"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Tunnel boring machine (TBM) tunneling is an ever-increasing prospect for underground construction, and, with each new tunnel bored, there are unknown elements. When boring through the earth, even extensive geotechnical baseline reports can miss fault lines, water inflows, squeezing ground, rock bursting and other types of extreme conditions. This article draws on the consider-able field service experience within Robbins to analyze successful methods of dealing with the most challenging conditions encountered. Many tunnel projects are located in areas with relatively poor access along the tunnel alignment and bored under extremely high overburden. These two factors often result in limited geological information. It would be reason-able to state that the deeper the tunnel, the greater the lev-el of uncertainties. When faced with these uncertainties everyone involved with the project including the owner, the contractor and the machine supplier must be prepared to tackle geological surprises. This article describes the problematic geological conditions and associated difficulties faced on three separate projects and the measures that were taken to overcome these difficulties.

This article advances an innovative concept explaining the connotation and technical system of "green" mining. The important theoretical foundations of "green" mining are the distribution and development characteristics of joints, fractures and bed separations, and the seepage and flow behavior of methane and water in the broken rock strata following coal mining. The main focus of "green" mining technique includes water-preservation in mining areas, coal mining under buildings with bed separation spaces grouted to retard surface subsidence, partial extraction and backfill mining, simultaneous extraction of coal and coal-bed methane, coal roadway supporting and underground disposal of rock waste, underground coal gasification, etc.

The Green Mountain Copper Prospect is an unusual stratiform base metal occurrence located in the California Foothills Copper-Zinc Belt. Mined for rich secondary oxide-carbonate ores since 1863, the first serious exploration of the primary ores was in the 1970's. Unlike typical volcanogenic deposits of the district, stratigraphy consists of greywackes overlain (?) or intruded by an intermediate to felsic volcanic unit. Shales and siltstones overlie the volcanic unit. Massive pyrrhotite, chalcopyrite and sphalerite occur at the top of the volcanic unit. Later intrusion and deformation metamorphosed the sequence and created a SW plunging anticline. Characteristic magnetic and electrical geophysical signatures are present and reviewed.

Lime addition to acid mine drainage waters (AMD), possibly accompanied by aeration, has been the basis for their remediation by precipitation of predominantly metal hydroxides. An efficient novel alternative technique causes precipitation of double M(II)-M(III) hydroxy sulfates which readily precipitate and are easily dewatered. When the Fe(II)/Fe(III) + Al(III) ratio of the AMD is appropriate, the Green Precipitate (GP) Process provides a more effective alternative to normal liming/aeration. The process can be adapted to give a continuous in-line treatment by the choice of suitable computer-controlled reactors designed to improve the liming efficiency, reduce scaling and control the rate of treatment. The processes are described in terms of the chemistry involved.

"Kaiser Resources Ltd. has completed feasibility studies resulting in approval of $277,500,000 for the construction of the Greenhills Surface Coal Mine, located 35 km north of Sparwood, B.C. The mine is designed to produce 1,800,000 tonnes per year of metallurgical coal over a 20-year period by conventional truck /shovel methods.The project is in the detailed design phase, with some preparation work being carried out on the site. The major construction activities are scheduled to commence in the spring of 1981. Mine development and pre-stripping will commence in July 1981, with first coal production scheduled for mid 1983.This paper will be a general description of the project and will include a discussion of the geology, reserves, mine planning, coal handling and processing, surface facilities and related topics.IntroductionProject DefinitionThe Greenhills Project is designed to produce, by conventional truck I shovel surface mining methods, 1,800,000 clean tonnes of metallurgical coal per year for twenty years . The production/ life constraints were chosen as being meaningful for planning and evaluation purposes after appraisal of the market potential and are not limited .by economic reserves. Substantial additional strippable reserves are available which would facilitate expansion of the proposed mine life and/ or capacity. In fact , all the facilities are designed so that they can be expanded in the future to accommodate a doubling of annual coal production, and certain portions of the coal processing facilities are being installed with an annual capacity of 3,600,000 tonnes. The mine will also produce approximately 2,400,000 clean tonnes of oxidized thermal coal, mostly in the first decade."

The Greymouth Coal Project is a proposal to mine by the application of proven hydraulic, and room and pillar underground mining methods; and to process the equivalent of an average of 1.7 million tonnes per year over a forty-year period. The proposed mine site is located ten kilometres north of Greymouth, on the west coast of the South Island of New Zealand. Of the three separate mineable seam groups, the thick Main Seam Group, which makes up 77% of the total resource, will be mined by hydraulic mining technology.   It is proposed to market and produce a high volatile bituminous type coal with low ash (6%) and sulphur (0.4%). Because the inherent ash content is low, the requirement for coal treatment will only be the use of a single cell jig circuit to control the effects of rock dilution and partings. The resulting coal product is tentatively planned to be transported by rail to the port at Lyttelton for loading on to Panamax or Post Panamax size vessels.

"This paper describes some energy reduction measures Implemented at the Griffith Mine. Before proceeding, I will give you a brief overview of the operation.The Griffith Mine is an iron ore mining, concentrating and Pelletizing complex located in the red lake area of northwestern Ontario. The operation is owned by Stelco Inc. And is managed By Pickands Mather & Co. of Cleveland, Ohio. Operation of the concentrator commenced in 1968 and it has been producing at a nominal capacity of 1,500 ,000 tons annually.The plant flow sheet is illustrated in this slide. Run of the mine ore is crushed in a single stage 54"" gyratory crusher. Reclaim ore feeders draw from an open stockpile to feed two 32' diameter by 12' long autogenous mills, each driven by 6800 hp. Two s·rages of wet magnetic separation, roughers and cleaners, and primary cyclones complete the primary circuit. Cleaner concentrate is then ground in four 14' diameter by 28' long pebble mills rated at 2000 hp. The secondary grinding circuit is closed by cyclones and fine screens. Secondary cyclone overflow is further upgraded through hydro-separators, finisher magnetic separators and flotation.The magnetic concentrate produced is then thickened, filtered, balled and fired in three vertical shaft furnaces to produce a high quality hematite pellet."

"The Griffith Mine is located 200 miles northeast of Winnipeg and is 30 miles south of Red Lake, Ontario. Owned by The Steel Company of Canada Limited and managed by Pickands Mather & Co., it has an annual rated capacity of 1.5 million tons of high-grade iron ore pellets with an analysis of 66.3 per cent iron and 4.5 per cent silica.Development of the mine and construction of the plant got underway in January of 1966 and operations began in February of 1968. The orebodies outcrop on the shore of Bruce lake and run under the lake. The ore in its crude form is a metasedimentary low-grade magnetite and occurs in a highly folded banded formation. The average iron analysis of the crude is 29 per cent and the over-all weight recovery is 32 per cent.In order to mine the portion of the orebody under the lake, the soft varved clay on the lake bottom was hydraulically dredged and a dyke constructed of rock, with till used to provide an impervious seal on the lake side of the dyke, prior to dewatering.Mining is carried out by conventional open-pit tech-niques. Ore dressing is achieved by combining one crushing stage and a two-stage grinding system, with beneficiation by magnetic separation, hydroseparation and flotation. Agglomerating is done with conventional balling drums and pelletizing is carried out in shaft furnaces.Of interest and discussed in detail are the 32-foot autogenous cascade mills ."

"The response of the surrounding rock mass to the creation of mining excavations determines the ultimate load on a pillar support system. In conditions where the ground is relatively soft and weak, the full overburden weight can be transferred to the pillar system. However, in stiffer and stronger rocks, a greater portion of the overburden load is transferred to the unmined coal barriers or abutments, and the pillar stress is reduced. This paper makes use of numerical models to examine the interaction between typical pillar systems and the surrounding rock mass for weak and strong geological conditions at various spans and depths of cover. The concepts of structural failure and functional failure of pillars are used to assess pillar performance when pillars are deformed beyond their peak resistance. The results show that the span-to-depth ratio is an important factor in determining the pillar stress and the ultimate deformation of pillars. The ultimate pillar strain appears to be closely related to the functional success of pillar systems.INTRODUCTIONThe National Institute for Occupational Safety and Health (NIOSH) was requested by Congress to study the safety of retreat pillar mining following the Crandall Canyon Mine disaster that took place near Price, Utah in August, 2007. As a part of this study, an investigation was made to better understand how the rock mass responds to the creation of mining excavations and how the loads are distributed among the pillars and the surrounding abutments or barriers. The outcome of these investigations contributed to the development of a modified loading model for the ARMPS-2010 (Mark, 2010) method ofretreat mining pillar design.Structural and Functional Failure of PillarsPillar design is typically conducted by estimating the pillar strength and stress, and then sizing the pillars so that an adequate margin of safety exists between the expected strength and stress. The pillar strength can be defined as the maximum resistance of a pillar to axial compression (Brady and Brown, 1985). If a pillar is loaded beyond its peak strength, load shedding or yielding can occur, and the pillar is considered to have failed as a structure. Structural failure generally refers to a loss of loadcarrying capacity."

The 1980's have produced an outstanding revival of the Australian gold industry. In contrast to some previous mining booms, the economic aspects of the growth in gold production are soundly based. In general the new developments have been highly profitable and this is beneficial to all parties- whether they be government, shareholders employees or suppliers. Mining companies are showing a large commitment to gold exploration with approximately 50% of Australian exploration budgets now being directed towards gold. An increasing proportion of this expenditure is being channelled towards the eastern States with Queensland 'being the major centre of interest. Further strong growth in production is expected with some 50 projects likely to come on stream before 1990. A very significant aspect of the foundation for this growth is a continuation of the existing tax status of the industry.

THIS essay is an attempt to present the elementary facts and ideas concerning the growth of crystals, especially of metals. Freezing, like all heterogeneous processes, is a process of nucleation and growth; it manifests itself in metallurgy primarily in the freezing of ingots. The preceding paper in this volume discusses nucleation; the paper to follow describes ingot solidification. The present paper will treat of the growth of nuclei to large crystals, embracing a long series of phenomena, many of which are familiar to metallurgists.† Crystal growth, it will be immediately noted, occurs not only in the freezing of metals, but in the freezing of all substances; and it is a prominent and familiar phenomenon in crystallization from solutions, aqueous or otherwise, and in crystallization from the vapor phase. It will be necessary to include such systems in this discussion, for so much of value to metallurgists has been learned from their study. Apart from this, so far as metals are concerned, it will be simpler to limit the discussion to the freezing of pure metals and of solid solutions. Other types of freezing-eutectic, peritectic, etc.-will be excluded, though they have much of interest and of importance. BASIC MECHANISM OF FREEZING Ideally, nucleation and growth processes can best be considered from the point of view of isothermal reaction and upon the assumption of random nucleation. In. such an ideal case, nuclei form at random, and at a characteristic rate either constant or changing with time, and grow to spherulites at a characteristic rate, constant or changing with time, Fig. I.. On the simplest assumption, the reaction rate-in this case the rate of freezing-is a resultant of these two component rates. The reaction rate equation may be written in terms of these two component rates. The formation of pearlite from austenite closely approaches this ideal case. The ideal case forms a very useful point of departure in considering real cases. Various factors of reality produce deviations from this assumed ideality. In freezing, the isotropic medium, the melt, passes to the anisotropic solid, the crystal. Crystals are lattices of atoms, and the unit cell is often referred to as a crystal unit. Rosenhain, many years ago, represented the nucleation and growth process on freezing in the manner shown in Fig. 2, feeling, intuitively, that there must be some crystal unit in crystal growth. It now appears that this may be true, though his picture requires much elaboration. The greatest deviation from ideality lies in the nonisothermal nature of the freezing process in metals. To approach this properly it should be recalled that many nonmetallic substances freeze slowly. "Two continuous coal zones occurring in a 2500f t (762 m) thick section of the Upper Carboniferous Pictou Group (Westphalian C and D) have been encountered in five offshore wells drilled f or oil and gas in the Gulf of St. Lawrence. They correlate, by means of fossil spores, with thick coals of the Mabou and Inverness coalfields situated at the eastern margin of the Basin. Therefore, the presence of a large submarine coalfield of at least 18000 sq mi. (46 620 km'] is indicated. This compares with an estimated size of 14 000 sq mi. (36260 km' ) for the Sydney Coal Basin.Accessibility is provided only by the small onshore parts of the Mabou and Inverness fields, because farther offshore the coals lie too deep to be mined (below 4 OOO f t ([ 219 m)).Two near-shore coal exploration wells indicate that at Mabou Mines the younger Inverness coals occur at a mineable depth to at least 5 km from land. A structure contour map of this area shows a reserve of 125 million tons in two Inverness equivalent seams that are 8.5 ft (2.6 m) and 4.5 ft (f.4 m) thick. The Mabou coals, however, reach too great a depth close to shore to possess mineable resources.A structure contour map of the Inverness submarine area, based on mine data and one off shore seismic profile, indicates inferred mineable resources of 160 million tons, in two seams that are 7ft (2.1 m) and 4.5 ft (f.4 m) thick . A detailed deep seismic survey will be necessary to substantiate the structure and resource estimates"",Maceral and coal quality percentage diagrams of five column samples of these seams (the 7 Foot and 13 Foot Seams of Inverness Equivalents) show a bright banded coal, high in reactive macerals, which will produce a weak coke unsuitable f or metallurgical purposes. A thermal coal with a rank of H. V. ""B"" Bituminous (0.64% Ro) is present, averaging 12000 btu llb (6 660 kcai/kg), 10% ash and 6% sulphur, of which 60 to 80% is derived from pyrite.Microlithotype profiles of the major Mabou and Inverness coals show characteristic successions that can be used f or seam identification and proved valuable f or structural interpretations.The time of faulting in the major fracture zone on the eastern margin of the Gulf of St. Lawrence Basin has been deduced from coal rank variations. It occurred in pos tPermian, probably Early Triassic time."

The Gwillim Mine, located 8 km northwest of Chibougamau, is underlain by two mafic volcanic cycles separated by a dominantly felsic cycle. These rocks are part of the Gilman Formation of the Roy Group. The mafic rocks are cut by weak shear zones containing the "North" and "Main" veins, both east-west striking gold-bearing structures dipping 75° to the north. The felsic cycle, which is also gold-bearing, contains textures suggesting a syngenetic origin. The Gwillim Mine presents an excellent opportunity for the study of what may be two coexisting gold deposits, a volcanogenic and a veine type.

THE H.B. MINE and concentrator are located in the Nelson Mining Division of British Columbia, on the north side of the Sheep Creek valley about seven miles southeast of the town of Salmo. It is an operation of The Consolidated Mining and Smelting Company of Canada, Limited (Cominco), which has its metallurgical plants at Trail, about 25 miles west. The property was originally staked about 1907 by Horton and Benson, whose names are commemorated in the designation "H. B." Up until 1927, a considerable amount of development work was done in the upper levels by various companies. Cominco purchased the ?property in 1927 and carried out intermittent work until 1946. Following this, an extensive surface diamond drilling programme and a subsequent underground exploration were undertaken. Sufficient ore was outlined 'to warrant the construction of a 1,000: ton concentrator. This was started in April, 1952, and completed by the spring of 1953. However, because of adverse base metal market conditions, operation was deferred until May, 1955.

Outstanding advances among the metals, marked the mineral production records for Canada during the first half of 1925. Production of metals and non-metals reached a total value of $90,347,698. as compared with $84,307,385. in the first half of 1924, an increase of 7.16 per cent. Metals and non-metals produced during the calendar year 1924 were valued at $17 4,202,537.; production during the first half of 1925 showed a sum equal to 51.86 per cent of this total. Advances among the metals were general. Gold rose to a new record. Lead advanced 62.41 per cent over the high mark attained in the first half of 1924. Nickel production was well maintained. Copper was up a million pounds. Silver showed increased values. Zinc followed the trend in lead to almost double the output recorded in the first half of 1924. Cobalt production continued to improve. Despite the loss due to the protracted strike in the Nova Scotia coal mines which reduced the output of coal in that province; during the six months ending June, to less than half the tonnage reported in the first six months of 1924, improved conditions throughout the rest of the non-metal mining field resulted in the production of non-metals during the half-year reaching a total value of $31,199,233. as compared with $36,844,286. in the first half of 1924; the loss in the value of coal output al one amounted to $5,723,873, while the decrease in the aggregate value of non-metals amounted to only $5,645,053.

Ammonia present in a variety of mine/mill waste waters was almost completely oxidized to nitrogen by chloride mediated electro-oxidation. A closed circuit operation of preconcentration of ammonia on a suitable adsorbent (e.g. zeolite or a resin) followed by brine stripping of ammonia and electrolysis of the strip liquor seems to have sound technical, economic and environmental merits. The removal of ammonia, cyanide, cyanate or thiocyanate from a mine/mill waste water (e.g. gold mill), either individually or collectively, is also feasible by chloride mediated electro-oxidation technique. Further R&D investigations are necessary for the development of a closed circuit operation for the removal of ammonia, cyanide and thiocyanate; and also to obtain technical data for process cost estimates

The Hallandsås project with a long history, first started in 1991, is well known in the tunneling community. In the past it has been stopped twice for technical and environmental reasons. The third attempt started in 2004 using a dual mode rock TBM. The heterogeneous highly fractured and abrasive rock mass as well as large areas of water bearing zones present most difficult subsurface conditions. The tunneling concept is based on the use of watertight segmental lining and a TBM able to operate in open mode, possibilities for extensive pre excavation grouting and high pressure closed mode operation up to 13 bar to cover exceptional conditions.

In Northland, New Zealand, localised large deposits of halloysite clay have been produced by in-situ alteration of rhyolite volcanics of Pliocene or Pleistocene age, which were erupted through pre-Pliocene sedimentary strata and locally through Pleistocene basalt; and which were subsequently onlapped by further basalt flows. Alteration to clay has been a result of both hydrothermal and of meteoric processes, and decreases in intensity of alteration with increasing depth. The raw material is crushed and fractionated to produce a product of less than two micron particle size, which is of exceptional whiteness and brightness. The unique properties of the clays render them highly desirable for specialised ceramic purposes.
