Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

This paper compares the modeling results obtained by using a single seed waveform (SS) versus multiple seed waveforms (MS) as inputs to the Multiple Seed Waveform (MSW) blast vibration model. The field data for the comparison was collected at a coal mine. The results of the comparison showed that the MSW model yields more accurate predictions and significantly higher correlation to field measurement when using MS as input. When using a single seed waveform as input, different seed waves result in different predictions of PPV. In contrast, using the multiple seed waveforms as an input, more consistent results are obtained.

There is a lack of information concerning the movements of water on the continental shelf, particularly with regard to data obtained regularly over significant periods of time. The Horace Lamb Centre for Oceanographic Research has recently deployed data acquisition systems on the sea bed off the coast of South Australia to record tides, swell, and ocean temperatures. Some of these early records are described. The swell data are pertinent to future offshore operations with drilling rigs, barges, and other equipment, while the temperature variations give evidence of significant bottom currents, apart from those due to swell.

A.): Recent regional mapping by the Geological Survey of Western Australia has revealed a total thickness of at least 30,000 ft for the Proterozoic sediments in the Hamersley Basin. In this succession the asbestos-bearing beds of the Brockman Iron Formation at Wittenoom are overlain by some 20,000 ft of younger sediments and volcanics. The use of industrial minerals by the chemical industry as raw materials for manufacturing and in processing spans a wide assortment of minerals. This chapter aims to supplement rather than duplicate the commodity chapters. Its objective, to describe the uses of industrial minerals in the manufacture of chemicals, is attained by considering minerals used by the chemical industry in terms of the chemical element required and by giving particular emphasis to market factors. The mineral aspect, which is covered in the commodity chapters, is subordinated and attention is focused on the chemical and market aspects. Condensing this broad subject into a few pages permits treating only the most important elements derived from industrial minerals. The elements derived from petroleum, hydrogen and carbon, which quantitatively dominate as raw materials for the chemical industry, are omitted, as are the metallic elements and the minerals covered in other "use" chapters such as phosphorous, potash, and nitrogen for fertilizers and titanium dioxide for pigments. After eliminating on this basis, our definition leaves six elements of major importance: boron, bromine, chlorine, fluorine, sodium, and sulfur. These elements are treated individually under separate headings. Salt brines have particular importance as raw material sources for the chemical industry as shown in Table 1 "Salt Brine Derivatives" (Anon., 1967), which charts the chemical compounds derived from four types of brines: (1) Owens Lake type brines which are sources of boron and sodium compounds; (2) Midland type brines from which bromine, iodine, and chlorides of calcium, magnesium, potassium, and sodium are derived; (3) Searles Lake type yielding boron, bromine, lithium, magnesium, potassium, and sodium compounds; and (4) Silver Peak type produced mainly for lithium. Table 2 (Jones, 1973) affords an overview of the chief industrial minerals, the chemical products derived from them, and end uses of the products.

"The Brunswick Mine ore requires fine grinding to achieve liberation of minerals. With 25mm slugs as the grinding media in the ball mill, there was a limitation of a P80 of 25 microns on the CuPb regrind circuit. Results from pilot plant testing showed that the separation efficiency could be increased through fine grinding with a P80 of 15 microns.Options considered were the capital expenditure for new fine grinding equipment such as a vertimill or the use of the present ball mill equipment with smaller media. The Wheelabrator Allevard Enterprise (WAE) millpebs product was chosen because it was a smaller media with a distribution of steel pellets between 5 to 12 mm.With millpebs the grinding product was reduced in size from a P80 of 25 microns to a P80 of less than 19 microns. The power draw to achieve the finer grind was 13% lower while the media consumption was 57% higher.The millpebs grinding media was also tested on the zinc regrind mill. The purpose of the trial was to understand what the media consumption and power draw would be with the charge lowered to achieve an equivalent grind as obtained with the slugs. The media consumption was approximately the same and the power draw was reduced by 30%.INTRODUCTIONThe Brunswick Mine ore is a complex Pb-Zn-Cu-Ag sulphide ore with the grain size of the minerals varying from submicron blebs to particles up to several mm in size. The ore is ground progressively finer in stages. The first step of grinding occurs in the SAG mill circuit. The product sizing 80% passing 95 microns is fed to the CuPb prefloat stage (Cormier et al 2002). The tails from the prefloat is then further ground to a P80 of 35 microns for the CuPb rougher flotation. The final liberation for the CuPb circuit takes place in the CuPb regrind circuit. The regrinding mill further liberates the cleaner scavenger concentrate, second cleaner tails and third cleaner tails. The reasoning for the progressive grinding is to achieve fine grinding without having galena over ground due to its high specific gravity that preferentially sends it to the cyclone underflow."

The Liberty Bridge in Greenville, South Carolina is a curved, cable-stayed, pedestrian bridge with primary support provided by two towers. The bridge is part of the Falls Park redevelopment. The bridge was originally designed to be supported on drilled piers. Alternately, the bridge was supported on a combination of micropiles and anchor piles because of difficult access and subsurface conditions. The use of micropiles and anchor piles led to improved construction scheduling and reduced costs. Micropiles were used to resist compressive loads. Anchor piles were used to resist tensile forces and to eliminate excessive settlement of pile caps into underlying fill during pre-stressing. This paper describes the site history, bridge construction, site and subsurface conditions encountered, and the decisions that led to support of the bridge using micropiles and anchor piles and foundation construction.

About 70% of the world?s total boron reserves are located in Turkey. Boron minerals are typically concentrated by attrition scrubbing followed by screening and classification to remove clay minerals and other impurities. These wet concentration methods, however, result in a considerable amount of accumulated tailings in large tailings dams. The tailings are approximately 28% solids and contain 14% B2O3. Problems caused by these tailings can be minimized by employing dry beneficiation techniques involving heat treatment. This method has been successfully tested on laboratory and pilot scales. During heat treatment, borax undergoes expansion in volume. The amount of expansion is dependent on the particle size, temperature and duration of heat treatment. Because the optimum temperature and the duration of the heat treatment are not well defined, the boron ore acts as a calcined clay mineral and is discarded along with the gangue. It is important to predict such parameters using mathematical formulations. Thus, a mathematical model is used to predict the required temperature and duration period prior to heat treatment. In the experimental study presented here, a series of systematic thermal tests was carried out in a muffle furnace. Two variables, temperature and time, were studied in a response surface design. A quadratic regression model was chosen and regression coefficients were calculated. Using a regression equation, the volume expansion of the particles in various circumstances can be predicted. The mathematical test results are then correlated with the experimental results to relate the optimum expansion of borax with heating time and temperature and with borax grades and recoveries.

A qualitative test program was sponsored jointly by the University of Alaska and Eriez Magnetics, in order to study placer gold nugget recovery using an eddy-current separator. Various gold nuggets from Alaska, ranging from 6.4 mm to 63.5 mm in size, were tested. All testing was performed using an Eriez Laboratory ESC, Model REA, fitted with 2000 gauss, surface field strength magnetic rotor. The belt speed was set to 91 meters per minute and the rotor speed was set to 2500 rpm in the forward direction. The splitter was adjusted to just reject the largest pieces of rock that measured an average 76 mm x102 mm x 127 mm. Based on the results of this limited test work, it can be recommended to placer gold mine operators, that Eriez Magnetics ECS technology is a viable and practical concentration technology for coarse gold recovery. Silver and platinum were also tested.

Anisotropic behaviours of OhshimaOshima granite were investigated by carrying out uniaxial compression tests, together with observations of microcracks under an optical microscope. The elastic modulus tensor of Oshima granite at 10 per cent strength,E10, were calculated using theoretically formulation of elastic modulus tensor in terms of the second-rank crack tensor for pre-existing open microcracks. Furthermore, with the assumption of orthorhombic elasticity, the nine moduli of Oshima granite E10were determined by uniaxial compression test. Twenty-onemoduli of samples, calculated without assuming any symmetry, also showed orthorhombic features. Direction of the symmetry axes agreed well with the orientation of preexisting cracks. Comparison of crack tensor-based formulation results with the experimentally determined one show that the anisotropy of elasticity of Oshima granite is caused by pre-existing open microcracks, not by pre-existing healed microcracks and also crack tensor formulation can be used, as a first-order approximation at least, to describe the anisotropy of Oshima granite induced by pre-existing open microcracks. ZUSAMMENFASSUNG: Das anisotrope Verhalten des Oshima-Granits wurde mit Hilfe voneinachsigen Kompressionstests und durch die Betrachtung von Mikrorissen unter einem optischenLichtmikroskop untersucht. Various seam conditions had been examined during longwall production at RAG German hard coal mines. Based on the field data a prediction method was developed considering separation planes as influencing factors for roof falls at the face. The characteristics, orientation, and density of separation are important factors on the occurrence of roof falls. In addition to geological influences, the performance parameters and geometry of support must be considered and optimized for the prediction and prevention of roof falls in a longwall face. Based on detailed macro- and microscopic investigations of e coal seams considering various ranks of carbonization, the origin and characteristics of separation planes are determined and a strata-specific influence on roof fall is interpreted. It was assessed that the coal face fracturing and displacement caused by the stress depend on the floor and roof strata conditions due to activation of the existing separation planes. Field studies are shown giving an idea of influences by advance rates of the longwall face, support delay and hydraulic pressure to the roof control in longwall faces in depth of more than 3000 ft. The analysis includes the application of specific software developed for longwall roof fall prediction as a result of a European research project. The software allows a laminar graphic display of roof fall probability, which is used for recognition of risk areas and risk management.

Keywords: Pyrometallurgy, ROMA, Norway, furnace, fines The ROMA project is a competence project on optimizing the usage of raw materials and energy in the ferromanganese, silicon, and aluminium industries. One of the items investigated in this project is the effect of fines on the furnace, and the usage of agglomerates versus lumpy ore. From lab-scale experiments it is shown, that for the charge to agglomerate in the furnace, the charge has to be exposed to a temperature higher than about 1200°C, independent of fines content. The fines less than 1 mm also showed higher agglomeration properties, compared to 2?4 mm, which showed no agglomeration property at all. The properties of agglomerates are very dependent on the pre-treatment of the agglomerate. Sinter and pellets, which have been heated to high temperatures, show a better strength, but also a lower CO reactivity. Sinter also shows a lower melting temperature compared to the ore.

A mine production locomotive, the world?s first applied industrial fuelcell vehicle, has been designed and tested in the context of a number of proof-of-concept projects supported by the Fuelcell Propulsion Institute, an international consortium of stakeholders dedicated to facilitate the commercialization of fuelcell vehicles. The locomotive?s simple electric-powered motor and controller system was ideal to study a number of requirements related to fuelcell power application: hydrogen power plant operation, underground hydrogen use, risk and regulatory requirements, production performance versus the conventional battery-powered locomotive version under measured duty cycles and production requirements, power plant design improvements for other fuelcell vehicles, and cost-benefit aspects. This article will focus on the design and testing aspects of this project which has led to successful performance demonstrations of the vehicle under surface and underground conditions at two mines. It will also present information on the creation of a safe, reliable and efficient power plant which fulfilled its design objectives, including longer productivity time for a given vehicle fill-up.

Stope support systems, typically consisting of props and packs, are used extensively in the gold and platinum mining industry to stabilize the rock mass in the excavation vicinity and to reduce the hazard associated with rockfalls and rockbursts. The design of stope support systems was historically based predominantly on past experience and practices, and cost considerations. The recent development of a new and improved support design methodology, has led to the potential for significant increases in worker safety and support cost savings. This paper reviews some of the fundamental rock mass and support criteria that form the basis of the improved support design methodology. The considered design criteria are the height of potential fall; quasi-static stope closure rates; dynamic stope closure rates; compressive hangingwall stresses; discontinuity spacing, orientation and interface properties; effect of support length (stoping width); effect of compression rate; consistency of support performance; a real coverage; support spacing; and zone of support influence. The proposed support design methodology combines both the zone of influence and key block stability theories, resulting in optimized support systems for rockfall and rockburst conditions. The site-specific methodology consists of two stages: (i) a tributary area analysis, and (ii) a zone of support influence and a stability analysis, considering hangingwall failure due to buckling, shear and block rotation, which gives maximum safe spacing of individual support units. The methodology is suitable for designing support systems for both blocky hangingwalls and hangingwalls fragmented by face-parallel extension and shear fractures.

The Toulon tunnel project is a key component of the Highway corridor on the French Riviera between Marseille and Nice and is a fundamental transportation infrastructure for the region. The first design studies started in the 1960?s. The tunnel project is made of 2 separate tunnels: South and North tunnel. The North tunnel was built between 1992 and 2002 after 10 years of difficult construction (for example, on March 15th, 1996, a collapse of the tunnel in the ?Marchand Area? generated a sink hole in the city Center, causing 20 months delay). The South tunnel alignment runs parallel to the North tunnel at the same depth below Toulon historical downtown (20 to 40m depth with some areas with less than 10m of cover). The Risk management program was studied to be a State of the Art program in urban tunneling construction. The Hydro Geological Context The whole project area is covered with backfills and colluviums of 1 to 4m thick. The Schist, marls and sandstones, randomly distributed along the alignment below the first layer have different characteristics. While the Schist are pretty compact, the marls are fractured. The other layers have very different characteristics, and many faults are located along the alignment, especially in the limestones. The Breccia are heavily decompressed and deformable, as well as the other soils located in the Marchand Area. This 80m long area is where the settlements need to be controlled with an extensive monitoring program. The complete Geological profile is presented in Figure 1. [ ]

When one looks back and does assessments of what has been the key ingredient in achieving success in our safety efforts, there are many issues that spring to mind and cover a wide array of elements. One does not know which has the ultimate maximum effect. In his presentation yesterday, Alan Peers, our SHEQ leader, covered a lot of detail with the way we are managing safety at Modikwa. I have attempted to identify some of the key essentials in making the mine safe. Passion Safety is addressed with a passion that is unbelievable at times. When I get phoned because an incident has occurred and in reporting the incident, there are already action plans being put in place to prevent re-occurrences, you understand the commitment to put employees? safety first. You hear the disappointment in your subordinates? voices when they have to share the fact that an incident or accident has occurred.

Mintek was contracted by Oriel Resources plc in 2004 to conduct a preliminary assessment study on smelting ferronickel from the large but relatively low grade Shevchenko nickel ore deposit in Kazakhstan. A detailed feasibility study (?DFS?) was awarded to Bateman BV following promising results from the preliminary assessment study (?PAS?) and small-scale pilot plant test work. As part of the DFS, a total of 450metric tons of ore was collected from three different locations in the deposit and shipped to Mintek. After sampling, the bulk sample was prepared for smelting. This comprised crushing, screening and calcining with each step done separately for the three different lots. The chemical composition of the ore from the deposit is relatively high in silica and therefore falls out of the MgO to SiO2 range that is normally processed by pyrometallurgical means to produce ferronickel. The main objective of the testwork was to demonstrate that a method could be found and successfully applied to smelt these siliceous ores and produce an acceptable nickel-containing alloy. A 2-m ID DC arc furnace was installed with a twin-cathode (electrode) configuration to replicate the proposed commercial-scale furnace and also incorporated an advanced design of water-cooled copper panels in the lower furnace sidewalls to withstand the aggressive nature of the high SiO2-containing slags. The reductant used was a high-volatile coal from Karaganda in Kazakhstan. The reductant addition was adjusted to achieve a 90% recovery of nickel. In the assessment of the process metallurgy, some deviation from equilibrium conditions was observed. This deviation is ascribed to the intermittent existence of a smelting bath partially covered with feed during the testwork. Optimization of smelting conditions should result in the metallurgy (slag metal equilibrium) being closer to the theoretical predictions and make the production of a 20% nickel alloy possible at 90% nickel recovery. The solution for the successful processing of these siliceous ores was found in the attainment of the necessary operational stability through the control of arc length, operating temperature, open-bath smelting conditions and flux additions of up to 10% dolime for the most siliceous of the three ore types. The test work carried out by Mintek provided the basis for the process flow sheet and engineering design of the DC arc smelting furnaces for the Shevchenko Nickel project.

Tunnel boring machine (TBM) development continues to take on not only larger diameters, but also more difficult ground conditions. Ground conditions consisting of unstable faces with water pressures up to 15 bar have required innovative design approaches. A systems approach has been required to produce tunneling machines that can safely excavate the tunnel in an efficient, cost-effective manner that stresses production and labor saving approaches. This article reviews the current design developments and present recent projects where these most recent principles have been applied. The world?s growing population and rapidly proceeding urbanization are boosting an enormous demand for new and high-capacity infrastructures to secure the mobility of goods and people. Densely populated areas face the challenge of providing efficient traffic infrastructure such as a modern public transport system. At the same time, the maintenance and modernization of supply and disposal structures for water, sewage, energy and communication are essential. Last but not least, are supra-regional transportation routes such as long distance water diversion schemes. All of these projects are as demanding and are characterized by a tight time schedule. In this context, the demand for efficient tunnels for traffic and utility lines is increasing, as new infrastructures often leads underground because of limited space above ground. Also, tunnels are the obvious choice to cross natural barriers like mountain ranges. To handle the complex and variable geological conditions and other constraints, such as low overburden, high water pressure or long tunnel drives while facing increasing demand of a quick completion of a project, mechanized tunneling sets standards regarding safety, profitability and environmental protection in traffic tunneling projects as well as in the trenchless construction of utility tunnels. Different machine types and working principles have been developed to master various geological and hydrogeological requirements. Continuous activity in research and development leads to constant improvement of the mechanized tunneling process.

The more than 40,000 mining industry professionals that are expected to attend the National Mining Association’s(NMA) MINExpo International2000 trade show and conference will be treated to the exhibition’s largest display ever. The exhibit is expected to cover more than 60,385 m2(650,000 sq ft) and feature more than 1,300 exhibitors. The event is set for Oct. 9-12, 2000 at the Las Vegas Convention Center in Las Vegas, NV. Richard L. Lawson, NMA’s president and chief executive officer, said, “MINExpo 2000 will showcase everything that mining is proudest of. This is the largest premier mining exposition in the world.” MINExpo is held every four years.
