This paper explores the use of a Process Safety Management (PSM) system to manage risk in the mineral processing and metallurgical refining industries. Important elements of a PSM system are described. International regulations and non-regulatory guidelines covering PSM for high hazard industries are reviewed. Case studies of incidents in the mineral processing and metallurgical refining industries are presented to understand the lessons learned from failure to manage risks using a PSM system. Conclusions are drawn to adopt PSM in the minerals and metals processing industry as a tool for loss prevention and risk management.

This paper discusses why Argyle Diamonds adopted thickened tailings disposal in 1985 and how the system was developed over the next seven years. A wide ranging review of the thickened tailings disposal system and the tailings deposit it was producing was carried out during 1992 and 1993. The results of that review led to the abandonment of thickened tailings disposal at Argyle because of serious concerns about the stability of the resultant tailings deposit. The exposure to the risk of liquefaction and an assessment of the consequences if this happened to the thickened tailings deposit are shown. The criteria for selecting the new tailings disposal strategy are then described. The final section of the paper discusses the management strategies that have been put in place. These are designed to: ò reduce the risk posed by the thickened tailings deposit to an acceptable level; ò safely store tailings to the end of mine life; ò produce a land form that can be decommissioned to the satisfaction of Government regulatory bodies.

"In the last decade Afitex developed the Draintube™ geocomposite, which provides simultaneous drainage and waterproofing functions. This new type of drain tubes planar geocomposites (DTPG) differs from other geocomposites as the drainage core is composed of multiple corrugated and perforated pipes instead of biaxial or triaxial nets. High-accuracy, solid-state digital reference barometers have proven to be well suited to conducting underground barometric pressure surveys. The only problem is their expense; at least A$4000 each. For this reason, and because barometric pressure surveys are not a routine undertaking, very few mines have been able to justify the purchase of suitable instruments. The author knows of only three Australian metalliferous mines that own them.However, this may soon change given the recent development of low-cost, mass-produced miniature barometric pressure transducers. These transducers were developed to provide an altimeter function in the latest generation of smartphones. The main purpose of the inbuilt altimeter function is to allow the smartphone to be aware of its altitude when the global positioning system satellite signal is lost.This paper describes a prototype barometer that was constructed using one of the most accurate of the latest generation of piezo-resistive smartphone barometric pressure transducers. The prototype instrument was constructed for less than five per cent of the cost of a commercially available digital reference barometer. The prototype was subsequently tested against a reference barometer at the University of Queensland’s Experimental Mine to verify its accuracy and suitability for measuring pressure differences between underground survey stations.It is expected that the instrument’s low cost will pave the way for barometric pressure surveys to become more commonly used in mines. In turn, the resulting improvement in knowledge of major underground pressure losses should allow mine ventilation officers and engineers to optimise their ventilation systems and potentially reduce overall operating costs.An alternative option would be to use a barometer-equipped smartphone as a survey instrument, provided that its inbuilt barometer module is sufficiently accurate.CITATION:Derrington, A S, 2015. In response to the implementation of a new schedule for iron ore fines in the International Maritime OrganizationÆs (IMO) International Maritime Solid Bulk Cargoes (IMSBC) Code, improved measures for the management of moisture have been developed. In particular, prediction of cargo moisture allows management of iron ore fines within the supply?chain to facilitate the safe?shipping of iron ore fines.An ARIMA model, augmented to allow for shipment tonnage, has been developed to predict the moisture level from previous shipments of the same product. The model explains about 60 per cent of the moisture variance. It was found that inclusion of other available information, such as ore composition, train/rake assays and recent rainfall at port and mine, did not add to the predictive power of the model.CITATION:Everett, J E, Linden, D and Maney, P, 2013. This paper will introduce some innovative strategies for simulator training that are intended to improve the level of safety and operational performance of heavy equipment operators. The key strategy is to design simulator training to deliver improved work performance outcomes. This strategy is underpinned by a process for engaging workers in knowledge discovery and transfer that has been developed as an outcome of an Australian Coal Association Research Program (ACARP) funded project conducted at open cut coal mines in Australia. The process is expected to deliver improvements in the safety and operational performance of heavy equipment operators by informing training content about how experienced operators manage unexpected situations around the site. Tapping into the tacit knowledge that experienced operators gather over time can provide site specific and timely information about situational cues used to manage the unexpected. This type of information complements basic skills training by helping the trainee understand the operational context in which the acquired skills will be applied. Inexperienced operators have traditionally gained experience through their own actions, observing the actions of others and by taking the advice of mentors who have æbeen there, done thatÆ and developed operational decision making skills for safe and productive work performance. Leadership and mentoring techniques that contribute to effective knowledge delivery will also be discussed.Other simulator training strategies include refresher training, change management, emergency response training and testing the usability of new equipment and technologies prior to their introduction on-site. A key challenge ahead for the resources sector is the development of knowledge elicitation techniques that suit the operational workforce. Many young mining engineers do not consider consulting as a possible job, believing that many years of working on-site is required to achieve practical experience, develop expertise and acquire industry contacts. However, mining consultancies employ engineers with varying levels of experience, including part-time university students, graduates and engineers who are relatively new to the industry. This paper outlines the role of a young engineer in a large mining consultancy and the benefits to both the consultancy and the client of having a mix of consultants with a diverse range of experience. From the authorÆs perspective, the work experience, lifestyle and rewards of mine consulting are compared to site based work, including a discussion on whether consulting is able to offer the professional development that a young engineer needs.

Morning Star Gold aims to enter the ranks of Australia's gold producers, revitalising the Eastern Victorian Goldfi elds in the second half of 2010 - the fi rst gold production in around 50 years from the Morning Star Gold mine, which was once Australia's largest under WMC. This presentation will outline the redevelopment of the Morning Star Gold Mine from exploration to production. Morning Star Gold NL's (MCO) key project area is centred at the historic mining town of Woods Point and nearby Matlock (to the southeast) and Gaffney's Creek (to the northwest), all of which are located in the lower part of the Victorian Alps roughly 60 kms south of Mansfield. Morning Star has literally numerous historic mines and gold bearing 'dykes' on its tenements, many with a rich production history of high-grade gold from quartz reefs and alluvial sources. The area encompassing the Woods Point goldfi eld and the multiple high-grade gold bearing dyke structures located north towards Jamieson and south towards Walhalla formed one of the most significant mining fields in Australian history. Historical gold production was at least 155 t or around six million ounces of over a century of mining with an average production grade exceeding 27 g per tonne. The bulk of this recorded production came from just a handful of mines and of those only three were mined to any great depth. One of the key historical production hubs in this area was the famous 'Morning Star' mine, which is situated adjacent to Woods Point township. The Morning Star mine produced almost one million ounces at 27 grams per tonne, operating fairly continuously between 1861 and 1959 with a break after Jan 1939 due to the devastating 'Black Friday' bush fi res across eastern Victoria. During the early 1940s, the Morning Star was Australia's biggest gold mine. The Morning Star mine along with Walhalla ensured that this goldfield was third behind Bendigo and Ballarat as Victoria's biggest hardrock gold production centre. Morning Star Gold has rebuilt its 100 per cent owned 'Morning Star' mine with a view to producing gold in the second half of 2010. Morning Star is exploring its regional dykes in order to estimate an overall gold endowment and rank local production sources to feed its proposed mill at the Morning Star mine site. Key initial targets (all within 11 kms of the Morning Star mine) are the Waverly, All Nations, Rose of Denmark, Hunts, Wallaby and Loch Fyne dykes, all of which historically produced high grade gold, much of it gained from only shallow workings above the water table in the weathered zone of rocks. Historically the Morning Star mine was developed over 24 levels to a depth of 940 m via one large vertical shaft and a smaller, deeper inclined shaft at the 1900 feet level underground. At present MCO has dewatered and completely refurbished all infrastructure to the ten level (~310 m below surface). MCO knows there are a number of substantially under-explored and undeveloped or partially developed but not stoped areas within the Morning Star mine. As a result, MCO has been carrying out extensive channel sampling and an underground diamond drilling program targeting areas of the mine that were largely unexplored and undeveloped by the last operators of the mine - Western Mining Corp Ltd (Gold Mines of Australia). Four thousand, fi ve hundred pages of archival WMC records have helped the cause significantly. The shaft refurbishment and reconstruction stage is complete. This has been a program that originally commenced back in 1993 and has cost around $22 M. This refurbishment paved the way for the extensive underground resource focused diamond drilling program, which MCO completed in 2008 with two company-owned and operated drill rigs from drill cuddies on level 9 (900 feet underground) of the mine. This drilling, which encountered some stunning grades intersections and multiple visible gold hits, has outlined further production opportunities including 0.4 m @ 1392 g/t Au (MS 410). Metallurgical testing conducted by Gekko Systems of samples obtained from the exploration program found gravity concentration and a leach circuit gave recoveries of ~95 per cent to 99 per cent, or ~92 per cent from gravity alone at a coarse crush (300 ¦m). Currently, the plan is to use gravity to fi rstly produce dore bars on-site from the free milling gold and secondly a saleable concentrate for CIL processing off-site. A purpose built 10 t/h (~80 000 t/a) gravity treatment plant was subsequently ordered from Gekko and is expected to be installed onsite and commissioned in the Q3 2010. The plant features fi ne crushing with a vertical shaft impactor, followed by gold concentration by inline pressure jig, inline spinner and Wilfley table. This is an ABSTRACT ONLY. The Bentley underground mine, which is located 60km north of Leonora, Western Australia, is owned and operated by Round Oak Minerals Pty Limited. A high-grade Zn-Cu Volcanogenic Massive Sulphide (VMS) orebody, Bentley was discovered in 2008 with production commencing in 2012. Long-hole open stoping is employed to mine the steeply dipping ore body, and in recent years the Avoca method has been adopted. This requires all development in a particular area of the mine to be completed prior to stoping commencing in a bottom up sequence. Voids are commonly backfilled with loose Run-of-Mine (ROM) waste loose rockfill (LRF); or alternatively cemented rockfill (CRF) can be used. Stopes in the 3860 AOS and 3900 AOS panels were located below previously extracted Avoca zones, with LRF situated immediately above them. These high-grade stopes were originally designed with rib and sill pillars to maintain control of the existing backfill during extraction. Due to changing factors over time, including mining economics, alternative options to achieve full extraction of the stopes were reviewed, including: • Mining small successive full height stopes which would involve drawing a blanket of LRF down with the extracted ore (stoping under waste); • Stabilisation of the existing LRF above the stopes and implementing a primary/secondary stoping sequence, with the primary stopes backfilled using a cement fill. An assessment of these options was undertaken which determined that the second full extraction method was the most beneficial to the operation, and could be achieved using a cement grout (Minova’s FB200 grout) to stabilise the LRF in conjunction with implementing a sequence where the primary stopes would be backfilled with cemented aggregate fill (CAF). Footwall drives were developed adjacent to the rockfilled stopes, allowing for the installation of horizontal grouting conduits, as well as providing access to the top of the planned 3860 and 3900 level stopes for the placement of CAF. An interval at the base of the previously rockfilled stopes was grouted at multiple horizons to achieve the required thickness, with a comprehensive quality assurance/quality control (QA/QC) process undertaken. Primary stopes were CAF backfilled via an arrangement of downhole winzes, linking the top of primary stopes to cross-cuts developed off the associated footwall drives. CAF was delivered underground through an open reticulation network that was extended to the specific levels. Overall the successful implementation of the revised mining method effectively increased ore recovery while maintaining control of overbreak and dilution, providing significant financial upside. This was achieved while maintaining effective controls in light of the changes to the risk profile. Stabilisation of the existing rockfill has allowed the extraction of stopes to full height, including the originally designed sill pillars, representing an average 23% increase in ore tonnes on the initial design, with dilution from the stope crowns averaging less than 1%. The use of CAF in association with primary/secondary stoping, eliminating the need for rib pillars, has yielded an average 47% increase in ore tonnes compared to the base case mine design.

The Obuasi Mine has been in operation for the past 100 years. Within this period a host of mining methods have been employed during its various changing phases. The causes for the changes are multiple, ranging from orebody capability, dynamics, technology, safety, economics, production levels, geotechnical, etc. Mining methods applied have also ranged from timber square setts, conventional cut-and-fill, shrinkage stoping, sublevel caving, mechanised cut-and-fill, longitudinal and transverse open stoping, and lately longitudinal retreat mining method (LRMM).The Obuasi Mine has a world class underground resource potential of about 27 million ounces. A third of this is in reserve. The orebody is subvertical, with multiple lodes and with widths ranging from 1.5 - 40 m. It stretches along 8.5 km N/S strike. A cursory statistical analysis on the various resource ore widths show a higher percentage (60 per cent) of the resource being within the range of about 1.5 - 10 m. Thus a high proportion of the resource is amenable to narrow vein mining methods. This has enhanced the choice of LRMM as an alternative method. Also, with the increasing cost of mining over time, LRMM, which eliminates expensive and time consuming footwall drives, will become far more advantageous as compared to the transverse/longitudinal open stoping methods which currently constitute about 40 per cent of the total underground production.The LRMM currently delivers about 20 per cent of the Obuasi Mine production. Plans are underway to improve total underground production from the current average of 160 000 tpm to about >180 000 tpm in the next few years and about 20 - 40 per cent more in the foreseeable future. Recoverable ounces will also increase from 26 000 to about 34 000 ounces per month. The proportion of LRMM in the business plan is also expected to increase from the current modest level to about 50 per cent. Despite its tremendous advantages in terms of economics and productivity, the method is facing some challenges during its inception. The Northern Sewerage Project (NSP) is a $650 M infrastructure project that will increase the capacity of the sewerage system for Melbourne’s growing northern suburbs while also helping to protect the Merri and Moonee Ponds creeks by significantly reducing the number of sewage overflows that can occur after heavy rainfall. The NSP consists of two stages: Stage 1 (NSP1) will be owned and operated by Melbourne Water Corporation, and Stage 2 (NSP2) will be owned by Yarra Valley Water and operated by Melbourne Water Corporation.Glass-fibre reinforced plastic (GRP) was used on both NSP stages, however, since Stage 1’s schedule for installation of GRP was slightly ahead of Stage 2’s, this paper will only focus on Stage 1’s use of GRP.NSP1 involves the construction of 7.9 km of new sewer tunnel ranging in diameter from 1.6 to 2.5 m, five main access shafts at depths of up to 65 m, and connections to the existing sewerage system including several access manholes. Construction began in 2007, and project completion is expected in 2012.The original design for NSP1 specified the use of high-density polyethylene (HDPE-) line precast concrete pipes as the final tunnel lining and HDPE-lined reinforced concrete for the access shafts and manholes. During construction, the rising cost of precast concrete pipes and the introduction of an additional GRP pipe supplier in the market justified a review of the tunnel final lining material. The contractor-proposed GRP pipe alternative was accepted for the final tunnel lining based on time and cost savings that were possible without compromising the design intent. After gaining experience with GRP through this design change, Melbourne Water Corporation reviewed and accepted GRP as the final lining for five smaller connection manholes. The use of GRP for the manholes was particularly inventive as the redesign made use of the approved tunnel GRP pipes as both a structural and corrosion-resistant lining.This paper discusses the use of GRP on NSP1; how the change in material, from concrete to GRP, affected the design philosophies assumed during detailed design; and how construction methods were modified to handle the less rigid GRP material. Details on the structural, hydraulic and durability considerations for the design are included, as are details about the time and cost savings and the quality and safety improvements that were achieved during construction.

The Woodcutters deposit, 65 km south- southeast of Darwin, N.T., consists of a series of veins of pyrite, sphalerite, and galena with some silver, cadmium, antimony, and arsenic minerals. The veins are hosted by the Early Proterozoic Whites Formation, a sequence of black slates with interbedded carbonate rocks and minor ash tuffs. The known orebodies occur about 300 to 500 metres above the contact of Whites Formation with the underlying Coomalie Dolomite, which consists of dolomite and magne- site. The stratigraphic setting is lithologic- ally similar to that of the Mt. Isa deposit, Queensland. The orebodies are replacement veins occ- urring along fault and dyke structures which are sub-parallel to the axes of a double-nosed faulted anticline. The orebodies are in part associated with two unusual carbonatized lam- prophyric dykes. The dykes were intruded after the fold and its slaty cleavage were formed. The ore has replaced the dykes and host rocks and is therefore late-stage hydrothermal miner- alization. The known orebodies are open at depth. A "bonanza" of high-grade ore occurs at the upper part of the largest orebody and is currently the subject of an open cut mine planning study by a consortium of companies headed by Nicron Resources Ltd.

Environmental Geochemistry International (EGi) and the Ian Wark Research Institute (IWRI) collaborated in a three-year Australian Minerals Industry Research Association (AMIRA) research project (Project Number P387A) focussed on the prediction and kinetic control of acid mine drainage. As part of this project, single addition, and sequential net acid generation (NAG) testing was carried out on sulfide mineral samples. The test samples comprised a range of synthetic waste materials prepared using a quartz bulk and controlled quantities of the commonly occurring sulfide minerals; pyrite (FeS2), pyrrhotite (Fe7S8), arsenopyrite (AsFeS), chalcopyrite (CuFeS2), chalcocite (Cu2S), covellite (CuS), bornite (Cu5FeS4), galena (PbS) and sphalerite (ZnS). NAG test results clearly show that there are significant differences between the acid forming potential of sulfide minerals. The NAG test results for each mineral were compared with theoretical acidity values (NAPP*). The NAPP* calculation was based on stoichiometric oxidation reactions assumed for each of the sulfide minerals. Sequential NAG values for pyrite corresponds closely to predicted NAPP* values, confirming the assumption that pyrite oxidation occurs according to the stoichiometry assumed. For the pyrite sample with 5 wt per cent S, two NAG stages were sufficient for all of the sulfur to be released to solution. Since sulfides in waste materials will generally be dominated by pyrite, results also confirm the validity of including NAG testing for ARD evaluation of most waste rock. NAG testing confirms that of the sulfide minerals examined, only pyrite, pyrrhotite, arsenopyrite and chalcopyrite are acid producing, ie able to produce leachate with a pH less than 4.5. In the case of sphalerite, galena, bornite, chalcocite and covellite the pH of the final NAG solution was greater than 4.5. The significant differences between the acid forming potential of different sulfide minerals could lead to overestimation of acid producing potentials based on NAPP values calculated from total per cent S alone, ie assuming all S is present as pyrite. The oxidation of galena during the NAG test resulted in the consumption of acid. The examination of the galena surface by X-ray photoelectron spectroscopy (XPS) after oxidation in peroxide solution indicated the presence of polysulfide. The reaction of sulfide to polysulfide is acid consuming, thus explaining the net acid consuming behaviour of galena in the NAG test. This reaction pathway may provide a possible explanation for the lower than expected acid production observed for some of the other sulfide minerals examined.

Safety and stability of underground mine openings, coal pillars and water dams in the near vicinity of operating surface mines are endangered from excessive blast induced vibrations. This paper deals with the investigations carried out in four coal mines. Simultaneous measurements at equal distances from the source indicated higher level of vibration on the surface than those produced in the underground workings. The values of peak particle velocity (PPV) measured on coal pillars, body of water dam were lower than those measured on roof and junctions of the roadways. The parting of coal (3-4.5 m) between two sections vibrated with higher PPV compared to the roof of top section and floor of bottom section. In one case the influence of total charge fired in a round was significant in generation of vibration compared to the maximum charge/delay where monitoring was done vertically below the source.

The pilot scale testing of Australian coals for their NO, emissions was carried out at the ACTC to investigate the influence of coal properties, furnace conditions and different NO,, control strategies on NO,, emissions. The NOx emissions for the Australian coals tested were found to vary over a wide range and primarily depended on combustion temperature and excess oxygen conditions. These coals did not exhibit any fundamental difference in NOx levels to other world coals. Under similar combustion conditions, the conversion of coal nitrogen to NO. was found to be best correlated with coal rank parameter, R, max per centage. The use of staged combustion as a control strategy was shown to reduce their NOx levels by 56 - 77 per cent.

Mining for sand was active in South Delhi during the last four decades disregarding all norms of safety. The aim of the paper is to highlight what is proposed to be done for mining in Delhi. Apart from the damage to the landscape, the earlier operations gave rise to visual pollution to the scenic Delhi area and there is immediate need of land restoration and landscaping. Future mining should be planned so that the resultant land surface completely merges with the topography and gradually assimilates with the green belt. Restoration of old workings, land reclamation and details of plantation in old pit areas are included. The project also provides a demon- stration site to show the impact of eco-development leading to new legisla- tive measures, as it is in the Capital of India itself and therefore more accessible than any other area. What has been suggested for mining is not the latest and the most modern equipments from the Western point of view but the right equipment to meet the specific requirement of the country. Financial projections are included alongwith technical details of the project to show the benefits to the society. It is concluded that eco-development is essential for the mining in the Twenty- First Century.

Recent work on computerised mineral resource databases at the Institute of Geological and Nuclear Sciences Limited has concentrated on the development of the GERM, REGCHEM, and MINLIT databases, and their integration in a Geographic Information System using MapInfo software on an IBM-compatible microcomputer local area network. GERM (GEological Resource Map) is a mineral deposit inventory with national coverage and more than 10 300 entries. REGCHEM (REGional Exploration GeoCHEMistry) is a database of stream sediment analyses from open-file mining company reports, containing more than 14 000 sample sites from the Coromandel and west Nelson regions. MINLIT (MINeral LITerature bibliography) is an annotated bibliography of literature on topics relating to the exploration and geology of mineral deposits in New Zealand, and includes more than 5000 entries for published books and papers, and unpublished mining company reports, university theses, and government reports. Master sets of GERM and MINLIT continue to be maintained on a VAX mainframe network but REGCHEM is operated only in the PC environment using the Microsoft ACCESS database management system software.

A growing demand within the minerals industry for sustainable development and the introduction of carbon taxes have resulted in a significant focus on improving the energy efficiency of comminution circuits. However, in this drive for improved energy efficiency the role of classification has been overlooked. Inefficient classification and large recirculating loads reduce circuit capacity, result in over-grinding and higher specific energy consumption.It is well understood that classification increases mill capacity; therefore, closed ball mill-cyclone circuits have become an industry standard. However, due to limitations in cyclone classification efficiency, the optimum recirculating load is limited to about 250 per cent. A simple relationship quantifying the effect of classification efficiency and recirculating load on grinding circuit capacity is presented in this paper. The possibility of improving the capacity of ball mill circuits by improving classification efficiency is discussed, considering the current practical limitations of classification equipment. Simulation tools were used to evaluate circuits with cyclones and with high frequency screens to identify opportunities to improve energy efficiency. The investigation also identified potential benefits to flotation recovery due to a reduction in over-grinding.CITATION:Jankovic, A and Valery, W, 2012.
