in addition to providing the reader with a feeling for those topics which may be of most immediate interest. 1.1.1 History of the Process Modern Day Precious metal heap leaching technology has developed over the past decade, although the principles of heap leaching per se, as well as those of gold extraction through cyanidation have a long history (see discussion below). However, it is in the past 10 years that heap leaching has developed into an efficient method of treating oxidized gold and silver ores. It has proven to be both an efficient way to extract precious metals from small, shallow deposits, as well as an attractive way to treat large, low- grade, disseminated deposits. Heap leaching has several advantages compared to conventional milling (i .e., crushing, grinding, and agitation leaching). In general, these advantages include simplicity, lower capital and operating costs, shorter start-up times, and less intensive environmental regulatory concerns. A possible disadvantage of currently existing heap leach technology is a potentially lower percentage of metal extraction being obtained from the ore than would be the case with conventional milling (Potter, 1981; Hiskey, 1985; and Thorstad, 1987) The actual principle of heap leaching has a long history as inferred previously. For example, mines in Hungary recycled copper-bearing solutions through waste heaps in the mid-sixteenth century (Hiskey, 1985), and Spanish miners percolated acid solutions through large heaps of oxide copper ore on the banks of the Rio Tinto around 1752. Industrial mineral by-products occur as a result of mining and processing a primary mineral product. By-products result from concentration and separation of accessory minerals during processing or from the primary mineral during processing, for example dusts. Many companies have found it rewarding to evaluate mineral by-products from their operations and determine potential products and applications. By-products need to be evaluated for physical and chemical properties to determine possible applications and markets. Market studies are needed to determine application specifications, price structure of the market, competitive products, and to determine marketing regions. The by-products need to be evaluated for beneficiation and processing to meet these market requirements.

Researchers at the University of Alabama have developed a method for producing fuzzy linguistic models of mineral processing equipment directly from engineering data. These fuzzy linguistic models rely exclusively on data collected from the processing systems being modeled and do not presuppose an understanding of the physics of the system. The fuzzy linguistic models use descriptive rules and explicit membership functions that provide a precise, nonmathematical description of the processing equipment. A genetic algorithm is employed to simultaneously select rules and tune membership functions used in the fuzzy models. The result is an extremely flexible approach to modelling that is applicable in a variety of linear and nonlinear systems. The effectiveness of this approach to modelling is demonstrated using a four-input, two-output model of a grinding circuit.

With the growing urban population and the concern for traffic congestion and pollution (emissions control), public transportation is becoming more and more attractive to both city dwellers and managers. To gain access to the central area of the city, the sub- way remains the most viable alternative, despite its higher cost when compared to above ground or elevated transportation systems. There are relatively few regulations and criteria for subway ventilation, particularly when compared with mine ventilation. The main document that provides guidance and general recommendations for subway ventilation and environmental control is the Sub- way Environmental Design Handbook, published in 1976 (2nd edition) by the U.S. Department of Transportation, Office of Re- search and Development. Many of the subway systems in existence today have been designed and built with ventilation features adequate for normal train operation, but their design does not consider stringent criteria for such emergency conditions as a train fire in a tunnel. The National Fire Protection Association's Standard for Fixed Guideway Transit Systems known as NFPA 130 as well as the ASHRAE Handbook - HVAC Applications (1995) provide specific design and operation requirements for subway ventilation systems. Some of the existing, old subways are upgrading their ventilation systems to comply with the new regulations. The ability of a particular ventilation system design to provide adequate ventilation during normal and emergency conditions can be evaluated using computer modeling and simulation techniques. A train fire will cause a sudden change in the tunnel ventilation pattern by adding an unsteady and fast-growing source of heat. The hot air and gasses created by a fire will tend to flow uphill, possibly against the normal flow, producing a 'backlayering effect." To prevent this effect from happening, enough ventilation must be provided and the governing criterion to establish the required airflow is called 'Critical Velocity." Several software packages are available for special applications on tunnel and station ventilation as well as to model the spread of smoke and heat in case of a major tunnel fire, using Computational Fluid Dynamics. This paper presents the current issues in subway ventilation, as shared by the public transit community around the world, together with modern alternative design concepts vis-à-vis the more stringent emerging Fire-Life Safety criteria. Examples of recent studies and ventilation systems design are provided.

The dissolution behavior of palladium powder in thiourea solutions has been studied. The variables studied included temperature, stirring speed, and concentrations of thiourea, acid and ferric ion. Palladium was found to be soluble in thiourea solutions in the presence of an oxidant such as ferric ion. The dissolution of palladium in thiourea solutions was found to be chemical reaction controlled with an apparent activation energy of about 2S.3 kJ/mol.

The adsorption of ethoxylated sulfonates and nonionics of the same ethoxylation number?on a low-ash coal has been studied. The surfactant adsorption has been investigated as a function of concentration, pH, temperature and ionic strength for three homologues. Zeta potential of coal particles and surface tension of supernatants has been concurrently determined to eludicate the orientation of sudactant molecules on the coal surface as well as the mechanism of adsorption process. The adsorption data are utilized to calculate thermodynamic parameters such as the free energy of adsorption and isoteric heats of adsorption.

Aeration curing, over- a long duration in wet- red stockpiles, shows promise as an affordable pretreatment method for oxidizing gold encapsulating sulfide minerals, and possibly organic matter, in low grade refractory ores. Following aeration pretreatment, the ore is reclaimed, neutralized, and cyanide leached by conventional practices. Large tonnages of subeconomic material, including mine stripping waste, may he converted into ore by this new technology. The results presented show that a successful application of this technology will likely require crushing to about -2 cm (-3/4 in.), followed by aeration in a wet stockpile for long periods, up m three years. Natural convection may he inadequate, hut forced air ventilation, at modest pressures and power consumption, is affordable.

Pennsylvania has 4,000 km (2,500 miles) of streams that do not meet water-quality standards due to acid mine drainage (AMD), making AMD the greatest cause of water pollution in the state. The problem is mainly caused by abandoned coal mines. To abate AMD, Pennsylvania is using a watershed-based approach that includes remining as a key component of a comprehensive restoration strategy. The Tangascootack Creek Water-shed was targeted for AMD restoration through remining. Geologic mapping and overburden data were analyzed using Earth Vision software to identify areas of coal reserves that could be remined to improve water quality. Incentives are offered to encourage mining in these designated areas.

The packed column flotation cell is an awesome separation device. Virtually unknown and untested, the packed column flotation cell is one of the ten most important developments in mineral processing of the 21st century. Though experimental and industrial data is limited, it is clear that the packed column flotation cell is two to three orders of magnitude better than the conventional flotation cell and one to one and a half orders of magnitude better than the open column flotation cell. With the modern packing taking up less than one percent of the packed column, column blocking does not occur. For a packed column operating 24 hours a day, 365 days a year, the estimated mean time between failures (MTBF) is one hundred thousand hours or roughly twelve years. With no moving parts, lower maintenance costs, lower power consumption and inherent operating stability, the packed column is a powerful separating machine. Because there is little meaningful data for comparing the packed column to the conventional flotation cell and the open column flotation cell, much of the material presented in this paper are guesstimates. They are disinterested guesstimate made by the authors after extensive theoretical work and reviewing the known facts. In the authors' opinion, no available data contradicts the guesstimate. The authors expect that there may be errors as large as three hundred percent. Even three hundred percent errors will not change the conclusions of this paper. The packed column flotation cell is a formidable separating device. To understand the problems of comparing the three types of flotation cells, consider the fact that the three cells may have different modes of separation. One of the authors, Meloy, believes that in the conventional cell, the primary particle separation takes place in the liquor with secondary separation taking place in the froth, while for open column flotation, all the separation takes place in the froth. However, he believes that in the packed column a large part of the separation may take place on the walls of the packing. If this is true, not only is the method of modeling each type of cell different, but the method of particle selection in the cell is different. This means that one type of cell may be better at differentiating between two different types of particles. There is a growing body of literature on open and packed column flotation. Investigation of the traps in LiF:Mg, a multi-peak phosphor, was carried out using simple phosphorescence decay analysis. Half lives at room temperature of the trapping centres (peaks -lb, -la, -1, 0, 1, 2, 3a and 3) in the phosphor after irradiation with [a] radiation were found to be 3 s, 5 s, 28 s, 4 min, 15 min, 1 h, 6 h and 2 d respectively. Justification for the method of analysis is pro- vided and the results of measurements compared with published data. The effects of pre-annealing at 400°C for 1 h and 80°C for 24 h on the trapping configuration and on the trapping efficiencies were also studied.

A performance study of oxygen self-rescuers of varying durations from the United States and other countries was undertaken as an assessment of present worldwide technology. The apparatus were tested on a Breathing and Metabolic Simulator in the Life Support laboratories of the Bureau's Pitts- burgh Research Center. Parameters monitored during the testing were inhaled levels of C02, 02, inhaled gas temperature, and breathing resistance. The metabolic demand placed on the apparatus represented the average demand of the 50th percentile miner performing a 60-min man-test 4, as described in 30 CFR 11H. Results presented include apparatus service life and subjective comments as well as averages and peaks of monitored parameters.

Mine ventilation simulation software has become an essential tool for the mining industry to design and manage mine ventilation operations. Building a well-calibrated mine ventilation network is time consuming, however, and requires great effort. In the last several decades, the mining industry has seen a steady increase in the use of air velocity sensors to monitor airflow and detect unexpected abnormal airflow in some key locations in underground mines. The present research work was carried out to explore the possibility and methodology of calibrating a ventilation network model using atmospheric monitoring system (AMS) data on a routine and ongoing basis.

Douglas W. Fuerstenau (DWF) grew up on a farm and in several small communities in rural South Dakota during the depression years, where he learned the value of hard work. Working hard can produce results but being clever and hardworking will give you a bright future. In his later school years, Fuerstenau’s family moved to Rapid City where his intellectual curiosity and capacity helped him graduate with a B.S. in Metallurgical Engineering from the South Dakota School of Mines and Technology in 1949. His technical interests led to graduate studies and completion of a master’s degree from the Montana School of Mines in 1950 and Sc.D. degree from the Massachusetts Institute of Technology (MIT) in 1953.
