Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

The Sunrise deposit is one of AustraliaÆs largest and most profitable gold deposits. It has consistently and spectacularly outperformed every resource model created. Currently over the life of the Sunrise pit there has been a 50 per cent under estimation of ounces mined. Without a solid basis for predicting the tonnes and grade or the spatial location of mineralisation, mine planning becomes problematic. Recently, through a better understanding of the mineralising processes the reconciliation of the resource model has improved to be in the order of ten per cent under on ounces mined. This paper will describe how the mine geologists at Placer Granny Smith have implemented the changes required to improve the resource model. Work began with the development of a three-dimensional lithological and structural model based on historical drilling and pit mapping. Continual updating of the geological model with grade control logging, assays and in pit face mapping, now allows for the on-going improvement of the resource model and the planning of mine development drilling.

The MMG Century mine, which is the largest zinc/lead mine in Australia, began operating in 1999 and ceased mining from the open pit in July 2015, with processing completed in early 2016. Closure plans were developed from 2006, and detailed planning for closure commenced in early 2011 with the appointment of a dedicated closure planning resource. The site will continue to be managed during the active and passive closure phases to complete rehabilitation commitments and demonstrate that closure completion criteria have been achieved.Completion of the mining/processing phase of the project provides an opportunity to reflect on successes, missed opportunities and lessons learnt with respect to planning and execution of mine closure. This paper highlights examples of success at Century and also identifies key learnings and areas for improvement in progressive closure outcomes. Specific examples examine mine planning and execution involving bulk movement of waste rock and construction of permanent waste encapsulation landforms. The paper also provides recommendations for other sites that are planning for closure.Decisions made during the operational phase of the project life can result in a significant increase in closure liability and reduce the likelihood of achieving lease relinquishment. The Olympic Dam deposit is truly a world-class IOCG-Ag deposit, the world’s fourth largest deposit of copper and the world’s largest known single uranium deposit. Traditional and non-traditional methods have been used to accurately model the geology and mineralisation of the Olympic Dam deposit. Conditional simulation models have long been used as the most robust method for estimating risk and uncertainty in mine planning decisions at other mines around Australia and the world. There are many challenges associated with simulating the Olympic Dam deposit at a selective mining unit size sufficient for underground mine planning purposes. These include the size of the deposit, resulting in hundreds of millions of grid blocks; the disseminated nature of the zoned sulfide mineralisation. And lastly the direct relationship of the relative abundances of the sulfide mineral species that are crucial to the Olympic Dam smelter performance. Several geostatistical techniques have been utilised in the workflow for the Olympic Dam Conditional Simulation model to address these challenges. The key techniques are: • Projection Pursuit Multivariate Transform to de-correlate the main elements before simulation. • Pluri-Gaussian implicit boundary simulation method to simulate the mineralisation domains. • Spectral simulation method of de-trended and de-correlated attributes with account of local varying variance. The resulting conditional simulation models have been validated against histograms, grade-tonnage curves, cross-plots and assessed for variogram reproduction. This paper presents how the final conditional simulation models compare to benchmarking models created using alternate estimation techniques of ordinary kriging and local uniform conditioning. It also presents a validation process using historical samples to test final estimate accuracy of the simulation models.

Ground support installation into the face of development headings has become an accepted practice at many Australian hard rock mines. Ground support is installed in the development face to reduce personnel exposure to rockfall while conducting work tasks such as marking up, installing lifter tubes, charging, and face sampling. The ground support system installed in the face is typically friction bolts and mesh. This support system can be effective at reducing rockfall occurrence, but it also increases the cost of development per metre, in terms of capital for the ground support, jumbo and operator capital allocation and the installation time slowing each drill and blast cycle. Other issues associated with meshing and bolting the development face include management of rock bolts and mesh sheets in the fired dirt of every cut, damage to services such as vent duct, geometry issues when installing ground support onto irregular development profiles and operator error. Fibrecrete installation onto the development face for support is an option considered by many mines, but it can be difficult to manage due to the removal of visual indications of misfires and slowing the mining cycle due to the wait time for fibrecrete curing. The use of Thin Spray Liner (TSL) products in underground hard rock mines has been relatively infrequent in the past, mainly due to issues with cost, health and safety, and liner strength. Kanmantoo Mine investigated the use of TSLs for the purpose of development face support, while collecting data on the costs and practicalities associated with their use. Several TSL products were trialled and a preferred TSL product was selected for use as face support. Cost savings, drill and blast cycle timing improvements and other benefits were achieved while meeting the requirement to reduce rockfall risk.

To ensure that this investment is effective, the Owner or Manager must have a clear understanding of the information required from the Project controls systems. If appointing a single Managing Contractor, the Owner or Manager must stipulate what is and what is not required from the Project Controls Systems. There are numerous computer programs available for scheduling and cost control and it is important to select systems which best serve the Owner's needs.

This is an integrated geology and plant improvement study arising from the heterogeneity seen in mineralogy and gold association at the Obuasi goldmine in Ghana. This heterogeneity comes from the two ore types mined, the one being a sulphide ore, mainly composed of gold-bearing arsenopyrites. This gold is refractory and exhibits a strong relationship between gold and As. The other is quartz veins, that are high grade and free milling. The installed circuit consists of comminution – Gravity -Flotation – BIOX – Leaching and Adsorption processes. The opportunity tested involved improved mass pull - related flotation recovery of Au in the Sulphide stream; while balancing the need for a minimum Sulphide grade for the Bio-oxidation process, occasioned by the removal of 25 to 30 % of the (quartz) stream through preconcentration by Ore sorting. The current circuit is then utilized for the sulphide route, while a smaller grind-gravity circuit is utilised for the quartz stream in a relatively inexpensive way, followed by a common leaching and adsorption circuit. Added advantages of the strategy includes faster recovery of gold from the quartz stream, with less consumption of reagents from saving on flotation and BIOX reagents for this stream. Screening and Ore sorting was successfully tested with laser technology, achieving over 98% separation of quarts (containing up to 40% of the gold). The plant schedule of Ore, the suggested circuit design for the Ore sorting, the results obtained from the different flotation regimes, as well as consequent financial modelling have been presented and discussed in this paper. This illustrates the opportunity that arises out of a detailed analysis of material versus installed circuits, even if the gains in pre-concentration do not necessarily involve rejection of gangue material. An added opportunity for future investigation, briefly discussed but not tested in this paper, would be to maintain current mass pulls but with increase in throughput (by the space created by the sorted material). This will of course require the mining ability. It could also add LOM value by altering the cut-off grade.

In this paper, three types of value-adding in the minerals industry are discussed: downstream processing, improving the efficiency of existing processing and increasing the knowledge base and expertise of the staff. It is shown that, while downstream processing seems to offer a good way of improving Australia's trade position, there are a number of impediments to such development, notably, limited access to the appropriate technology and overseas markets. Transport costs can also be a problem. To reverse the situation will require more positive policies by government. In the meantime, something can be done towards adding value by increasing the efficiency of existing processes. Examples are given, showing how it might be possible to make economies in reagent costs in the gold industry, how innovative technology has led to more complete processing of a tailings dump, how research might lead to an increase in the throughput in the production of synthetic rutile and how it is possible to leach selectively from gold/copper ores.

At the Century mine in North West Queensland, MMG produces lead and zinc concentrates. The current production rate is close to 500 000 t of zinc per annum. Graphite is the main gangue in the orebody and a substantial quantity of chemicals are used to achieve an acceptable depression of graphite and other gangue materials in the current operation. In the future, it is anticipated that the content of graphite in the orebody will increase while the zinc grade will gradually drop. This scenario illustrates the importance of an optimised suite of flotation chemicals to maximise concentrate grade and recovery. As an alternative to the current reagent suite where dextrin is used to depress graphite, laboratory test work and subsequent plant trials were conducted with alternative depressants. These depressants facilitated both graphite depression and elevated zinc grades in the rougher concentrate. As an added benefit, it was also observed that the silica content in the zinc concentrate was reduced in the presence of the biopolymer selected for the plant trials. Ground penetrating radar (GPR), a well-established geophysical technique generally used for shallow civil engineering applications, has, in the last decade, found increasing acceptance as a complementary tool in mineral resource evaluations. As new sites are trialled with the method, a greater understanding is being garnered regarding the most suitable environments for long-range radar. Although successful projects have been conducted worldwide for mineral sands, iron ore and alluvial projects – tropical weathering environments have produced some of the most impressive results. Specifically, tropical nickel laterites have shown to be, with some notable exceptions, ideal GPR environments.Such high-resolution remote sensing has particular relevance to bulk commodities or those which possess significant rheological variation within geological units, and has led to some innovative downstream uses within exploration, project development and the larger mining industry. Low-loaders or 'floats' for relocating equipment in surface mines are limited to a capacity of approximately 350 t. In the iron ore industry it is desirable to be able to rapidly relocate excavators in order to maintain production rates within appropriate quality limits. In addition, the ability to rapidly relocate heavy equipment raises the possibility of improving the quality of repairs by performing maintenance in controlled shop conditions. However, many iron ore operations operate hydraulic excavators in the 600 to 800 t range, with newly released excavator models exceeding 800 t. These excavators are too large to float using conventional technology. This paper examines the potential for using multi-bogie transporters to relocate large hydraulic excavators in surface mines. Recent innovations have seen the development of modular transporters which can be coupled together to achieve larger platform areas. When separated, and with their drive bogies neutralised, these units are able to be towed behind prime movers and relocated between mines via public roads. The potential cost savings for relocating a Terex RH170 excavator in an open pit coal mine are examined. In the first six months of 2008, the analysis showed the potential for a 38 per cent reduction in total travel time with a corresponding revenue increase of approximately $350 k. The paper recommends that a feasibility study be conducted into offering a heavy equipment relocation service within the Pilbara region.

Hancock Prospecting Pty Ltd (HPPL) drilled two deep diamond stratigraphic holes (ECD0001 and RHD0296) within the eastern Fortescue Valley in the Pilbara of Western Australia. Located south of the Roy Hill Iron Ore project, the holes aimed to define the stratigraphy of the Marra Mamba Iron Formation (MMIF) of the Chichester Range and its south-dipping extensions below recent cover. Previous attempts to define the MMIF of the Chichester Range have resulted in differing interpretations due to the data available at the time.The unweathered MMIF beneath the Fortescue Valley in the HPPL drill holes is characterised by dolomitic carbonate interbedded with stilpnomelane-rich shales, chert and banded iron formation (BIF) with hematite staining. This intersection equates to ~75 m of carbonate-facies iron formation, as opposed to the ~230 m thick oxide-facies BIF and shale sequence encountered south of the Fortescue Valley.Various detailed geological core analyses and interpretations from ECD0001 and RHD0296 were integrated with regional diamond and reverse circulation (RC) holes from across the Fortescue Valley. Interpretation of the data indicates that the eastern Fortescue Valley MMIF represents a thinned thickness sequence of the whole formation. The MMIF of the Chichester Range and Fortescue Valley has gamma logs that correlate strongly in parts to the type section used for MMIF south of the Fortescue Valley. Therefore, the outcropping MMIF in the Chichester Range represents an eroded equivalent of the æreducedÆ thickness MMIF observed in ECD0001 and RHD0296.Data from deep diamond drill holes across the valley, from the Hamersley Basin to the Carawine Basin, were correlated. The correlation shows that the depositional site of the MMIF in the HPPL diamond drill holes is consistent with a proximal ramp setting, in which iron-rich shales wedge up-palaeoslope and shelf-derived carbonate turbidites wedge down-palaeoslope. Not only is it the occurrence of these shales that define the MMIF, but they also determine the northward potential to develop orebodies, as they are the source of iron from which iron-rich oxides and carbonates of metamorphic or diagenetic origin form.CITATION:Hurst, J, KrapeP, B and Hawke, P, 2013. A residue storage facility (RSF) was designed to cater for Davidson Creek Area and Robertson Range Area operations under the FerrAus Pilbara iron ore development project in the Pilbara Region, Western Australia. Annual ore production was about 15 million tonnes over a period of up to 15 years. The RSF was designed to handle a tailings production rate of 2.2 Mt/a over the project life with ability to be expanded at a later date. The composition of tailings was predominantly -45 ¦ material with some spiral reject material. The fraction of solids was expected to be between 40 per cent and 50 per cent solids by weight.The RSF was designed as an above ground storage facility, integrating with a mine waste dump planned close to the selected RSF site. This integration helped to increase the stability of the RSF embankment and to reduce the construction cost. The main source of construction material for the remaining part of the RSF embankment was selected to be mine waste. The RSF was designed to be constructed in stages by considering the availability of mine waste according to the mining schedule. The use of mine waste for construction of RSF embankments has several benefits over use of borrowed material. Tailings pond safety monitoring is vital for preventing dam failures and ensuring environmental safety. Unmanned Aerial Vehicles (UAVs) can enhance tailings pond monitoring with flexible operational cycles and the ability to collect remote sensing data from inaccessible areas. While many mining operations have adopted UAVs for tailings pond management, their full potential in data acquisition and subsequent analytical processing remains largely untapped. This paper explores various case studies where UAVs have been utilised for monitoring key aspects of tailings ponds such as beach line indicators, dam displacement and erosion, slurry discharge processes, and tailings pond capacity. We also present examples where data derived from UAVs have been instrumental in slope stability analysis, seepage modelling, Tailings Dam Breach Analysis (TDBA), and risk prediction. By integrating UAV technology with traditional ground-based sensor techniques, this approach significantly enhances the precision and reliability of hazard prevention and management in tailings pond operations.

The Thabazimbi iron ore deposit comprises a series of stratabound orebodies, on average 18 - 25 m thick that are aligned along the faulted basal contact between the Penge Iron Formation and dolostones of the Malmani Subgroup, Transvaal Supergroup. Field geological relationships and petrographic observations suggest that the formation of high-grade iron ores took place after contact metamorphism related to the intrusion of the Bushveld Complex (2.06 Ga) but before the deposition of red beds of the Waterberg Group at ca 1.9 Ga. Hard, high-grade iron ores are predominantly composed of martite and microplaty hematite. Goethite is restricted to supergene-modified ores close to the Late Cretaceous û Early Cenozoic Old African land surface. At depth, the high-grade hematite ores interfinger with low-grade dolomite-hematite and calcite-hematite ores. Iron oxides (martite, microplaty hematite) present in these low-grade ores are texturally indistinguishable from those in the associated high-grade ores, ie ore formation is genetically associated with carbonate metasomatism. Microthermometric studies of fluid inclusions in sparitic carbonates and rare megaquartz reveal their hydrothermal origin. Two compositionally distinct aqueous fluids of shallow crustal origin are identified, namely a Ca-Na-rich high-salinity brine (26 wt per cent NaClequivalent, TH ~160¦C), and an aqueous Mg-K-bearing fluid of intermediate salinity (8.3 - 12.6 wt per cent NaClequivalent, TH~120 - 150¦C). The results confirm that high-grade hematite orebodies at the Thabazimbi deposit owe their origin to an event of extensive oxidative carbonate metasomatism (calcification), similar to that recognised at other important high-grade iron ore deposits in the world. Hydrothermal ore formation is both structurally and lithologically controlled. Size and quality of the orebodies was improved by Late Mesozoic-Cenozoic deep lateritic weathering as carbonate-hematite ores were enriched to friable high-grade ores, but this supergene modification is second in importance compared to the primary event of hydrothermal ore formation.

"Most new mining projects require an initial assessment of the locations and volumes of potential acid forming (PAF) waste that is likely to be moved and dumped in surface waste stockpiles. This requires sampling of the waste rock zones and submitting the samples for a series of tests. Key to the outcome of the waste characterisation process is obtaining sufficient samples that are representative of the various rock types both in number and spatial distribution.Various regulatory bodies throughout Australia and the rest of the world are currently struggling with how to define minimum amounts of sampling to achieve realistic estimates without imposing excessive costs and time constraints on a project.This paper addresses two aspects of the process: what constitutes a æsufficientÆ number of samples how to use those samples in determining volumes of likely PAF material.Geostatistical methods of characterising orebodies with large numbers of samples are routinely used in the mining industry by geologists and geostatisticians to estimate and classify Mineral Resources. In theory exactly the same methods can be used to estimate PAF waste locations and volumes.In practice the number and spatial distribution of dedicated PAF waste samples at feasibility stage is typically so small that it is almost impossible to estimate the spatial distribution of PAF material with any degree of confidence at all. Consequently, the common approach is to characterise a few samples from a lithological unit and then assume that they represent the unit as a whole. Valuation of mining companies and their underlying assets prepared by brokers' analysts are, by necessity, approximations. The reliability of the valuations is entirely proportional to the quality of available data, the reliability of forecasts and the nature of underlying commodity price assumptions. The latter is perhaps the greatest single cause of gross variations in company and asset earnings estimates and- valuations. Assumptions are often necessary regarding commodity price achievement based on commodity hedge positions. Public disclosure from listed companies provides a sound base from which to project earnings, cashflows and balance sheets. The uncommon disclosure of projected production and cost schedules often reduces the reliability of future estimates. The application of reserve and resource estimates is contentious with practitioners applying differing weightings to importance of resources in estimating mine lives (and therefore cashflow streams). Valuation methods applied in equity markets usually focus on relative value and the identification of pricing anomalies. The commonest ""rules of thumb"" applied in the valuation of mining companies (and therefore their underlying assets) is with the use of year specific price/earnings (P/E) and price/cashflow (P/CF) ratios, with low multiples suggesting undervalued assets, high multiples overvalued situations (but these rules can often be clouded by market expectations of underlying commodity price movements). Another simply applied valuation tool is to examine the examine the value placed on reserves and resources by the market. Inexpensive gold in the ground (expressed as $ of market capitalisation per ounce of reserves or resources) might indicate a buying opportunity, but the market does discount the value of gold in the ground prior to mine development and where development risk is perceived.

This paper addresses critical environmental, health, regulatory and scientific concerns related to gold mining activities in the specific area of Barberton, Mpumalanga, South Africa. The paper thus calls for informed decisions to be made by the authorities to safeguard both the environment and people’s health. The novelty of this study is based on its comprehensive nature of the simultaneous assessment of both cyanide and potential toxic elements (PTEs) in tailings, while taking into account their composition variability over time. By combining these aspects, it is then possible to provide a holistic understanding of the contamination risks associated with mining tailings and thus justify the need for regular monitoring and supervision to ensure compliance against regulatory limits. Additionally, this paper considered investigating the dynamic nature of tailings, which are subject to changes in their chemical composition due to exposure to weathering by various agents. In this study, the determination of weak acid dissociable (WAD) and free cyanide in tailings was performed using a validated method employing the South African Council for Mineral Technology (MINTEK) Laboratory Cynoprobe. The PTEs in tailings were determined by an inductively coupled plasma optical emission spectrometer (ICP-OES). The results revealed that the tailings contained primarily substantial amounts of PTEs, including Fe (87 566 to 37 800 mg/L), Cu (271 to 110 mg/L), Mn (2100 to 680 mg/L), Ni (400 to 390 mg/L), Zn (180 to 68 mg/L), Cr (645 to 510 mg/L), As (450 to 128 mg/L), and Pb (up to 35 mg/L). The cyanide concentrations in tailings varied between 0.20 mg/L and 0.26 mg/L for free CN (with a mean of 0.23 mg/L in water leaching), 0.23 mg/L to 0.28 mg/L for free CN (with a mean of 0.25 mg/L in alkali leaching), 0.24 mg/L to 0.35 mg/L for WAD CN (with a mean of 0.28 mg/L in water leaching), and 0.31 mg/L to 0.39 mg/L for WAD CN (with a mean of 0.35 mg/L in alkali leaching). The measured concentration of cyanide and the PTEs was compared to the minimum permissible levels enforced by the South African Department of Water and Sanitation (DWS) and the Water Research Commission (WRC) regulations that govern the disposal of hazardous constituents into the environment. Additionally, it was observed that the concentrations of contaminants in the tailings exceeded the acceptable minimum limits for hazardous waste exposure, posing risks to the environment and human health.

Although the majority of current steel production is supported by iron ore sourced from high-grade haematite deposits, the long-term growing demand for steel has led to higher raw material prices and opened the way for many new magnetite deposits to also be developed. There is a rich and long history of magnetite ore processing in Western countries, including large operations such as Cleveland Cliffs in the USA and LKAB in Sweden, as well as smaller operations like Savage River in Tasmania. The challenge for virtually all magnetite operations is to minimise operating costs, which is dominated by the cost of power required to fine grind the ore in order to achieve acceptable concentrate iron grade together with low impurity content. This remains the case today and will be even more important in the future when a carbon tax is expected to become a significant addition to the operating costs for magnetite deposits. Historically, the lowest operating cost was achieved by multistage fully autogenous grinding with integrated magnetic separation steps between the stages. The major benefit of fully autogenous grinding is the elimination of steel grinding media costs and the need to discriminate between steel and magnetite in coarse magnetic separation. The separation step between grinding stages progressively reduces the amount of material to be ground. Application of more efficient grinding technologies developed in the last 20 years, including high pressure grinding rolls (HPGR) for fine crushing and stirred milling for fine grinding, has provided opportunities to further reduce the operating costs associated with comminution. Both technologies are already implemented in some magnetite processing operations, although in limited capacity. The results of a theoretical option study for high capacity processing of a hard, fine-grained silica-rich magnetite ore is presented in this paper, with the emphasis on comminution circuit options. Several circuit options are ranked based on a net present value analysis incorporating an estimate of carbon tax added in the operating cost. The study demonstrates the significant advantages of applying more efficient autogenous grinding technologies.
