Researchers at the National Institute for Occupational Safety and Health (NIOSH) are conducting research to improve the ventilation of large-opening underground stone mines. The mine ventilation in these mines are characterized by: (1) challenges related to keeping airflow velocities high enough to effectively remove or dilute airborne contaminants, (2) low pressure losses needed to move large air volumes of air, and (3) costly stoppings needed to direct ventilation airflows. Each of two field studies was dedicated to specific focus areas. The first focus area was the characterization of underground mine ventilation airflow. The primary means of airflow control in stone mines is through the use of freestanding booster fans. The primary goal of this study was to identify the effectiveness of various fan types and optimize their placement. Both traverse and grid-based airflow readings were taken at various distances from the fans, ranging from 70 ft to 425 ft away from the fan. The air readings were taken using a vane-anemometer and a handheld hotwire anemometer. The data was used to develop precise computational fluid dynamics models. The second focus area was to determine diesel particulate matter (DPM) concentrations in the mine. For large-opening underground stone mines, controlling DPM exposures dictates the design of the ventilation system. Therefore, researchers carried out the DPM sampling campaign to identify potential areas of concern within the mine. The samples collected during the campaign were analyzed using NIOSH Method 5040. The sampling was done using the Mine Safety and Health Administration’s (MSHA) compliance sampling methodology.

The effect of the overflow to underflow (O/U) flowrate ratio of a dense medium cyclone on its performance is tested utilizing a 6" dense medium cyclone and density tracers. The results show that the O/U flowrate ratio is clearly correlated with the separation cut point D50, the separation efficiency Ep, the density differential 0, and short circuiting of the cyclone. Within certain limits, a higher O/U ratio results in a better separation efficiency, a higher D50 and an increased density differential. A lower O/U ratio, on the other hand, reduces the cut point shift and density differential. The phenomenon of short circuiting to the underflow, which is especially significant for fine particles, is directly connected with an excessively low O/U ratio.

As indoor positioning provides particular challenges due to the unavailability of GPS signals, various systems such as ultrawideband (UWB), radio frequency identification (RFID), ultrasound, and wireless local area network (WLAN) have been proposed in recent years. Some of these technologies are currently being marketed and some are still being developed. UWB technology allows for higher precision while also reducing power consumption. Hence, the underground automation and localization systems can use this technology for more accuracy and robustness. This article discusses new robust UWB modules used for underground positioning and collision avoidance with regard to human safety in underground mining operations.

Ontario Power Generation is building a 10.4-km water diversion tunnel in Niagara Falls, Canada. The tunnel is a two-pass, cast-in-place prestressed concrete-lined pres-sure tunnel with a nominal internal diameter of 12.7 m. It is mostly situated within the Queenston formation which swells in contact with fresh water and has very aggressive groundwater. Tunneling in this material presents challenges in both design and construction. The tunnel design includes for a membrane to eliminate the potential for rock swelling and to protect the concrete liner from the ground conditions. A unique three-stage Geotechnical Baseline Report (GBR) is being used and is expected to provide an equitable method of dealing with any variations in subsurface conditions.

The location and orientation of fire suppression nozzles is critical in suppressing mobile mine equipment fires. To effectively suppress such fires, optimization needs to be considered for the suppression nozzle locations and discharge angle. Detailed experiments were conducted to study the effectiveness of fire suppression agents with different nozzle configurations. A variety of fire suppression systems including dry chemical, wet chemical, dual agent (dry and wet chemical), carbon dioxide, and water mist were used. Six types of nozzle locations were studied, and their fire suppression effectiveness were compared. Results from this work highlight potential considerations when determining effective fire suppression designs.

"Tunneling and Underground works are unequivocally subject to a diversity of inherent uncertainties associated with the geotechnical, hydro-geological and environmental regime that surrounds them. On many occasions, these uncertainties can provoke loss events of considerable consequences. The present contribution elaborates on losses that the insurance market has suffered in the recent years, following a construction failure event. These losses are being considered and analysed on a quantitative basis through evaluation of cost related data, with explicit discretization of the applied construction methodology and of the subsequent developed failure type since both of which are considered of major importance. Given the identified persistence and regularity of some loss-contributing factors, recommendations are provided on the basis of a proactive risk engineering management approach that are envisaged to reinforce the understanding of project risks and alleviate the incurred insurance cost. 1 INTRODUCTION The underground construction sector has always been a very challenging area for all involved parties, including the insurance market. Main reason for that remains the uniqueness of the uncertainties sources and the subsequent inherent risks that the tunneling and underground projects are exposed to. In many instances, these risks materialize, leading to loss events with substantially high impact on reinstatement cost and incurred delays, which depending among others on the insurance coverage will have to be bared by the Insurance market. In the recent decades and as the tunneling industry boomed and expanded with many major and significant projects undertaken around the world, the insurance market was faced with additional challenges. Increased competition during tender coupled with the scale and complexity of the projects and politically driven decision making exerted pressure on the project timelines, completion dates and budget. The pressure for on-time or even early project completion- to avoid liquidated damages or achieve bonus payments, respectively- along with cost reduction efforts and the incentive for innovative failure events. That, in turn, put additional pressure on the Insurance market which had experienced a significant amount of losses with substantial cost. One of the triggered mitigation actions was the introduction of the Code of Practice for Risk Management of Tunnel Works (ITIG, 2006 & 2012; MunichRe 2006, Wannick, 2007, Spencer, 2008, Adeyemo, 2011)."

Introduction This paper attempts to summarize some of the major criticisms of drug testing. Eight problems with drug testing are discussed. These are: • It does not measure something employers have a reasonable need to know, • It is a distortion of priorities, • Drug use is not known to be associated with increased risk of occupational injuries, • Drug testing is proposed as a dangerously simplistic solution to poorly conceived problems, • In the absence of safeguards, it is unacceptably inaccurate, • In the absence of treatment commitments and confidentiality protection for persons dependent on drugs, it is an unethical practice of medicine, • Drug testing is an invasion of privacy, and • It will have a negative effect on treatment of persons with drug or alcohol dependencies. Employers' need to know It does not measure something employers have a reasonable need to know. It is reasonable for employers and fellow workers to want to know if workers are functionally impaired (intoxicated) while on the job. Drug testing does not measure impairment. It measures the concentration of drug metabolities in urine or blood. Unlike measurements of blood alcohol concentration (BAC), drug metabolites are not correlated with impairment or intoxication. They indicate only drug use in the one to two weeks before a urine sample. And, in some instances, they indicate chronic use. Impairment caused by drug use, on the other hand, occurs immediately and declines rapidly within hours. Drug metabolites may linger for days afterwards. Drug metabolite concentrations in urine are a function of (Morgan, 1984; Schartz and Hawks, 1985; MMWR, 1983): • the amount and purity of drug consumed before taking the test, • when the drug was consumed, • whether the person was a chronic user, in which case a certain amount of drug metabolites would be stored in body fat and slowly released, • the person's metabolic rate, and • for metabolites in urine, any dilution effects of urine. In an earlier study (Deschênes et al., 2009), gold and silver were shown to be efficiently extracted from samples taken from the Kupol Mine in Russia using lead nitrate in a cyanide leach system. In the current study, a com-parison of the technical and economic merits of a new technology called CANMET enhanced leaching process (CELP) was made with the cyanidation/acidification-volatilization-recycling (AVR) process using eight metal-lurgical variability samples, as well as a large composite sample, which was to represent the initial two years of Kupol mill feed. The composite sample leach feed assayed 33.4 g/t Au and 289 g/t Ag. The metallurgical results from the large composite sample demonstrated that CELP produced very similar gold and slightly higher silver leach extractions compared to the conventional cyanidation/AVR leach conditions at 1/2 the cyanide concentra-tion (1,000 ppm NaCN in CELP vs. 2,000 ppm for cyanidation/AVR leach conditions). It was estimated that the total cost per ton was approximately the same for the two technologies (total cost of CELP with the use of Merrill-Crowe at $46.48/t, with cyanidation/AVR at $46.13/t). However, the implementation of CELP would eliminate a capital expenditure of approximately $5 million for an AVR system based on a 2005 feasibility level cost estimate. Eight variability samples used to confirm the composite sample results showed that, on average, the gold leach extractions with CELP and the conventional leach conditions were very similar, but the average silver leach extraction was 2.1% higher for CELP. Furthermore, CELP silver leach extraction was 4.6% higher on average compared to conventional cyanidation for the four mid-grade variability samples evaluated. The decision was made to remove AVR cyanide recovery from the flowsheet and implement CELP at Kupol. The ability to use the conventional leach equipment with CELP and simplify the flowsheet was also a factor, given the remote location of the Kupol Mine.

A prevalent technique for the recovery of silver from wastes generated in silver-tungsten electrical contact fabrication is electrowinning. Such scrap is placed in a permeable plastic basket which constitutes the anode compartment and this basket is suspended in a larger cell in which the cathode is mounted. A silver nitrate solution is used as the electrolyte and, upon application of an electric field, the silver migrates to the cathode where it deposits out. At times, there is tungsten migration to the cathode compartment and consequent co-deposition with the silver. This W contamination, which may amount to several thousand ppm, necessitates additional purification of the deposited Ag and its removal would therefore be advantageous. The elimination of the W impurity would also extend the operating period of the cell with corresponding increase in the Ag output and savings in labor costs.

"Sound Transit’s University Link and Northgate Link projects will add 6.75 miles (10.9 km) of twin-bored tunnel to Seattle’s light rail system, extending the route north from downtown Seattle. The tunnels are being constructed primarily in glacial soils using pressurized-face tunnel boring machines. Cover over the tunnel alignment varied from 13 feet to over 300 feet (4–91 m). Since the tunnels are located in densely populated urban areas, controlling ground movements and monitoring settlement were essential to success of these projects. Hundreds of geotechnical instruments, including extensometers, surface settlement points, and near-surface settlement points, were used in both projects to monitor settlement. The observed data were compared to settlement predictions made prior to tunneling, and analyses performed to update empirical factors to better estimate settlements. Recommendations for designing efficient and cost-effective settlement monitoring programs are provided based on the review of the monitoring data and analyses.INTRODUCTIONThe paper includes data from Sound Transit’s University Link and Northgate Link projects. These projects will add 6.75 miles of twin-bored tunnel to Seattle’s light rail system. The tunneling for University Link will extend the light rail system from downtown Seattle to the University of Washington. Tunneling from downtown Seattle to the Capitol Hill Station was completed under the U230 contract, and tunneling from the Capitol Hill Station to the University of Washington Station was completed under the U220 contract. The Northgate Link extension will extend the light rail line north of the University of Washington, including approximately 3.5 miles (5.6 km) of twin tunnels between the University of Washington Station and the Maple Leaf Portal and two stations, U District Station and Roosevelt Station. Tunneling for the Northgate Link project is still underway as part of the N125 project. At the time of writing, tunneling from the Maple Leaf Portal to Roosevelt Station has been completed. Therefore data labeled as N125 in this paper only include the portion of the alignment from the Maple Leaf Portal to Roosevelt Station."

[Portland cement is a product made from a combination of limestone and argillaceous material which would supply the necessary amounts f calcium, silica, alumina and iron to form cementious compounds when fired at high temperatures. The resulting mixture, usually around 80% limestone and 20% argillaceous, is ground nine (80% -200 mesh), blended and then fired1 (2600-2700°F) to produce a cement clinker from which the final cement product is produced by grinding. The argillaceous component can be shale, clay, slate or other sources of the individual components of silica, alumina and iron. This paper deals with a case where a clay component was under consideration. The planned Plant site was to be located at a well defined limestone deposit. When it came to the argillaceous component, the nearest uniform deposit of clay was over 50 miles away and would require purchase of land or clay from the owner plus the daily delivery costs to the plant site. This situation made the plant site economically questionable. It was decided that a local clay deposit, which was available at the plant site but was orginally not considered due to lack of chemical uniformity and its high free silica content, would be reconsidered.]

The multiple-criteria decision analysis is a technique that helps to find an optimal solution by evaluating conflicting criteria. In this study, an approach to perform multiple-criteria decision analysis for cut-off grade optimization, which considers the diverse stakeholder groups prevalent in the mining industry, is presented. The optimization model developed in this approach links the resource block model to the financial model, enabling the process to either maximize profit, net present value, revenue to the State, or extend the mine's life. To identify the optimal cut-off grade in each case, the Excel Solver function is utilized. Although it has limitations, the Solver function can still be modified to handle models with thousands of block values for determining input variables based on the target value. Following stakeholder engagement and dialogue, a single weighted average cut-off grade can then be determined. This approach is demonstrated for nine South African Witwatersrand gold mines.

By offering savings in space and capital cost compared to ducted axial and centrifugal fan systems, horizontally mounted jet fans are commonly used worldwide to ventilate vehicular tunnels. They discharge high-velocity jets to transfer momentum to the surrounding airstream to induce longitudinal flow in the same direction as the jet. Vertically mounted jet fans installed in exhaust shafts have been successfully used to ventilate a large passenger rail overbuild in New York City. A comparison of shaft dimensions, vertical placement of the jet fans and measured shaft airflows is presented to evaluate optimum jet fan configurations. Measured shaft airflows are compared to airflows predicted by computational fluid dynamics (CFD) simulations of a three-dimensional model of the overbuild.

In the summer of 1990, a study was conducted to document large tractor (i.e., Caterpillar Dl1N) utilization in surface coal mining. The tractors were found to be utilized as primary production machines and in combination systems with blast casting and draglines and blast casting and excavator/truck systems. In addition, dozers were used to prebench ahead of a primary dragline system. The study also established ranges and averages for Dl1N operating costs and mechanical availability. Based on simulated production of various tractor dozing applications, the potential for low cost coal stripping was suggested (Hrebar, 1991). A follow-up study, also funded by Caterpillar Inc., was conducted in the summer of 1991 to determine actual production rates in two dozer stripping applications. These actual production rates would then serve as a foundation for estimates of stripping costs. This paper presents the results of the studies in a contour and a cast application.

"On January 9, 2014, a large chemical spill occurred on the bank of the Elk River near Charleston, WV. Within hours, the potable water supply for more than 300,000 people was contaminated, and the incident captured national headlines for weeks. Although the primary chemical, crude 4-methylcyclohexanemethanol, or MCHM, is deemed nonhazardous, the impacts of the spill are significant – with all stakeholders, including the public, members of industry, and relevant government agencies experiencing losses. This case highlights the need for a more proactive approach to environmental management, shifting from crisis control and remediation to crisis prevention and risk management. INTRODUCTION On the morning of January 9, 2014, approximately 10,000 gallons of crude 4-methylcyclohexanemethanol (MCHM) leaked from a 48,000-gallon storage tank sitting on the bank of the Elk River near Charleston, West Virginia (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal, and is deemed nonhazardous in terms of transportation and storage regulations and also toxicity (CDC, 2014a). Freedom Industries, Inc., a producer and vendor of specialty chemical products, had acquired the storage facility where the spill occurred from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein & McWhirter, 2014). The spill site was located just over a mile upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a “Do Not Use” order was issued for all customers (WVAW, 2014a). With little information readily available regarding the potential human health effects of MCHM, the US Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the “Do Not Use” order remained in effect for a total of eight days, until January 17, when the WVAW distribution water was confirmed to have MCHM concentrations below this level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at concentrations as low as 0.55 ppb by individuals experienced in odor identification (Rosen et al., 2014); and even individuals with no such training can detect pure MCHM at just 7 ppb (WVTAP, 2014). The fact that this chemical can be sensed at such low levels – indeed far below those considered safe – undoubtedly added to growing public concern. In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until February 25 when MCHM levels were measured below 2 ppb (WVAWb, 2014), and eventually replaced the affected filter media (WVAWc, 2014)."

The USBH Mining Systems and Ground Control Generic Research Center funded a project leading to the improved planning of airflow and refrigeration in underground mines experiencing hot working conditions. The project was broken down into approximately five phases: 1. Visiting some of the known hot mines in the U.S. and sending out a questionnaire to determine other mines which might be experiencing hot working conditions in the future. 2. Choosing two mines in which to conduct measurements to determine the insitu rock thermal conductivity and diffusivity, and comparing these values with laboratory determined values. 3. Investigating computer programs available for mine climate simulation, and testing these programs with case study examples from the two mines where the rock thermal properties were measured. 4. Considering improved ways of estimating the wetness factor and experimenting with the use of heat flux meters to more directly measure the rock heat inflow into the airstream. 5. Applying the results of these investigations to simulating the climate for future mine development and production areas at the two- mines. The project is approximately two thirds completed. At the time of writing, the insitu measurements at one of the mines are still being recorded. This paper will be describing the procedures and results of the insitu rock thermal property measurements, and give a brief description of some of the simulated results obtained for a mechanised cut and fill stope case study from the Homestake Gold Hine in South Dakota.

The Federal Coal Mine Health and Safety Act of 1969 had far reaching effects upon many aspects of mining, ventilation being no exception. In this paper, good ventilation procedures are briefly reviewed, and attention is focused on the changes brought about by the new Act. By way of illustration the ventilation system for a typical Pittsburgh Block system is developed and the requirements of the fan determined. This is then solved by an available computer program, and the application is evaluated. Figure 1 represents a projected system for a coal property 1 by 3 miles in plan at a depth of 1000 feet with a coal thickness of 6 feet and desired daily tonnage of approximately 7200 tons on a two-shift basis. Only one half of the mine is shown (north area) with a mirror image mining arrangement to be developed concurrently for the south area. Six working units are projected, three in each area, one on pillar extraction, another developing panel entries and a third on main entry development. A two-split system is used for each working section so that the continuous miner and bolting crews each have a separate split of air, and no equipment need pass through a line brattice. The old requirement of 6000 cfm at the last open crosscut has been increased to 9000 cfm, and for the first time a minimum quantity (3000 cfm) is stipulated at the face thus requiring either line brattice or an auxiliary fan. With the respirable dust requirement limited to 3 mg/m3, the use of a blower system is precluded and exhaust systems are virtually the only choice. Maintaining the line brattice within 10 feet of the working face at all times as required by the new law has a definite effect on the mining cycle and is most difficult at best. In the highly gaseous Pittsburgh seam located in south-western Pennsylvania and northern West Virginia quantities of 20,000 cfm per split are not uncommon to maintain the methane percentage below the required l%

Particle size segregation is a common phenomenon in many particulate processes and has many undesirable effects. The size segregation which occurs on a belt conveyor causes a serious problem for on-line particle size measurement using image-based techniques. Whether particle size distributions measured from the top layer of the belt are representative of the total particle population is largely dependent on the degree of size segregation. Therefore, it is important to understand the nature of such size segregation in order to make appropriate corrections for on-line particle size analysis. Monte Carlo simulation has been used to provide segregation information on particles with simple geometry and the results can help to determine important factors in experiments with irregularly shaped particles. It is expected that the relationship between the particle size distribution of the top layer and that of the entire particle population on a belt conveyor can be formulated as a model based function.

The Pyke asbestos deposits were discovered by Kennecott Explorations (Australia) Pty. Ltd., in 1969 during the course of base metal exploration on the South Island of New Zealand. Extensive occurrences of chrysotile asbestos were found associated with the Red Mountain Ultramafics in the headwaters of the Pyke River, and to the north and south along the ultramafic belt for a distance of 70 kilometers. Surface evaluation and drilling by Kennecott in 1970 and 1971 identified numerous small deposits of good grade fibre in the Little Red Hills between the Pyke and Barrier Rivers. Additional drilling and three adits have been completed by other companies since that time. The property is presently held by Lime and Marble, Ltd. Of Nelson, New Zealand, and Kennecott Explorations (Australia) Pty. Ltd. This paper will present the regional and local setting of the asbestos deposits, including the geology and nature of mineralization. In addition, the exploration methods used by Kennecott in the discovery and evaluation of these deposits will be outlined. Subsequent work by other companies will be mentioned, but not described in any detail. This subsequent work did not alter Kennecott's conclusions or produce any serious changes in the interpretation of geology and mineralization. The author acknowledges the contribution of the many employees of Kennecott Explorations (Australia) Pty. Ltd., who have been involved in the project, particularly Ramon Farmer, N. G. Comer, and A. W. McConie, who were responsible for completion of field work and reporting on the project. A literature review on quantitative analysis of mine safety studies revealed that investigators use a wide range of statistical techniques for the analysis of mine injury data. Researchers have addressed the various issues of accident causation and prevention through analysis of mine injury data collected from mines. These studies are generally based on historical accident/injury data without considering the injured person/population at risk. In this paper, a public health approach, based on scientific methods of epidemiology, is presented to address mine safety issues. Epidemiology is the scientific study of disease and injury patterns among populations in time and space. Originally, epidemiology was concerned with epidemics of communicable diseases. Then it was extended to non-communicable diseases. Safety researchers found that epidemiologic methods are the best tools for injury research, because the occurrences of injury and the occurrences of disease are quite similar. As a result, some researchers started applying this concept to injury research. This paper presents some of the techniques used in epidemiologic studies and illustrates the application of a case-control study design to evaluate the role of some risk factors associated with injuries in underground coal mines. In this study, the injured persons were identified and, simultaneously, interviews were conducted to find the significant risk factors responsible for occupational injuries in coal mines.
