"Few issues are more important to the National Mining Association (NMA), the U.S. mining industry and NMA’s MINExpo International 2016 exhibitors than safety.Global mining safety and health is a journey necessitating continuous improvement, collaboration and innovation on the part of both mining operators and their suppliers.MINExpo 2016, which takes place Sept. 26- 28, 2016, in Las Vegas, provides attendees with the opportunity to view technologies to help refine management of mining safety and health, utilizing NMA’s CORESafety system. CORESafety ‘One of the best’Launched by NMA in 2011 and moving forward ever since, CORESafety is driving continuous improvement in safety and health performance among participating industry companies by shifting from a command-andcontrol regulatory approach to a risk-based systems approach built on the Plan-Do-Check-Act model (Fig. 1).Considered an exemplary safety and health management system, NMA’s CORESafety system helps mining operators change their approach to safety and health management by enhancing the safety and health culture throughout their organizations. Although it is challenging to consistently and completely control all the factors and variables occurring in daily mining operations, experience has shown that applying systematic controls to as many variables as possible is the most successful approach for improvement in mining safety and health.When embarking upon the development of the CORESafety framework, NMA’s leadership set an ambitious goal to eliminate fatalities and reduce the injury rate 50 percent in five years, or the “0-50-5” goal. While the goal has not been met yet, the steady progress has been extraordinary with fatalities among participating companies dropping 45 percent during the period 2011-2015 (Fig. 2). Data also demonstrate that, as participating companies increase their level of implementation, total injury rates have also declined (Fig. 3).In a September 2015 interview with NMA, Penn State University mining engineering Professor Jeffrey Kohler praised CORESafety as “an inspired and timely program that brings together key elements to achieve safety and health goals.”"

This report describes the development of guidelines for design, inspection and maintenance of automatic couplers. In order to devise the guidelines, performance requirements for automatic couplers were carefully established. This was done through a comprehensive survey of coupler manufacturers and extensive observation of automatic couplers in service in a broad spectrum of mines. The capability of the couplers to withstand the demands of the mining environment in terms of strength characteristics, gathering range, curve negotiability, and other factors were all carefully evaluated. An analysis of coupler related accidents for the years 1972-1976 was carried out to provide information on the safety and reliability of existing automatic couplers. This formed an important input to the development of the guidelines. It is expected that these guidelines will be useful to coupler manufacturers for establishing designs for improved automatic couplers, and to mine operators for maintaining their couplers in a safe and good operating condition throughout their expected lifetime. Three conventional uplift tests and 12 Rapid Pile Load Tests (RLT) were performed on total of fifteen 16-inch square pre-cast concrete driven piles at a test site in Emeryville California in December of 2000. The uplift tests were performed in accordance with ASTM standards, and the RLTs were performed using a FUNDEX RLT. The RLT is carried out by dropping a mass of 25-tons on top of the pile, thereby inducing a force that is measured by a load cell. An optical lens records the vertical deflection of LED's that are mounted on the pile. Ultimate uplift and compression capacities of each of the fifteen piles were determined using the Davisson method. Soil parameters were calibrated by back-calculation (cohesion and internal angle of friction) using both the results of conventional uplift load versus deflection curve, as well as the results of the triaxial compression tests. A two-dimensional finite difference model including soil elements, and structural elements representing the test pile was setup using the computer program Fast Lagrangian Analysis of Continua (FLAC). FLAC was used to simulate both the uplift as well as the RLT. Strength increase of cohesive material was also accounted for in the analysis. Numerically simulated and theoretically computed pile capacity curves and axial deformation variation with applied load curves are in close agreement to the observed curve during the field test. Conclusions are made from this test site regarding the factors influencing results of RLT.

New sedimentological and biostratigraphic data indicate that the Leron Formation in the Markham Suture Zone, Papua New Guinea (PNG) represented Pleist- ocene alluvial fan-delta deposition in shallow marine to lacustrine environments. The distribution of sedimentary regimes in the Leron Formation indicates that during collision zone evolution, various tectono-sedimentary regimes do not always track the movement of the triple-junction systematically as idealized by Crook's (1989a) model. The migration of various tectono-sedimentary regimes are also controlled and influenced by the effects of second-order geometric features of the convergent margins. Leron Formation in the Markham Valley represents late to post-collisional marine to fluvial molasse. Foraminifera indicate that the collision and concomitant suturing of the Markham valley occurred after 1.32Ma.

Overcoring tests were conducted at the Near-Surface Test Facility(NSTF) to assess the suitability of five techniques (U.S. Bureau of Mines borehole deformation gauge (BDG), Commonwealth Scientific and Industrial Research Organization (CSIRO) hollow inclusion stress cell, Luleå triaxial gauge (LuH gauge), and Council for Scientific and Industrial Research (CSIR) doorstopper) for in situ stress determination in a closely jointed basalt. This effort is in support of the Basalt Waste Isolation Project, which is studying the feasibility of locating a nuclear waste repository in the basalts of the Hanford Site in southeastern Washington. This paper presents the results from the overcoring study that formed the basis for selection of two techniques to be used during the further exploration of the basalt formations at depth.

Lithium-Ion batteries (LIBs) have the potential to go into thermal runaway which introduces a new ignition source in hazardous environments such as gassy underground mines that may contain methane in the surrounding air. Researchers from the National Institute for Occupational Safety and Health (NIOSH) used an accelerating rate calorimeter (ARC) to induce thermal runaway in individual lithium-titanium-oxide (LTO) and lithium-nickel-cobalt-aluminum-oxide (NCA) LIB cells within a sealed enclosure with either no methane, a lean, a stoichiometric, or a rich methane-air mixture in the surrounding canister to determine if these LIB types would ignite the methane, causing an additive effect on overpressures. Canister pressures and maximum temperatures reached by the cells, surrounding air, and exhausted gases were measured in this study and remained consistent with varying methane concentrations.

AFTER more than fifty years of investigation and continuing failure in service, a satisfactory mechanism to explain stress corrosion cracking in homogeneous alloys is still lacking. The relative chemical activity of grain boundaries and adjacent grains, inferred from the boundary structure, has been used to explain intergranular cracking in a brass.' This concept is probably correct as a general hypothesis, but it requires much more extensive development in order to explain satisfactorily the observed phenomena. For example, the specific function of the solute in an alloy must be considered, because intergranular failure does not occur in pure metals,' which are structurally similar to homogeneous alloys. Also, in this connection, an inter crystalline fracture path in binary copper alloys can be changed to a transcrystalline path by relatively small ternary additions.' The fracture path in ß brass is predominantly transcrystalline,' in contrast with an inter crystalline path in a brass. Whether the two different fracture paths are the result of a difference in composition, crystal structure, or the state of long range order is unknown and cannot be determined with ß brass, because only the ordered state can be obtained at room temperature. The study of chemical activity at structural sites in the Cu-Zn system is restricted by the formation of opaque films in an ammonia environment in which stress corrosion cracking is generally observed. Furthermore, both components of the alloy are chemically active and the interpretation of observations is therefore relatively complicated. The Cu-Au system is a better choice for the purpose of studying structural factors. In particular, CuAu is very susceptible to cracking in ferric chloride and opaque films are not formed in this solution; it may be obtained in different states of long and short range order; and, finally, only copper is oxidized in dilute ferric chloride, which considerably simplifies the chemical problem. To provide information necessary for a more general explanation of stress corrosion cracking in homogeneous solid solutions, the following structural factors have been investigated in a Cu-Au alloy having a composition corresponding to Cu,Au: 1) the effect of long and short range order; 2) the structural sites from which copper is removed from the annealed and cold worked polycrystalline alloy; and 3) the effect of annealing time and temperature on structural reactions in the cold worked alloy. Experimental Procedure A Cu-Au alloy of nominal Cu3Au composition was cast from cathode copper and high purity gold and fabricated into sheet and wire. Chemical analysis for copper and gold indicated 48.92 wt pct Cu and 51.01 pct Au. Quantitative spectrographic analysis indicated 0.01 pct Ag, 0.002 pct Pd, 0.001 pct Pb, and 0.002 pct Fe, with traces of magnesium, silicon, and zinc. Presumably most of the remaining 0.055 pct corresponds to the limits of analytical precision. Heat treatments of wire and sheet were conducted in argon or vacuum. Although grouted piles have been used for several decades since the idea was proposed in the 1970s, their design is mostly based on experience and unpublished proprietary methods. There is little research on grouting drilled shaft tips and even less on grouting shaft sides. This paper presents the analysis of 4 instrumented pile loads tests on base and shaft grouted piles from different sites in Egypt; with emphasis on one of the tests, to demonstrate the analysis process. The data from the tests are analyzed to model the development of unit side friction (USF) with relative displacement. A numerical model using the finite element software PLAXIS is used to investigate the effect of grouting on the surrounding soil stress state. The paper combines the observations from full scale pile load tests and results from the numerical model to suggest an idealized behavior of side friction development with relative displacement of the grouted part of the shaft. The suggested model considers an envelope of Unit Side Friction (USF) with relative displacement, initially dominated by the grout resistance then at higher relative displacements, the friction with radially pressured soil takes over. The grout provides very high USF values up to a relative displacement of 2mm. Then, the USF drops to a typical pile soil side friction behavior; however, this friction is affected by the contact stress between pile and soil, which significantly increases due to compaction grouting. INTRODUCTION Base and shaft pile grouting has been significantly developing in recent years to accommodate the unfavorable conditions in major projects such as the high loads in high rise building and founding on problematic soil formations. Grouting aims to decrease settlements and increase the capacity of the pile under axial load. The technique is first introduced by Kraft (1974) but gained popularity in the 1980s. Typically there are two types of grouting; compaction, and permeation. Compaction grouting is more common in pile foundations practice, and many researchers have provided case histories of pile load tests on base grouted piles and base and shaft grouted piles (Gouvenot and Gabaix,1975; Stocker, 1983; Ghazali et al., 1988; Fleming, 1993; Kusakabe et al., 1994; Robson and Wahby,1994; Joer and Randolph, 1998; Hamza and Leoni, 1998; Mullins et al. 2000; Littlechild et al., 2000; Suckling and Eager, 2001; Dapp and Mullins, 2002; Ho, 2003; Fu and Zhou, 2003; Brettmann and NeSmith, 2005; Ruiz, 2005; Mullins, 2006; Dapp and Brown, 2010; Beadman et al., 2012; Nguyen et al., 2013; Miller et al., 2013; Sinnreich and Simpson, 2015; Hui and Lin, 2015). Compaction grouting is meant to densify the soil around the pile and below the base rather than permeate this soil and increase its cohesion. Research showed that grouting increases the capacity of the piles by a factor of 2~5. The variation in capacity increase depends on pile type, grouting process, and especially grouting pressure. Although there are many case histories of pile load tests on grouted piles but there is no well-defined agreed-upon method to predict grouted pile capacity as typically done in ungrouted piles. Few lab experiments discussed the shaft grouting effects on the soil especially the compaction grouting. The experiments concluded that the compaction grouting forms a homogeneous layer between the pile and the soil, and a residual effective stress is achieved simultaneously with a significant densification of the soil around the pile with noticeable decreasing in void ratio. (Fattahpour et al., 2015; Wang et al., 2015). Recently, studies used numerical analysis to investigate the behavior of the grouted piles and the changes in the surrounding soil (Chiang et al., 2013). An industrial port complex was constructed on the west coast of Leyte Island The work was started in December 1981 and completed in May 1984. This port complex includes a 665 m long wharf and can accommodate vessels ranging from a 60,000 DWT ore carrier to a 4 000 DWT general cargo ship. It now handles materials for and products from a copper smelter with an annual capacity of 130,000 t and a fertilizer plant with an annual capacity of 900,000 t. The Wharf structures consist mainly of interlocked steel pipe piles, steel sheet piles and steel pipe Piles. Material handling equipment such as unloaders, shiploaders, etc. were installed on the wharf. Site survey, planning, engineering design and construction aspects of the port development are discussed in this paper with particular emphasis on the wharf structure.

As part of the addition of eight new combined cycle generating units at the Florida Power & Light Sanford Plant, 2,129 piles were to be installed. The existing plant elements were supported on driven piles, and the variation in pile lengths during previous work, as well as available subsurface information, indicated a complex stratigraphy. At the request of the owner, the designer/construction manager developed a program to evaluate the use of pressure-grouted displacement piles. A pre-design load test program in areas that represented the apparent extremes in subsurface conditions indicated that the system was feasible; however, the presence of existing features that were to be demolished later precluded the clear delineation of subsurface conditions in advance of construction. A contract was negotiated with the pile installer based on the best estimate of potential pile lengths, with modifications to pile lengths to be set based on additional exploration. A total of 52 cone penetration soundings and five load tests were performed immediately prior to and during pile installation to establish correlations between the CPT data and pile performance. Pile depths were set based on these correlations. Pile installation was completed 6 weeks ahead of schedule and 15 percent under the original budget estimate. This paper documents the evaluation of pressure-grouted displacement piles as an emerging technology, and illustrates the benefits of a close working relationship between the owner, designer, and pile contractor at a site with variable subsurface conditions.

Ground characterized by hard boulders embedded in soft matrix represents one of the most challenging conditions for TBMs, as the cutterhead must cope with the non-optimal conditions of cutting hard blocks while simultaneously excavating through soft, soil-like matrix material. Dowden and Robinson (2001) and Hunt and Mazhar (2004) summarize several case histories relating to the problems of advancing a TBM through boulder-laden ground and provide guidance with respect to appropriate site investigations and baselining strategies. Some rock types constituting boulder-laden ground include: ?Sedimentary Rocks?cemented alluvial channel deposits, colluvium/talus, landslide debris, and glacial till. ?Igneous Rocks?agglomerates, lahars, and pyroclastic deposits. ?Metamorphic Rocks?tectonic mélange and cataclastic fault zones. ?Decomposed Rocks?corestone bearing saprolites. Disc cutters are commonly utilized to cut the hardblocks; however, as compared to the understanding of disc cutter behavior in hard continuous rocks, little is known regarding fundamental cutting mechanisms in highly heterogeneous bouldery ground conditions. To advance this understanding, a suite of full scale tests have been performed using a linear cutting machine at the Colorado School of Mines (CSM). This paper summarizes the observed cutting mechanisms and their practical implications for TBM excavations through boulder-laden ground.

The Izmit Bay bridge, a 3-km-long suspension bridge crossing of Izmit Bay, Turkey is planned to be constructed in one of the most seismically active places in the world. The site, which has the potential to experience significant earthquakes associated with the relative motion accommodated on the North Anatolian Fault, is underlain by deep deposits of soft soils, and areas of unstable and liquefiable soils. Characterizing the seismotectonic setting, foundation soil conditions, potential fault locations, as well as other geohazards was a critical component for the project concessionaire to finance the project and obtain meaningful bids for bridge design and construction. Given the Build Operate Transfer (BOT) funding mechanism, executing this work in the least possible time was critical to the concessionaire?s financing scheme. Fugro was retained to provide geotechnical, geological, and seismological evaluations for the project region, and to generate data for contractors to develop reliable designs and bids. The uncertainties in these data made it difficult for the BOT concessionaire to finance the project and tender Bridge design and construction. The investment in an early and comprehensive site characterization program to evaluate and quantify those uncertainties allowed the concessionaire to save schedule, provide a robust bid basis to bridge contractors, and persuade banks to finance what was perceived to be a relatively high risk project.

Deep Mix-in-Place soil stabilisation is achieved by forming columns of soil/cement arranged to overlap by secant intersection. Alternate columns are constructed initially and infilled. The cementitious material injected is designed to retard the strength of the resulting mix thereby allowing satisfactory overlapping. Columns are constructed using a designed large diameter mixing auger mounted on a smaller diameter auger stem. This is firstly screwed to the design depth removing only such spoil as to enable penetration of the auger, and subsequently counter rotated and extracted in a predetermined mixing pattern and grout injection rate to ensure continuous and homogeneous treatment through each successive level. The process is performed whilst monitoring auger depth, grout flow and pressure. Cementation Piling and Foundations Limited used this technique to stabilise allu­vium and to facilitate the construction, by conventional techniques, of a circular pumping station pit of 10M diameter to 10M below ground level. The work was carried out within 3M of adjacent domestic properties, and on a very restricted site in Elland, West Yorkshire. Ground conditions consist of 2M of cohesive alluvium becoming water bearing and increasingly sandy and gravelly with cobbles which overlies the mudstones at 10.0m depth. Sandstones beneath formed an aquiclude to artesian ground water with the underside of the open excavation approxi­mately 1.0M from the top of this material. Peripheral treatment into the mudstone, coupled with pressure relief wells, proved successful and final excavation depth was achieved in "the dry".

"Bubble area flux has been claimed to be a major tool to dimension flotation equipment. The paper describes laboratory scale (batch and 301 continuous cell using plant feed) and small-scale (1 ,8m3) industrial experiments with coarse-grained massive sulphide ore.Samples were taken with different froth thickness and air addition rates while the bubble size was measured with an UTC capillary device. Problems of averaging the bubble size measurements and the bubble area flux measurements are also discussed.Material balances were calculated using the commercial program BILCO. The flotation rate constants were calculated for pulp and for froth assuming that we can extrapolate the flotation rate constant of the pulp phase from tests using very thin froth thicknesses.The results indicate that the bubble area flux is not alone an adequate dimensioning tool. With fine sizes there was a nearly linear relationship between the bubble area flux and rate constant for constant impeller revolutions. For coarse sizes the form of the relationship is non-linear and seems to go through a maximum value. Thus the final flotation outcome is not a simple function of bubble area flux but depends on the particle size distribution and other more traditional flotation cell dimensioning variables. The effect of the froth residence time in the total recovery of coarse particles is large, but will not be treated in this paper."

1. Estimates of reserves were made for three coal beds in Hamilton County the - Lantana, Sewanee, and Richland. Nine coal beds have been mined at one time or another in the county, but not enough information was available to estimate reserves in any beds other than the three given. 2. Known measured and indicated reserves for all beds, based on a minimum thickness of 14 inches and on 1,800 short tons per acre-foot of coal in place, are estimated to be 53 million tons as of January 1, 1954. Of this total 35 million tons is in beds 28 inches and more thick. Areas in each bed were omitted from the estimate because available data relative to the bed characteristics were insufficient for making an estimate conforming with the definitions of measured and indicated coal adopted for this study. Should future drilling or development prove re-serves in these areas, such reserves should be added to the total estimated reserves determined by this investigation.

"Lax, misguided, and dangerous" is Rep. James Santini's description of the official US position on strategic nonfuel minerals. Santini (D-NV), chairman of the House Mines and Mining Subcommittee, criticized the Interior Department for "its long record of benign neglect" regarding the domestic minerals crisis. Santini made his remarks at a news conference at the American Mining Congress convention in San Francisco, where he released his subcommittee report "US Minerals Vulnerability: National Policy Implications." The report-said to be the first major congressional study of the minerals issue by a committee of jurisdiction is the result of a two-year inquiry. The investigation was undertaken, according to the chairman, "because no person or agency in the executive branch of government fully grasps the critical importance of nonfuel minerals to the nation's economy, defense, and quality of life." The report also urges a larger role for the Department of Defense, which has "stood passively by for many years as a consuming by-stander," according to the report. President Carter's incomplete Non-fuel Minerals Policy Review also drew the committee's ire; Santini calls it "a tragic waste that cost tax-payers $3.5 million and the loss of some 13,000 person-days." The subcommittee examined the following issues for its report: • The US minerals industry's inability to meet national raw material needs under a "hodgepodge of contradictory, restrictive, and irrational" federal regulations; • Dependency on foreign sources of strategic minerals in times of "increasing resource nationalism" and "Soviet mineral expansionism;" • National security needs for foreign mineral resources that do not have effective substitutes; • The US government's approach to national minerals policy "amid a growing realization that such a policy does not exist." The subcommittee report includes an analysis of public and private studies on nonfuel minerals and discusses a series of policy problem areas that negatively affect domestic production. Capital formation disincentives, tax policies, antitrust enforcement, environmental laws, health and safety regulations, technical problems, and restricted access to federally-controlled public lands contribute to what Santini calls a "tragic irony." "The irony is that we are a nation vastly rich in mineral resources yet our own government chooses to stifle development of these critical raw materials and thereby increase offshore reliance. The tragedy is that we also are ensuring US vulnerability to severe economic dislocations and a weakening of our national security." In just six years, from 1973-1978, the US trade deficit in nonfuel minerals rose from $2 billion per year to $8 billion, according to the report. The country imports more than 50% of 23 critical minerals, including 100% of its manganese, chromium, cobalt and platinum. The US is promoting its dependence on foreign mineral sources at the very time the security of many of those sources is becoming less certain, the report notes. Santini said the subcommittee investigation revealed a vulnerability that may be more serious and pervasive than that of foreign oil. The US may be able to develop alternate energy resources, but there are no effective substitutes for many nonfuel minerals. The report also discloses that US stockpiles of critical and strategic minerals are of inadequate supply and quality. Holdings of some vital minerals are far below present objectives, and for some there are no holdings at all, according to Santini. To resolve the current crisis, the report makes the following recommendations: • Complete immediately President Carter's Interagency Nonfuel Minerals Policy Review; • Implement (throught the Interior Department) the 1970 Mining and Minerals Policy Act; • Create an Office of Energy and Minerals with the same stature, power, and oversight responsibilities as the Council on Environmental Quality; • Ease federal restrictions on mineral exploration and development on public lands and implement a complete review of public land use policies; •Fund for and fulfill US nonfuel minerals stockpiling objectives; •Increase federal funding for non-

This paper presents part of the outcomes of the drill and blast optimization project carried out at Kisladag Gold Mine in Turkey. Kisladag Gold Mine is the largest gold mine in Turkey with a low-grade, bulk-tonnage, open pit operation that uses heap leaching for gold recovery. After blasting, the ore is fed to a three-stage crushing circuit which prepares crushed ore approximately -10 mm (0.39 in) for heap leaching. Crusher downtime analysis showed that blocky material and insufficient feed are causing significant downtimes with the primary crusher. This was supported by the fragmentation data collected to date. It was decided to initiate a fragmentation project to understand the variation in the run-of-mine (ROM) size with a view to provide a consistent feed to the crushing circuit. The study includes the analyses of the ore strength and structure, image analysis of the fragmentation pictures acquired from blasts, blast auditing and quality assurance and quality control (QA/QC) analyses. During this project, over 200 fragmentation pictures were acquired from 47 blasts to optimize ore and waste blasts. Ore strength varies between 30 and 120 MPa and rock quality designation (RQD) ranges between 30 and 100. A site-specific fragmentation model was used to calibrate the fragmentation for blast domains formed. In addition, alternative cases were simulated to achieve blasts designs with consistent ROM fragmentation size distribution. Alternative blast designs that include stemming length and powder factor changes were suggested. Long stemming length, drill cuttings use, free faced blasts, small shot sizes and poor stemming length control were some of the key issues causing coarse fragmentation.

Residual aeolian placers (RAPs) are formed by the wind, by preferential entrainment and removal of small and low-density detritusfrom a pre-existing, typically sedimentary sequence containing heavy minerals. Heavy minerals that are resistant to chemical alteration and either soft and malleable (gold), or extremely hard (diamond, sapphire, ruby, etc), are concentrated as a residual on the wind deflation surface, typically in the interstices between larger resistant clasts. RAPs form preferentially in arid or semi arid areas, where the vegetation is sparse or absent, and where topography and surface gradients are low, but may also form on beaches and in periglacial environments. RAPs consist of a continuous or discontinuous, planar or gently undulating horizon of heavy minerals in a single-clast-thick pavement of resistant pebbles, cobbles, or boulders, which mantles the present topography or palaeo-topography. Extreme wind erosion and surface deflation may condense the full pre-existing sequence into a layer of heavy minerals only. The grade of RAPs is substantially higher than in the precursor deposit, and commonly much higher than those typically found in most other placer types. Aside from their general occurrence as a thin pavement of resistant clasts, the best indicators of residual aeolian gold placers are the presence of wind-eroded stones (ventifacts) on the pavement, and the distinctive and unique shapes of aeolian gold grains themselves. The shape of the gold grains is progressively transformed by sand blasting during placer development, from grains with thickened rims, to toroidal grains on which the rims are æsplitÆ and peened inward toward the grain centre on one or both of the grain sides, to hemispherical or sub-spherical grains that are fully encapsulated by the in-turned rims. Gold that has undergone aeolian shape transformation is difficult to entrain and transport away from the site of aeolian concentration by anything other than mass movement or glacial processes. It is also easily retained by commercial recovery plants of any configuration.
