"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

The study aims to evaluate the pillar behavior during the depillaring operation by continuous miner (CM) technology. A panel with five headings developed as per the coal mine regulation of Indian coal mines [2] has been selected for the study. The depillaring operation has been carried out using CM technology after widening and heightening the original entries. An extensive field investigation has been carried out throughout the depillaring operation to access the mining conditions. Depillaring operation has been performed smoothly up to the middle of the panel (fourth row out of eight rows of four pillars each). Advanced blinking of the auto warning tell-tale (AWTT ) has also been observed on some of the instruments during the splitting of pillar no. 18. The panel shows extremely difficult mining conditions at this stage. The case has been numerically simulated using three-dimensional numerical techniques. The simulation result has indicated the high stress and yield condition on the working and the barrier pillar (surrounding goaf from both sides) after the extraction of pillar no. 18. The analysis has also indicated the marginal stability of the 2nd barrier pillar (namely “pillar B”). The failure of the barrier pillar could be the responsible for global instability of the panel as observed in the field. Understanding the importance of the barrier pillars, panel design has been modified by incorporating an additional row of barrier pillars. The stability of the modified panel has been evaluated. Both working and barrier pillars have been observed stable in the modified panel. This could be an alternative layout considering safety at the working face.

In the mineral industry, the objective of a mining venture is to extract, process, and market a mineral commodity in such a manner that the profit of the venture is maximized. In a mining venture, a company normally has control over mine production scheduling, the selection and location of processing facilities (mineral beneficiation plants, smelting plants, refineries, or blending facilities), processed ore blending, and the selection of markets. The flexibility of the above factors provides many alternatives to satisfy the needs of each market with regard to quantity and quality of ore. The purpose of this paper is to present a mathematical model for determining the alternative that yields the maximum profit. Production planning is generally concerned with the best utilization of fixed resources, more specifically, with the determination of production and inventory levels to meet fluctuating demand requirements. A mine can be any source of ore such as an entire mine, a production unit within a mine, or an independent supplier. The objective of production scheduling is to determine the times at which each mine will be used and to allocate a required level of production. Production planning and scheduling problems are intimately associated with the location of new and existing mines and processing facilities. Although the location of the mines is usually fixed, there can be considerable flexibility in locating new processing facilities, in the selection of the processing plant, and in blending the ore before and after processing.

Resumen ¦ Un sistema de comunicaciones “a través del suelo” (TTE) comercial y disponible fue evaluado en una mina de carbón subterránea en la zona Este del Estado de Kentucky. Este estudio de campo fue realizado como parte de una evaluación multisitio mayor de sistemas TTE disponibles. La mina detallada en este estudio se encontraba paralizada al momento de la evaluación, lo que proporcionó la representación más cercana a la realidad del medio ambiente en una mina luego de una operación de cierre. Para este estudio se probaron las comunicaciones superficie a subterráneo y subterráneo a superficie. El despliegue estándar del sistema TTE usando los procedimientos recomendados por el fabricante logró obtener comunicaciones claras y nítidas entre el socavón y la superficie. Otras disposiciones de las unidades arrojaron resultados mixtos. Existen planes para examinar más los fenómenos observados en esta mina.

The surge of activity in the solar energy market, spurred by government incentives, has created new and more efficient solar technologies, but issues surrounding land- and water-use have backlogged the full-scale development of solar farms. This paper explores siting solar power plants on degraded or polluted lands, such as mine tailings impoundments. Historically, mining and has been integral to Arizona?s economy, but has dwindled in relevance due to urban pressures and environmental regulations. Currently, it is legal to site solar power plants on degraded or polluted lands, but the question remains whether the economics of this degraded lands solar development is more favorable than developing pristine land. Therefore, we explore economics of developing mine tailings impoundments at both the institutional and financial level. Methods of research for this paper were interviews with government regulators, environmental advocacy organizations, farmers, and mining company executives. Financial models using extant tax incentives show a positive net present value for the installation of solar power plants, even when accounting for the costs of land remediation.

The economic advantages of large diameter ball mills can be compromised by unexpected capacity limiting conditions. These arise because media rotational flow to ore axial flow ratios, and the number of mill revolutions that ore is subjected to during residence, are both inversely proportional to mill diameter. Because of this, mixing efficiency and grinding kinetics may decrease and become capacity limiting when mill diameters reach a critical range. There are two possible scale-up problems involved in the design and use of mineral processing machinery: • Selecting the size and operating conditions for available larger equipment, to insure that operating results will match those obtained with smaller units; or •Extending the size ranges of equipment beyond existing limits. Previous research at Columbia University into flotation machine hydrodynamics has demonstrated the importance in scale-up of internal flow relationships; these perform a similar role in grinding (Arbiter and Harris, 1980).

In response to recent mining disasters and new mine rescue team legislation, NIOSH researchers conducted meetings across the United States with mine emergency response experts to investigate current needs and issues. Some of the issues include emergency response preparedness, mine rescue contests, real-life training capabilities, and training facilities. Many new teams are being formed that must be trained for mine rescue team competitions as well as be ready to respond to a variety of mine emergencies including a fire or explosion, a massive roof collapse, mine inundations, or vertical shaft rescue situations. Therefore, it is important that these teams are adequately prepared and trained. This paper presents a summary of domestic and international coal mine rescue training facilities and identifies those that provide unique, real-life, and/or state-of-the-art training. Research findings from this report will be used in further NIOSH investigations to improve coal mine rescue training. Great Salt Lake Minerals & Chemicals Corporation (GSL), a subsidiary of Gulf Resources & Chemical Corporation, is located on the east side of the Great Salt Lake, approximately 20 miles from Ogden, Utah. GSL processes brine from the north arm of the lake to produce potash, salt cake, salt and magnesium chloride brine. Potash fertilizer, in the form of K2SO4 is GSL's primary product. The GSL Sulfate of Potash Complex The GSL potash complex (depicted in Figure I) is divided into a solar evaporation pond system and the process plants. The solar,evaporation system consists of a 17,000-acre (68.8 Km2) ponding operation where the various salts deposit during the summer evaporation season, and a harvesting operation which recovers the deposits during the remainder of the year. The harvest is either preconcentrated in the flotation plant or processed directly in the chemical plant where sulfate of potash (SOP) is produced. The chemistry of the lake brine results in a solar deposit con¬taining primarily kainite (KCl-MgSO4.3H20), schoenite (K2SO4?MgSO4^ 6H2O), and halite (NaCI), with some carnallite (KCl.MgCl2.6H20) and epsomite (MgSO4.7H20). Other minor minerals may be deposited, depending upon the weather during the evaporation season. Each year, the solar deposit in the evaporation ponds will greatly depend upon the summer weather conditions, which have in the past proven to be quite unpredictable. Weather may alter both the quantity of the solar deposit and the composition of this deposit. The Earth Pressure Balanced (EPB) shields designed by Mitsubishi Heavy Industries, Ltd., are recently more enthusiastically accepted by tunnel boring specialists. This company having built 1,000 tunnel shields utilizes its vast experience to resolve the problem to excavate through big boulders scattered in rather soft alluvial soil. Several types of EPB shields which can cope with large size boulders are described. Such shields are equipped with both drag bits and rolling disc cutters on the cutterheads. Previously removal of boulders has presented safety problems for workers while in the muck chamber, or venturing in front of the shield. This report describes the problem and how the equipment was modified to deal with the problem.

I speak to you today on the subject assigned to me, "Subsidence - a Real or Imaginary Problem", as a representative of Consolidation Coal Company (a wholly owned subsidiary of Continental Oil Company). The information I relate to you is our experience in the Pittsburgh Coal Company Division of Consol and covers the Pennsylvania properties and mines with particular emphasis on the mines in the south- western portion of the State or in the immediate vicinity of Pittsburgh, Pa. Mining of Freeport and Pittsburgh seams of coal in the Pittsburgh vicinity dates back over one hundred years. Consol, or its predecessor, Pittsburgh Coal Company, has been actively engaged in mining in the area since Pittsburgh Coal Company was organized in 1900. Pittsburgh, Pennsylvania came into prominence as the Steel City largely because of the vast reserves of high grade metallurgical coal readily available in the immediate vicinity and also because of its favorable river transportation. Early in the 1900's we operated over 100 small coal mines, with operations located in the Counties of Allegheny, Fayette, Washington, Westmoreland. We have also operated mines in Somerset and Greene Counties. Titles in Pennsylvania can be in three estates: (1) title in fee, (2) title to the mineral, (3) mining rights permitting the removal and extraction of the mineral with or without waiver of surface damage. Consol has over the years acquired good legal title to the coal lands purchased from individual farm owners, with good broad mining rights, and in 98% or more of its title has a complete waiver of surface damage or the right to subside the surface land. In the early days of our Company, individual coal farms were purchased from land owners. Many of these titles severed the coal estate from the in fee title and good mining rights with waiver of surface were conveyed in the original severance deed. Thus, minable coal reserves were put together by the purchase of the coal under many individual farms in an area. In the early years, the mines practiced hand dug mining - later going to mechanical cutting with hand loading of the coal. Systems used were entry driving with room and rib panels. We have practiced full recovery mining as far as practical during the years of hand loading, as well as in the later years when the mines were mechanized. Mining system layouts, methods and procedures have changed from time to time to keep pace with improved mining machinery and new inovations, such as roof bolting and rubber tired haulage.

Among the methods of mineral processing, gravity concentration methods are, probably, those which have lower costs, higher capacities and lower operational complexity. In order to be really effective, these methods require a feed size distribution in which the particles are liberated. Further, there must be a significant difference (usually higher than 1-1.5) among the real densities of the particles. However, in most cases, the traditional gravity concentrators (jigs, shaking tables, spirals and dense medium hydrocyclones) lose efficiency (sharpness of cut and recovery) when the mean diameter of particles are lower than 0.05mm and the difference among densities are smaller than 1. Most gravity concentrators developed in the last 20 years are based on the use of centrifugal forces that create large "apparent gravitational gradient" seeking to overcome the limitations of particles size distribution. This paper presents the experience of the CVRD Research Center in evaluation and development of new process based on using this new generation of gravity concentrators.

The scope of work included excavation, rockbolt/shotcrete/wire mesh primary support and cast-in-place concrete lining of the (1) 216 m, top flared shaft, connecting 1.57 radian elbow, and 590 m tunnel to a 10.67 m finished diameter; (2) complex double bifurcation section; (3) excavation, support, installation and backfill concrete for three parallel, 135 m steel penstocks of 5.79 m finished diameter; (4) construction of a 43 m diameter reinforced concrete intake/discharge structure atop the shaft.

"Pareto analysis was used to investigate the failures database of a Hitachi EX1900 hydraulic shovel over one year of continuous operation. The hydraulic system and hydraulic links, hoses and piping were identified as the most critical system and subsystem, respectively. A three-parameter gamma distribution function was identified as the best fit for a probabilistic modeling of the time between failures (TBFs). This distribution was used in a Monte Carlo reliability analysis of the hydraulic system. Autoregressive integrated moving average (ARIMA), within the context of a time series analysis of the TBFs, was proposed as a viable alternative to the traditional technique of distribution fitting. The mean absolute percentage error (MAPE) metric was used to compare the two models. The MAPE value that resulted from the ARIMA was 66 percent smaller than that from the traditional technique in predicting the TBFs. The silicate slag and copper matte that are the byproducts of smelting copper in a reverberatory furnace contain SiO2, FeO, Fe3O4, CaO and Al2O3, and Cu, Fe and S, respectively. These components influence the oxide (Cuox), sulfide (Cusul) and total (Cutotal) copper content lost in the waste slag. This paper uses multiple linear regression analysis (MLRA) to determine the degree of influence. The results of calculations obtained using MLRA were compared to actual measured results and found to have a large degree of correspondence, with the coefficient of determination R2 = 0.975. These results indicate that slag composition and copper content in the matte influence the copper losses in the waste slag with a probability of above 97%. It was determined that, of the total copper content of the slag, 74% is in sulfide and 26% in oxide form, and that this ratio is not dependent on the slag or matte composition.

Solution mining, primarily by heap leaching, has emerged as an important method for mining gold and silver in the United States. Heap leaching offers many economic and environmental advantages over conventional production techniques, in particular for lean ores and old mine waste rock. In order to more effectively pursue research on heap, dump, and in situ solution mining of gold and silver, the Federal Bureau of Mines conducted an extensive survey of leaching practices and problems. This presentation summarizes trends and statistics. Leaching is projected to provide 20 pct of primary mine produced gold and 10 pct of primary silver by the end of 1982. Current leaching practices in the United States are reviewed; operational problems and research programs to alleviate those problems are discussed.

Does your company reward its top people? Does coal or copper pay more? Production or engineering? How is the synfuels industry going to influence mining salaries? Do high salaries attract and retain the best people? Do modern compensation systems ensure fairness or destroy entrepreneurial spirit? With our tax system and high standard of living why are we so salary conscious? Salary is not only important for what it can purchase but it is also a yardstick, measuring how well we are doing. We all need money to pay the bills and maintain our standard-of-living, but once we reach a certain point the psychological ramifications become more important than the purely practical side. Even necessary payments like the mortgage and the car are a function of our egos. Are our choices of cars and homes based on simple transportation and shelter?

A recent analysis of 145 TBM tunnels of more than 1000km total length has helped to quantify the time dependence of utilization (U). In fact U = Tm, where T is time in hours and m is a negative gradient of deceleration. In poor quality ground, gradient m has a strong dependence on rock mass quality Q, for example m ˜ -0.5when Q = 0.1. Good investigation is critical for ensuring faster TBM tunneling then drill-and-blasting, which tends not to slow down (in m/hr) as the tunnel length increases. Anew design model called QTBM allows more reliable prognoses be made, and occasionally will disqualify the TBM solution.

Introduction The high-density sludge process was originally developed by Bethlehem Steel Corp. for use on acid mine drainage (AMD) and diluted waste pickle liquor discharges requiring continuous chemical-treatment. This work began in the early 1970s and was first put into practical use around 1973. To date, there are approximately 15 operating systems that have been installed to treat mine drainage in the United States. In addition, there are another 10 systems installed outside of the United States. The dense-sludge process is an innovative and improved method for treating acidic metal-bearing streams in a manner that minimizes the volumetric generation of sludge for disposal. The technology utilizes a sludge-recycle process that forms particles with a low affinity for water. These particles are unlike conventional metal hydroxide solids that contain tightly bound interstitial waters. Typical acid mine drainage streams contain high sulfate concentrations that combine with calcium ions in lime-based treatment systems to form gypsum crystals. Compared to the form generated in conventional lime treatment systems, the physical form of the gypsum is radically altered when the dense-sludge process is utilized. The characteristics of the metals particles are also changed in this process, as the metal hydroxides are converted to metal oxides. The dense-sludge particles settle faster, dewater more readily, are more easily pumped and hold much less water than conventional precipitates. In some cases, because the metal content of the sludge is more concentrated, it can be economically recovered. In general, the conventional and dense-sludge treatment processes are very similar. The changes required to implement the dense-sludge process are minor, and, typically, much of the existing equipment can be utilized. The net results of the process are: reduced operational costs associated with sludge generation, improved process control (particularly pH control), reduced gypsum deposition (i.e., less scaling) on system components and reduced sludge generation resulting in less return water for treatment. Despite the relative simplicity of the process, the application of the technology requires careful discrimination, and the "experience factor" for implementation is an important consideration that should not be over looked.

The vast coal resources of North Slope, Alaska can be expected to attract substantial interest because the nearly flat lying coal beds of the coastal plan are specially amenable to surface mining. Mining on a scale large enough to justify coal development in this area could have severe environmental effects. The mitigation would be uncertain because large scale coal mining and reclamation operations have not been carried out in the arctic The deficiency of the scientific data base is another factor that requires consideration if coal mining has to develop in accordance with the objectives of PL 95-87. The objective of this paper is to establish the interrelationship between the problems due to permafrost and factors in reclamation of post mining land in order to find ways to transform nature in a rational and purposeful manner and to suggest specific reclamation approaches that may be appropriate in this region.

This formal project management system along with its outsourcing philosophy has served the growth agenda of Cyprus Amax extremely well. These preceding factors, coupled with the in-house retention of a core project group that fully understands the breadth of skills and abilities needed to undertake development of multiple large projects successfully, have been the cornerstone of Cyprus Amax’s project development success.
