An analysis method for modeling compensation grouting based on the finite element method is presented. By this method the design parameters of compensation grouting, grouting pressure and grouting volume, can be evaluated. Also the subsidence of a building due to tunnel driving as well as the lifting of the building by compensation grouting can be simulated. The stress changes in the tunnel lining and in the foundation of the building are computed as well. The application of the analysis method is demonstrated on two examples for compensation grouting in combination with construction of tunnels.

It is important to know the breakage mechanism in materials since this knowledge influences the results of subsequent grinding operations. There are two distinct failure mechanisms in breakage: one is tensile micro crack generation at low stresses, which leads to macroscopic failure by disintegration, and the other is formation of shear zones under heavier dynamic impact forces, which generates more fines as seen in crush zones in blasting. Tensile fracturing simply breaks the material into fragments. It is seen as the disintegration of the specimen into two or more separate fragments. This happens under the absence of lateral stresses and the material is free to expand. On the other hand, compressive-shear breakage produces finer fragments due to shear stresses. The first mechanism is observed in laboratory tensile and bending strength tests and the second mechanism is observed both in laboratory brittleness tests and in situ blasting operations under dynamic impact forces. The friability of rocks and ores can be determined by a brittleness test. A test apparatus to determine the friability value has been designed to suit limestone strength characteristics used in cement production. The friability and stored strain energy values of barite, marble, limestone and bauxite have been determined and compared with the corresponding Bond work index (Wi) and grindability index (G) of these materials. The physico-mechanical properties of the tested materials have also been determined to investigate their effect on friability and grinding. The relationships obtained between the indices were in surprisingly good agreement, with high correlations (0.99 and 0.97). The Bond work index and grindability index can therefore be estimated from the friability value, which can be determined more rapidly than the Bond test. But for certain rock types such as andesites the relationships does not hold. Key words: comminution, tensile stress, shear forces, brittleness test, strain energy.

S. A. Tomich (Consulting Geologist, Perth) : I would like to commend Mr. Haycraft on his presentation on the economic geology of the North End of Kalgoorlie. This is an intriguing area which is in the Kalgoorlie field but not of it, inasmuch as it is separated spatially from the main Golden Mile by the Golden Pike Fault. Mr. Haycraft has stressed the differences-the mineralogical and structural differences of economic significance between the two areas.I would like to ask whether he is satisfied that the lithological sequence in the North End is the duplicate of that in the main Golden Mile area irrespective of the postulated effects of erosion, and more specifically whether there is any chance that the quartz-dolerite-greenstone of the North End may be in any way dissimilar from the quartz-dolerite-greenstoneof the Golden Mile which is tellurude bearing.J. A. Haycraft: In relating the quartz-dolerite in the Hannan's North-Mount Charlotte area back to the Golden Mile, we find some quite close similarities. Although the rocks are similar they are not necessarily one and the same.At Mount Charlotte we have three varieties of dolerite- a coarse-grained gabbroic dolerite on the eastern side of the orebody, the host rock medium grained and on the western side, a fine grained dolerite. Further west we have an amphibolitic variety of quartz dolerite.Recent drilling south of the Golden Pike fault on the Enterprise No. 20 level had indicated a remarkably similar sequence of rocks. We drilled west through a wide section of medium grained rocks into a fine grained rock and then into amphibolitic quartz dolerite before intersecting the Golden Pike fault. "INTRODUCTION Bauxite, in Appalachian Alabama, occurs in the Rock Run and Goshen Valley districts in Cherokee County; in Nences Creek Valley near Jacksonville; near Anniston in Calhoun County; north of Fort Payne in DeKalb County; near Ashville in St. Clair County; and near Talladega in Talladega County.In February 1942, Preston E. Cloud, Jr., assistant geologist, Federal Geological Survey, began a study of these bauxite deposits and the geology relating thereto.The Bureau of Mines established field headquarters at Piedmont, Calhoun County, in July 1942. Mineral agreements covering a total of 11,097 acres were executed with 15 landowners in the Rock Run district; 38 in the Goshen Valley district, covering 7,575.5 acres; and 3 in the Nances Creek Valley district, covering 988.5 acres. Drilling was done on the properties of 9 land¬owners in the Rock Run district, and on the property of 1 landowner each in the Goshen Valley and the Nances Creek districts. Drilling began October 26, 1942, and ceased on May 17, 1943.Power drilling was supplemented by a labor crew that drilled hand-auger holes in shallow deposits and sampled .old shafts, mines, and mine dumps.In all, 222 holes for a total of 13,250 feet were drilled. Of these holes, 88 penetrated bauxite but too low in grade to constitute ore."

Although for many years considerable attention has been given to devising safe and efficient methods of using explosives, with some success, relatively little has been done to provide satisfactory storage facilities for them. This was known long before the present war began, but it was emphasized during the past 30 months? inspections of explosive magazines by employees of the Bureau of Mines in administration of the Federal Explosives Act of December 26, 1941. These inspections have revealed woeful in adequacy as well as definite hazards and the urgent need for obtaining better storage facilities either by remodeling old magazines or constructing ink new ones that would afford suitable protection for the contents against theft, fire, deterioration, and detonation. The purpose of this paper is to offer suggestions and to show by sketches and drawings some of the fundamental features of suitable magazines. Type of construction and available materials differ with different; localities, but the information herein contained can be applied to most magazines, whether already constructed or the construct: on of which is planned. It is hoped that this information will be helpful and will aid persons who are trying to provide suitable storage for explosives end detonators. It is also hoped that States and cities will be made aware of the unbelievable laxity practiced in storing explosives in the United States and will take some remedial action.

The Hishikari gold deposit, discovered by the Metal Mining Agency of Japan, has become a giant class epithermal gold deposit with the highest grade of gold yet recorded in the world through detailed exploration and development by the Sumitomo Metal Mining Co Ltd. The exploration strategy involved a regional approach combining geological and gravity surveys over a 900 km2 area which had historically produced over 120 t of gold. These surveys were carried out to grasp the common geologic features of epithermal gold deposits in the survey area. A genetic model for epithermal gold deposits in the area under regional survey was also considered based on detailed research of the Kushikino epithermal gold deposit which was in operation at that time. This model was applied to the targeted areas, and important relationships between mineralisation, host lithology, intensive propylitic alteration and gravity highs became apparent. The Hishikari block was selected in this way and bonanza gold was discovered during drilling. Since the development of the Hishikari mine, information useful for further exploration for the same type of deposit has been accumulated.

Foskor operates a mine in the Northern Province of the Republic of South Africa exploiting the igneous complex at Phalaborwa. It produces about 3 million tons of phosphate concentrate per year by means of froth flotation from 26million tons of ore of which 16 million is mined and milled onsite. The remaining 10 million tons originate at the Palabora Mining Company and is pumped as a slurry to Foskor. Two types of ore are mined and milled namely Foskorite and Pyroxenite. (The main mineral content of this ore is Apatite, Phlogopite and Diopside.) After three stages of crushing the ore is milled using 24 rod mills and 5 verti mills. The apatite is then recovered using four stages of froth flotation. The stocks of Foskorite ore are rapidly being depleted and the PMC tailings will also fall away by the year 2002 so that the whole mining operation will then be centred around the Pyroxenite ore. This will lead to substantial increases inproduction costs. The recoveries achieved in the flotation plant are largely dependent on the successful operation of the comminution section. Parts of this plant are now 35 years old and the advantages of re-engineering, new technology and automation can only be applied to a limited extent in an effort to reduce costs. It was then decided to investigate alternative milling methods, both wet and dry.

The grinding mill is probably the most common unit operation used in mineral processing. There is, however, much which can still be done in the way of optimization. This paper discusses the results of a well controlled pilot scale test in which the performance of a tumbling mill with a conventional rubber lining equipped with lifter bars is compared to that with a smooth rubber lining of the magnetic type. The results indicate that the smooth lining is about 14 percent more effective with respect to energy consumption. By optimizing the size of the grinding media, the mill capacity can be increased in many grinding operations. In this specific case the -20 mm ball charge required 26 percent less grinding energy than one with a -30 mm ball charge. The energy savings can. in most cases, be converted to increased capacity. The wear of the grinding media is difficult to assess over a short period test but the results suggest it is somewhat less when a smooth lining is used. The wear of the linings has not been included in this study but it is generally accepted that the use of the magnetic lining reduces the lining costs dramatically. Computer simulation using the Millsoft code, developed by Mishra e.a. confirms and explains the increased effectiveness of comminution when smooth lining and smaller grinding balls are used.

Introduction Some unique opportunities exist for industrial minerals in British Columbia and Alberta. The region's diversified geology, particularly in British Columbia, offers favorable environments for a rich variety of mineral resources. Many minerals have been produced for years. For some, however, the level of production is far less than the potential. Others have had production in the past, but are not currently produced. Still others are known to exist and may have been actively prospected. But they are not yet developed. In addition, a number of speculative possibilities have recently come to light. The potential significance is not fully confirmed, but is suggested in showings in favorable geologic settings. Location and geography British Columbia and Alberta are the westernmost provinces in Canada (Fig. 1). The region overlaps two major physiographic divisions of the country, the Cordillera and the Interior Plains. It also takes in a small segment of the Canadian Shield on the east. The entire territory is greater than 1.6 Mkm2 (618,000 sq miles). Most of British Columbia lies within the Cordillera, a mountainous belt some 800 km (500 miles) wide. The belt is made up of three main physiographic components, the Eastern, Interior, and Western subdivisions. The Eastern subdivision embraces the rugged, linear ranges of the Rocky Mountains, elevations between 2600 and 3300 m (8530 and 10,800 ft). They form a 100-km (62-mile) wide band straddling the BC-Alberta border and extending northwest across northern British Columbia. The Interior subdivision includes several ancient mountain ranges but is dominated by an intermountane plateau area of low relief (average elevation 1200 m or 3900 ft). The Western subdivision is dominated by the Coast Mountains (elevations between 1800 and 4000 m or 5900 and 13,123 ft). These form a band about 175 km (110 miles) wide into which are sculptured the spectacular fiords of the BC coastline. Alberta lies largely within the Interior Plains, a prairie region of low to moderate relief (elevations between 200 and 1200 m or 650 and 3940 ft) formed of nearly horizontal bedrock strata. The Plains surface rises gradually to the southwest toward the deformed belt of the Rocky Mountains, where the bedrock strata are upturned, folded, and faulted. To the east, the Plains merge with the Canadian Shield, which exposes the Precambrian crystalline basement of western Canada. The total population of the BC-Alberta region is about five million people. Minerals, forestry, and agriculture are the primary industries here. It has well developed infrastructures to service these industries. British Columbia is third among the Canadian provinces in metal mining - $1.1 billion in 1985 - and first in forest products. It is also second in hydro-electric power development. Alberta leads all provinces in total mineral production, largely due to its extensively developed petroleum and natural gas resources - $26.2 billion in 1985.

An important operational and environ-mental concern associated with in situ leaching activity is the balance between solution injection and recovery. Solution balance is an important indicator of the capability of a particular in situ hydrologic design to efficiently and effectively control and distribute leach solution within the target ore zone. The Bureau of Mines is investigating the factors that affect solution balance in an unsaturated ore deposit, as part of ongoing in situ leaching hydrologic investigations at the Cyprus Casa Grande Mine. Flow conditions in 58 injection and recovery wells at the Casa Grande In Situ Leaching Test Facility were monitored daily over an 18 month period. Changes in the balance between injection and recovery occurring during this period were evaluated using analytical and statistical methods. The results permit estimation of solution distribution and the volume of solution stored in microfractures and interstitial pore spaces within the deposit following 18 months of leaching. Approximately half of the difference between well injection and well recovery can be accounted for by solution movement through macrofractures into nearby drifts. Estimates of solution storage within the ore deposit at the end of 18 months of leaching range from 3.5 to 6.9 pet. of injected solution. These results also suggest that 13 to 26 pet. of the fracture/interstitial pore space in the deposit was saturated with solution after 18 months of leaching.

INTRODUCTION Radioactive deposits were observed in 1972 in some of the Upper Silesian coal mines. They were located mainly in the drains in galeries and on the inside surfaces of water pipes. They also caused some problems by accumulating in water pumps. It has been postulated that the deposits are produced by natural radioactive waters seeping from the rocks. Investigations were initiated to answer the following questions: - What is the composition and the amount of radioactivity in the deposits? - What radioisotopes are present in the water? - How are the radioactive deposits formed? - Do the radioactive waters also occur in other mines? - How does the radioactivity of the water depend on chemical composition? - What is the origin of the radioactive water? - Does the water and the deposits cause radiation hazards for miners? -How can the radiation hazard be reduced? METHODS OF MEASUREMENT Determination of Radium Isotopes in Water The commonly used methods of radium determination in water are either based on measurements of the radioactivity of 222Rn which is in equilibrium with 226Ra, or on the detection of alpha particles of the radium radioisotopes after chemical separation of radium from the water sample. The method based on radon activity measurements is very sensitive and does not require any chemical Separation, but it can be used for determination of 226Ra from the uranium series only, because the thorium daughter 220Rn has too short a half-life (55s to yield the required accuracy. The method developed by Goldin, 1961 [2] involved alpha-particle measurements in thin layers of RaS04 and BaS04 separated from the water. This method is not convenient for saline water and water with high barium concentration because the amount of barium carrier in this case is too large to obtain a thin layer of precipitate with sufficient activity. The Upper Silesian carboniferous waters are often saline with high barium content, so the method described by Goldin was not convenient for this case and it was necessary to change the detection system and modify the chemical preparation. The procedure developed by the authors for the determination of radium isotopes in water was as follows: - Depending on the Ba2+ content and the required sensitivity of measurement, a water sample of 200 cm3 to 3 dm3 was taken. - 10 cm3 of 0.25 M citric acid and 5 cm3 15M ammonia was added to form complex Ba2+ ions and avoid the immediate precipitation of BaSO4. (This was repeated as long as the addition of BaC12 did not form a precipitate.) - 1 cm3 of 1N solution of Pb(N03)2 as a carrier for radioactive isotopes of lead and 10 cm3 of 0.1 N BaC12 as a carrier for radium were added. - The sample was heated to the boiling point and the precipitation of RaS04, BaSO4 and PbS04 with 50% H2SO4 was carried out. - After several hours the sample was centrifuged and the precipitate was purified by washing with nitric acid and distilled water. - The precipitate was then redisolved in 20 cm3 0.125 M Na2EDTA and 3 cm3 6M ammonia and reprecipitated from the solution by dropwise addition of acetic acid to d pH of 4.5. At this value of pH, precipitation occurs only for the barium and radium sulfates, while lead and all other radioactive elements remain in the solution. The date and time of deposit precipitation was recorded. - The final barium-radium sulfate mixture was washed with distilled water and transferred to standard measurement vials. - Each vial containing a deposit had 6 cm3 of distilled water added and was then shaken vigorously. 12 cm3 of liquid gelling scintillator (INSTA-GEL UNISOLV-1 type) was then added and the vi 1 was shaken again. After a while the scintillator turns into a milky gel in which the deposit is uniformly distributed. - The standard sample of 226Ra was prepared in the same way. - The activity of the samples was measured using a liquid scintillation spectrometer. (In this case the TRICARB 3320 produced by Packard Instruments, was used). Tests run on standard radium solutions provided by Amersham Radiochemical Centre indicated that this method of measurement enables one to achieve an efficiency of almost 100% (within measurement error). For alpha particles no quenching effect was observed for the BaS04 concentration in the range up to 80 mg of BaS04 per 1 cm3 of liquid scintillator coctail (Fig. 1). Four oil discoveries were made in east and east central Texas during 1940, three of which represented new fields. In the Chapel Hill field, Smith County, oil was found where formerly only gas and distillate were produced. The three oil fields discovered are: Hawkins, Wood County; Pittsburg, Camp County; and Tehuacana, Limestone County. Important extensions were made to the following fields: Bazette, Navarro County; Long Lake, Anderson and Leon Counties; Cayuga, Anderson, Freestone and Henderson Counties; Opelika, Henderson County; and Talco, Titus and Franklin Counties. At the close of the year 54 oil and gas fields were producing in the district. There were 13 other fields, 11 oil and 2 gas, which have been abandoned. The East Texas field led the district in the number of completions for the year. During the year about 622 wells (oil and distillate wells included) were completed and 80 wildcats were drilled. Production and Proration During the year 170,424,685 bbl. of oil (distillate production included) were produced in the district—a decrease of 2.6 per cent from the 175,053,729 bbl. (distillate production included) during the preceding year. The district's production for the year 1938 was 182,369,484 bhl. As usual, most of the district's annual production was in the East Texas field, which produced 139,095,694 bbl. (distillate production in- cluded) during 1940, or 81.6 per cent of the total oil produced as compared with 141,333,317 bbl. (80.7 per cent) for 1939. The December estimated daily average production in the district was 424,565 bbl. The district accounted for 34.9 per cent of the state's annual production, compared to 36.7 pcr cent of the state's 1939 production. The decrease in production for 1940 may be attributed to changes made in proration schedules. Any material changes in the district's annual production may be considered for the most part as a reflection of proration-schedule changes affecting the East Texas field, since it is by far the most important field in the area. The entire state was subject to 71 shutdown days for the year; the East Texas field to 131; and the Rodessa field (Cass County, Texas, portion) to 5 days. No important changes were made in field rules and regulations during the year regarding the allocation of oil in individual fields in this district. Discoveries The Hawkins field is in southeastern Wood County in and principally north of the town of Hawkins. Development to date indicates that the field is somewhat of an elongated anticline trending slightly east of north and the proved oil area appears at this time to cover approximately 4000 acres. The oil production has been secured from the Woodbine sand, Upper Cretaceous; a considerable quantity of gas has been encountered in the Sub-Clarksville formation (Eagleford shale) in the higher part of the structure. The introduction of ovens for the production of metallurgical coke is believed to be due to L. L. Norton who operated an iron foundry in the vicinity of Connellsville, Pa. Persuaded by his foreman, an English immigrant named Nickols from Durham, L. L. Norton put up a 12-ft square oven which produced coke in 1833. The coal used was taken from a local mine at Mounts Creek. The oven seems to have been used in conjunction with the customary method of coking in mounds. It was in the Connellsville district also, in 1841, that two carpenters, Provence McCormick and James Campbell, formed a partnership with John Taylor, a stone mason, for the manufacture and sale of oven coke. The task of the mason was to construct the ovens, while the carpenters were to build the arks by which the coke could be taken by water to the market at Cincinnati. The following account of the enterprise was given by McCormick: "James Campbell and myself heard in some way that I do not now recollect that the manufacturing of coke might be made a good business. Mr. John Taylor, a stone mason, who owned the farm on which the Fayette coke works now stand, and who was mining coal in a small way, was spoken to regarding our enterprise, and proposed a partnership-he to build the ovens and make the coke and Mr. Campbell and myself to build a boat and take the coke to Cincinnati, where we heard there was a good demand. This was in 1841. Mr. Taylor built two ovens. I think they were about 10 feet in diameter. My recollection is that the charge was 80 bushels. The ovens were built in the same style as those now used, but had no iron ring at the top to prevent the brick from falling in when filling the oven with coal, nor had we any iron frames at the mouth where the coke was drawn. The top and mouth had to be repaired when they fell in. In the spring of 1842 enough coke had been made to fill two boats 90 feet long-about 800 bushels each-and we took them to Cincinnati down the Youghiogheny, Monongahela, and Ohio, but when we got there we could not sell. Mr. Campbell, who went with the boats, lay at the landing some two or three weeks, retailing out one boatload and part of the other in small lots at about 8 cents a bushel. Miles Greenwood, a foundryman of that city, offered to take the balance if he would take a small patent flour mill at $125.00 hi pay, which Mr. Campbell did. He had it shipped here. We tried it, but it was no good, and we sold it to a man in the mountains for $30.00, and thus ended our coke business." So successful did the coke subsequently prove to be in use that the three partners were asked to deliver more. Evidently they had had enough of the coke business, however, for they refused to have anything more to do with it. Few ovens were built between 1841 and 1855, and it is reported that in the latter year, "there were only 26 coke ovens along the river above Pittsburgh". Successful coke makers of these years included Mordecai Cochran, Richard Brookius, and Colonel A. M. Hill. It was the use of coke in 1859 in the Clinton furnace erected by Graff, Bennett and Co. in a plant on West Carson Street, Pittsburgh, that brought the real beginning of the coke-iron era in America. Here the successful use of Connellsville coke as a blast-furnace fuel was demonstrated beyond all possible doubt, and from the year 1859 the coking industry expanded tremendously. The main contents are (i) resources and its exploitation; covering status of exploration, reserves and resources, province wise occurrences, distribution, nature of mineralization and generation of fines, etc ; (ii) use of iron ore in iron and steel making, covering physical and chemical requirements for pig iron and sponge iron production; (iii) ore characteristics and beneficiation practice in vogue; (iv) projected steel demand and requirement of quality iron ore and provision thereof; (v) recovery of iron values from in situ low-grade ores and stacked fine rejects (-10/-6 mm) inclusive of slimes (-0.15 mm) from tailing ponds; (vi) agglomeration of fine concentrates preferably by pelletization; (vii) measures to augment iron ore availability for future requirements.

The viscosity of the CaO-SiO2-xMnO-yCaF2 slags (C/S=1.0; x=10, 40%; y=0 to 15%) was measured to clarify the effect of CaF2 on the viscous flow of molten slags at high temperatures. Furthermore, the Raman spectra of the quenched glass samples were quantitatively analyzed to investigate the structural role of CaF2 in a depolymerization of silicate networks. The critical temperature of the slags abruptly increased at 15%CaF2, which was confirmed to originate from a crystallization of cuspidine using XRD analysis. The viscosity of the slags continuously decreased by CaF2 addition in the 10%MnO system, whereas the viscosity of the 40%MnO system was not significantly affected by CaF2 addition. The effect of CaF2 on the viscosity of the slags was quantitatively analyzed using micro-Raman spectra of quenched glass samples accompanying with a concept of silicate polymerization index, Q3/Q2 ratio. A polymerization index continuously decreased with increasing content of CaF2 in the 10%MnO system, whereas it was not affected by CaF2 in the 40%MnO system. Consequently, the bulk thermophysical property of the CaO-SiO2-MnO-CaF2 slags was quantitatively correlated to the structural information.

Since 1980, more than 40 miners have lost their lives by being trapped and suffocating in stock-piles, bins, silos, and similar facilities. Mine Safety and Health Administration (MSHA) is concerned about these accidents, most of which could be prevented if management and miners appreciated the hazard. In February 1986, the coal industry had a suffocation accident that drew national attention. A group of engineers were standing on a coal stockpile. It gave way under their feet. Five of the engineers were buried in the coal and suffocated. As with all fatal accidents, MSHA investigated the cause of this tragedy. I also asked my staff to look into the history of similar accidents. I found out it is rare to have multiple fatalities due to suffocation in material. But, over the years, there has been a long series of individual fatalities from this type of accident. Here are some examples. In Oct. 1985, a coal mine employee in Virginia entered a hopper to collect a coal sample. The feeders were started while he was standing in the hopper. He sank into the coal and suffocated. In Sept. 1985, an employee at a Texas sand company climbed onto a surge pile, to check a problem with sand flow. When other employees found him, the sand had swallowed him. Only his face and one hand were showing. Though the victim's face was clear of the sand, he suffocated before rescuers could free him. Many people do not realize you can suffocate in this kind of situation, though your head is free. Every time you exhale, the material has room to shift and press harder against your chest. Then you have less room to inhale. If this goes on long enough, it gets to the point where you cannot inhale at all. That is what happened to the victim in this accident. In May 1985, a plant operator at a Wisconsin sand and gravel operation purposely slid into the drawhole of a surge pile. He tried to break up large rocks that were clogging the drawpoint. Material piled on the side of the surge pile slid down and buried him. The operator was wearing a safety belt and line. But he was covered by 3 to 3.7 m (10 to 12 ft) of material and could not be pulled free. This incident illustrates another point. A safety line can help. But it is not enough. Workers should never put themselves in a position where loose material can fall on them from above. In Jan. 1985, a young Utah coal miner was waiting for a ride home from a co-worker whose job was at the tipple. A jam developed in the coal bin. The men could not clear the jam from the outside. So the young miner jumped into the bin. He started digging with a shovel at the toe of the coal pile. It caved in on him and he suffocated. The young miner had only three months of mining experience. He had not been trained for work around the coal bin. Since 1980, 16 coal miners and 26 nonmetal miners have lost their lives in loose-material suffocation accidents. Suffocation accidents occur in many areas around mines - bins, hoppers, stockpiles, surge piles, and silos, among others. One recent accident occurred at a surface coal mine in Illinois. The victim was walking over an area that had been drilled and blasted the week before. He fell into a void that had been crusted over with earth. He too suffocated. And many materials have been involved in suffocation accidentscoal, crushed stone, sand, salt, different kinds of ore, ammonium nitrate pellets used in blasting, and a powdery byproduct of iron ore milling called calcine. In addition to fatal accidents, there are near misses. We do not know how many. Most of them are not reported. A person who gets trapped in material, but is rescued, is usually unhurt. MSHA sometimes hears about these accidents by chance, or because they are reported in the local media. We do know that near misses are happening far more often

As Australia’s mining industry painfully retrenches, mining executives and managers strive to remain viable by seeking significant productivity increases directly through process improvement and indirectly by improving leadership and workplace culture, the most powerful determinant of productivity. Culture is the combination of people’s attitudes and behaviour, together with leadership values and focus.Comprehensive new and automated data collection and management systems promise much for improving productivity, yet despite rapid advances in measurement and sharing of data, the use of data can reduce productivity unless behavioural aspects are considered and designed as part of implementation. This is because systems drive behaviour that in turn shapes attitudes to determine culture.Experience from many mine sites and other industries in three countries confirm that the most powerful drivers of people’s behaviour are the Measurement-Analysis-Reporting system and business leadership.This paper discusses fundamentals of practical, statistically valid process measurement, analysis and reporting systems before discussing associated leadership considerations for sustained improvement in safety, productivity, employee retention, leadership, accountability and commercial returns.The key to productivity improvement is improving core processes and important support processes. In turn, the key to process improvement is identifying the major process components and their interaction by measuring and then analysing their variation and level of performance. Understanding variation is often neglected yet is usually highly significant. It is essential that the fundamentals of statistically valid yet practical process and data analysis are understood and incorporated into system design to ensure productive, informed behaviour.The design and use of Measurement-Analysis-Reporting systems influences behaviour most profoundly and determines people’s effectiveness. Behavioural aspects are detailed by focusing on six primary drivers of human behaviour and five basic systems for managing businesses. These are integrated in a seven-step process and productivity improvement methodology that combines statistical, analytical, systems and behavioural factors.The paper includes development, implementation and tracking of a project plan for improving management and leadership systems to drive aligned behaviours.
