In safety work a study of past accidents is of value in determining means of reducing future accidents of the same character. This is particularly true with reference to coal-mine explosions. Data from publications of the United States Bureau of Mines and from published reports of the State inspector of coal mines of Colorado are presented in this circular in the belief that they will be of value to operators by indicating methods of preventing future coal-mine explosions. The tables included are complete only with regard to major disasters. The Bureau of Mines considers as a major disaster one in which five 'or more men are killed. However, this circular also includes a considerable number of minor disasters, Various explosions in the coal mines of Colorado from 1883 to 1932, inclusive, are listed with information available as to their probable cause, number of men killed and measures which, if they had been in use at that time, might have prevented them. John McNeil, first Colorado State coal-mine inspector, on page 43 of his first report for the period ended July 31, 1884, made the following recommendations for the prevention of explosions in the coal industry: True causes could be traced either to insufficient ventilation, carelessness and lack of rigid discipline in carrying on energetic ventilation and other precautions needed to be enforced in a mine giving off explosive gases. *** When any quantity of fine coal dust is observed in an entry, air course, or room it should be made damp and be removed from the mine, and thus beep every place as free from dust as possible. *** A system of bratticing close up to the face of each and every place in the mine from crosscut last made (whether gas should just then show its presence or not) which should not be more than 50 or 75 feet according to the nature of the mine.

The Bureau of Mines and the Alabama Power Co., jointly, conducted the first experiment undertaken in the United States in underground gasification of coal in place, at Gorgas, Ala. Milton H. Fies, manager of coal operations, Alabama Power Co., proposed the cooperative investigation to Arno C. Fieldner, chief, Fuels and Explosives Branch, Bureau of Mines; and assisted in supervising the experiment. The objectives of the experiment were to determine whether coal in place could be burned successfully and whether the burning could be controlled; the effect of the fire on the coal bed and the overlying strata; and the yield and quality of gas that could be obtained.

Controlling unwanted ground movement is critical in mining as well as many other highly hazardous environments, such as construction and tunnelling. Sharing knowledge about effective hazard management is paramount to improving health and safety and operational outcomes across industries, and some industry sectors have embraced this openness as part of the way that they do business. In 2010, the Australian Coal Association Research Program funded the RISKGATE project to build a body of knowledge for managing a broad range of hazards across mine sites, lease areas and mine infrastructure within a mine-life or product life cycle perspective (eg from exploration through to decommissioning, or design through to operation). The RISKGATE body of knowledge was assembled from Australian coal industry experts through a semi-structured bowtie analysis-based action research workshop cycle. By capturing operational knowledge from industry experts, RISKGATE serves to provide a cumulative corporate memory at a time of high personnel turnover in the coal industry.This paper presents an overview of RISKGATE topics within the geological and geotechnical domain (strata underground, ground control open cut, outburst, coal bumps and bursts, tailings dams and inrush) with a summary of individual topic structures, as well as showing the contrasts and inter-relationships between these ground management topics. "The start-up of new plants, extending from the commissioning of equipment to the production ramp-up, is a critical period where the processing circuit operation must be done in a structured and well controlled way to enable the gradual, safe and rapid achievement of production targets. The magnitude of the many risks associated with this pivotal period is accentuated in the actual context of low commodity prices. However, the implementation of a method that analyzes risks associated with a start-up, their probability of occurrence and their impacts leads to the identification of a series of preventive actions that can be applied in order to reduce potential negative impacts.Soutex has been involved in numerous start-ups over the last few years and has developed a structured methodology to assess this crucial period, maximize the pre-start-up preparation, ensure a smoother and safer ramp-up period and minimize the time needed before reaching commercial production.This paper describes a method to identify preventive actions to apply in order to reduce risks related to a plant start-up. Then, the various steps of the method are demonstrated using the industrial case study of the Canadian Malartic gold plant situated in Abitibi, Québec. The specific preventive actions, during start-up, the organisational measures and the ramp-up plan are detailed as they were effectively achieved. Finally, the post start-up actions are presented as well as some key results illustrating the achievement of their prior defined objectives.IntroductionPlanning a new plant or new circuit start-up in a mine installation involves various risks, both at the economical and safety levels. Unprepared staff and equipment can lead to safety issues that could threaten plant staff as well as the environment. Moreover, a poorly prepared start-up could often lead to equipment break-down, production stoppages and delays, leading to important economic impacts, delaying commercial production and therefore affecting directly the profitability of mining projects.It is common in the industry to have structured commissioning procedures for major equipment taken separately. Technical personnel from the equipment manufacturing company would usually come on site in order to perform the pre-operation verification, dry runs and the first hours of operations. This kind of assistance is usually focussed on one piece of equipment and neglects the interactions between the various equipment of the production chain. The commissioning phase is usually maintenance oriented and the information given by the manufacturing representative seldomly flows through the operation staff (in the case of this paper, the denomination “staff” refers to both managing and operating employees). The global process is usually not taken into consideration by the technical personnel sent on-site by the manufacturer during the commissioning phase. Many papers can be found describing the commissioning of new equipment focussing on the technology (Côté and Adante (2009), Wells et al (2007)) but without dealing with its integration process in the complete production chain."

Ocean mining has a number of problems which are critical to its development: ? Political hurdles on national and international stages ? The worldwide economic situation, with the possibliity of a recession ? Financial difficulties in funding further work ? Technical problems for the development of ocean mining and transport subsystems ? Demands for technology transfer This paper touches the last two points and provides some thoughts for discussion and creative objectification. The many technical problems of ocean mining will be identified and some proposed solutions will be investigated relative to the state-of-the-art technology of ocean mining and nodule transport. An outlook into the future attempts identification of the steps necessary to eventually develop components for workable systems that are not only technically but also economically feasible.

"Native antimony metal is rare and antimony generally occurs together with gold, lead, copper and silver. There are more than 100 antimony minerals, although the sulfide mineral stibnite is the main economic mineral.Antimony enters the supply chain either as a result of mining (primary production) or recycling (secondary production). China has by far the largest antimony resources and is, as a result, the world’s center for antimony mine production. While China remains the leading primary producer, accounting for 69 percent (80.5 kt or 88,700 st) of the global 117.5 kt (129,000 st) in 2017, market consolidation and regulatory inspections across China leading to closures of facilities have caused a significant decrease in Chinese output from 115.8 kt (127,600 st) in 2015 (81 percent of global production).Tajikistan and Russia are the next largest producers of antimony after China, both ramping up production in recent years. Russia could significantly increase its supply of antimony if Polyus moves ahead with supplying 10-12 kt/a (11,000–13,200 stpy) of contained antimony from gold tailings. Australia and Bolivia are also significant mine producers outside China. The value of the different ores varies based on the metal content of the concentrates, with the most valuable gold-bearing ores sourced from Australia’s Costerfield Mine and the least valuable mercury-bearing ores sourced from the Anzob Mine in Tajikistan. On average, Bolivian and Russian ores trade at an intermediate value between those from Costerfield and Anzob. Nearly all of the rest-of-world production is exported to China, where the ores and concentrates are processed into antimony ingots and oxides.Antimony ingot production is not as widespread as is the case with antimony mine production, owing to the high levels of pollution that are associated with antimony smelting. It is for this reason that smelting has been largely confined to China and other Asian countries ,such as Vietnam and Myanmar, where environmental legislation has historically been less stringent. Tajikistan has become a growing producer of antimony ingot since 2014, coming from the commissioned metallurgical plant at the Anzob Mine.Antimony oxide production is undertaken on four continents with China being the biggest producer. There is also a considerable amount of oxide production in Europe (mainly Belgium and France), which relies on imported antimony metal feedstock, mostly from China.A new antimony project under construction by Strategic and Precious Metals Processing (SPMP) in Oman made progress in 2016 and 2017, and after producing first metal in April 2016 in test facilities, was set to be commissioned in the first quarter of 2018. Commissioning had been moved back to June 2018, with a potential ramp-up to 2-3 kt/a (2,200 to 3,300 stpy) of antimony metal by the end of 2018. SPMP is also reviewing producing antimony oxide as part of its flow sheet."

"The Johnny Mountain gold mine in northern British Columbia, owned by skyline Gold Corporation, has been in operation since August 1988. Mill operating difficulties at the start of the operation resulted in a depleted cash flow situation and quick improvements were required to generate a self-sustaining financial basis of operation. In October 1988, Melis Engineering Ltd. was contracted to identify metallurgical difficulties, implement necessary modifications, and undertake mill operations and personnel training in order to achieve production targets.This paper presents the operating difficulties which existed at start-up and describes the methodology used to overcome production problems. Metallurgical testing results are described along with the modifications made to the milling circuit to suit ore metallurgy. Plant changes led to the operation of a simple gravity/flotation circuit to recover gold as bullion and in a copper concentrate. Operation of the originally installed cyanidation circuit was discontinued. This contributed to substantial savings in operating costs and virtually eliminated environmental problems. PROPERRTY LOCATION ANR DESCRIPTIONThe Johnny Mountain mine site is located in northern British Columbia along the Iskuit River about 60 km east of Wrangell, Alaska and 275 km north of Terrace, B. C.. The plateau is flanked to the north by Johnny Mountain and surrounded on the other sides by steep cliffs."

An exhaustive program of geologic studies and mapping has been conducted by the Geological Survey in the Iron River district of Michigan since May 1943. Means of contributing to the success of these studies were sought by the Bureau of Mines, and eventually a program involving geophysical studies and core-drilling was undertaken in the area. All of the work was done either to develop data that would contribute directly to geologic studies or to investigate geophysical methods that might be used as a primary means for the discovery of minable iron ore.

As the University of Hong Kong and their advisors planned their new Centennial Campus it became clear that they would have to re-provide existing water reservoirs on an adjacent site with new salt water reservoirs located underground in two new caverns. The existing reservoirs are owned by the Water Supplies Department who provide salt water for sanitary flushing and a separate potable water supply for regular use. Although underground reservoirs had not previously been adopted locally, the planning has been completed to a tight schedule and the construction is well advanced. The need to preserve historic buildings imposed constraints, as did the requirement to minimise potential construction nuisance for the local community and environment. This paper presents the background to the project and the work carried out to develop an innovative, sustainable and environmental acceptable solution. Construction progress to date is also presented.

This report 3/gives the results of a reconnaissance of the mining districts in Lander County, Nev., made from March 29 to April 20 and from May 9 to May 21, 1938, during which virtually all of the mining districts n the county were visited. No attempt has been made to include in this report all the mines and prospects in the various mining districts, and it should be stated that whether or not a property is mentioned herein has no. bearing on its merits. The report rovers the situation of the various districts, types of ore deposits, ownership and description of mines and prospects, information on past and current activity, and other information of economic interest likely to be useful to those interested in the development of our mineral resources. The names of the various districts mentioned are those generally used when they were organized for purposes of record and regulation. When originally organized, the districts embraced large areas with no well-defined boundaries, so that the names have little geographical significance.

Coal is increasingly viewed as one of the major fuel resources of this country for the remainder of this century. The need to extract a greater percentage of the coal in a given seam, together with advantages of improved safety, indicates that longwall mining will become more widely used within this time frame. A novel method of mining coal on a longwall face, utilizing high pressure water jets, is herein described. The mining machine proposed will exploit the stress conditions which exist on a longwall panel in order to achieve the required productivity while at the same time reducing respirable dust and methane ignition hazards. The water jets will excavate only the boundary of the extracted seam section, and thus the energy requirement of the machine will be minimized. The action of the machine is similar to that of an existing coal plow, except in that the wedge teeth of the plow are replaced by oscillating high pressure water jets. The machine can, therefore, cut to the depth and at the speed of existing shearer units at equivalent power levels. The removal of impact stresses from the machine will reduce downtime required for maintenance and replacement of picks and lower the load on the haulage motor of the system. The ability of the machine to increase the depth of cut without an increase in horsepower will further improve productivity when transportation systems to remove the greater production are in operation.

"SynopsisA number of important areas in nuclear fuel cycle, at both the front end and back end, offer ideal opportunities for the application of non-destructive evaluation techniques. These techniques do not only provide opportunities for non-invasive testing of e.g. irradiated materials, but also play an important role in the development of new materials in the nuclear sector. The advantage of penetrating radiation used as probe in the investigation and testing of nuclear materials makes X-ray and neutron radiography (2D) and tomography (3D) suitable for various applications in the total nuclear fuel cycle. The unique and different interaction modes of the two radiation probes with materials provide several opportunities. Their complementary nature and non-destructive character makes them most suitable for nuclear material analyses, analytical method development, and the evaluation of the performance of existing nuclear material compositions. This article gives an overview of the X-ray and neutron radiography/tomography applications in the field of nuclear material testing, and highlights a few of the success stories. Several selected areas of application in the nuclear fuel cycle are discussed to illustrate the complementary nature of these techniques as applied to nuclear materials.IntroductionDuring the development of new materials for the nuclear industry, materials testing and characterization are of the outmost importance to maintain safety standards and reliability. No compromise on safety in the workplace in any area within the total nuclear fuel cycle can be tolerated, and therefore most in-situ material characterization and testing is conducted by certified and qualified personnel schooled in destructive and non-destructive testing (DT and NDT) methods. Certification and qualification in NDT can be obtained through many training centres in South Africa in accordance with European, American, and other international standards (SGS, n.d.; SAIW, n.d.; African NTD Centre, n.d.)."

"Abstract Uncertain strength, deformation and flow properties in dry deep mixing columns often lead to conservative choices concerning design values where the design is regularly complemented with a surcharge fill. The surcharge not only leads to increased cost but also to logistical problems where the surcharge is a physical obstacle. The deformation properties of a ground improvement were studied by means of two small embankment load tests. The settlement measurements were also used in FEM analyses, where the effect of surcharge on creep settlements was investigated. Based on the results, the effects of the surcharge were considered negligible and could safely be ignored. The project is an example how the contractor can use a field test as an effective design approach.IntroductionGround improvement of soft soils with dry deep mixing columns, or lime-cement columns, is often combined with a surcharge fill. The main reason for using a surcharge is to force the primary settlement and some of the secondary settlement that would have occurred under the final embankment height alone, to take place under the surcharge loading. Another reason is to level out uneven settlements. Strength and deformation properties in improved soil cannot be sufficiently assessed in advance, before columns have been tested on-site, and this uncertainty often lead to conservative choices concerning design values. In order to compensate for a slow forecasted consolidation process, the design is regularly complemented with a surcharge. The surcharge not only leads to increased cost but also to logistical problems where the surcharge is a physical obstacle.Extensive research undertaken by Åhnberg (2006, 2007) shows, by means of triaxial and oedometer tests, that cementation processes and a load acting on the columns during hardening, makes the improved soil to behave as a heavily over-consolidated soil due to an increase in quasi pre-consolidation pressure. The effects of using a surcharge on the time for consolidation and creep properties are thus probably small, since the stress level does not normally exceed this quasi pre-consolidation pressure. A study presented by Venda Oliveira et al. (2013) shows that the stress level within small ranges has limited effect on the creep properties.This paper presents a case study where the deformation properties of a ground improvement using dry deep mixing columns was investigated by load tests on two small test embankments. The tests were performed at the Interchange Värtan project site, which is part of the Norra länken infrastructure project in Stockholm. The tests were not performed as a research project but simple load tests performed by the contractor with the purpose of rationalizing the design delivered by the client. One purpose of the load tests was to assess the deformations when the columns are loaded temporarily by overlaying concrete works. Another purpose was to provide some input data to a study of the effects of using surcharge on creep settlements. The analysis was conducted as finite element modeling using the commercial code PLAXIS, where creep settlements can be simulated using the built-in Soft Soil Creep model."

It is becoming increasingly common practice to re-use existing foundations for new projects, especially on urban sites where geotechnical conditions are congested. A knowledge of the foundation dimensions and capacity is essential for this to take place, however, as-built records are often non-existent. Non-Destructive evaluation (NDE) is now becoming an important tool in assessing old foundation properties and their potential for re-use. Pile dimensions are also critical in the power industry, when they are supporting transmission towers which need to be upgraded, and may carry additional wind loading. This paper describes the application of various NDE techniques that are available and how they can be used to determine pile length, shape and capacity in difficult site conditions, including foundations under structures.

"1. IntroductionThe Falconbridge Owl Creek open pit gold mine is located 17 km northeast of Timmins and 2.5 km west of the Kidd Creek metallurgical site. Diamond drilling first commenced in 1964 with open pit production commencing in 1982. Other operations within close proximity are the Canamax-Pamour Bell Creek Mine and the Falconbridge Hoyle Pond Mine.Since 1982, 1,216,000 tonnes assaying 4.5 g/t have been processed in six concentrators. This paper summarizes the metallurgy achieved with special emphasis on some of the unique problems encountered in the processing of this refractory ore.2. GeologyThe Owl Creek deposit, located 3 km north of the Porcupine-Destor fault is in easterly trending carbonaceously altered volcanic and sedimentary rocks. The deposit strikes east and dips steeply north.The ore body can be best described as a wedge shaped sandwich of volcanic basalt between the surrounding sedimentary greywacke. The north and south volcanic-sedimentary interfaces are marked by graphitic zones. The south graphitic contact is more continuous with its thickness varying from 5 m to 20 m as compared to 0.5 m to 3 m to the north. Quartz veins occur throughout the deposit with a predominant vein running along the south contact where it is characterized by numerous graphitic inclusions. Some veins carrying gold values intrude into the sediments in the north. With depth the sedimentary and volcanic zones taper while the quartz-graphitic zones appear to become more dominant."

Many cases of derailments, train accidents and traffic interruptions due to the slope failure and rockfall have been reported in the past on Konkan Railway trackline because of unstable rock slopes. The authorities installed stability enhancement measures such as wire netting, rock bolting and shotcreting which were found insufficient. The failed geotechnical measures prompted authorities to excavate the unstable slopes. Rock blasting is generally not preferred for the excavation of slopes along the transportation route. Stabilisation and widening of slopes by blasting need closure of the route for a considerable time. Discontinuing traffic on Konkan Railway route was undesirable as it connects two important port cities Mumbai and Mangalore. The paper presents an innovative directional controlled blasting (DCB) technique for excavating three unstable slopes (i.e. cuttings) in hilly terrain without disturbing the traffic. New empirical formulae for small geometry blasting to estimate burden and throw have been developed. A unique sequence of firing and excavation with powder factor varying between 0.04 and 0.39 kg/m3 irrespective of the actual free face was used for rock blasting. The process controlled the throw of blasted material over the track and resulted in a smooth and stable wall. The slope angle has been changed from 80 to 82° to 45–47° and probability of wedge, planar and toppling failures was eliminated. About 100 such accident-prone slopes (52 km cumulative length, 7% of total route) situated at discrete locations along Konkan Railway route were stabilised using the technique. No incident of rockfall and slope failures along these slopes has been observed after the stabilisation.

Educational campaigns conducted by the manufacturers of explosives, the Bureau of Mines, mining companies, and safety organizations have contributed greatly in reducing accidents caused by explosives arid blasting. Despite these campaigns and the dissemination of information, accidents from explosives and blasting continue to be the direct cause of scores of fatalities and hundreds of nonfatal accidents annually, not only in the mineral industry but essentially wherever explosives are used. The experience of many mining companies and other users of explosives has proved that accidents from explosives can be prevented or averted by the proper training of employees in the handling and using of explosives and by the enforcement of definite rules and instructions for their use. Unfortunately, throughout the years numerous false impressions and ideas have grown up and around the use of explosives. Many of these can be traced back to the manner in which the users of explosives were originally taught, if instructed at all; and as with many types of misinformation in nearly all walks of life, those false ideas are decidedly persistent and being so cause much damage. Many if not most of the users of explosives gained their knowledge of explosives and blasting by watching others and more or less "absorbing" the procedures and thrones of the other fellow, who acted unconsciously as teacher or instructor. A study of explosives accidents reveals the results of such "teachings" when the causes of the accidents are determined, and as might be expected the results very often are far from good. Seafloor massive sulfides (SMS) study in the Russian Exploration Area at the northern equatorial Mid Atlantic Ridge began after signing the contract with International Seabed Authority in 2012. In order to evaluate the SMS resources in the course of exploration the major and rare metals with economic value should be determined. The evaluation is based on geochemistry and mineralogy and particularly on the type of rare metals occurrence in the SMS. As a case study research of the Semyenov cluster of SMS deposits have been conducted.
