"The International Organization of Standards defines risk as “the effect of uncertainties on achieving objectives (15).” In mining, many uncertainties can influence the outcome of personal or professional objectives and are often difficult to predict. Risk management, the minimization and control of adverse effects from exposure to identified risks, is a well-established concept that can summarized as “being smart about taking chances” (12). Risk management now consists of management processes unique to numerous organizations across many industries, as varying sectors encounter a variety of needs, including the analysis and mitigation of risks related to physical security, product liability, information security, various forms of insurance, regulatory compliance, and workplace safety (12). The reoccurrence of multiple fatality events in the US mining industry, especially the underground coal sector, suggests an opportunity for improved methods of major safety hazard identification, assessment, and mitigation. A risk management approach would include ready identification and mitigation of these risks, inclusive stakeholder engagement, and rapid integration. While many solutions to reducing the risk of mine disasters have been proposed including stricter regulation and improved technology, a comprehensive risk management approach has yet to be fully integrated in the US mining industry. In December of 2006, R. Larry Grayson of the National Mining Association’s Mine Safety Technology and Training Commission called for “a new paradigm for ensuring safety in underground coal mines, one that focuses on systematic and comprehensive risk management as the foundation from which all life-safety efforts emanate (7).” A risk management approach has already been implemented with success in Australia’s minerals industry as part of a transition during the mid-1990s from a prescriptive based health and safety attitude to a more proactive, duty-of care [risk-based] philosophy (13, 16, 17).FUNDAMENTAL ELEMENTS OF RISK MANAGEMENT TECHNIQUES The fundamental components of a comprehensive risk assessment/management program are well-established within the realm of mining health and safety. Figure 1 depicts the elements of a typical mining hazard management framework. The first step in any safety-based risk management system is to identify and assess hazards by their location, nature, and magnitude within a mine. Next, a decision is made about choosing to eliminate, mitigate, or tolerate each hazard. Ideally, a hazard will be eliminated during the design stages of mining, though dynamic mining conditions often require continuous improvement and reevaluation of existing hazard controls. If elimination is not possible, hazard mitigation actions can consist of methods, rules, equipment, competencies, or other mechanisms. If a hazard is simply tolerated, as many inherent mining hazards must be, then specialized administrative controls must be utilized to minimize losses. During this process, the performance of each action is monitored and modified as needed (13)."

ABSTRACT Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage organizations to work together, executing such risk assessments comes with challenges. This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

Ice frozen from a dilute chemical solution is mechanically weaker than ice frozen from pure water. This phenomenon is the basis of a practical method for reducing the strength of a mass of frozen coal. If a chemical additive is applied to moist coal, prior to its shipment during freezing weather, the resulting frozen coal can be more easily broken than in the absence of the chemical additive. This greatly expedites the unloading and handling of coal during winter. There has been considerable research into the nature and mechanical strength of ice frozen from various solutions. The literature relating to this process has been reviewed as has the literature relating to the chemical treatment of coal and mineral particulates. Some important articles are discussed and speculations are made about the nature of the ice-weakening process. A laboratory evaluation of the effects of chemical additives on the mechanical strength of ice and frozen coal has been made. An apparatus designed for this purpose is described, and some results obtained with it are discussed.

The Wachovia Headquarters and Arts and Cultural Campus project in Charlotte, NC includes a 1.5 million square foot, 48-story office tower, two museums, an Afro-American cultural center and a 1,200 seat theater for the performing arts. Public access to the Cultural Campus is through two pedestrian tunnels constructed under city streets connecting to parking garages. The 5.7 m wide by 4.7 m high mined tunnels were procured through a design-build contract. The tunnels were designed as initial/final shotcrete linings with a sprayed water barrier sandwiched between layers of shotcrete. In addition, the mix design was supplemented with a cementitious admixture to develop internal water resistant properties in the shotcrete.

In this article, the cementation of major impurities from zinc sulphate leach solution by the addition of zinc dust was studied by considering different parameters including time, mesh size, and percentage of excess zinc dust. The output obtained from various experiments was utilized for the design of experiments. A multi-level categorical factorial design with a quadratic model was employed during this study. Results were predicted to minimize the concentration of impurity metals in the solution by altering the parameters. Furthermore, these predicted results were validated by performing another experiment. Successfully, it showed that the predicted and experimental data values were close to each other. The predicted optimized values were 40 min, 100% excess zinc dust, and 300 mesh size for copper; 42 min, 100% excess zinc dust, and 300 mesh size for cobalt; 37 min, 100% excess zinc dust, and 300 mesh size for cadmium; and 34 min, 100% excess zinc dust, and 300 mesh size zinc dust for nickel. At these optimized values, the experimental results showed 99.99%, 55.7%, 99.6%, and 76.6% cementation efficiencies for copper, cobalt, cadmium, and nickel respectively. The statistical design of experiments for the effective cementation of impurities corroborates the economic feasibility of optimizing parameters for the proposed process.

If the communication/tracking system of an underground mine is severely damaged in a large mine accident or explosion, a surface based seismic system could be the only available tool for locating any trapped miners. This paper documents a field test of a seismic system at a room-and-pillar coal mine in Eastern Kentucky. Testing was performed at 6 different depths (450, 575, 750, 1175, 1525 and 1700 ft) above the mine. For each location underground, the miners pounded a ?set? of 5 ?repetitions? on the roof with each repetition being about one second apart. Over the 3 day field test, 63 sets of underground miners? pounding were detected by the seismic system at depths up to 1175 ft and at horizontal offsets up to 890 ft. Also during the testing, the observed signal strength generated by pounding with a crib blocks was seen to average 180% stronger than the observed signal strength generated by pounding with a sledgehammer. Ultimately, this field test was very successful and definitively demonstrated the ability of the seismic system to detect potential trapped miners? pounding up to a depth of 1175 ft.

Coal mine safety of the past was plagued by disasters that took the lives of thousands of miners. Through the years new mining methods, improved laws, and a commitment from the mining industry has reduced the frequency of such incidents to a minimum. History tells us that coal was mined in the early 1700's in Virginia primarily for use by blacksmiths -- most of that mining was done in the Richmond area. It was in those mines that methane was encountered; as workings became more extensive, methane explosions became more frequent. In the early 1800's the first recorded explosion occurred near Richmond, Virginia -- 40 men were killed.

Propelled by the invention of the transistor at Bell Laboratories, germanium's unique properties have advanced our electronic world by many years. Germanium has found major uses in infrared optics, fiber optics and a revised vigor as a catalyst for PET plastic production. Other advanced applications are in various stages of development. The solid state electronics industry has been the driving force behind the accelerating demand for gallium since the late sixties. Past developments of substrates for light emitting diodes, lasers and magnetic bubble memories are paling in view of the current excitement over the use of semi-insulating gallium arsenide for high performance integrated circuits. This application requires gallium metal and gallium compounds of extraordinary purity.

A new sanitary sewer was installed in Pleasant Hill, California using microtunnelling methods. The project consisted of 693 meters of 1372 mm and 1981 mm concrete pipe. A Herrenknecht AVN 1200 was used to install the nominal 1372 mm pipe in three drives. The machine was then fitted with an oversized shield used to install the large diameter pipeline. This articulated shield allowed the excavation of a 2362 mm diameter hole with a 1524 mm machine-- a technique new to North America. This paper reviews the design considerations for the articulated shield and reports on the project challenges and solutions to overcome these challenges.

Oxide ores can consume enormous amounts of expensive sulfuric acid. The problem is amplified with increasing heap heights. One solution is to provide a thin liner and drainage system between each lift of ore, thereby intercepting the pregnant leach leach solution (PLS) after it exits the active lift. However, these liner systems require special attention to hydraulics, slope stability and geomembrane design criteria. Inadequate attention to the first two can result in reduced recoveries, increased acid consumption andsystem failures. The geomembrane selection criteria are different from the base liner, and optimizing its selection can have significant effects on the operation’s bottom line.

Functional mineral pigments are used in large quantities in paint, paper, plastics, rubber, textiles, agriculture, and several other industries. U.S. consumption in 1969 is estimated at over 5 million tons a year, valued at nearly $150 million, as shown in Table 1. Originally extender pigments were employed only to lower cost. They were thought of merely as adulterants to replace more expensive prime pigments and binders. However, over the years their role has become better understood, and today their main functions are: - Increasing strength, stability, and service life. - Improving surface properties. - Enhancing application and processing properties. - Extending prime pigments. - Controlling viscosity and package stability - Lowering overall costs of the finished product. The principal minerals used as functional pigments are kaolin, ground calcium carbonate, talc, and asbestos. Other minerals include barytes, attapulgite, diatomite, mica, ground silica, wollastonite, and nepheline syenite. Several synthetic products, including precipitated calcium carbonate; precipitated, gel, and fumed silica; alumina trihydrate; and barium sulfate, compete with mineral products. They are more expensive, but often have more closely controlled particle size or other properties because they can be tailor-made according to the requirements. This paper covers only natural products selling for $10 or more a ton, approximately the lower limit for pigment grades. Data refer only to consumption in pigment applications. For example, such applications as filter aids for diatomite are not included. Key properties of the major mineral pigments are given in Table 2. It is important to note that these properties are primarily physical rather than chemical. In general, these products are sold to performance specifications for what they can do rather than on chemical composition specifications on what they contain.

During the 1980s, a revolution has been taking place in industrial minerals marketing. Transportation deregulation and a handful of creative marketing and distribution managers have completely changed the rules for marketing many industrial minerals. Minerals are being transported much farther than in prior decades and at much less cost. Bulk and package terminals are becoming increasingly common as a means to take advantage of freight rate savings and to provide enhanced service to faraway customers. Water and rail transportation are being used increasingly to penetrate markets considered unreachable just a few years ago. No longer do the textbook rules of thumb for industrial minerals marketing apply. This author recalls three of those rules he was taught in mineral economics graduate school in the 1970s. As recorded in class notes: • construction aggregates are seldom shipped more than 32 km (20 miles) from the quarry; • cement is sold within 240 km (150 miles) of the mill; and • industrial sand is sold regionally - it is freight constrained to a market radius of no more than 320 to 400 km (200 to 250 miles) from the plant. And yet, the author looks up in 1989 and observes overland movements of aggregates of many hundred miles, overland movements of cement and industrial sand of 1600 km (1000 miles) or more, and water movements of both aggregates and cement into the United States from sources halfway around the world. Truly, industrial minerals are in the midst of a marketing revolution. The companies that have led this revolution have enjoyed considerable financial success in the 1980s. They have enhanced their profitability by increasing market share, capacity utilization, and average selling prices by taking product to remote markets where premium prices are available. They have consistently outperformed their competition, many of whom remain unaware that the marketing revolution going on around them is actually a transportation/distribution revolution. The successful industrial minerals company of the next 10 to 15 years must participate in the transportation revolution as it continues to evolve. "In Europe, and therefore in Hungary opening a new mine is harder and harder. Depletion of mines, unfavourable legal and economic changes, and growing public concerns are the main reasons. Social and economic interests require total utilization of mineral raw materials of the operating mines, and furthermore utilization of waste heaps of the former mines. There are 5-6000 waste heaps of the operating and the closed mines in Hungary. 2000 of these waste heaps have already been estimated, and researches show that total amount of waste heaps can be calculated to 1 billion tones. These researches left out of consideration waste heaps exploited and placed in deep lignite mining. In several projects and researches we analysed processing of waste heap and possibilities to use its components. A processing technology for utilizing waste heaps has been developed based on laboratory and pilot plant tests. Great amount of useful materials of quarrying waste heaps can be utilized with this technology. Tests were also done for processing waste heap of a former ferrous metals mining. The developed processing technology contributes to use these waste heaps as aggregate for cement industry, as dielectric and aggregate for construction. The essay presents in details main characteristics of waste heaps, technologies already used for processing waste heaps of quarrying and ferrous metals mining, mass balances and qualitative parameters of the end products."

Various methods have been developed through US Bureau of Mines research to remove gas in advance of, during, and subsequent to mining. These include drilling horizontal or cross-measure boreholes from a mine opening, and vertical boreholes stimulated for drainage in advance of mining or for gob gas drainage. Techniques and equipment now exist that allow drilling a borehole at any angle to the vertical, and even along an arc. Stimulation treatments have also been developed that provide for increased gas production capability. This paper describes the long-term production history of five select gas drainage installations. The Bureau of Mines conducted or participated in four studies. The remaining study was conducted by the Equitable Gas Co. Principles of storage, release, and migration of coal bed gases The amount of gas contained within any given area of a coalbed depends on the coal's inherent storage capacity and the physical conditions of the coalbed. The gas storage capacity of coal is primarily a function of rank. As such, it cannot be affected by the presence of drainage holes (Kim, 1977). Certain physical conditions of the coalbed can, however, be changed that will affect the release and migration of gas. By pumping water out of the coalbed through the vertical boreholes, two important changes occur - pressure is reduced and water saturation is lowered. Reducing reservoir pressure releases gas as the coalbed adjusts and equilibrates gas to the change. Increasingly large amounts of gas are released for each incremental decrease in pressure. Lowering the water saturation level raises the permeability of the coalbed to gas and allows the gas to more readily migrate to the wellbore (Kisse and Edwards, 1975). For horizontal boreholes completed near mine openings, the coalbed has already undergone a pressure or water saturation reduction phase. This condition thus allows such boreholes to be highly productive with minimal water production. Secondary Ion Mass Spectroscopy (SIMS) was used to study copper ion adsorption, and its effect on subsequent sulfide collector adsorption, on pyrite and pyroxene minerals under flotation related conditions. Copper was found to adsorb on both of these minerals and cause enhanced sulfide collector adsorption. The distribution and depth profile of copper and collector on these mineral surfaces were obtained by using SIMS in the imaging mode. The results of this study have provided a valuable insight to understanding pyrite and gangue flotation observed frequently in operating plants.

This paper presents a nonlinear integer goal programming model that optimizes a mineral production schedule while considering the penalties and bonuses based on quality variations in the final product. It involves five coal quality goals plus a profit objective in its formulation and uses the weighting method of goal programming for a solution. The goals to be sought involve the sulfur, ash, heat content, moisture content, and ash softening temperature of the blended coal product. The model is linear in its handling of most of these goals but uses a piecewise linear integer formulation for the sulfur and heat content bonus and penalty schedule. The model is applicable to any mineral product blended from several different sources and subject to a varying price dependent on blended product quality or qualities. However, it seems to be most pertinent to optimizing production schedules for coal deliveries to power plants. Accordingly, the model is applied to a hypothetical coal production problem where multiple coal mines are operated to supply a final blended product to a power plant. In this example, the price paid for the coal is assumed to be a function of five of the most important coal qualities. The problem is solved as a direct function of the coal supply contract, thus removing the necessity of determining the weighting coefficients on a subjective basis.

Zarshuran gold deposit, with the total reserves of some 12.5 million tons of 4.18 g/ton ore, is considered to be an important gold mining project in the area. In this paper the feasibility studies of the whole project is carried out on the basis of several economic variables. Given that mining data are often of uncertain nature, these economic variables have therefore been estimated under the conditions of uncertainty. The resultant Net Present Value is hence also obtained under the same conditions. For this purpose, a model is first devised with the help of Excel and COMFAR software. The model is then developed that considers different scenarios which would result in different expected values for the economic variables. It is concluded that the NPV of the project is most sensitive to gold and silver price and that other variables such as discount rate, operating and capital cost all affect the feasibility of the project with lower degree of severity. Finally, risk analysis is also carried out using Mont Carlo technique, in order to estimate the most probable value of NPV. The average NPV calculated in this way is US$ 49.7 Million, whilst without due consideration of risk, this would be US$ 36.6 Million. The techniques used, together with the procedures adopted in this paper, can be used in feasibility studies of all mining projects. Such decision would result in more accurate Net Present Values than expected from other frequently used methods.

The present is rooted in the past. This is particularly true in the case of mining operations. Coal mining in the United Kingdom is considered an old Industry comprising today 246 Coal Mines many of which have worked coal reserves in their locality for over 100 years. More than half the current producing mines lie in the Yorkshire and Midlands coalfields. They are in the oldest age group and produce 70% of the Industry's output. Some 50% of the remaining U.K. workable reserves amounting to approximately 98,000 million tons are still available to these regions. Fig. 1 Over the past two decades a gradual erosion of demand for coal in the U.K. has virtually halved the Industry's capacity. The number of Mines has reduced by 53% during the past 10 years. Productivity has increased by some 42% mainly through improvements in the central coalfields which has enabled costs to remain broadly competitive with oil.

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

"Sound Transit’s East Link Project, a $3.7 billion (2015 $), 14-mile light rail extension, will connect downtown Seattle with the rapidly developing east side cities of Bellevue and Redmond. Among the most technically challenging aspects of the Project is the 2,435 foot long Downtown Bellevue Tunnel (DBT), which will be constructed through the central core of Bellevue underneath 110th Avenue NE using a combination of Sequential Excavation Methods (SEM) and cut and cover tunneling. The 1,980 foot long central SEM component of the DBT will be mined at shallow depth through challenging and complex geology, comprised of glacially overconsolidated soils, containing both regional and perched groundwater conditions and in close proximity to existing high rise buildings. The paper shares lessons learned from the planning decisions and risk based approach to the investigation, design and procurement processes which shaped the DBT configuration and enabled a successful transition to the project’s construction stage. INTRODUCTION The East Link Project is a voter-approved $3.7 billion (2015 $), 14-mile extension of the existing Sound Transit (ST) light rail transit (LRT) system from downtown Seattle, across Lake Washington via I-90, to the cities of Bellevue and Redmond on the east side of Lake Washington. With targeted completion in 2023, East Link will provide riders with an efficient and reliable connection between the largest population and employment centers on the east side and downtown Seattle. The project alignment is indicated in Figure 1 below.ST completed the Preliminary Engineering (PE) phase of the East Link project in late 2011. Subsequently the final design contract for the easternmost seven miles of the project was awarded to the H-J-H Team, a joint venture comprised of HNTB, Jacobs Engineering and Hatch Mott MacDonald in February 2012. H-J-H’s final design scope includes approximately 7 miles of double-track LRT between the east end of the I-90 East Channel Bridge and the Overlake Transit Center Station. While the project scope is extensive and diverse, the focus of this paper is the DBT. The DBT is located immediately north of the East Main Station, and travels approximately one half mile through downtown Bellevue under 110th Avenue NE between Main Street and NE 6th Street; terminating at the Downtown Bellevue Transit Center (BTC) Station. The location and extent of the DBT is indicated in Figure 2."
