The company has identified the management of Health and Safety as a key capability to sustain future growth, and to add value to the business. It has set a target of being the best practice in managing Health and Safety.   This paper examines the company's activities in the Health and Safety area, and how it sees that value can be added by preventing and containing losses through accidents involving people, property, process and the environment. Solid Energy is confident that a Planned approach to managing Health and Safety can achieve this. It will explore historical events, strategic changes, Health and Safety systems, Benchmarking, Performance Measures, initiatives, progress to date and challenges for the future.

Longwall mining techniques were introduced into U.S. bituminous coal mining operations in the early 1960s. These techniques were gradually adopted by many mines during the 1970s. By 1983, considered to be the peak of the longwall mine census, there were 118 active longwall faces in 79 coal mines throughout the United States. Beginning in 1984, longwall faces began a decline in numbers that has continued to the present day (Fig. 1). The geographic distribution of longwall faces today is shown in Fig.2 according to region. A breakdown of recent longwall mine production by region is shown in Table 1. An approximate breakdown of present longwall face installations by product (metallurgical and steam) is shown in Table 2.

The distribution of copper between liquid silver and liquid iron, Fe-C, and Fe-C-Si alloys was studied at 1600°C. From the data and the activity of copper in silver obtained from the phase diagrams, the activity coefficients of copper in iron up to 3 atomic pct are log y, = 0.90 — 2.4 N, and y": = 8.0. The effect of carbon up to saturation is expressed as log y = 1.8 Ni. The effect of silicon up to 11 atomic pct is insignificant. SLOWLY rising copper content of steel being produced is focusing attention on the lack of information on the behavior of copper in liquid steel. Chou' measured the activity coefficient of copper in liquid iron through its distribution between liquid silver and iron. He obtained a value of 9.12 at infinite dilution. Chipman' calculated the activity coefficient of copper in liquid iron at 1600°C from the phase diagram and obtained a value of 12. Ameiln and Pfeiffer- investigated the possibility of removing copper from liquid steel through the use of lead, silver, and bismuth as solvents. They also calculated that the activity coefficient of copper in liquid iron would be increased five or sixfold by addition of 4 pct C. Langenberg and Lindsay' are investigating the effectiveness of lead and sodium sulfide in removing copper from liquid iron. The experimental method was similar to that used by Chou' and Chipman and Floridis." Silver, copper, and iron were melted together, the copper distributing itself between the two layers so that at equilibrium a,.,, (in Ag) - a,.,, (inFe) and N,.,, (in Ag) yc,, (inFe) (in Ag). [21 N,.,, (inFe) Thus, from the concentration of copper in each layer and the activity coefficient of copper in liquid silver, the activity coefficient of copper in liquid iron can be calculated. The effect of an alloying element such as carbon on the activity coefficient of copper can also be determined from its effect on the distribution, provided the new element is essentially insoluble in silver. The one serious limitation of this method is the increasing mutual solubility of silver and iron in the presence of a third component. The mutual solubilities of pure iron and silver are negligible, but increase with increasing copper concentration beyond the range investigated. Ag-Cu System—The activity coefficient of copper in silver at 1600°C had to be estimated, since direct experimental data are not available. Kawakami" measured the heat of mixing of Ag-Cu alloys at 1200°C by a calorimetric technique. Scheil' calculated the heat of mixing in this system by use of the phase diagram, assuming that the solution is random and that the heat of mixing is independent of temperature. Over the past three years the UK together with Sweden, Norway, Finland, Italy have been taking part in a EU funded Leonardo da Vinci programme. More recently EFEE have participated in the project known as the EU15/10/200715/10/2007Excert project which is aimed at addressing the perceived loss of explosives expertise in several EU nations. This could have implications for explosives safety and industrial competitiveness. The paper outlines the work that has so far been done in phase 1 and describes the work planned for the second phase of the project. The paper will describe work in the UK to develop competencies for all explosives workers in both the civil and defence sectors. These competencies in the form of National Occupational Standards have now been approved in the UK and are accompanied by a range of 24 new National Vocational Qualifications launched in May 2006. The paper will show how the standards define knowledge and skills and how these form the basis for vocational qualifications as well as higher education awards and professional qualifications. This UK initiative is now being considered in other EU countries and the project has now been extended to also include Germany, France, Portugal, Czech Republic, Lithuania and Estonia. To support the occupational standards and qualifications work is also being carried out to improve learning provision in the explosives area. In addition to formal educational and conventional training courses a series of workplace learning initiatives are being pursued as well as internet learning packages. Examples of these initiatives will be given.

A rapid, accurate method for predicting the dew points of gas condensate systems and their subsequent normal and retrograde phase behavior with pressure decline has been developed. The method predicts dewpoint pressure, volume percent liquid, condensate content of produced gas, and the liquid and gas compositions at pressures below the dewpoint pressure. Use of a modified form of the BWR equation of state, which was established by regression calculations on experimental data from several gas condensates, provides depletion behavior predictions that are significantly improved over predictions using K-values based on convergence pressures and Standing's correlation for density. INTRODUCTION Prediction of complex hydrocarbon-mixture thermodynamics is fundamental to reservoir and gas process engineering. In particular, accurate descriptions of phase behavior are needed to plan production operations for gas-condensate reservoirs. Although analysis of reservoir behavior during producing and cycling operations can be made for simple reservoir models, extensions of these studies to more complex reservoir systems are still difficult, both with regard to numerical techniques and the phase behavior of the fluids. This report presents a mathematical method for handling the complex phase relationships encountered in retrograde condensates; it represents an advance in reservoir behavior technology in that it will aid the development of closed-form mathematical representations of total production systems. A typical phase diagram for a complex hydrocarbon mixture is shown in Fig. 1. When the reservoir temperature T, is higher than the critical temperature Tc, and lower than the critical-condensation temperature Tct' the mixture is a gas condensate. Decreasing the pressure on a condensate while maintaining nearly constant temperature will result in two phases forming at the dew point. Further decrease in pressure will cause additional liquids to be formed. This phenomenon is called retrograde condensation. As pressure continues to decrease, normal phase behavior regions are encountered (liquid vaporizes as the pressure decreases) until the low-pressure dew point is encountered. When a gas-condensate reservoir is produced, gas is withdrawn and the volume of fluids in the reservoir remains nearly constant as the pressure declines. Heavier components tend to accumulate in the liquid phase as a result of retrograde condensation, while lighter components tend to concentrate in the vapor phase. The founder of social psychology is sometimes identified as Kurt Lewin. In a 1947 article, Kurt Lewin coined the term ‘group dynamics’. He described this notion as the way that groups and individuals act and react to changing circumstances. Lewin theorised that when a group is established it becomes a unified system with unique dynamics that cannot be understood by evaluating members individually. The discipline of social psychology really emerged out of World War II and tackled questions associated with human judgement and decision-making by the Nazis. For a comprehensive look at the development of social psychology look at Abelson, Frey and Gregg (2004).Social psychology is concerned with the way social arrangements affect human judgement and decision-making. What the author has done is apply the knowledge of this new discipline to an understanding of risk, safety and leadership. This paper will explain some of the developments in social psychology, such as the discovery of the bystander effect, Groupthink, bounded rationality, heuristics, risk homeostasis and the Milgram effect.The social psychology of risk tackles the way decisions are made about risk in their social context. Unless organisations understand that decision-making is socially conditioned, they are unlikely to be able to tackle the complexities of culture change or the development of risk maturity.The paper will offer a new understanding of why people do what they do, how risk makes sense and why a ‘dumb down’ approach to safety is dangerous. This preliminary bulletin was written as the first of a series of several on the significance of drafts in steam-boiler practice, the succeeding bulletins to be along the same lines but of a more advanced character. The conclusions arrived at are tentative, and are the result of a study of one of the many problems growing out of a general plan of the United States Government to increase the efficiency with which the coals of the country are being used. Greater efficiency requires better boiler and furnace design. The experimental study of drafts on which the conclusions presented in this bulletin are based formed part of the fuel-testing investigations that were carried on by the technologic branch of the United States Geological Survey and have been transferred to the Bureau of Mines. The experiments made seem to indicate that it is possible to double or treble the capacity of a plant without making any radical changes the furnaces and boilers. These increases require about double and treble the quantities of air to be put through the fuel beds and boilers. It also seems probable that rebafHing the boilers will often permit the capacity to be doubled or trebled, while still getting more steam than formerly per pound of coal for uses outside the boiler room. These experiments were undertaken with the object of clarifying ideas concerning the passage of air through fuel beds and boilers. Measured weights of air were passed through two beds of lead shot, series, one of which remained always the same and represented a boiler; the other being varied as to size of shot and depth of bed, and representing a fuel bed. Careful observations were made of the weight of air passing through the beds per minute. All data were plotted in many charts, so as to permit the study of them from several points of view. A number of laws were deduced bearing on the relative amounts of power required to force air through fuel beds of various thicknesses, composed of various sizes of coal, and through boilers of various lengths and areas of gas passages. An important part of the discussion relates to an increase in the capacity of boilers by increasing the amount of power applied to pressure and exhausting fans and thus forcing several times as much air through the fuel beds and boilers. may be possible, as a result of these investigations, to raise the rate of working the boiler heating surface to three or even four times its present value. Such an increase would undoubtedly mean new designs of grates, stokers, furnaces, and boilers, especially fitted for high rates of working. Fan equipments designed to supply three or four times as much air under several times the pressure would be provided with more efficient engines, thus giving an additional factor favoring high-capacity working. It must be borne in mind, as stated above, that the results are tentative. It will cost money to force gases at high speeds through fuel beds and boilers, and there will soon be pressing need of such quantitative data as will enable the largest possible part of the energy imparted by the fans to be advantageously utilized. Effective tools are not only essential for the design of practical and efficient underground mine ventilation systems but must also play an integral part in the operation of the mine. One such tool is VUMA-coal ventilation simulation software. VUMA-coal is Windows-based and has been specially designed for the simulation of underground coal mines and includes user-friendly coal specific interfaces and three-dimensional graphics designed to facilitate in the construction and analysis of networks. VUMA-coal can be utilised during the design phase of new collieries, be used to conduct ?what-if? and optimisation studies, and be integrated into the ?day-to-day? ventilation related operations. In addition, VUMA-coal can be used to simulate dust and gas distribution in a ventilation network, as well as predict the impact of control measures. This paper describes the interactive VUMA-coal ventilation simulation software and its underlying design features and outlines a case study of how VUMA-coal has been integrated into Anglo Coal?s Ventilation and Occupational Hygiene Engineering [VOHE] department as part of the standard planning and evaluation process in their operational mines as well as long term planning and future mining operations.

The PT. Adaro Coal Mine targeted in this research is located in South Kalimantan, Indonesia. The coal production of this mine is increasing year, with about 35 M tons of coal produced in 2006. This mine faces many ground control problems, especially instability of the footwall. Large-scale landslides occur often in the footwall; this may be because of weak rock and the effect of footwall rock weathering. Hence, prompt measures must be developed and followed in order to preserve the stability of the footwall and make this mine safe. Additionally, the final pit design is now being reconsidered because of the increasing demand for and high price of coal. With these issues in mind, this paper describes a solution for improving the stability of the footwall and discusses the final pit design based on the results of an on-site investigation and numerical analysis.

"The primary objective of the Barrick Goldstrike expert system is to achieve the desired liberation size for the autoclave circuit while maximizing throughput. In addition, when downstream limitations (autoclave circuit or others) restrict the milling circuit tonnage the expert system further improves grinding product size. Since its implementation, the expert system has provided expected throughput improvements while improving or maintaining circuit performance.Barrick Goldstrike and MinnovEX Technologies Inc. teamed up to install an expert system to control Line I and I1 of the autoclave grinding circuits. The dissimilar equipments of the two grinding circuits coupled to different production objectives, skepticism among operators, and the variability of the ore types processed in each line challenged the implementation.To ensure implementation success, Barrick Goldstrike dedicated four administrators full time to the project. The system administrators included two metallurgists, a process control engineer and a mill trainer. As usual, the control room operators were the experts who provided the operational logic for the foundation of the control system. The administrators and all eleven of the control room operators signed off on acceptance of the system. Current online utilization is 95%.INTRODUCTIONThe grinding circuit for Barrick Goldstrike’s autoclave process consists of two SAG Mill Ball Mill lines. Line 1 grinding circuit is comprised of a 22 by 8 foot, 2500 horsepower Allis Chalmers (AC) variable speed SAG mill, followed by a 12.5 by 14 foot, 1250 horsepower AC ball mill, and a 13.5 by 18 foot, 1800 horsepower Dominion ball mill in parallel. SAG mill recycle from the primary discharge screen (+5/16 inch) is processed through a 1352 Nordberg Omnicone crusher (4.5 foot). A secondary SAG discharge screen (+1/8 inch) closes the SAG circuit. The ball mill circuit is closed with 6 Krebs super D20 cyclones. Line 2 grinding circuit is comprised of a 24 by 12 foot 4000 horsepower Fuller variable speed SAG mill followed by a 16.5 by 30.5 foot 5000 horsepower Fuller ball mill. SAG mill recycle from the primary discharge screen (+5/16 inch) is processed through a 1560 Nordberg Omnicone crusher (5 foot). A secondary SAG discharge screen (+1/8 inch) closes the SAG circuit. The line 2 ball mill circuit is closed with 12 Krebs super D20 cyclones. The underground Meikle ore is typically fed through Line 1. Achieving the desired product size is critical for successful processing of Meikle ore. Open pit Betze ore is fed to Line 1 when Meikle ore is unavailable. Line 2 processes Betze ore exclusively. Throughput is maximized when processing Betze ore as the product size is less critical. The products of these two circuits are combined as feed to the autoclave circuit. The ore types and the autoclave circuit demands determine the required grinding circuit product size and maximum combined mill feed tonnage."

"INTRODUCTION The so called Porto Maravilha Project consists of the re-urbanization of a 500 ha area in the Rio de Janeiro port region. Several changes in local infrastructure are being implemented, including a significant change in the road system, with the demolition of the former “Via Perimetral” viaduct and construction of 4 new tunnels: • Saúde tunnel - 35 m long (Waimberg et al, 2014); • “Binária tunnel” - 1012 m long; • “Via Expressa” twin tunnels - 1850 m long each way. The tunnels were excavated almost completely in rock using conventional drill and blast methodology and only some short stretches in weathered rock, associated to faults zones, residual soils and marine sediments close to the tunnel portals. During excavation of access shaft and “Via Expressa” and “Binária” tunnels significant ground settlements as well as settlements of buildings founded both on shallow and deep foundations were measured. These values could not be associated to conventional tunneling induced settlement troughs. This paper focuses on the “Via Expressa” tunnels, presenting a brief description of the project as a whole, its geological and geotechnical background, monitoring results, and a discussion of possible causes of the measured settlements. PROJECT DESCRIPTION The Porto Maravilha Project is an integrated multidisciplinary action on behalf of the Rio de Janeiro city council with support from the State and Federal Governments. In addition to creating new conditions for working, housing, transport, culture and leisure for the resident population, it also promotes expressive economic development in the region (Portonovo, 2015). Porto Maravilha Project is carrying out actions for the recovery of the region’s historical heritage, as well as promoting the population´s social and economic development. One of the main benefits of the Porto Maravilha Project is the construction of the new traffic system in the port area, consisting of the Express and Binário roadways. The new project, which will replace the current Perimetral Rodrigues Alves viaduct system, will increase traffic flow capacity by 27%."

"Explosions in bituminous coal mines are preventable. The combination of adequate ventilation, thorough rock-dusting, and water on the cutter bars of mining machines and of empty and loaded cars will certainly reduce the hazard to a minimum, if not remove it entirely. This is the attitude taken by the Woodward Iron Co. in lessening or removing the dust and gas explosion hazard from its Dolomite and Mulga mines.This report was made possible through the willingness of Frank H. Crockard, president of the Woodward Iron Co. to make records available and to authorize their publication for the benefit of the coal-mining industry.The compilation of these data and preparation of maps was facilitated by the cooperation of M. Heard, general superintendent of coal mines, J. B. Whatley, assistant general super-intendent of coal, and R. P. Whetmore, chief mining engineer.The Dolomite and Mulga mines are about 16 miles southwest of Birmingham, Jefferson County, Ala., on a company owned and operated railroad which connects ore mines, coal mines, coke ovens, and furnaces.In the Dolomite property there is a 30°, 860-foot, double-tracked slope for hoisting, three manway and air slopes and one air shaft, all in rock.At Mulga there is a 220-foot hoisting shaft, an air shaft and two manways, and material and air slopes, all in rock to the coal bed.The coal in both mines comes from the Pratt bed, which is the next coal above the Mary Lee bed in the Warrior Basin. The coal is horizontal or slightly undulating, with thickness varying from 36 to 72 inches and averaging 50 inches.The method of mining is essentially room-and-pillar with modifications for mechanized sections. The mines produced approximately 1,100,000 tons with 2,328,309 man-hours exposure n 1929 and 900,000 tons with 1,697,436 man-hours exposure in 1930.Through the work of the safety department the entire personnel in the coal mines, as well as in other departments, makes safety an essential part of efficient operation.Both mines are gassy; Dolomite produces about 334,000 cubic feet and Mulga about 919,000 cubic feet of methane every 24 hours.The ventilation maps, two of Dolomite mines (figs. 1 and 2) and one of Mulga (fig. 3) give as much of the details of ventilation as is practicable to put on maps of such scale. As will be seen from these maps there are two main ventilating fans in Dolomite mine and two in Mulga mine and the general direction of the air travel is indicated by long arrows to fan 1 and short arrows to fan 2 in each mine. Each fan has a gas or steam auxiliary drive and each mine has a complete fan and power unit ready for operation in emergency.In Dolomite, recently taken data show that fan 1 exhausts 82,000 cubic feet of air per minute at 3.2-inch water-gage pressure, with an analyzed methane content of 0.15 per cent, and fan 2 exhausts 115,500 cubic feet of air at 3.8-inch pressure and 0.10 per cent of methane. In addition, a Capell fan at No. 2 air shaft is a standby or emergency unit.In Mulga recently taken data show that fan 1 exhausts 83,000 cubic feet of air per minute at 3.0-inch pressure and 0.18 per cent of methane, and fan 3 exhaust 119,000 cubic feet of air per minute at 5.4 inch pressure and 0.35 per cent of methane. Fan 2 at No. 2 shaft is a standby or emergency unit for the Mulga mine.The ventilation is on a split system in both mines. All part split and total returns are measured, sampled, and analyzed weekly, requiring one man two days each week. During December, 1930, 77 samples were collected and analyzed in Mulga and 78 in Dolomite. Two weekly reports covering part of December, 1930, are given here so that they can be studied to show control of ventilation in the Dolomite and Mulga mines. In the event that a split sample shows more than 0.50 per cent of methane, immediate steps are taken to lower the explosive-gas content. The reports follow:"

Upgrading of a low-grade titanium slag (~55% TiO2) with high silica, calcia, magnesia and iron contents has been studied using sulphation roasting technique using SO2/air gas mixture followed by hydrochloric acid leaching. XRD analysis of the sulphated slag reveals the presence of kennedyite (Fe2O3. MgO. 3TiO2), rutile (TiO2), calcium sulphate (CaSO4) and calcium silicate (3CaO.SiO2) phases. The residue after leaching contains mainly kennedyite, rutile phases and free silica with traces of calcium silicate phase. Synthetic rutile that has about 83% TiO2 can be produced at the optimum conditions. Thermodynamic calculations give rational explanations of the obtained results.

Today?s modern mining operations have literally dozens of applications and systems generating data whose information very often, are not crossed or centralized for operational decisions. In remote locations, the information and situation awareness is typically limited to the location itself. In addition, access to that information is limited to a small, select group of personnel that have received training on how to interpret the results. When the situation affects another area of the mine then information is relayed to the applicable areas involved. Few workers have time to analyze the situation thoroughly, to see how it affects the mines symbiotic relationship within all areas of production. If the capability to correlate and measure events and situations that affect your overall key performance indicators are not in place, the opportunity to identify process improvement becomes more difficult.

Industrial Minerals production in New Zealand is limited in both quantity and range, but imported in quantities that make New Zealand the world's biggest consumer per head of population. The use of industrial minerals is assured as both population and standard of living increase regularly. But to develop these minerals in the New Zealand environment requires the mineral, or its market (both local and export), to have characteristics that negate the remote geographical location. Volume and/or uniqueness can achieve this.

"Detonation delay timing has been an important aspect of quality rock blasting for decades. Detonators that enable delay timing have improved over the years especially with the recent advent of precision electronic detonators. The positive effects of precision delay timing have been observed repeatedly in the field with improved fragmentation, heave and overall blast control. Three-dimensional computational capabilities in terms of software, material models and computer speed have recently made possible the computer simulation of multiple blast holes in a bench configuration with precise delay timing between holes. Some of the key physical phenomena enabled by precise delay timing include: 1) collision and interaction of stress waves produced by adjacent blast holes, 2) reinforcement of stress waves from multiple holes as they reflect from the bench face and ground surface, 3) reflection of stress waves from “dynamic free surfaces” created by previously detonating holes, and 4) deformation induced fracturing, cracking and fragmentation. Simulating rock blasting in 3D has shed light on the physics involved and is resulting in an evolution of our understanding of fragmentation mechanisms. Routine use of these 3D computational tools will result in a much better understanding of the complex physics set in motion by the rock blasting process and allow predictionof improved blasting."

An engineering investigation was carried out on the technical and economic feasibility of implementing flexible liner/backpacking support systems In an advancing tailgate longwall mine. The system for a model mine consists of a circular tube constructed of liner plates and flyash backpacking. Flyash is pneumatically stowed into the tailgate. Small amounts of cement may be mixed to improve the support capability of the fill In difficult local ground conditions. Water is added inside the nozzle immediately prior to spraying. Results of theoretical analyses and a field test show that the designed system is technically feasible, can be cost-competitive, provides more stable ground conditions, and can Increase productivity and coal recovery, when compared with existing retreat longwall systems. In certain mine layouts, the new system can reduce the direct mining costs by a significant amount. The system would be particularly applicable to deeper coal beds with bad ground conditions.

This paper describes a comparison of the fatigue failure responses of older versus younger lumbar spine cadaver motion segment specimens. These specimens were repeatedly subjected to loads simulating those experienced by the spine in lifting a 9 kg load in different torso flexion postures (0, 22.5, and 45 degrees of flexion). An older sample of 36 motion segments (average age 81 years + 8 SD) is compared with a younger sample of 18 motion segments (average age: 47 years + 9 SD) with respect to the number of cycles to failure and results of survival analyses evaluating the effects of flexion and bone mineral content with and without the younger data set. Compared to the older sample of spines, the younger sample exhibited many more cycles to fatigue failure in all the torso flexion conditions (10020 versus 8267 average cycles to failure in 0 degrees flexion, 7124 versus 3262 in 22.5 degrees flexion, and 3229 versus 263 cycles to failure in 45 degrees flexion). The increased cycles to failure in young specimens is likely due to the increased bone mineral content (BMC) in younger motion segments (30.7 g + 11.1 g per motion segment versus 27.8 + 9.4 g). Cox regression analyses modeling both flexion effects and BMC indicate that betas are similar when just older spines are modeled and when younger specimens are included in the analysis. Betas for survival analysis models with and without younger specimens generally show little change for flexion (2.716 versus 2.756 for 22.5 degrees [0 degrees referent], 3.926 versus 3.380 for 45 degrees [0 degrees referent]). Bone mineral content shows a similar protective effect in both models (Betas: -0.127 versus -0.087). Overall, comparison of the models indicate that risk ratios are similar for 22.5 degrees flexion, slightly increased for 45 degrees flexion and slightly more protective influence predicted for BMC in the combined sample versus the older sample alone.
