A special type of shotcrete is used to improve the performance of open-type TBM in difficult situations. The Hakamagoshi and Johana tunnels are expressway evacuation tunnels of 3.2 km and 5.1 km in length, respectively (total 9.1 km) that were excavated by one open-type TBM. The geological conditions at the sites were estimated to be good before excavation. However, 95% of the rock turned out to be two ranking orders worse than the estimated classes. Therefore, we used a special shotcrete for bad rock conditions, and succeeded in excavating the tunnel at an average rate of 270 m/month.

Inertisation is used to enhance the safety of underground mine areas either to avoid the potential for a combustion event or to stabilize a situation after an ignition, fire or heating. The primary objective of a recently completed Australian Coal Association Research Program (ACARP) study was to review coal mine inertisation and in particular to focus on use of the Polish mine fire simulation software Ventgraph to gain better understanding of how various inertisation units interact with the complex ventilation behavior underground during a substantial fire. Critical aspects targeted for examination under the project grant included location of the unit for high priority fire positions, size of borehole or pipe range required, time required for inertisation output to interact with and extinguish a fire, effects of seam gases on fire behavior with inertisation present and main fan management. A major accomplishment of the project has been to take findings from the simulation exercises to develop inertisation related modifications to the Ventgraph fire simulation program in conjunction with the Polish program authors. Selected mines were ?evaluated or audited?as to the ability to deliver inert gases generated from GAG units to high priority underground fire locations .A coding system has been developed and many potential underground mine fire sources cannot be successfully inertized with the GAG docked at the current specified point; this is particularly so for fires in extended areas of workings or in panels. There is a limit to the ability of the GAG jet engine to deliver exhaust down smaller dimension borehole. There is a need to examine the use of the GAG for production or proactive uses in a wider application in Australian mines responding to recovering from mine fires, spontaneous combustion heating?s, elimination of the potential explosibility of newly sealed gobs or inert mines or mine sections on closure. Some of the current uses of low flow inertisation facilities could be more effectively undertaken with the GAG unit.

The Bay Area Rapid Transit (BART) District will be extending BART service south along an 8.7-km (5.4-mile) -long corridor in the city of Fremont, California as part of the Warm Springs Extension (WSX). The WSX project will link the existing BART system to the future BART to San Jose extension. A segment of WSX is located in Fremont Central Park (referred to as the Fremont Central Park Subway Project). The Fremont Central Park Subway is considered the most complicated and geotechnically challenging portion of the project with an underground subway box being constructed near the active Hayward Fault which is capable of ground motions up to 0.75 g and is located in an aquifer which is used for water storage and supply. Soil-structure interaction analysis was used in the design of the subway box to resist deformations due to earthquake ground motions. A water-tight excavation support system has been designed to allow subway box construction while minimizing impacts to the aquifer.

San Martin Mine is an open pit, gold heap leach operation located approximately 70 kilometers from the Honduran capital of Tegucigalpa. Mining operations commenced in 2000 and were completed late in 2007, while heap leaching operations continue. Reclamation was executed concurrently with operations to the extent practical. The area has distinct rainy and dry seasons and closure objectives include: prevention and mitigation of acid rock drainage, sediment and erosion control, and mitigation of visual impacts. Final closure is well underway at San Martin and this paper describes the closure processes conducted to date, problems encountered, and the subsequent lessons learned.

"The District of Columbia Water and Sewer Authority (DC Water) is implementing its $2.6 billion DC Clean Rivers Project to reduce combined sewer overflows to the District’s receiving waterbodies and mitigate chronic sewer flooding in District neighborhoods. The First Street Tunnel project is a major component of the DC Clean Rivers Project, designed to mitigate sewer flooding and basement backups in the District’s historic and densely populated Bloomingdale neighborhood. Bloomingdale has been historically affected by sewer flooding and was severely impacted by four storms in the summer of 2012 that caused significant damage to homes, the environment and public property. As a result, DC Water accelerated the design and construction of the First Street Tunnel in order to mitigate the effect storms have on the undersized sewers serving the neighborhood. The infrastructure designed to mitigate flooding, including the tunnel, are located within the highly urbanized neighborhood – with some structures being less than ten feet from residents front door steps. The success of a large public works project of this magnitude and in this unique location depends on a well informed and supportive public. With the local community impacted by flooding but concerned about heavy construction impacts, this paper details the components of a successful public outreach plan and lessons learned throughout the design and construction process. Early identification of community mitigation, working cooperatively with stakeholders and frequent dissemination of accurate information required the project team to establish a culture of problem solving collaboration with the community. BACKGROUND Chronic Flooding in the Northeast Boundary Sewershed The Northeast Boundary sewershed, in which the Bloomingdale neighborhood is located, is served primarily by a combined sewer system and has a long history of chronic flooding problems during moderate to severe storm events. The sewer system that serves the Northeast Boundary drainage area was constructed by the Federal Government in the late 1800's when developed areas of the District terminated at what is now Florida Avenue. Since construction of the system, population within the sewershed area has grown at an exponential rate. In meeting the growing needs of the population, the District experienced development that transformed previously low-density rural areas into new communities. Impervious areas have increased drastically since the sewer system was constructed and consequently, the existing sewer system does not have the capacity to convey storms with return frequencies beyond the 2-year storm without flooding. As a result of the capacity limitations, many of the low-lying areas have become chronic flooding areas due to the surcharging of sewers."

It is undisputed that an interaction between mine fires and mine ventilation systems exists. The ventilation carries the poisonous and sometimes explosive combustion products, which constitute the greatest danger of mine fires, through the mine and it provides the oxygen on which the intensity of the fire depends. The fire, on the other hand, can produce considerable ventilation disturbances which make it difficult to predict the path along which the combustion products travel. The opinions, how serious a threat such fire generated ventilation disturbances are, vary. Where shallow deposits are mined and brisk ventilation currents are used, it is frequently felt that these disturbances are of no serious concern. Where mine workings extend over larger elevation differences and where such disturbances have been encountered and been the cause of some larger disasters, their assessment is an integral part of all fire emergency plans. The author felt that the potential of digital computers for the design of ventilation emergency plans, contrary to their acceptance for conventional ventilation planning for more than two decades, is not yet fully utilized. He designed a program for the simulation of ventilation systems under emergency conditions in particular fires. Development of improved transportation networks in major urban centers has led more and more to underground construction, the only reasonable solution for decreasing surface traffic congestion. Excavations required by these projects usually cause decompression of the surrounding soil mass which results in subsidence of several centimeters. While these magnitudes of subsidence have no consequences in undeveloped areas, they are often unacceptable in urban areas where existing buildings, structures and utilities are endangered. Recent tunneling projects for the extension of the Paris Metropolitan Transit System (R.A.T.P.) underground network were carried out with this in view and gave rise to the development of a number of innovative techniques for minimizing ground decompression and subsidence. As shown in Figure 1, the recent R.A.T.P. expansion program includes a new express metro, 46 km long, crossing Paris from East to West, and the extension of many existing metro lines in the suburbs. From the start, R.A.T.P. was faced with many unique technical problems for which there was little precedent experience. The structures to be built have large dimensions, with cross-sections typically ranging from 40 to 300 m2, and exceptionally to 600 m2. The new lines are often very deep because the shallow profile is already congested by the existing metro, interceptors, sewers and a very large number of utilities. As a result, new construction is often below ground water level by as much as 20 m. Moreover, the geology varies widely from hard limestone with compressive strength over 1500 kg/cm2 to inconsistent water bearing sand. Finally, curves in the East-West line must have a minimum radius of 500 m which frequently forces the alignment to pass below existing structures, buildings and monuments.

Silicon is the second most abundant element on earth, exceeded only by oxygen. Silicon dioxide forms a crystalline structure of repeating SiO2 molecules which form characteristically hard, durable crystals. As a result, crystalline silica is Ubiquitous in the crustal rocks, sediments and the mineral products derived from mining. An examination of the history of studies that identify silica as a human and animal carcinogen is presented in the context of the development of a public health policy in California. The implications for the cost of conducting aggregate and other mining operations in the state are discussed.

The year 2010 will mark the 100th anniversary of the beginning of the research efforts by the U.S. government to improve the health and safety of miners working in the United States. This research began in 1910 with the establishment of the U.S. Bureau of Mines (the Bureau) and it has continued during the past 10 decades, even after the closure of the Bureau in 1996. The research on safer mining technology, formerly done by the Bureau, has now been combined with National Institute for Occupational Safety and Health (NIOSH) studies of miners? health to form a unified federal research program to improve the health and safety of miners. This article describes this research and places some of the accomplishments into perspective with other activities in the U.S. Mine Safety and Health Time Line (insert).

The first version of the longwall strata control and maintenance system (LSCMS) has been successfully developed by the Strata Control Re¬search Team at the University of Alabama. The system includes a software package designed to monitor shield leg pressure, interpret the data and predict the forthcoming roof loads and the existence of geological anomalies. It can also detect maintenance problems of the face equipment. The LSCMS was installed underground and connected to the surface terminals at Drummond's Shoal Creek Mine in Alabama. The system has proven to be a useful and practical system for improving safety and efficiency in coal-mining operation. The Shoal Creek Mine is a new, large coal mine operated by Drummond with two longwall panels. The Blue Creek seam is the primary source of coal production. It is located at a depth of 275 to 440 m (900 to 1,450 ft) below the surface with an approximate 2% southwest dip. Typical strata conditions are represented by two stratigraphic columns (Figs. la and b). A fireclay/dark shale parting (called Middleman) 0.2- 0.7-m- (1- to 2.5-ft-) thick exists between the Mary Lee and Blue Creek seams. Mining conditions vary from area to area. This mainly depends on the Blue Creek seam thickness that ranges from 0.9 to 2.9 m (3 to 9.5 ft). The thickness of the Mary Lee and the parting is relatively consistent with a to¬tal of 0.9 m (3 ft). Although vegetation can be successfully established on fine taconite tailings, innovative approaches are needed to reclaim the coarse tailings used for dam construction. Through a series of cooperative projects with various government agencies and industries, EVTAC has been able to develop a unique closure plan. This plan will not only meet current reclamation standards, but also provide mitigation for a disturbed wetland and possibly create a tree plantation in the interior of the basin. By using waste products and exchanging materials, reclamation of the entire basin will be accomplised in a cost effective manner.

Design considerations and construction techniques were implemented to address groundwater infiltration from a confined aquifer during the excavation and lining of the Cady Marsh Flood Relief Tunnel. This 3-m finished diameter, 2-km long tunnel was successfully completed cost-effectively in an urban setting near East Chicago, Indiana. The soft ground was mined using a conventional tunnel boring machine with hydraulically actuated flood doors and supported with steel ribs and timber lagging. The tunnel was lined with cast-in-place concrete. Dewatering was implemented in settlement prone, boulder-laden ground in residential neighborhoods during construction to depressurize and partially dewater the aquifer at the tunnel horizon. This paper describes the design decisions and construction methods utilized to mitigate and limit infiltration into the tunnel during construction and operation, as well as manage ground settlement.

John Marks said in his acceptance of the Hartman Award in 2008 ? ?I guess that without the occasional complaint, your mine is probably over-ventilated? (Marks, 2008). Miners are seldom in this situation and often spend much of the time trying to find enough air to keep the operation running. With examination there are some things that can be done to make the best use of ventilation systems. Should there be surplus air, in which some of these options may result - financial and other benefits can be obtained. Some of the issues mine operators have needed to address recently are increased airflow requirements to dilute diesel particulates in metal and non-metal mines. Also, operations are faced with demands from reserves that are deeper, hotter and further away from fans and shafts, not to mention increased production demands. Mine ventilation air is a costly commodity, especially by the time it has been heated or cooled and moved to the bottom of the shaft and through the mine airways. Most of the options presented in this paper are useful for metal non-metal operations but some may be applicable to coal too. OPTIONS FOR IMPROVEMENT Some of the areas to be addressed are shop ventilation methods, auxiliary equipment areas, dedicated intakes and returns, optimizing development, examination of mine airway utilization and alternative ventilation methods. UNDERGROUND SHOPS First, examine what is being done with the underground shop air. If it is being coursed directly to the returns there may be some potential to better utilize this, in most cases, relatively contaminant free air. Research the legal options for ventilating mine shops. MSHA regulations in 30CFR 57.4761 (1) allow the following options: To confine or prevent the spread of toxic gases from a fire originating in an underground shop where maintenance work is routinely done on mobile equipment, one of the following measures shall be taken: use of control doors or bulkheads, routing of the mine shop air directly to an exhaust system, reversal of mechanical ventilation, or use of an automatic fire suppression system in conjunction with an alternate escape route. The alternative used shall at all times provide at least the same degree of safety as control doors or bulkheads. Discussion This regulation gives some operators flexibility in ventilating underground shop facilities. If shop air is to be coursed through the work area, and then directly to the mine return, the question should be asked ? is the quality of air good enough to be reused in the mine, and if so, what are the risks? Can the risks be accommodated and managed to accomplish the change? If so, a considerable increase in mine level airflow could be attained by reusing shop air. Clearly, regulations are a minimum standard, and can be exceeded. Metal non-metal mine regulations are not as specific as coal due to the multitude of different mining methods and conditions utilized. Risks must be thoroughly examined and addressed to assure a safe environment. Issues Determine what contaminants are being produced in the work area ? welding fumes, diesel exhaust, paint fumes, chemicals and solvents, etc. Survey the area and determine exposures. MSHA requires exposure monitoring in 30 CFR 57.5002 (1) of dust, gasses mists and fumes to assure air quality is being met. Perform surveys and examine existing data to quantify personal and area concentrations of contaminants. Often there is adequate circulation such that exposure levels in shop work areas are low. After sending shop air back into the system any contaminants would be further lowered by dilution with other mine intake air downstream. Look at the shop work schedule. If only on day shift, the other two shifts and possibly weekends have no activity or source of contamination to affect the ventilation system. The worst case planning scenario would be a shop fire. Study and simulate how this would affect the mine and shop area. MSHA regulations require an escape route and a fire suppression system be installed and maintained as follows in 30 CFR 57.4671 (1): (d) Automatic fire suppression system and escape route. If used as an alternative, the automatic fire suppression system and alternate escape route shall meet the following requirements: (1) The suppression system shall be? (d)(1)(i) Located in the shop area; (d)(1)(ii) The appropriate size and type for the particular fire hazards involved; and (d)(1)(iii) Inspected at weekly intervals and properly maintained. (2) The escape route shall bypass the shop area so that the route will not be affected by a fire in the shop area. Case Study In an underground room and pillar mine, active mine workings were advancing further away from the mine shaft with deteriorated returns and leakage having a negative effect on ventilation airflows to the face. Mine level airflows were 236 m3/s (500,000 cfm) and shop airflows were 24 m3/s (50,000 cfm) (see Figure 1A). No additional capacity was available at the main surface fan. A solution was to utilize the shop air then send it to the mine areas instead of directly to the return. Considerable information on shop air quality and exposures from many years of operation was examined and no negative data was discovered. A risk analysis determined that moving forward with a shop air reuse program was desirable. A plan was developed, analyzed and decision to proceed approved by mine management. The change was simulated on the computer with the USBM/MTU MFire program before being implemented. This included utilizing fire scenarios to examine potential exposures to inby mining sections and plan escape routes. "Longwall abutment loads are influenced by several factors, including depth of cover, pillar sizes, panel dimensions, geological setting, mining height, proximity to gob, intersection type, and size of the gob. How does proximity to the gob affect pillar loading and entry condition? Does the gob influence depend on whether the abutment load is a forward, side, or rear loading? Do non-typical bleeder entry systems follow the traditional front and side abutment loading and extent concepts? If not, will an improved understanding of the combined abutment extent warrant a change in pillar design or standing support in bleeder entries?This paper details observations made in the non-typical bleeder entries of a moderate depth longwall panel; specifically, it explores data collected from borehole pressure cells and roof extensometers, observations of the conditions of the entries, and numerical modeling of the bleeder entries during longwall extraction. The primary focus was the extent and magnitude of the abutment loading experienced due to the extraction of the longwall panels. Due to the layout of the longwall panels and bleeder entries, the borehole pressure cells (BPCs) and roof extensometers did not show much change due to the advancing of the first longwall. However, they did show a noticeable increase due to the second longwall advancement, a maximum of about 4 MPa of pressure increase and 5 mm of roof deformation. The observations of the conditions showed little to no change from before the first longwall panel extraction began to when the second longwall panel had been advanced more than 915 meters. Localized pillar spalling was observed on the corners of the pillars closest to the longwall gob as was an increase in water in the entries. In addition to the observations and instrumentation, numerical modeling was performed to validate modeling procedures against the monitoring results and evaluate the bleeder design. ITASCA Consulting Group’s FLAC3D numerical modeling software was used to evaluate the bleeder entries. The results of the models indicated only a minor increase in load during the extraction of the longwall panels. These models showed a much greater increase in stress due to the development of the gateroad and bleeder entries—about 80% development and 20% longwall extraction. The FLAC3D model showed very good correlation between modeled and expected gateroad loading during panel extraction. The front and side abutment extent modeled was very similar to observations from this and previous panels."

An assessment of U.S. western hardrock miners? noise exposures was conducted as part of a multi-year National Institute for Occupational Safety and Health (NIOSH) survey of noise exposures in each sector of the mining industry. Noise from selected mining equipment and operator noise exposures were measured, analyzed and tabulated for dissemination to the participating sites and are being used to direct NIOSH research and interventions to address the greatest noise hazards. Eighty-two noise dosimeter measurements were obtained, along with time-motion studies as the miners operated hardrock mining machines. Of the operators, 96% had daily noise doses that exceeded the U.S. Mine Safety and Health Administration?s (MSHA) permissible exposure level. The average gold miner dosages while operating the following equipment were: haul trucks - 261%, load-haul-dumps (LHDs) - 235%, single boom drills - 221%, bolters - 214% and dual boom drills - 163%. The worst exposure level was a silver miner with a daily dose of 873%. Time-motion data showed that this miner?s exposure accumulated most rapidly while operating a jackleg drill. These results will be used to help prioritize noise control development by NIOSH and other partners.

Decisions made during a response to a mine fire or an explosion greatly affect the final outcome. The safety of miners should be the primary concern with accounting for all of the miners the first issue for resolution. If miners have not safely evacuated from the mine, the last known location of the miners at the time of the event and anticipated travel routes are primary exploration areas. After the mine is evacuated, there are many very important decisions that must be made quickly and often under great stress. It is critical that the mine operator establish an orderly method to direct the mine emergency response. Designation of a Command Center, staffed with personnel charged with developing plans, is a common method of establishing control. Scheduling of key mine managers will be critical, since decisions must be made 24 hours a day and people working for extended periods with little or no sleep do not always make clear or cognizant decisions. Internal communications, passing information on to subsequent shifts, and external communications with families and media must be accurate and timely. Assembling resources for responding to the mine emergency is critical for an effective response, and pre-planning can pave the way to a successful recovery. Sampling mine gasses and accurately determining the composition of gas mixtures is a common tool used to develop response plans in all fires and explosions. It is essential that preparations for emergency situations are made well in advance of a disaster. Effective communications are essential for mine management to successfully respond to a mine emergency. This paper discusses some considerations which need to be addressed in directing a response to a mine fire or explosion.

The Manapouri Power Station is an underground hydro-electric station located in the South Island of New Zealand, and was constructed in the 1960s. Between 1997and 2002, a second 10m diameter tailrace tunnel was constructed to increase the out-put and to improve the overall hydraulic efficiency and output of the station. A critical economic and project approval consideration for the second tunnel was the outage time required for the breakthrough into the power station. The power station is in part dedicated to providing power to an aluminum smelter with the balance to the National Grid, and apart from the physical problems associated with providing an alter-native power supply to the smelter, the loss of generation revenue and supply risk were significant issues. Innovative planning reduced the planned outage time from several months to21 days, and the breakthrough was eventually achieved in 11 days as a result of additional value engineering measures implemented during construction. This paper describes the engineering optimization that took place during the planning stages of the project and later during construction, as well as describing the construction methodology used.

In April 1984 a subsidence depression formed directly beneath a two-story frame residence, near Linton, IN, causing 2 ft of separation between the floor and walls, and damaging both the foundation and interior of the building. The Office of Surface Mining undertook the rehabilitation of the area under its emergency response program. This paper describes the investigation performed to determine the cause, severity, and extent of the problem. Abandoned mine workings were found approximately 55 to 60 ft below the house. Based on the findings, remedial measures were prescribed. An abatement program consisting of grouting the mine voids was recommended, and specifications prepared. These were implemented, and the area has been successfully stabilized.

Face and pillar bursts, bumps and bounces are violent failures that occur in underground coal mines in response to a complicated interplay of face and pillar geometry, seam depth, coal properties and interactions between roof, seam and floor strata. Additional complications arise from the presence of gas, mainly methane, and associated pressure and flow that vary with time and are further influenced by the rate of face advance. Where gas plays a dominant role, the term outburst is often used; where stress concentration is primary, terms such as burst, bump or bounce may be used. Hargraves [1983] presents an often cited review of outburst phenomena with emphasis on Australian conditions. Beamish and Crosdale [1998] indicate outbursts have occurred for over a hundred years and present tabulations of outburst numbers in major coal basins throughout the world. A central consideration is the rate of depressurization that occurs in the course of mining. Outbursts are not limited to coal seams and adjacent strata; they also occur in salt and potash mines [Molinda et al 1988]. Stress concentration and gas pressure act in consort, so the combined action is important to understanding the mechanics of bursts or outbursts and to the development of measures to reduce the associated hazard. Gas flow is a complicated phenomenon associated with diffusion of gas from micro-pores and the flow of gas along cleats. Laboratory measurements indicate both are dependent on stress and pressure [Harpalani and Ouyang 1998]. Moreover, gas permeability in porous media is often many times water permeability [Klinkenberg 1941]. However, there is also evidence that gas and water permeability are similar at elevated stress and follow Darcy?s law [Dabous et al]. Gas flow is also of importance to mine ventilation. Price et al [1973] in cooperation with personnel of the former U.S. Bureau of Mines developed finite difference models to estimate methane emission in mines in the Pocahontas No. 3 and Pittsburg seams using a premining pressure of 4.69 MPa (680 psi) and Darcy's law implying laminar flow. However, the effects of stress were not taken into account. In this regard, mine measurements of permeability are usually based on Darcy?s law [Peide 1990]. Early models of gas flow with stress effects were one-dimensional in space and time (x,t) and were developed with the goal of estimating the gas pressure profile from the face into the seam as time passed [Schlanger and Paterson 1987, Zou and Yu, 1999]. A zone of high permeability adjacent to the mining face was recognized in these early models. Later models based on the popular finite element method allowed two spatial dimensions (x,y) and variation in time (t) and for coal seam heterogeneity [Tang et al, 2002]. Zhu et al [2007] formulated a set of nonlinear equations involving a complicated stress-pressure-permeability relationship for application to coal seam gas flow and concluded the coupling was important but failure of coal should also be considered. Wold et al [2008] used a hybrid finite difference - finite element model interleaved with a reservoir simulation model to study outbursts based on extensive mine and laboratory measurements. An important feature of the modeling was the use of spatially variable material properties and Monte Carlo simulation to obtain quantitative estimates of outburst conditions and probabilities. Connell [2009] used the same reservoir simulation model as Wold but used the popular finite difference code FLAC3D for hydro-mechanical responses to evaluate two permeability relationships, one depending on porosity and the other on stress, and concluded that permeability changes with gas production were more complex than computed by either permeability model. Gadde [2010] in an integrated laboratory and field study of coal pillar strength explored the effect of spatial variability and concluded that design based on average strength was acceptable in some circumstances, although gas pressure was not an issue. Liu et al [2011] also applied a FLAC code to analyze two-dimensional gas flow and deformation of layered but homogeneous strata in a Chinese coal mine using a strain-softening model and concluded that damage and gas pressure were important to strata safety relative to the case without gas.

The Uetliberg Tunnel in Switzerland is the longest tunnel in Zurich?s new Western Bypass Expressway, connecting the Birmensdorf bypass in the west with the existing Zurich-Chur national motorway in the east. Thus the tunnel links the Zurich-West motorway interchange with the Zurich-South interchange (shown in Figure 1). The Uetliberg Tunnel project comprises two parallel tubes, each about 4.4 km long. The two tunnel tubes are connected every 300 m by a transverse walkway and every 900 m by a transverse roadway (Figure 2). The SOS niches are spaced 150 m apart. Portal stations with machinery rooms are located at the west and east portals. The tunnel falls with a gradient of 1.6% from the west portal at Wannenboden to the east portal at Gänziloo. The Project costs are about CHF 1.12 billion. The carcass construction started in March 2001. The Opening of the Uetliberg Tunnel is planed in spring 2009.
