While preparing this paper, I soon realized I had a problem in defining what injuries should be classified as "personal." Admittedly, in some cases, falls of persons, sprains and strains, pulling and pushing injuries, and back injuries may have been caused by the individual. But, I believe the number of injuries directly attributed to an individual's carelessness is relatively small in comparison to the total number of injuries in those categories. For discussion purposes, the injuries most often considered to be personal, that is, caused by individuals are those classified as back injuries. My paper will be devoted to back injuries because this category represents the greatest number of injuries occurring in coal mining. A man's best friends are his wife, his back, and his dog. The back, however, has a reputation of not being as faithful as his other two friends.1 "Industrial back injury is one of the major occupational health problems plaguing industry to- day. According to national surveys, back injury accounts for more lost time than any other occupational injury and costs companies thousands of dollars in compensation, medical fees and over- time pay."2 A study conducted by USA Today found that back ailments afflict an estimated 75 million Americans, have totally disabled about 2.5 million adults, lead to as many as 200,000 required surgeries each year, are a leading cause of absenteeism and disability, are the second leading cause of hospitalization, are responsible for 18 million doctor visits a year and cost about 93 million work-days annually. The cost to industry is estimated at $14 billion a year.3 During the past 30 years, a great deal of accident prevention education has been directed toward the reduction of back injuries. However, this effort has not produced any great change in the incidence of the injury. Back injuries should be of the utmost concern to everyone here today. I don't need to tell you what the incidence rate is for back injuries, nor will I use any statistics to inform you of the incidence rate for the coal mine industry or any specific mine. It is only important to know that the number of back injuries is excessive. Back in- juries resulting from lifting have been and continue to be one of the most constant and undiminishing problems in the occupational environment. The fact is, we use our backs to per- form work that the back was not designed to do. Historically, manual materials handling related accidents have consistently accounted for the largest percentage of all industrial accidents. In fact, over 25 percent of all industrial accidents are related to handling of materials.4 In underground coal mining operations, material handling accidents account for even larger percentage of lost time injuries. In 1983, for example, they accounted for 34 percent of all lost time injuries and 32.6 percent of all non-lost time injuries. 5 I want to use one example of a task that will illustrate what I have just said: the building of a concrete block stopping. Each solid concrete block will weigh from 60 to 70 pounds. To build a stop- ping 6 feet high and 20 feet wide will take approximately 80 blocks. On the average, these 80 blocks will be handled at least four times from loading into the conveyance for transportation to being placed while constructing the stopping. In fiscal 1996, the US Congress directed that the US Bureau of Mines (USBM) be closed. Certain functions were reassigned to other agencies. The remaining employees were separated and the unoccupied sites were decommissioned. As a result, 12 locations were closed and about 1,200 employees were terminated in March 1996. About 600 employees at five locations were reassigned to other federal agencies. This article provides a summary of the continuing functions, the agencies involved and the points of contact for each continuing function. The functions that were discontinued are also identified. The functions that have been maintained and transferred to other agencies are: ? Portions of USBM's Mineral Information and Analysis program formerly conducted in Washington, DC and Denver, CO were assigned to the US Geological Survey (USGS). ? The Mineral Land Assessment program in Alaska (Anchorage and Juneau) was transferred to the US Bureau of Land Management (BLM). ? The Health and Safety Research program in Pittsburgh, PA and Spokane, WA was reassigned on an interim basis to the US Department of Energy (DOE). In fiscal year 1997, it was permanently assigned to the National Institute for Occupational Safety and Health (NIOSH). NIOSH is part of the Centers for Disease Control and Prevention within the US Department of Health and Human Services. ? Environmental Research at Pittsburgh, PA was assigned to DOE. ? Materials Research at Albany, OR was assigned to DOE. ? The helium program was assigned to BLM, where it will be managed pending privatization. ? Coal and noncoal mine maps were transferred to the Office of Surface Mining Reclamation and Enforcement (OSMRE). The following locations were closed: ? Research centers in Denver, CO; Minneapolis, MN; Reno, NV; Rolla, MO; Salt Lake City, UT and Tuscaloosa, AL. ? Field operations centers in Denver, CO and Spokane, WA. ? Washington, DC headquarters office. To provide a concise picture of all of the affected functions, the following is a synopsis of the USBM program functions and facilities immediately before the closure. There were two principal programs: information and analysis, and research.

"Design of effective mine water treatment plants depends on the ability to reliably predict treatment outcomes, but systems with multiple chemical and physical treatment processes and recycle loops can be difficult to model. GoldPHREEQC is a modeling framework that integrates mass and flow routing capabilities of GoldSim software and water quality calculations provided by PHREEQC, and is well-suited to modeling parameters of interest in the mining industry, especially metals and sulfate. Physical processes such as membrane separation and greensand filtration are modeled by defining mass rejections for each parameter and flow routing based on vendor projections and bench- or pilot-scale equipment testing. GoldPHREEQC models chemical precipitation processes such as HDS metals removal and gypsum precipitation by (1) performing surface-aging and solubility calculations in PHREEQC and (2) using the GoldSim framework to direct supernatant and precipitate to subsequent process steps. Users can apply this modeling framework to predict effluent water quality, chemical requirements, sludge production, and required equipment capacity to support design, troubleshooting, and cost estimating. Pit and underground mine operators typically use a lot of big equipment tires. Just as typically, they spend a lot of money they shouldn't on these tires. That's because tire use and maintenance are not given enough attention. That often occurs because mine management and personnel do not know the basic facts of tire use and care. If a piece of large equipment is damaged or ruined, a good explanation better he forthcoming. A ruined tire is often just a shoulder shrug. But that sort of fatalistic acceptance can cost tens or hundreds of thousands of dollars a year extra for big equipment tires. That does not have to be. There are some basic tire considerations that, if followed, will extend tire life and lower tire costs. Most companies have the inevitable conflict between the production department, which resists any restrictions that may reduce productivity, and the maintenance department. Production success is based on meeting a schedule of yards moved, while maintenance success is based on availability and equipment cost. When production falls behind schedule and equipment speeds are increased, maintenance may soon find its tire costs rapidly increasing, unless everyone understands the tires' vulnerability to damage. Unless the production department understands the problem, or restraint is applied by top management, costs may become exorbitant. At one job site, tire costs alone exceeded $250,000, to move 55 dam3 (72 000 yd3) of dirt on an 11-km cycle. The operation temporarily caught up with its schedule, but tire costs and attendant downtime soon made this a not-for-profit exercise. Because an operator must always attempt to move payload at the lowest cost, there are cases when higher than normal tire costs must be accepted. For example, in pioneering work with tandem engine scrapers, steep grades shorten tire life to less than normal. But the steep grades make shorter haul cycles possible, yielding more trips per hour than alternative earth-moving methods.

The Waste Isolation Pilot Plant (WIPP) project is authorized by legislation to provide a demonstration of the safe disposal of radio- active wastes generated by the national defense programs and activities of the United States. The U.S. Department of Energy (DOE) is proceeding with the implementation of the WIPP project in accordance with the "Department of Energy National Security and Military Application of Nuclear Energy Authorization Act of 1980" (PL 95-164).

Within the research project "Mine Development" funded by the "Austrian Fonds zur Forderung der wissenschaftlichen Forschung (FWF) " (funds for scientific research) between 1991 and 1993 150 underground hardrock mines in Europe, Northern America, Australia and Japan have been visited to analyze their general design and layout. The main aim of the project was to develop strategies for accessing new underground hardrock mines and to establish a mine planning database. The investigation included mines in high developed countries only, with a comparable labor cost situation. This paper presents the results of one part of the project, which deals with investigations into the influence of different parameters on the design of the primary access for a mine.

Volcanic activity occurred during six periods of Arizona's geologic history. Precambrian volcanism occurred in two pulses at 1.8-1.6 billion years (b.y.) ago and at 1.2-1.1 b.y. ago. Volcanism occurred again in mid-Jurassic and Laramide (Late Cretaceousearly Tertiary) time. Volcanic rocks from these times are not used commercially in any significant quantity. Middle and late Cenozoic magmatic activity covered large areas of Arizona with voluminous volcanic rocks. During the mid-Tertiary magmatic pulse, explosive silicic magmatism, with eruption of areally extensive ash-flow tufts, covered large areas in the Basin and Range Province, while the Colorado Plateau remained largely unaffected. Basalt eruption also occurred during and immediately after silicic volcanism, leading to compositionally diverse suites of volcanic rocks. Late Cenozoic volcanism occurred throughout Arizona and was dominated by wide-spread eruption of basalt flows with local cinder-cone clusters and siliceous volcanic deposits. Perlite deposits near Picketpost Mountain in northeastern Pinal County account for the bulk of the industrial uses of mid-Cenozoic volcanic rocks. Other perlites of presumed middle Tertiary age are present in central, southeastern, and western Arizona, but to date remain unexploited. Arizona Fire Agate is a valuable gem material and is associated with volcanic rocks of middle Tertiary age. Basaltic rock of this age also is utilized in the manufacture of rock wool for insulation. Late Cenozoic volcanism produced most of the industrial-quality volcanic rocks. Basalt, cinder cones, and materials of rhyolitic composition are commercially exploited by municipal and Federal agencies and private industry for aggregates, road conditioners and maintenance materials, asphaltic concretes, cinder block construction, pozzolans, and landscaping. These deposits are generally confined to the Flagstaff and Springerville areas. A famous peridot gem locality is associated with late Tertiary basalts at Peridot Mesa on the San Carlos Indian Reservation. Important products derived from altered volcanic materials of late Cenozoic age include clay and zeolite. Arizona's volcanic-rock sequences are still incompletely known, particularly in central and western Arizona. The potential for recognition and exploitation of important new deposits of useful volcanic materials is high.

INTRODUCTION The planning process for projects requiring construction of tunnels in urban environments can be highly complex and involves interaction with professionals of many disciplines. Typically the initial step in the planning process is to establish basic design criteria for the tunnel or tunnel system consistent with its intended purpose. Such criteria might include acceptable tunnel geometry, minimum and maximum slope or other associated criteria. Ideally, once the basic criteria have been established, the next step in the process might include establishing technically feasible horizontal and vertical alignment and shaft siting alternatives and developing "ball park" construction and operational cost estimates for the alternatives. Comparisons can then be made regarding the relative technical and economic merits of the alternatives and the most favorable alignment(s) are selected for further study or preliminary design. Typically the last and perhaps the most important task that is conducted during the planning stage includes identifying issues and potential problems to be addressed during the design and construction stages of the project. Many issues warrant consideration during the planning phase of a tunneling project. For the purposes of this paper, these issues can be divided into two broad categories: technical and non-technical. Important technical issues include, among others, general design criteria, geotechnical and geoenvironmental issues, and constructibility issues. Important non-technical issues include, among others, right-of-way and easement related considerations, project scheduling constraints, both short and long term environmental impacts and overall cost/benefit aspects of the project. With respect to influence on the planning process, these two groups of issues often cannot be separated completely. Future ore reserves of major gold producers in North America and elsewhere will consist of refractory gold ores. About 60 % of the current U.S. gold production is mined in Nevada. Several of the large gold operations in this state indicate that 40 to 90 % of their future ore feeds will contain refractory gold. To com­pound problems, average head grades for refractory U.S. gold ores had fallen from 8 - 9 g/mt to about 4 g/mt by 1991. In Latin America, 52 % of the gold operations will require milling for optimal recovery. Deeper reserves are, like in North America, refractory. Therefore a special treatment process is needed in order to allow for the gold ore to be leached downstream in a conven­tional cyanidation process for a high gold recovery. There are several options available for this oxidation process like treat­ment of the gold bearing ore in pressure oxidation, in a biooxi­dation or in a roasting plant depending on the reason for the refractoriness. If organic carbonaceous matters and sulfides are the prima­ry reasons for the refractoriness, whole ore roasting is a pre­ferred method for the treatment of this type of ore. Since dry ore is to feed to the roaster, a dry grinding mill is advantageous. For the dry grinding system, the moisture content and the ore char­acteristic is of importance. Those parameters decide the mode of the grinding system. For autoclave systems common wet grind­ing systems can be applied. This paper deals with technical aspects for dry grinding plants and gives an outlook on and pos­sibilities for new grinding systems.

Diatomaceous earth or diatomite is a sedimentary rock of organic origin that is composed principally of the silicious skeletal remains of diatoms. Diatoms are microscopic single cell acquatic plants that are related to algae. Diatoms first became abundant in the Cretaceous period from about 65 to 135 million years ago. Most of the commercial deposits are of Pliocene through recent age, and are younger than 35 million years. Diatoms are still abundant in present day lakes and seas. The living diatom is supported and protected by two half cell walls or valves united by a connecting band or girdle. These valves are composed of opaline silica extracted from the water in which the diatom lives. After death of the diatom, the silicious remains settle in the lake or sea and accumulate at variable rates approaching several millimeters per year for very active diatom colonies. This accumulation of silicious sediments, after decay of the organism, burial, and uplift, can result in the formation of diatomaceous earth deposits.

Direct-reduction processes for converting iron ores to metallic iron in the solid state have been available for some time; and whenever scrap becomes expensive, there is a new wave of interest in these processes. It is well established that direct reduction, coupled with electric-furnace steelmaking, is a technically viable method for making steel and in some circumstances is economically competitive with the conventional blast-furnace--basic-oxygen-process (BOP) route to steel. This raises a number of interesting questions, several of which could be of great interest to the coal and ore mining industries. For example, what are the quality requirements of the raw materials for direct-reduction--electric-furnace steelmaking? Would they be different from the present requirements for blast-furnace--BOP steelmaking? Other questions relate to how much steel will be made by the direct-reduction route in the future. The quality questions can be discussed with a fair degree of certainty, but a prediction of the amount of steel that will be produced by the direct-reduction--electric-furnace route is beyond the scope of this paper. However, an attempt will be made to identify some of the factors that will influence whether there is a swing to direct reduction and the rate at which it may occur.

During blasting, rock breaks and moves but standard grade control practices do not adjust ore waste boundaries to cater for blast movement. Rock movement resulting from blasts can result in mischaracterization of grade boundaries resulting in ore loss and dilution. Ore loss takes place when ore is mischaracterized as waste and sent to waste dumps, and dilution occurs when waste is mischaracterized as ore and sent for processing. Las Bambas operations of MMG Peru, realized the economic impact of blast movement on ore loss and dilution and adjust ore boundaries using limited blast movement measurements. This is a step in the right direction but has limitations for everyday application. This paper discusses the advantages of disadvantages of current blast monitoring techniques and introduces an operational tool to estimate polygon movements for different blast designs and confinement conditions. Blast movement estimates from this new tool have been compared against the estimates from the current tools for several production blasts and the results were quite promising.

Originally, the term solution mining applied to a dissolution, washing, or "brining" of soluble salts such as halite (common table salt), potash or trona. After the mid-sixties, however, the definition has been expanded to include heap and dump leaching, in-place (in situ) leaching, removal of oil from tar sands, extraction of sulfur from deep wells by the Frasch process and the recovery of metalliferous solutions from geothermal sources. Solution mining is also known as borehole, non-shaft, or chemical mining; the extraction of soluble mineral values from a variety of ores using appropriate solvents and chemicals. Solution mining frequently involves chemical changes; for example, copper usually goes into solution as copper sulfate, gold and silver as a cyanide compound, and uranium as a sulfate or uranyl tricarbonate. The valuable constituent is removed from the solid body of the ore by mass transfer to a aqueous solution.

On the Earth's Moon, obvious hazards to humans and machines are created by extreme temperature fluctuations, low gravity, and the virtual absence of any atmosphere. The most important other environmental factor is ionizing radiation. Less obvious environmental hazards that must be considered before establishing a manned presence on the lunar surface are the hazards from micrometeoroid bombardment, the nuisance of electro-statically-charged lunar dust, and an alien visual environment without familiar clues. Before man can establish lunar bases and lunar mining operations, and continue the exploration of that planet, we must develop a means of mitigating these hazards.

In this investigation, the changes in the grindability of both ulexite and colemanite related with the different structural modifications created by the heat treatment were examined with respect to work index (Wi). The effects of the heat treatment temperature and time on grindability index were also investigated. It was observed that, heat treatment significantly affected on the grindability of ulexite. The work index of original ulexite was found 7.11 kWh/shton and decreased to 3.5 kWh/shton level within 200-480°C for 60 minutes. On the other hand, the work index of original colemanite was found as 6.48 kWh/shton and no appreciable change was observed when heated up to 320°C. Thermally assisted comminution can be used to improve the grindability characteristics of ulexite. Besides, the different behaviour of ulexite and colemanite under heat treatment can be used for their separation. The artificial mixture of ulexite and colemanite could be separated with over 90% recovery by utilizing their different grindability characteristics by thermal treatment, semi-autogeneous grinding and screening.

This paper aims at the application of quantitative criteria for evaluating the mineral potential of square cells representing areas in terrains being explored at a reconnaissance level. The assumption is made here that the evaluation methods used by an expert economic geologist can be organized into geological models for establishing associations within imaged areas, that correspond to pixels with similar characteristics. UTM-registered multiple data sets have been constructed which include seasonal LANDSAT 2 and 4 images, SEASAT scenes, selected airborne radar images, digitized aeromagnetic, gravity, bedrock geology and topographic elevation maps. The area of interest is located in the Bathurst Inlet, District of MacKenzie, N.W.T., where a preliminary mineral potential evaluation was performed by the Geological Survey of Canada to assess its suitability for a national park. The multiple data sets described in this paper can be used for many studies at different resolutions, for comparing processing methods in remote sensing and, most importantly, for evaluating the physical and spatial relationships between the different images in registered data sets.

The redesign and reinforcement of a highwall located adjacent to a $10,000,000 shopping center, destabilized by active subsidence, was conducted after an extensive geotechnical investigation. The highwall was excavated in a hillside where a coal seam had been retreat mined and augered. Engineering geological mapping, determination of the properties and behavior of the intact rock, fractured rock, and urethane grout material, and soil were required to characterize the highwall and conduct a stability analysis. Remedial measures included re-excavating portions of the highwall to reduce the slope angle, and pressure grouting of the immediate roof.

With the utilization of most of the "high dam" sites for hydro-electric power and the completion of most railroad routes, many tunnel contractors were extremely doubtful that tunnel driving would continue to be an important element of heavy contracting. Experience has proved otherwise. The ever-increasing need for rapid transit and for pollution control have generated, and will continue for the indefinite future to generate, a very high level of tunneling activity. At the same time, the costs of construction have traveled an upward curve in the tunnel industry, as in all other types of construction. As a result, Owners, Engineers and Contractors have sought ways and means of constructing more economically through the development and use of labor-saving devices and techniques. The development of mining machines and lasers has contributed tremendously to such economy. Before giving you a contractor's viewpoint on Owner-Engineer-Contractor relationships in tunneling, it may be well to provide some background as to matters which give rise to that viewpoint. Construction contractors in general, and tunnel contractors in particular, have always had to contend with the physical environment in which the work has to be accomplished. However, when a building contractor has constructed his foundation, and certainly after he has "closed in", he deals thereafter with known physical conditions, or at least with conditions which he could reasonably have estimated would occur. Except, perhaps, for the hazard as to earthquakes, he has cleared Nature's threats. A tunnel contractor, however, must give thought to what the physical environment may do to his job, each and every day that he is driving a tunnel. Rock melting technology can provide in-situ glass liners in nuclear waste package emplacement holes to reduce permeability and increase borehole stability. Reduction of permeability would reduce the time and probability of groundwater contacting the waste packages. In-creasing the stability of the storage boreholes would enhance the retrievability of the nuclear waste packages. The rock melting hole forming technology has already been tested in volcanic tuff similar to the geology at the proposed nuclear waste repository at Yucca Mountain, Nevada.

"INTRODUCTION While the United States coal mining uses prescriptive regulation under the Title 30 Code of Federal Regulation enforced by the Mine Safety and Health Administration (MSHA), Australia uses a risk based approach. Australian regulation is individual to each state. In Queensland, Australia this risk based approach to address principal hazards is underwritten by the Queensland Coal Mining Safety and Health Act of 1999 and the Coal Mining Safety and Health Regulation of 2001. Each Queensland Mine is required to have an overarching Health Safety and Environment Management System, of which Safety Management Plans are an element. “Safety Management plans (SMP’s) are intended to formalise the process by which mines address principal hazards and other related matters to ensure the safety of mine personnel.”7 THE QUEENSLAND CONTEXT In Queensland, the basic elements of a coal mine’s safety and health management system must provide for risk identification and assessment, hazard analysis, hazard management and control, and the reporting and recording of relevant safety and health information and data.5 (Part 2 Division 1 Section 6(a-d)) The initial guidance for writing Safety Management Plans comes from the Approved Standard for Mine Safety Management Plans and Guidance Note QGN09- Reviewing the Effectiveness of Safety and Health Management Systems. The Guidance Note and Approved Standard are supplemental to the Act and Regulation, and put in place to provide more clarity around the topic. The definitions in this section come from the Approved Standard and are included to provide context for those who may not be familiar with Australian or Queensland law and regulation. A hazard is defined as a source of potential harm or a situation with a potential to cause loss, while a principal hazard is a hazard with a potential to cause multiple fatalities. A risk is the chance of something happening that will have an impact upon objectives which is measured in terms of consequences and likelihoods. It is important to be able to make the distinction between a hazard and a risk. For instance, a lion is a hazard, while the risk associated with a lion is getting eaten. As defined by the Queensland Approved Standard for Mine Safety Management Plans, a Safety Management Plan (SMP) requires a systematic definition of all the actions necessary to ensure that mining operations are carried out safely. A SMP includes, but is not limited to organisational structures, planning, activities, responsibilities, practices, risk identification, audits and reviews.7 The goal of a SMP management plans is to mitigate the risks to bring them to an As Low As Reasonably Achievable (ALARA) level."
