The State of California, Department of Transportation is currently retrofitting a major interchange near the San Francisco area. The project is located in a highly traveled area of Oakland with light rail commuter service near many footings. Over 600 micropiles are being used in conjunction with CIDH piles for footing stabilization. A specially designed Soil Nail pile will support footings for a new light rail utility structure. Pile testing features in this contract serve as points of interest for future foundation designs. The presentation focuses on substructure issues of this incterchange retrofit. Video techniques for monitoring exposure, such as NIOSH’s “Helmet-CAM,” employ both real-time dust monitors and mobile video cameras to assess workers’ respirable dust exposures. Many real-time personally worn dust monitors utilize light scattering sensing elements, which are subject to measurement biases as a function of dust type (size, composition, shape factor) and environmental conditions such as relative humidity. These biased and inaccurate dust measurements impair the monitor’s ability to properly represent actual respirable dust concentrations. In the testing described, instrument mass concentration data was collected using three different types of commonly used commercial off-the-shelf personal dust monitors and compared to a reference standard. This testing was performed in a calm air (Marple) dust chamber in which three units of each make and model (for a total of nine monitors) were used for each test. Equivalency factors (EF, a multiplier to match the Thermo TEOM 1400a reference instrument) ranged between 0.746 and 1.879 across all dusts and environmental conditions tested, and between 0.821 and 1.519 on the ISO test dust.

Issues between resource utilization and resource preservation often develop in connection with coal mining. One such conflict exists in eastern Utah where PacifiCorp and Energy West Mining Company are working with State and Federal agencies to address the resource recovery/resource preservation dispute. The effects of underground longwall coal mining on golden eagle (Aquila chrysaetos) cliff nesting sites have been studied at the Cottonwood Mine since 1986. Subsidence, escarpment spalling, and the nesting activities of the local eagle population were monitored.

In this issue, the Bureau of Mines announces the availability of the latest of its Mineral Perspectives series: ?MP-4, mineral Industry of Western Europe." This publication describes the mineral industries of 19 Western European countries. For readers unfamiliar with this new series, reproduced below and on the two succeeding pages, is the section dealing with the mineral industries of France. Information regarding acquisition of this publication is found inside the back cover of this issue. FRANCE France is a moderately important producer of minerals, including iron ore, coal, potash, fluorspar, and uranium. In addition, France is a significant processor of imported minerals, chiefly for its own consumption. In general, it has exportable surpluses only of a little iron ore (low-grade ore exported to Luxembourg) and some nonmetallics.

Fire in a coal mine can be disastrous on many levels. In most cases, when a fire is detected, mining operations cease and all personnel are evacuated. Underground coal-mine fires are difficult to extinguish, can burn for a long time, and often result in an extended period of lost production. In a worst-case scenario, the mine is permanently sealed and abandoned, resulting in lost jobs, lost natural resources, and possible devastation of the local economy. In some cases, seals placed within the mine to isolate the fire could eliminate the need for sealing the entire mine and the subsequent extended delay in returning the mine to full operation. Previous attempts to place remote seals to extinguish mine fires in active mines have been largely unsuccessful. The West Elk mine experienced an underground fire in early 2000. The fire could not be accessed and extinguished from the active mine workings. A method of constructing seals remotely by drilling boreholes into the affected area was needed to help isolate and extinguish the fire. This paper describes the process by which remote seals were constructed that successfully isolated the fire and provided a means of extinguishing the fire by flooding the area with water. The period of lost production in the West Elk mine was considerably shorter than is typical in similar situations.

Indian tribes own a substantial portion of the West's minerals. More importantly, Indian reservations contain coal, oil and gas, uranium, and oil shale reserves in unusually large and unencumbered blocks. Many tribes are now prepared to intensify development of their large reserves knowing that they can offer energy partners the unique opportunity to develop major energy facilities free of overlapping local, state, and federal permitting authorities. The tribes, therefore, can offer mineral developers both substantial reserves and an unusual degree of freedom from a multiplicity of government regulation. In the past, Indians and energy companies enjoyed only partial success as business partners. Indian tribes lacked the resources and know-how to act as equal partners with large oil and gas and mining operators. The tribes relied on the Bureau of Indian Affairs (BIA) to initiate and execute reservation energy projects, and as a result the tribes never developed adequate tribal structures and the expertise to responsibly manage their energy resources. At the same time industry made little effort to understand and learn the laws, customs, and institutions existing on Indian reservations. To the contrary, industry found it easier to bypass the tribes and deal with BIA officials. But the situation has changed. In 1975 the tribes organized an effective coalition, the Council of Energy Resources Tribes (CERT), which recently acquired substantial federal assistance to bring the tribes a level of technical and business expertise that begins to equal that of industry. Because the tribes have changed the rules of the game, industry must now change and learn to work directly with tribal governments. A working knowledge of Indian law, practices, customs, and policies must be acquired by energy companies before beginning negotiations with tribal governments for development of reservation energy resources.

The concept of using travelling magnetic fields created by linear motors was stimulated by Laithwaite who advanced ideas on motors with shaded poles (Laithwaite, 1968), rack and pinion motors (Laithwaite, 1970) as well as low speed, brushless motors and three dimensional motors (Laithwaite, 1973a and 1973b).Sudan (1963) and Sundberg (1969) had applied the concept to metallurgical processes. However, the use of travelling magnetic fields has not been used widely to produce conditions for the separation of magnetic from non-magnetic minera1s or even magnetic materials from non-magnetic materials, although some attempts were made by McCarthy (1922), Davis (1934), Dean, Gottschalk and Davis (1934) and Gottschalk (1934).Laithwaite (1973, a) while working with linear motors made suggestions for designing separators and in the work described here, the observations of Laithwaite and others are extended to determine the characteristics of the motor with a view to using it as a magnetic separator.

By March 2001, shrinking global markets, reduced consumer spending and declines in domestic manufacturing and industrial output ended the longest economic expansion in US history and pushed the nation’s economy into its first recession in a decade. Aggravated by the terrorist attacks of Sept. 11, the recession led to large reductions in the domestic production of processed mineral materials. Some of the most significant production declines in the US metals industry were registered by aluminum, copper and steel producers. They faced strong foreign competition, higher energy costs and lower prices for their products. However, homebuilding and other domestic construction sectors — major users of nonmetallic mineral products such as cement, brick, glass and stone — remained strong enough to help raise the output of industrial mineral materials slightly above previous year levels. A strong US dollar continued to weaken the competitive stance of US metal and nonmetallic mineral materials companies alike against foreign producers in markets at home and abroad.

"The pressure oxidation autoclave at the Goldcorp’s Red Lake Gold Mines (RLGM) was commissioned in 1991. Since commissioning, the circuit has been well maintained and its capacity increased to mechanical and gaseous oxygen limitations. With advances in available technology, opportunities to produce more sulphide concentrate and a changing business environment, an optimization project has been initiated at RLGM. A multi-phase approach has been developed to optimize both the operations and economics of the circuit. This paper describes the history and future of the autoclave circuit at RLGM.AcknowledmentsWe would like to thank the operations and maintenance staff at Red Lake Gold Mines for their support and openness of mind to enact changes to enhance the operation. We would also like to acknowledge the members of the integrated team from Red Lake maintence, metallurgy, operations and corporate technical services that did the hard work to push past the status quo in order to advance productivity.OpportunityThe Red Lake Gold Mines (RLGM) pressure oxidation (POx) circuit has been in operation for over 25 years processing the high sulphide content gold bearing minerals. Increased sulphide concentrate production from improved flotation recovery and increasing gold sulphide association at depth has led to the POx circuit becoming a throughput bottleneck. The opportunity exists to process increased tonnages of sulphide concentrates and increase gold production. In order to realize this value, it will be necessary to increase the throughput capacity of the POx circuit.BackgroundThe POx autoclave at the RLGM, commissioned in 1991, was Canada’s first gold autoclave. It was installed to replace the two-stage fluid bed roaster that was built when the Campbell mine and mill were constructed in the late Forties. The circuit was originally designed to process 12.5 t/d sulphide sulphur in about 50 t/d of Campbell flotation concentrate. Following the merger of the Campbell and Red Lake Mines in 2006, the POx circuit started processing flotation concentrate from both Campbell and Red Lake Mills. In recent years, the circuit has been processing about 18 t/d sulphide sulphur in 75-85 t/d flotation concentrate."

During the 1980’s, laboratories opened throughout the United States to serve the analysis needs of a rapidly growing asbestos abatement industry. The samples analyzed were either collected during the abatement of known asbestos-containing materials or were suspect asbestos-containing construction materials. Many asbestos analysts received minimal training and used analytical protocols that were industry specific. False positives (reporting a sample as containing asbestos when none is present) went unnoticed or had minor repercussions. The mindset of the industry was to err on the conservative side – it would be better to identify a non-asbestos fiber as asbestos than visa versa. With growing public awareness of the health hazards of asbestos, other materials began to be tested for asbestos content. Rock and ores began to be tested for naturally occurring asbestos by these laboratories using the same protocols or an in-house protocol. Analysts were given no additional training and accreditation, in most cases, it was not required. With no consistency in sample collection, testing protocol or expertise in analysis, significant discrepancies in final results can be expected. If we further complicate this scenario with misidentification of naturally occurring look-a-likes for asbestos, expensive false positives are likely. To avoid the repercussions of misidentification of naturally occurring chrysotile asbestos from look-a-like fibers in rock and ores, correct sampling and analysis protocols with more stringent scientific diagnostic criteria are needed.

Stillwater Mining Co. (SMC) is engaged in developing, extracting, processing and refining palladium, platinum and associated metals (PGM)from the J-M Reef. This geological formation is located in Stillwater, Sweet Grass and Park counties in southern Montana. Associated byproduct metals include rhodium, gold, silver, nickel and copper. SMC’s current mining operations are near Nye, MT. The company is currently developing a second underground palladium and platinum mine, the East Boulder project, about 21 km (13 miles) west of the Nye operations. A 5,650-m (18,550-ft) tunnel is required to gain access to the ore-bearing horizon (J-M Reef).

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. "The Cortez SAG mill liner design was recently modified to improve the mill operating energy efficiency. In addition to the expected benefits, the design change unexpectedly played a significant role of verifying the proposed grinding circuit expansion for the Cortez Hills project as well as illustrating current SAG mill bottlenecks. This paper will discuss the evaluation that addressed operator and maintenance observations, historical PLC data, and “crash stop” observations to calculate charge motion, packing between lifters, and slurry transport through grate and pulp lifters. The performance of the new shell and discharge end liner design will also be discussed as well as their role in the possible grinding circuit expansion.INTRODUCTIONCortez Gold Mines commenced operation in 1969 with its Mill #1 rated at a nominal 2,000 tons per day. Mill #1 was a conventional rod mill/ball mill circuit followed by an 8-stage CIP circuit. In the early 1990s the Pipeline deposit was discovered and subsequently Mill #2 was built to process the increased ore volume. Mill #2, commissioned by mid-1997, was a nominal 10,000 tons per day SABC circuit followed by an 8-stage CIL circuit for oxide gold ore. Operating and commissioning details of this plant have been published elsewhere (Powell, 1998). By late 1998, there was not enough ore volume to support both mills so the smaller, higher operating cost Mill #1 was put under its current care-and-maintenance status. A simplified flowsheet of the current mill is shown in Figure 1."

After the 1989 Loma Prieta Earthquake, the California Department of Transportation (Caltrans) initiated an overview of the Caltrans Bridge Design Specifications (BDS) and bridge seismic design practice in California. This overview is continuing under the direction of the Applied Technologies Council (ATC-32), to review and revise as necessary existing standards, performance criteria, specifications, and practices for design and construction of new bridge structures within California. The ATC-32 project began in August 1991 and is expected to be completed in 1995. Since 1990, Caltrans project-specific seismic criteria have been developed and implemented on the I-280 and I-480 projects in San Francisco. Caltrans and an independent seismic safety review panel continue to develop and implement these criteria on other projects. In 1991 and 1992, project-specific seismic criteria were used on the I-880 reconstruction project in Oakland. The seismic safety review panel considered the I-880 seismic design criteria a state-of-the-art methodology to ensure seismic safety against collapse of bridge structures subjected to a maximum credible seismic event. This paper highlights the 1992 "I-880 Reconstruction Project Seismic Design Criteria Review" report prepared by the Seismic Safety Review Panel and Caltrans Division of Structures and will compare pre-1989 BDS and AASHTO methodologies to the I-880 criteria.

In Eastern Kentucky, the Darby seam has had an extensive history of coal bumps and pillar bursts. The combination of high overburden, strong rock and coal, subjacent and superjacent mines, and retreat mining has produced incidences of violent pillar failure. The influence of these factors on pillar stability and the potential to bump is illustrated in two case histories. In the first case history, a bump occurred unexpectedly during the retreat mining of a five heading continuous haulage panel. Alternative cut sequences and pillar dimensions were analyzed to identify a geometry that would reduce the potential for future coal bumps or bounces during retreat mining. The viability of the alternatives was constrained by the reach of the continuous haulage system, the overburden depth, and the presence of thick sandstones in the main roof. Each alternative was numerically modeled cut-by-cut to examine the mechanism of stress transfer, as a pillar row is retreat mined. This approach highlighted which cuts were being made in highly stressed portions of a pillar and therefore more prone to coal outbursts. Validation of the numerical modeling was accomplished by comparing the cuts where bumps and outbursts had occurred underground to the highly stressed cuts shown within the models. Recommendations of the barrier pillar width between adjacent panels, pillar dimensions, and changes in the retreat mining sequence were proposed based upon the overburden thickness, geology, and the result of the numerical modeling. The second case history documents a nine-entry panel 3,600-feet long that was retreat mined a distance of 2,064-feet without a significant roof fall or roof-floor convergence. Mine management became concerned about the potential for a coal bump or a large roof collapse within the gob and the resulting air blast. The stress on the pillar line and within the remnant pillars in the gob was quantified through numerical modeling. Changes in mining practices on the current panel were implemented to avoid a bump in the active panel. A barrier was established between the current and the future panels to protect the future panel against abutment pressures from the void area. A change in pillar centers was proposed in the adjacent panel to absorb the side abutment pressure from the gob of the active panel.

Upon completion of the 6,200 m (20,340 lineal ft; lft) North Tunnel drive for the Portland East Side Combined Sewer Overflow (CSO) Tunnel project in Portland, OR, the slurry tunnel boring machine (TBM) had to be returned from the Port Center shaft at Swan Island to the Opera shaft at the main mining site for the 2,700 m (8,860 lft) South Tunnel drive. At the Port Center shaft, the main 513-t (565-st) TBM and TBM tailshield were hoisted out of the shaft in two pieces using four 200-t (220-st) strand jacks mounted on a 635-t (700-st) hydraulic boom gantry system supported by Emmert International?s custom gantry tracks designed to span the shaft. After lifting the TBM components out of the shaft, the gantry system loaded the TBM components onto two Goldhofer platform trailers, which then transported the TBM and tailshield to a barge using a roll-on/roll-off method.
